RESUMO

ABSTRACT Introduction: Lung diseases are common in patients with end stage kidney disease (ESKD), making differential diagnosis with COVID-19 a challenge. This study describes pulmonary chest tomography (CT) findings in hospitalized ESKD patients on renal replacement therapy (RRT) with clinical suspicion of COVID-19. Methods: ESKD individuals referred to emergency department older than 18 years with clinical suspicion of COVID-19 were recruited. Epidemiological baseline clinical information was extracted from electronic health records. Pulmonary CT was classified as typical, indeterminate, atypical or negative. We then compared the CT findings of positive and negative COVID-19 patients. Results: We recruited 109 patients (62.3% COVID-19-positive) between March and December 2020, mean age 60 ± 12.5 years, 43% female. The most common etiology of ESKD was diabetes. Median time on dialysis was 36 months, interquartile range = 12-84. The most common pulmonary lesion on CT was ground glass opacities. Typical CT pattern was more common in COVID-19 patients (40 (61%) vs 0 (0%) in non-COVID-19 patients, p < 0.001). Sensitivity was 60.61% (40/66) and specificity was 100% (40/40). Positive predictive value and negative predictive value were 100% and 62.3%, respectively. Atypical CT pattern was more frequent in COVID-19-negative patients (9 (14%) vs 24 (56%) in COVID-19-positive, p < 0.001), while the indeterminate pattern was similar in both groups (13 (20%) vs 6 (14%), p = 0.606), and negative pattern was more common in COVID-19-negative patients (4 (6%) vs 12 (28%), p = 0.002). Conclusions: In hospitalized ESKD patients on RRT, atypical chest CT pattern cannot adequately rule out the diagnosis of COVID-19.

Abstract The scope of this study is to analyze the prevalence of advanced stage diagnosis of cervical cancer and its association with individual and contextual socioeconomic and healthcare service indicators in Brazil. A cross-sectional study was conducted using cervical cancer cases in women aged 18 to 99 years, from 2006 to 2015, extracted from the Hospital Cancer Registry (HCR) Integrator. Contextual variables were collected from the Atlas of Human Development in Brazil; the National Registry of Health Institutions (NRHI); and the Outpatient Information System. Multilevel Poisson Regression with random intercept was used. The prevalence of advanced stage diagnosis was 48.4%, revealing an association with older age groups (PR 1.06; CI 1.01-1.10), black, brown, and indigenous race/skin color (PR 1.04; CI 1.01-1.07), lower levels of schooling (PR 1.28; CI 1.16-1.40), no marital partner (PR 1.10; CI 1.07-1.13), public referral to the health service (PR 1.07; CI 1.03-1.11), and lower rates of cytological examination (PR 1.08; CI 1.01-1.14). The results reinforce the need for improvements in the national cervical cancer prevention program in areas with low coverage of oncotic cytology.

This article seeks to reflect on the relationship between adolescence and body at present, from the point of view of psychoanalytic theory. In order to do this, we analyze the conception of the body in psychoanalysis starting from the works of Freud and other contemporary authors, through the concepts of hysteria, drive, self, and stage of the mirror. In this purpose, we highlight the function of the self-image in the formation of the Self in the mirror stage to understand the impacts of the destabilization of this image with the physical transformations of adolescence. We consider, then, adolescence a re-edition of the stage of the mirror where the subject, who no longer sees himself as a child but does not yet recognize his adult image, has the unity of the Self destabilized, having to reinvent a position for himself in front of the Other. Finally, we questioned the possible impacts brought by the increasing virtualization of the image on screens for these subjects

ABSTRACT Background: Gastric cancer (GC) represents a public health problem in Colombia and worldwide. Since most patients are at advanced stages at the time of diagnosis. it is necessary to develop management strategies as conversion therapy (CT). Objective: The aim of this study was to estimate the results of CT for treating patients with advanced and GC at Instituto Nacional de Cancerología de Colombia (INC). Material and methods: We included patients with incurable gastric cancer who underwent induction chemotherapy and intended curative surgery between 2010 and 2021. The clinical and pathological data and survival of the patients included were retrospectively reviewed. Overall survival (OS) was calculated from the time of initiation of chemotherapy until the date of death. Survival functions were estimated using the life table and Kaplan-Meier methods. and survival curves at 3 and 5 years were constructed. Results: 23 patients were analyzed; mean age was 56 years. and 17 (74%) were men. The most common criterion indicating unresectability was a T4b tumor in 13 cases (56.5%). All the patients underwent CT. Median follow-up was 28 months. Eleven patients developed disease recurrence (52%). Median survival was 41.2 months. and 3- and 5-year OS was 57.7% and 38.5%. respectively. Conclusions: CT provided an acceptable OS rate for selected patients with incurable advanced GC. This strategy requires an adequate selection of patients and multidisciplinary management in reference oncology centers.

