Tertiary lymphoid structures (TLS) are ectopic lymphoid structures in cancers that are largely associated with favourable prognosis. However, the prognostic value of TLSs in oral squamous cell carcinoma (OSCC) is largely unknown, and the association between tumour infiltrating lymphocytes (TILs) and TLSs has been rarely explored in OSCC. In this study, associated markers of TLS, including peripheral node address (PNAd) in high endothelial venules, CD20 in B cells and CD3 in T cells, were examined in 168 OSCC patients, and survival analysis was performed between TLS-positive and TLS-negative cohorts. We detected the presence of TILs by staining CD8+ cytotoxic T cells and CD57+ NK cells as well. TLSs appeared as highly organized structures in 45 (26.8%) cases. TLS-positive patients had a better 5-year overall survival (OS) rate (88.9% vs. 56.1%, P < 0.001) and relapse-free survival (RFS) rate (88.9% vs. 63.4%, P = 0.002). Moreover, the presence of TLS was an independent prognostic factor for both the 5-year OS rate (hazard ratio [HR] = 3.784; 95% confidence interval [CI], 1.498-9.562) and RFS rate (HR = 3.296; 95% CI, 1.279-8.490) in multivariate analysis. Furthermore, a higher density of CD8+ T cells and CD57+ NK cells was found in TLS-positive sections than in TLS-negative counterparts (P < 0.001), and their combination provided a higher predictive accuracy (AUC = 0.730; 95% CI, 0.654-0.805). In conclusion, our results suggest that TLS is an independent positive prognostic factor for OSCC patients. These findings provide a theoretical basis for the future diagnostic and therapeutic value of TLSs in OSCC treatment.

Objective: To develop a standardized venous thromboembolism (VTE) prevention program for burn patients and verify its safety and effectiveness by comparing with traditional thrombus prophylaxis. Methods: All burn patients admitted and met selection criteria betweem April 2017 and September 2018 were included. Patients between January 2018 and September 2018 were included as the interventional group to implement standardized VTE prevention programs, while patients between April 2017 and December 2017 were included as the control group and traditional active and passive exercises were used to prevent VTE. There was no significant difference in the age, gender, ethnic group, marriage, education, occupation, type and site of the injury, burn area, operation time, and hospital stay between the two groups ( P>0.05), which was comparable. The incidence of VTE, number of cases of tissue or organ hemorrhage, survival rate of skin grafting, and time of wound healing were compared. Results: The incidence of VTE was obviously lower in the interventional group (1.56%, 1/64) than in the control group (10.17%, 6/59) ( χ2=-2.05, P=0.04). No bleeding occurred in any tissue or organ in the two groups. The survival rate of skin grafting and the time of wound healing were 89.06% (57/64) and (11.78±3.08) days respectively in the interventional group and 91.53% (54/59) and (11.66±2.30) days respectively in the control group; and the differences between the two groups were not statistically significant ( χ2=0.21, P=0.65; t=-0.22, P=0.83). Conclusion: The standardized VTE prevention program can effectively prevent the occurrence of VTE, and its safety is relatively high.

Objective@#To investigate the role of PI3K/Akt signaling pathway in ischemic rats underwent cardiac shock therapy.@*Methods@#Adult male Sprague Dawley (SD) rats weighing 220-250 g were used to establish a heart failure model by ligation of the left anterior descending coronary artery. Rat models were defined by echocardiographic assessment at 4 weeks post operation and heart failure rats were randomly divided into 4 groups,namely heart failure group (HF group, 9 cases),heart failure+cardiac shock waves therapy group (HF+CSWT group, 9 cases),heart failure+inhibitor(HF+LY294002 group, 9 cases),heart failure+cardiac shock waves therapy group+inhibitor (HF+CSWT+LY294002 group, 9 cases),and another 9 sham-operated SD rats served as control group (sham group, 9 cases). At 8 weeks postoperation, echocardiography was used to evaluate cardiac function in each group,myocardial infarct size was measured by TTC staining,the apoptotic index of rats cardiomyocytes were detected by TUNEL method,the myocardial mRNA expression of apoptosis-related factor was detected by real-time quantitative PCR, the protein expression levels of PI3K/Akt signaling pathway and apoptosis-related pathways were detected by Western blot.@*Results@#(1) Eight weeks after operation, left ventricular end diastolic diameter (LVEDD) and left ventricular end systolic diameter (LVESD) were significantly lower in HF+CSWT group than in HF group (all P<0.05), left ventricular ejection fraction (LVEF) and left ventricular shortening rate (LVFS) were significantly higher in HF+CSWT group than in HF group (all P<0.05),LVEF was significantly lower in the HF+ CSWT+ LY294002 group than in HF+ CSWT group (P<0.05). (2) Myocardial infarct size was significantly lower in the HF+ CSWT group than in HF group ((5.57 ± 0.51)% vs. (25.56 ± 0.56)%, P<0.05), which was significantly higher in the HF+CSWT+LY294002 group than in HF+CSWT group ((12.90±2.34)% vs. (5.57±0.51)%,P<0.05). (3) The cardiomyocyte apoptotic index was significantly lower in the HF+CSWT group than in the HF group ((30.25±6.12)% vs. (53.85±9.89)%,P<0.05), which was significantly higher in the HF+CSWT+LY294002 group than in the HF+CSWT group ((46.12±3.42)% vs.(30.25±6.12)%,P<0.05). (4) The myocardial mRNA expression of Bcl-2 was significantly higher, while myocardial mRNA Bax and Caspase-3 expression were significantly lower in HF+CSWT group than in HF group and HF+CSWT+LY294002 group (all P<0.05). (5) The expression levels of p-Akt, Bcl-2 and pro-Caspase-3 in myocardial tissue were significantly higher in the HF+CSWT group than in the HF group and HF+CSWT+LY294002 group (all P<0.05), which were significantly lower in the HF+LY294002 group than in the HF and HF+CSWT+LY294002 groups (all P<0.05). Myocardial Bax protein expression was significantly lower in the HF+CSWT group than in the HF group and the HF+CSWT+LY294002 group (all P<0.05), which was significantly higher in the HF+LY294002 group than in the HF group (P<0.05).@*Conclusion@#CSWT improves cardiac function and inhibits cardiomyocyte apoptosis through PI3K/Akt signaling pathways in this rat HF model.

