RESUMO

The initiation of lactation is the critical initial event contributing to breastfeeding. It is of great significance for maternal lactation and neonatal access to breast milk. However, various factors limit the successful initiation of lactation and increase the risk of poor neonatal prognosis. We provide an overview of the mechanism, significance, method, time, and factors that influence lactation initiation behavior, and summarize related guidelines aiming to gain more insight into the lactation initiation behavior to promote maternal lactation and improve neonatal prognosis.

Objetivou-se analisar a aplicação da ocitocina influenciando na eficiência da ejeção do leite; ao ponto de ocorrer um aumento da produção e mudanças na composição. Foram utilizados 45 animais, durante 105 dias, divididos em três tratamentos, no tratamento 1 (T1) os animais receberam 0,5ml de soro fisiológico, logo após realizava-se a ordenha destes animais, sendo considerado o tratamento controle, no T2 os animais receberam ocitocina antes do início da ordenha, e logo após ordenhava-se estes animais; no T3 os animais receberam ocitocina ao final da ordenha e reordenhava-se estes animais. Para aferir a produção das vacas foram usados medidores de leite, com pesagens de produção realizadas quinzenalmente. Após aferido a produção de todos os animais, as amostras de leite foram coletadas e encaminhadas para as análises posteriores. As variáveis físico-químicas do leite analisadas foram: porcentagem de gordura, lactose e proteína. Os resultados foram submetidos à análise de variância e posteriormente, ao teste de comparação de médias, com diferença estatística significativa de 5%. De acordo com os resultados obtidos a ocitocina influenciou na produção leiteira com um aumento na produtividade diária por animal, mas não se observou diferenças significativas nas médias de porcentagem de gordura, lactose e proteína do leite.

To measure the cow’s milk production, milk meters were used, and were performed fortnightly. After measured the production of all animals, samples were collected and sent for analysis. The physicochemical variables analyzed were: percentage of fat, lactose and protein. The results were submitted to ANOVA and subsequently the mean comparison test, with statistical significance of 5%. According to obtained results oxytocin influence the milk production with a significant increase in daily productivity per animal, but there was no significant difference in percentage of fat, lactose and milk protein.

Objective. The aim of this study was to provide molecular evidence of C. burnetii in sheep and goats from some herds of Valledupar, Cesar, Colombia. Materials and methods. Fifteen herds of sheep and goats were chosen by convenience to investigate the infection by C. burnetii, during March and April of 2013. 328 female goats and 66 sheep from 15 herds were included in this study. Milk from ewes and vaginal mucus samples from goats were analyzed by Polymerase Chain Reaction for DNA detection of transposase gene (IS1111) of C. burnetii. Results. DNA of C. burnetii in 6% (4/66) of sheep's milk and 0.6% (2/328) vaginal mucus from goats was found. 13% (2/15) of the herds had at least one infected animal. Discussion. Our findings suggest the circulation of C. burnetii in sheep and goats from some herds of Valledupar, Colombia, and it highlights the possibility of occurrence of infections in humans and animals. Conclusions. The detection of C. burnetii in sheep milk could represent a public health risk factor for people who consuming raw milk, cheeses or people associated to agriculture and livestock handling. Further studies are necessary to evaluate other routes such as tick's bite, feces, milk from goats and vaginal mucus from sheep of this region of Colombia.


Objetivo. El objetivo de este estudio fue proporcionar evidencia molecular de infección por C. burnetii en ovinos y caprinos de algunos rebaños de Valledupar, Cesar, Colombia. Materiales y métodos. Quince rebaños de ovinos y caprinos fueron seleccionados a conveniencia para investigar la infección por C. burnetii, durante marzo y abril de 2013. En este estudio se incluyeron 328 caprinos y 66 ovinos de 15 rebaños. La leche procedente de ovinos y muestras de moco vaginal de caprinos fueron analizados mediante PCR (Reacción en Cadena de Polimerasa) para la detección de ADN del gen transposasa (IS1111) de C. burnetii. Resultados. Se encontró ADN de C. burnetii en 6% (4/66) de leche de oveja y 0,6% (2/328) de moco vaginal de cabras. El 13% (2/15) de los rebaños tenían al menos un animal infectado. Discusión. Nuestros hallazgos sugieren la circulación de C. burnetii en ovinos y caprinos de algunos rebaños de Valledupar, Colombia, y destaca la posibilidad de ocurrencia de infecciones en humanos y animales. Conclusiones. La detección de C. burnetii en la leche de oveja podría representar un factor de riesgo para la salud pública de las personas que consumen con frecuencia leche cruda, quesos o personas que trabajan en la agricultura y manipulación de ganado. Otros estudios son necesarios para evaluar otras rutas como la mordedura de la garrapata, las heces, la leche de las cabras y el moco vaginal de las ovejas de esta región de Colombia.

