Introducción. Los quistes esplénicos son entidades anatómicas y clínicas poco frecuentes, con una incidencia aproximada de 0,07 %. Se clasifican como quistes esplénicos primarios, que contienen revestimiento epitelial y se subdividen en parasitarios y no parasitarios según su etiología, y quistes secundarios, que no poseen revestimiento epitelial en la luz quística y suelen ser el resultado de un traumatismo abdominal. Por lo general, son asintomáticos y se pueden encontrar de manera incidental durante estudios de imagen o en cirugía. Los síntomas están relacionados con el tamaño de los quistes. El tratamiento ideal es la resección quirúrgica, que puede ser total o parcial. Caso clínico. Paciente femenina de 23 años, sin antecedentes de trauma, con dolor abdominal intermitente, de varios meses de evolución. En los estudios imagenológicos se identificó un pseudoquiste esplénico gigante. Fue tratada mediante esplenectomía total por laparoscopia, sin complicaciones quirúrgicas. Resultados. Tuvo una adecuada evolución postoperatoria. Conclusión.El diagnóstico de quiste esplénico se realiza mediante estudios imagenológicos y se confirma con el análisis histopatológico. La esplenectomía total ha sido el tratamiento tradicional; sin embargo, ahora mediante la implementación de abordajes mínimamente invasivos, se prefiere la esplenectomía parcial, con el fin de preservar tejido esplénico y su función inmunológica.


Introduction.Splenic cysts are rare anatomical and clinical entities, with an approximate incidence of 0.07%. They are classified as primary splenic cysts, which contain epithelial lining and are subdivided into parasitic and non-parasitic depending on their etiology, and the secondary splenic cysts, which do not have an epithelial lining in the cystic lumen and are usually the result of abdominal trauma. They are usually asymptomatic and can be found incidentally during imaging studies or in surgery. The symptoms are related to the size of the cysts. The ideal treatment is surgical resection, which can be total or partial. Clinical case. A 23-year-old female patient, with no history of trauma, with intermittent abdominal pain, lasting several months. Imaging studies identified a giant splenic pseudocyst. She was treated by total laparoscopic splenectomy, without surgical complications. Results. She had an adequate postoperative evolution. Conclusion. The diagnosis of splenic cyst is made through imaging studies and confirmed with histopathological analysis. Total splenectomy has been the traditional treatment; however, now through the implementation of minimally invasive approaches, partial splenectomy is preferred, in order to preserve splenic tissue and its immunological function.

Introducción. El esplenúnculo se ha descrito con una incidencia global del 10 al 30 %. Puede ser una entidad de origen congénito o adquirida, frecuentemente está asociada a trauma abdominal o antecedente de esplenectomía por diversas causas. Caso clínico. Mujer en edad media, con antecedente de trauma abdominal y esplenectomía por ruptura traumática, quien 30 años después presenta un cuadro de dolor abdominal. Los estudios imagenológicos identificaron una masa sólida intrapancreática. Resultados. Fue llevada a pancreatectomía distal. Los hallazgos histológicos e inmunohistoquímicos confirmaron que la masa pancreática correspondía a un esplenúnculo intrapancreático adquirido, asociado al evento traumático previo. Conclusión. Los esplenúnculos suelen constituir un "incidentaloma". Los estudios imagenológicos se encuentran limitados, pues la tomografía computarizada, la resonancia nuclear magnética y la ultrasonografía presentan características imagenológicas similares con los tumores pancreáticos hipervascularizados, por lo que se debe practicar el estudio histopatológico durante su valoración. Esta entidad se debe incluir dentro de los diagnósticos diferenciales, con mayor énfasis en aquellos pacientes con historia de trauma abdominal y esplenectomía asociada, un escenario en el que esta lesión puede simular una neoplasia sólida del páncreas, con características malignas.


Introduction. Splenunculus has been described with an overall incidence of 10 to 30%. It can be an entity of congenital or acquired origin, it is frequently associated with abdominal trauma or a history of splenectomy for various reasons. Clinical case. Middle-aged woman, with a history of abdominal trauma and splenectomy due to traumatic rupture, who 30 years later presents with abdominal pain. Imaging studies identified a solid intrapancreatic mass. Results. She was taken to OR for distal pancreatectomy. The histological and immunohistochemical findings confirmed that the pancreatic mass corresponded to an acquired intrapancreatic splenunculus, associated with the previous traumatic event. Conclusion. Splenuncles usually constitute an "incidentaloma". Imaging studies are limited, since computed tomography, magnetic resonance imaging, and ultrasonography present similar imaging characteristics with hypervascularized pancreatic tumors, so histopathological study must be included during their evaluation. This entity should be included in the differential diagnoses, with greater emphasis on those patients with a history of abdominal trauma and associated splenectomy, a scenario in which this lesion can simulate a solid neoplasm of the pancreas, with malignant characteristics.

