The article focuses on the application of CAD/CAM technology in dentistry, specifically in the manufacturing of complete dentures. This advance has transformed the way these dentures are designed and produced, with the manufacturing of layers emerging through successive 3D printing technologies such as stereolithography, digital light processing and liquid crystal display. In the present review clinical and biomechanical aspects were delved into, evaluating retention, adaptation of the base, resistance to flexion and elastic limit of the prostheses. The results indicate that digital prostheses offer better retention and adaptation compared to conventional methods, although there are no statistically significant differences in bending resistance. Economic aspects were also explored, highlighting the cost reduction and time savings in the clinical process with the digital approach. Patients experience greater satisfaction with 3D printed dentures in terms of comfort and efficiency. However, there are limitations, such as the difficulty in the aesthetic evaluation of monochromatic dentures, in addition to the dependence on conventional techniques to evaluate the patient's functionality. CAD/CAM technology has revolutionized the manufacturing of total dental prostheses, offering advantages and benefits. These technological advance promises to maintain its relevance in the scientific and dental community.

ABSTRACT Introduction : Various authors mention that correct planning of a removable partial denture can offer adequate satisfaction in terms of aesthetics and function. In cases where aesthetics is a priority, a design without visible clasps is proposed. In addition, this elaboration does not demand higher costs, if the practitioner reaches the corresponding diagnosis and establishes the most convenient design for each specific case. Objective: The objective was to carry out a systematic review on the perception of quality of life and satisfaction in patients with removable partial dentures. Materials and methods: Thirteen scientific articles were included using the PRISMA methodology for the identification, review and inclusion of the texts that were part of this study. The articles were selected considering the title, abstract and objective. Systematic reviews, meta-analyses, comparative studies, and literature reviews that included criteria about satisfaction in patients with removable partial dentures where evaluated. The articles were read in their entirety: the objectives, methodology and conclusions of each one of them were analyzed. Results: The most common complaint was the aesthetic result, followed by pain during chewing. It is recommended to offer periodic control of the removable partial denture to evaluate its correct functioning and fit. Conclusion: The success of the treatment with removable partial dentures can be evaluated through the precise diagnosis of the case, considering previous experiences and taking into account the expectations of the patient. Also, understanding the patient's lifestyle, socioeconomic level, periodontal health status, and personality, may be factors that allow the practitioner to reduce the risk of future failure for the treatment with removable partial dentures.

RESUMO

Background: Denture adhesives are alternatives used to improve retention, stability, comfort and satisfaction in patients with complete dentures. Evidence on the effectiveness of denture adhesives on resorbed mandibular ridges is scarce. Among the many commercially available denture adhesives, the ideal material for the severely resorbed mandibular ridge remains in dispute. Objective: The aim of this study was to evaluate the effectiveness of different quantities of four commercially available denture adhesives on the retention of mandibular complete dentures in severely resorbed ridges. Materials and Methods: A resorbed edentulous mandibular ridge model was manufactured in acrylic resin. A denture base was made and three loops were attached to it. Four commercially available denture adhesives (Fixodent, Perlie White, Fiftydent and Polident) were tested in amounts of 0.2 g, 0.4 g, 0.6 g, 0.8 g and 1.0 g. The acrylic resin model was evenly moistened with 1 ml of water and a weighted amount of adhesive material was applied to the denture base. The universal testing machine engaged the loops fixed on the denture base and applied the vertical displacement force. The maximum vertical displacement force values were recorded for each denture adhesive material at different amounts. Statistical calculation was performed using Kruskal Wallis with Bonferroni post hoc correction. Results: Statistically significant differences were observed in the mean values of the vertical displacement force for adhesive amounts of 0.2 g, 0.4 g, 0.6 g and 1.0 g (p<0.05) between the four adhesive materials tested. Statistically significant differences were observed when four denture adhesives were compared to each other in different amounts (p<0.05). Statistically significant differences were observed between different amounts for each of four denture adhesives (p<0.05). Conclusions: Among the four materials tested, Polident showed greater effectiveness at 0.6 g and 0.8 g, Fittydent at 0.6 g, 0.8 g and 1 g, Fixodent at 0.4 g and 0.6 g and Perlie White at 1 g and 0.8g quantity to resist vertical displacement. forces on the severely resorbed mandibular crest. Using an appropriate amount of denture adhesive allows for proper retention of the denture; Replacement of this adhesive is necessary once a day.

