Objective: To describe the prevalence of alcohol consumption and the burden of hemorrhagic stroke and hypertensive heart disease attributed to alcohol consumption in adults aged ≥20 years in 31 provinces in China from 2005 to 2018. Methods: Data from several national representative surveys was used to estimate provincial alcohol exposure level of adults aged ≥20 years from 2005 to 2018 by using kriging interpolation and locally weighted regression methods. Global disease burden research method and data, and China's death cause surveillance data were used to calculate the population attributable fraction (PAF) of hemorrhagic stroke and hypertensive heart disease and the deaths due to alcohol consumption in men and women aged ≥20 years in 31 provinces in China. China census data of 2010 were used to calculate the attributable standardized mortality rate. Results: In 2005 and 2018, the prevalence of alcohol consumption was 58.7% (95%CI: 57.8%-59.5%) and 58.4% (95%CI: 57.6%-59.3%), respectively, in men and 17.0% (95%CI: 16.6%-17.4%) and 18.7% (95%CI:18.1%-19.3%), respectively, in women. The daily alcohol intake was 24.6 (95%CI: 23.8-25.3) g and 27.7 (95%CI: 26.8-28.7) g, respectively, in men and 6.3 (95%CI: 6.0-6.5) g and 5.3 (95%CI: 5.0-5.6) g, respectively, in women. Alcohol exposure level was higher in the provinces in central and eastern China than in western provinces. The lowest exposure level was found in northwestern provinces. From 2005 to 2018, the PAF of hemorrhagic stroke death due to alcohol consumption increased from 5.5% to 6.8%, the attributable deaths increased from 50 200 to 59 100, while the PAF of hypertensive heart disease death due to alcohol consumption increased from 7.0% to 7.7%, the attributable deaths increased from 15 200 to 29 300. The PAF of hypertensive heart disease and hemorrhagic stroke was higher in men than in women, and in central and eastern provinces than in western provinces. In 2018, the standardized mortality rates of hemorrhagic stroke and hypertensive heart disease attributed to alcohol consumption were 4.58/100 000 and 2.11/100 000, respectively. Conclusions: The prevalence of alcohol consumption in men and daily alcohol intake of drinkers were relatively high in China, especially in eastern provinces. Alcohol exposure level was lower in women than in men. Regional measures should be taken to reduce the alcohol intakes in men and current drinkers in order to reduce the health problems caused by alcohol consumption.

BACKGROUND@#Alcohol consumption is a prevalent behavior that is bi-directionally related to the risk of type 2 diabetes. However, the effect of daily alcohol consumption on glucose levels in real-world situations in the general population has not been well elucidated. This study aimed to clarify the relationship between alcohol consumption and all-day and time-specific glucose levels among non-diabetic individuals.@*METHODS@#We investigated 913 non-diabetic males and females, aged 40-69 years, during 2018-2020 from four communities across Japan. The daily alcohol consumption was assessed using a self-report questionnaire. All-day and time-specific average glucose levels were estimated from the interstitial glucose concentrations measured using the Flash glucose monitoring system for a median duration of 13 days. Furthermore, we investigated the association between all-day and time-specific average glucose levels and habitual daily alcohol consumption levels, using never drinkers as the reference, and performed multiple linear regression analyses after adjusting for age, community, and other diabetes risk factors for males and females separately.@*RESULTS@#All-day average glucose levels did not vary according to alcohol consumption categories in both males and females. However, for males, the average glucose levels between 5:00 and 11:00 h and between 11:00 and 17:00 h were higher in moderate and heavy drinkers than in never drinkers, with the difference values of 4.6 and 4.7 mg/dL for moderate drinkers, and 5.7 and 6.8 mg/dL for heavy drinkers. Conversely, the average glucose levels between 17:00 and 24:00 h were lower in male moderate and heavy drinkers and female current drinkers than in never drinkers; the difference values of mean glucose levels were -5.8 for moderate drinkers, and -6.1 mg/dL for heavy drinkers in males and -2.7 mg/dL for female current drinkers.@*CONCLUSIONS@#Alcohol consumption was associated with glucose levels in a time-dependent biphasic pattern.

