RESUMO

Objective@#To investigate the feasibility and effects of a mobile app-based home cycling exercise program compared to home cycling exercise without additional monitoring system. Compared with fitness facilities or outdoor exercise, home-based exercise programs effectively improve physical performance in an indwelling community. However, a flexible, informal environment may decrease motivation and impair adherence to physical exercise. Mobile devices for aerobic exercise and mobile applications provide real-time monitoring, immediate feedback, and encouragement to increase motivation and promote physical performance. We investigated the feasibility and effects of a mobile app-based home exercise program on body composition, muscular strength, and cardiopulmonary function. @*Methods@#Between February and May 2023, 20 participants were randomly allocated to the intervention (mobile application with a tablet) and control groups, and they performed aerobic exercise using a stationary bicycle for ≥150 minutes per week for 6 weeks (≤30-minute exercise session, with 3-minute warm-up and 3-minute cool-down). Karvonen formula-based heartrate defined the weekly increase in exercise intensity. Outcome measures included body-composition parameters, isokinetic knee flexor and extensor strength tests, cardiopulmonary exercise test results, and rate of target heart rate (HR) achievement. Participants were assessed at baseline and after the intervention. @*Results@#Unrelated personal events led two participants to drop out. The intervention and control groups had similar baseline characteristics. Compared with the control group, in the post-intervention isokinetic strength test, bilateral knee flexor and extensor power, and time to target HR achievement significantly increased each week in the intervention group. @*Conclusion@#Home-based exercise to achieve long-term cardiovascular fitness with portable electronic/mobile devices facilitates individualized exercise using real-time feedback to improve motivation and adherence.

Objectives@#. Air pollution is an increasing global concern, and its effect on allergic inflammation has attracted the attention of many researchers. Particulate matter (PM) is a major component of ambient air pollution, and heavy metals are the primary toxic constituents of PM. As previous studies on the impact of air pollutants on allergic inflammation did not adequately mimic real-world atmospheric exposure, we developed an experimental model to investigate the effects of aerosolized air pollutants on nasal epithelial cells and fibroblasts. @*Methods@#. We collected particulate matter 2.5 (PM2.5) samples from ambient 24-hour air samples obtained in Seoul from August 2020 to August 2022, and then conducted component analysis for metallic constituents. Primary nasal epithelial cells and nasal fibroblasts, obtained and cultured from the turbinate tissues of human participants, were treated with PM2.5. The associations of heavy metals identified from the component analysis with cytokine expression were investigated. A three-dimensional (3D)-hybrid culture model, consisting of co-culture of an air-liquid interface and nasal fibroblast spheroids, was constructed to observe the impact of aerosolized air pollutants. @*Results@#. Among the heavy metals, Si was the predominant component of PM2.5, and Zn showed the highest correlation with the concentration of PM2.5 in Seoul. PM2.5, Zn, and Si increased the production of epithelial cell-derived cytokines, and PM2.5 and Zn exhibited similar trends with one another. Exposure of the 3D-hybrid model to aerosolized PM2.5 and Zn resulted in elevated periostin, alpha-smooth muscle actin, and fibronectin expression in fibroblast spheroids, and those without an epithelial barrier exhibited a similar increase in periostin expression. @*Conclusion@#. Ambient air pollutants in the form of aerosols increase the expression of allergic inflammatory cytokines in both nasal epithelial cells and fibroblasts. Regulations on air pollution will help reduce the global burden of allergic diseases in the future.

Achalasia is an esophageal motility disorder characterized by loss of esophageal peristalsis and impaired relaxation of the lower esophageal sphincter. Patients with achalasia often complain of persistent symptoms for several years before diagnosis. On the other hand, achalasia diagnosed as a sudden esophageal food impaction is uncommon, and no report has been issued on the diagnosis and successful treatment of achalasia in a 95-year-old patient. We report a case of achalasia diagnosed by high-resolution esophageal manometry and timed barium esophagography after food material removal by endoscopy in a 95-year-old woman who visited the hospital due to sudden esophageal food impaction and was successfully treated by endoscopic balloon dilatation.

