Background: Multiple studies have shown that standardized admission order sets (AOSs) in general as well as specific clinical diseases improved adherence to evidence-based patient management, minimized variability, improved workflow, and augmented patient safety. These order sets are intended to provide a framework for treating patients and planning a course of care. The objective of this study is to assess the implementation of SAOS in a Pediatric Intensive Care Unit (PICU) of a new public-sector hospital in Pakistan.Methods: A descriptive, cross-sectional study was conducted on all children (age: 1 month to 15 years) admitted to PICU from October to December 2022. All admission orders were paper-written based on the mnemonic "AD CAVA DIMPLS". All data were collected on Microsoft Excel and descriptive statistics were applied.Results: A total of 932 admissions were done during the first three months. The most common age and admitting diagnosis were one-year-old and lower respiratory tract diseases respectively. The overall adherence to AOS was 71.68%. Nine components of AOS [AD CAVA DIM] had 90.77% appropriate utilization.Conclusions: We observed a very high rate of implementation of AOS in our PICU.

The COVID pandemic has instilled fear in the community creating challenges for household data collection. A community-based household survey was planned to assess knowledge and perception about air pollution caused by vehicular traffic and related ailments among residents of Delhi along with willingness to accept (WTA)/willingness to pay (WTP) discount/premium to discontinue/continue the use of private vehicles for their daily office commute. Access into households was difficult due to heightened sensitivity in post COVID situation. Respondents were not available during working hours. Researchers reached out to participants at workplaces, community events, during weekends or after office hours. Safety of researchers due to odd working hours was a concern. Researchers in post-pandemic world need to plan for multiple modes to access participants. Timelines of the data collection phase need to be sensitive for higher proportion of refusals and necessary permissions. Higher investments in effective communication skills are necessary.

RESUMO Este estudo tem como objetivo apresentar os resultados da tradução, validação transcultural e avaliação preliminar de uma ferramenta, originalmente desenvolvida no Reino Unido, para orientar pesquisadores brasileiros na elaboração de projetos e pesquisas de implementação rigorosos e de alta qualidade: ImpRes-BR. Seguindo boas práticas atualmente estabelecidas para validação transcultural de instrumentos e escalas, a ferramenta, juntamente com seu guia de utilização, foi traduzida e retrotraduzida, submetida a um teste piloto com 20 profissionais de saúde e avaliada por um painel de 10 especialistas que atribuíram os valores utilizados para os cálculos do Índice de Validade de Conteúdo ao nível do item (IVC-I) e escala (IVC-E). Nesse processo, além de índices de validade conceitual superiores à 90%, foi observado um IVC-I de pelo menos 0,90 em todos os domínios da ferramenta e seu guia e um IVC-E de 0,98. Estabelecida a validade da ferramenta e seu guia, a mesma foi aplicada em 14 projetos de pesquisa em fase de planejamento ou execução e foi reconhecida enquanto um instrumento potente para autoanálise das equipes na qualificação de seus projetos e fortalecimento destes em relação aos princípios da Ciência de Implementação.
ABSTRACT This study aims to present the results of the translation, cross-cultural validation and preliminary evaluation of a tool, originally developed in the United Kingdom, to guide Brazilian researchers in the elaboration of projects and rigorous and high-quality implementation research: ImpRes-BR. Following currently established good practices for cross-cultural validation of instruments and scales, the tool, together with its user guide, was translated and back-translated, subjected to a pilot test with 20 health professionals and evaluated by a panel of 10 specialists who assigned the values used for the calculations of the Content Validity Index at the item level (CVI-I) and scale (CVI-E). In this process, in addition to conceptual validity indices greater than 90%, an IVC-I of at least 0.90 was observed in all domains of the tool and its guide, and an IVC-E of 0.98. Having established the validity of the tool and its guide, it was applied in 14 research projects in the planning or execution phase and was recognized as a powerful instrument for self-analysis of the teams in the qualification of their projects and strengthening them in relation to the principles of the Implementation Science.

