El control del comportamiento de la percepción de cansancio a lo largo del macrociclo de entrenamiento en decatlonistas constituye un indicador para la valoración del proceso de asimilación psicofísica del trabajo realizado acorde a la etapa del macrociclo de entrenamiento por la que transita el atleta. Sin embargo, el insuficiente conocimiento sobre el tema en el decatlón ha limitado el uso de este indicador en el control psicológico de la preparación en este deporte. El objetivo de este trabajo consiste en determinar el comportamiento de la percepción de cansancio en las diferentes etapas del macrociclo de entrenamiento, en decatlonistas de la selección nacional de atletismo de Cuba. Es un estudio descriptivo, en el que se aplica la entrevista para un diagnóstico inicial y la Escala de Borg (modificada), para la medición de la percepción de cansancio. Fueron realizadas 416 mediciones de la percepción de cansancio, durante las etapas de preparación física, general, especial y competitiva. Como principales resultados, la percepción de cansancio aumentó en la etapa de preparación física general en una magnitud de 3.48, en la etapa especial en un 3.43 y en la competitiva en un 2.56. La percepción de cansancio en los decatlonistas estudiados aumentó, significativamente, posterior a las cargas de entrenamiento en cada una de las etapas estudiadas y disminuyó, a medida que avanzó el macrociclo de entrenamiento. La etapa competitiva resultó ser aquella de menor cansancio percibido pre- y poscarga, y la de preparación física general, la de mayor aumento en este proceso.


O controle do comportamento da percepção da fadiga ao longo do macrociclo de treinamento em decatletas constitui um indicador para a avaliação do processo de assimilação psicofísica do trabalho realizado de acordo com a etapa do macrociclo de treinamento pela qual o atleta passa. Contudo, o conhecimento insuficiente sobre o tema no decatlo tem limitado a utilização deste indicador no controle psicológico da preparação neste esporte. O objetivo deste trabalho é determinar o comportamento da percepção da fadiga nas diferentes etapas do macrociclo de treinamento, em decatlonistas da seleção cubana de atletismo. Trata-se de um estudo descritivo, no qual é aplicada a entrevista para diagnóstico inicial e a Escala de Borg (modificada) para mensurar a percepção de fadiga. Foram realizadas 416 medidas de percepção de fadiga durante as etapas de preparação física, geral, especial e competitiva. Como principais resultados, a percepção de fadiga aumentou na etapa de preparação física geral em uma magnitude de 3,48, na etapa especial em 3,43 e na etapa competitiva em 2,56. A percepção de fadiga no decatleta estudado aumentou significativamente após as cargas de treinamento em cada uma das etapas estudadas e diminuiu à medida que o macrociclo de treinamento avançava. A etapa competitiva acabou sendo a que apresentou menor fadiga percebida pré e pós-carga, e a etapa de preparação física geral, a que apresentou maior aumento nesse processo.


The control of the behavior of the perception of tiredness throughout the training macrocycle in decathletes constitutes an indicator for the assessment of the process of psychophysical assimilation of the work carried out according to the stage of the training macrocycle through which the athlete goes. However, insufficient knowledge on the subject in the decathlon has limited the use of this indicator in the psychological control of preparation in this sport. The objective of this work is to determine the behavior of the perception of tiredness in the different stages of the training macrocycle, in tiredness of the Cuban national athletics team. It is a descriptive study, in which the interview is applied for an initial diagnosis and the Borg Scale (modified) to measure the perception of tiredness. 416 measurements of the perception of tiredness were carried out during the stages of general, special and competitive physical preparation. As the main results, the perception of tiredness increased in the general physical preparation stage by a magnitude of 3.48, in the special stage by 3.43 and in the competitive stage by 2.56. The perception of tiredness in the decathletes studied increased significantly after the training loads in each of the stages studied and decreased as the training macrocycle progressed. The competitive stage turned out to be the one with the least perceived pre- and post-load tiredness, and the general physical preparation stage was the one with the greatest increase in this process.

SUMMARY: Calcium-activated chloride channel regulator 1 (CLCA1) is associated with cancer progression. The expression and immunologic function of CLCA1 in stomach adenocarcinoma (STAD) remain unclear. In this investigation, the expression of CLCA1 in STAD tissues and its involvement in the progression and immune response of STAD were examined using databases such as cBioPortal, TISIDB, and UALCAN. In order to validate the expression level of CLCA1 protein in gastric adenocarcinoma, thirty clinical tissue specimens were gathered for immunohistochemical staining. The findings indicated a downregulation of CLCA1 in STAD patients, which was correlated with race, age, cancer grade, Helicobacter pylori infection, and molecular subtype. Through the examination of survival analysis, it was identified that diminished levels of CLCA1 within gastric cancer cases were linked to decreased periods of post-progression survival (PPS), overall survival (OS), and first progression (FP) (P<0.05). The CLCA1 mutation rate was lower in STAD, but the survival rate was higher in the variant group. The correlation between the expression level of CLCA1 and the levels of immune infiltrating cells in STAD, as well as the immune activating molecules, immunosuppressive molecules, MHC molecules, chemokines, and their receptor molecules, was observed. Gene enrichment analysis revealed that CLCA1 may be involved in STAD progression through systemic lupus erythematosus (SLE), proteasome, cell cycle, pancreatic secretion, and PPAR signaling pathways. In summary, CLCA1 is anticipated to function as a prognostic marker for patients with STAD and is linked to the immunization of STAD.


