During the past two decades, the videolaryngoscope (VDL) has become a valuable and effective tool for the management of the airway, not just in the realm of anesthesiology, but also in other medical specialties in clinical scenarios requiring tracheal intubation. In countries such as the United States, this represents over 15 million cases in the operating room and 650,000 outside the OR. The overall accumulated incidence of difficult airway is 6.8% events in routine practice and between 0.1 and 0.3 % of failed intubations, both associated with complications such as desaturation, airway injury, hemodynamic instability and death. Notwithstanding the fact that the VDL has proven advantages such as improved visualization of the glottis, higher first attempt success rates, and a shortened learning curve, most of the time its use is limited to rescue attempts or as a secondary option. The aim of this article is to comment the advantages and limitations of the VDL vs. the direct laryngoscope in a wide range of clinical settings, including the operating room, intensive care units, emergency departments, pediatrics, obstetrics, and Covid-19 to consider its routine use.


En las últimas dos décadas, el videolaringoscopio (VDL) se ha convertido en una herramienta valiosa y eficaz para el manejo de la vía aérea no solo en el ámbito de anestesiología, sino en otras especialidades médicas durante escenarios clínicos que requieren la intubación traqueal y las cuales, en países como Estados Unidos corresponden anualmente a más de 15 millones dentro de salas de cirugía y 650.000 fuera de ella. Aproximadamente, hay una incidencia global acumulada de 6,8 % de eventos de vía aérea difícil en la práctica rutinaria y 0,1 al 0,3 % de intubaciones fallidas, ambas asociadas a complicaciones como desaturación, daño en la vía aérea, inestabilidad hemodinámica y muerte. Pese a que el VDL ha demostrado ventajas como mejoría de la visualización de la glotis, aumento de tasa de éxito al primer intento y menor curva de aprendizaje, su uso en la mayoría de las veces se ve limitado como dispositivo de rescate o de manera secundaria. El propósito de este artículo es comentar acerca de las ventajas y limitaciones del VDL vs. el laringoscopio directo en un variado número de escenarios clínicos, como salas de cirugía, unidades de cuidado intensivo, emergenciología, pediatría, obstetricia y covid-19, con el fin de considerar si su uso debiera hacerse de manera rutinaria.

ABSTRACT This study aimed to verify the use of photobiomodulation in swallowing difficulties in individuals who had a severe form of COVID-19. This case report was based on a quasi-experimental, quantitative study, with primary data collected from a non-probabilistic sample of 13 adults (aged ≥ 19 to < 60 years) of both sexes, who had the severe form of COVID-19. Swallowing was assessed with the Dysphagia Risk Assessment Protocol, and the intervention used photobiomodulation. Descriptive statistics were used. It was found that all research participants' risk for dysphagia in water and pudding swallowing tests improved from before to after the intervention. It can be concluded that photobiomodulation had positive results in speech-language-hearing practice to treat swallowing difficulties in adults who were affected by the severe form of COVID-19, intubated, tracheostomized, and needed an alternative feeding route, as the swallowing difficulties improved.


RESUMO O objetivo foi verificar o uso da fotobiomodulação na dificuldade de deglutição em indivíduos que desenvolveram a forma grave da COVID-19. Trata-se de um relato de casos, a partir de um estudo quase experimental, quantitativo, com coleta de dados primários e com amostra não probabilística, realizado com 13 indivíduos adultos (idade ≥ 19 a < 60 anos) de ambos os sexos, que tiveram a forma grave da COVID-19. Foi realizada a avaliação da deglutição pelo Protocolo de Avaliação para o Risco de Disfagia (PARD) e a intervenção com fotobiomodulação. Foi utilizada a estatística descritiva. Verificou-se que todos os participantes da pesquisa apresentaram melhora no risco para disfagia no teste de deglutição de água e de alimento pastoso do momento pré para o pós-intervenção. Pode-se concluir que a utilização da fotobiomodulação na prática fonoaudiológica para tratar a dificuldade de deglutição dos adultos que foram acometidos pela forma grave da COVID-19, intubados, traqueostomizados e necessitaram fazer uso de via alternativa de alimentação, trouxe resultados positivos, com melhora da dificuldade de deglutição.

Resumen Objetivo. Esta investigación tiene como objetivo describir la población atendida y los beneficios del programa "Lineamiento de atención integral por Cuidados Paliativos en el Área de Salud de Palmares, para pacientes con SARS-CoV-2 severo" con ventilación mecánica asistida, hospitalizados en unidades de cuidados intensivos de la Caja Costarricense del Seguro Social durante la pandemia en Costa Rica, del 10 de junio al 31 de octubre del 2021. Métodos. Estudio descriptivo mixto de la población y los beneficios del programa por medio de indicadores cuantitativos obtenidos del informe de aplicación y cualitativos a través de la realización de una encuesta de satisfacción a usuarios en el período de estudio. Resultados. El programa atendió a 13 pacientes, brindó apoyo en manejo del duelo a los familiares de cinco pacientes que ya habían fallecido y a los cuidadores de ocho pacientes hospitalizados, también ofreció 70 consultas médicas en un período promedio de 62 días. La edad promedio de los pacientes fue de 55 años; doce pacientes presentaban algún factor de riesgo, siendo la obesidad y la falta de esquema de vacunación completo (dos dosis) los más frecuentes. Durante la implementación del programa fallecieron cinco pacientes más, obteniendo una tasa de mortalidad del 78%. A los pacientes que sobrevivieron se les atendió en promedio por 135 días. Con el apoyo del equipo, los pacientes lograron la recuperación clínica de las complicaciones respiratorias y funcionales. Lograron independencia total de actividades básicas de la vida diaria y reinserción social en menos de tres meses después del alta hospitalaria. En general, se documentó una percepción positiva del programa en el 100% de los usuarios, con cambios en el nivel de sufrimiento manifestado por las familias y el apoyo en el manejo del duelo. Conclusión. Con la optimización de uso de los recursos ya existentes en la Caja Costarricense del Seguro Social y la participación social, este programa abordó aspectos no atendidos previamente para los pacientes que sufrieron COVID 19 severo, como el manejo del sufrimiento y/o duelo durante la hospitalización y al egreso. Además, el programa colaboró en la recuperación clínica y funcional de los usuarios sobrevivientes y documentó múltiples beneficios percibidos por el paciente, sus familias, la institución y la sociedad, como el abordaje del sufrimiento, la adecuada atención al duelo, la organización de los recursos asistenciales y la pronta recuperación física, emocional y social del paciente sobreviviente.


Abstract Aim. This research aims to describe the population served and the benefits of the program "Guidelines for integral care for patients with severe SARS-CoV-2 by Palliative Care at the Area de Salud de Palmares " to patients with assisted mechanical ventilation, hospitalized in intensive care units of the Caja Costarricense del Seguro Social during the pandemic in Costa Rica, from June 10 to October 31, 2021. Methods. This study is a mixed descriptive investigation that analyzes quantitative indicators from the report on the program's application and qualitative indicators from a satisfaction survey for the users in the study period. Results. The program treated 13 patients, provided grief management support to the relatives of five patients who had already died and to the caregivers of eight hospitalized patients, and offered 70 medical consultations in an average period of 62 days. The average age of the patients was 55 years; twelve patients had some risk factors, and obesity and lack of a complete vaccination schedule (two doses) were the most frequent. During the program implementation, five more patients died, obtaining a mortality rate of 78%. The average treatment day for surviving patients was 135 days. Through the team's support, patients accomplished clinical recovery from respiratory and functional complications. They got independent basics activities of daily living and social reintegration in less tan three months after discharge. A positive perception of the program was documented in 100% of the program's users, with changes in the level of suffering manifested by the families and the support in grief management. Conclusion. With the optimization of the use of existing resources in the the Caja Costarricense del Seguro Social and the social participation, this program addressed aspects not previously attended to patients who suffered severe COVID 19, such as the management of suffering and/or grief during hospitalization and upon discharge. In addition, the program helped in the clinical and functional recovery of the surviving users, and documented multiple benefits perceived by the patient, their families, the institution, and society, such as the approach to suffering, adequate bereavement care, organization of care resources, and early physical, emotional and social recovery of the survivor patient.

