Objective To understand the consumption of hand hygiene(HH)products and HH compliance in in-tensive care units(ICUs)of secondary and higher grade medical institutions(MIs)in Shanghai, and provide basis for further monitoring of HH among health care workers(HCWs).Methods Through healthcare-associated infec-tion surveillance system, the consumption of HH products and HH compliance in ICUs from secondary and higher grade MIs in Shanghai in 2017-2021 were analyzed. Results 105 ICUs from 74 MIs were included in analysis, the average consumption of HH products was 79.24(44.88-258.63)mL/(bed·day),with statistically significant difference among different types of ICUs(P<0.001).The average consumption of HH products increased from 65.75 mL/(bed·day)in 2017 to 87.55 mL/(bed·day)in 2021,showing an increasing trend year by year(P<0.001).HCWs'HH compliance rate was 82.13%,with the highest in nurses(86.59%)and the lowest(48.90%)in medical technicians,HH compliance rates of HCWs of different occupations were statistically significant different(P<0.001).Among the implementation modes of HH,39.86%used running water for hand washing,42.27%used alcohol-based hand rub to wipe hands,13.22%didn't take HH measures, and 4.65%didn't take HH mea-sures when wearing gloves, with statistically significant differences among different HH implementation modes of HCWs(P<0.001).There was a positive correlation between the average consumption of HH products per bed·day and HCWs'HH compliance rate(r=0.703,P<0.05).Conclusion The average consumption of HH products per bed·day and HH compliance rate of HCWs in ICUs in Shanghai presents an increasing trend year by year. There are differences in the average consumption of HH products per bed·day and HH compliance rate among different types of ICUs.The implementation of HH can be evaluated by continuously surveillance on the average consumption of HH products per bed·day.

Coronavirus is human-to-human transmissible and often leads to nosocomial outbreak in the early stage, which poses a great threat to healthcare workers.Due to confined space, crowed population and some aerosol-generating medical procedures, hospital is vulnerable to coronavirus transmission and nosocomial infection. We review the mechanism and risk factors of coronavirus aerosol in hospital and summarize the evidence of aerosol transmission of three coronaviruses and the impact of air flow.Furthermore, we evaluate the effectiveness of personal protection in the above-mentioned scenarios for the prevention and control of nosocomial infection.

Objective To understand the present status of special reports relating to microbiology at China provincial-level annual academic conference of healthcare-associated infection (HAI), and provide basis for the training and capacity-building of HAI management staff. Methods The conference arrangements for annual academic conference of provincial HAI quality control centers and HAI management societies from January 2015 to May 2017 were collected, and the special reports on microbiology were summarized and analyzed. Results A total of 35 annual academic conferences held by 17 provinces/municipalities directly under the central government/autonomous regions were included in the study, most conferences (42.31%) were concentrated on the fourth quarter. 15 annual academic conferences were related to microbiology report, accounting for 5.91% of the total reports, reporting time accounted for 4.81% of total time. There were various forms of microbiological thematic reports, subject reports, literature exchange, and interactive exchange accounted for 68.96%, 24.14%, and 6.90% respectively. The proportion of topics related to microbiology reports increased to a certain extent, but the proportion of reporting time decreased. Conclusion At present, the proportion and reporting time of microbiology reports in China provincial-level annual academic conference of HAI is relatively low, it is necessary to increase the number of microbiology reports in the future annual conference of HAI.

OBJECTIVETo establish a cell model with HBV secretion by plasmid transfection with whole HBV genes into hepatic L02 cells, and to analyze the effect of HBV on the expression of HLA-A, B, C and MICA/B in L02 cells.

METHODSThe mock control plasmid was built by digesting pEcob6 plasmid with EcoR I at the HBV DNA site and ligating the fragment without HBV DNA. The L02 cells were transfected with pEcob6 or mock plasmid and pcDNA3 1-neo by liposome. The expressions of HBsAg, HBcAg/HBeAg and HBV DNA were detected by immunofluorescence assay, Abbott enzymoimmunoassay, or FQ-PCR. The expressions of HLA-A, B, C and MICA/B were determined by FACS and the differences in the two molecules were analyzed.

RESULTSAfter the transfected cells were selected by G418, the HBsAg and HBeAg in the supernatant of L02-HBV cells were 24.78(S/N) and 4.117(S/N). The quantity of HBV DNA was 9.67 x 10(4) copies/ml, but in L02-mock and in L02 cells they were all negative. Under a confocal microscope HBsAg was brightly shown in the cytoplasm while HBcAg showed dimly in the cytoplasm or in the nuclei. By using FACS, the L02 and L02-mock cells showed some low expressions of HLA-A, B, C and a little expression of MICA/B, while the expression of the two molecules was higher in L02-HBV and the differences were significant (P < 0.05).

CONCLUSIONA cell model with the expression of HBV antigens and the secretion of HBV DNA was established by pEcob6 transfection into L02 cells, the transcription or replication of HBV gene might induce stronger expressions of HLA-A, B, C and MICA/B on hepatic cells. They might be related to the immune injuries of hepatic cells.