Objective: To evaluate the implementation effect of serious illness medical insurance in Guang Xi Zhuang Autonomous Region of western China. Study design: Through the collection of 2017-2021 Guang Xi serious illness medical insurance specific policy making such as fund usage, serious illness compensation, medical expenses data, and data analysis of a serious illness medical insurance effect. Method: Literature research, Policy text analysis, quantitative data collection method, using Excel and SPSS 19.0 data descriptive statistical analysis and comparative analysis. Results: Serious illness medical insurance has had some effect, e.g., from 2017 to 2021, the utilization rate of serious illness medical insurance fund in Guang Xi was 109.49% and 103.87% respectively, the fund balance rate was -9.45% and -8.54% respectively, and the accumulated balance was -2.3871 million CNY and -70.7955 million CNY. Conclusion: The serious illness medical insurance has reduced the burden of large medical expenses of patients to a certain extent, but the fund is under too much pressure, and there is a payment deficit. The coverage and security of serious illness medical insurance need to be expanded and strengthened, and the cooperation mechanism with commercial insurance institutions should be gradually explored to improve the serious illness medical insurance.
Objetivo: evaluar el efecto de la implementación de seguros médicos para enfermedades graves en la Región Autónoma Guang Xi Zhuang, al oeste de China. Diseño del estudio: a través de la recopilación de pólizas de seguro médico específicas para enfermedades graves de la región de Guang Xi entre 2017-2021, como por ejemplo: uso de fondos, compensación por enfermedades graves, datos de gastos médicos y análisis de datos del efecto de seguros médicos para enfermedades graves. Método: investigación de literatura, análisis de textos de políticas públicas, metodología de recolección de datos cuantitativos utilizando Excel y SPSS 19.0, análisis estadístico descriptivo de datos y análisis comparativo. Resultados: el seguro médico para enfermedades graves ha demostrado tener efectos, tales como: entre 2017 y 2021, la tasa de utilización de fondos de seguros médicos para enfermedades graves en Guang Xi fue de 109,49 % y 103,87 % respectivamente, las tasas de saldo del fondo fue de -9,45 % y -8,54 % respectivamente y el saldo acumulado fue de -2,3871 millones de CNY y -70,7955 millones de CNY. Conclusión: El seguro médico para enfermedades graves ha reducido en cierta medida la carga de los grandes gastos médicos de los pacientes, pero al existir un déficit de pagos está bajo demasiada presión. Por ello, es necesario ampliar y fortalecer la cobertura y seguridad del seguro médico para enfermedades graves, y explorar gradualmente mecanismos de cooperación con instituciones de seguros comerciales.

Desenho do Estudo: Através da coleta de políticas específicas de seguro médico para doenças graves de Guang Xi 2017-2021, uso de fundos, compensação por doenças graves, dados de despesas médicas e análise de dados de um efeito de seguro médico para doenças graves. Método: Pesquisa na literatura, análise do texto da política, método de coleta de dados quantitativos usando Excel e SPSS 19.0, análise estatística descritiva de dados e análise comparativa. Resultados: Seguro médico de doenças graves teve um certo efeito. Em 2017 e 2021, a taxa de utilização do fundo de seguro médico para doenças graves em Guang Xi foi 109,49% e 103,87% respectivamente, a taxa de saldo do fundo foi -9,45% e -8,54% respectivamente e o saldo acumulado foi -2,3871 milhões de yuans e -70,7955 milhões de yuans. Conclusão: O seguro médico para doenças graves reduziu o ônus de grandes despesas médicas dos pacientes até certo ponto mas o fundo está sob demasiada pressão e há um déficit de pagamentos. A cobertura e segurança do seguro médico de doenças graves necessitam ser expandidas e fortalecidas, e o mecanismo de cooperação com instituições de seguros comerciais deve ser gradualmente explorado para melhorar o seguro médico para doenças graves.

Objetivo: Descrever a experiência da elaboração do Protocolo de Enfermagem da Atenção Primária e o gerenciamento das Unidades Básicas de Saúde de Rio Branco Acre. Métodos: Trata-se de um relato de experiência, sobre a elaboração de um Protocolo, instrumento potente para a implementação de melhorias e a gerência de Unidades Básicas de Saúde. A elaboração do protocolo foi realizada por etapas: Etapa 1: Composição da Comissão; Etapa 2: Pesquisa e Construção; Etapa 3: Consulta Pública; Etapa 4: Finalização da Construção. Resultados: Foi constituído uma comissão de elaboração do protocolo. Foi observado a implementação de melhorias na assistência prestada à população durante o período de execução do projeto. A frente do gerenciamento das Unidades Básicas de Saúde, foi possível vivenciar de perto o cenário da Atenção Primária à Saúde, (financiamentos, composição de equipes e atributos), conhecer a capacidade gerencial e política para organização da assistência à saúde, como também implementar melhorias relacionadas à gestão da clínica e diretrizes clínicas. Conclusão: A elaboração e implementação de um Protocolo de Enfermagem da Atenção Primária agrega não apenas conhecimento para a prática, vislumbra mais valorização profissional, onde possibilita o rompimento de uma barreira centrada na figura médica, ao atribuir à enfermagem papel relevante e conquistada, com protocolos de atendimento capazes de suprir as demandas existentes nas Unidades. Além de permitir o fortalecimento, visibilidade técnica, científica e a ampliação do acesso às pessoas à Atenção Primária à Saúde como parte significativa e resolutiva pela enfermagem. (AU)
Objective: To describe the experience of elaborating the Primary Care Nursing Protocol and the management of Basic Health Units in Rio Branco Acre. Methods: This is an experience report on the elaboration of a Protocol, a powerful instrument for implementing improvements and managing Basic Health Units. The preparation of the protocol was carried out in stages: Stage 1: Composition of the Committee; Stage 2: Research and Construction; Stage 3: Public Consultation; Step 4: Completion of Construction. Results: A protocol drafting committee was set up. The implementation of improvements in the assistance provided to the population during the project execution period was observed. Ahead of the management of Basic Health Units, it was possible to experience the Primary Health Care scenario closely, (financing, composition of teams and attributes), to know the managerial and political capacity for the organization of health care, as well as to implement related improvements. clinic management and clinical guidelines. Conclusion: The elaboration and implementation of a Primary Care Nursing Protocol adds not only knowledge to practice, it envisions more professional appreciation, which makes it possible to break a barrier centered on the medical figure, by attributing a relevant and conquered role to nursing, with protocols services capable of meeting the existing demands in the Units. In addition to allowing the strengthening, technical and scientific visibility and the expansion of people's access to PHC as a significant and decisive part of nursing. (AU)