The objective of the study is to assess the precision of digital impressions in removable partial dentures through an in vitro study on the degree of abutment adaptation. A Kennedy Class III model with a prosthetic space between elements 43 and 47, featuring niches in the mesio-occlusal and cingulum regions, respectively. Conventional impressions were performed in subgroups CONC and CONM, while digital scanning was conducted in subgroups DIGC and DIGM. Simplified cobalt-chromium alloy frameworks were manufactured using the lost-wax technique on plaster and resin models. The degree of adaptation of the structures was evaluated by impressing the niches with condensation silicone, qualitatively assessing perforations, and quantitatively measuring the mold thickness under a stereomicroscope after cross-sectioning. Regular adaptation was more prevalent among the experimental groups. CONC showed a higher mean degree of abutment adaptation, while CONM had a lower mean. The study factors, impression technique, and type of abutment seat, were not statistically significant, with no interaction among the variables. Occlusal and cingulum abutment measurement points, in both impression techniques, showed no statistically significant difference. Digital scanning yielded better results in terms of abutment adaptation, with smaller average gaps between the abutment seat and the metal structure, making it clinically acceptable. The type of abutment seat and the impression technique did not have a statistically significant impact on abutment adaptation. The impression technique does not represent a factor influencing the adaptation of occlusal and cingulum abutments at different measurement points. (AU)

(AU)

RESUMO

The choice of retaining material that will replace the lost properties of the tooth, in which it is fragile and with loss of dentin structure, is important. The success of the treatment will depend on the qualities presented in it. Therefore, the objective of this work, through a literature review, is to present the advantages and disadvantages observed in the use of fiberglass post intraradicular retainer. This paper reviews the most relevant articles on the specificities presented in the fiberglass post retaining material, published in the period (2003 to 2020). The bibliographic search was performed in PubMed, Lilacs, Scielo, Google Scholar databases. In view of the selected articles on the subject, it is possible to observe the authors' acceptance of the application of the use of retainers, highlighting that in the treatment of cases with extensive coronary destruction, the aim is to have the functional and retentive capacity returned, in addition to the tension resistance. Therefore, theThe use of fiberglass posts in weakened teeth is effective as the material of choice. In cases that require reconstruction and return of tissue integrity, it has sufficient advantages for its selection. Despite the disadvantages it has, these do not outweigh the benefits to the point of making its use unfeasible. However, knowing when, where and how to treat is always important in any procedure for a satisfactory final outcome.

Objective@#To explore an accurate method to obtain an intraoral model of patients with specific limited mouth opening (microstomia) due to systemic scleroderma.@*Methods@#This study followed medical ethics, and informed consent has been obtained from patients. A case of Ken's Type I mandibular dentition defect scleroderma with limited mouth opening was addressed with digital technology as the leading method combined with the traditional impression method of segmental impression. Individual trays were made based on the patient's left and right mandibular dentition, and segmented molds were obtained. Simultaneously, intraoral scanning was performed to obtain the morphological data of both the soft and hard tissues of the upper and lower mandibles. After each part of the model was obtained, the mandibular model was scanned and digitally aligned to form the final denture model, and the final removable partial denture was designed and made by computer aided design/computer aided manufacturing (CAD/CAM) technology. At the same time, combined with the literature, the diagnosis and treatment of removable partial denture in patients with limited mouth opening were retrospectively analyzed.@*Results@#The denture was well retained and achieved a good repair effect. The patients expressed satisfaction with the mastication efficiency and other functions of the denture. The findings of the literature review show that the integration of digital technology with the traditional impression method, along with computer fitting, can accurately obtain the patient's oral model and facilitate successful follow-up repairs. However, when the anterior mandibular dentition of the patient is absent, the margin of error is increased in this procedure, which deserves further exploration.@*Conclusion@#Utilizing digital technology as the leading method, combined with the traditional impression method of segmental impression, for the repair of dental defects in patients with limited mouth opening, has proven to be effective. Thus, patients report a positive medical experience with high satisfaction, indicating that this approach is worthy of clinical promotion.

