RESUMO

Introduction: The SARS CoV 2 infection has resulted in several health, economic, and social crises in all areas. The disease shows a substantial biological diversity in humans causing a series of sequels in the trans- or post-infection period in the entire organism. Case Report: The manifestations that occur in the oral cavity and pharynx have not been evaluated. In this study, two clinical cases are reported. The first patient, a 67-year-old male, presents erosive lesions on the dorsal surface of his tongue after SARS CoV 2 infection. Results: Therapy consisting of reinforcing oral cleaning, use of antifungal solutions, mouthwashes containing superoxidation solution and B complex was given to the patient. The reported lesions improved satisfactorily. The second case, a 47-year-old male patient, presented vesiculobullous lesions on the lingual and labial mucosa accompanied by severe painful symptoms after SARS CoV 2 infection. An incisional biopsy was performed. The histopathological result was compatible with pemphigus vulgaris, and the treatment protocol was started with 0.1% topical mometasone and 2g miconazole gel, observing adequate involution of the lesions after 20 days. Conclusions: The aim of this study is to report on the lesions affecting the oral cavity and pharynx in post-COVID patients with the aim of carrying out a thorough intraoral examination, establishing a clinical or histopathological diagnosis to implement a specific treatment plan in each case to improve the health and quality of life of the patients. Keywords: SARS-CoV-2; Oral manifestations; Oral ulcer; Pemphigus; Mouth; Mucous membrane.

Introduction: bite trauma or traumatic ulcer is an acute injury of the oral mucosa whose etiology is mechanical trauma or irritation of the affected soft tissue. Objective: to report a multidisciplinary treatment of self-inflicted lower lip trauma. Case report: 17-year-old male patient, paraplegic, was bedridden due to neuromotor limitations and, involuntary and reflexive, bit the lips. This movement traumatized the region of the lower lip causing the traumatic ulcer, which did not heal. Initially, an ethyl vinyl acetate mouthguard was installed, but the patient did not adapt. Then, three sessions of laser therapy (photobiomodulation) was performed weekly and point, along the traumatic ulcer, in red and infrared radiation (660nm; 808nm; 100mW; 2J/cm2 Laser DUO MMO), combined with elastic bandages (the Therapy Taping® method) to prevent invagination into the oral cavity, to move the lower lip away from his teeth in order to avoid trauma to the oral mucosa. Conclusion: the presentation of this report demonstrated some possibilities for the treatment of a traumatic injury, and its management is still a major clinical challenge in Dentistry, especially for patients with motor and/or cognitive impairments.

RESUMO

Introduction: Traumatic ulcerative granuloma with stromal eosinophilia is an uncommon condition of the oral mucosa with a chronic course, usually affecting the tongue. Case Report: Clinically it presents as a chronic ulcer, with raised and indurated borders, rarely presented as a tumor. Histologically it shows a diffuse mixed inflammatory infiltrate, rich in eosinophils. The etiology of this lesion is still unclear; however, chronic irritation from traumatic agents is considered a major initiating factor. In some cases, the presence of CD30+ mononuclear cells within the lesions suggest the possibility of a CD30+ lymphoproliferative disorder. This article presents a case of a traumatic ulcerative granuloma with stromal eosinophilia manifested in a 56-year-old female with a solitary ulcerated tumor inside the right cheek. Conclusion: It was diagnosed based on clinical data and histopathological features. In a brief literature review, the entity has been characterized, analyzing its etiology and nature.