Frente a la gestión de las Unidades Básicas de Salud, fue posible vivir de cerca el cenário de la Atención Primaria de Salud (financiación, composición de equipos y atributos), conocer la capacidad gerencial y política para la organización de la asistencia a la salud, así como implementar mejoras relacionadas, con la gestión de la clínica y direcciones clínicas. Conclusión: La elaboración e implementación de un Protocolo de Enfermería de Atención Primaria aporta no sólo conocimiento a la práctica, sino que aporta mayor valorización profesional, al permitir el rompimiento de una barrera centrada en la figura médica, para atribuir a la enfermería un papel relevante y conquistado, con protocolos de atención capaces de superar las demandas existentes en las Unidades. Además de permitir el fortalecimiento, la visibilidad técnica y científica y la ampliación del acceso de las personas a la Atención Primaria de Salud como parte significativa y resolutiva por parte de la enfermería. Background: Exclusive breastfeeding is vital for maternal and child health, yet challenges persist. This study in a Kerala tertiary care hospital aimed to assess prevalence, identify barriers, and explore cultural influences. Aim was to determine the prevalence of exclusive breastfeeding, uncover the barriers mothers face, and explore cultural factors affecting infant feeding choices.Methods: Utilizing Google forms, 274 postpartum women were surveyed. Participants provided data on awareness, practices, challenges, and factors influencing their infant feeding choices.Results: While 94.9% were aware of the ideal breastfeeding duration, only 74.1% practised exclusive breastfeeding until six months. Inadequate breast milk, sore nipples, and family pressure emerged as challenges. The timing of solid food introduction significantly impacted exclusive breastfeeding.Conclusions: Our study offers insights into exclusive breastfeeding practices. Culturally sensitive interventions, comprehensive support systems, and healthcare provider engagement are crucial. This study echoes recent research, emphasizing the multifaceted nature of exclusive breastfeeding practices, calling for diverse and context-specific strategies to empower mothers and ensure infants receive the best nutritional start.

Background: India introduced fractional-dose inactivated poliovirus vaccine (fIPV) into its routine immunization program as part of the polio eradication and endgame strategic plan. The study aimed to assess the uptake of the 3rd dose of fIPV among eligible beneficiaries and explore the implementation challenges perceived by auxiliary nurse midwives (ANMs) at two primary healthcare centers in urban Bengaluru. Methods: A mixed-method study was conducted between April and May 2023. A cross-sectional design was used for quantitative objectives, while in-depth interviews were conducted with ANMs for qualitative insights. Data were collected through vaccination registers, questionnaires, and key informant interviews. The study population included all eligible beneficiaries for the 3rd dose of fIPV from January to March 2023. Results: Out of 296 eligible beneficiaries, 139 (47%) had received the 3rd dose of fIPV. The challenges faced by ANMs in fIPV3 implementation included difficulties in recording and reporting activities, fear of managing adverse events, lack of confidence in administering intradermal injections, obtaining consent from parents, inadequate training, and concerns about vaccine wastage and increased workload. Conclusions: The uptake of fIPV3 was suboptimal, and ANMs encountered several challenges during implementation. To enhance fIPV3 integration into the vaccination schedule, the health system needs to improve awareness among the community, strengthen health worker capacity, and provide better training and support. Strengthening routine immunization programs can contribute to sustained polio eradication efforts.