High-altitude hypoxia exposure can lead to phospholipase D-mediated lipid metabolism disorder in spleen tissues and induce ferroptosis. Nonetheless, the key genes underlying hypoxia-induced splenic phospholipase D and the ferroptosis pathway remain unclear. This study aimed to establish a hypoxia animal model. Combined transcriptomic and proteomic analyses showed that 95 predicted target genes (proteins) were significantly differentially expressed under hypoxic conditions. Key genes in phospholipase D and ferroptosis pathways under hypoxic exposure were identified by combining Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) enrichment analysis techniques. Gene set enrichment analysis (GSEA) showed that the differential gene sets of the phospholipase D and ferroptosis signaling pathways were upregulated in the high-altitude hypoxia group. The genes in the phospholipase D signalling pathway were verified, and the expression levels of KIT and DGKG were upregulated in spleen tissues under hypoxic exposure. Subsequently, the mRNA and protein expression levels of genes from the exogenous pathway such as TFRC, SLC40A1, SLC7A11, TRP53, and FTH1 and those from the endogenous pathway such as GPX4, HMOX1, and ALOX15 differentials in the ferroptosis signalling pathway were verified, and the results indicated significant differential expression. In summary, exposure to high-altitude hypoxia mediated phospholipid metabolism disturbance through the phospholipase D signalling pathway and further induced ferroptosis, leading to splenic injury.

Xiaoyaowan has the effects of soothing the liver, invigorating the spleen, regulating menstruation, and nourishing blood. It is often used to treat the distending pain of the chest and hypochondriac, loss of appetite, dizziness, and irregular menstruation caused by liver depression and spleen deficiency. The formula originated from the Sinisan of Treatise on Cold Damage(《伤寒论》) in the Han dynasty and was officially formulated in the Song dynasty's Taiping Royal Prescriptions(《太平惠民合计局方》). In the Ming and Qing dynasties, it was developed into more comprehensive formulas such as modified Bazhentang, modified Xiaoyaowan, and other formulas. In recent years, Xiaoyaowan has become a classic formula for treating many symptoms of liver depression. The Pharmacopoeia of the People's Republic of China(《中华人民共和国药典》), 2020 edition, records Xiaoyaowan and modified Xiaoyaowan. Modern clinical research has further expanded the therapeutic range of this formula. Through literature research, it is found that there are certain reports on the clinical application and quality analysis of Xiaoyaowan, but the relevant literature lacks collation so far. Therefore, relevant literature was consulted and sorted out. The paper summarized the treatment of mammary hyperplasia, depression, irregular menstruation, melasma, menopausal syndrome, and polycystic ovary syndrome with Xiaoyaowan, as well as the quality analysis of paeoniflorin, ferulic acid, and glycyrrhizinic acid of Xiaoyaowan. It is expected to lay a foundation for further research on clinical application, pharmacodynamic mechanism, and quality control of this classic formula.

ObjectiveTo systematically review the existing evidence of Tongxiening(TXN) Granules in the treatment of diarrhea-predominant irritable bowel syndrome (IBS-D) with liver Qi invading spleen syndrome,and evaluate the clinical comprehensive value from the 6+1 dimension,providing references for national medical decision-making,drug clinical application promotion,pharmaceutical services,etc. MethodFollowing the relevant standards of the "Guidelines for the Management of Clinical Evidence and Value Evaluation of Drugs", a combination of qualitative and quantitative approaches based on evidence-based medicine,questionnaire surveys,pharmacoeconomic evaluation,etc.,was employed. Multi-criteria decision analysis (MCDA) model was employed to comprehensively evaluate the clinical evidence and value of TXN Granules in the treatment of IBS-D with liver Qi invading spleen syndrome from the "6+1" dimensions of effectiveness,safety,economy,innovation,suitability,accessibility, and traditional Chinese medicine (TCM) characteristics Result① Safety: Based on the evaluation of known risks and adequacy of safety evidence, TXN granules were considered to have controllable risks with sufficient safety evidence. ② Effectiveness: Meta-analysis showed that TXN granules combined with probiotics could reduce the intestinal symptom score level of IBS-D patients [mean difference (MD)=-2.29, 95% confidence interval (CI) (-3.20, -1.38), P<0.01], and TXN granules combined with conventional treatments (such as Bifidobacterium triple viable capsules,pinaverium bromide tablets,or trimebutine maleate tablets)effectively improved the comprehensive improvement rate of IBS-D patients [relative risk (RR)=1.19, 95%CI (1.14,1.25), P<0.01]. TXN granules significantly improved abdominal pain [RR=1.99, 95%CI (1.62,2.44), P < 0.01] and diarrhea efficacy rate [RR = 1.56, 95% CI (1.07,2.25), P<0.05] in IBS-D patients. TXN granules were comparable to pinaverium bromide tablets in reducing HAMA score [MD = 0.29, 95% CI (-0.30, 0.88), P=0.34], HAMD score [MD=0.35, 95%CI (-0.31,1.00),P=0.30], and improving IBS-QOL score [MD = 0.28, 95%CI (-1.70,2.26), P=0.78]. Based on the quality of evidence and evidence value assessment of effectiveness, it was considered that there was sufficient evidence of effectiveness for this product. ③ Economy: TXN granules are a national medical insurance category B product. After 5 years of simulation with three kinds of Markov models, it was considered that TXN granules were the advantageous treatment option with stable results and good economy. ④ Innovation: TXN granules adhered to original innovation, conducting research and development from clinical application, product components, and production processes. They have obtained national TCM new drug certificates, first prizes from the China Association of Chinese Medicine for Science and Technology, and excellent awards for Chinese patents, providing sufficient innovative evidence in clinical, corporate, and industrial aspects. ⑤ Suitability: The information service of this drug is comprehensive. The questionnaire survey results showed that this drug was suitable for clinical doctors, nurses, pharmacists, and patients in terms of individual compliance, system, and management across multiple dimensions. The overall assessment showed sufficient evidence of suitability for this product. ⑥ Accessibility: The daily cost of TXN granules accounts for 2.43% of the median disposable daily income of urban residents and 9.26% of rural residents, which is moderate and reasonable compared to similar drugs. There is sufficient evidence of affordability, and it is sold nationwide covering all 31 provinces, municipalities, and autonomous regions, with full accessibility evidence and clear results. TCM characteristics: TXN granules originate from the prescription for treating diarrhea in Danxi's Experiential Therapy. They are derived from classical formulas and have undergone multiple clinical trials involving 5 000 cases since their launch, accumulating a certain amount of human use experience. Based on the principles of evaluating TCM characteristics, TXN granules highlight TCM characteristics. The comprehensive value measured using CSC V2.0 software is 0.83 points based on the results of “6+1” dimensions. ConclusionBased on the evaluation results of TXN granules in various dimensions and the comprehensive evaluation score, it is considered that there is sufficient clinical value evidence for TXN granules in treating IBS-D with liver Qi invading spleen syndrome, which can provide references for clinical decision-making and pharmaceutical management. It is suggested for future research to conduct network Meta-analysis, conduct horizontal comparisons of similar TCM treatments for IBS-D, improve the quality of evidence-based evidence, and fully leverage the clinical value advantages of TXN granules.