RESUMO

Trials within cohorts (TwiCs) are design methods derived from randomized controlled trials (RCTS). They have been widely used in chronic disease areas such as tumors and cardiovascular diseases. The basis of the TwiCs design is a prospective cohort of specific diseases. When RCTS need to be implemented, some patients meeting the inclusion and exclusion criteria are randomly sampled from the cohort to receive "trial interventions", while the remaining patients in the cohort who meet the inclusion and exclusion criteria continue to receive conventional treatment as control groups. By comparing the efficacy differences between the intervention measures of the trial group and the control group, the efficacy of intervention measures was evaluated. Within the cohort, the same process could be repeated to carry out multiple RCTS, so as to evaluate different intervention measures or compare the efficacy of different doses or timing of interventions. Compared with classical RCTS, TwiCs make it easier to recruit patients from the cohort and have higher external validity, providing a new research paradigm for improving the efficiency and applicability of RCTS in clinical practice. However, TwiCs may also face the challenge of poor compliance of patients in the cohort. Researchers need to take effective measures to control these patients in the design and operation of TwiCs. This article focused on the methodological key points during the implementation of TwiCs, including multi-stage informed consent (patients are informed of consent at three stages: entering the cohort, entering the trial group, and after the trial), randomization procedures (only random sampling of patients from the cohort to receive "trial interventions"), sample size calculation, and statistical analysis methods. The article also compared the differences between TwiCs and traditional RCTS and illustrated TwiCs research design and analysis with examples, so as to provide new research ideas and methods for clinical researchers.

RESUMO

@#Objective    To investigate the clinical effect of 3D computed tomography bronchial bronchography and angiography (3D-CTBA) and guidance of thoracoscopic anatomic pulmonary segmentectomy by Mimics software system. Methods    A retrospective analysis was performed on patients who underwent thoracoscopic segmentectomy in the Department of Thoracic Surgery of Affiliated People's Hospital of Jiangsu University from June 2020 to December 2022. The patients who underwent preoperative 3D-CTBA using Materiaise's interactive medical image control system (Mimics) were selected as an observation group, and the patients who did not receive 3D-CTBA were selected as a control group. The relevant clinical indicators were compared between the two groups. Results    A total of 59 patients were included, including 29 males and 30 females, aged 25-79 years. There were 37 patients in the observation group, and 22 patients in the control group. The operation time (163.0±48.7 min vs. 188.8±43.0 min, P=0.044), intraoperative blood loss [10.0 (10.0, 20.0) mL vs. 20.0 (20.0, 35.0) mL, P<0.001], and preoperative puncture localization rate (5.4% vs. 31.8%, P=0.019) in the observation group were better than those in the control group. There was no statistically significant difference in the thoracic tube placement time, thoracic fluid drainage volume, number of intraoperative closure nail bin, postoperative hospital stay, or postoperative air leakage incidence (P>0.05) between the two groups. Conclusion    For patients who need to undergo anatomical pulmonary segmentectomy, using Mimics software to produce 3D-CTBA before surgery can help accurately identify pulmonary arteriovenous anatomy, reduce surgical time and intraoperative blood loss, help to determine the location of nodules and reduce invasive localization before surgery, and alleviate patients' pain, which is worthy of clinical promotion.