Objective To investigate the effect of fluoxetine-exposure during pregnant period on depression-like behaviors and hippocampal BDNF in offspring mice.Methods Sixteen C57BL/6 pregnant mice were randomly divided into fluoxetine group (Flu group) and normal saline group (NS group),including 8 mice in each group.And the mice in the Flu group and NS group were treated with fluoxetine (10 mg/kg) and normal saline respectively by intragastric administration from the date of conception 9 to day 18.The adult offspring mice were divided into 8 groups including 4 subgroup with 8 in each group based on gender and circadian rhythm.That was Flu-MD group,Flu-MN group,Flu-FD group,Flu-FN group,NS-MD group,NS-MN group,NS-FD group and NS-FN group;"M" and "F" stands for gender;and "D"and "N" stands for circadian rhythm.The depression-like behavior of offspring adult mice was observed by tail suspension and forced swimming test,and the expression of brain derived neurotrophic factor (BDNF) in hippocampus was detected by Western blot.Results (1) There was no deformity in the adult offspring mice of NS group and Flu group;(2) In the tail suspension test and forced swimming test,there was no significant difference in cumulative immobility time between the Flu group and the NS group (P>0.05).There was no significant difference in cumulative immobility time between male and female mice in NS group or Flu group (P>0.05).There were significant differences in immobility time between Flu-MD group and Flu-MN group (tail suspension test:Flu-MD group (175.94± 30.93)s,Flu-MN group (127.65±44.91)s;forced swimming test:Flu-MD group (108.17±44.92)s,Flu-MN group (169.00±62.50) s,P<0.05),and there were no significant difference in the rest groups.(3) There was no significant difference in the expression of BDNF protein between the Flu group and NS group (P>0.05).Conclusion There is no phenotypic abnormalities on offspring when female mice were exposed to fluoxetine during their pregnancy period.It does not increase the occurrence of depression-like behavior of the adult offspring and does not affect the expression of BDNF in hippocampus.

Objective To review the package inserts of children′s common antineoplastic agents in Shanghai ,identify problems ,and seek countermeasures .Methods Reference summarizing and data analysis were used .Results The labeling rate of dosage for children in package inserts was only 38 .2% .There were no uniform standards for dosage modification . 70.6% of package inserts used the following ambiguous words to describe children dosage ,such as “no reliable reference” or“activity and safety are uncertain” .Only a few package inserts used words like“use with caution”or“not recommend”or“con-traindicated” .Conclusion The off-label use of children′s anti-tumor medications is unavoidable due to the limited children dos-age information in the package inserts .We recommend that hospital should have a protocol for off-label use which should be ex-amined by the ethics committee .The off-label uses and package insert standardization should be regulated by the related depart-ments in the government .Pharmaceutical manufacturers are encouraged to carry out post market research on children′s drug .