Objective:To analyze the influence of forming direction on the surface characteristics, elastic modulus, bending strength and fracture toughness of printed parts and the relationship between forming direction and force direction, and to provide scientific basis and guidance for the clinical applica-tion of oral denture base resin materials. Methods:The 3D printing technology was used to print denture base resin samples. The shape and size of the samples referred to the current standard for testing conven-tional denture base materials. The samples used for physical performance testing were cylindrical(with a diameter of 15 mm and a thickness of 1 mm)and printed at different angles along the Z axis(0°,45°,90°).Scanning electron microscope was used to observe the microscopic topography of the different sam-ples.The color stability of different samples was observed by color stabilizer. The surface roughness of the samples was analyzed by using surface roughness tester. The Vickers hardness was measured to ana-lyze the hardness of the samples. The samples used for mechanical performance testing were rectangular(elastic modulus and bending strength:A length of 64 mm,a width of 10 mm,and a height of 3.3 mm;fracture toughness:A length of 39 mm,a width of 8 mm,and a height of 4 mm),divided into two groups:W group and H group. The W group was printed from the bottom up along the Z axis with the length × width as the bottom surface parallel to the X,Y axis plane, while the H group printed from the bottom up along the Z axis with the length × height as the bottom surface parallel to the X,Y axis plane. The forming angles of both groups were equally divided into 0°,45°,and 90°.The elastic modulus, bending strength and fracture toughness of different samples were studied through universal mechanical testing machine.SPSS 22.0 software was used for statistical analysis. Results:The microscopic topogra-phy and roughness of different samples were closely related to the printing direction, with significant differences between the 0°,45°,and 90° specimens. The 0° specimens had the smoothest surface(roughness<1 μm).The surface of the 45 ° specimen was the roughest(roughness>3 μm).The microhardness of the 0° sample was the best[(196.13±0.20)MPa],with a significant difference com-pared with the 90° sample[(186.62±4.81)MPa,P<0.05].The mechanical properties of different samples were also closely related to the printing direction. The elastic modulus, bending strength, and fracture toughness of the 45° samples in the W group were the highest compared with the other groups. The results of elastic modulus showed that in the H group, the 45° specimens had the highest elastic mo-dulus,which was significantly different from the 0° and 90° specimens(P<0.05).The elastic modulus of 0° and 45° specimens in the W group were higher than those in 90° specimens(P<0.05).The bending strength results showed that there was no significant difference between the specimens from dif-ferent angles in the H group. The bending strength of the 90° specimens in the W group was the smallest, and there was a significant difference between 90° and the 0° and 45° specimens(P<0.05);And the bendind strength of the 0° and 45° specimens in the W group was significantly higher than that of the 0° and 45° specimens in the H group(P<0.05).The fracture toughness results showed that the fracture toughness of the H group specimens was lower than 1.9 MPa m1/2,which was specified in the denture base standard. The 45° samples in the W group were the highest, with significant differences compared with the 0° and 90° samples(P<0.05).And the 90° samples of the W group specimens were lower than 1.9 MPa m1/2.And the fracture toughness of the 45° specimen in the W group was significantly higher than that of all the specimens in the H group(P<0.05).Conclusion:The 0° samples had rela-tively better physical properties. The 45° samples had the best mechanical properties. But the fracture toughness of specimens(H group and 90° samples of W group)did not yet meet clinical requirements. That indicated that the characteristics of the 3D printing denture base resin were affected by the printing direction.Only when the performance of the printed samples in all directions met the minimum require-ments of the standard, they could be used in clinical practice.

