RESUMO

Aims: This study aims to comprehensively examine the surface morphology of fiber posts after undergoing various disinfection methods, utilizing scanning electron microscopy (SEM) and energy dispersive X-ray spectroscopy (EDX). Materials and Methods: Twenty-one fiber posts were randomly allocated into seven experimental groups, each consisting of three samples. The disinfection methods employed were as follows: GC - no disinfection treatment; GAL - immersion in 70% alcohol, following the manufacturer's recommended protocol; GHP - soaking in 2.5% sodium hypochlorite for a duration of 10 minutes; GCL - soaking in 2% chlorhexidine gluconate for a period of five minutes; GAC - 30-second etching with 35% phosphoric acid; GPH - soaking in 10% hydrogen peroxide for a duration of 20 minutes; and GSL - autoclave sterilization. Following the disinfection procedures, SEM was employed to scrutinize the surface topography of the posts, while EDX was utilized to identify the chemical elements present on the sample surfaces. Subsequently, a descriptive analysis was conducted on the SEM images and EDX data. Results: SEM analysis revealed that all groups exhibited regions with epoxy resin-coated fibers alongside sections with exposed glass fibers. Analysis of the EDX data indicated that there were no significant differences in the predominant chemical elements across the groups. Carbon (C) and oxygen (O) registered the highest peaks, followed by silicon (Si), zirconium (Zr), sodium (Na), aluminum (Al), and calcium (Ca). Conclusions: The disinfection methods under investigation did not induce substantial alterations in the surface morphology of the fiber posts.

RESUMO

SUMMARY: Diabetes is a form of endocrine disease. Dual-energy X-ray Absorptiometry (DXA) provides a detailed view of the body composition to find out what makes people with diabetes different from those with other diseases. We scanned 371 patients with DXA to analyze their body composition parameters. Three hundreds and seventy one patients (178 women/193 men), who with different diseases, with a mean±SD Body Mass Index (BMI) of 25.32±8.3 kg/m2 were included. The body composition of 371 patients was assessed. Bone Mineral Density (BMD), Fat Weight, Lean Weight, waist-to-hip ratio, Lean Mass Index (LMI), Fat Mass Index (FMI), the relationship between Fat percentage and BMI were analyzed. The 371 patients included 156 diabetics and 215 non-diabetics. Non-diabetic patients also included 5 obesity patients, 9 patients with fatty liver, 39 patients with hypertension, 22 patients with hyperlipidemia, 18 patients with cardiovascular disease, 11 patients with chest and lung disease, 4 patients with chronic disease, 14 patients with brain disease and 93 patients with other diseases. Among 156 diabetic patients, 129 had VAT > 100 cm2 and 27 had VAT ≤100 cm2. The lean weight (LW) of male diabetic patients was significantly higher than that of female diabetic patients. The fat weight (FW) of female patients with diabetes was significantly higher than that of male patients. The waist-hip ratio (WHR) was 1.37 ± 0.25 in male diabetic patients and 1.18 ± 0.21 in female diabetic patients. Among the 215 non-diabetic patients, the obese and fatty liver patients, which the weight (WT) (obesity: 83.87 ± 8.34 kg fat liver: 85.64±28.60 kg), FW (obesity: 28.56 ± 4.18 kg fat liver: 28.61 ± 10.79 kg), LW (obesity: 52.62 ± 9.64 kg fat liver: 54.29±17.58 kg), BMI (obesity: 28.76 ± 1.88 kg/m2 fat liver: 29.10 ± 5.95 kg/m2), was much higher than other patients. Diabetes patients had less fat mass than non- diabetic patients; the difference was around 2 kg. BMI is also a modest number. BMD doesn't differ all that much. Non-diabetic patients with fatty liver obesity and cardiovascular disease had higher fat mass and BMI than patients with other illnesses. Body composition can provide precise information on the makeup of different body areas, but further in-depth exams are required to ascertain the body's endocrine profile.