Background:Inclisiran is a cholesterol-lowering small interfering RNA treatment licensed in the UK for lowering low-density lipoprotein cholesterol (LDL-C). VICTORION-Spirit (NCT04807400) is an implementation science study designed to provide evidence for inclisiran implementation within the National Health Service. The aim was to describe the process of patient identification employed in VICTORION-Spirit.Methods:A Phase IIIb, multicentre, randomisedcontrolled study, VICTORION-Spirit is evaluating inclisiran implementation in participants with atherosclerotic cardiovascular disease (ASCVD) or ASCVD-risk equivalents and elevated LDL-C. Feasibility Assessment and Recruitment System for Improving Trial Efficiency (FARSITE) software utilising natural language search functions identified patients who may benefit from inclisiran. FARSITE searches were performed within Salford, Manchester, Trafford and Bury Clinical Commissioning Groups to identify individuals with elevated LDL-C or total cholesterol and pre-existing cardiovascular disease (CVD) or at risk of ASCVD.Results:FARSITE used ‘total cholesterol >4 mmol/l’ terminology rather than ‘LDL-C’; the former yielded >3 times the number of eligible patients. The search for individuals with pre-existing CVD identified 24,196 people in a population of 560,969 (4.3%); including ‘total cholesterol >4 mmol/l’ identified 10,431 individuals with pre-existing CVD and elevated total cholesterol. Searches for individuals at risk of ASCVD identified 65,457 people, narrowing to 26,580 at risk of ASCVD plus elevated total cholesterol. The most discriminatory SNOMED concept codes and their prevalence within the dataset can inform national approaches to develop similar searches. Conclusions: FARSITE searches employed in VICTORION-Spirit identified a population at risk of ASCVD in Greater Manchester, England, who may benefit from a cholesterol-lowering medication such as inclisiran.

Introducción: La vivencia de experiencias adversas en la infancia (EAI) se asocia a mayor riesgo de presentar conductas de riesgo y enfermedades crónicas a largo plazo. A nivel mundial, se han comenzado a pesquisar y abordar en las atenciones de salud. Sin embargo, en Chile aún no existe una sugerencia a nivel nacional para incorporarlo. El fin de este artículo es proveer de información y recomendaciones a la práctica clínica. Métodos: Se realizó una revisión exploratoria en PubMed, LILACS y PsycInfo usando el marco metodológico del Joanna Briggs Institute para identificar la literatura disponible sobre implementación de intervenciones en detección y abordaje de EAI dentro de un sistema de salud, con foco en atención infantil ambulatoria. Resultados: Se encontraron 34 estudios atingentes al tema, los que muestran que implementar mecanismos de detección y abordaje de EAI es factible y aceptable, tanto para el personal de salud como para los usuarios, pero debe ser realizado dentro de un modelo de cuidado que incorpore a las familias y comunidad, además de trabajar con los equipos en capacitación, gestión del cambio, implementación y evaluación. Conclusiones: La Atención Primaria de Salud (APS) resulta ser un lugar privilegiado para su implementación dada la cercanía y relación de confianza que se establece con las familias. Este estudio muestra que es posible implementar un modelo de detección y abordaje de EAI en APS, lo que resulta crucial dentro de su rol preventivo-promocional si se quiere generar un impacto en la salud de niños, niñas y adolescentes ahora y en el futuro.
Introduction: Adverse childhood experiences (ACEs) are associated with an increased risk of developing risky health behaviors and chronic diseases in the long term. Screening for ACEs is beginning to be implemented worldwide in healthcare settings due to their known impact on present and future health. However, in Chile, there are no recommendations to incorporate ACEs screening into usual care. Methods: A scoping review was conducted using the Joanna Briggs Institute methodological framework to identify available literature on the implementation of interventions aimed at the detection and management of ACEs within a health system, specifically pediatric primary care. The search included PubMed, LILACS, and PsycInfo databases. Results: A total of 34 studies were included. They show that screening for ACEs is feasible and acceptable for both health care providers and users. However, it must be implemented as a part of a model of care that considers families and communities, besides working with health teams in training, change management, implementation, and evaluation. Conclusions: Primary Health Care (PHC) is a privileged setting for screening implementation because of the longitudinal and trust relationships established with families. This study concludes that it is possible to implement a model for detecting and managing ACEs in PHC, which will be crucial for its promotional and preventive role if there is a desire to generate an impact on infant and adolescent health now and in the future.

In the technology era, noncommunicable diseases are increasing rapidly especially in developing countries which is majorly attributed to behavioural or lifestyle risk factors. These factors are unhealthy diet, physical inactivity, and tobacco and alcohol consumption. These factors stem up in adolescent age due to its constant exposure and lack of monitoring by family and government as this group is believed to be healthy. Investing in adolescent health is prime with combined intervention with focus on multiple vistas like program science, behavioural science, implementation science and policy science in order to reduce the behavioural risk factors, which predispose to the noncommunicable diseases in adult life.