RESUMO

BACKGROUND:In recent years, additive manufacturing(also known as 3D printing)has gradually become the mainstream method for producing titanium alloy brackets for removable partial dentures.Heat treatment,as an important method to improve the mechanical properties of 3D printed titanium alloys, has become a current hot topic of attention.OBJECTIVE:To summarize the main heat treatment technologies currently applied to 3D printed titanium alloy specimens(including annealing, solution aging, hot isostatic pressing, and other heat treatments)and their effects on the mechanical properties and microstructure of 3D printed titanium alloy specimens, providing a theoretical basis for improving the heat treatment technology of removable partial denture titanium alloy supports.METHODS:A computer search was conducted on research materials related to 3D printed titanium alloy heat treatment in databases such as CNKI,PubMed,and ScienceDirect. The search period was from 2012 to 2023.A total of 61 articles were selected based on inclusion and exclusion criteria.RESULTS AND CONCLUSION:(1)Using conventional annealing techniques to treat 3D printed titanium alloy specimens, keeping them at 500-900 ℃ for 2-4 hours, can effectively increase the elongation of 3D printed titanium alloy specimens.(2)Compared to conventional annealing techniques, solid solution aging treatment is more complex, and the titanium alloy specimens after solid solution aging treatment exhibit outstanding yield strength and better corrosion resistance. However, the 3D printed titanium alloy specimens after solid solution aging treatment have no advantage in terms of ductility.(3)Hot isostatic pressing treatment can reduce the internal defects of 3D printed titanium alloy specimens, significantly increase the elongation of 3D printed titanium alloy specimens, and increase their fatigue life.(4)Rapid heat treatment can significantly improve the elongation of 3D printed titanium alloy specimens, and the speed is faster. In terms of elongation improvement and heat treatment efficiency,it has more advantages than conventional annealing in the past.(5)The improvement of elongation of 3D printed titanium alloy specimens by cyclic heat treatment exceeds that of conventional annealing.Cyclic heat treatment can significantly improve the grain structure of 3D printed titanium alloy specimens, but the heat treatment time is too long and the efficiency is low.

RESUMO

Aim: This study aimed to assess the effect of two resilient attachments used for retention of mandibular over-denture supported by an implant and a natural tooth on the supporting structures throughout a one-year follow-up period. Materials and methods: Fourteen patients with maxillary completely edentulous arches and only one standing natural mandibular canine were selected. Each patient received a dental implant with 10.5 mm length and 3.8 mm diameter at the canine region of the contralateral side of the mandible. The patients were divided into two groups according to the type of attachment used (n=7); BC group included patients who received mandibular over-dentures retained by bar clip attachment supported by an implant on one side and a natural tooth on the other side. BS group involved patients who received a mandibular implant splinted to the natural canine by bar joint attachment utilizing soft liner material. The patients of both groups were rehabilitated with conventional maxillary complete dentures. Clinical and radiographic assessment were performed immediately after over-denture delivery, six and twelve months from denture insertion. Clinical evaluation involved gingival index. Radiographic evaluation included measuring mesial, distal, buccal and lingual bone height differences. Results: BC group recorded significantly higher bone loss compared to BS group around both dental implants and natural teeth. Conclusion: Considering the limitations of this study, it could be concluded that soft liner retained over-dentures are more effective in decreasing bone resorption around the implants and natural teeth compared to clip retained over-dentures.

RESUMO

Abstract Objective: To evaluate the utility of panoramic radiographs in pre-prosthetic screening of edentulous arches. Material and Methods: Panoramic radiographs taken for three years were retrospectively analyzed. Observations from the radiographs shall be categorized and classified into either of the two categories, namely: 'findings with minimal impact on denture fabrication' and 'findings which affect denture fabrication and require further evaluation.' Anatomic variations, jaw pathologies, and residual ridge resorption patterns were assessed. Results: This study included the initial screening of 23,020 panoramic radiographs, out of which 505 (showing either one or both edentulous arches) were included for the study purpose. The age range of the subjects was from 21 to 94 years. 52.6% of the radiographs showed positive findings. More than half of the radiographs belonged to the males (52.5%). Hyperpneumatization of the maxillary sinus, crestal position of the mental foramen, and retained root fragments were the most common entities noted in the radiographs. Changes in the mental foramen were significantly higher in males than females (p=0.002). Conclusion: Observations from this study showed that panoramic radiographs have high utility for screening edentulous arches, and they should be used in routine clinical practice before denture fabrication.