El regulador 1 del canal de cloruro activado por calcio (CLCA1) está asociado con la progresión del cáncer. La expresión y la función inmunológica de CLCA1 en el adenocarcinoma de estómago (STAD) aún no están claras. En esta investigación, se examinó la expresión de CLCA1 en tejidos STAD y su participación en la progresión y respuesta inmune de STAD utilizando bases de datos como cBioPortal, TISIDB y UALCAN. Para validar el nivel de expresión de la proteína CLCA1 en el adenocarcinoma gástrico, se recolectaron treinta muestras de tejido clínico para tinción inmunohistoquímica. Los hallazgos indicaron una regulación negativa de CLCA1 en pacientes con STAD, que se correlacionó con la raza, la edad, el grado del cáncer, la infección por Helicobacter pylori y el subtipo molecular. Mediante el examen del análisis de supervivencia, se identificó que los niveles reducidos de CLCA1 en los casos de cáncer gástrico estaban relacionados con períodos reducidos de supervivencia posterior a la progresión (PPS), supervivencia general (OS) y primera progresión (FP) (P <0,05). La tasa de mutación CLCA1 fue menor en STAD, pero la tasa de supervivencia fue mayor en el grupo variante. Se observó la correlación entre el nivel de expresión de CLCA1 y los niveles de células inmunes infiltrantes en STAD, así como las moléculas activadoras inmunes, moléculas inmunosupresoras, moléculas MHC, quimiocinas y sus moléculas receptoras. El análisis de enriquecimiento genético reveló que CLCA1 puede estar involucrado en la progresión de STAD a través del lupus eritematoso sistémico (LES), el proteasoma, el ciclo celular, la secreción pancreática y las vías de señalización de PPAR. En resumen, se prevé que CLCA1 funcione como un marcador de pronóstico para pacientes con STAD y está vinculado a la inmunización de STAD.

Objective To explore the feasibility and operability in identifying the therapeutic dominant stages of traditional Chinese medicine(TCM)based on subdivision model of disease course.Methods The hierarchical Bayesian model was used to differentiate the disease course of 125 cases of premature ovarian failure(POF),and the disease course of POF were divided into the occult stage,diminished ovarian reserve(DOR)stage,premature ovarian insufficiency(POI)stage,and POF stage.An then the paired sample t-test,Pearson correlation analysis and expert in-depth interview were used for the analysis of the therapeutic effects of TCM for POF at various stages.Results(1)Compared with POF stage,DOR and POI stages were frequently intervened by Chinese patent medicine.(2)In DOR(complicated with POI)stage and POF stage,there was significant difference between the degree of TCM intervention and the therapeutic effect(t =-3.70,P<0.001).(3)The degree of TCM intervention was positively correlated with treatment outcomes in the DOR stage(r = 0.679,P<0.001),so did in the POF stage(r = 0.432,P<0.001),but the correlation in the POF stage was slightly lower than that in the DOR stage.(4)The results of in-depth interviews with experts of TCM gynecology showed that in the concealed phase of POF,the prognosis would be most favorable if TCM regulation and intervention were performed.In the DOR stage and POI stage,treatment with Chinese medicine prescriptions usually brought about better curative effect and prognosis.For the patients at POF stage,the therapeutic effect of TCM depended on the patients'compliance and the treatment course,and the effect was relatively not as good as that of the previous stages.Conclusion In the DOR stage and POF stage,the higher the degree of TCM intervention,the better the prognosis will be achieved for the patients treated with western medicine.In the POF stage,the efficacy of TCM intervention is reduced to a certain extent compared with the DOR stage.The results indicated that it is feasible and operable to identify the TCM therapeutic dominant stages based on the subdivision model of disease course.

This paper summarized the clinical experience of Professor GE Linyi in treating ulcerative colitis (UC) by stages with the method of clearing. Professor GE believes that the core pathogenesis of UC is dampness and heat in the intestines, and by taking the method of clearing as the basis, he proposed four methods for treatment of UC including clearing and transforming, clearing and dispersing,clearing and moisterning, clearing and nourishing. The pathogenesis of UC in its active stage is dampness and heat in the intestines, congestion and stagnation of qi and blood, and accumulation of stasis toxins, for which the treatment method is to clear and transform, accompanied by clearing and dispersing method. In terms of the clearing and transforming method, Bai Tou Weng Decoction (白头翁汤) combined with Haungqin (Radix Scutellariae), Machixian (Herba Portulacae) and Pugongying (Herba Taraxaci) is taken as the basic prescription to clear and transform dampness and heat, cool blood, resolve toxins and stop dysentery. For the clearing and dispersing method, medicinals to rectify qi such as Chaihu (Radix Bupleuri), Cuxiangfu (Vingar Rhizoma Cyperi), Muxiang (Radix Aucklandiae), Zhiqiao (Fructus Aurantii), and Binlang (Semen Arecae), as well as those to regulate blood such as Danggui (Radix Angelicae Sinensis), Cebaiye (Cacumen Platycladi) and Diyutan (Radix Sanguisorbae Carbonisatus) are recommended. The pathogenesis of the remission stage is healthy qi depletion and lingering pathogen of dampness and heat stasis toxin in the intestines, for which the method of clearing and nourishing, clearing and moistening can be used; the latter is mainly for people with yin fluids injury, and self-made Qingrun Yichang Decoction (清润益肠汤) is recommended, while the former is for those with spleen and stomach weakness, and self-made Qingyang Jianpi Decoction (清养健脾汤) can be used.