Objective:To investigate the hot topics and trends of implementation science in the transformation of clinical practice guidelines, in order to provide ideas and references for clinical managers to reasonably apply and implement scientific promotion guidelines.Methods:CiteSpace6.2.R3 software was used to visualize the literature, which retrieved from the core database of Web of Science, including the number of articles, countries and high-frequency keywords and keyword clustering and emergence for visual analysis, etc.Results:A total of 4 593 articles were included in the final analysis. Since 1993, the number of published papers had increased year by year. The hot topics focused on primary care, attitudes, knowledge translation, clinical trials, risk factors and machine learning. The research trends included older adults and artificial intelligence.Conclusions:The rapid development of implementation science in guideline translation research suggests that scholars from various countries, especially hospital administrators should reasonably apply implementation science framework to integrate evidence into clinical practice, and promote the implementation of clinical practice guidelines.

@#<strong>BACKGROUND AND OBJECTIVE</strong><p style="text-align: justify;" data-mce-style="text-align: justify;">The utilization of information and communications technology (ICT) to support health, known as eHealth, is a crucial enabler of universal healthcare. It is important to identify various aspects that could support or hinder eHealth, especially in limited-resource settings. This study determined the factors influencing the implementation of eHealth solutions in the Philippines, in consideration of the development process and initial outputs of the Philippine eHealth Strategic Framework and Plan 2014-2020.</p><strong>METHODS</strong><p style="text-align: justify;" data-mce-style="text-align: justify;">The descriptive-qualitative study was conducted among 15 municipalities/cities in the Philippines, recognized as early adopters of eHealth programs. Records review of eHealth solutions and key informant interviews among stakeholders (i.e., physicians and nurses) per study site were facilitated to gather data. Using directed content analysis, contextual, process, and content factors influencing eHealth implementation in the country were synthesized.</p><strong>RESULTS</strong><p style="text-align: justify;" data-mce-style="text-align: justify;">Results showed a range of eHealth solutions in the selected facilities, majority of which involved electronic medical records. Various contextual, process, and content-related factors could serve as facilitators or barriers to eHealth implementation in the country. Particularly, contextual factors include individual characteristics (ICT experience/training, organizational commitment, readiness for change), perceived need/urgency for eHealth (provisions, policies, regulatory issues), and third-party involvement for financial/technical support. Meanwhile, process-related factors involve implementation team practices, appropriate top-down and bottom-up approaches in leader/member engagement, and resource management (ICT equipment, stable internet connection, power supply). Content-specific factors mainly include the eHealth design (complexity, adaptability to local context and service demands, interoperability or the capacity to connect or exchange information with other platforms/systems). Notably, limitations across the three dimensions could make eHealth implementation more complicated, which could lead to poor time management and resource wastage.</p><strong>CONCLUSION</strong><p style="text-align: justify;" data-mce-style="text-align: justify;">This study highlighted the importance of a multidimensional understanding of factors that influence the utility of eHealth in the health system. There is a need for leadership and governance, stakeholder engagement, resource and funding, implementation readiness, appropriate design of eHealth solutions, and proper training to ensure the successful implementation of eHealth in the country.</p>

OBJECTIVE To investigate the implementation effects of the national centralized drug volume-based procurement policy (abbreviated as “national centralized procurement policy”) in Guangxi Zhuang Autonomous Region prefecture, and to provide a reference for the future centralized drug procurement work of the medical institution. METHODS Drug procurement data before and after policy implementation were included in the study. The six secondary indicators (such as availability, affordability, and drug safety) and eighteen third-level indicators (such as completion rate of agreed purchase volume, affordability level, drug revenue proportion) were introduced, guided by the policy objectives and issues of concern to policy beneficiaries. Descriptive statistics was adopted to analyze the data before and after policy implementation (in 2019 and 2020) in terms of differences and change trends. RESULTS In terms of accessibility, the participation rate of medical institutions in Guangxi Zhuang Autonomous Region was 92.55%, the proportion of diseases involved and median completed procurement rate were 40.16%, and 287.82% respectively, and the total centralized delivery rate was 97.20%. In terms of affordability, the total reduction amplitude in drug price was 74.80% from 2019 to 2022; the charge for medicine per capita in hospitalization, the proportion of medicine used for outpatient service and hospitalization, decreased by 17.61%, 10.22%, and 20.10% in order; the burden levels on medical fares for patients were all below 1 in addition to chronic diseases, and anti-tumor drugs. In terms of the impact on medicine, the ratio of adverse drug reaction event cases in 2022 was 66.00%, an increase of 1.29% compared to the previous; since the implementation of the policy, 12 drugs from local pharmaceutical enterprises from Guangxi Zhuang Autonomous Region had passed the consistency evaluation, and the market concentration rate of the top 8 pharmaceutical companies was less than 20.00%. In terms of the impact on healthcare and medical insurance, the public medical institutions achieved generic substitution for originator drugs mostly until 2022; about 9.12% of drugs that were non- centrally purchased in the same category were used; 63.39% of people under investigation did not show a need for a second dressing change; drug expenditure decreased by 2.459 billion yuan. CONCLUSIONS The national centralized procurement policy achieves a significant effect in Guangxi Zhuang Autonomous Region. On the other hand, attention should be paid to these suggestions as follows: expanding the category of drugs used in clinic, conducting clinically comprehensive evaluation of selected drugs, and improving reasonable allocation strategy, etc.