ObjectiveTo observe the clinical efficacy of modified Yigongsan combined with multi-enzyme tablets and bifidobacterium triple live powder on infantile anorexia with spleen-stomach Qi deficiency syndrome. MethodA total of 112 infantile patients anorexia with spleen-stomach Qi deficiency syndrome treated at Hebei Provincial Hospital of Traditional Chinese Medicine from January 2022 to June 2023 were enrolled and divided into a control group and an observation group, with 56 cases in each group, according to a random number table. Children in the control group were treated with multi-enzyme tablets and Bifidobacterium triple live powder, while those in the observation group were treated with modified Yigongsan in addition to the treatment in the control group. During the study, one case dropped out in the control group and two cases dropped out in the observation group. The clinical efficacy of the two groups of children was compared, including changes in traditional Chinese medicine (TCM) syndrome scores (main symptoms, secondary symptoms, tongue, and pulse), time to restore normal food intake, and increase in body weight. Changes in calcium, iron, zinc levels, hemoglobin, and albumin levels before and after treatment, as well as changes in gastrointestinal hormones such as gastrin and motilin, vasoactive intestinal peptide, somatostatin, neuropeptide Y, orexin, and leptin, were observed. The occurrence of adverse reactions in the two groups of children during the study was also recorded. ResultThe total effective rate of children in the control group after treatment was 85.19% (46/54), while that in the observation group was 98.15% (53/54) (χ2 =5.939, P<0.05). Compared with the control group, the time for food intake to return to normal in the observation group was shorter, and the increase in body weight was greater (P<0.05). Compared with the results before treatment, the TCM syndrome scores (main symptoms, secondary symptoms, tongue, and pulse) in both groups of children significantly decreased, while the levels of calcium, iron, zinc, hemoglobin, albumin, gastrin, motilin, neuropeptide Y, and orexin increased, and the levels of vasoactive intestinal peptide, somatostatin, and leptin decreased (P< 0.01). Compared with the control group after treatment, the improvement in the above indicators in the observation group was more significant (P<0.01). The incidence of adverse reactions in the two groups of children during the treatment period was similar, and the difference was not statistically significant. ConclusionModified Yigongsan combined with multi-enzyme tablets and Bifidobacterium triple live powder is highly effective in treating infantile anorexia (spleen-stomach Qi deficiency syndrome). After treatment, symptoms of the children were improved,appetite and food intake increased, gastrointestinal function was improved, body weight increased, and adverse reactions were few, indicating that the treatment was safe and reliable.

Objective:To explore the effects of Jianpi Bushen Jiedu Prescription on the proliferation and migration of hepatocellular carcinoma cells; To discuss its possible mechanism.Methods:Using human highly metastatic liver cancer cell line (HCCLM3) as the research object, they were randomly divided into control group and TCM group (100, 200, 400, 800, 1 600, 3 200 μg/ml Jianpi Bushen Jiedu Prescription) and Western medicine group (2.5, 5, 10, 20, 40 μmol/L sorafenib) using a random number table method. Cell viability was detected using cell counting reagent (CCK-8) method; HCCLM3 cells were divided into control group and TCM (Jianpi Bushen Jiedu Prescription 800 μg/ml) group and combined group (Jianpi Bushen Jiedu Prescription 800 μg/ml +sorafenib 20 μmol/L). Western blot method was used to detect the protein expressions of kinase/signaling transducer and transcriptional activator (JAK2/STAT3) pathway related proteins (p-JAK2, JAK2, p-STAT3, STAT3) in each group.Results:Compared with the control group, viability and mobility of HCCLM cell in TCM group and Western medicine group decreased ( P<0.01 or P<0.05); compared with the control group, the protein expressions of P-JAK2, JAK2, P-STAT3 and STAT3 in the TCM group and the combined group decreased ( P<0.05), and the JAK2 protein expression in the combined group was lower than that in the TCM group ( P<0.05). Conclusion:Jianpi Bushen Jiedu Prescription can inhibit the proliferation and migration of HCC cells by regulating JAK2/STAT3 pathway.

