BACKGROUND@#The treatment of choice for pilomatricomas is surgical excision; however, data for the optimal timing of treatment and cosmetic outcomes are limited.@*OBJECTIVE@#This study aimed to investigate the optimal timing of treatment in pilomatricomas by considering clinicopathological findings and cosmetic outcomes.@*METHODS@#Seventy-three pilomatricomas patients aged ≤15 years were retrospectively reviewed. Patients were classified into early excision (disease duration ≤12 months, group A) and delayed excision groups (disease duration >12 months, group B). Tumor characteristics, and histopathological features with evolutionary stages were assessed. Cosmetic outcomes were evaluated by the Modified Vancouver Scar Scale (MVSS), 5-point patient satisfaction score, and complication rates.@*RESULTS@#Group A showed better cosmetic outcomes than group B in the MVSS (1.53±1.22 vs. 3.68±1.84), 5-point patient satisfaction score (4.08±0.89 vs. 3.18±1.01), and complication rates (11.8% vs. 36.4%), respectively (p<0.05). Secondary anetoderma, tent sign, calcification, and late regressive stage (evolutionary stage IV) were more common in group B, (p<0.05). Moreover, evolutionary stages showed a positive correlation with mean MVSS (r=0.670, p<0.05).@*CONCLUSION@#Early excision (disease duration ≤12 months) provides superior cosmetic outcomes compared to delayed procedures. Early recognition, diagnosis, and management for pediatric pilomatricomas is important to improve overall cosmetic outcomes.

Background@#A recent study suggested a possible role of skin barrier dysfunction in the pathogenesis of rosacea, which leads to irritation symptoms. Gamma linolenic acid (GLA) is an essential omega-6 fatty acid that is known to restore defective epidermal skin barrier. GLA supplementation has not previously been performed in rosacea patients. @*Objective@#To investigate the efficacy and safety of adding GLA to minocycline compared to minocycline alone in rosacea patients. @*Methods@#This prospective, double-blind, randomized, placebo-controlled trial enrolled 31 rosacea patients. They were randomly assigned to receive 320 mg/day of GLA (Evoprim®) (n=16) or placebo (n=15) in addition to 100 mg/day of minocycline for 8 weeks. Investigator's global assessment (IGA) and patient's global assessment (PGA) were used to assess clinical severity at weeks 0, 4, 8, and 12. Biophysical parameters including melanin index, erythema index, transepidermal water loss (TEWL), lipid concentration, and stratum corneum hydration were measured. @*Results@#In the GLA group, a higher proportion of patients achieved treatment success (IGA≤1) at week 8 (68.75% vs. 33.33%) and patient satisfaction (PGA≥3) at weeks 8 (75.0% vs. 40.0%) and 12 (81.3% vs. 46.6%). Both groups, throughout 12 weeks of treatment, revealed a trend toward improvement in erythema index, melanin index, TEWL, and stratum corneum hydration. Particularly, there was a significant difference in TEWL and stratum corneum hydration over time between the two groups (p=0.033, p=0.003, respectively). No serious adverse event was observed in both groups. @*Conclusion@#GLA is beneficial as an additional therapeutic option for rosacea patients treated with minocycline.

Background@#Palmoplantar pustulosis is a specific form of localized pustular psoriasis occurring on the palm and sole. Multiple therapeutic options, including topical and systemic agents as well as phototherapies, are available for palmoplantar pustulosis; however, treatment outcomes are not satisfactory in most cases. Recently, a gain-switched 311-nm Ti:Sapphire Laser was developed and showed good treatment response in vitiligo and atopic dermatitis. @*Objective@#To investigate the efficacy and safety of the 311-nm Ti:Sapphire Laser in the treatment of palmoplantar pustulosis. @*Methods@#A total of 24 patients with palmoplantar pustulosis were treated with a 311-nm Ti:Sapphire Laser twice a week for up to 32 sessions and had a 3-month follow-up visit. The treatment dose started at 300 mJ/cm 2 and was increased by 50 mJ/cm 2 at each subsequent session. The Palmoplantar Pustular Psoriasis Area and Severity Index score, 5-grade patient satisfaction score, and adverse events were evaluated. @*Results@#The mean Palmoplantar Pustular Psoriasis Area and Severity Index score decreased from 8.31±3.31 at baseline to 4.75±2.70 at 16 sessions, 3.26±2.18 at 32 sessions, and 4.05±2.19 at follow-up visit. In the subgroup analysis, non-smokers and emollients user groups showed better responses in Palmoplantar Pustular Psoriasis Area and Severity Index (p=0.033 and p=0.027, respectively). Adverse effects, including burning sensation and transient erythema, were limited and well-tolerated. @*Conclusion@#The 311-nm Ti:Sapphire Laser can be considered as a treatment option for palmoplantar pustulosis.Moreover, habitual risk factor modifications, such as smoking cessation and steady use of emollients, can impact treatment outcomes in patients with palmoplantar pustulosis.