Introducción: La enfermedad por SARS-CoV-2 se convirtió en un importante problema de salud pública. El 5 % de los pacientes requirió intubación endotraqueal por insuficiencia respiratoria aguda hipoxémica, y se generó un aumento de consulta por estenosis traqueal. Se realizó el trabajo con el objetivo de expresar los resultados de la cirugía de resección traqueal en pacientes con estenosis post COVID 19. Material y métodos: Estudio prospectivo, observacional, analítico. Se incluyeron 11 pacientes. La evaluación prequirúrgica y postquirúrgica fue la misma en todos los pacientes. Se valoró en el postquirúrgico la ventilación y la fonación hasta los 30 días. Se utilizó la clasificación de Clavien y Dindo para calificar a las complicaciones post-quirúrgicas, con seguimiento hasta los 60 días postquirúrgicos. Análisis estadístico: se aplicó prueba de Wilcoxon para comparar los resultados. Resultados: El 27.2 % de los pacientes tuvieron complicaciones postquirúrgicas. Fue estadísticamente significativa la comparación de la ventilación entre el prequirúrgico y el postquirúrgico (p<0.05) con mejoría en el postquirúrgico. Al comparar la fibrobroncoscopía prequirúrgica con la postquirúrgica (diámetro de la luz traqueal) también el resultado fue estadísticamente significativo (p <0.05). Conclusión: Los resultados obtenidos son similares a los expresados en la literatura. La cirugía de resección traqueal es un procedimiento seguro y efectivo y debe ser considerada como tratamiento de primera línea para la estenosis traqueal.


Introduction: The disease caused by SARS-CoV-2 has become a significant public health issue. 5 % of the patients required endotracheal intubation due to acute hypox emic respiratory failure, leading to an increased number of consultations for tracheal stenosis. This work was done with the aim of presenting the results of tracheal resection in patients with post-COVID-19 stenosis. Material and methods: Analytical, prospective, observational study. 11 patients were included. The preoperative and postoperative evaluation was the same for all patients. Post-surgical ventilation and phonation were assessed up to 30 days. The Clavien and Dindo classification was used to grade post-surgical complications, with follow-up extended up to 60 post-surgical days. Statistical analysis: the Wilcoxon test was used to compare the results. Results: 27.2 % of the patients had postoperative complications. The comparison of pre- and postoperative ventilation (p < 0.05) was statistically significant, with improve ment in the postoperative period. When comparing pre- and postoperative fiberoptic bronchoscopy (tracheal lumen diameter), the result was also statistically significant (p < 0.05). Conclusion: The results obtained are similar to those expressed in the literature. Tra cheal resection is a safe and effective procedure and should be considered as first-line treatment for tracheal stenosis.

Introducción: la intubación orotraqueal en pediatría aumenta la supervivencia en patologías graves; sin embargo, una consecuencia es el desarrollo de estenosis subglótica cuya población tiene más riesgo de desarrollarla por diferencias anatómicas con los adultos. La incidencia de estenosis subglótica posterior a intubación orotraqueal ha disminuido en el tiempo según lo reportado en la literatura. En este estudio se evaluó la prevalencia de la patología en nuestra población y su objetivo fue describir las características clínicas, demográficas y diagnósticas en población pediátrica, diagnosticados con estenosis subglótica en el Hospital San Vicente Fundación, posterior a intubación orotraqueal y evaluar la prevalencia del diagnóstico en el período mencionado. Materiales y métodos: se realizó un estudio retrospectivo mediante revisión de historias clínicas de pacientes con códigos CIE 10 de estenosis laríngea o subglótica secundaria a procedimientos y con código CUPS de intubación orotraqueal; se realiza un análisis descriptivo según la distribución de las variables. Resultados: la prevalencia de estenosis subglótica fue del 13 %. La mayoría fueron hombres, la causa principal de intubación fueron las infecciones respiratorias inferiores, principalmente bronquiolitis; las comorbilidades fueron prematuridad, trastorno de deglución y síndrome bronco-obstructivo. Hubo una mediana 19 días de intubación y las estenosis de bajo grado se diagnosticaron más frecuente. Conclusiones: la estenosis subglótica es una complicación de la intubación orotraqueal pediátrica, en este estudio con una prevalencia del 13 %. Los pacientes que desarrollan esta patología requieren la realización de uno o más procedimientos para obtener una vía aérea permeable con estancias hospitalarias prolongadas.


Introduction: Pediatric orotracheal intubation has increased survival rates in severe illnesses; however, one of the consequences maybe the development of subglottic stenosis. This population is at a higher risk of developing it due to anatomical diffe-rences compared to adults. The incidence of subglottic stenosis following orotracheal intubation has decreased over time as reported in the literature. This study aimed to assess the prevalence of this condition in our population and describe the clinical, demographic, and diagnostic characteristics in the pediatric population diagnosed with subglottic stenosis following orotracheal intubation at the San Vicente Fun-dación Hospital. Additionally, it aimed to evaluate the prevalence of this diagnosis during the mentioned period. Materials and Methods: A retrospective study was conducted by reviewing medical records of patients with ICD-10 codes for laryn-geal stenosis and for subglottic stenosis secondary to procedures. Also, the patients with the procedure code for orotracheal intubation were identified. A descriptive analysis was performed based on the distribution of variables. Results: The preva-lence of subglottic stenosis was 13%. There was a majority of male cases, and the primary cause of intubation was lower respiratory tract infection, mainly bronchio-litis. The most frequent comorbidities included prematurity, swallowing disorders, and broncho-obstructive syndrome. Median duration of intubation was 19 days, and lower-grade stenosis was more commonly encountered. Conclusions: Subglottic ste-nosis is a complication of pediatric orotracheal intubation, with a prevalence of 13% in this study. Patients who develop this condition require one or more procedures to establish a patent airway and often experience prolonged hospital stays.

ABSTRACT Objective: To identify risk factors for nonresponse to prone positioning in mechanically ventilated patients with COVID-19-associated severe acute respiratory distress syndrome and refractory hypoxemia in a tertiary care hospital in Colombia. Methods: Observational study based on a retrospective cohort of mechanically ventilated patients with severe acute respiratory distress syndrome due to SARS-CoV-2 who underwent prone positioning due to refractory hypoxemia. The study considered an improvement ≥ 20% in the PaO2/FiO2 ratio after the first cycle of 16 hours in the prone position to be a 'response'. Nonresponding patients were considered cases, and responding patients were controls. We controlled for clinical, laboratory, and radiological variables. Results: A total of 724 patients were included (58.67 ± 12.37 years, 67.7% males). Of those, 21.9% were nonresponders. Mortality was 54.1% for nonresponders and 31.3% for responders (p < 0.001). Variables associated with nonresponse were time from the start of mechanical ventilation to pronation (OR 1.23; 95%CI 1.10 - 1.41); preintubation PaO2/FiO2 ratio (OR 0.62; 95%CI 0.40 - 0.96); preprone PaO2/FiO2 ratio (OR 1.88. 95%CI 1.22 - 2.94); and radiologic multilobe consolidation (OR 2.12; 95%CI 1.33 - 3.33) or mixed pattern (OR 1.72; 95%CI 1.07 - 2.85) compared with a ground-glass pattern. Conclusion: This study identified factors associated with nonresponse to prone positioning in patients with refractory hypoxemia and acute respiratory distress syndrome due to SARS-CoV-2 receiving mechanical ventilation. Recognizing such factors helps identify candidates for other rescue strategies, including more extensive prone positioning or extracorporeal membrane oxygenation. Further studies are needed to assess the consistency of these findings in populations with acute respiratory distress syndrome of other etiologies.