Metabolic associated fatty liver disease has become the most important chronic liver disease in China.Its mechanism is not completely clear.Professor Sheng Guoguang examines the symptoms and seek the cause,treats the disease by stages from the perspective of"earth-obstructing and wood-stagnation,phlegm and blood stasis",summarizes the core pathogenesis of each stage,and prescribes drugs for the pathogenesis.In the early stage,the disease is mainly caused by earth-obstructing and wood-stagnation and phlegm and blood stasis and the corresponding treatment method should be activating spleen to eliminate depression,relieving phlegm and promoting blood circulation,with common use of modified Sizhu Decoction combined with Erchen Decoction.In the middle stage,phlegm and blood stasis transform into heat are the main pathogenesis,and the appropriate treatment is clearing liver and purging fire,and relieving phlegm and promoting blood circulation,with common use of modified Xiaochaihu Decoction combined with Erchen Decoction.In the late stage,weakness of the internal organs is the main pathogenesis,and it is appropriate to nourish liver and fortify the spleen,tonifying the kidney and consolidate the root,supplementing with reducing phlegm and activating blood circulation,with common use of modified Yiguan Decoction combined with Liujunzi Decoction in modification,which has achieved confirmed clinical efficacy.

Objective@#To learn the status and influencing factors of development among infants at ages of 0 to 36 months in Xiaoshan District, Hangzhou City, so as to provide the reference for promoting healthy development of infants.@*Methods@#Infants at ages of 0-36 months who underwent physical examination in Child Health Clinic of Xiaoshan District Community Health Service Center were selected in 2022. General data of infants and their mothers were collected through questionnaires, and the development status of infants was screened by Age and Stages Questionnaire (third edition). Factors affecting the development status were identified using a multivariable logistic regression model.@*Results@#A total of 2 519 infants were investigated, including 1 339 males (53.16%) and 1 180 females (46.84%). There were 608 infants with abnormal development of at least one functional area of communication (CM), gross motor (GM), fine motor (FM), problems solving (CG) and personal-social (PS). The abnormal rate was 24.14%, and the abnormal rates of the above functional areas were 9.77%, 6.59%, 7.98%, 6.39% and 9.33%, respectively. Multivariable logistic regression analysis showed that gender (male, OR=1.563, 95%CI: 1.191-2.052), mother's childbearing age (≥35 years, OR=1.411, 95%CI: 1.001-1.988), mother's educational level (lower than junior college, OR=1.460, 95%CI: 1.116-1.912) were factors affecting abnormal development of CM; preterm birth (OR=2.323, 95%CI: 1.315-4.103) was factors affecting abnormal development of GM; gender (male, OR=1.654, 95%CI: 1.225-2.232) was factors affecting abnormal development of FM; gender (male, OR=1.511, 95%CI: 1.086-2.102) and mode of delivery (cesarean section, OR=1.460, 95%CI: 1.060-2.010) were factors affecting abnormal development of CG; gender (male, OR=1.340, 95%CI: 1.019-1.763) and birth weight (low birth weight, OR=1.985, 95%CI: 1.149-3.432) were factors affecting abnormal development of PS.@*Conclusions@#The rate of abnormal development among infants at ages of 0 to 36 months in Xiaoshan District is 24.14%. Gender, preterm birth, mode of delivery, birth weight, mother's childbearing age and mother's educational level could affect the development status of infants.

Introducción: El cáncer de pulmón representa la causa más común de muerte por enfermedades malignas en el mundo. La tasa de respuesta al tratamiento es pobre aún en estadios iniciales y depende de varios elementos. Objetivo: Evaluar la respuesta a la quimiorradioterapia en carcinomas de pulmón en estadios iniciales según hábito de fumar, histología y etapa clínica al diagnóstico. Materiales y métodos: Se realizó un estudio de evaluación, analítico, transversal, retrospectivo. El universo estuvo constituido por los 45 pacientes con diagnóstico citohistológico de cáncer de pulmón de células no pequeñas en etapas desde la IA a la IIIA, que recibieron tratamiento de primera línea con quimiorradioterapia, atendidos en el Centro Oncológico Provincial de Matanzas, en el período de enero de 2017 a diciembre de 2019. Resultados: Se obtuvo respuesta completa en un 36,6 % de los no fumadores y en el 25 % de los fumadores. En cuanto a respuesta completa a la quimiorradioterapia de los tumores de pulmón según histología, en los adenocarcinomas fue del 10 %; en los epidermoides, 22,6 %, y en los adenoescamosos, 50 %. En etapas IA-IB presentó respuesta completa el 50 % de los casos, en las IIA-IIB el 37,5 %, mientras que en la IIIA predominó la respuesta parcial. Conclusiones: Los mejores porcientos de respuesta completa se obtuvieron en los no fumadores, con tipo histológico adenoescamoso y en etapas IA-IB.


Introduction: Lung cancer is the most common cause of death from malignant diseases in the world. The response rate to treatment is poor even in the initial stages and depends on several elements. Objective: To evaluate the response to chemoradiotherapy in lung carcinomas in early stages according to smoking habit, histology and clinical stage at diagnosis. Materials and method: An analytical, cross-sectional, retrospective evaluation study was carried out; the universe consisted of 45 patients with a cytohistological diagnosis of non-small cell lung cancer in stages from IA to IIIA, who received first-line treatment with chemoradiotherapy, attended at the Provincial Cancer Center of Matanzas, in the period of January 2017 to December 2019. Results: A complete response was obtained in 36.6% of non-smokers and in 25% of smokers. Regarding complete response to chemoradiotherapy of lung tumors according to histology, in adenocarcinomas it was 10%, in epidermoids, 22.6%, and in adenosquamous cell, 50%. In stages IA-IB, 50% of the cases presented a complete response, in stages IIA-IIB, 37.5%, while in IIIA partial response predominated. Conclusions: The best percentages of complete response were obtained in non-smokers, with adenosquamous histology type and in stages IA-IB.