Liver depression and spleen deficiency syndrome is a common syndrome type of ulcerative colitis (UC). Based on the theory of 'collaborative treatment of liver and spleen', TCM shows its effect and less adverse reactions in the treatment of UC with liver depression and spleen deficiency. The internal treatment of TCM based on syndrome differentiation and treatment by stages, as well as the external treatment of TCM based on TCM enema therapy, acupuncture therapy and acupoint application therapy, can relieve symptoms, improve relevant clinical indicators and improve the quality of life of patients. The mechanism studies show that the TCM with the effect of soothing the liver and activating the spleen plays a therapeutic role by regulating signal pathway conduction, regulating intestinal flora, and improving the level of inflammatory factors.

The pre-metabolic disease state is the body state of substance metabolism disorder that has not yet reached the physical and chemical indicators of the disease, and abnormal glucose metabolism is often the key link of metabolic disorder. In TCM, the healthy function of the spleen is the cornerstone of the production and distribution of fine substances. This article discussed the pre-metabolic disease state based on the theory of "overflow of Five Qi" in the Nei Jing, taking the loss of spleen preparedness as the starting point, in order to provide new ideas and directions for the prevention and treatment of clinical metabolic diseases.

RESUMO

Objective:To evaluate the clinical efficacy of Rougan Jianpi therapy combined with Mesalazine enema in the treatment of distal ulcerative colitis (DUC) with Ganyu-Pixu syndrome.Methods:This study is a prospective parallel randomized controlled study. From November 2021 to May 2022, 115 patients with DUC in Changshu Hospital Affiliated to Nanjing University of Chinese Medicine were selected and randomly divided into a control group of 58 patients and a study group of 57 patients. The control group was treated with Mesalazine enema, and the study group was treated with Rougan Jianpi Decoction on the basis of the control group. Both groups were treated for 8 weeks. The severity of the disease was evaluated by using the Modified Mayo Scale, and the clinical and endoscopic efficacy was evaluated based on the changes in the score before and after treatment. TCM syndromes were scored from four aspects: abdominal pain, diarrhea, mucopurulent bloody stool, Rectal tenesmus, and anal burning. The quality of life was evaluated by using the IBD Quality of Life Scale (IBDQ). The serum TNF-αand IL-10 level were detected by using ELISA method. The adverse reactions during treatment were observed and recorded.Results:The total effective rate of the study group was 91.23% (52/57), while that of the control group was 79.31% (46/58). The difference between the two groups was no statistically significant ( Z=1.47, P=0.143). The proportion of patients in the study group who achieved clinical response[40.35% (23/57) vs. 51.72% (30/58)] and clinical relief [59.65% (34/57) vs. 43.11% (25/58)] the total proportion were significantly higher than those in the control group [100.00% (57/57) vs. 94.83% (55/58); Z=1.97, P=0.049]. The proportion of patients with endoscopic response [52.63% (30/57) vs. 56.90% (33/58)] and mucosal healing [33.33% (19/57) vs. 17.24% (10/58)] the total proportion in the study group were significantly higher than those in the control group [85.96% (49/57) vs. 74.14% (43/58); Z=2.23, P=0.026]. after treatment, the IBDQ score [(194.3±15.1) vs. (172.6±18.4), t=6.90] in the study group was significantly higher than that of the control group. After treatment, the patient's TCM symptom score: mucopurulent bloody stool [(1.7±0.8) vs. (2.0±0.6), t=2.03], rectal tenesmus [(1.5±0.6) vs. (1.8±0.6), t=2.32] and anal burning [(1.3±0.6) vs. (1.6±0.7), t=2.38] in the control group were significantly lower than those in the control group ( P<0.05). The level of IL-10 [(53.3±8.0) ng/L vs. (49.0±6.7) ng/L, t=3.10] in the study group after treatment was higher than that of the control group ( P<0.01), and TNF-α [(28.3±7.9)ng/L vs. (36.8±8.1)ng/L, t=3.58] was lower than that of the control group ( P<0.01). There were no significant adverse reactions observed during the treatment in both groups. Conclusion:Method of Rougan Jianpi combined with mesalazine enema can improve the clinical symptoms of DUC patients, improve clinical efficacy, and have good safety.