ABSTRACT Syphilis is a sexually transmitted infection caused by the bacteria Treponema pallidum. Over the last years there has been an increase of the incidence of this disease due to the growing infection by the human inmunodeficiency virus (HIV) and the inmunosupression. Known as "the great imitator", there are many signs and symptoms that can simulate, being the oropharyngeal presentation infrequent. We present the case of a 31-year-old man who came to our service for bilateral otitis media. Physical examination revealed a mass in the cavum and ulceration in the posterior pharyngeal wall that was biopsied in the outpatient clinic. The result was an intense chronic inflammatory infiltrate of probable infectious origin, so it was decided to take a new biopsy under general anesthesia. On the day of the intervention, the patient reported the appearance of a generalized erythematous rash, but predominantly on the bottoms of the feet and on the palms of the hands. Questioned about his background, the patient recognized risky sexual behavior and being an HIV carrier, so the diagnosis was oriented towards a probable secondary syphilis that was subsequently confirmed by serology. The ulcerations of the oropharynx, the mass of the cavum and the rash subsided with antibiotic treatment and corticoid regimen. When the manifestations of the head and neck are the initial presentation of syphilis, its diagnosis is often delayed due to the lack of knowledge of the physicians of the typical presentation forms in this location. Early diagnosis is essential to prevent the spread of the disease and its complications.

RESUMO

Abstract Ankylosing Spondylitis (AS) is an inflammatory rheumatic disease that affects the axial skeleton and the sacroiliac joints. Recent studies investigated the link between AS and oral diseases, particularly periodontitis. Others suggested that periodontitis may have a role in the pathogenesis of rheumatic diseases. Objective: The aim of this study is to investigate the association between AS and oral conditions. Material and Methods: This research was conducted using the UK Biobank Resource under Application Number 26307. The UK Biobank recruited around 500000 participants throughout Great Britain. Clinical records were available for 2734 participants. Two case-control studies were conducted based on whether AS was self-reported or clinically diagnosed. Oral conditions were identified using self-reported reports of oral ulcers, painful gums, bleeding gums, loose teeth, toothache, and dentures. The association between AS and oral conditions was assessed using logistic regression adjusted for age, gender, educational level, smoking status, alcohol consumption, and body mass index. Results: A total of 1307 cases and 491503 control participants were eligible for the self-reported AS study. The mean age was 58 years for the cases [7.5 standard deviation (SD)] and 57 years for the control groups (8.1 SD). Also, 37.1% of the cases and 54.2% of the control participants were females. Among the oral conditions, only oral ulcers were strongly associated with AS [1.57 adjusted odds ratio (OR); 95% confidence interval (CI) 1.31 to 1.88]. For the study of clinically diagnosed AS, 153 cases and 490351 control participants were identified. The mean age for both cases and control groups was 57 years; 7.6 SD for the cases and 8.1 for the control group. Females corresponded to 26.1% of the cases, and 54.2% of the control participants. Clinically diagnosed AS was associated with self-reported oral ulcers (2.17 adjusted OR; 95% CI 1.33 to 3.53). Conclusion: Self-reported and clinically diagnosed AS populations have increased risk of reporting oral ulcers. Further investigations are required to assess the link between a specific type of oral condition and AS.

RESUMO

Abstract Radiation combined injury, a life-threatening condition, has higher mortality than simple radiation injury. The aim of the present study was to analyze the efficiency of Aloe vera and silver nanoparticles in improving the healing of ulcerated oral mucosa after irradiation. Thirty male Albino mice were divided into five groups: control, radiation, Aloe vera (AV), silver nanoparticles (NS), and AV+NS. The mice were exposed to whole body 6Gy gamma-radiation. After one hour, 20% acetic acid was injected into the submucosal layer of the lower lip for ulcer induction. The animals received topical treatment with the assigned substances for 5 days. Lip specimens were subjected to hematoxylin and eosin and anti alpha-smooth muscle actin immunohistochemical staining. Results demonstrated occurance of ulcer three days post irradiation in all groups except in the AV+NS group where only epithelial detachment was developed. After seven days, data revealed persistent ulcer in radiation group, and almost normal epithelium in the AV+NS group. A significant reduction of epithelial thickness was detected in all groups at the third day as compared to control. At the seventh day, only the AV+NS group restored the epithelial thickness. Area percent of alpha-smooth muscle actin expression was significantly decreased in radiation group at the third day followed by significant increase at the seventh day. However, all treatment groups showed significant increase in alpha-smooth muscle actin at the third day, which decreased to normal level at the seventh day. Our study demonstrated the efficiency of Aloe vera and silver nanoparticles in enhancing ulcer healing after irradiation.