ABSTRACT OBJECTIVE This study aimed to measure the proportion of Uber use instead of drinking and driving in ten Brazilian capitals, in 2019. METHODS A cross-sectional survey was developed in ten Brazilian capitals. Data were collected in agglomeration points (AP) and sobriety checkpoints (SC). Based on responses to a standardized questionnaire, the proportion of drivers who used Uber instead of drinking and driving was measured for total sample of each methodology and stratified by municipality, age group, gender, education level, and type of vehicle. Fisher's exact test was used to make comparisons between the strata. RESULTS A total of 8,864 drivers were interviewed. The most used means of transport to replace driving after drinking alcohol was the Uber system (AP: 54.6%; 95%CI: 51.2-58.0. SC: 58.6%; 95%CI: 55.2-61.9). Most of these users were aged from 18 to 29 years, women, with at least one higher education degree. According to the AP methodology, the highest magnitude of this indicator was found in Vitória (ES) (71.0%; 95%CI: 63.5-77.5), whereas the lowest was observed in Teresina (PI) (33.1%; 95%CI: 22.7-45.5). According to the SC methodology, the highest magnitude of the indicator was also found in Vitória (ES) (78.3%; 95%CI: 68.8-85.5), whereas the lowest was observed in Boa Vista (RR) (36.6%; 95%CI: 26.8-47.7). CONCLUSION In Brazilian capitals, the study showed higher proportions of Uber use instead of drinking and driving. This type of scientific evidence on factors associated with road traffic injuries presents the potential to guide public health interventions.

ABSTRACT OBJECTIVES To estimate the prevalence of weekly, monthly and abusive alcohol consumption in Brazil in 2013 and 2019, compare the period estimates, and verify the magnitude of the differences. METHODS Analysis of data on alcohol consumption in the adult population (18 years or older) from the National Health Survey (PNS), 2013 and 2019. The number of interviewees in 2013 was 60,202 and 88,531 in 2019. The samples were characterized according to demographic, socioeconomic, health, and alcohol consumption variables and differences in proportions in the period were compared using Pearson's c2 test, with Rao-Scott approximation and a 5% significance level. Multivariate Poisson regression models were estimated for the outcome variables of monthly, weekly and abusive consumption of alcoholic beverages, in order to estimate the magnitude of the differences between the 2013 and 2019 PNS estimates, using the prevalence ratio (PR). Models were adjusted per sex and age group and stratified per sex and demographic region. RESULTS There was a difference in the distribution of the population according to race, occupation, income, age group, marital status, and education. There was an increase in alcohol consumption for all outcome variables, with the exception of weekly consumption in males. The PR of weekly consumption was 1.02 (95%CI 1.014-1.026), and in females the PR was 1.05 (95%CI 1.04-1.06). The highest PRs in the general population and per sex occur for abusive consumption. The increase in weekly consumption per region occurred in the South, Southeast, and Central-West regions. CONCLUSIONS Males are the main alcohol consumers in Brazil; the PRs for both males and females show that there was an increase in monthly, weekly and abusive consumption in the research period; it is noteworthy that females have increased their consumption pattern with greater intensity than males.

Introduction: psychoactive substances have been part of the existence of humanity for thousands of years and have been used for different purposes, consolidating its use and abuse in recent years as a highly prevalent condition in our society. They are a large number of substances, and also various forms of classification. In terms of world prevalence, to date, there is no gold standard method that allows the exact measurement of substance consumption to each geographical region, which makes the study difficult and constitutes a challenge when making comparisons between regions. However, in terms of world prevalence, the most consumed substance is alcohol, followed by cannabis, opioids, amphetamines, and cocaine respectively. Objective: characterize adult patients of the mental health polyclinic of the Hernán Henríquez Aravena Hospital with a diagnosis related to substance use and establish the main substances consumed. Material and Method: A cross-sectional descriptive study was carried out in the target population by reviewing the clinical records of adult patients treated at the mental health polyclinic of the Hernán Henríquez Aravena Hospital, during the years 2018, 2019, and 2020. The antecedents collected from the study were analyzed according to the general population and compared according to gender. Results: In the sample studied, 73.2% of the users are male, alcohol being the substance most consumed, followed by cocaine, marijuana, and benzodiazepines