BACKGROUND: UV radiation is the most important risk factor in the development of basal cell carcinoma (BCC). However, the association between tumorigenesis and the progression of BCC and UV exposure is complex, and the exact mechanisms remain to be elucidated. OBJECTIVE: We tested the hypothesis that UV radiation affects BCC progression by triggering the tumor cells to escape from immune surveillance and by altering the cell-to-cell adhesion molecules. METHODS: A retrospective study was conducted on 33 tissue samples of BCCs that underwent surgical resection between January 2010 to June 2018. We reviewed patient characteristics and evaluated Fas, Fas-ligand, CD8, PD-L1, E-cadherin, and P-cadherin expression in the tumor and peritumoral region by immmunohistochemical (IHC) staining. RESULTS: BCCs in sun-exposed areas tended to have a shorter disease duration, but more patients reported an increase in size. Tumors in sun-exposed areas showed significantly lower Fas expression (p=0.005) and significantly higher Fas-ligand expression (p=0.022) compared to those in sun-protected areas. Peritumoral CD8 expression of tumor infiltrating lymphocytes (TILs) was less prominent in sun-exposed areas (p=0.011). E-cadherin expression was reduced in sun-exposed areas compared with that in sun-protected areas (p=0.006). Meanwhile, P-cadherin expression was preserved in most of the tumors in both the sun-exposed (95.2%) and sun-protected areas (100%). CONCLUSION: This study showed that in BCCs, UV exposure induces Fas downregulation in the tumor, which helps the tumor to evade host immunity. Furthermore, UV exposure induces Fas-L upregulation in the tumor, which results in the apoptosis of CD8+ tumor-infiltrating lymphocytes. In addition, reduced expression of E-cadherin in sun-exposed areas indicates that UV exposure in BCCs leads to altered cell-to-cell adhesion, which allows further tumor motility.

To achieve early detection of HIV infection, physicians must be aware of symptoms related to HIV infection. Dermatologists, in particular, can play an important role, because more than 90% of patients infected with HIV will develop at least one type of dermatologic disorder, which may be the first indicator of the existence of infection. A 26-year-old male presented with a one-year history of intractable pruritus associated with multiple excoriations and small ulcers. Initially, his condition clinically resembled adult atopic dermatitis. However, no improvement was observed despite treatment including narrowband ultraviolet B (UVB), antihistamine, and topical and systemic corticosteroid. After a week, the patient developed seizures and was diagnosed with diffuse large B-cell lymphoma of the brain. HIV testing was performed, and the patient was revealed to be HIV-positive. He began highly active antiretroviral therapy (HAART), and the pruritus and skin condition improved dramatically. Herein, we report a case of HIV infection for which the presenting sign was intractable pruritus.

BACKGROUND: Recent studies have revealed that several factors including Demodex mites induce an excessive immune response and that the skin lesions seen in patients with rosacea may develop as a result of excessive inflammation. Therefore, acaricidal and anti-inflammatory treatments such as systemic metronidazole have been shown to be efficacious for rosacea. OBJECTIVE: This study aimed to evaluate the efficacy of systemic metronidazole in the treatment of rosacea. METHODS: A group of 39 patients diagnosed with rosacea who visited the Department of Dermatology of Ulsan University Hospital from April 2014 to July 2015 was investigated in this single-center retrospective study. Patients were treated with metronidazole 250 mg 3 times daily for 3 weeks. The response was evaluated using serial photographs of the patients. According to clinical improvement, patients were classified into an effective group and a failure group. RESULTS: After treatment, 29 patients (74.4%) responded to systemic metronidazole treatment. Fourteen patients (48.3%) in the effective group were well controlled without recurrence for 20.1 weeks. Patients in the effective group showed a higher detection rate of Demodex mites and proportion of the papulopustular rosacea subtype than patients in the failure group. CONCLUSION: The results of this study showed that systemic metronidazole was a tolerable and effective option for the treatment of patients with papulopustular rosacea with Demodex mites. Moreover, the early treatment of inflammatory lesions leads to an improved prognosis.

