Cold and heat belong to the eight-principal syndrome differentiation of traditional Chinese medicine, which can reflect the rise and fall of Yin and Yang in the body and the Yin and Yang nature of the disease. At present, traditional Chinese medicine has an inconsistent understanding of cold and heat in acute coronary syndrome. The emphasis on pathogenic factors of cold and heat is biased, and the elements of cold and heat syndrome are not fully reflected in the scale. Therefore, the literature has been reviewed from the perspectives of etiology, pathogenesis, symptom elements, and test signs with drugs. From the perspective of etiology, both cold evil and heat evil can increase the risk of acute coronary syndrome. It was previously believed that acute coronary syndrome occurs frequently in cold climates such as winter and spring. Based on this understanding, hot weather can also induce acute coronary syndrome, and different temperatures have different effects on patients of different ages and with different underlying diseases. In addition, artificial pathogenic factors such as excessive consumption of cold food and refrigeration air conditioners were added. From the perspective of pathogenesis, on the basis of the traditional ''asthenia in origin and asthenia in superficiality'' and ''phlegm stagnation'', it is found that Yin-cold and fire-heat can both cause paralysis of the heart chakra and pain induced by the blockage. The pathogenesis of acute coronary syndrome characterized by heat stagnation and coldness featuring heartburn should be distinguished from gastroesophageal reflux disease. Moreover, the pathogenesis of Yin cold coagulation and pulse stagnation and wind obstruction are different. The acute coronary syndrome is in line with the wind characteristics of frequent changes and can be treated with wind medicine. From the perspective of syndrome elements, the syndrome elements such as cold condensation, heat accumulation, and toxicity are analyzed, and the use of basic syndrome elements and their combination forms facilitates clinical and scientific research. In addition, according to the test sign with the drug, it can be seen that the attributes of cold and heat of traditional Chinese medicine prescriptions for acute coronary syndrome can be explained according to the temperature-sensitive transient receptor potential (TRP) ion channel, thus proving the pathogenesis of cold and heat of acute coronary syndrome.

Background It is unclear if there is any combined effect of air pollutants and non-optimal temperature on metabolic syndrome, or any molecular mechanisms of related signaling pathways in the process, which requires urgent systematic research. Objective To observe the effects of combined exposure to PM2.5 and non-optimal temperature on metabolic damage at gene and protein levels in mice, and elucidate the role of related signaling pathway in crucial organs. Methods A total of 60 six-week-old male C57BL/6J mice were randomly divided into six groups: a normal temperature-filter air group (TN-FA), a normal temperature-concentrated PM2.5 group (TN-PM), a heat-filter air group (TH-FA), a heat-concentrated PM2.5 group (TH-PM), a cold-filter air group (TC-FA), and a cold-concentrated PM2.5 group (TC-PM). The Shanghai Meteorological and Environmental Animal Exposure System (Shanghai-METAS) was used to provide combined exposure settings of air types [concentrated PM2.5 and filter air (FA)] and temperatures [normal (22°C), cold (4°C), and heat (30°C)] for 4 weeks. Skeletal muscle and white adipose tissue (WAT) of the mice were sampled at the end of exposure, and transcriptomics and Western blot (WB) assay were adopted to observe selected gene and protein expression levels in the samples respectively. Results The transcriptomics results indicated that the PM2.5 exposure enhanced the number of differentially expressed genes. Specifically, 4820 genes were differentially expressed in the TN-PM mice compared to the TN-FA mice at normal temperature, and 1143 genes were differentially expressed in the Tc-PM mice compared to the Tc-FA mice in the cold environment. The phosphatidylinositol 3-kinase/protein kinase B (PI3K/AKT) signaling pathway and the endoplasmic reticulum protein processing pathway were identified as the most significant pathways in metabolic injury resulting from combined exposure to PM2.5 and non-optimal temperature exposure. The WB results showed that exposure to PM2.5 in the normal temperature and the cold environments led to a significant increase in the expression of p-AKT in WAT (P<0.01, P<0.05) and a significant decrease in the expression of GLUT4 (P<0.05, P<0.01). In skeletal muscle, exposure to PM2.5 led to a significant decrease in GLUT4 (P<0.05) in all environments, with a consistent trend of change as observed in WAT. Conclusion Cold/heat exposure might promote PM2.5-induced metabolic disorder through suppression of the AKT/GLUT4 pathway, aggravating metabolic damage.