Community-acquired pneumonia is one of the respiratory conditions that causes the greatest demand for medical care, and is responsible for the largest number of deaths from infectious diseases in Cuba. The objective of the study was to determine the behavior of selected characteristics in patients hospitalized for pneumonia and to specify the existence of associations between some of these characteristics. An observational study, with a descriptive design, was carried out, which included 1,809 patients hospitalized for pneumonia between January 2012 and February 2020. Characteristics related to the basic, clinical-radiological conditions, and relative to management and evolution were analyzed, through analysis bivariate and multivariate (logistic regression). The series consisted mainly of elderly patients (79%), while 20% were bedridden. This condition was significantly associated with the state of advanced dementia (OR 7.6[5.5;10.4]) and was decisive in the "overlapping" presentation of the process (OR 1.5[1.09;2]). The "overlapping" presentation of pneumonia was significantly associated with late admission (OR 1.6[1.2;2.2]). As conclusions, the place occupied by several elements in the characteristics of pneumonia morbidity is ratified: advanced age, presence of comorbidities, and non-classical presentation of the process. Interrelationships of practical importance were found between the presence of comorbidities, the clinical forms of presentation, the time of admission, and the use of antimicrobials during the patient's prehospital care. The role of bed rest in the radiological extension of the pneumonic process and in the presence of medium or large pleural effusion at the time of admission is highlighted.

BACKGROUND: Adolescents are more prone to develop health risk behaviors. Among these, early consumption of psychoactive drugs is relevant. AIM: To assess the association between early consumption of illicit drugs, tobacco and alcohol, suicide behaviors, risky sexual behaviors, eating habits and physical activity in school age adolescents from Valparaiso, Chile. MATERIAL AND METHODS: The online world survey of scholar health to identify the health risk behaviors was answered by 550 adolescents aged 15 to 18 years, from a public educational institution at Valparaiso, Chile. Results: Early consumption of alcohol, smoking and illicit drug use was reported by 59, 57 and 44% of respondents, respectively. The frequency of psychoactive drug consumption during the last month, risky sexual behaviors and missing breakfast was higher among those with an early onset of alcohol and illicit drug consumption and smoking. The frequency of suicidal risk behaviors and low consumption of fruits and vegetables was higher among those with an early onset of illicit drug use and smoking. A low physical activity level was associated with early smoking onset. Health risk behaviors were associated with early onset of alcohol and illicit drug consumption and smoking. CONCLUSIONS: Early onset of alcohol and illicit drug consumption and smoking is associated with health risk behaviors in Chilean adolescents.

OBJECTIVE: To evaluate alcohol consumption among state public elementary school teachers before and during the COVID-19 pandemic and the factors associated with who increased their alcohol consumption during the pandemic. METHODS: Websurvey-type epidemiological survey, carried out with primary education teachers from public schools in Minas Gerais. A probabilistic sample with proportionality for state teachers was estimated. The collection took place from August to September 2020 via digital form and had the support of the State Department of Education. The dependent variable adopted was the increase in the consumption of alcoholic beverages by teachers during the pandemic. Poisson Regression was used, adopting a significance level of 5%. RESULTS: 15,641 teachers from 795 Minas Gerais municipalities participated in the study. Regarding the consumption of alcoholic beverages, before the pandemic 46.9% of teachers consumed at least once a week and during the pandemic 7.1% reported that they were drinking more than they used to. Higher consumption of alcoholic beverages was observed during the pandemic among men, with a lower age group, higher family income, those who had great difficulty to carry out school activities, those dissatisfied with work, those who started having sleep problems and in those where a family member or friend developed symptoms serious problems of COVID-19. CONCLUSIONS: The results showed that most participants maintained their consumption or decreased it during the pandemic. This increase was associated with sociodemographic and economic factors, working conditions during the pandemic, behaviors/life habits and health conditions.

