Objective To provide anatomical and clinic basis for endoscopic mono-endonasal transsphenoidal surgery in the treatment of patients with sellar lesions. Methods Forty patients with sellar lesions (38 with pituitary adenoma and 2 with Rathke's cysts), admitted to our hospital from June 2006 to October 2009, were chosen; their clinical data and treatments were retrospectively analyzed. Results In 38 patients with pituitary adenoma, total resection was achieved in 35 (92.1%) and subtotal resection was achieved in 3 (7.9%). All the 2 patients with Rathke's cysts were received total resection. Follow-up showed that 33 patients with pituitary adenoma was tumor-free and the tumor in the other 5 got controlled. Conclusion endoscopic mono-endonasal transsphenoidal surgery is safe and easy to perform with little trauma for the sellar diseases.