OBJECTIVE:Cold water immersion methods are not standardized in terms of operational indicators such as immersion temperature, duration and depth, leading to controversy over the efficacy of recovery from exercise fatigue in skeletal muscle. In this article,we analyze the effects of cold water immersion on muscle injury, muscle soreness and muscle strength recovery under different factors,in order to find the best immersion implementation plan, and thus provide evidence for the recovery of muscle fatigue. METHODS:A search of CNKI,WanFang Data,Web of Science, and PubMed databases was conducted for relevant literature published from January 1,2000 to August 15,2023.A total of 4 759 articles were initially retrieved, with 4 735 articles excluded through screening and 24 articles finally included. The Physical Therapy Evidence Database Scale was used to assess the methodological quality of the included literature, and Stata-MP 16 software was used to perform effect size combinations, subgroup analyses,Meta-regression,sensitivity tests, and publication bias analyses. RESULTS:(1)The article included a total of 24 randomized controlled trial studies, including 617 subjects, with overall high legal quality.(2)Meta-analysis showed that cold water immersion can significantly reduce creatine kinase blood value[standardized mean difference(SMD)=-0.17,95%confidence interval(CI):-0.29 to-0.05,P<0.01],alleviate muscle pain(SMD=-0.60,95%CI:-0.81 to-0.38,P<0.01),and promote maximum muscle strength recovery(SMD=0.17,95%CI:0.05 to 0.30,P<0.01).(3)Subgroup analysis showed that:The immersing regimen with water temperature>14 ℃(SMD=-0.48,95%CI:-0.76 to-0.20,P<0.01)and duration of 12-14 minutes(SMD=-0.38,95%CI:-0.61 to-0.15,P<0.01)had the best effect in reducing creatine kinase blood values, and had a more significant intervention effect on endurance exercise(SMD=-0.45,95%CI:-0.71 to-0.20,P<0.01),while the immersion regimen with water temperature<10 ℃(SMD=-0.61,95%CI:-0.79 to-0.43,P<0.01),duration<12 minutes(SMD=-0.76,95%CI:-0.98 to-0.53,P<0.01),and immersion depth above the iliac spine(SMD=-0.74,95%CI:-0.97 to-0.52,P<0.01)had the best effect on relieving muscle soreness, and had a more significant analgesic effect after endurance exercise(SMD=-0.42,95%CI:-0.61 to-0.22,P<0.01).(4)Meta regression showed that immersion water temperature, immersion duration, and exercise type were important regulatory factors affecting the effect size of creatine kinase;immersing water temperature and immersing depth were important regulatory factors affecting the effect size of visual analogue scale score, while exercise type was an important regulatory factor affecting the maximum isometric muscle strength effect size. CONCLUSION:(1)Evidence of extremely low to moderate strength suggests that cold water immersion can effectively reduce muscle damage, alleviate muscle soreness, and promote muscle strength recovery.(2)In terms of reducing muscle injury, immersion water temperature, immersion duration, and exercise type are significant regulatory factors that affect the efficacy of immersing.Among them, immersion water temperature>14 ℃ and duration of 12-14 minutes are the best solutions to reduce muscle injury after exercise, and the immersing effect is better for endurance exercise.(3)In terms of reducing muscle soreness, immersion water temperature and immersion depth are important regulatory factors that affect the intervention effect.Among them, immersion water temperature<10 ℃,duration<12 minutes, and immersing depth above the iliac spine are the best solutions to reduce muscle soreness, and have a better analgesic effect after endurance exercise.(4)In terms of promoting muscle strength recovery, exercise type is a key regulatory factor that affects the maximum isometric muscle strength effect.

The prescription of Xiangsu Powder (香苏散) reflects the exquisite balance in treating both the superficial and deep layers, harmonizing the body's vital energy and blood, and addressing the functions of the triple energizer simultaneously. Its application in treating cold epidemics has been a long time, yet its importance in modern disease treatment is often overlooked. Based on the treatment principles demonstrated by Xiangsu Powder in treating cold epidemics, it is proposed that in the treatment of modern epidemic diseases, the dual principles of cold and heat should not be neglected. When cold pathogens stagnate at the surface, they should be dispersed and detoxified, using methods that induce sweating to expel them from the surface. When cold and dampness coexist, efforts should be made to regulate the flow of vital energy, blood, and body fluids to restore the proper functioning of the three burners. When cold-induced pathogenic factors are severe and toxic, aromatic herbs should be utilized to dispel turbidity and avoid contamination. Throughout the entire course of treating epidemic diseases, attention should be paid to inducing sweating without damaging the body's vital energy, while also protecting the body's righteous qi, with the aim of providing refe-rence for the treatment of modern epidemic diseases.