RESUMO Objetivo: Identificar fatores de risco em pacientes submetidos à ventilação mecânica devido à síndrome do desconforto respiratório agudo grave associada à COVID-19 e hipoxemia refratária irresponsivos ao decúbito ventral em um hospital terciário na Colômbia. Métodos: Estudo observacional baseado em coorte retrospectiva de pacientes submetidos à ventilação mecânica devido à síndrome do desconforto respiratório agudo grave associada ao SARS-CoV-2 em decúbito ventral devido à hipoxemia refratária. O estudo considerou resposta a melhora ≥ 20% na relação entre pressão parcial de oxigênio e fração inspirada de oxigênio após o primeiro ciclo de 16 horas em decúbito ventral. Os pacientes irresponsivos foram considerados casos, e os responsivos foram considerados controles. Controlamos as variáveis clínicas, laboratoriais e radiológicas. Resultados: Foram incluídos 724 pacientes (58,67 ± 12,37 anos, 67,7% do sexo masculino). Destes, 21,9% eram pacientes irresponsivos. A mortalidade foi de 54,1% nos irresponsivos e de 31,3% nos responsivos (p < 0,001). As variáveis associadas à ausência de resposta foram tempo do início da ventilação mecânica até o decúbito ventral (RC de 1,23; IC95% 1,10 - 1,41); relação entre pressão parcial de oxigênio e fração inspirada de oxigênio pré-intubação (RC de 0,62; IC95% 0,40 - 0,96); relação entre pressão parcial de oxigênio e fração inspirada de oxigênio anterior ao decúbito ventral (RC de 1,88; IC95% 1,22 - 2,94); e consolidação radiológica de múltiplos lobos (RC de 2,12; IC95% 1,33 - 3,33) ou padrão misto (RC de 1,72; IC95% 1,07 - 2,85) em comparação com um padrão em vidro fosco. Conclusão: Este estudo identificou fatores associados à ausência de resposta ao decúbito ventral em pacientes com hipoxemia refratária e síndrome do desconforto respiratório agudo devido ao SARS-CoV-2 em ventilação mecânica. O reconhecimento desses fatores ajuda a identificar os candidatos a outras estratégias de resgate, incluindo decúbito ventral mais prolongado ou oxigenação por membrana extracorpórea. São necessários novos estudos para avaliar a consistência desses achados em populações com síndrome do desconforto respiratório agudo por causa de outras etiologias.

Introdução: Disfagia é um distúrbio de deglutição com sinais e sintomas específicos, caracterizada por alterações em qualquer fase ou entre as fases da dinâmica de deglutição, de origem congênita ou adquirida, podendo gerar prejuízo pulmonar, nutricional e social. É um transtorno frequentemente encontrado no centro de tratamento intensivo (CTI). Sendo assim, a identificação precoce dos principais agentes etiológicos para transtornos de deglutição é essencial para promover uma assistência fonoaudiológica mais adequada. Objetivo: Verificar a associação entre características epidemiológicas e clínicas com o desfecho contraindicação fonoaudiológica de alimentação por via oral em pacientes internados em um CTI. Métodos: Estudo transversal que avaliou pacientes internados no CTI submetidos a avaliação clínica da deglutição no período entre outubro de 2018 e maio de 2019. O nível 1 da Escala funcional de ingestão por via oral (FOIS) foi considerado de maior risco para aspiração respiratória e comparado com os níveis FOIS 2-7. Variáveis epidemiológicas e clínicas foram obtidas a partir dos registros dos pacientes. Análises univariadas e multivariadas foram realizadas para identificar associações e efeitos entre as variáveis e o desfecho contraindicação da alimentação por via oral. O nível de significância adotado foi de 5% e as análises foram realizadas no programa SPSS v.21.0. Resultados: Foram incluídos 128 pacientes (64,9% submetidos a intubação orotraqueal ­ IOT; idade de 60 ± 15,3 anos). Pacientes com FOIS 1 permaneceram mais dias em IOT, tiveram a internação no CTI prolongada e a cada dia de internação apresentaram risco de 5% de contraindicação da alimentação por via oral na avaliação fonoaudiológica. Conclusão: Foi evidenciada associação entre maior tempo de intubação orotraqueal, além de maior tempo de internação prévio, com a contraindicação da alimentação por via oral.


Introduction: Dysphagia is a swallowing disorder with specific signs and symptoms, characterized by alterations in any phase or between phases of swallowing dynamics, of congenital or acquired origin, which can lead to pulmonary, nutritional and social damage. It is a disorder often found in the intensive care unit (ICU). Therefore, the early identification of the main etiological agents for swallowing disorders is essential to promote more adequate speech therapy assistance. Objective: To verify the association between epidemiological and clinical characteristics with the outcome speech-language pathology contraindication for oral feeding in patients admitted to an ICU. Methods: Cross-sectional study that evaluated patients admitted to the ICU who underwent clinical evaluation of swallowing between October 2018 and May 2019. Level 1 of the Functional oral intake scale (FOIS) was considered at higher risk for respiratory aspiration and compared with FOIS levels 2-7. Epidemiological and clinical variables were obtained from patient records. Univariate and multivariate analyses were performed to identify associations and effects between variables and the outcome contraindication of oral feeding. The significance level adopted was 5% and the analyses were performed using the SPSS v.21.0 program. Results: 128 patients were included (64.9% undergoing orotracheal intubation - OTI; age of 60 ± 15.3 years). Patients with FOIS 1 spent more days on OTI, had a prolonged stay in the ICU and each day of hospitalization had a 5% risk of contraindication of oral feeding in the speech-language pathology assessment. Conclusion: There was an association between longer times of orotracheal intubation, in addition to longer previous hospitalization time, with the contraindication of oral feeding.


Introducción: La disfagia es un trastorno de la deglución con signos y síntomas específicos, caracterizado por alteraciones en cualquier fase o entre fases de la dinámica de la deglución, de origen congénito o adquirido, que pueden conducir a daño pulmonar, nutricional y social. Es un trastorno que se encuentra a menudo en la unidad de cuidados intensivos (UCI). Por lo tanto, la identificación temprana de los principales agentes etiológicos de los trastornos de la deglución es fundamental para promover una asistencia logopédica más adecuada. Objetivo: Verificar la asociación entre las características epidemiológicas y clínicas con el desenlace fonoaudiológico contraindicación para alimentación oral en pacientes internados en una UTI. Métodos: Estudio transversal que evaluó a pacientes ingresados en UCI a quienes se les realizó evaluación clínica de la deglución entre octubre de 2018 y mayo de 2019. Nivel 1 de la Escala de ingesta oral funcional (FOIS) fue considerado de mayor riesgo para aspiración respiratoria y comparado con los niveles de FOIS 2-7. Las variables epidemiológicas y clínicas se obtuvieron de las historias clínicas de los pacientes. Se realizaron análisis univariados y multivariados para identificar asociaciones y efectos entre las variables y el resultado contraindicación de la alimentación oral. El nivel de significación adoptado fue del 5% y los análisis se realizaron con el programa SPSS v.21.0. Resultados: se incluyeron 128 pacientes (64,9% sometidos a intubación orotraqueal - IOT; edad de 60 ± 15,3 años). Los pacientes con FOIS 1 pasaron más días en OTI, tuvieron una estancia prolongada en la UCI y cada día de hospitalización tenían un 5% de riesgo de contraindicación de alimentación oral en la evaluación de patología del habla y lenguaje. Conclusión: Hubo asociación entre mayor tiempo de intubación orotraqueal, además de mayor tiempo de hospitalización previa, con la contraindicación de alimentación oral.