Objective To analyze the metabolomics characteristics of chronic atrophic gastritis(CAG)patients with liver-stomach qi stagnation and spleen-stomach weakness syndromes based on non-targeted metabolomics technology,and to identify the serum differentiated metabolites related to traditional Chinese medicine(TCM)syndrome of CAG patients,so as to provide a reference for the objectification of syndrome differentiation.Methods Sixty patients with CAG were included,including 30 cases of liver-stomach qi stagnation syndrome and 30 cases of spleen-stomach weakness syndrome.Fasting blood of 5 mL was collected from the cubital vein of patients in the two groups,and the serum levels of metabolites were detected by ultra-high-performance liquid chromatography-mass spectrometry(UPLC-MS)methods.The principal component analysis(PCA),orthogonal partial least squares-discriminant analysis(OPLS-DA),and cluster analysis were used to screen the differentiated metabolites of CAG patients with liver-stomach qi stagnation syndrome and spleen-stomach weakness syndrome.Finally,metabolite pathway analysis was performed for the obtained differentiated metabolites using the KEGG database.Results The results for the screening of differentiated metabolites showed that significant differences of amino acid derivatives and small peptide metabolites were presented between CAG patients with liver-stomach qi stagnation syndrome and CAG patients with spleen-stomach weakness syndrome.The amino acid derivatives consisted of N-acetylglycine,histamine,O-phosphoserine,selenomethylselenocysteine,and methyl-tyrosine.And the small peptide metabolites consisted of tyrosine-leucine-phenylalanine,histidine-alanine-glutamate-lysine,L-asparagine-L-proline-L-serine,and L-isoleucine-L-isoleucine.Conclusion Differences in amino acid metabolism exist between CAG patients with liver-stomach qi stagnation syndrome and those with spleen-stomach weakness syndrome,and metabolites such as N-acetylglycine,intermethyltyrosine,and O-phosphoserine may be the potential biomarkers for distinguishing liver-stomach qi stagnation syndrome from spleen-stomach weakness syndrome in CAG patients.

Objective To investigate the clinical efficacy of Bushen Jianpi Recipe(mainly composed of Astragali Radix,Epimedii Folium,Dioscoreae Rhizoma,Salviae Miltiorrhizae Radix et Rhizoma,Cervi Cornus Colla,Astragali Complanati Semen,Polygoni Multiflori Radix Preparata,Polygonati Rhizoma,Puerariae Lobatae Radix,and Rhei Radix et Rhizoma)on patients with type 2 diabetes mellitus(T2DM)complicated with dyslipidemia and differentiated as spleen-kidney deficiency type,and to observe its effect on the level of adiponectin(ADP).Methods Ninety patients with T2DM complicated with dyslipidemia and differentiated as spleen-kidney deficiency type were randomly divided into western medicine group,Chinese medicine(CM)group,and combination of CM and western medicine group(hereinafter referred to as combination group),and each group had 30 patients.All of the 3 groups were given conventional hypoglycemic treatment.Moreover,the western medicine group was given oral use of Atorvastatin Calcium Tablets,CM group was given Bushen Jianpi Recipe,and the combination group was given Atorvastatin Calcium Tablets together with Bushen Jianpi Recipe orally.The course of treatment lasted for 8 weeks.The changes of traditional Chinese medicine(TCM)syndrome scores,glucose and lipid metabolism indexes,fasting insulin(FINS),insulin resistance index(HOMA-IR)and serum ADP levels of the three groups were observed before and after the treatment.After treatment,the efficacy of TCM syndrome of the three groups was evaluated.Results(1)After 8 weeks of treatment,the total effective rates for TCM syndrome efficacy in the western medicine group,CM group,and combination group were 66.67%(20/30),90.00%(27/30),and 93.33%(28/30),respectively.The intergroup comparison showed that the TCM syndrome efficacy of the CM group and the combination group was significantly superior to that of the western medicine group(P<0.05).(2)After treatment,the TCM syndrome scores in all of the three groups were decreased compared with those before treatment(P<0.01),and the decreases of the scores in both CM group and combination group was superior to that in the western medicine group(P<0.05 or P<0.01).(3)After treatment,the levels of lipid metabolism parameters of total cholesterol(TC),triglyceride(TG),high-density lipoprotein cholesterol(HDL-C),and low-density lipoprotein cholesterol(LDL-C)in the three groups were improved to various degrees compared with the pre-treatment levels,of which the levels of TC,TG,and LDL-C were significantly decreased,and the level of HDL-C was significantly increased in comparison with that before treatment,and the differences were statistically significant(P<0.05 or P<0.01).The intergroup comparison showed that the decrease of TC and LDL-C and the increase of HDL-C in the CM group were inferior to those in the western medicine group and the combination group(P<0.05 or P<0.01).(4)After treatment,the levels of glucose metabolism parameters of fasting plasma glucose(FPG),2-hour postprandial glucose(2hPG),glycated hemoglobin(HbA1c),FINS,and HOMA-IR in the CM group and the combination group were significantly decreased compared with those before treatment(P<0.05 or P<0.01),while only the levels of FPG,2hPG,and HOMA-IR in the western medicine group were decreased compared with those before treatment(P<0.05 or P<0.01).The intergroup comparison showed that the patients in the decrease of FPG,2hPG,HbA1c,FINS,and HOMA-IR levels in the CM group and the combination group was significantly superior to that in the western medicine group(P<0.05 or P<0.01).(5)In terms of adipokines,the serum ADP level in the three groups after treatment was significantly increased compared with that before treatment(P<0.05 or P<0.01),and the increase of serum ADP level in both CM group and combination group was significantly superior to that in the western medicine group(P<0.05).Conclusion Bushen Jianpi Recipe has certain effect on regulating lipid metabolism,and has obvious advantages in improving clinical symptoms and insulin resistance,lowering blood glucose,and increasing ADP level in patients with T2DM complicated with dyslipidemia and differentiated as spleen-kidney deficiency type.