Sepsis is a global health problem; progression to septic shock is associated with a marked increase in morbidity and mortality. In this setting, increased plasma lactate levels demonstrated to be an indicator of severity and a predictor of mortality, and are usually interpreted almost exclusively as a marker of low tissue perfusion. However, a recent paradigm shift has occurred in the exegesis of lactate metabolism and its biological properties. Indeed, metabolic adaptation to stress, even with an adequate oxygen supply, may account for high circulating lactate levels. Likewise, other pathophysiological consequences of sepsis, such as mitochondrial dysfunction, are associated with the development of hyperlactatemia, which is not necessarily accompanied by low tissue perfusion. Interpreting the origin and function of lactate may be of great clinical utility in sepsis, especially when circulating lactate levels are the basis for resuscitative measures.

RESUMO

Objective To study the effect of dual-mediated brain targeting liposomes(RVGPR9-SSL)as delivery vehicles on the blood brain barrier(BBB)permeability of doxorubicin(DOX),providing a new strategy for brain drug delivery. Methods The dual-mediated brain targeting liposomes(RVGPR9-SSL)were prepared by thin film dispersion/leading compound method. And the chemo-therapeutic drug DOX was encapsulated in RVGPR9-SSL(DOX@RVGPR9-SSL).Brain microvascular endothelial cells(BMVEC)were cultured and used to construct in vitro BBB models. The BBB model was then evaluated by a 4h leakage test, transmembrane resist-ance value(TEER)measurement, and tight junctions between cells observed by scanning transmission electron microscope(SEM).After the successful construction of the BBB model, the integrity of RVGPR9-SSL after crossing the BBB was investigated by confocal laser scanning microscopy using fluorescence a resonance energy transfer(FRET)pair. The effect of RVGPR9-SSL administration on BBB in-tegrity was evaluated by comparative analysis of BBB morphology and TEER values before and after liposome administration. The BBB per-meability of DOX@RVGPR9-SSL was investigated by fluorescence spectrophotometry. Results The encapsulation efficiency of DOX@RVGPR9-SSL was 97.25%.The TEER values of the constructed BBB model were all greater than 200Ω·cm2,and it was observed by SEM that the BMVEC cells were closely arranged and there were obvious tight junctions, indicating that the in vitro BBB model was suc-cessfully established and could be used for the investigation of BBB permeability. The 4h BBB cumulative permeability of DOX@RVGPR9-SSL was greater than 10%,which was significantly higher than that of free DOX.And both BBB and liposomes maintained good integrity after administration. Conclusion RVGPR9-SSL can significantly improve the BBB permeability of DOX,indicating that it is a very promising brain drug delivery vehicle.

RESUMO

Objective:To analyze the application value of thromboelastography (TEG) in the prediction of lower limb deep vein thrombosis (DVT) after high-energy ankle injury surgery. Methods:The clinical data of 62 patients undergoing high-energy ankle injury surgery who received treatment in Wenzhou Hospital of Integrated Traditional Chinese and Western Medicine from January 2019 to December 2021 were retrospectively analyzed. Based on the occurrence of lower limb DVT, the patients were divided into a control group (without lower limb DVT, n = 42) and an observation group (with lower limb DVT, n = 20). The conventional coagulation function indicators and TEG parameters were compared between the two groups. The receiver operating characteristic (ROC) curves were used to investigate the value of classic coagulation function indicators and TEG parameters in the prediction of lower limb DVT after high-energy ankle injury surgery. Results:The activated partial thromboplastin time in the observation group was (33.29 ± 3.40) seconds, which was significantly shorter than (35.66 ± 3.36) seconds in the control group ( t = 2.59, P < 0.05). The levels of fibrinogen and D-dimer in the observation group were (3.83 ± 0.46) g/L and (1.77 ± 0.43) g/L, respectively, which were significantly higher than (3.47 ± 0.51) g/L and (1.56 ± 0.35) g/L, respectively, in the control group ( P = -2.68, -.05, both P < 0.05). Prothrombin time and thrombin time showed no significant differences between the two groups (both P > 0.05). Coagulation reaction time and cell agglutination formation time in the observation group were (3.80 ± 0.83) minutes and (3.38 ± 0.51) minutes, respectively, which were significantly lower than (4.49 ± 1.21) minutes and (3.82 ± 0.55) minutes in the control group ( t = 2.30, 2.96, both P < 0.05). The maximum clot strength and coagulation rate (α angle) in the observation group were (78.69 ± 9.22) mm and (83.37 ± 9.30) °, respectively, which were significantly greater than (68.33 ± 9.10) mm and (71.25 ± 8.35) ° in the control group ( t = -4.17, -5.15, both P < 0.05). The ROC curve analysis found that the area under the ROC curve describing the prediction of combined indexes for lower limb DVT was 0.983, which was significantly greater than the area under the ROC curve describing the prediction of a single index for lower limb DVT (all P < 0.05). Conclusion:Activated partial thromboplastin time, fibrinogen, D-dimer, coagulation reaction time, red blood cell agglutination formation time, maximum clot strength, and α angle are significantly different between patients with DVT in the lower limb after surgery for an ankle injury and those without DVT. The combination of these indicators can serve as the basis for preventing and treating thrombosis in patients after high-energy ankle injury surgery, demonstrating remarkable practical advantages.