Introducción: La espondilitis anquilosante es una artropatía que se encuentra dentro del grupo de artropatías seronegativas, siendo la entidad más común y representativa de ellas. Se caracteriza por inflamación crónica del esqueleto axial, con dolor de espalda de tipo inflamatorio y rigidez progresiva. Típicamente se manifiesta en pacientes jóvenes que inician su sintomatología entre los 20 y 30 años. La osificación progresiva en la espondilitis anquilosante envuelve los cartílagos articulares y discos de la columna vertebral, con el desarrollo de puentes óseos entre las vértebras. Esta fusión hace difícil, sino imposible, el abordaje axial para lograr una anestesia epidural o espinal, e incluso una dificultad aumentada del manejo de la vía aérea por la fusión de las vértebras cervicales. Caso clínico: En este reporte de caso se presenta una paciente femenina de 40 años, ya con diagnóstico de espondilitis anquilosante, que acude para una histerectomía total abdominal, que en primera instancia se intenta un abordaje neuro axial fallido y posteriormente se procede al uso de anestesia general con manejo de la vía aérea. Conclusión: La intubación en pacientes despiertos, es un tema ampliamente descrito en la literatura el cual se puede realizar de distintas maneras, en nuestro caso optamos por el uso de bloqueos de la vía aérea superior con posterior uso de fibroscopio para asegurar la intubación endotraqueal. Introduction: Ankylosing spondylitis is an arthropathy that falls within the group of seronegative arthropathies, being the most common and representative entity of them. It is characterized by chronic inflammation of the axial skeleton, with inflammatory back pain and progressive stiffness. It typically manifests in young patients who begin to show symptoms between 20 and 30 years of age. Progressive ossification in ankylosing spondylitis involves the articular cartilage and discs of the spine, with the development of bony bridges between the vertebrae. This fusion makes the axial approach difficult, if not impossible, to achieve epidural or spinal anesthesia, and even increases the difficulty of airway management due to the fusion of the cervical vertebrae. Case report: This case report presents a 40-year-old female patient, already diagnosed with ankylosing spondylitis, who underwent a total abdominal hysterectomy, where a failed neuraxial approach was first attempted and then general anesthesia with airway management was used. Conclusion: Intubation in awake patients is a subject widely described in the literature which can be performed in different ways, in our case we opted for the use of upper airway blocks with subsequent use of fiberscope to ensure endotracheal intubation. Abstract Introduction Endotracheal intubation is a procedure associated with a high level of exposure to the COVID-19 virus. This has led to the search of alternatives to reduce the risk of contamination, including the so-called aerosol box. Objective To compare time and difficulty of orotracheal intubation when using the aerosol box in a simulated setting. Methodology Observational study conducted with the participation of 33 anesthetist physicians and anesthesia residents; groups were compared in terms of time and intubation difficulty using a conventional Macintosh laryngoscope and the McGRATH™ MAC (Medtronic) videolaryngoscope with or without aerosol box. In order to determine performance with the intubation maneuver, crude hazard ratios were estimated, and a Cox multivariate regression model was built, adjusted by anesthetist years of experience and difficulties during the procedure. Results On average, the aerosol box increased intubation time by 7.57 seconds (SD 8.33) when the videolaryngoscope was used, and by 6.62 (SD 5.74) with the Macintosh. Overall, 132 intubations were performed, with 121 successful and 6 failed first-time attempts (4 with the use of the aerosol box); 16 participants (48.48%) reported difficulty handling the box. With the use of the Macintosh, intubation was found to be faster than with the videolaryngoscope (cHR: 1.36 [95% CI 0.64-2.88]; adjusted HR: 2.20 [95% CI 0.73-6.62]). Conclusions The use of the aerosol box and personal protective equipment in a simulation setting hinders the intubation maneuver and may result in protracted execution time.


Resumen Introducción La intubación endotraqueal es un procedimiento que se relaciona con alto nivel de exposición al virus de COVID-19, llevando a la búsqueda de alternativas para disminuir el riesgo de contaminación, entre ellas la llamada Caja de aerosoles. Objetivo Comparar el tiempo y dificultad en la intubación orotraqueal usando la caja de aerosoles en escenarios simulados. Metodología Estudio observacional, con participación de 33 médicos anestesiólogos y residentes de anestesia; se compararon los grupos en tiempo y dificultad de intubación con uso de laringoscopio convencional Macintosh y videolaringoscopio McGRATH™ MAC (Medtronic) utilizando la caja de aerosoles y sin ella. Para determinar el rendimiento en la maniobra de intubación se calcularon Hazard ratios crudos, se construyó modelo multivariado de Regresion de Cox ajustado por años de experiencia como anestesiólogo y dificultades durante el procedimiento. Resultados La caja de aerosoles aumentó en promedio el tiempo en segundos para la intubación con video-laringoscopio en 7,57 (DE: 8,33) y con Macintosh 6,62 (DE: 5,74). Se llevaron a cabo 132 intubaciones, 121 exitosas en el primer intento y 6 fallidas (4 con el uso de la caja de aerosoles). 16 participantes reconocieron alguna dificultad al manipular la caja (48,48 %). Con el uso de Macintosh se identificó tendencia a la intubación más rápido que con el videolaringoscopio (HRc: 1,36 [IC 95 %: 0,64-2,88]; HR ajustado: 2,20 [IC 95 %: 0,73-6,62]). Conclusiones Utilizar la caja de aerosoles y equipo de protección personal en un escenario simulado dificulta la maniobra de intubación y puede prolongar el tiempo de ejecución.

RESUMO Objetivo verificar os desfechos de deglutição e alimentação de pacientes pediátricos submetidos à intubação orotraqueal (IOT) prolongada, considerando aqueles que evoluíram para traqueostomia após. Métodos estudo de coorte retrospectivo, realizado por meio da análise de prontuários de pacientes admitidos em Unidade de Terapia Intensiva Pediátrica e acompanhados até a alta hospitalar, entre março de 2017 e dezembro de 2018. Resultados dos 51 pacientes incluídos, 64,7% eram do gênero masculino e a mediana de idade foi de 6,7 meses. Pacientes submetidos à IOT por mediana de sete dias apresentaram disfagia orofaríngea (DOF) leve e, quando submetidos a mais de 14 dias, apresentaram DOF moderada/grave, distúrbio alimentar pediátrico (DAP) com características de recusa alimentar e contraindicação de alimentação por via oral na alta hospitalar. Dentre os pacientes, 74,5% foram submetidos apenas à IOT e 25,5% evoluíram para traqueostomia, após. Pacientes traqueostomizados apresentaram maior ocorrência de alta hospitalar com DOF moderada/grave, DAP com características de recusa alimentar e uso de via alternativa de alimentação, em comparação a pacientes sem traqueostomia (p=0,001). Comparado ao diagnóstico inicial, pacientes não traqueostomizados tiveram diagnóstico final com graus mais leves de disfagia (p<0,001). Conclusão o tempo de IOT e a presença de traqueostomia são fatores associados ao diagnóstico fonoaudiológico de DOF moderada/grave, à presença de sinais de DAP com características de recusa alimentar e à necessidade de via alternativa de alimentação, persistentes até a alta hospitalar, sendo achados fonoaudiológicos frequentes entre os desfechos de deglutição/alimentação em pediatria.