Crohn's disease and ulcerative colitis are lifelong chronic inflammatory conditions, with many patients requiring ongoing immunomodulatory drug therapy for maintenance treatment. Recent therapeutic goals in inflammatory bowel disease (IBD) are not only aimed at symptomatic remission but also at achieving mucosal healing to improve the natural course of the disease. In this context, therapeutic approaches are being applied in clinical settings that involve early and appropriate use of drugs, such as immunomodulators or biologics, that have the potential to induce healing of the inflamed intestine before irreversible intestinal damage occurs. All drugs that continuously control intestinal inflammation in IBD can heal the mucosa and potentially reduce the incidence of colitis-associated bowel cancer; however, the continuous use of immunosuppressants can potentially increase the risk of malignancies. The safety issues of the drugs used in clinical practice are partly confirmed during their development processes or shortly after initial marketing, but in other cases, they are estimated through post-marketing case reports or epidemiological studies, sometimes decades after drug approval. This review explores the risks associated with malignancies related to the treatment of IBD, focusing on drugs currently approved in Republic of Korea.

Background/Aims@#The efficacy of concurrent chemoradiotherapy (CCRT) or esophagectomy for locally advanced esophageal squamous cell carcinoma (ESCC) is unclear. This study compared the survival and recurrence of patients with locally advanced ESCC after definitive CCRT and surgery. @*Methods@#A retrospective study was conducted on patients with locally advanced ESCC who underwent CCRT or esophagectomy at Kosin University Gospel Hospital from January 2010 to December 2016. The patients’ baseline characteristics, pathology, recurrence rate, and three-year/five-year overall survival were obtained Results: This study evaluated ESCC patients with cT1-T2, N+ or cT3-T4, or N, who were treated by definitive CCRT (n=14) or esophagectomy (n=32). No significant difference was noted between the two groups, except for the location of the cancer and performance state. The respective three- and five-year overall survival rates were 30.8% and 23.1% in the CCRT group and 40.2% and 22.5% in the esophagectomy group (p=0.685). In the CCRT group, three patients (21.4%) had a complete response, and two (66.7%) had a recurrence. In the esophagectomy group, an R0 resection was achieved in 28 (87.5%) patients, and a recurrence occurred in 18 (64.3%). The median disease-free survival in the CCRT and esophagectomy groups was 14 and 17 months, respectively (p=0.882). @*Conclusions@#These results showed no significant difference in survival between the definitive CCRT and surgery as the initial treatment. Nevertheless, larger prospective studies will be needed because of the retrospective nature and small number of patients in this study.

Objectives@#Few studies have reported long-term follow-up after treatment of gastric mucosaassociated lymphoid tissue (MALT) lymphoma. In this single-center study, we investigated longterm treatment outcomes in patients diagnosed with gastric MALT lymphoma. @*Methods@#The study included 80 patients diagnosed with gastric MALT lymphoma, who were followed up at a single center between January 2005 and December 2019 after Helicobacter pylori eradication therapy, radiotherapy, or chemotherapy. We evaluated complete remission, improvement, or recurrence of the lesion. Follow-up over >60 months was classified as long-term follow-up, and the progression-free survival rate was recorded. @*Results@#Following H. pylori eradication treatment, complete remission occurred in 85.9% (55/64) of H. pylori-positive and 50.0% (3/6) of H. pylori-negative patients. All patients with gastric MALT lymphoma who did not respond to H. pylori eradication therapy (100.0% [6/6]) achieved complete remission following administration of local radiotherapy. We observed no deaths on long-term follow-up (>60 months), and the progression-free survival was 101 months. @*Conclusions@#In this study, patients with gastric MALT lymphoma showed excellent survival rates, progression-free survival, and prognosis on long-term follow-up. Prospective studies are warranted to determine the long-term prognosis of gastric MALT lymphoma after treatment.