Objective To observe the clinical efficacy of Lipi Qushi Recipe in the treatment of diarrhea-predominant irritable bowel syndrome(IBS-D)of spleen deficiency and dampness superabundance type.Methods Seventy IBS-D patients with spleen deficiency and dampness superabundance type were randomly divided into an observation group and a control group,with 35 patients in each group.The control group was given oral use of Trimebutine Maleate Tablets,and the observation group was given Lipi Qushi Recipe.The two groups were treated for 4 consecutive weeks.The changes of traditional Chinese medicine(TCM)syndrome scores,irritable bowel syndrome severity scoring system(IBS-SSS)scores,Hamilton anxiety scale(HAMA)scores,Hamilton depression scale(HAMD)scores,irritable bowel syndrome quality of life scale(IBS-QOL)scores,and serum interleukin 6(IL-6)level of the two groups were observed before and after the treatment.After treatment,the TCM syndrome efficacy and safety of the two groups were evaluated.Results(1)During the treatment process,one case quitted spontaneously,3 cases fell off(failing in following the treatment schedule for some reasons),and one case lost to follow-up in the observation group;in the control group,3 cases quitted spontaneously and 2 cases lost to follow-up.Eventually,there were 30 patients in each of the two groups completing the full course of treatment.(2)After 4 weeks of treatment,the total effective rate for TCM syndrome efficacy in the observation group was 93.93%(28/30),and that in the control group was 80.00%(24/30).The intergroup comparison showed that the efficacy of TCM of the observation group was significantly superior to that of the control group(P<0.05).(3)After treatment,the TCM syndrome scores,IBS-SSS scores,HAMA scores,HAMD scores and IBS-QOL scores of the two groups were significantly decreased compared with the pre-treatment scores(P<0.05),and the degree of the decrease in the observation group was significantly superior to that in the control group(P<0.05).(4)After treatment,the serum IL-6 level of patients in the two groups was decreased compared with that before treatment(P<0.05),and the degree of the decrease in the observation group was significantly superior to that in the control group,with a statistically significant difference(P<0.05).(5)In the course of treatment,no obvious adverse reactions occurred in the two groups of patients,with a high safety.Conclusion Lipi Qushi Recipe exerts certain effect for the treatment of IBS-D patients with spleen deficiency and dampness superabundance type,which can significantly alleviate the intestinal symptoms of the patients,reduce the intestinal inflammatory reaction of the patients,improve the anxiety and depression emotions,and improve the quality of life of the patients.

Objective To observe the clinical efficacy of the modified Huangan Lipi Decoction(mainly composed of Sclerotium Poriae Pararadicis,Paeoniae Radix Alba,Atractylodis Macrocephalae Rhizoma,Polygalae Radix,Acori Tatarinowii Rhizoma,and Curcumae Radix)combined with acupuncture at Sifeng(EX-UE10)points in the treatment of children with tic disorders(TD).Methods Seventy cases of TD children with spleen deficiency and liver hyperactivity syndrome were randomly divided into the treatment group and the control group,with 35 cases in each group.The control group was treated with conventional western medicine of Tiapride Hydrochloride Tablets,while the treatment group was treated with the combination of the modified Huangan Lipi Decoction and acupuncture at Sifeng points.Both groups were treated for 8 weeks.The changes of the Yale Global Tic Severity Scale(YGTSS)item scores of tic muscle group,tic frequency,tic intensity,complexity and interference degree as well as their total scores in the two groups were observed before and after treatment.Moreover,the clinical efficacy and safety in the two groups were evaluated.Results(1)After 8 weeks of treatment,the total effective rate of the treatment group was 88.57%(31/35),and that of the control group was 68.57%(24/35).The intergroup comparison showed that the total effective rate(tested by chi-square test)and the overall therapeutic efficacy(tested by rank-sum test)of the treatment group were significantly superior to those of the control group,and the differences were statistically significant(P<0.05).(2)After treatment,the YGTSS item scores of tic muscle group,tic frequency,tic intensity,complexity and interference degree as well as their total scores in the two groups were significantly lower than those before treatment(P<0.01),and the effect on lowering the above scores of the treatment group was significantly superior to that of the control group,the differences being all statistically significant(P<0.01).(3)During the treatment,the incidence of adverse reactions in the treatment group was 2.86%(1/35)and that in the control group was 8.57%(3/35).The intergroup comparison showed that the incidence of adverse reactions in the treatment group tended to be lower than that of the control group,but the difference was not statistically significant(P>0.05).Conclusion Modified Huangan Lipi Decoction combined with acupuncture at Sifeng points exert certain effect in the treatment of TD children with spleen deficiency and liver hyperactivity syndrome,and its efficacy is superior to that of the western medicine Tiapride Hydrochloride Tablets.