Referring to ZHANG Xichun's experience in treating dysentery from cold-fire accumulation, the treatment of ulcerative colitis (UC) in this paper can be divided into three stages including cold-fire accumulation stage, excessive heat and putrid intestine stage, and healthy qi deficiency and pathogen lingering stage. For people with slippery and excess pulse in the cold-fire accumulation stage, Xiaochengqi Decoction (小承气汤) added with Baishao (Radix Paeoniae Alba) and Gancao (Radix et Rhizoma Glycyrrhizae) can be used for purgation, while those with deficient pulse, Huazhi Decoction (化滞汤) or Xieli Decoction (燮理汤) can be used. In the excessive heat and putrid intestine stage, Tongbian Baitouweng Decoction (通变白头翁汤) and Jiedu Shenghua Elixir (解毒生化丹) are suggested. In the healthy qi deficiency and pathogen lingering stage, it is advised to use Jiedu Shenghua Elixir added with Shanyao (Rhizoma Dioscoreae), and Sanbao Porridge (三宝粥). Additionally, the medication rules, dosage and administration characteristics of Huanglian (Rhizoma Coptidis)-Rougui (Cortex Cinnamomi), Yadanzi (Fructus Bruceae), Diyu (Radix Sanguisorbae), Shanyao and Liuhuang (Sulphur) by ZHANG Xichun have been summarized with the help of modern pharmacological research, so as to provide new ideas for the treatment of UC by TCM.

“Deficiency cause, cold accumulation, and qi stagnation” originates from Essentials from the Golden Cabinet (《金匮要略》), which is a guiding principle for the pathogenesis of women's diseases, pioneering the differentiation and treatment of women's diseases based on patterns, and having a profound influence on future generations. Following the classical principles and simplifying the complexities, this paper explored the pathogenesis and mechanism of polycystic ovary syndrome (PCOS) from the perspective of “deficiency cause, cold accumulation, and qi stagnation”, and believed that depletion of essence and blood, long-term accumulation of internal cold, and qi constraint and blood stasis are the causes of PCOS, with depletion of essence and blood, and lack of nourishment of zang-fu (脏腑) organs as the root, and cold pathogen invasion, qi constraint and blood stasis as the branch. The main treatment principle is “treating deficiency with supplementation”, and dispelling pathogen while reinforcing healthy qi, along with “treatment of cold by warming” and “treatment of stagnation by dispersing”. This is of great significance for the treatment of polycystic ovary syndrome. Clinically, these methods can be used flexibly to guide treatment and formula selection for PCOS, with the goal of harmonizing qi and blood and regulating menstruation.

Objective@#To compare the effects of 10-methacryloyloxydecyl dihydrogen phosphate (10-MDP)- and silane-based primers with those of MDP-based primers on zirconia-resin bonding via multiple levels of aging.@*Methods@#Zirconia blocks were divided into 4 groups (n = 21) according to the primer used: MDP-based Z-Prime Plus (ZP), silane-based Monobond-S (MS), MDP- and silane-based Clearfil Ceramic Primer (CCP) and no primer (Blank). After pretreatment with or without the primers followed by bonding with cement Duo-Link, each group was subdivided into 3 subgroups (n = 7) according to aging level: 24 hours of water storage at 37 ℃ (24 h), 30 days of water storage at 37 ℃(30 d), and 30 d plus 3 000 thermal cycles (30 d/TC). After aging, shear bond strength (SBS) tests and failure mode analyses were conducted@*Results@#ZP, MS and CCP groups had greater SBSs than did the BLANK group (P<0.01). From 24 h to 30 d, the shear bond strength significantly increased (P<0.05); however, the shear bond strength decreased significantly from 30 d to 30 d/TC (P<0.01) and fell below baseline (30 d/TC vs. 24 h, P<0.01). Within the primer groups, CCP exhibited a higher SBS than ZP and MS at each aging level (P<0.001). The bonding strength of ZP was greater than that of MS at 30 d (P = 0.029) but lower than that of MS at 30 d/TC (P = 0.037). From 30 d to 30 d/TC, the percent decrease in the bonding strength of ZP was significantly greater than that of MS (82.43% vs. 64.90%).@*Conclusion@#MDP-based primers function better for zirconia-resin bonding when they contain silane coupling agents.