Background@#The Gout Impact Scale (GIS), part of the Gout Assessment Questionnaire 2.0, measures gout-specific health-related quality of life (HRQOL). This study aimed to translate the GIS into Korean and validate the Korean version (K-GIS) using generic HRQOL measures. @*Methods@#The GIS was translated into Korean and back-translated into English. We asked patients aged 18 years or older who met the 2015 gout classification criteria to fill out the questionnaires (from January 2022 to June 2022); the K-GIS (5 scales [0–100 scores each]), along with the Korean version of Health Assessment Questionnaire (HAQ) and EuroQol-5 dimension (EQ-5D). We investigated the internal consistency, construct validity, and discriminative validity for gout characteristics of K-GIS. The K-GIS form was administrated to patients 4 weeks later to assess the test-retest reliability using the intraclass correlation coefficient (ICC). @*Results@#One hundred patients completed the questionnaire. The mean ± standard deviation age of the patients was 53.0 ± 15.1 years, and 99.0% of the patients were men. All scales had high degree of internal consistency (Cronbach’s α = 0.59 to 0.96) and test-retest reliability (n = 18, ICC = 0.83 to 0.94, all P 6 mg/dL, frequent gout flares in the past year, and fewer comorbidities. In contrast, neither the HAQ nor the EQ-5D could discern these subsets of patients. @*Conclusion@#The K-GIS is a reliable and valid HRQOL measure for patients with gout. Higher K-GIS scores were associated with clinical characteristics leading to unfavorable outcomes, which were not demonstrated by the HAQ and EQ-5D.

Purpose@#To explore the subjective eye health status of Korean adolescents by the extent of Internet use in 2009-2010 and smartphone use in 2020. @*Methods@#We secondarily analyzed the results of the Korea Youth Risk Behavior Survey performed by the Korean Disease Control and Prevention Agency that enrolled Korean adolescents aged 12-18 years. Changes in internet use time from 2009 to 2019 and smartphone use in 2020 were examined. Questionnaires exploring subjective eye health were administered in 2009, 2010, and 2020. We performed complex logistic regression analysis. @*Results@#A total of 748,490 subjects were enrolled. Korean adolescents used the Internet for an average of 2.48 hours (h) on weekdays and 4.06 h on weekends in 2019, thus significantly more than the 2009 averages of 1.93 h on weekdays and 2.96 h on weekends (both p < 0.001). In 2009 and 2010, the average internet use time on weekends significantly and positively correlated with subjective eye problems (odds ratio = 1.161, p < 0.001). The average smartphone use time was 4.72 h on weekdays and 6.56 h on weekends in 2020. In that year, the average smartphone use time on weekends (odds ratio = 1.049, p < 0.001) and smartphone overdependence (odds ratio = 8.636, p < 0.001) significantly and positively correlated with the presence of subjective health problems. @*Conclusions@#Internet and smartphone use time on weekends affect subjective eye health.

Teratomas are the most common congenital tumors and contain cells from the ectoderm, mesoderm, and endoderm. They are mainly located in the central axis of the body. The tumors are most commonly found in the sacrococcygeal region, followed by the gonadal site and mediastinum, and rarely in the head and neck. Teratomas can cause various clinical symptoms depending on the location of the mass and may result in feeding difficulties or respiratory distress. We present a case of oropharyngeal teratoma accompanied by respiratory distress and persistent feeding difficulties, leading to compression of the temporomandibular joint, which in turn caused subluxation.