This article introduces the clinical approach and acupuncture characteristics of the traditional Chinese medicine practitioner Professor YU Hai-Bo in treating paediatric cerebral palsy using the"Jianpi Yishen Triple-Needle Grouping Acupoints".Guided by the theory of growth and development of"viscera-meridian-brain"growth and development,Professor YU believed that"insufficiency of spleen and kidney"is the core pathogenesis of paediatric cerebral palsy,and the treatment concept of"treating from the spleen and kidney"was proposed.He inherited and innovated the triple-needle grouping acupoints therapy and establishing the system of"Jianpi Yishen Triple-Needle Grouping Acupoints".Before regular acupuncture,the abdomen and dorsum are pricked to freely regulate the middle energizer,and the upper limbs are selected as"Hegu(LI4),Waiguan(SJ5),Quchi[(LI11),three acupoints on the hand]+ Neiguan(PC6)";the lower limbs are selected as"Zusanli(ST36),Sanyinjiao(SP6),Taichong[(LR3),three acupoints on the foot];"Shenmai(BL62),Zhaohai(KI6),Yongquan(KI1)",spleen and kidney are regulated simultaneously,and the head acupoints include Sishencong(EX-HN1),intelligence tri-needling,cerebral tri-needling,temporal tri-needling,mind-calming needling and bilateral Fengchi(GB20).In order to regulate the spirit and benefit the intellect,the matching acupoints are modified according to the disease and the syndromes.At the same time,it is supplemented with music therapy and auricular point seed-pressing.Emphasis is placed on the simultaneous regulation of"child-parent-doctor"and"treating the person"rather than the"treating the disease".

Coronary heart disease(CHD)is a common cardiovascular disease in clinic,which can be classified as"chest qi-obstruction"and"heart pain"in traditional Chinese medicine.Professor LI Yan believes that CHD can be differentiated as the syndrome of deficiency in origin and excess in superficiality,and the deficiency in origin is characterized by the deficiency of spleen and the failure of spleen and stomach in transportation,while the excess in superficiality manifests as the obstruction of visible pathogens such as phlegm turbidity,blood stasis,qi stagnation,and cold accumulation.The clinical treatment should focus on regulating the spleen and stomach,using Astragali Radix,Atractylodis Macrocephalae Rhizoma,Codonopsis Radix,Schisandrae Chinensis Fructus to strengthen the spleen and nourish the heart in order to treat the root cause.Moreover,therapies of activating yang to remove turbidity,activating blood to remove stasis,soothing liver to rectify qi,and warming yang to dissipate the cold can be adopted.Therefore,the symptom and root cause will be treated simultaneously,the unblocking and supplementing effect will be achieved,and then the spleen-qi is vigorous,the heart qi is in abundance,the blood stasis and the phlegm are removed,and the chest yang can be stretched.As a result,the heart vessels are unblocked,and the symptoms of chest qi-obstruction will be relieved.Professor LI Yan's experience from the perspective of spleen and stomach can provide reference for the clinical diagnosis and treatment of CHD with traditional Chinese medicine.

Based on the theory of traditional Chinese medicine(TCM)and clinical manifestations of seborrheic alopecia,Professor YANG Yu-Feng believes that the fundamental pathogenesis of seborrheic alopecia is due to spleen deficiency with dampness accumulation,and dampness and fat sharing the same origin.The internal accumulation of dampness and turbidity results from the deficiency of spleen,and then the dampness and turbidity blended with heat attack the head and face.For dampness and fat share the same origin,the turbid fat forms,and then overflows and blocks the hair roots,which causes the qi and blood being difficult to go upward and nourish the hair roots,and then induces hair loss.Among them,spleen deficiency with dampness accumulation is the core pathogenesis of the disease,heat activating the movement of dampness is the key pathogenesis to the disease,and the disharmony of qi and blood is the pivot of the pathogenesis.For the treatment of seborrheic alopecia,Professor YANG Yu-Feng suggested that the hair root should be unblocked and the hair quality should be nourished by blood essence,and emphasized the importance of the method of invigorating the spleen and removing dampness,controlling oil and removing fat in dredging the hair root and relieving seborrheic alopecia.It is proposed that invigorating the spleen and removing dampness is the key,expelling wind and clearing heat is for assistance,and harmonizing qi and blood serves as the pivot.