Resumen El proceso de cambio hacia la práctica de actividad física todavía no ha sido adecuadamente caracterizado. Uno de los asuntos en discusión es si este proceso puede ser descrito en términos de etapas. Se llevó a cabo un estudio con el objetivo de examinar patrones de continuidad o discontinuidad de variables del Modelo Procesual de Acción en Salud dentro de las etapas propuestas por el Modelo Transteórico, y así valorar en qué medida la evidencia apoya la existencia de etapas. Para ello, se recogieron datos transversales de estudiantes universitarios (N = 490, edad = 22.5 años, DT = 6.57) y se hicieron ANOVA, contrastes planeados y análisis de tendencias polinómicas, siguiendo las recomendaciones de Sutton (2000). Los resultados para varias de las variables fueron compatibles con supuestos sobre la existencia de etapas. Sin embargo, para otras variables, los resultados no apoyan la existencia de etapas. Estos hallazgos proveen información útil para esfuerzos de integración de distintos modelos. Se discute sobre las implicaciones teóricas y prácticas de estos.


Abstract The process of change towards the practice of physical activity has not yet been adequately characterized. One issue under discussion is whether this process can be described in terms of stages. A study was carried out in order to examine patterns of continuity or discontinuity in variables of the Health Action Process Model within the stages proposed by the Transtheoretical Model, and thus assess to what extent the evidence supports the existence of stages. For this, cross-sectional data of university students (N = 490, mean age 22.5 years (SD = 6.57) were collected, and ANOVAs, planned contrasts, and polynomial trend analyses were performed, as recommended by Sutton (2000). The results for several variables were compatible with assumptions about the existence of stages. However, for other variables results did not support the existence of stages. These findings provide useful information for efforts to integrate different models. The theoretical and practical implications are discussed.

Introdução: A atuação interdisciplinar entre a fonoaudiologia e a nutrição colabora para um atendimento mais integral entre os usuários da saúde, fato que demonstra a importância de pesquisar suas correlações. Objetivo: O objetivo deste estudo foi verificar se existe integração entre a atuação do fonoaudiólogo e do nutricionista, identificando as publicações existentes, as especialidades contempladas, bem como a atuação dos profissionais nos diferentes ciclos de vida e níveis de atenção à saúde. Metodologia: Foram analisados 3.551 artigos eletrônicos disponíveis eletronicamente dos periódicos CODAS, CEFAC, Distúrbios da Comunicação e Audiology Communication Research, entre os anos de 2013 e 2022, por meio de um Protocolo de Classificação de Artigos. Resultados: Dentre as publicações analisadas, 274 (7,7%) integram a Fonoaudiologia e a Nutrição, especialmente na Especialidade da Motricidade Orofacial (n=124; 26,9%), no Ciclo Vital Infantil (n=132; 43%) e no Nível de Atenção Secundário (n=97; 35,4%). Conclusão: Concluiu-se que existe interface entre as duas profissões, porém, ainda é necessário fortalecer a atuação nos diferentes ciclos de vida. Além disso, incentivar a prática profissional em outros níveis de atenção, especialmente na primária, para prevenção dos distúrbios alimentares e promoção da alimentação saudável entre usuários do sistema de saúde.

Objective: The purpose of this study was to verify if there is integration between the performance of the speech therapist and nutritionist, identifying the existing publications, the specialities contemplated, as well the performance of professionals in different life cycles and levels of health care. Methods: 3.551 electronic articles were analyzed, in the electronic journals CODAS, CEFAC, Distúrbios da Comunicação and Audiology Communication Research, between the years 2013 and 2022, through an Article Classification Protocol. Results: Among the analyzed publications, 274 (7,7%) integrate Speech Language and Hearing Sciences and Nutrition, especially in orofacial motricity (n=124; 26,9%), Children's Life Cycle (n=132; 43%), and secondary care level (n=97; 35,4%). Conclusion: it was concluded that there is an interface between the two professions, although it is still necessary to strengthen the performance in the different life cycles. Furthermore, to encourage professional practice at other levels of care, especially in the primary, to prevent nourishment disorders and promote healthy eating among users of the health system


Introducción: El rendimiento interdisciplinario entre la terapia del habla y la nutrición contribuye a una atención más integral entre los usuarios de la salud, un hecho que demuestra la importancia de investigar sus correlaciones. Objetivo: El objetivo de este estudio fue verificar si existe integración entre la terapia del habla y la nutricionista, identificando las publicaciones existentes, las especialidades contempladas, así como la actuación de los profesionales en diferentes ciclos de vida y niveles de atención a la salud. Métodos: Fueron analizados 3.551 artículos electrónicos, en las revistas electrónicas CODAS, CEFAC, Distúrbios da Comunicação y Audiology Communication Research, entre los años 2013 a 2022, a través de un Protocolo de Clasificación de Artículos. Resultados: Entre las publicaciones analizadas, 274 (7,7%) integran terapia del habla y Nutrición, especialmente en motricidad orofacial (n=124; 26,9%), Ciclo de Vida Infantil (n=132; 43%), y secundaria nivel asistencial (n=97; 35,4%). Conclusións: Se concluyó que existe una interfaz entre las dos profesiones, aunque aún es necesario fortalecer el desempeño en los diferentes ciclos de vida. Además, incentivar la práctica profesional en otros niveles de atención, especialmente en el primario, para prevenir trastornos de la alimentación y promover la alimentación saludable entre los usuarios del sistema de salud.