Colorectal strictures are uncommon in patients with ulcerative colitis (UC). An extranodal marginal zone B-cell lymphoma of mucosa-associated lymphoid tissue (MALT) lymphoma is rarely involved in the colon but may be associated with inflammatory bowel diseases. A 41-year-old female with a six-year history of UC presented with a severe stricture of the sigmoid colon that prevented the passage of a colonoscope. A histological examination revealed non-specific inflammation and fibrosis without dysplasia or cancer.Despite conventional treatment, including mesalazine and azathioprine for one year after that visit, the stricture persisted. In addition, diffuse, edematous exudative inflammation and multiple shallow ulcers were observed in the distal rectum, revealing a MALT lymphoma testing positive for CD20, CD43, CD5, and Bcl-2, but negative for CD3, CD10, CD23, and cyclin-D1. Four weekly doses of rituximab were administered. Follow-up colonoscopy performed one month after treatment revealed slight improvement in the rectal lesion without remnant histological evidence of a MALT lymphoma. In addition, the stricture showed marked improvement, and the colonoscope could pass easily through the stricture site. This is the first case report on an improvement of a severe sigmoid colon stricture in a patient with UC after rituximab treatment for a concomitant rectal MALT lymphoma.

Achalasia, a rare motility disorder of the esophagus, is generally accepted as a premalignant disorder. This paper presents the case of a 72-year-old male with achalasia and synchronous superficial esophageal cancer who experienced dysphagia symptoms for five years. As achalasia is associated with an increased risk of esophageal cancer, both can be treated simultaneously if detected at the time of diagnosis. Achalasia and synchronous esophageal cancer are rarely detected and treated endoscopically. This paper reports a case of concurrent successful treatment.

Objective@#To analyze the microimplant (MI) displacement pattern on treatment with a maxillary skeletal expander (MSE) using cone-beam computed tomography (CBCT). @*Methods@#Thirty-nine participants (12 males and 27 females; mean age, 18.2 ± 4.2 years) were treated successfully with the MSE II appliance. Their pre- and post-expansion CBCT data were superimposed. The pre- and post-expansion anterior and posterior inter-MI angles, neck and apical inter-MI distance, plate angle, palatal bone thickness at the MI positions, and suture opening at the MI positions were measured and compared. @*Results@#The jackscrew plate was slightly bent in both anterior and posterior areas. There was no significant difference in the extent of suture opening between the anterior and posterior MIs (P > 0.05). The posterior MI to hemiplate line was greater than that anteriorly (P 0.01). @*Conclusions@#In the coronal plane, the angulation between the anterior MIs in relation to the jackscrew plate was greater than that between the posterior MIs owing to the differential palatal bone thickness.

Purpose@#We evaluated the difference in clinical outcome according to the use of double-needle stabilizer in flanged intrascleral fixation of intraocular lens (IOL).Method: We retrospectively analyzed the medical records of 44 patients (44 eyes) who underwent flanged intrascleral fixation of IOL. Groups 1 and 2 (respectively) underwent surgery that did not and did employ a double-needle stabilizer. We evaluated the best-corrected visual acuity (BCVA), intraocular pressure, spherical equivalent (SE), refractive prediction error (RPE), IOL tilt and decentration, and other complications. @*Results@#The postoperative BCVA, SE, and RPE did not differ between the two groups (p = 0.672, p = 0.131, p = 0.247). IOL tilt was significantly less in group 2 than in group 1 (p = 0.038, p = 0.015 [horizontal and vertical]) (3.96 ± 2.01° horizontally and 4.17 ± 2.07° vertically in group 1; 2.59 ± 2.02° horizontally and 2.50 ± 1.92° vertically in group 2). IOL decentration was also significantly less in group 2 than in group 1 (p = 0.001, p = 0.004 [horizontal and vertical]) (0.44 ± 0.21 mm horizontally and 0.45 ± 0.20 mm vertically in group 1; 0.25 ± 0.09 mm horizontally and 0.27 ± 0.13 mm vertically in group 2). @*Conclusions@#Use of a double-needle stabilizer during flanged intrascleral fixation of IOL significantly reduced postoperative IOL tilt and decentration, and may thus improve postoperative visual quality.