Objective To investigate the readiness for hospital discharge of patients who had undergone spinal operation at West China Hospital, Sichuan University, to guide the medical care and health education. Methods A total of 164 patients after spinal operation who have readying to discharge were selected and investigated by General Information Questionnaire, Activity of Daily Life Scale and Readiness for Hospital Discharge Scale. Apply the Excel for data inputting, SPSS19.0 for statistical analysis and choose rate, x±s and so on for statistical description according to the types of data. Results Of all the 164 patients, 47 cases (28.7%) weren′t well prepared, 117 cases (71.3%) were well prepared;the score of Readiness for Hospital Discharge Scale was (142.38±34.23 points and the average score of all the articles was (7.12±1.72) points. Conclusions The readiness for hospital discharge score of patients who had undergone a spinal operation at West China Hospital, Sichuan University is low and their readiness for hospital discharge is inadequate. Medical staff can improve the patients′readiness for hospital discharge by strengthening health guidance in order to promote the recovery of patients.

Medical adhesive is widely used in the medical care environment,but the medical staff pays little attention to skin injury caused by it.The incidence of medical adhesive-related skin injury is relatively high,and the patients may suffer from pain,infection and so on,result in lowering quality of life.On the basis of literature review,the author reviewed the epidemiology,pathophysiology,types,risk factors,prevention and treatment of the medical adhesive-related skin injury to provide the basis for prevention and care of such skin injury.

BACKGROUND:Many studies concerning influence of traditional Chinese medicine on animal movement capacity have been conducted.However,studies on the improvement of exercise capacity by traditional Chinese medicine are few,especially comparisons among traditional Chinese medicineOBJECTIVE:To explore the influence of different traditional Chinese medicine preparations on the exercise capacity in mice to screen compound preparations with precise effects.METHOD:A total of 80 8-week-old pure male Kunming mice were randomly assigned to 8 groups(n=10):control and compound 1-7 groups.There were no significant differences in body mass of all groups The compound 1 comprised Xuejie and Shexiang;compound 2 comprised Qingui,Baizhu and Baishao;compound 3 comprised Baizhu,Baizhi and Sanqi;compound 4 comprised Baizhi,Ruxiang,Danggui and Radix glycryrrhizae;compound 5 comprised Guiwei;compound 6 comprised Sichuan Honghua and Danshen;compound 7 comprised Danggui and Shexiang.Of the 7 prescriptions,compound 1-5 were empirical formula,compound 6 was common medicine used during training,and compound 7 was prescription of traditional Chinese medicine The 7 compound groupswere respectivelytreatedwith 1.17,0.78,0.78,1.17,7.02,5.53,and 11.70 g/kg per day for 4 weeks,and control group was intragastrically infused with normal saline.From day 4 to 6,load-free swimming training was performed 20 minutes a day for 3 days,followed by swimming for 6 days a week after 1 day of rest.The intensity was gradually increased frOm 30 minutes per day by 10 minutes per day for 2 hours,followed by loading swimming with load of 2%of the body mass,increased by 1%per day until the final experiment.During the entire procedure,a great intensity swimming training was performed every week to make the mice exhausted.Mice training and serum creatine kinase and lactate dehydrogenase 4 weeks following treatment were observed.RESULTS AND CONCLUSION:A total of 80 mice were included in the final analysis The compounds showed little influence on mice body mass;the compound 2 and compound 6 groups had longest exhaustive time All compounds inhibited serum creatine kinase activity following exercise,in particular compound 6 and 2(P<0.05).Compared with control group,the serum lactate dehydrogenase activity of each compound group was decreased,in particular,compound 2 and 6 groups showed significant differences(P<0.05).Compound 2 and 6 significantly enhanced the exercise capacity of mice and delayed injury of body.The mechanism may highly relate to components in the compounds,Danggui,Sichuan Honghua and Danshen.

Objective This paper focuses on the investigation of the occurrence and influential factors of post-traumatic stress disorder (PTSD)in SARS patients, hospital staffs and the public expo sed to SARS. Methodes Three groups were assessed by scale of self-compiled stressor Questionnaire, impact of event scale-revised (IES-R) , PSSS, SCSQ, SES and EPQ. Results 1. The incidence of PTSD symptoms in the three groups were 55.1% ,25.8 % and 31.18 %. 2. The main cause of the different incidence was the difference of the stressor intensity and characters. Negative coping was the risk factor of SARS patients and the public; Self-esteem was the protected factor of SARS patients and the hospital staffs; Social support was a protected factor on the hospital staffs. 3. Comparing to the first investigate in September,2003 ,the data of follow-up study in September,2004 showed that the prevalence items significant decreased. Conclusions 1. The investigation concludes that all three groups of people exposed o SARS exhibit PTSD symptom. SARS patients have the highest occurrence ( P < 0.01 ). 2. The main influencing factors of different incidence of PTSD in different groups are intensities and characteristics of stressors. Different people who had PTSD symptoms have different influencing factors. 3. With the change of time , the PTSD symptoms significant relieve with the attenuation gradually of exposure degree of stressor.