Background Cold environments reduce work efficiency and increase the risk of frostbite. Objective To investigate the effects on cold protection and manual performance of rescue gloves in cold environments, and to preliminarily verify the feasibility of related quantitative characterization methods. Methods An experiment was conducted in a large and a small artificial climate chamber. 12 male right-handed university students were recruited. The experiment was divided into two parts, with 0-10 min as the baseline phase and 10-50 min as the test phase. Subjects sat quietly in a 16 ℃ large chamber, and hands were exposed to a small chamber at 0 ℃ either barely (control group) or with rescue gloves (experimental group). Skin temperatures of the five fingers (thumb, index finger, middle finger, ring finger, and little finger), as well as the dorsal and palm of the hands were measured. In addition, grip strength, manual dexterity, maximum finger flexion, and tactile sensitivity of the hands were measured as the indicators of manual performance. Results In the control group, cold-induced vasodilation occurred when the skin temperature of the five fingers were lowered to about 10 ℃. The maximum rate of change in skin temperature of the control group was in the following order: thumb>index finger>middle finger>little finger>ring finger>dorsal>palm, 165.7 ℃·h−1 in the thumb, 80.0 and 89.9 ℃·h−1 in the palm and dorsal respectively. In addition, the minimum skin temperature of the thumb was 6.6 ℃. Compared with the control group: the rate of change of skin temperature in the experimentalgroup decreased by about 50%; the grip strength decreased by 15% after wearing the rescue gloves (P<0.05); no differences in manual dexterity, maximum finger flexion, and tactile sensitivity were statistically significant (P>0.05). Conclusion This study proved that the rescue gloves improved the thermal comfort of hands while better balancing the effect of cold exposure on manual performance. The quantitative characterization methods can evaluate the protective performance and manual performance of the rescue gloves, which provides a reliable theoretical foundation and reference basis for further experiments.

ObjectiveTo evaluate the clinical efficacy of modified Banxia Xiexintang combined with vedolizumab (VDZ) in the treatment of active moderate to severe Crohn's disease (CD) with the syndrome of cold and heat in complexity and the effect of the therapy on intestinal flora. MethodEighty patients were randomized based on the random number table method into a control group (40 cases) and an observation group (40 cases). The control group was treated with VDZ, and the observation group was treated with modified Banxia Xiexintang (1 bag per day) combined with VDZ. The treatment in both groups lasted for 14 weeks and the follow-up lasted until the 52th weeks. The CD activity index (CDAI), CD simplified endoscopic score (SES-CD), inflammatory bowel disease questionnaire (IBDQ) score, and syndrome score of cold and heat in complexity were assessed before treatment, after treatment, and at the end of follow-up. The levels of hemoglobin (HGB), hematocrit (HCT), albumin (ALB), C-reactive protein (CRP), interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α), interleukin-17 (IL-17), and fecal calprotectin (FC) were measured before and after treatment. Intestinal flora was examined before and after treatment. The safety of the therapy was evaluated. ResultCompared with those before treatment, the scores of CDAI, SES-CD, and the syndrome of cold and heat in complexity decreased (P<0.05) and the IBDQ score increased after treatment (P<0.05). Compared with those before treatment, the scores of CDAI, SES-CD, and the syndrome of cold and heat in complexity increased (P<0.05) and the IBDQ score decreased (P<0.05) at the end of follow-up. After treatment and at the end of follow-up, the observation group had lower scores of CDAI, SES-CD, and syndrome of cold and heat (P<0.05) and higher IBDQ score (P<0.05) than the control group. Moreover, the observation group had higher clinical remission rate(χ2=4.381,3.962) and response rate(χ2=5.541,4.306) and lower non-response rate(χ2=6.646,4.306) than the control group at the two time points (P<0.05). The endoscopic remission rate(χ2=4.072,3.985) and response rate(χ2=4.528,5.161) in the observation group were higher than those in the control group (P<0.05). After treatment, the HGB, HCT, and ALB levels in both groups elevated, and the observation group had higher levels than the control group (P<0.05). The treatment in both groups lowered the levels of CRP, IL-6, TNF-α, IL-17, and FC (P<0.05), and the observation group had lower levels of CRP, IL-6, TNF-α, IL-17, and FC than the control group after treatment (P<0.05). The relative abundance of Bifidobacterium, Lactobacillus, and Prevotella increased (P<0.05), while that of Proteus, Klebsiella, and Enterococcus decreased (P<0.05) in the two groups after treatment. Moreover, the changes in the relative abundance of these bacteria in the observation group were more obvious than those in the control group (P<0.05). No adverse reactions related to the modified Modified Banxia Xiexintang were observed during the study period. ConclusionModified Banxia Xiexintang combined with VDZ can play a synergistic role and has good short-term and long-term efficacy. This therapy can improve the nutritional status, regulate intestinal flora, and reduce inflammatory injury in the treatment of moderate to severe active CD patients with the syndrome of cold and heat in complexity without causing severe adverse reactions.