ABSTRACT Purpose To verify the swallowing and feeding outcomes of pediatric patients undergoing prolonged OTI, considering those who progressed to tracheostomy afterward. Methods Retrospective cohort study, carried out by analyzing the medical records of patients admitted to the Pediatric ICU and followed up until hospital discharge, between 03/2017 and 12/2018. Results Of the 51 patients included, 64.7% were male and the median age 6.7 months. Patients undergoing OTI for a median of 7 days had mild dysphagia and when submitted for more than 14 days had moderate/severe dysphagia and PFD with characteristics of food refusal, with contraindication to oral feeding at hospital discharge. 74.5% of the patients underwent OTI only and 25.5% progressed to tracheostomy afterward. Tracheostomized patients had a higher occurrence of hospital discharge with moderate/ severe oropharyngeal dysphagia, pediatric feeding disorder (PFD) with characteristics of food refusal and alternative method of feeding compared to patients without tracheostomy (p=0.001). Non-tracheostomized patients had a final diagnosis with milder degrees of dysphagia when compared to the initial diagnosis (p<0.001). Conclusion The time of OTI and the presence of tracheostomy are factors associated with the speech-language pathology diagnosis of moderate/severe oropharyngeal dysphagia, presence of signs of PFD with characteristics of food refusal and the need for an alternative method of feeding that persists until hospital discharge, being frequent findings among the swallowing/feeding outcomes in pediatrics.

Estudios previos han caracterizado la disfagia en pacientes críticos hospitalizados que requieren intubación y ventilación mecánica invasiva. A raíz de la pandemia COVID-19 es necesario conocer las características deglutorias de pacientes diagnosticados con la enfermedad para su manejo. El objetivo de este estudio es analizar las características deglutorias de pacientes críticos extubados con y sin diagnóstico de COVID-19. Se llevó a cabo un estudio de cohorte retrospectivo con una muestra a conveniencia de 43 sujetos mayores de 15 años, ingresados al Hospital San Juan de Dios (Santiago, Chile) entre el 01 de junio y el 31 de agosto de 2020, intubados con o sin diagnóstico de COVID-19. Del total de sujetos, 22 padecieron de COVID-19 quienes estuvieron significativamente más días intubados que aquellos sin la patología (p=0,002). Inmediatamente posterior a la extubación orotraqueal, más del 90% de la muestra presentó disfagia. No hubo diferencia significativa en el nivel FILS ni asociación significativa en el grado de severidad de la disfagia entre sujetos con y sin COVID-19. Tampoco hubo diferencia significativa en el nivel FILS entre los grupos a los 10 días post-extubación. El nivel FILS a los 10 días aumentó significativamente en aquellos sujetos con COVID-19 (p=0,016) y sin COVID-19 (p=0,004). En la muestra, el nivel FILS y grado de severidad de la disfagia de los pacientes con y sin COVID-19 no mostraron diferencias estadísticas, siendo alto el porcentaje de disfagia en ambos grupos, lo que se podría asociar a la intubación orotraqueal y al tubo orotraqueal. Es necesaria la incorporación del fonoaudiólogo dentro de los equipos de Unidades de Pacientes Críticos para el manejo de los pacientes con COVID-19 y disfagia. Además, se recomienda continuar con más estudios en el área.


Previous research have described the deglutition disorders in critical hospitalised patients who required intubation and mechanical ventilation. In the pandemic context, it is mandatory to study both level and grade of dysphagia in patients suffering from COVID-19. The aim of this study is to analyse the deglutition features of extubated critical patients with and without COVID-19. A retrospective cohort study was performed, considering a convenience sample of 43 patients from 15 years old hospitalised at 'Hospital San Juan de Dios' between June 1st and August 31th 2020, who were intubated, with and without the diagnosis of COVID-19. 22of out 43 patients were diagnosed with COVID-19 who were intubated for significantly more days in comparison with those without COVID-19 (p=.002). After the intratracheal extubation, 90% of the sample was diagnosed with dysphagia. There was no significant difference in the FILS score nor significant association in dysphagia severity between patients with and without COVID-19. After 10 days post extubation, there was no significant difference in the FILS score between both groups. The FILS score increased significantly in the COVID-19 (p=.016) and non-COVID-19 (p=.004) patients after 10 days post extubation. Post extubation, there are no statistical differences in the FILS score and dysphagia severity in critical ill patients with and without COVID-19, with a high percentage of dysphagia in both groups which could be associated with intratracheal intubation and endotracheal tubes. The incorporation of speech and language therapists in Critical Care Units is mandatory. Furthermore, it is recommended to perform extra research in the area.

Introducción. La causa más común de estenosis laringotraqueal benigna es iatrogénica, secundaria a intubación orotraqueal. El manejo quirúrgico sigue siendo la alternativa que ha mostrado mejores resultados a largo plazo. El objetivo de este estudio fue analizar la experiencia en el manejo quirúrgico de la estenosis laringotraqueal durante 15 años en un hospital de alta complejidad en Colombia.Métodos. En este estudio se revisaron las historias clínicas de todos los pacientes tratados con reconstrucción de estenosis traqueal benigna, entre los años 2005 y 2020. Para el análisis estadístico se usaron métodos de estadística descriptiva, con análisis de frecuencias y medidas de tendencia central o de dispersión.Resultados. Se identificaron 38 pacientes con estenosis laringotraqueal, con un grado variable de estenosis. La nasofibrolaringoscopia fue bien tolerada y segura para determinar el grado y la longitud de la estenosis. El uso de tomografía sirvió para determinar la extensión y las características anatómicas. Los resultados del presente estudio son similares a los de la literatura en cuanto a complicaciones, mortalidad y falla de la anastomosis. Las complicaciones más frecuentemente reportadas son reestenosis, infección del sitio operatorio, lesión nerviosa y fístula, que en general se presentan en un tercio de los pacientes. Conclusiones. La experiencia de manejo quirúrgico de estenosis laringotraqueal en este hospital permite concluir que la reconstrucción traqueal es una opción segura en nuestro medio. Las tasas de éxito y de falla del tratamiento son equiparables a las reportadas en la literatura.


Introduction. The most common cause of benign laryngotracheal stenosis is iatrogenic, secondary to orotracheal intubation. Surgical management continues to be the alternative that has shown better long-term results. The objective of this study was to analyze the experience in the surgical management of laryngotracheal stenosis for 15 years in a high-level complexity hospital in Colombia.Methods. Medical records of patients treated with reconstruction of benign tracheal stenosis between 2005 and 2020 were reviewed. Descriptive statistical methods were used, with frequency analysis and measures of central tendency or dispersion.Results. Thirthy-eight patients with laryngotracheal stenosis were identified, with a variable degree of stenosis. Nasofibrolaryngoscopy was well tolerated and safe to determine the degree and length of the stenosis. The use of tomography served to determine the extension and anatomical characteristics. The results of the present study are similar to those of the literature in terms of complications, mortality and failure of the anastomosis. The most frequently reported complications are restenosis, surgical site infection, nerve injury, and fistula, which generally occur in one third of patients.Conclusions. The experience of surgical management of laryngotracheal stenosis in our hospital allows us to conclude that tracheal reconstruction is a safe option in our environment. Treatment success and failure rates are comparable to those reported in the literature.