ObjectiveTo investigate the effects of care bundles on the lactation of mothers of preterm infants.MethodsFrom January to June 2015, postpartum women who had preterm deliveries in First Maternity and Infant Hospital Affiliated to Tongji University with the neonates hospitalized in neonatal intensive care unit were enrolled in this study. They were randomly divided into intervention group (n=25) receiving bundle care (assist the mothers to determine lactation target; pump/express breastmilk within six hours after birth and keep a lactation diary to record the times and volume of pumped/expressed milk) and control group (n=22) given normal mammary guidance only. The time of the first pumped/expressed milk, the times for pumping/expressing milk a day, the total milk volume per day and rate of breastfeeding were compared between the two groups. Statistical analyses were conducted using two independent samplest-test,Chi-square test and nonparametric test.ResultsThe initial time of pumped/expressed milk of the intervention group was (6.1±3.4) h after delivery, significantly earlier than the control group [(10.7±9.3) h](t=-2.301,P=0.026). The times for pumping/expressing milk per day were significantly more in the intervention group than in the controlgroup on the 1st, 2nd and 3rd day after delivery [(4.2±2.2) vs (3.0±1.6); (6.2±1.1) vs (4.7±1.9); and (7.1±1.9) vs (5.9±1.9) times, respectively](t=2.083, 2.564 and 2.194, allP<0.05). From the second postpartum day, the milk volume of the intervention group was more than the control group [M(range) were 10.0(25.0) vs 2.0(5.0) ml] (Z=-2.879,P=0.005); and on the 21st day, the milk volume of the intervention mothers reached 800.0(295.0) ml, still higher than the control group [300.0(155.0) ml](Z=-3.179,P=0.001). The primarily breastfeeding (adding formula milk≤2 times a day) rate in the intervention group was significantly higher than in the control group on the 42nd postpartum day [68% (17/25) vs 41% (9/22),χ2=5.874,P=0.045].ConclusionThe care bundles on lactation can improve the lactation of the mothers of preterm infants.

The present work investigated the possible relationship between central and peripheral oxytocin (OT) release during milking in experimental ewes. Ten multiparous ewes were divided into four groups according to milk ejection stimuli: exclusive machine milking (EM), mixed-management milking and suckling, lambs separated during the night and reunited with their mother after morning milking (MMS); mixed-management with manual milking (MMM), and exclusive suckling (ES) lambs also separated during the night. Simultaneous sampling of cerebrospinal fluid (CSF) and blood was performed during milking. The means, standard deviations, variation coefficients, and minimum and maximum CSF and plasma OT concentrations were the following, respectively: 257.88 ± 265.90 pg/ml, 103.11%, and 11.70 and 1000.00 pg/ml. No statistically significant correlations were found between OT concentrations in the CSF and plasma samples (EM: -0.26; ES: -0.19; MMM: 0.05; MMS: 0.04). The OT concentration in CSF was not influenced by milk ejection stimuli, although plasma OT was higher in the MMM (679.80 ± 25.63) and MMS (591.82 ± 30.56) groups compared with the EM and ES groups. Additionally, plasma OT concentrations were higher in the OME group (381.04 ± 22.09) compared with the AE group (218.82 ± 27.04). In conclusion, no positive correlations were found between central and peripheral OT concentrations during milking and suckling. Plasma OT concentrations differed as a function of milking management and had consequences for both milk ejection and production. Plasma but not CSF oxytocin concentrations were influenced by different milk ejection stimuli.


Foi investigada a possivel relação entre as concentrações de ocitocina no líquido cefalorraquidiano e no soro em diferentes formas de ordenha em ovinos. Foram utilizadas dez ovelhas multiparas, divididas em quatro grupos de acordo com o estímulo para ejeção do leite; ordenha exclusivamente mecânica (EM), ordenha mista mamada com os carneiros separados das mães durante a noite e reunidos a elas pela manhã para amamentação (MMS); ordenha mista com ordenha manual (MMS); apenas amamentação natural (ES). Foram coletadas amostras de fluído cerebroespinhal e de sangue, simultaneamente, durante as ordenhas. A média, o coeficiente de variação e os valores máximos e mínimos de ocitocina do plasma foram respectivamente 257,88 ± 265,90 pg/ml, 103,11%, e 11,70 e 1000,00 pg/ml. Não foram encontradas correlações entre as concentrações centrais e plasmáticas de ocitocina (EM: -0,26; ES: -0,19; MMM: 0,05; MMS: 0,04). Não foi evidenciada influência do tipo de estimulo para ejeção do leite nas concentrações centrais de ocitocina. Entretanto, as concentrações plasmaticas de ocitocina foram maiores nos grupos MMM (679,80 +- 25,63) e MMS (591,82 +- 30,56), quando comparadas às dos grupos EM e ES. Além disso, as concentrações plasmáticas de ocitocina foram maiores no grupo de OME (381,04+-22,09) em relação ao grupo AE (218,82 +-27,04). Os resultados obtidos sugerem que as concentrações plasmáticas de ocitocina são mais sensíveis ao tipo de ordenha que as concentrações centrais deste hormônio.

The purpose of this quasi-experimental study was to explore the effect of aromatherapy massage on the mood, the milk ejection reflex, and the immunoglobulin A of the breast milk of mothers who gave birth through a Cesarean section delivery. Twenty mothers who had Cesarean section were selected as an experimental group, and twenty-two were in the control group. Lavander and Rosemary oil mixed with Jojova carrier oil was used to massage the back, both axillar and breasts. Aromatherapy massage was done once a day for 20 minutes by the researcher. Each session consisted of 4 minutes for warm-up, 14 minutes for massage and 2 minutes for closure. The levels of IgA within the breast milk was analyzed by an immunoturbidimeter assay (Cobas INTEGRA, Roche, Swiss) before and after aromatherapy massage. Mood and milk ejection reflex were measured by self-reports at the same time. The data were analyzed using SPSS 7.5 and the hypotheses were tested by ANCOVA and the Pearson coefficient correlation. The results were as follows: 1) Score of mood increased significantly after the use of aromatherapy massage. 2) Score of milk ejection reflex increased significantly after the use of aromatherapy massage. 3) Level of IgA of breast milk did not change significantly after the use of aromatherapy massage. 4) After the use of aromatherapy massage, there wasn't any correlation among mood, milk ejection reflex, and level of IgA of breast milk. In conclusion, the results suggest that aromatherapy massage is an effective nursing intervention to enhance the mood and the milk ejection reflex and to increase the rate of breastfeeding in the breastfeeding mothers under stresses like a Cesarean section.