Background and Objective@#The Nutrition Care Process (NCP) is a systematic method used by dietitians to provide high-quality nutrition care resulting in good patient outcomes. This study aimed to assess the NCP implementation and use of NCP Terminologies (NCPT) among hospital dietitians in the Philippines. Specifically, the study aimed at assessing the knowledge, perception, and practices on NCP and use of NCPT and correlate them with the dietitians' education, and professional and employment profile; and explain the barriers and facilitators of the practice of NCP and use of NCPT among hospital dietitians in the Philippines. @*Methods@#The knowledge, perception, and practices (KPP) on NCP and NCPT of the dietitians employed in the Philippine Department of Health’s licensed level 3 hospitals were determined using a validated questionnaire. Significant factors associated with the KPP were also determined. The barriers and facilitators of the practice of NCP and NCPT were determined using focus group discussion and key informant interviews of chief clinical dietitians and hospital administrators, respectively. @*Results@#The study revealed that majority of the participants had a high level of knowledge on NCP and NCPT, positively perceived its implementation, and more than half of them implement NCP and NCPT in the hospitals. The participants’ knowledge on NCP and NCPT was significantly associated with research involvement and active membership in a professional organization. While the practice of NCP and NCPT was significantly associated with having NCP-related trainings, frequency of trainings, and active membership in a professional organization. The barriers to NCP implementation were insufficient resources; lack of orientation, trainings, and support; organizational and administrative constraints; pandemic constraints; insufficient time; and lack of confidence to conduct NCP. While the facilitators of implementation were collaboration, dedication, and commitment of the healthcare team; institutionalization of NCP laws and policies; budget allocation for NCP-related activities; monitoring and consistency of NCP implementation; and work schedule. @*Conclusion@#The findings suggest that the implementation of NCP and NCPT in the Philippines needs further support from the institution, professional organizations, and policy makers by developing strategies to cope with the barriers, and strengthen the facilitators and factors associated with practice.

The policy of long-term prescription for chronic diseases in China is gradually being improved and implemented, and external long-term prescription dispensing is being encouraged. The long-term prescription policy runs through the links of drug supply, equipment, use and policy, involving government departments such as medical security and health, as well as stakeholders such as patients, medical institutions and designated detail pharmacies. There are still some problems in the external dispensing of long-term prescriptions, such as the disunity of drug catalogue and the need for coordination among regulatory parties in the policy link; the need to improve the participation enthusiasm and service ability in the equipment link; the increased difficulty of prescription management, the need to improve the circulation platform in the use link. The promotion of external long-term prescription policy requires health insurance, medical service, and the medicine industry co-development, multi-party participation, and policy coordination. Among them, the “dual channel” policy, the policy of centralized medicine procurement, and the pharmacy included in outpatient overall management policy have all played a positive role in promoting the implementation of external long-term prescription dispensing for chronic diseases. It is necessary to improve supporting policies and implement regulatory responsibilities in the policy link, promote drug classification and service capabilities in the equipment link, improve the electronic prescription circulation platform, and strengthen prescription management in use link, so as to promote the implementation of external long-term prescription dispensing.
RESUMO
Tropical diseases, notably neglected tropical diseases and infectious diseases of poverty, remain major health problems endangering the poorest and most-marginalized people in the world. The Special Programme for Research and Training in Tropical Diseases (TDR), which is co-sponsored by the World Health Organization, the United Nations Children’s Fund (UNICEF), the United Nations Development Programme (UNDP) and the World Bank, is an important programme that helps facilitate, support, guide and coordinate global efforts to combat tropical diseases. On July 2023, TDR formally issued its 2024—2029 strategy, which proposed the direction and proprieties of global tropical disease prevention and control in the next six years. Based on its original focus on supporting researchers and research institutions from low and middle-income countries to conduct research on tropical diseases and building their research capabilities, this strategy proposed some new developments, which mainly included incorporating tropical disease prevention and control into the overall framework of addressing major global health challenges and achieving the health goals set by the United Nations Sustainable Development Goals (SDGs) to combat tropical diseases and contribute to achieving health goals of SDGs in a collaborative and integrated manner; supporting implementation research and encouraging practitioners and social innovators to participate in research to enable generation of solutions that may be used to solve local health problems; promoting and encouraging the One Health concept and interdisciplinary and cross-departmental collaboration; shifting gradually its focus from disease prevention and control to addressing the health needs of the poorest and most-marginalized populations. These new developments deserve the attention of personnel and institutions in China dedicated to the prevention and control of tropical diseases in order to help their future researches and activities.
RESUMO
OBJECTIVE To provide reference for the smooth implementation of the “dual channel” management policy for China’s medical insurance negotiated drugs. METHODS Based on Smith policy implementation process model, the dilemmas for the implementation of “dual channel” policy for medical insurance negotiated drugs were analyzed from four aspects: implementation details and regulatory system, drug selection, drug provision and quality control, the situation of medical insurance funds and information technology capabilities. The corresponding promotion strategies were put forward. RESULTS & CONCLUSIONS The “dual channel” policy for medical insurance negotiated drugs in China might face implementation difficulties such as a lack of clear implementation rules and a full process supervision system, the suitability and operability of some medical insurance negotiated drugs need to be considered in the “dual channel” management, difficulties in drug allocation and quality control, differences in the management and operation of medical insurance funds in different regions, and insufficient informatization capability. In this regard, this study suggests that measures, such as improving the implementation rules of the “dual channel” policy, enhancing the rationality of the “dual channel” drug catalog, establishing a dynamic exit mechanism for “dual channel” pharmacies, promoting professional delivery services, and improving the electronic prescription circulation platform, which can be taken to enhance the implementation effect of the “dual channel” policy.