RESUMO

AMP-activated protein kinase(AMPK)is a conserved cellular energy receptor that plays an important role in regulating cell growth, proliferation, differentiation, autophagy, phosphorylation, crosstalk, and glucose and lipid metabolism.AMPK is activated during low-energy or other extreme conditions, and it is suppressed by an excess of nutrients to maintain the energy balance.Additionally, the regulatory mechanism of the AMPK signaling pathway mediating ferroptosis reflects its unique role.AMPK plays a specialized regulatory function in various organelles, which provides a new direction for disease therapy. It is also a therapeutic target to prevent diseases such as reproductive system diseases, aging, cancer, inflammation and cardiac dysfunction. This article reviews cellular energy imbalance.AMPK stimulates its potential therapeutic potential in diseases and drugs through diverse signal regulatory mechanisms. It provides a new treatment for different system diseases. This review summarizes the diverse regulatory mechanisms of the AMPK signaling pathway and provides a theoretical reference for cancer therapy and other disease therapies targeting AMPK.

RESUMO

Objective To investigate the predictive value of serum placental growth factor(PLGF)/soluble fms-like tyrosine kinase-1(sFlt-1),combined with the placental three-dimensional energy Doppler index(3D-PDI)in preeclampsia(PE).Methods From January 2021 to December 2022,120 pregnant women with PE risk factors were selected and followed up until 1 week after delivery.Serum PLGF and sFlt-1 levels were measured at routine prenatal check-ups at 14 to 20 weeks gestation. The PLGF/sFlt-1 ratio was calculated, and placental 3D-PDI was detected by ultrasound, including the vascularization index(VI),blood flow index(FI),and vascularization-blood flow index(VFI).Based on whether PE occurred after 20 weeks of pregnancy, cases were divided into PE(55 cases)and control groups(65 cases). The PE group was further divided into mild PE(35 cases)and severe PE groups(20 cases)based on the severity of the di-sease.The differences in PLGF/sFlt-1 and 3D-PDI between the groups were compared in terms of a statistical analysis of the correlation between PLGF,sFlt-1,and 3D-PDI.The receiver operating characteristic curve(ROC)was plotted, and the predictive value of each index on PE alone or in combination was analyzed. Results The systolic blood pressure(SBP),diastolic blood pressure(DBP),24 h protein-uria level, preterm birth rate,NICU admission rate, and preconception BMI in the PE group were higher than those in the control group(P<0.05).The two groups had no differences in age, gestational age, pregnancy history, and fertility history(P>0.05).The serum PLGF/sFlt-1 of the PE group was lower than that of the control group, and the serum PLGF/sFlt-1 of the severe group was lower than that of the mild group(P<0.05).The 3D-PDI index of the PE group was lower than that of the control group, and the 3D-PDI index of the severe group was lower than that of the mild group(P<0.05).Pearson's correlation analysis indicated that PLGF and VFI were signifi-cantly positively correlated(P<0.01),and sFlt-1 was significantly negatively correlated with VFI(P<0.01).ROC curve analysis showed that PLGF/sFlt-1,VI,FI,and VFI all had predictive value for PE and the value of VI,FI,and VFI jointly predicted PE,and was higher than that of various parameters(AUC = 0.951).Serum PLGF/sFlt-1,VI,FI,and VFI combined predicted the highest value(AUC=0.987).Conclusion In patients with PE,serum PLGF,sFlt-1,and placental VFI are significantly correlated.Serum PLGF/sFlt-1,placenta VI,FI,and VFI are reduced in early pregnancy, and the combined application of the four indicators has the highest efficacy in predicting PE,providing a possible reference for the early clinical screening or prediction of PE.