BACKGROUND: Vitamin D is known to play roles in immunity through the stimulation of Toll-like receptors, regulating pro-inflammatory cytokine production, and increasing antimicrobial peptide production. Recent studies have revealed the role of vitamin D deficiency in the immunopathogenesis of atopic dermatitis (AD). OBJECTIVE: We aimed to assess the correlation between vitamin D concentrations and the severity of AD and also elucidate the relationship between the amount of sunlight exposure and serum 25-Hydroxy vitamin (25(OH)D) levels using the sunlight exposure questionnaire. METHODS: Sixty-one Korean patients with AD and 120 healthy control patients were enrolled. We evaluated the disease severity using the SCORAD index and measured the serum levels of 25(OH)D, and total IgE levels and eosinophil counts. All patients completed a one-week recall sunlight exposure questionnaire. RESULTS: There was no significant difference in the 25(OH)D levels between the two groups. Eosinophil counts (p <0.001) were significantly higher in patients with AD than in control patients. A significant inverse correlation (r= -0.309, p=0.015) was found between the SCORAD score and 25(OH)D levels. A significant positive correlation (r=0.236, p=0.001) was found between the weekly sunlight exposure score and the serum levels of 25(OH)D. CONCLUSION: Our study suggests that vitamin D deficiency is related to the severity of AD and the serum 25(OH)D concentration is positively correlated with the sunlight exposure score.

BACKGROUND: Atopic dermatitis (AD) in school-aged children may affect their daily activities and psychological well-being. It can also have a negative impact on the child's behavior. Little is known about the quality of life (QOL) and psychiatric symptoms in school-aged children and their relationship to disease severity. OBJECTIVE: The purpose of this study was to document the impact of AD on QOL and psychiatric symptoms in school-aged children by disease severity. METHODS: Seventy-eight AD patients aged 7approximately19 years were included in this study. Participants completed the Children's Dermatology Life Quality Index (CDLQI), Kovacs' Children's Depression Inventory (CDI), and the Korean Attention-Deficit Hyperactivity Disorder Rating Scale (K-ARS). Disease severity was evaluated using the SCORAD (SCORing Atopic Dermatitis) index. RESULTS: The SCORAD was 28.95+/-17.92 (mean+/-SD), and the CDLQI was 8.04+/-6.29. The CDLQI was significantly correlated with the SCORAD. Psychiatric symptoms (CDI and K-ARS) did not show a correlation with the SCORAD, but they worsened with increasing CDLQI score. There was no statistically significant difference among the different age groups. CONCLUSION: We demonstrated that AD had an impact on school-aged children's QOL and psychological well-being. Severity of AD correlated with QOL decrements, indicating greater QOL impact with greater disease severity. Psychiatric symptoms were more related to subjective impairment of QOL than objective disease severity.

BACKGROUND: Dermoscopy has been suggested as a useful tool for diagnosing various skin diseases. Recently, the possibility of using dermoscopy to predict the response to treatment has emerged. OBJECTIVE: The purpose of this study was to determine whether dermoscopic findings corresponded to clinical acne scar types. This study also aimed to discover which dermoscopic findings predict the response to acne scar treatment. METHODS: The dermoscopic findings of 39 participants undergoing atrophic acne scar treatment with fractional photothermolysis were evaluated. Patients were divided into two groups according to the duration of acne scar persistence. RESULTS: Patients with a relatively short duration of acne scar persistence usually achieved better treatment outcomes. Dermoscopic findings showed no obvious differences according to clinical acne scar type. But high hair follicle density can be considered a predictive factor of treatment effects. CONCLUSION: Our study indicated that acne scar improvements can be predicted by dermoscopically observing hair follicle density.