While numerous studies have evaluated humoral responses to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) vaccines, data on the cellular responses to these vaccines remain sparse. We evaluated T cell responses to ChAdOx1-nCoV-19 and BNT162b2 vaccinations using an interferon gamma (IFN-γ) release assay (IGRA). ChAdOx1-nCoV-19- and BNT162b2-vaccinated participants initially showed stronger T cell responses than unvaccinated controls. The T cell response decreased over time and increased substantially after the administration of a BNT162b2 booster dose. Changes in the T cell response were less significant than those in the anti-receptor-binding domain IgG antibody titer. The study results can serve as baseline data for T cell responses after SARS-CoV-2 vaccination and suggest that the IGRA can be useful in monitoring immunogenicity.

Purpose@#This study investigated the microbiological profile and clinical characteristics of culture positive (CP) contact lens-related bacterial keratitis (CLBK) and compared them to culture negative (CN) cases. @*Methods@#We retrospectively analyzed 142 CLBK patients hospitalized between January 1998 and December 2022. The CP and CN groups consisted of 79 eyes (100 isolates) and 63 eyes, respectively. We investigated the microbiological profile in the CP group and compared the epidemiology, predisposing factors, clinical characteristics, and treatment outcomes between the CP and CN groups. Poor treatment outcomes were defined as a final best-corrected visual acuity (BCVA) ≤ 0.5, worsened BCVA after treatment, or the need for surgical intervention. We evaluated the risk factors for poor treatment outcomes in the entire cohort through two-proportional Z-test analysis. @*Results@#In CP group, gram-negative bacteria accounted for 85%, and common isolates were Pseudomonas (27%) and Serratia (26%) species. There were significant differences in mean age (27.0 years vs. 33.8 years, p = 0.009) and a history of therapeutic bandage contact lens wear (13.9% vs. 28.6%, p = 0.031) between the CP and CN groups. However, there were no significant differences in symptom duration, prior use of topical antibiotics, sleeping with wearing CL, epithelial defect size, hypopyon, and poor treatment outcomes. Significant risk factors for poor treatment outcomes in the entire group included deep infiltration (Z = 2.88), epithelial healing time ≥ 7 days (Z = 2.30), initial BCVA < 0.1 (Z = 2.20), over date use of contact lens (Z = 2.10), and epithelial defect size ≥ 5 mm2 (Z = 2.05). @*Conclusions@#There were no significant differences in clinical characteristics and treatment outcomes between the CP and CN groups. Purpose@#We used optical coherence tomography angiography to analyze the macular microvasculature of normal eyes of healthy Korean children; we derived normative data stratified by age. @*Methods@#We retrospectively analyzed the medical records of 68 children (126 eyes) aged from 3 to 15 years. Optical coherence tomography angiography was performed on 3-mm2 regions centered on the fovea. The vascular densities of the superficial and deep capillary plexuses and the foveal avascular zone were determined, and analyzed by age. We sought correlations between age, gender, axial length, and the spherical equivalent. @*Results@#The foveal, superficial capillary plexus vessel densities were 17.41 ± 5.41, 20.53 ± 4.76, 21.78 ± 5.73, and 22.95 ± 3.91% in children aged 3-5, 6-8, 9-11, and 12-15 years, respectively. The vessel densities of the foveal deep capillary plexus were 30.20 ± 6.59, 34.54 ± 7.16, 35.81 ± 6.02, and 37.38 ± 5.16% in those aged 3-5, 6-8, 9-11, and 12-15 years, respectively. Significant differences by age were apparent (p = 0.002, p < 0.001, respectively). The foveal vessel densities of the superficial and deep capillary plexuses were positively correlated with axial length (R2 = 0.153, p < 0.001; R2 = 0.158, p < 0.001, respectively) and the foveal avascular zone area was negatively correlated with axial length (R2 = 0.085, p < 0.001). @*Conclusions@#The vessel densities of the superficial and deep capillary plexuses measured via optical coherence tomography angiography in normal eyes of healthy Korean children differed by age, and correlated significantly with axial length.