RESUMO

Objective:To investigate the value of multi-parametric analysis based on dual-layer detector spectral CT (DLCT) in predicting the initial recurrence risk for papillary thyroid carcinoma (PTC).Methods:From November 2021 to October 2022, 102 PTC patients confirmed by pathology were retrospectively collected at the First Affiliated Hospital of Nanjing Medical University in this cross-sectional study. There were 25 males and 77 females, with an age of (42±13) years old. The initial recurrence risk assessment for PTC patients was categorized into a low-risk group (75 cases) and an intermediate-high-risk group (27 cases). Clinical data, including age, gender, body mass index, history of nodular goiter, history of Hashimoto thyroiditis, and preoperative thyroid function, were collected. Tumor morphological features, including size, location, shape, aspect ratio, the degree of thyroid capsule contact, calcification, and cystic change, were evaluated. Quantitative DLCT parameters, including iodine concentration (IC), standardized iodine concentration (NIC), effective atomic number (Z eff), standardized effective atomic number (NZ eff), electronic density (ED), CT values under different energy levels (40-200 keV, 30 keV intervals) and slope of energy spectrum curve (λ HU) both in the arterial and venous phase were measured. The differences in clinical, morphological features, and spectral CT quantitative parameters between the two groups were compared using independent sample ttest, Mann-Whitney U test, or χ2 test. Multivariate logistic regression analyses were used to construct three models based on clinical and morphological features, quantitative DLCT parameters and their combination, respectively. The receiver operating characteristic curve was used to evaluate the predictive performance of these models for the initial recurrence risk of PTC patients, and the area under the curve (AUC) was compared using the DeLong test. Results:Significant differences were found in gender, lesion long diameter, lesion short diameter and calcification between the low-risk group and intermediate-high-risk groups ( P<0.05). The arterial phase IC, arterial phase Z eff, arterial phase λ HU, arterial phase CT 40 keV, venous phase NIC and venous phase NZ eff in intermediate-high-risk group were significantly lower than those in the low-risk group ( P<0.05). The logistic regression analysis revealed that the clinical model included gender ( OR=2.895, 95% CI 1.047-8.002, P=0.040) and lesion long diameter ( OR=1.142, 95% CI 1.042-1.251, P=0.004), with an AUC of 0.720, sensitivity of 63.0%, and specificity of 78.7% in predicting the initial recurrence risk of PTC patients. The DLCT quantitative parameter model included arterial phase IC ( OR=0.580, 95% CI 0.370-0.908, P=0.017), venous phase NIC ( OR=0.077, 95% CI 0.011-0.536, P=0.010), and venous phase NZ eff ( OR=0.002, 95% CI 0.001-0.103, P=0.009), with an AUC of 0.774, sensitivity of 71.9%, and specificity of 70.0%. The AUC of the combined model was 0.857, with a sensitivity of 74.1%, and specificity of 88.0%, outperforming the clinical model ( Z=2.92, P=0.004) and the DLCT quantitative parameter model ( Z=2.07, P=0.046). Conclusion:Multi-parametric analysis based on DLCT can help predict the initial recurrence risk for PTC, and combining it with clinical and morphological features, the predictive accuracy can be improved.