Introdução: ciclo de vida é atualmente considerado elemento essencial à formulação e implantação de políticas públicas pautadas no cuidado integral à saúde. Objetivo: analisar a produção científica da Revista Distúrbios da Comunicação no período de 2016 a 2020, tendo como objeto de discussão as publicações da área da Fonoaudiologia por ciclos de vida. Método: estudo retrospectivo, de abordagem metodológica bibliométrica, analisou 297 artigos considerando as seguintes variáveis: ano de publicação, registro da região geográfica (do primeiro autor), descritores, áreas temáticas, tipo de estudo e tamanho da amostra, segundo faixas etárias (0-6; 7-11; 12-18;19-59 e >60). A análise dos dados foi descritiva, numérica e percentual. Resultados: dos 297, estudos, 258 fizeram referência a uma ou mais faixas etárias e 39 não mencionaram qualquer idade. Dentre os 258, os estudos mais frequentes foram de criança (138; 46,2%), seguido pelos de adultos (122; 41,1%). A amostra variou quanto à mediana entre 27 (adulto) a 38 (crianças: 0-6 anos) sujeitos. A região Sudeste registrou maior número de artigos relacionados a todos os ciclos, assim como os estudos do tipo observacional, com destaque para os realizados com adultos (98; 33,9%). Pouca menção foi feita a descritores que identificam os ciclos de vida (75; 6%), sendo idoso (25; 38%) e criança (23; 35%) os mais registrados. Conclusão: os dados evidenciam a premência de alinhamento das pesquisas às políticas públicas de saúde, bem como ressalta a importância do uso de descritores adequados para maior circulação do conhecimento produzido pela área. Introduction: life cycle is currently considered an essential element for the formulation and implementation of public policies based on comprehensive health care. Objective: to analyze the scientific production of Communication Disorders Journal from 2016 to 2020, having as an object of discussion publications in the field of Speech Therapy by life cycles. Method: a retrospective study, with a bibliometric methodological approach, analyzed 297 articles considering the following variables: year of publication, registration of the geographic region (of the first author), descriptors, thematic areas, type of study and sample size, according to age groups (0 -6; 7-11; 12-18; 19-59 and >60). Data analysis was descriptive, numerical and percentage. Results: of the 297 studies, 258 referred to one or more age groups and 39 did not mention any age. Among the 258, the most frequent studies were children (138; 46.2%), followed by adults (122; 41.1%). The sample ranged in terms of median from 27 (adult) to 38 (children: 0-6 years) subjects. The Southeast region recorded the highest number of articles related to all cycles, as well as observational studies, especially those carried out with adults (98; 33.9%). Little mention was made of descriptors that identify life cycles (75; 6%), being elderly (25; 38%) and children (23; 35%) the most registered. Conclusion: the data show the urgency of aligning research with public health policies, as well as highlighting the importance of using appropriate descriptors for greater circulation of knowledge produced by the area. Objetivo: analizar la producción científica de la Disturbios de la Comunicación de 2016 a 2020, teniendo como objeto de discusión publicaciones en el campo de la Logopedia por ciclos de vida. Conclusión: los datos muestran la urgencia de alinear la investigación con las políticas públicas de salud, además de resaltar la importancia de utilizar descriptores apropiados para una mayor circulación del conocimiento producido por el área. El término pediatría surge del griego paidos (niño) e iatrea (curación). Inicialmente, la pediatría fue descrita como una de las ramas independientes de la medicina en Europa Central durante la segunda mitad del siglo XIX; se independizó de áreas como la obstetricia y la medicina interna de forma progresiva. Fue descrita por primera vez en 1722 en el tratado Paedojatreja práctica por el profesor de anatomía y teoría médica Theodor Zwinger de la Universidad de Basileam. Los escritos más antiguos de los que se tiene conocimiento de enfermedades en población pediátrica datan de 2100 a. de C., en donde primaba el componente mágico-religioso (1). En Colombia, sus primeros reportes inician en 1868 con el inicio de la época denominada «Segunda etapa de la medicina colombiana¼, tras la fundación de la Universidad de los Estados Unidos de Colombia en Bogotá, en donde se implementa la cátedra de obstetricia y patología especial de las mujeres y niños, por el médico y presidente Santos Acosta. Posteriormente, se separan las asignaturas de obstetricia y patología especial de las mujeres, originándose la cátedra opcional de patología e higiene de la infancia. En 1891, José Ignacio Barberi, bogotano graduado de la Universidad Nacional de Colombia en el año 1881, se convierte en el primer especialista en dictar la asignatura Clínica infantil. En 1911, con la cátedra de pediatría en la Universidad de Antioquia, nace la especialidad en ese departamento, y el 27 de julio de 1917 se funda la Sociedad Colombiana de Pediatría, la cual propende por el bienestar de los niños y adolescentes, su familia y la sociedad, para el desarrollo de la pediatría como disciplina del hombre (2,3).


The term pediatrics comes from the Greek paidos (child) and iatrike (medicine). Initially, pediatrics was described as one of the independent branches of medicine in Central Europe during the second half of the 19th century. It progressively became independent from areas such as obstetrics and internal medicine. It was first described in 1722 in the treatise Paedojatreja práctica by the professor of anatomy and medical theory Theodor Zwinger of the University of Basel. The oldest known works on diseases in the pediatric population date back to 2100 B.C., in which the magical-religious component was predominant (1). In Colombia, its first reports begin in 1868 with the start of the period called the "Second stage of Colombian medicine". After the foundation of the Universidad de los Estados Unidos de Colombia in Bogotá. The subject of obstetrics and special pathology of women and children was implemented by physician and President Santos Acosta. Subsequently, the subjects of obstetrics and special pathology of women were separated, creating the optional subject of childhood pathology and hygiene. In 1891, José Ignacio Barberi, a Bogotá native graduated from the Universidad Nacional de Colombia in 1881, became the first specialist to teach the subject Children's medicine. In 1911, the specialization was born in the department of Antioquia with the subject of pediatrics at University of Antioquia. The Colombian Society of Pediatrics was founded on July 27, 1917, which advocates for the well-being of children and adolescents, their families and society, to develop pediatrics as a human discipline (2,3).