Objective To observe the clinical efficacy of Modified Tongxie Yaofang(Important Formula for Relieving Diarrhea with Pain)in the treatment of patients with diarrhea-predominant irritable bowel syndrome(IBS-D)with liver-depression and spleen-deficiency syndrome.Methods Sixty patients with IBS-D of liver depression and spleen deficiency type were randomly divided into treatment group and control group,30 patients in each group.The patients in the treatment group were given modified Tongxie Yaofang,and the patients in the control group were given Pivacurium Bromide Tablets(Dicetel).Both groups were treated for a 4-week course of treatment.The changes of traditional Chinese medicine(TCM)syndrome scores,irritable bowel syndrome quality of life(IBS-QOL)scores,Hamilton Anxiety Scale(HAMA)scores of the two groups were observed before and after the treatment.Moreover,the efficacy for overall symptomatic improvement in the two groups was evaluated according to the grading of irritable bowel syndrome symptom severity score(IBS-SSS),and the efficacy for improving single symptom such as abdominal pain and diarrhea as well as the relapse after 4 weeks of drug cessation in the two groups were compared.Results(1)After 4 weeks of treatment,the total effective rate for overall symptomatic improvement in the treatment group was 83.33%(25/30),and that in the control group was 43.33%(13/30).The intergroup comparison(tested by chi-square test)showed that the efficacy for overall symptomatic improvement in the treatment group was significantly superior to that in the control group(P<0.01).(2)After 4 weeks of treatment,the total effective rate for improving single symptom of abdominal pain and diarrhea in the treatment group was 80.00%(24/30),90.00%(27/30),and that in the control group was 43.33%(13/30),46.67%(14/30),respectively.The intergroup comparison(tested by chi-square test)showed that the efficacy for improving single symptom of abdominal pain and diarrhea in the treatment group was significantly superior to that in the control group(P<0.01).(3)After 4 weeks of treatment,the TCM syndrome scores,IBS-QOL scores,and HAMA scores of patients in both groups were significantly decreased compared with those before treatment(P<0.05 or P<0.01),and the decrease in the treatment group was significantly superior to that in the control group(P<0.01).(4)After 4 weeks of drug withdrawal,the recurrence rate of the treatment group was 24.00%(6/25),which was significantly lower than that of the control group(61.54%,8/13),and the difference was statistically significant between the two groups(P<0.05).Conclusion Modified Tongxie Yaofang exerts certain effect in treating patients with IBS-D of liver depression and spleen deficiency type,and the decoction is effective on decreasing the scores of symptoms of abdominal pain and diarrhea as well as TCM syndrome scores,improving the quality of life of the patients,and alleviating the anxiety status of the patients.

This article summarized Professor GUAN Guo-Hua's clinical experience in treating central serous chorioretinopathy(CSC)in Lingnan area.Based on the theory of"macula due to the spleen dysfunction",and by taking the geographical and climatic characteristics of Lingnan area as well as the body constitutional features of Lingnan residents into account,Professor GUAN Guo-Hua proposed that spleen deficiency leading to damp encumbrance was the fundamental pathogenesis of CSC in Lingnan area,and liver and kidney were gradually affected in the middle and late stages of CSC,which finally resulted into blood stasis and water retention.For the treatment of initial attack of CSC,the focus was on treating the spleen,and Erchen Decoction was adopted as the basic prescription for modified application to strengthen the spleen and drain dampness;for the treatment of CSC in the middle and late stages,the emphasis was on simultaneous treatment of the liver,spleen and kidney as well as blood and water,and Zhujing Pills and Wuling Powder were adopted as the basic prescriptions for nourishing the liver and kidney and for strengthening the spleen,activating blood and promoting urination.The treatment of the spleen is advocated throughout the whole treatment process,and the medication of drugs should be modified based on syndrome differentiation and according to the specific conditions,thus to achieve significant results.

Professor TANG Shui-Fu believes that the development of renal proteinuria in chronic kidney disease is due to the deficiency of spleen and kidney,which leads to the malfunction in lifting lucid yang and lowering turbid yin of the body and the internal retention of turbid-toxin.The early stage of renal proteinuria is mainly induced by qi deficiency of spleen and kidney,and the middle and late stage of renal proteinuria results from the long-lasting spleen and kidney deficiency together with damp-toxin accumulation and is characterized by the mixture of healthy-qi deficiency and pathogenic-qi excess.Aimed at the key pathogenesis of the dysfunction of ascending and descending of qi movement,Professor TANG Shui-Fu treated the renal proteinuria with the principal method of strengthening the spleen and tonifying the kidney,combined with method of eliminating dampness,draining turbidity and dissolving stasis targeted at the pathogenic-qi excess of damp-turbidity and stasis-toxin diffusing in the triple energizer.Meanwhile,the use of the method of replenishing and elevating Qi for regulating the qi movement of the spleen and kidney was stressed.In the early stage of renal proteinuria,the Chinese medicines with the actions of promoting the lifting of lucid yang such as Cimicifugae Rhizoma and Bupleuri Radix were added on the basis of herbs for strengthening the spleen and benefiting the kidney,so that the essence can be consolidated and proteinuria can be eliminated.In the middle and late stage of renal proteinuria,the course of the disease lasted a long time,and the internal attack of the pathogens caused the reversal and chaos of the lucid yang and turbid yin.In this case,treatment should follow the principal method of lowering turbid yin and by the assistance of the herbs for replenishing and elevating qi such as Astragali Radix,Atractylodis Macrocephalae Rhizoma,Poria,Dioscoreae Rhizoma,Cimicifugae Rhizoma,and Bupleuri Radix,so that the ascending and descending of qi movement can be coordinated,the lucid yang is raised and turbid yin is directed downward,the renal function is protected and the progression of chronic kidney disease is slowed down.
