RESUMO

Background: Increasing chronic diseases challenges the health systems of low- and middleincome countries, including Cameroon. Type 1 diabetes (T1D), among the most common chronic diseases in children, poses particular care delivery challenges. Aim: We examined social representations of patients' roles and implementation of T1D care among political decision-makers, healthcare providers and patients within families. Setting: The study was conducted in Yaoundé, Cameroon. Methods: Eighty-two individuals were included in the study. The authors conducted semistructured interviews with policy makers (n = 5), healthcare professionals (n = 7) and patients 'parents (n = 20). Questionnaires were administered to paediatric patients with T1D (n = 50). The authors also observed care delivery at a referral hospital and at a T1D-focused nongovernmental organisation over 15 days. Data were analysed using thematic content analysis and descriptive statistics. Results: Cameroonian health policy portrays patients with T1D as passive recipients of care. While many practitioners recognised the complex social and economic determinants of adherence to T1D care, in practice interactions focused on specific biomedical issues and offered brief guidance. Cultural barriers and policy implementation challenges prevent patients and their families from being fully active participants in care. Parents and children prefer an ongoing relationship with a single clinician and interactions with other patients and families. Conclusion: Patients and families mobilise experience and lay knowledge to complement biomedical knowledge, but top-down policy and clinical practice limit their active engagement in T1D care. Contribution: Children with T1D and their families, policy makers, healthcare professionals, and civil society have new opportunities to contribute to person-centred care, as advocated by the Sustainable Development Goals.

Abstract Starch samples from eight legumes cultivars instar of one variety of Vigna unguiculata L. (Cowpea), one variety of Vigna subterrenea V. (Bambara groundnut) and six varieties of Phaseolus vulgaris L. (Common bean), grown in Cameroon were isolated, and their physicochemical and pasting properties were evaluated. The objectives of the study were to investigate the starch properties and processing characteristics of different bean varieties, and to establish the basic foundation of improving the functionality of beans and their starch grown in the region. The result revealed significant differences amongst the properties of the starches. The swelling power of the legume starch isolates put them in the category of highly restricted-swelling starch. This characteristic is desirable for the manufacture of value-added products such as noodles and composite blends with cereals. The pasting properties were determined using a rapid visco analyzer, and various legumes bean starches exhibited different pasting profiles. The high breakdown viscosity (BV) was founded for Cowpea and Bambara groundnut and confirmed their low. ability to resist heat and shear stress when compared to Common bean varieties studies. The factors which influence the pasting characteristics resulting to decrease in peak viscosity (PV), trough viscosity (TV) and final viscosity (FV) of starch are attributed to the interaction of starch with the protein, fat, etc. which depended to their variety.


Resumo Foram isoladas amostras de amido de oito cultivares de leguminosas ínstar de uma variedade de Vigna unguiculata L. (feijão caupi), uma variedade de Vigna subterrenea V. (amendoim Bambara) e seis variedades de Phaseolus vulgaris L. (feijão comum), cultivadas nos Camarões, e suas propriedades físico-químicas e de pasta foram avaliadas. Os objetivos do estudo foram investigar as propriedades do amido e as características de processamento de diferentes variedades de feijão e estabelecer as bases básicas para melhorar a funcionalidade do feijão e do amido cultivado na região. O resultado revelou diferenças significativas entre as propriedades dos amidos. O poder de inchamento dos isolados de amido de leguminosas os coloca na categoria de amido com inchamento altamente restrito. Esta característica é desejável para o fabrico de produtos de valor acrescentado, tais como macarrão e misturas compósitas com cereais. As propriedades de pasta foram determinadas usando um analisador rápido de visco, e vários amidos de feijão leguminosos exibiram diferentes perfis de pasta. A alta BV foi fundada para o amendoim Cowpea e Bambara e confirmou sua baixa. capacidade de resistir ao calor e tensão de cisalhamento quando comparado com estudos de variedades de feijão. Os fatores que influenciam as características colantes resultantes da diminuição do pico de viscosidade (PV), da viscosidade mínima (TV) e da viscosidade final (FV) do amido são atribuídos à interação do amido com a proteína, gordura, etc., que dependem da sua variedade.