An epidermoid cyst is a benign tumor found anywhere in the body. However, the occurrence of epidermoid cysts in the thymus is extremely rare, with only six cases reported worldwide. The correct diagnosis of thymic epidermoid cysts is often difficult due to the unusual location and nonspecific imaging findings. Herein, we present a case of a thymic epidermoid cyst in a 37-year-old female with clinical information and chest CT findings. Further, we have reviewed previous literature reports describing imaging findings of thymic epidermoid cysts.

Background and Objectives@#We evaluated the symptom improvement, surgical outcomes and post-operative complications of the figure of 8 anchoring suture technique using polycaprolactone (PCL) nasal mesh for the treatment of caudal septal subluxation.Subjects and Method We conducted a retrospective study of patients who underwent between March 2020 and March 2021 endonasal septoplasty using the figure of 8 anchoring suture technique and a PCL nasal mesh as a protective supporting graft. Fourteen patients were divided into two groups, the allergic and non-allergic rhinitis group. Symptom improvements were assessed using the Nasal Obstruction Symptoms Evaluation (NOSE) scores and visual analog scale (VAS) scores for epistaxis and headache. Post-operative patient-reported subjective changes in nasal obstruction were also recorded. All evaluations were conducted one to four months post-operatively. @*Results@#Post-operative endoscopic examination revealed that all patients had their septum straightened. The mean post-operative NOSE scores in overall and each item were significantly lower than the mean NOSE scores in the pre-operative period (p<0.05). The mean post-operative NOSE scores for patients both with and without allergic rhinitis were also significantly decreased compared to those in the pre-operative period (p<0.05). A decrease in VAS scores was significant (p=0.008) for headache but not for epistaxis (p=0.141). All patients reported improvement of subjective nasal obstruction post-operatively. @*Conclusion@#The use of figure of 8 anchoring suture technique with a PCL nasal mesh as a protective and supportive graft was proven to be a successful method for correcting caudal septal subluxation.

Nasopharyngeal swabs have been widely to prevent the spread of coronavirus disease 2019 (COVID-19). Nasopharyngeal COVID-19 testing is a generally safe and well-tolerated procedure, but numerous complications have been reported in the media. Therefore, the present study aimed to review and document adverse events and suggest procedural references to minimize preventable but often underestimated risks. A total of 27 articles were selected for the review of 842 related documents in PubMed, Embase, and KoreaMed. The complications related to nasopharyngeal COVID-19 testing were reported to be rarely happened, ranging from 0.0012 to 0.026%. Frequently documented adverse events were retained swabs, epistaxis, and cerebrospinal fluid leakage, often associated with high-risk factors, including severe septal deviations, pre-existing skull base defects, and previous sinus or transsphenoidal pituitary surgery. Appropriate techniques based on sufficient anatomical knowledge are mandatory for clinicians to perform nasopharyngeal COVID-19 testing. The nasal floor can be predicted by the line between the nostril and external ear canal. For safe testing, the angle of swab insertion in the nasal passage should remain within 30° of the nasal floor. The swab was gently inserted along the nasal septum just above the nasal floor to the nasopharynx and remained on the nasopharynx for several seconds before removal. Forceful insertion should be attempted, and alternative examinations should be considered, especially in vulnerable patients. In conclusion, patients and clinicians should be aware of rare but possible complications and associated highrisk factors. The suggested procedural pearls enable more comfortable and safe nasopharyngeal COVID-19 testing for both clinicians and patients.

The Good’s syndrome is a rare immunodeficiency disease characterized by thymoma and hypogammaglobulinemia. The most common cause of infection in patients with Good’s syndrome is the recurrent sinopulmonary infection. One may assume that there could be an unknown immunocompromising condition such as Good’s syndrome in cases of chronic rhinosinusitis refractory to treatment. We experienced a patient with Good’s syndrome, presenting with chronic rhinosinusitis refractory to treatment. We report this case with a brief review of the literature.