Introduction Alcohol consumption is a public health problem that affects university students. The legality of alcohol facilitates consumption among the young and makes it the most used drug in this population, impacting health and university education. Objective To identify alcohol consumption among nursing students of a public institution in Minatitlán, Veracruz, Mexico. Methods Descriptive cross-sectional study, conducted between October and November 2020. A total of 260 nursing students participated voluntarily and anonymously. The information was collected using two instruments. The first one inquired about personal data, and the second one evaluated drinking through the Alcohol Use Disorders Identification Test (AUDIT). Results We found that 64.6% of students reported drinking at least once, with a mean age at drinking onset of 16.3 years. The preferred beverage was beer (53.6%), and the main providers of alcoholic beverages were friends (62.5%) and family (34.5%). A total of 59.6% of students had a low risk drinking patter, 4.6% had a high-risk drinking patter, and 0.4% reported dependence. There were no significant differences between drinking patterns and leisure time activities (X2 = 25.970; p = 0.355). There was a significant relationship between students' alcohol consumption patterns and age, current semester, drinking at least once, and drinking with family and friends (p < 0.01). Conclusions There is a close relationship between the initiation and maintenance of alcohol consumption with nursing students' social, family, and school context. These findings should encourage health programs that follow up on alcohol consumption during university training.

Depression is considered the most frequent mental illness; it is a cause of disability and constitutes a modifiable risk factor for the future development of dementia. The objective of tis study was to determine the frequency of major depression in elderly aged 85 and over and its possible association with dementia and with sociodemographic and clinical aspects. A descriptive cross-sectional study was carried out in 202 elderly who were treated in the protocolized consultation ¨The care of cognitive disorders in the elderly ¨, of the Hermanos Ameijeiras hospital, during the period between February 2016 and May 2016. 2020. 55.9% of the elderly were women and 64.9% were between 85 and 89 years old. A significant association was found between major depression and having comorbidity (p = 0.001), being a smoker (p = 0.003) and being a drinker (p <0.001). Major depression and dementia were diagnosed in 12.9% of the elderly, with no significant relationship (p = 0.731). The frequency of major depression in elderly aged 85 and over who were treated in a care consultation for cognitive disorders in the elderly is considered high. An association was identified between factors that deteriorated health and the presence of major depression, but not between it and dementia.

Non-communicable Diseases and Conditions (NCDs) represent the biggest cause of morbidity and mortality in Brazil and the world. They comprise two large groups of events: Chronic Non-Communicable Diseases (NCDs), characterized mainly by cardiovascular diseases, chronic respiratory diseases, neoplasms and diabetes mellitus, and external causes, such as accidents and violence. In this sense, the national VIGITEL, carried out since 2006, provided important data on risk factors for Brazil and capitals. This report is the result of the first survey carried out in the State, with representation for the State of Goiás and the five macro-regions. This data will support the new Goiás DANT Coping Plan (2023-2032)

Abstract The association between alcohol consumption by students with family conflicts, parental supervision and quality of relationships was investigated. A cross-sectional study was conducted with 1,265 adolescents from 21 schools. The Brazilian version of the Drug Use Screening Inventory/DUSI was used, and absolute and relative prevalence for categorical variables was estimated, while bivariate analyses and Pearson's Chi Square test were conducted. For the magnitude of the association between the dependent and independent variables, prevalence ratios were estimated using Poisson multivariate regression. There was an association between alcohol consumption and drug use by family members, frequent discussions and parental lack of awareness about adolescents' activities. Family supervision, a good relationship with parents/guardians, establishing an affective family bond and parents' knowledge about teenagers' whereabouts, were protective factors for alcohol use. The conclusion was that the high rate of schoolchildren who consume alcohol is worrying, liable to health complications in general, and a harmonized family relationship is fundamental. The implementation of measures of intervention between school and the Family Health Strategy, can contribute to the establishment of a protective bond.

Abstract This is a cross-sectional study with 2,909 participants (aged ≥18 years) from the baseline of the Cohort of Universities of Minas Gerais [CUME - Coorte de Universidades Mineiras] which verified the association between alcohol consumption and overweight. Data on sociodemographic factors, lifestyle, eating habits, anthropometric factors and clinical conditions were collected through an online questionnaire. Body mass index ≥ 25 kg/m² was considered an indicator of overweight. The daily consumption of alcohol was evaluated in grams (alcohol) and according to type in milliliters (beer, wine, hard liquor). The prevalence of alcohol consumption and overweight was 73.6% and 40.8%, respectively. There was a significant tendency of an increase in overweight with higher beer consumption (tendency p value of 0.038), which was not observed for the other types of alcohol. After sensitivity analyses, alcohol consumption was associated to overweight, with a tendency of increase in prevalence with higher daily consumption. There is a crucial need to curb the widely accepted idea that a low or moderate alcohol consumption is not harmful to one's health, and to be cautious of such a proposition. The influence of alcohol consumption regarding weight gain must be considered in public health policies and policies of alcohol consumption control.