Resumo O objetivo foi analisar os nexos entre o Marco de Sendai para a Redução do Risco de Desastres, os Objetivos de Desenvolvimento Sustentável (ODS) e os princípios do Sistema Único de Saúde (SUS), no contexto da emergência de saúde pública da pandemia de COVID-19, e suas potenciais implicações para a saúde da população. O estudo é qualitativo, transversal, exploratório, com dados coletados junto a profissionais de saúde com experiência em práticas assistenciais e de gestão de risco de emergências e desastres, e tratados pelo software Iramuteq para análise lexical. O corpus textual foi apresentado através da classificação hierárquica descendente, que originou sete classes aglutinadas em três categorias denominadas: resposta a desastres no contexto do SUS; prevenção de futuros riscos de desastres; e ações de preparação e recuperação a partir do Marco de Sendai e dos ODS. Ressaltou-se aspectos sobre os efeitos diretos e indiretos da pandemia de COVID-19 e os desafios relacionados à redução do risco de desastres preconizados no Marco de Sendai, apontando a necessidade de fortalecer a cultura de segurança e sustentabilidade no âmbito do SUS, o que se coaduna com os ODS, bem como aos determinantes sociais da saúde.


Abstract The aim of this study was to analyze the connections between the Sendai Framework for Disaster Risk Reduction, the Sustainable Development Goals (SDGs), and the principles of Brazil's Unified Health System (SUS) in the context of the public health emergency caused by the COVID-19 pandemic and its potential implications for population health. This qualitative, cross-sectional, exploratory study collected data from health professionals with experience in emergency and disaster risk management and treatment practices, which were then processed using the Iramuteq software for lexical analysis. The textual corpus was presented through a descending hierarchical classification that resulted in seven classes grouped into three categories: disaster response in the context of SUS; prevention of future disaster risks; and preparedness and recovery actions based on the Sendai Framework and the SDGs. The study highlighted aspects related to the direct and indirect effects of the COVID-19 pandemic and the challenges related to disaster risk reduction as advocated by the Sendai Framework, emphasizing the need to strengthen the culture of safety and sustainability within the SUS, which aligns with the ODS and social determinants of health.

Deficiency, stasis, water and toxin are of great significance in the pathogenesis and pathologic evolution of chronic heart failure (CHF). Based on "deficiency, blood stasis, water and toxin", the pathogenesis and treatment of CHF were discussed in this article. It was found that in the pathogenesis, deficiency--deficiency of heart qi and deficiency of heart yang were the origin of the disease, and blood stasis, water and toxin were the markers of the disease. Among them, blood stasis was the central pathological link, and also an important mechanism that could aggravate the disease and cause a vicious cycle; water-phlegm and water dampness were the basic pathological products; toxin-heat toxin, water toxin, and stasis toxin were the final results of disease progress and product accumulation. In terms of treatment, CHF can be divided into four stages: early, middle, late and end. In the early stage, tonifying qi and regulating heart can be used for the treatment of root cause, and promoting blood circulation and water can be used for the treatment of symptoms; tonifying qi and yin and reinforcing the healthy qi, reducing blood stasis, purging turbid, and eliminating pathogenic factors can be used in the middle stage; reducing blood stasis and removing toxic materials should be used in the late stage, supplemented with warming yang and increasing urine excretion; astringing yang,generating body fluids, tonifying qi and yang should be used in the end stage. At the same time of treating by stages, attention should be paid to adhering to a holistic concept and dialectical treatment; pay attention to timing and flexible medication; adopting a combination of Chinese and Western approaches and integrating them.

Objective@#To understand the early life factors that influence cardiometabolic risk factors in children and adolescents, so as to provide effective measures to curb cardiometabolic risk factors such as hypertension and diabetes in children and adolescents.@*Methods@#Data were sourced from the 2020 follow up survey of the Xiamen Adolescent Development Cohort. The study involved 1 197 subjects for whom completed anthropometric examination and blood biochemistry testing data, as well as early life data. Early life and sociodemographic data were obtained through questionnaire surveys, while cardiometabolic indicator data were sourced through physical examinations and blood testing. Logistic regression analysis was performed to analyze the impact of early life factors on the cardiometabolic risk factors after adjusting for gender, age, and family history.@*Results@#The prevalence rate of cardiometabolic risk factors clustering in children and adolescents in Xiamen was 17.96%, with boys (26.67%) reporting higher rates than girls (9.64%), and the difference was statistically significant ( χ 2=57.69, P <0.01). For every additional early life risk factor, the risk factors of obesity increased 0.35 times ( OR=1.35, 95%CI=1.03-1.78, P <0.05). Post term pregnancy may be a primary early life risk factors for cardiometabolic risk factors, and it was associated with an increased risk of cardiometabolic risk factors clustering (OR=2.45, 95% CI =1.11-5.41) and high triglycerides ( OR=3.25, 95%CI =1.39-7.61)( P <0.05).@*Conclusion@#Increased cardiometabolic risk factors in youth is associated with early life adverse factors. It is crucial to pay greater attention to post term pregnancy as an early life factor and to consider obesity as a cardiometabolic risk factors. Controlling early life adverse factors is important for the prevention of cardiovascular disease.