Introduction: les algies pelviennes aiguës sont responsables d'une morbi-mortalité importante. L'objectif de ce travail était de décrire leurs aspects épidémiologiques, cliniques et thérapeutiques à Yaoundé.Méthodes: nous avons mené une étude transversale descriptive avec collecte prospective des données au Service de Gynécologie-Obstétrique de l'Hôpital Gynéco-Obstétrique et Pédiatrique de Yaoundé du 1er avril au 31 juillet 2015. Nous avons inclus toutes les femmes admises pour douleur pelvienne dont l'évolution était inférieure à un mois et ayant accepté de participer à l'étude. Nous avons exclu les femmes qui étaient au troisième trimestre de grossesse ou en post-partum. Le logiciel Epi info version 3.5.4 a servi à l'analyse des données. Ces données ont été présentées sous forme de fréquence et de pourcentage.Résultats: au total 5915 femmes ont consulté pendant la période de l'étude dont 125(2,11%) étaient des algies pelviennes aiguës. La moyenne d'âge était de 29,5 ± 6,9 ans. Les étiologies des douleurs étaient les infections génitales hautes (36,8%) et la grossesse extra-utérine (18,4%). Le traitement surtout médical (92,8%), associait antibiotiques (65,5%), anti-inflammatoires (56,9%) et antalgiques (39,7%). La chirurgie a été réalisée chez 25(20%) patientes par laparotomie (80%) et cœlioscopie (20%)L'indication chirurgicale était la grossesse extra-utérine dans 76% des cas. La régression de la douleur était obtenue chez 99% des cas. Conclusion: les d'algies pelviennes aigues survenaient chez les femmes jeunes, dues aux infections génitales hautes et à la grossesse extra-utérine étaient principalement. En cas de grossesse extra-utérine le traitement chirurgical était surtout la laparotomie

RESUMO

We are reporting a case of Acute Post-Infectious Flaccid paralysis also commonly known as Guillain-Barré Syndrome (GBS) in a patient with confirmed COVID-19 infection. GBS often occurs following an infectious trigger which induces autoimmune reaction causing damage to peripheral nerves. So far, only 8 cases have been described in association with COVID-19. This is the first to be described in Tanzaniain an African Child, and probably the first in the continent. This report is presented for clinicians to be aware and for the medical fraternity to look into this unusual presentation which may shed some more light on possible pathways of the pathogenesis and clinical manifestations. We recommend that the presentation of GBS with acute respiratory distress should warrant extra precaution and a testing for COVID-19 especially when the symptoms of COVID-19 are protean

RESUMO

Coronavirus disease (COVID-19) is an unprecedented pandemic. COVID-19 is a highly contagious and potentially fatal respiratory infection which has spread within three months of its outbreak to more than 173 countries, causing 3.7 million infections and 256,551 deaths at this writing. Unfortunately, no treatment or vaccine currently exists for COVID-19, although several clinical trials are on-going to find a definite solution to this pandemic. Prevention through public health measures remain the best strategy recommended till date. This prevention involves physical distancing and compulsory confinement at home in several European countries, in the UK and USA. Unfortunately, home confinement decreed in most high-income countries like France has been dangerous for women, victims of psychological, physical and sexual violence from their intimate partner. Violence between intimate partners has become an unintended consequence of the stay-at-home policy against COVID-19. Since the promulgation of a home confinement decreed in many high resource settings (USA, UK, Europe, Canada, Australia, etc), the rate of violence between intimate partners has increased tremendously resulting to the worst scenario, women's death in some of these countries. The stay-at-home law is not yet a national decree in several low resource settings like Africa, where COVID-19 has not been declared an epidemic in several countries. However, intimate partner violence has been reportedly described as a real violation of women's right before the advent of the COVID-19 pandemic in the African continent. This commentary highlights the effects of intimate partner violence due to COVID-19 confinement in France and extrapolates what may be the effect of an implementation of a COVID-19 confinement law in Cameroon. Also, the authors suggest recommendations to lessen the burden of intimate partner violence in countries with a stay-at-home policy

RESUMO

Confirmation of COVID-19 infection is still challenging in Cameroon due to lack of sufficient RT-PCR tests and screening capacity. International organizations as well as philanthropic donators initially provided these tests. Due to limited kits availability, mass screening is currently a luxury that the country cannot afford. This impacts on estimates of disease prevalence, on the understanding of its burden and delays the implementation of targeted preventive measures. Besides RT-PCR, chest CT-scan could be a useful tool for screening purposes. However, its sensitivity and cost make its integration in management algorithms difficult. We discuss below the challenges and potential solutions that could ease the diagnosis of COVID-19 infection in Cameroon

RESUMO

La pandémie à Coronavirus 2019 (COVID-19) touche les pays d´Afrique sub-Saharienne depuis le mois de mars 2020. Au-delà des désastres sanitaire et économique causés, se pose un problème psycho-socio-culturel en rapport avec la gestion des corps de personnes décédées de cette maladie; ce problème est susceptible d´entraver la bonne marche de la stratégie de riposte. Au Cameroun par exemple, la gestion actuelle de ces dépouilles ne fait pas l´unanimité. En effet, les restrictions appliquées à l´inhumation, bien que récemment assouplies proscrivent entre autres tout transfert interurbain des dépouilles. A la lumière des considérations culturelles africaines de la personne décédée, des dissensions créées entre les familles et le corps médical, de la législation et des données scientifiques disponibles, cet article analyse les risques et les bénéfices de l´inhumation des dépouilles par les familles. Il propose ensuite des solutions qui concilient la dignité (en laissant les familles enterrer leurs morts dans les domiciles), et la sécurité (en assurant une conservation hermétique et la surveillance d´un officier de police judiciaire). L´application de ces solutions pourraient améliorer la confiance de la population envers le système de santé et contribuer positivement aux stratégies de prévention, d´identification et de prise en charge des cas de COVID-19