RESUMO

Objective To explore the prognostic factor and its predictive value of patients with Wilson disease-related acute-on-chronic liver failure(WD-ACLF).Methods The clinical data of 70 patients diagnosed as WD-ACLF admitted to the Department of Encephalopathy of the First Affiliated Hospital of Anhui University of Chinese Medicine from January 1,2017 to January 1,2022 were retrospectively collected.According to the 12-week prognosis, patients were divided into survival group(n=36)and death group(n=34).The data of the two groups were analyzed by univariate and multivariate logistic analysis to screen the prognostic risk factors and evaluate their predictive value. The model coefficient is omnibus tested, and the model-fitting degree is evaluated by the Hosmer-Lemeshow test.ROC curve was used to analyze the prognostic value for WD-ACLF between the new model and chronic liver failure-sequential organ failure assessment(CLIF-SOFA)score,model for end-stage liver disease(MELD)score and Child-Turcotte-Pugh(CTP)score.Results A total of 70 WD-ACLF patients were enrolled in present study, including 36 cases in survival group[22 males and 14 females with median age of 30.0(17.3,40.0)]and 34 cases in death group[25 males and 9 females with median age of 34.0(28.8,41.0)].Univariate analysis showed that the course of disease, prothrombin time(PT),activated partial thromboplastin time(APTT)were shorter in survival group than that in death group, the white blood cells(WBC),international normalized ratio(INR),aspartate transaminase(AST),total bilirubin(TBIL),blood urea nitrogen(BUN),creatinine(Cre)and ceruloplasmin(CER)levels and the proportion of infection, ascites, and upper gastrointestinal bleeding were lower in survival group than those in death group, however, the proportion of infection, ascites and upper digestive bleeding in the survival group were lower than those in the death group.Meanwhile, the red blood cells(RBC),hemoglobin(Hb),Na+ and total cholesterol(TC)level in the survival group were higher than those in the death group(P<0.05 or P<0.01).The results of multivariate logistic regression analysis showed that disease course(OR=1.176,95%CI 1.043-1.325),INR(OR=7.635,95%CI 1.767-32.980),TBIL(OR=1.012,95%CI 1.003-1.021),and upper gastrointestinal bleeding(OR=11.654,95%CI 1.029-131.980)were independent risk factors affecting the prognosis of WD-ACLF(P<0.05).Based on the results of logistic regression analysis,a joint model for predicting the prognosis of WD-ACLF was established. The AUC of the model for evaluating the prognosis of WD-ACLF was 0.941,which was greater than the CLIF-SOFA score(AUC=0.802),MELD score(AUC=0.897),and CTP score(AUC=0.722).Conclusions The course of disease,TBIL,INR,and upper gastrointestinal bleeding are risk factors that affect the prognosis of WD-ACLF.The prognosis model established based on this can more accurately predict the prognosis of WD-ACLF patients, and its predictive value is superior to CLIF-SOFA score,MELD score, and CTP score.

RESUMO

BACKGROUND:Due to the lack of mechanical stimulation generated by functional loading, alveolar bone atrophy and bone loss in the missing area prevents the effective embedding of dental implants. Therefore,it is necessary to implant bone meal during implantation to compensate for the lack of alveolar bone height on the stability of the implant. OBJECTIVE:To derive the optimal time point for second-stage restorations from a biomechanical perspective by means of finite element analysis, thereby providing a biomechanical basis for selecting the correct time point for second-stage restoration to shorten the clinical course of dental implantation. METHODS:Three-dimensional finite element models of the maxilla were developed for normal,16-deficient with conventional implant surgery(model B)and 16-deficient with internal maxillary sinus lift(model A).Model A was filled with hydroxyapatite bone powder between the mucosa and the maxillary sinus floor, and Model B and the normal maxillary bone model did not require bone grafting. The healing time of bone grafting was set at 3-9 months postoperatively, and a force of 200 N was applied to the adjacent teeth at 3-5 months postoperatively, and directly to the implant at 6-9 months, simulating the load applied to the implant in the second stage of restoration.Biomechanical analysis of the three models was performed with the aid of the implants and the surrounding hard and soft tissues. RESULTS AND CONCLUSION:The stress on Model A and Model B was around 103 MPa and 95 MPa respectively when the force was directly loaded onto the implant.Bone grafting increased the stress values on the implant, while not doing bone grafting reduced the stress on the implant and the whole system. As the strength of the bone powder increased, the stress values on the alveolar bone decreased. When the healing time was up to 6-9 months postoperatively, the stress values were ranked as follows:normal model<Model B<Model A,when the force was loaded in the implant or the corresponding first molar sites. The stress on the maxilla after dental implantation was higher than that on the normal maxilla. There was a slight downward trend in cancellous bone stress in Model A and significantly lower stress levels in Model B than in the normal mandible model at 6-9 months. To conclude, the use of the original alveolar bone for restoration can improve the survival rate of the implant;the second-stage restoration can be considered after 6 months when sufficient alveolar bone height is obtained after implantation. In clinical application, the timing of second-stage restoration can be adjusted appropriately on the basis of the patient's bone quality, lifestyle, age and other factors after comprehensive determination.