Organ preservation fluid could mitigate cold ischemia injury and maintain normal function of the grafts. At present, how to reduce a series of injury caused by cold ischemia of donor liver and improve the preservation quality of grafts are the hot and challenging spots in this field. Currently, preservation fluid in clinical practice has not achieved ideal preservation effect, especially for the protection of marginal donor organs. In the context of severe donor shortage, the key solution is still to explore the optimal preservation protocol for donor liver to prevent grafts from cold ischemia injury. In this article, the mechanism of donor liver injury during cold ischemia, the classification and evolution of donor liver preservation fluid were summarized, the development direction and challenges of donor liver preservation fluid were discussed, aiming to provide novel ideas and references for the research and development of donor liver preservation fluid.

Kampo medicine has been used for patients with pneumophagia to improve deficiency and stagnation of qi. Here, we report a case in which symptoms were alleviated by treating spleen yang deficiency and liver qi stagnation as the main pathogenesis. The patient was a 75-year-old woman. She was diagnosed with atrophic gastritis after a close examination of her gastric discomfort. She took antacids and proton pump inhibitors, but the condition did not improve. Her abdominal distension and belching also appeared and worsened, so she requested Kampo treatment. Since she had interior cold with qi stagnation, we prescribed tokito that is able not only to warm interior cold but also to adjust qi sufficiently. The pathology of pneumophagia is generally considered to be stagnation of qi mainly and concomitant with deficiency of qi. However, for those having interior cold like this case, a medicine that can firmly perform warm supplementation is necessary, and tokito is considered to be a candidate for the condition. Strangled pain is mentioned as the main targeted symptom of tokito, and it is often administered for abdominal or chest pain. Although, in this case, abdominal pain might have occurred if the condition had worsened further, it is important to utilize tokito in the case of qi stagnation and yang deficiency regardless whether patients have pain or not.

Orthostatic dysregulation often occurs during adolescence and presents with symptoms such as dizziness, difficulty waking up in the morning, nausea, anorexia, general malaise, and headache. Herein we describe a case of delayed sleep-phase syndrome in a 16-year-old female who had been treated with Kampo medicine since the age of 14 for orthostatic dysregulation. After taking renjuin, her condition improved and she was able to attend school. However, two winters later, her symptoms flared up and she was hospitalized. Electro-acupuncture test revealed her severe interior cold, and a simple sleep analysis showed that she was not getting deep sleep. We hypothesized that her interior cold contributed to her sleep disorder. After taking sekiganryo for the purpose of warming therapy, her waking time and sleep depth improved. This case showed that interior cold can cause sleep disorders and that warming therapy with sekiganryo may contribute to the improvement of sleep disorders.

This study investigated the physiological responses and effects of exercise training under hypoxic conditions at the skeletal muscle level induced by reducing muscle temperature in cold water environment. Participants were divided into two intervention groups, cooling and control conditions, according to the water temperature of 15°C and 33°C where the training were conducted in. Eight participants in each group performed submaximal cycling exercise in the water for 30 minutes at the lactate threshold (LT) intensity, three times a week for four weeks (12 sessions). LT intensity was assessed at pre- and post-intervention in a 33°C water temperature environment. A progressive load cycling test was performed on land to assess maximal oxygen uptake (VO2max) and Wingate test was conducted to measure anaerobic power. In the cooling group, working muscle deoxygenation increased during submaximal and maximal exercise, suggesting an improvement in the muscle oxygen extraction capacity. However, no effects on aerobic capacity such as VO2max or LT intensity were observed. The improvement in mean power and time to peak during the Wingate test in the cooling group indicated that LT intensity exercise training in a cold water environment would increase anaerobic power.