Introdução: As alterações da deglutição pós-extubação são amplamente identificadas e estudadas, nas quais a disfagia é identificada nas fases oral e faríngea, acompanhada de penetração laríngea e aspiração traqueal. Entretanto, as alterações miofuncionais orofaciais em pacientes pós-extubação ainda não estão bem descritas em Pediatria. Objetivo: Verificar a influência da intubação orotraqueal (IOT) nas alterações miofuncionais orofaciais do lactente. Método: Estudo transversal, realizado em uma Unidade de Terapia Intensiva Pediátrica no período entre novembro de 2015 a setembro de 2016. Os participantes foram divididos em dois grupos: grupo estudo, com diagnóstico médico de cardiopatia congênita, pós-operatório de cirurgia cardíaca, em IOT por no mínimo 6 horas, e grupo controle composto por lactentes de 0 a 6 meses, previamente saudáveis, que não tiveram histórico de qualquer IOT anterior. Após a seleção, ambos os grupos foram submetidos ao Protocolo de Avaliação de Disfagia Pediátrica. Resultados: Postura e tônus labial, postura de língua, palato e qualidade vocal estiveram significativamente associados ao uso de IOT, pressão intraoral e padrão de sucção. Ao analisar a distribuição de frequência da comparação com e sem IOT, observa-se que não há um padrão específico que indique se a associação é prejudicial, protetora ou não interfere no padrão das características miofuncionais nesses casos. Conclusão: A influência da IOT foi encontrada nas estruturas miofuncionais orofaciais de bebês, quando comparados a bebês não intubados.


Introduction: Post-extubation swallowing changes are widely identified and studied, in which dysphagia is identified in the oral and pharyngeal phases, accompanied by laryngeal penetration and aspiration. However, orofacial myofunctional changes in post-extubation patients are still not well described in pediatrics. Objective: Verify the influence of orotracheal intubation on orofacial myofunctional changes in lactates. Methods: Cross-sectional study, performed in a Pediatric Intensive Care Unit from November 2015 to September 2016. Participants were divided into two groups: study group, with medical diagnosis of congenital heart disease, post-cardiac surgery, undergoing OTI for at least 6 hours, and the control group was composed of infants aged 0 to 6 months, previously healthy, who did not have any previous IOT. After selection, babies from both groups were submitted to the same assessment protocols. The clinical evaluation of the child's orofacial myofunctional structures was performed using the Pediatric Dysphagia Assessment Protocol. Results: Lip posture, lip tone, tongue posture, palate, and vocal quality were significantly associated with the use of OIT, intraoral pressure and suction pattern. When analyzing the frequency distribution of the comparison with and without OIT, it is observed that there is no specific pattern that indicates whether the association is harmful, protective or does not interfere in the pattern of myofunctional characteristics in these cases. Conclusion: OIT influence was found in orofacial myofunctional structures in infants, when compared to babies who were not intubated.


Introducción: Los cambios en la deglución post-extubación están ampliamente identificados y estudiados, en los que se identifica disfagia en las fases oral y faríngea, acompañada de penetración y aspiración laríngea. Sin embargo, los cambios miofuncionales orofaciales en pacientes post-extubación todavía no están bien descritos en pediatría. Objetivo: Verificar la influencia de la intubación orotraqueal sobre los cambios miofuncionales orofaciales en lactatos. Metodos: estudio transversal, realizado en una Unidad de Cuidados Intensivos Pediátricos desde noviembre de 2015 hasta septiembre de 2016. Los participantes se dividieron en dos grupos: grupo de estudio, con diagnóstico médico de cardiopatía congénita, postoperatorio cardíaco, sometidos a IOT durante al menos 6 horas y el grupo control, compuesto por lactantes de 0 a 6 meses, previamente sanos, que no tiene alguna IOT anterior. Después de la selección, los bebés de ambos grupos fueron sometidos a los mismos protocolos de evaluación. La evaluación clínica de las estructuras miofuncionales orofaciales del niño se realizó mediante el Protocolo de Evaluación de Disfagia Pediátrica. Resultados: La postura de los labios, el tono de los labios, la postura de la lengua, el paladar, la calidad vocal se asociaron significativamente con el uso de ITO, la presión intraoral y el patrón de succión. Al analizar la distribución de frecuencias de la comparación con y sin ITO, se observa que no existe un patrón específico que indique si la asociación es dañina, protectora o no interfiere en el patrón de características miofuncionales en estos casos. Conclusión: la influencia de la ITO se encontró en las estructuras miofuncionales orofaciales en los bebés, en comparación con los bebés que no fueron intubados.

Abstract Patients with hemodynamic instability have a sustained systolic blood pressure less or equal to 90 mmHg, a heart rate greater or equal to 120 beats per minute and an acute compromise of the ventilation/oxygenation ratio and/or an altered state of consciousness upon admission. These patients have higher mortality rates due to massive hemorrhage, airway injury and/or impaired ventilation. Damage control resuscitation is a systematic approach that aims to limit physiologic deterioration through strategies that address the physiologic debt of trauma. This article aims to describe the experience earned by the Trauma and Emergency Surgery Group (CTE) of Cali, Colombia in the management of the severely injured trauma patient in the emergency department following the basic principles of damage control surgery. Since bleeding is the main cause of death, the management of the severely injured trauma patient in the emergency department requires a multidisciplinary team that performs damage control maneuvers aimed at rapidly controlling bleeding, hemostatic resuscitation, and/or prompt transfer to the operating room, if required.


Resumen Un paciente politraumatizado hemodinámicamente inestable es aquel que ingresa al servicio de urgencias con una presión arterial sistólica menor o igual de 90 mmHg, una frecuencia cardiaca mayor o igual a 120 latidos por minuto y un compromiso agudo de la relación ventilación/oxigenación y/o del estado de conciencia. Por esta razón, existe una alta mortalidad dentro de las primeras horas de un trauma severo ya sea por una hemorragia masiva, una lesión de la vía aérea y/o una alteración de la ventilación. Siendo el objetivo de este artículo describir el manejo en urgencias del paciente politraumatizado hemodinámicamente inestable de acuerdo con los principios de control de daños. El manejo del paciente politraumatizado es una estrategia dinámica de alto impacto que requiere de un equipo multidisciplinario de experiencia. El cual debe de evolucionar conjunto a las nuevas herramientas de diagnóstico y tratamiento endovascular que buscan ser un puente para lograr una menor repercusión hemodinámica en el paciente y una más rápida y efectiva estabilización con mayores tasas de sobrevida.