With the reform of the medical and health system entering a critical period,public hospitals have also exposed new risks and challenges in economic operation.As an important means of hospital standardized management,internal control can better prevent and resolve the risk of hospital economic operation and ensure the sustainable operation of the hospital.By interpreting the requirements of current national policies on hospital internal control,it analyzes the functional positioning of financial and accounting supervision in hospital internal control,shares the internal control implementation path of sample hospitals from the perspective of financial and accounting supervision,and puts forward suggestions on strengthening internal control construction of public hospitals in the new era,in order to lay a good foundation for the high-quality development of hospitals.

Cet article porte sur les perceptions que les Enseignants-chercheurs de l'Université des Lettres et des Sciences Humaines Bamako ( (ULSHB) ont de l'Assurance Maladie Obligatoire. L'objectif du travail est d'analyser ces différentes perceptions. Au plan méthodologique, l'approche qualitative a été exclusivement utilisée. Les entretiens individuels de type semi-directif ont été réalisés auprès de 26 enseignants-chercheurs.Les résultats révèlent une divergence de position quant aux perceptions. En effet, la quasi-totalité des enquêtés n'ont pas été sensibilisés sur la mise en Åuvre de l'AMO. De même, ils n'ont pas été associés et affirment n'avoir pas été consentant quant au mode de prélèvement des cotisations sur le salaire. Quant à la gouvernance, elle est jugée mitigée, dans la mesure où elle est caractérisée parla politisation du système, le népotisme et le favoritisme dans les recrutements au sein de l'AMO. Sa pérennité est compromise au regard des pratiques qui l'entourent. Les efforts doivent porter sur la sensibilisation des couches socioprofessionnelles pour une adhésion de masse surtout avec la nouvelle forme d'assurance qui est le Régime d'Assurance Maladie Universelle
This article examines the perceptions of teachers and researchers at the Université des Lettres et des Sciences Humaines de Bamako (ULSHB) regarding compulsory health insurance. The aim of this study is to analyze these different perceptions. Methodologically, the qualitative approach was used exclusively. Semi-directive individual interviews were also used. The results reveal a divergence in perceptions. In fact, almost all of those interviewed had not been made aware of the implementation of AMO. Similarly, they had not been involved, and said they had not consented to the payroll deduction method. As for governance, it is deemed chaotic, insofar as it is characterized by corruption, politicization of the system, nepotism and favoritism in AMO recruitment. Its sustainability is compromised by the practices that surround it. Efforts must be focused on raising awareness among the socio-professional strata to ensure mass take-up, especially with the new form, the Universal Health Insurance Scheme.