RESUMO

BACKGROUND:Currently, there is a lack of large sample studies to analyze the bone metabolism level of patients with femoral head necrosis of different etiologies and stages, which is not conducive to the development of better necrosis-promoting repair strategies. OBJECTIVE:To study the bone metabolism of patients with osteonecrosis of the femoral head with different etiologies and Association Research Circulation Osseous(ARCO)stages. METHODS:A retrospective study was performed on 401 patients diagnosed with osteonecrosis of the femoral head as the trial group, and 81 healthy subjects as the control group. The trial group could be divided into three groups according to different etiologies:steroid-induced osteonecrosis of the femoral head, alcoholic osteonecrosis of the femoral head and traumatic osteonecrosis of the femoral head, and were divided into stages Ⅱ/Ⅲ/Ⅳ according to different ARCO stages.Seven bone metabolism-related indicators of all subjects were collected, including bone metabolism-regulating hormone 25-hydroxyvitamin D and bone conversion markers:N-terminal propeptide of type Ⅰ procollagen, degradation product of type Ⅰ collagen,n-terminal middle molecular fragment of osteocalcin, general biochemical markers of bone metabolism:serum calcium, serum phosphorus, serum alkaline phosphatase. The bone metabolism levels of each group were compared and the independent factors associated with osteonecrosis of the femoral head were determined by binary Logistic regression analysis. RESULTS AND CONCLUSION:Compared with the control group, levels of degradation product of type Ⅰ collagen,N-terminal propeptide of type Ⅰ procollagen,n-terminal middle molecular fragment of osteocalcin, serum phosphorus and alkaline phosphatase in the trial group were significantly increased(all P<0.05).Based on the presence or absence of the disease, according to binary Logistic regression analysis, degradation product of type Ⅰ collagen,N-terminal propeptide of type Ⅰ procollagen, and n-terminal middle molecular fragment of osteocalcin were independent factors associated with osteonecrosis of the femoral head. The levels of degradation product of type Ⅰ collagen and N-terminal propeptide of type Ⅰ procollagen in three groups of patients with different etiologies were higher than normal reference values. The bone metabolism-regulating hormone 25-hydroxyvitamin D and serum calcium in the alcoholic osteonecrosis of the femoral head group were higher than those in the other two groups(P<0.05).The level of bone metabolism-regulating hormone 25-hydroxyvitamin D in steroid-induced and traumatic osteonecrosis of the femoral head groups was lower than the normal value. There were no significant differences in seven bone metabolism-related indicators in patients with ARCO stages Ⅱ,Ⅲ and Ⅳ osteonecrosis of the femoral head(all P>0.05),but degradation product of type Ⅰ collagen and N-terminal propeptide of type Ⅰ procollagen in these three groups were higher than normal reference values.Bone metabolism-regulating hormone 25-hydroxyvitamin D in patients with ARCO stage Ⅱ and ARCO stage Ⅳ was lower than the normal reference value. It is concluded that the bone metabolism level of osteonecrosis of the femoral head patients was abnormal. The degradation product of type Ⅰ collagen and N-terminal propeptide of type Ⅰ procollagen of osteonecrosis of the femoral head patients with different etiologies and ARCO stages were all higher than the normal reference value, and they were in a state of high bone turnover.Degradation product of type Ⅰ collagen,N-terminal propeptide of type Ⅰ procollagen and n-terminal middle molecular fragment of osteocalcin may be risk factors for the pathogenesis of osteonecrosis of the femoral head.

RESUMO

BACKGROUND:Stage Ⅲ Kümmell's disease is characterized by a high degree of vertebral compression and posterior wall defects. Most of the patients are elderly people with severe osteoporosis and various medical diseases.Clinically, some surgical methods are often at high risk and are controversial. OBJECTIVE:To investigate the clinical efficacy of screw placement combined with transpedicular impaction bone grafting in the treatment of stage Ⅲ Kümmell's disease. METHODS:The clinical data of injured vertebral screw placement combined with transpedicular impaction bone grafting in treatment of stage Ⅲ Kummell's disease from May 2016 to August 2021 were retrospectively analyzed.Visual analog scale score,Oswestry disability index, anterior vertebral heights, kyphotic Cobb angle and American Spinal Injury Association(ASIA)impairment scale were used to evaluate the effects of surgery. The operation time, intraoperative blood loss and complications were recorded.CT scans were used to evaluate the healing of injured vertebrae at the final follow-up visit. RESULTS AND CONCLUSION:(1)A total of 26 patients were included, with 7 males and 19 females,at the age range of 62-81 years[mean(69.7±4.8)years].The follow-up time was 18-60 months[mean(35.1±8.9)months].The average operative duration was 133.5 minutes(100-165 minutes),and the average intraoperative blood loss was 285.3 mL(210-350 mL).(2)Visual analog scale and Oswestry disability index scores 1 week after surgery were significantly lower than those before surgery.(3)At 1 week after surgery, the anterior vertebral height corrections and the Cobb angle were(9.0±0.7)mm and(16.2±1.0)°,respectively.During the follow-up period, the loss of vertebral height and kyphosis correction were(5.1±0.3)mm and(8.0±0.4)°,respectively.(4)14 patients(54%)had ASIA grade D before operation, which recovered to grade E at the last follow-up.CT scan showed that all patients achieved good osseous union.(5)Complications occurred in seven patients(27%),including hypostatic pneumonia in two cases, postoperative superficial wound tissue liquefaction in two cases, and adjacent vertebral compression fractures in three cases.(6)It is concluded that screw placement of the injured vertebra combined with transpedicular impaction bone grafting can rapidly rebuild spinal stability, effectively relieve pain and improve neurological function in the treatment of stage Ⅲ Kümmell's disease. This technique is an effective and relatively minimally invasive surgical option.
