RESUMO

Mandible defects could be caused by congenital malformations, trauma, osteomyelitis, tumor resection. If large areas are included for reconstruction, those are primarily due to tumor resection defects. The large jaw defect results in a problem about mastication, swallowing, occlusion and phonetics, and poor esthetics causes a lot of inconvenience in daily life. It is almost impossible to be a part underwent mandibular resection completely reproduced, should be rebuilt artificially. This case is of a patient who was diagnosed with squamous cell carcinoma pT1N0M0, stage I in February 2004 and received surgery (combined mandibulectomy and neck dissection operation (COMMANDO) in oromaxillofacial surgery) in March 2004, by implant assisted removable partial denture. We could obtain good retention and stability through sufficient coverage and implant holding. Follow up period was about four years. Mandibular left third molar regions have been observed to have resorption of surrounding bone, and periodic check-ups are necessary conditions.

BACKGROUND: Coronary artery disease are increasing in recent years among Korean due to change of dietary pattern. We conducted a cross sectional study to analyze various factors for low HDL cholesterol knowns as a independent risk factor for coronary artery disease. METHODS: At the health care center of Asan Medical Center in 1999, 7259 people without cardiovascular disease, cerebrovascular disease, hyperlipidemia were selected. They were exmined weight, height, blood pressure. Other data was drawn from questionnaire, blood chemistry. SPSS (version 7.5) was used for analysis of this data. RESULTS: 1) The means of HDL cholesterol level was higher in women than men. 2) The means of HDL cholesterol of men was lower in subgroup of age below 45 years old, obesity, lower total cholesterol( OR =200 mg/dL), current smoking, sedantary life style than others. In women, it was lower in subgroup of over 55 years old, obesity, diabetes, hypertention, lower total cholesterol( OR =200 mg/dL) than others. By multilple logistic analysis, obesity, triglyceride, current smoking, moderate alcohol drinking were significant factors associated with low HDL cholesterol (< 42 mg/dL, 25 percentile of study paricipants). CONCLUSION: These resluts suggest that obesity, high triglyceride, current smoking are independent risk factors and moderate alcoholic drinking is a negative risk factor of low HDL cholesterol in this study participants.

RESUMO

The purpose of this study was about the experience of middle aged women's aging process. The qualitative research method used was the phenomenological approach. This study tried to discover meaning of life and to understand real experiences of their life and to explore the essence of middle aged women's experience of aging. Furthermore, this study tried to establish of concept of middle age women's holistic health and to help of theoretical development to improve the health. The interviewees were 12 members. The data were collected through in-depth interviews with audiotape recording done by the investigators over a period of five months. The data were analyzed Colaizzi method, new data were continuously coded into statements, cored meanings, themes, groups of themes, and categories. The data were separated into 4 categories; body change, feeling the miss, wise coping and planning the future. The group of themes belonging to 4 categories were body change, sexual change; fleeting, tiredness, attachment of the youth; acceptance, a situation as the adult, fill up the hollow feeling, management body; dying peacefully, preparing for the senior age, being ready to pass away.

Recent study has demonstrated that the long-acting somatostatin analogue administration effectively prevented initial renal growth in diabetic and uninephrectomized rats. In the present study we examined long-term effect of somatostatin analogue (Sandostatin) on renal enlargement in uninephrectomized-diabetic rat5. Animals were divided into 4 groups: (1) normal control rats (C) (n = 7), (2) uninephrectomized rats (NPX) (n = 7), (3) uninephrectomized-diabetic rats (NPX + DM) (n = 7) and (4) NPX + DM rats treated with Sandostatin (NPX + DM + Tx) (n = 9). All animals had free access to diet (50% protein) and water during the experimental period. To the NPX + DM + Tx rats, 2.5 micrograms of Sandostatin was given subcutaneously twice a day for 8 weeks. Periodic observations were done at 0, 4 and 8 weeks. After 8 weeks. NPX rats (0.540 +/- 0.017 (SEM)) had higher fractional kidney weights (FKW) (wet kidney wt/body wt) compared to C rats (0.410 +/- 0.014) (p < 0.0005), and both NPX + DM rats (0.983 +/- 0.098) and NPX + DM + Tx rats (1.091 +/- 0.042) had higher FKW compared to C rats (p < 0.0001) and NPX rats (p < 0.005), respectively. But no significant change of FKW was observed between NPX + DM rats and NPX + DM + Tx rats. Systolic blood pressure, BUN, serum creatinine, glomerular filtration rate and 24 hour urine protein excretion in NPX + DM rats were not different from those in NPX + DM + Tx rats.(ABSTRACT TRUNCATED AT 250 WORDS)

BACKGROUND: Left ventricular hypertrophy is common in chronic renal failure patients and may contribute increased risk of cardiovascular morbidity and mortality. We evaluated the left ventricular morphology and function in renal transplant recipients to find the relationship between hemodynamic changes and morphologic and functional improvement after transplantation. METHODS: Serial echocardiographic evaluations were performed in 27 adults(20 men and 7 women) at the time of transplantaion and posttransplantation 1 month and 4 months. The average duration of hemodialysis was 16+/-24 months(mean+/-S.D.). RESULTS: At the time of transplantation, the hematocrit level was 21+/-6% and posttransplantation 1 month and 4 months, that was increased to 39+/-5% and 42+/-7%, respectively(p<0.001). Left ventricular mass index by echocardiography was decreased significantly from 246+/-87g/m2(pre-KT) to 169+/-38g/m2(post-KT 1 month) and 153+/-40g/m2(post-KT 4 months), respectively (p<0.001). Interventricular septal thickness and left ventricular posterior wall thickness were decreased significantly after 4 months of transplantation. Left ventricular systolic and diastolic dimensions were also decreased significantly after 1 month and 4 months of transplantation. Left ventricular volumes and cardiac output were also decreased significantly. But A/E ratio, ejection fraction and fractional shortening did not change significantly. CONCLUSION: These findings showed that pretransplant high output state was resolved radipidly(within 1 month) but the diastolic function did not improved after transplantation 1 month and 4 months.