Se realizó una investigación experimental tipo ensayo clínico controlado simple ciego con el fin de evaluar la relajación muscular y los predictores de vía aérea difícil en pacientes programados para cirugía general en el Hospital Central Universitario Dr. Antonio María Pineda. La muestra estuvo conformada por 100 pacientes distribuidos aleatoriamente en cuatro grupos de 25 pacientes cada uno. En los grupos Experimental-1 (E-1) y Control-1 (C-1) se utilizó una dosis del bloqueante neuromuscular Bromuro de Rocuronio de 0,6 mg/kg y en los grupos Experimental-2 (E-2) y Control-2 (C-2) de 1 mg/kg. La edad promedio de los pacientes fue de 34,8 ± 9,8 años; en los grupos E-1 y E-2, los predictores de vía aérea difícil predominantes fueron distancia esternomentoniana (32% y 42%), distancia tiromentoniana (24% y 40%), distancia interincisivos clase I (88% y 92 %), circunferencia de cuello  40 cm (16% y 8 %), Mallampati (88% y 40%), extensión atlanto-occipital (28% y 20%) y protrusión mandibular (28% y 20%). En el 72% y 80% de los pacientes de los grupos experimentales y control no hubo intento adicional de intubación orotraqueal (IOT); el tiempo invertido para alcanzar la IOT fue < 1 minuto en el grupo C-2 (64%) y E-2 (72%). Existen diferencias estadísticamente significativas entre el número de intentos para alcanzar la IOT, la presencia de predictores de vía aérea difícil y la dosis de bloqueante neuromuscular utilizada lo que evidencia de que a medida que se aumenta la dosis del medicamento hay mayor posibilidad de éxitos en la IOT, aun cuando estén presentes predictores de vía aérea difícil(AU)


An experimental simple blind controlled clinical trial was carried out to evaluate muscle relaxation and predictors of difficult airway in patients scheduled for general surgery at the Hospital Central Universitario Dr. Antonio María Pineda. The sample consisted of 100 patients randomly distributed into four groups of 25 patients each. Patients from the Experimental-1 (E-1) and Control-1 (C-1) groups received 0.6 mg/kg of the neuromuscular blocking agent Rocuronium Bromide while Experimetal-2 (E-2) and Control-2 (C-2) patients received a dosage of 1 mg/kg. Average age of participants was 34.8 ± 9.8 years. Predictors of difficult airway in E-1 and E-2 were sternomental distance (32% and 42%, thyromental distance (24% and 40%), interincisive distance class 1 (88% and 92%), neck circumference  40 cm (16% and 8%), Mallampati (88% and 40%), atlanto-occipital extension (28% and 20%) and mandibular protrusion (28% and 20%). In 72% and 80% of patients from the E and C groups there was not an additional attempt of orotracheal intubation (OTI); the time invested to reach the OTI was less than one minute in 64% of patients from the C-2 and 72% of the E-2. There are statistically significant differences between the number of attempts to reach the OTI, presence of predictors of difficult airway and the dose of Rocuronium Bromide which means that as the drug dosage increases, there is a greater possibility of success in the OTI, even when predictors of difficult airway are present(AU)

GOALS: Determine the success rate of guided intubation with a flexible bronchofibroscope (BFC) through supraglottic devices: I-gel or Fastrach, establish the degree of fibroscopic visualization and determine the insertion time of the laryngeal mask airway (LMA) and endotracheal intubation. METHODS: Experimental, prospective, comparative and randomized study of 60 ASA I-II-III patients, older than 18 years, undergoing elective surgeries that required general anesthesia and endotracheal intubation. After anesthetic induction, the assigned LMA was placed and intubation with BFC was performed through it. Both the success of laryngeal mask insertion and intubation were recorded, as well as the time (in seconds) equired for these procedures. The vision of the laryngeal structures was classified according to the Brimacombe Scale. RESULTS: The groups analyzed presented similar general characteristics. Insertion time of he LMA was significantly lower with I-gel than Fastrach (median 12 s vs 16 s. Respectively, p = 0.001). Also, in the intubation time BFC guided through the laryngeal mask, differences in favor of I-gel were found, with a median of 72 s. vs 89 s. (p = 0.001). Regarding the degree of glottic visualization with BFC, no statistically significant differences were found between both groups.CONCLUSIONS: Although both devices had the highest success rate (both in their placement and in intubation through them), with I-gel the intubation was performed quicker.


OBJETIVOS: Determinar la tasa de éxito de intubación guiada con broncofibroscopio flexible (BFC) a través de los dispositivos supraglóticos: I-gel o Fastrach, establecer el grado de visualización fibroscópica y determinar el tiempo de inserción de la máscara laríngea y de intubación endotraqueal. MATERIALES Y MÉTODOS: Estudio experimental, prospectivo, comparativo y aleatorizado de 60 pacientes ASA I- II ­III, mayores de 18 años, sometidos a cirugías electivas que requerían anestesia general e intubación endotraqueal. Luego de la inducción anestésica, se procedió a colocar la máscara laríngea asignada y se realizó la intubación con BFC a través de la misma. Tanto el éxito de inserción de máscara laríngea como de intubación fueron registrados, así como el tiempo (en segundos) requerido para dichos procedimientos. La visión de las estructuras laríngeas fue clasificada de acuerdo a Escala de Brimacombe. RESULTADOS: Los grupos analizados presentaron características generales similares. El tiempo de inserción de la ML fue significativamente menor con I-gel que con Fastrach (medianas: 12 s. vs 16 s. respectivamente, p = 0,001). También en el tiempo de intubación guiada con BFC a través de la máscara laríngea, se encontraron diferencias en favor de I-gel, con una mediana de 72 s. vs 89 s. (p = 0,001). Respecto al grado de visualización glótica con BFC, no se encontraron diferencias estadísticamente significativas entre ambos grupos. CONCLUSIONES: Aunque ambos dispositivos tuvieron la máxima tasa de éxito (tanto en su colocación como en la intubación a través de ellos), con I-gel se realizó la intubación de manera más rápida.

ABSTRACT Purpose to verify the efficacy of speech therapy in the early return of oral intake in patients with post-orotracheal intubation dysphagia. Methods It was a double-blinded randomized controlled trial for two years with patients of intensive care units of a hospital. Study inclusion criteria were orotracheal intubation>48hours, age≥18 years old, clinical stability, and dysphagia. Exclusion criteria were tracheotomy, score 4 to 7 in the Functional Oral Intake Scale (FOIS), neurological disorders. Patients were randomized into speech treatment or control group (ten days of follow-up). The treated group (TG) received guidance, therapeutic techniques, airway protection and maneuvers, orofacial myofunctional and vocal exercises, diet introduction; the control group (CG) received SHAM treatment. Primary outcomes were oral intake progression, dysphagia severity, and tube feeding permanence. Results In the initial period of study, 240 patients were assessed and 40 (16.6%) had dysphagia. Of this, 32 patients met the inclusion criteria, and 17 (53%) received speech therapy. Tube feeding permanence was shorter in TG (median of 3 days) compared to CG (median of 10 days) (p=0.004). The size effect of the intervention on tube feeding permanence was statistically significant between groups (Cohen's d=1.21). TG showed progress on FOIS scores compared to CG (p=0.005). TG also had a progression in severity levels of Dysphagia protocol (from moderate to mild dysphagia) (p<0.001). Conclusion Speech therapy favors an early progression of oral intake in post-intubation patients with dysphagia. Clinical Trial Registration: RBR-9829jk.


RESUMO Objetivo verificar a eficácia da fonoterapia no retorno precoce da via oral em pacientes com disfagia pós-intubação orotraqueal. Métodos Ensaio clínico controlado, randomizado, duplo-cego, realizado por dois anos com pacientes de Unidades de Terapia Intensiva de um hospital. Os critérios de inclusão foram intubação orotraqueal>48 horas, idade ≥18 anos, estabilidade clínica e disfagia. Foram excluídos pacientes com traqueotomia, 4 a 7 pontos na Escala Funcional de Ingestão Oral (FOIS), distúrbios neurológicos. Os pacientes foram randomizados para grupo tratado (GT) ou grupo controle (GC) (dez dias de acompanhamento). O GT recebeu orientações, técnicas e manobras terapêuticas, exercícios vocais e miofuncionais orofaciais, introdução da dieta por via oral; o GC recebeu tratamento SHAM. Os desfechos foram progressão da ingestão oral, gravidade da disfagia e via alternativa de alimentação. Resultados Inicialmente foram avaliados 240 pacientes, desses 40 (16,6%) apresentaram disfagia. Trinta e dois pacientes preencheram os critérios de inclusão e 17 (53%) receberam terapia fonoaudiológica. A permanência da alimentação por sonda foi menor no GT (mediana de 3 dias) em comparação ao GC (mediana de 10 dias) (p=0.004). O tamanho do efeito da intervenção sobre o tempo de permanência com sonda nasoentéroica foi estatisticametne significativo entre os grupos (Cohen's d=1.21). O GT apresentou progresso nos escores FOIS em comparação ao GC (p=0.005). O GT também teve uma progressão nos níveis de gravidade do PARD (de disfagia moderada a leve) (p<0.001). Conclusão A terapia fonoaudiológica favorece uma progressão precoce da ingestão oral em pacientes pós-intubação com disfagia. Registro de Ensaio Clínico: RBR-9829jk.