RESUMO

Abstract Denture biofilm acts as a potential reservoir for respiratory pathogens, considerably increasing the risk of lung infections, specifically aspiration pneumonia, mainly 48h after hospital admission. The establishment of a straightforward, affordable, and applicable hygiene protocol in a hospital environment for the effective control of denture biofilm can be particularly useful to prevent respiratory infections or reduce the course of established lung disease. Objectives To evaluate the anti-biofilm effectiveness of denture cleaning protocols in hospitalized patients. Methodology The maxillary complete dentures (MCDs) of 340 hospitalized participants were randomly cleaned once using one of the following 17 protocols (n=20): brushing with distilled water, toothpaste, or neutral liquid soap (controls); immersion in chemical solutions (1% sodium hypochlorite, alkaline peroxide, 0.12% or 2% chlorhexidine digluconate), or microwave irradiation (650 W for 3 min) combined or not with brushing. Before and after the application of the protocols, the biofilm of the intaglio surface of the MCDs was evaluated using two methods: denture biofilm coverage area (%) and microbiological quantitative cultures on blood agar and Sabouraud Dextrose Agar (CFU/mL). Data were subjected to the Wilcoxon and Kruskal-Wallis tests (α=0.05). Results All 17 protocols significantly reduced the percentage area of denture biofilm and microbial and fungal load (P<0.05). The highest percentage reductions in the area of denture biofilm were observed for 1% hypochlorite solution with or without brushing and for 2% chlorhexidine solution and microwave irradiation only in association with brushing (P<0.05). The greatest reductions in microbial and fungal load were found for the groups that used solutions of 2% chlorhexidine and 1% hypochlorite and microwave irradiation, regardless of the association with brushing (P<0.05). Conclusions A single immersion for 10 min in 1% sodium hypochlorite, even in the absence of brushing, proved to be a straightforward, rapid, low-cost, and effective protocol for cleaning the dentures of hospitalized patients.

This study evaluated the in vitro and in vivo efficacy of nanofiber (NF) mats of polycaprolactone (PCL) incorporated with nystatin (NIS) in the treatment of denture stomatitis (DS) in animal models. NFs were synthesized with different concentrations of NIS, totaling four solutions: pure PCL, PCL/NIS 0.045 g, PCL/NIS 0.090 g, and PCL/NIS 0.225 g. The release of NIS was analyzed by Ultraviolet-Visible spectroscopy. The ability of the mats to inhibit Candida albicans biofilm, the main etiological factor of DS, was assessed by dividing five groups (N=5) composed of a group with C. albicans cell control and with pure PCL, in addition to the three concentrations of NIS. Next, cell viability in human keratinocytes (HaCat) was analyzed using the resazurin colorimetric test. Five groups were divided (N=10): cell control, pure PCL, and the three concentrations of NIS. In albino Wistar rat animal models (N=18), palatal devices (PD) made of acrylic resin were fabricated to simulate total prostheses and used to induce DS. For this, PD contaminated with C. albicans were cemented in the molar region of the animals' oral cavity and remained in the mouth for 48 hours. After this period, the PDs were removed, and the animals were divided into three groups: (C) control; (B1) treated with PCL/NIS 0.045 g mats, and (B2) PCL/NIS 0.225 g, with N=6. Then new, uncontaminated PDs were cemented in the animals' oral cavity and remained for another 48 hours. After this period, the animals were euthanized, UFC/ mL counts were performed, and the palates were collected for histological analysis. The standard NIS curve obtained showed an R2 of 0.99. The three concentrations of NF showed NIS release, with a peak at 6 h and values of 66.26 µg/ mL for PCL/NIS 0.045 g, 333.87 µg/ mL for PCL/NIS 0.090 g, and 436.51 µg/ mL for PCL/NIS 0.225 g, remaining constant until the end of the experiment. The groups with NIS reduced fungal biofilm growth by 2.5 log10 compared to the untreated groups, Control and PCL, with no significant statistical difference. No cytotoxicity was observed in HaCat cells, with cell viability of 93.7% for PCL/NIS 0.045 g, 72.6% for PCL/NIS 0.090 g, and 72.4% for PCL/NIS 0.225 g. Induction of DS in the three groups was possible; however, there was no significant reduction in UFC/ mL counts of C. albicans in groups B1 and B2. Histological analysis of group C revealed infiltration of Candida hyphae in the keratinized layer, presence of inflammatory cells forming micro abscesses, and a discreet inflammatory infiltrate in the connective tissue underlying the infected epithelium. No epithelial alterations were found in groups B1 and B2, concluding that NFs demonstrated in vitro antifungal activity and were effective in preventing hyphal penetration into palatal tissue in animals with PD.(AU)