Objective@#To investigate the association of the risk of overweight/obesity in children and adolescents with their early life factors and parental weight.@*Methods@#From June 1 to June 30 of 2022, the stratified randomized cluster sampling method was used to collect information using questionnaires from 5 370 primary and secondary school students in Hohhot City, Inner Mongolia Autonomous Region, and multivariate analysis were performed using multiple Logistic regression model estimation in a generalized linear model for parental BMI and early life factors in children and adolescents.@*Results@#The prevalence of overweight/obesity was 32.2% , and the prevalence of overweight/obesity among male students was 37.8%, which was higher than that of female students 26.1% χ 2= 84.59 , P <0.01). The results of the multirariate Logistic regression anaysis showed that the prevalence of overweight/obesity was higher in only paternal overweight ( OR=1.52, 95%CI =1.30-1.77), only maternal overweight ( OR=1.61, 95%CI = 1.31 -1.97), and parentl overweight ( OR=2.42, 95%CI =2.03-2.87)( P <0.05). The risk of overweight/obesity was higher in children with high birth weight ( OR=1.29, 95%CI =1.08- 1.55 ), children born by caesarean section ( OR=1.32, 95%CI =1.17- 1.48 ), and children whose mothers gained excessive weight during pregnancy ( OR=1.24, 95%CI =1.06-1.46), and the results were statistically significant ( P <0.05).@*Conclusions@#Overweight parents, excessive maternal weight gain during pregnancy, cesarean delivery, and high birth weight are associated with increased risk of overweight/obesity in children and adolescents. Attention should be paid to these aspects in obesity prevention and control.

@#Introduction: Tobacco smoking causes various chronic diseases and adds costs to healthcare systems. The current smoking cessation interventions mostly target smokers who are ready to quit and are reactive in recruitment. Recently, mobile phones have become a new tool for promoting smoking cessation. The aim of this paper is to present a study protocol on a quasi-experimental study investigating the effects of a workplace mHealth intervention for smoking behaviour. Methods: A quasi-experimental study will be conducted among employees in an academic institution in Malaysia. The intervention group will receive a theory based WhaSTOP module via WhatsApp and will be compared with the control group (receive usual care). The primary outcomes are stage movement and the number of cigarettes per day. The secondary outcomes include knowledge of smoking, quit attempt, nicotine dependence, 7-day point prevalence of smoking abstinence, and the Transtheoretical Model constructs. A generalised estimating equation analysis will be performed to determine the effects of the intervention. Discussion: This protocol will provide a novel method to proactively approach smokers regardless of readiness to quit and to guide them through the stages of change so that they will be ready to take action to quit. This research will also provide insight into whether the intervention can be utilised as an additional tool for smokers at the workplace to quit smoking. Trial Registration: The trial was registered with the Iranian Registry of Clinical Trials (Registry Number IRCT20220415054539N1).

Objective@#To investigate the associations between early life exposure to particulate matter with an aerodynamic diameter less than 2.5 μm (PM 2.5 ) and the risk of autism spectrum disorder (ASD) among school aged children.@*Methods@#A total of 165 children with ASD and 165 age and gender matched typical development (TD) children were recruited. Children s basic information were obtained via questionnaires, and the severity of ASD symptoms was assessed with Social Responsiveness Scale (SRS). Early life PM 2.5 exposure (preconception, entire pregnancy, and the first two years after birth) were extracted from the Tracking Air Pollution in China (TAP) datasets. Conditional Logistic regression and generalized linear model were used to evaluate the associations of early life exposure to PM 2.5 with the risk and the ASD severity symptoms, respectively.@*Results@#The PM 2.5 exposure of ASD group during preconception[(55.08±9.34)μg/m 3], entire pregnancy[(50.44±8.71)μg/m 3], the first year after birth [(45.04± 8.25 )μg/m 3] and the second year after birth [(40.19±7.12)μg/m 3] were significant higher than those in TD children [(47.66± 7.63 , 44.19±7.16, 38.95±6.07, 35.76±5.65)μg/m 3]( t =7.94, 7.13, 7.70, 6.32, P <0.05). After adjusting for potential confounding, each increase of 1 μg/m 3 in PM 2.5 was associated with higher risk of ASD during preconception ( OR=1.21, 95%CI =1.13-1.29), entire pregnancy( OR=1.18, 95%CI =1.11-1.26), the first year after birth ( OR=1.30, 95%CI =1.18-1.43) and the second year after birth ( OR=1.29, 95%CI =1.17-1.42). No similar results were observed regarding the analyses of SRS total and sub scale scores( P >0.05).@*Conclusion@#Early life exposure to PM 2.5 is relate to the risk of ASD, these findings indicated that more attention should be paid to ambient PM pollution in the early life prevention and control of ASD.

Transmembrane emp24 domain (TMED) gene is closely related to immune response, signal transduction, growth and disease development in mammals. However, only the Drosophila TMED gene has been reported on insects. We identified the TMED family genes of silkworm, Tribolium castaneum, tobacco moth and Italian bee from their genomes, and found that the TMED family gene composition patterns of one α-class, one β-class, one δ-class and several γ-classes arose in the common ancestor of pre-divergent Hymenoptera insects, while the composition of Drosophila TMED family members has evolved in a unique pattern. Insect TMED family γ-class genes have evolved rapidly, diverging into three separate subclasses, TMED6-like, TMED5-like and TMED3-like. The TMED5-like gene was lost in Hymenoptera, duplicated in the ancestors of Lepidoptera and duplicated in Drosophila. Insect TMED protein not only has typical structural characteristics of TMED, but also has obvious signal peptide. There are seven TMED genes in silkworm, distributed in six chromosomes. One of seven is single exon and others are multi-exons. The complete open reading frame (ORF) sequences of seven TMED genes of silkworm were cloned from larval tissues and registered in GenBank database. BmTMED1, BmTMED2 and BmTMED6 were expressed in all stages and tissues of the silkworm, and all genes were expressed in the 4th and 5th instar and silk gland of the silkworm. The present study revealed the composition pattern of TMED family members, their γ class differentiation and their evolutionary history, providing a basis for further studies on TMED genes in silkworm and other insects.