ABSTRACT Objectives. To validate the implementation drivers scale among first-level mental health care professionals in Colombia. The scale is designed as a tool to guide the implementation of strategies that effectively reduce gaps in mental health care. Methods. The Active Implementation Framework was adopted, which is a widely used model for measuring implementation. The participants included 380 individuals (55.56% men) - 349 health personnel trained in the Mental Health Gap Action Programme (mhGAP) and 31 territorial personnel in charge of planning mental health strategies at the territorial level in Colombia. To assess the critical dimensions of mhGAP implementation, we developed a scale of 18 items based on the active implementation framework. We conducted content validity assessments and exploratory factor analysis to evaluate the scale. We used the Organizational Readiness for Knowledge Translation scale as a comparative standard. Results. The implementation drivers scale identified four dimensions: system enablers for implementation, accessibility of the strategy, adaptability and acceptability, and strategy training and supervision. These dimensions had Cronbach alpha values of 0.914, 0.868, 0.927, and 0.725, respectively, indicating high internal consistency. In addition, all dimensions demonstrated adequate correlation with the Organizational Readiness for Knowledge Translation scale. Conclusion. The implementation drivers scale effectively determines the adaptability and implementation of various components of mental health programs, particularly those focusing on community-based approaches and primary care settings. As such, this scale can contribute to the more effective implementation of strategies outlined by global and local political frameworks, thus improving mental health care.
RESUMO Objetivos. Validar a escala de determinantes da implementação entre profissionais do primeiro nível de atenção à saúde mental na Colômbia. A escala foi concebida como uma ferramenta para orientar a implementação de estratégias que reduzam efetivamente as lacunas na atenção à saúde mental. Métodos. Foi adotada a Estrutura de Implementação Ativa, um modelo amplamente utilizado para medir a implementação. O estudo incluiu 380 indivíduos (55,56% homens): 349 profissionais de saúde treinados no Programa de Ação para Reduzir as Lacunas em Saúde Mental (mhGAP, na sigla em inglês) e 31 profissionais dos territórios encarregados de planejar estratégias de saúde mental em nível territorial na Colômbia. Para avaliar as dimensões essenciais da implementação do mhGAP, criou-se uma escala de 18 itens com base na Estrutura de Implementação Ativa. Foram realizadas avaliações da validade do conteúdo e uma análise fatorial exploratória para avaliar a escala. A escala de prontidão organizacional para tradução de conhecimentos (OR4KT, na sigla em inglês) foi utilizada como padrão de comparação. Resultados. A escala de determinantes da implementação identificou quatro dimensões: facilitadores sistêmicos de implementação; acessibilidade da estratégia; adaptabilidade e aceitabilidade; e capacitação e monitoramento da estratégia. Essas dimensões tiveram valores de alfa de Cronbach de 0,914, 0,868, 0,927 e 0,725, respectivamente, indicando alta consistência interna. Além disso, todas as dimensões demonstraram correlações adequadas com a escala OR4KT. Conclusão. A escala de determinantes da implementação avalia efetivamente a adaptabilidade e a implementação de vários componentes dos programas de saúde mental, especialmente componentes que se concentram em abordagens baseadas na comunidade e ambientes de atenção primária. Dessa forma, essa escala pode contribuir para uma implementação mais efetiva de estratégias delineadas por estruturas políticas mundiais e locais, melhorando assim a atenção à saúde mental.
RESUMEN Objetivos. Validar la escala de impulsores de implementación en profesionales de la salud mental del nivel de atención primaria en Colombia. La escala está diseñada como una herramienta para orientar la implementación de estrategias que permitan reducir de manera efectiva las desigualdades existentes en la atención de salud mental. Métodos. Se adoptó el marco de implementación activa, que es un modelo ampliamente utilizado para medir este tipo de implementaciones. Los participantes fueron 380 personas (55,56% hombres), de las cuales 349 eran profesionales de la salud capacitados mediante el Programa de acción mundial para superar las brechas en salud mental (mhGAP, por su sigla en inglés) y 31 formaban parte del personal territorial encargado de planificar estrategias de atención de salud mental a nivel territorial en Colombia. Para evaluar los dominios cruciales de la implementación del mhGAP, elaboramos una escala de 18 puntos basada en el marco de implementación activa. Para evaluar la escala se realizaron determinaciones de la validez de contenido y un análisis factorial exploratorio. Como patrón de referencia se utilizó la escala Predisposición Organizacional a la Transferencia del Conocimiento para el Cambio de Práctica Clínica. Resultados. La escala de impulsores de la implementación determinó cuatro dominios: facilitadores del sistema para la implementación, accesibilidad de la estrategia, adaptabilidad y aceptabilidad, y capacitación en la estrategia y supervisión. Estos dominios presentaron valores de alfa de Cronbach de 0,914, 0,868, 0,927 y 0,725, respectivamente, lo que indica una coherencia interna elevada. Además, todos los dominios mostraron una correlación adecuada con la escala Predisposición Organizacional a la Transferencia del Conocimiento para el Cambio de Práctica Clínica. Conclusión. La escala de impulsores de la implementación permite determinar de manera efectiva la adaptabilidad y la implementación de diversos componentes de los programas de salud mental, en particular de los que se centran en enfoques basados en la comunidad y en entornos de atención primaria. En este sentido, esta escala puede contribuir a una implementación más eficaz de las estrategias esbozadas en los marcos políticos locales y mundiales, con la consiguiente mejora de la atención de salud mental.