Abstract: Introduction: the coronavirus pandemic disclosed the need for safe orotracheal intubation not only for the patient, but also for the professionals involved in the procedure. Therefore, training and technique reviews became necessary. Objective: this article aims to propose the application of the Rapid Cycle Deliberate Practice (RCDP) strategy for the training of anesthesiologists in orotracheal intubation in people with confirmed or suspected COVID-19 and to present a guide for the application of this strategy in this situation. Method: This is a methodological study presenting the theoretical and operational aspects for the Rapid Cycle Deliberate Practice application and a guide constructed from the search for evidence published in journals and official recommendations published by the institutions linked to the Brazilian and international health area. Results: the main theoretical aspects reported are related to the three principles on which Rapid Cycle Deliberate Practice is based: maximizing time in deliberate practice, targeted feedback, and explicit psychological security. As for the operational aspects, it is highlighted that the training must be carried out with a maximum of six people. An error must be interrupted, prescriptive feedback must be given, and the task must be performed again until the participant reaches mastery. As for the procedure technical specificity, a guide to the application of the rapid cycle deliberate practice is presented with the sequence for the adequate airway management of hypoxemic patients with suspected or confirmed COVID-19 infection. Conclusion: it is concluded that the assessed instructional strategy showed to be promising for the training with mastery learning in all professionals who will perform the procedure of orotracheal intubation while facing the severe forms of COVID-19, minimizing the risk of contamination.


Resumo: Introdução: A pandemia por coronavírus revelou a necessidade de intubação orotraqueal de forma segura não apenas para o paciente, mas igualmente para os profissionais envolvidos no procedimento. Para isso, treinamentos e revisões de técnicas se tornam necessários. Objetivo: Este artigo tem por objetivos propor a aplicação da estratégia de Prática Deliberada em Ciclos Rápidos (PDCR) para treinamento de anestesiologistas na intubação orotraqueal em pessoas confirmadas ou suspeitas com Covid-19 e apresentar um guia para aplicação dessa estratégia nessa conjuntura. Método: Trata-se de estudo metodológico que apresenta aspectos teóricos e operacionais para a aplicação da PDCR e um guia de aplicação construído a partir da busca de evidências publicadas em periódicos e recomendações oficiais divulgadas pelos órgãos vinculados à área da saúde brasileira e internacional. Resultado: Os principais aspectos teóricos relatados são concernentes aos três princípios que baseiam a PDCR: maximização do tempo em prática deliberada, feedback direcionado e segurança psicológica explícita. Quanto aos aspectos operacionais, destaca-se que o treinamento deve ser realizado com o máximo de seis pessoas. Deve-se interromper o erro, fornecer um feedback prescritivo e pedir que a tarefa seja realizada novamente até atingir a maestria. Quanto às especificidades técnicas do procedimento, apresenta-se um guia de aplicação da PDCR com a sequência para o adequado manuseio de vias aéreas de pacientes hipoxêmicos suspeitos e positivos para Covid-19. Conclusão: A estratégia instrucional estudada mostra ser propícia a treinar com maestria os profissionais que realizarão o procedimento de intubação orotraqueal no enfrentamento das formas graves da Covid-19, visando minimizar o risco de contaminação.

Introduction: Jaw fractures are one of the most common sites of maxillofacial injuries. The location of the jaw makes it very vulnerable to direct impacts. The purpose of treatment is to restore aesthetic function. Case Report: The purpose of this report is to present a case of comminuted mandible fracture in a young male patient, treated urgently due to the need for maintenance of the airways, where fracture osteosynthesis surgery was submitted. Conclusion: The initial assessment of these trauma patients should follow the Trauma Life Support protocol, and structural damage should be investigated. Introdução: As fraturas de mandíbula são um dos locais mais comuns de lesões maxilofaciais. A localização da mandíbula a torna muito vulnerável a impactos diretos. O objetivo do tratamento é restaurar a função estética. Relato de Caso: O objetivo deste relato é apresentar um caso de fratura cominutiva de mandíbula em paciente jovem do sexo masculino, atendido com urgência devido à necessidade de manutenção das vias aéreas, onde foi submetido cirurgia de osteossíntese da fratura. Conclusão: A avaliação inicial desses pacientes com trauma deve seguir o protocolo do Suporte de Vida no Trauma, e devem ser investigados danos estruturais.

Introducción: La principal causa de morbi-mortalidad pediátrica incluye el compromiso de la vía aérea, por lo que una correcta intubación salva la vida del paciente. El objetivo de este estudio fue determinar las variables predictivas de diámetro y longitud de inserción de tubo endotraqueal en niños intubados en el área de terapia Intensiva del Hospital Vicente Corral Moscoso, Cuenca-Ecuador. Métodos: En el presente estudio observacional con una muestra no probabilística tipo censo, se determinó el diámetro del tubo endotraqueal (DTE) utilizado y la profundidad de inserción mediante rayos x para determinar la Longitud del Tubo Endotraqueal (LTE). Se utiliza correlación y regresión lineal para predecir DTE y LTE en base a Edad, Peso y Talla de niños de 1 a 120 meses de edad. Se utiliza estadística no paramétrica. Resultados: Fueron 102 casos que constituyeron una frecuencia de intubación del 30.6 %, el motivo más frecuente fue patología respiratoria. La ecuación de predicción del DTE=3.54+0.14*(edad años) + 0.04*(Peso kg). R2=0.65, P=0.0001, para LTE=10.36+0.31*(edad años) + 0.18*(Peso kg). R2=0.65, P=0.0001. Conclusiones: El diámetro y longitud del tubo endotraqueal se correlacionan con variables propias de cada paciente como edad y peso


Introduction: The main cause of morbidity and mortality in pediatrics includes the compromise of the airway and ventilation, proper intubation will save the patient's life, being therefore transcendent to consider age, height, weight and sex. Objective: To determine the predictive variables of diameter and length of endotracheal tube insertion in children aged 1 to 120 months admitted to the Intensive Care area of Vicente Corral Moscoso Hospital. Methods: Correlational study, with a sample of 102 intubated patients, determined the size of the tube used and the depth of insertion using x-rays; For qualitative variables descriptive statistics were used, for the quantitative Kolmogorov-Smirnov test, in the bivariate analysis the Mann Whitney U and Spearman correlation was used. Subsequently, the multivariate linear regression analysis was executed, the final equation was modeled and Pearson coefficients were obtained, finally ANOVA analysis was performed. Results: The frequency of intubation was 30.6%, the most frequent reason was respiratory pathology. The diameter of the tube can be predicted individually using age, weight and height, and by combining age and weight the prediction increases to 64.9%. To estimate the depth of the tube, weight (63.4%), age and height are useful, and combining age and weight increases the accuracy to 65.2%. There is no correlation between diameter, insertion length and sex. Conclusions: The diameter and length of the endotracheal tube correlate with each patient's own variables such as age and weight