Systemic lupus erythematosus is a chronic auto-immune disease with multiple clinical expressions varying from feeling generally run down with cutaneous lesions to severe systemic compromise affecting several organs. As with similar diseases there may be involvement of the oral mucosa, with areas of erythema, erosions and ulcers as well as white discoid lesions, all more pronounced with increasing disease severity. We present the case of a young girl diagnosed with systemic lupus erythematosus three years before who presented facial, lip and gingival lesions associated with bacterial biofilm and other contributing local factors. It is important that dental surgeons be aware of the possibility of finding systemic lupus erythematosus lesions in a patient´s mouth, it being one of the first signs of the disease, thereby helping to reach a timely diagnosis by referring the patient to a specialist. Similarly, the physician should consider treating oral lesions as these have an important effect on the patient´s quality of life.

The description of the Bednar's ulcer is uncommon in the current literature. It has been associated with the traumatic effect of the bottle's nipple and/or no orthodontic soothers while breastfeeding. We present a newborn of 20 days of life attended at the emergency room for irritability, with the only finding on physical examination of two oral ulcers. We describe the clinical presentation, evolution and treatment. The normality of the diagnostic test, clinical characteristics and evolution lead to the diagnosis of Bednar's ulcer.

Behcet's syndrome, also known as Behcet's disease is a chronic autoinflammatory process of low frequency and unknown etiology. It is a vasculitis affecting arteries and veins of all sizes that causes an alteration of endothelial function and is expressed clinically by organ damage at various levels. It has a higher incidence in countries located in the ancient Silk Road (East Mediterranean, Middle East and East Asia). Its pathogenesis involves genetic, microbial and immunological factors.Symptoms vary from person to person and according to geographical regions, but the most common are oral and genital ulcers, eye inflammation (uveitis, iritis and retinitis), skin lesions and arthritis. In some cases they can present central nervous system, gastrointestinal tract and kidney. As a guide to the diagnosis criteria of International Study Group of Behcet's disease are used. Treatment includes topical and systemic drugs. The most commonly used are steroids, tumor necrosis factor inhibitors and immunosuppressants(AU)

RESUMO

Aim Describe the location of traumatic lesions of the oral mucosa that develop after the installation of complete dentures, as well as to quantify the number of post-operative controls that are required. A descriptive study was conducted by examining 84 patients who attended the dental center of Universidad de Los Andes, San Bernardo, during the period from July 2012 to July 2013. A sample of 120 edentulous patients was obtained. After the fabrication and installation of the complete dentures, at least 3 post-operative controls were performed and the location of oral lesions was recorded. Documentation of the association between the patient's clinical variables and the appearance of oral lesions during the first 3 controls was performed using a logistic regression. Results For maxillary dentures, 5 post-operative visits were made and 6 controls for mandible dentures. In the upper jaw the anatomical areas of higher incidence of traumatic injuries were: canine fossa (23.9%), average bridle (23.1%), and distobuccal sulcus (20.1%). In the lower jaw, the highest number of lesions were recorded on the anterior lingual flank (16.5%), anterior and posterior lingual flank (13.4%), and distobuccal sulcus (12.8%). A significant association was observed between subjects who reported consumption of cholesterol-lowering medications and the development of traumatic lesions of the oral mucosa (OR: 0.25 and 95% CI: 0.055-0.939). The installation of complete dentures does not determine that the treatment has ended. Post-operative controls are needed to assess areas of erythema and ulceration.