RESUMO

The approach combining disease, syndrome, and symptom was employed to investigate the characteristic changes of blood stasis syndrome in a rat model of steroid-induced osteonecrosis of the femoral head(SONFH) during disease onset and progression. Seventy-two male SD rats were randomized into a healthy control group and a model group. The rat model of SONFH was established by injection of lipopolysaccharide(LPS) in the tail vein at a dose of 20 μg·kg~(-1)·d~(-1) on days 1 and 2 and gluteal intramuscular injection of methylprednisolone sodium succinate(MPS) at a dose of 40 mg·kg~(-1)·d~(-1) on days 3-5, while the healthy control group received an equal volume of saline. The mechanical pain test, tongue color RGB technique, gait detection, open field test, and inclined plane test were employed to assess hip pain, tongue color, limping, joint activity, and lower limb strength, respectively, at different time points within 21 weeks of modeling. At weeks 2, 4, 8, 12, 16, and 21 after modeling, histopathological changes of the femoral head were observed by hematoxylin-eosin(HE) staining and micro-CT scanning; four coagulation items were measured by rotational thromboelastometry; and enzyme-linked immunosorbent assay(ELISA) was employed to determine the levels of six blood lipids, vascular endothelial growth factor(VEGF), endothelin-1(ET-1), nitric oxide(NO), tissue-type plasminogen activator(t-PA), plasminogen activator inhibitor factor-1(PAI-1), bone gla protein(BGP), alkaline phosphatase(ALP), receptor activator of nuclear factor-κB(RANKL), osteoprotegerin(OPG), and tartrate-resistant acid phosphatase 5b(TRAP5b) in the serum, as well as the levels of 6-keto-prostaglandin 1α(6-keto-PGF1α) and thromboxane B2(TXB2) in the plasma. The results demonstrated that the pathological alterations in the SONFH rats were severer over time. The bone trabecular area ratio, adipocyte number, empty lacuna rate, bone mineral density(BMD), bone volume/tissue volume(BV/TV), trabecular thickness(Tb.Th), trabecular number(Tb.N), bone surface area/bone volume(BS/BV), and trabecular separation(Tb.Sp) all significantly increased or decreased over the modeling time after week 4. Compared with the healthy control group, the mechanical pain threshold, gait swing speed, stride, standing time, and walking cycle of SONFH rats changed significantly within 21 weeks after modeling, with the greatest difference observed 12 weeks after modeling. The time spent in the central zone, rearing score, and maximum tilt angle in the open field test of SONFH rats also changed significantly over the modeling time. Compared with the healthy control group, the R, G, and B values of the tongue color of the model rats decreased significantly, with the greatest difference observed 11 weeks after modeling. The levels of total cholesterol(TC), total triglycerides(TG), low-density lipoprotein-cholesterol(LDL-C), and apoprotein B(ApoB) in the SONFH rats changed significantly 4 and 8 weeks after modeling. The levels of VEGF, ET-1, NO, t-PA, PAI-1, 6-keto-PGF1α, TXB2, four coagulation items, and TXB2/6-keto-PGF1α ratio in the serum of SONFH rats changed significantly 4-16 weeks after modeling, with the greatest differences observed 12 weeks after modeling. The levels of BGP, TRAP5b, RANKL, OPG, and RANKL/OPG ratio in the serum of SONFH rats changed significantly 8-21 weeks after modeling. During the entire onset and progression of SONFH in rats, the blood stasis syndrome characteristics such as hyperalgesia, tongue color darkening, gait abnormalities, platelet, vascular, and coagulation dysfunctions were observed, which gradually worsened and then gradually alleviated in the disease course(2-21 weeks), with the most notable differences occurred around 12 weeks after modeling.

OBJECTIVES@#Currently, the research results regarding the bilateral temporomandibular joint symmetry in patients at different ages with unilateral complete cleft lip and palate (UCLP) are still controversial. In this study, the position of condyle in the articular fossa and morphology of condyle in UCLP patients at different developmental stages was measured and analyzed to explore the asymmetry difference, which can provide a new theoretical basis for the sequential therapy.@*METHODS@#A total of 90 patients with UCLP were divided into a mixed dentition group (31 cases), a young permanent dentition group (31 cases) and an old permanent dentition group (28 cases) according to age and dentition development. Cone beam computed tomography (CBCT) images were imported into Invivo5 software for 3D reconstruction, and the joint space, anteroposterior diameter, medio-lateral diameter, and height of condylar were measured, and its asymmetry index was calculated.@*RESULTS@#The asymmetry index of condylar height and anteroposterior diameter among the 3 groups, from small to large, was the mixed dentition group<the young permanent dentition group<the old permanent dentition group (both P<0.05). There was no significant difference in condylar anteroposterior diameter and asymmetry index between the mixed dentition group and the young permanent dentition group (both P>0.05), all of them were lower than those in the old permanent dentition group (both P<0.05). Compared with the normal side, the height of fracture condyle was smaller among the 3 groups (all P<0.05), and the anterior joint space was smaller (P<0.05) and the posterior joint space was larger (P<0.05) in the mixed dentition group.@*CONCLUSIONS@#In patients with UCLP, the asymmetry of condylar morphology increases with age, but the condylar position tends to normal. These results suggest that early treatment has important clinical significance for the morphologic development of temporomandibular joint in UCLP patients.