To document the frequency of cutaneous manifestation of systemic lupus erythematosus [SLE]. It was a case-series collected from Dermatology Department and all four Medical Units of Bahawal-Victoria Teaching Hospital of Quaid-e-Azam Medical, Bahawalpur. A total of 100 patients with diagnosis of SLE and fulfilling the inclusion criteria were included in the study. All the patients were evaluated for the cutaneous manifestations of SLE. All the information was collected in a specially designed proforma and analyzed with the help of SPSS version 10. The mean age of the patients was 25.97 +/- 4.64 years. 92% patients were female and 8% were male. Out of 100 patients, 85% patient presented with cutaneous manifestations. Photosensitivity was seen in 40 [40%] patients, discoid rash in 35 [35%], malar rash in 25 [25%], and oral ulcers in 24 [24%]. SLE is predominantly seen among female patients of young age. Photosensitivity is most common presentation, followed by discoid rash

RESUMO

Objectives: This study aimed to assess the hypothesis that recurrent denture-related sores association may be associated with the risk of oral cancer. Methods: We conducted a hospital-based case-control study comprising 71 new cases of oral cancer in two hospitals in São Paulo, Brazil, and 240 controls without cancer, recruited from outpatient units of the same hospitals. All cases had histologically confirmed squamous cell carcinoma in anatomic sites of the mouth that may be specifically consi-dered at risk of sores by ill-fitting dentures. Denture-related sores were assessed by the self-report of recurrent oral sores due to the use of ill-fitting complete removable dental prosthesis. Associations were assessed by multivariate logistic regression conditioned on socio-demographic and behavioral characteristics. Results: The association between ill-fitting dentures and oral cancer was statistically significant in the multivariate model: odds ratio 3.98; 95% confidence interval 1.06 - 14.96. The specific assessment of association between tumors in the lower jaw and sores by mandibular dentures confirmed this result: odds ratio 6.39; 95% confidence interval 1.49 - 29.52. Conclusion: The potential contribution of denture-related sores to oral carcinogenesis still fuels controversies. This study reinfor-ces the hypothesis that recurrent denture-related sores may be associated with the risk of oral cancer. Results reported here also suggest that an appropriate application and monitoring of dental prosthesis represent a non-negligible scope for cancer prevention. .

Introduction: Palatal torus, or torus palatinus, is a benign bone alteration that may cause some discomfort during phonation or swallowing. When its growth or persistent exposition produces unpleasant symptoms, it must be surgically removed. Case presentation: We treated an 82-year-old female patient who consulted for a painful ulcerous lesion she had had for a year and which produced halitosis and discomfort when swallowing. During the oral cavity physical exam we observed a hard protuberance on the midline. It showed a perforation and ulceration of the lateral posterior palatal mucosa on the left side. We considered the following diagnosis: palatal torus, osteoma, pyogenic granuloma, or a soft-tissue neoplasia. We decided to surgically remove it and to perform a histopathological examination. Clinical evolution was satisfactory with complete resolution one month after surgery. The histopathological examination showed hyperostosis along with chronic inflammation of the soft tissues and a simple hyperplasia of the mucosa, which in turn indicated a palatal torus. Discussion: We report an unusual case of spontaneous exposition of a palatal torus which took up almost all of the hard palate area. Its resection is described and we inform the histopathological findings. In the literature review, we did not find a previous report of a spontaneous exposition of a palatal torus.

RESUMO

Introduction: Oral tuberculosis is a chronic granulomatous disease caused by Mycobacterium tuberculosis. According to the World Health Organization (WHO), nearly 2 billion people are infected with tuberculosis. Objective: To report a rare case of oral tuberculosis with ulcers at two different sites. Materials and methods: In order to emphasize the need to consider tuberculosis in the differential diagnosis of non-healing ulcers of oral cavity and the role of oral physician in preventing the spread of such lesions, this article presents a case of oral ulcers at two different sites, diagnosed as tuberculosis ulcer. Results: Oral cavity, which is considered to be a mirror of systemic health, rarely manifests tuberculosis. With increasing number of tuberculosis cases and unusual form of the disease in the oral cavity, the cases are likely to be misdiagnosed. Conclusion: An oral clinician should have a thorough knowledge of tuberculosis and its oral manifestations.