Abstract The SHAHRP program was effective reducing drinking and alcohol - harms in Australia, but cross-cultural adaptation is required before replication. This study aimed at assessing the feasibility of SHAHRP in Brazil focused on implementation and acceptability. A mixed-methodsdesign was used: quantitative for implementing the program and evaluation and qualitative for acceptability. The quantitative design was a pilot of a randomized controlled trial. Private schools were randomly divided into four intervention (n=160) and four control (n=188) schools. Student's mean age was 12.7 years. The fidelity of implementation and likely outcome measures were assessed. Qualitative data on acceptability were provided by students and teachers. The percentage of implementation varied from 62.5% to 87.5%. Behaviours such as alcohol-harms requires a larger cohort and longer follow-up to be adequately evaluated. The risk reduction approach and activities had good acceptability from students and teachers. Quantitative and qualitative outcomes on knowledge and decision-making indicated possible improvement in SHAHRP schools. The program is feasible and well accepted in a Brazilian setting, opening the way for a more comprehensive evaluation and dissemination.

SUMMARY: The association of alcohol consumption with type 2 diabetes has been explained by increased insulin sensitivity, anti-inflammatory effects, or effects of adiponectin. The aim was to launch a consistent relation between alcohol intake and insulin sensitivity. Several databases (MEDLINE, EMBASE, Scopus and Web of Science) were searched from 1990 to April 2020 for studies in English, using MeSH terms and text words involving to alcohol consumption and insulin sensitivity. Protocol registered on PROSPERO CRD42020205107. A total of seven original articles were analyzed, where four collected data through cross-sectional study, two papers with randomized crossover design, and one used a non-randomized study. The protective effect of moderate alcohol consumption on type 2 diabetes has been described, where an improvement on insulin levels has been shown in adults between 26.5-57 years old. Our research shows that alcohol effects on blood insulin levels could vary depending of the type of alcoholic drink ingested; and that alcohol intake increased leptin and adiponectin levels, suggesting that alcohol consumption may increase glucose catabolism promoting insulin sensitivity via leptin and adiponectin. However, original studies should consider time of exposure, age, dosage, ethnicity, and alcohol type in order to conclude right affirmations.

Abstract The scope of this study was to verify alterations in cigarette and alcohol consumption in a sample of adolescents from the state education system of the city of Curitiba in the State of Paraná between 2015 and 2017. It involved a longitudinal prospective cohort study conducted with 799 adolescents (11 to 15 years of age) between 2015 and 2017. The Youth Risk Behavior Surveillance System (YRBS) questionnaire evaluated mild and excessive alcohol and tobacco consumption. Modifications of alcohol and tobacco consumption were evaluated through Generalizable Estimating Equation (GEE) models with Poisson distribution to obtain Incidence Risk Ratios (IRR) with 95% confidence intervals (95%CI). The "time" variable was treated as an independent variable along with other possible predictors (gender, age bracket, nutritional status, socioeconomic level, head of household education, physical activity and screen time). Tobacco consumption (IRR= 1,32, 95%CI = 1,02; 1,71), mild (IRR= 1,32, 95%CI = 1,13; 1,53) and excessive (IRR= 1,21, 95%CI = 1,04; 1,40) alcohol consumption increased between 2015 and 2017. This study indicates that adolescents participating in this research are modifying their behaviors towards a less healthy lifestyle.

OBJECTIVES: To describe the prevalence and factors associated with the use of alcoholic beverages among medical students. Estimate the prevalence of smoking. METHODS: Cross-sectional study (census) with a self-administered questionnaire in medical students at a public university in Bahia (n = 616) enrolled from the first to the eighth semester. Multivariate analysis using Cox Regression identified the factors associated with the use of alcoholic beverages, through three outcomes: presence and frequency of use and the pattern of use called binge drinking. RESULTS: 556 students were studied (90.2% response), among which, 58.7% drink, 21% consume alcohol at least once a week and 36% do binge drinking. The male population and those more physically active at leisure time showed a higher prevalence of alcohol use, either due to the higher frequency of use or the use of more doses on one occasion. School variables were associated with one or another method of measuring alcohol use. The prevalence of smoking was 4.8%. CONCLUSION: The use of alcoholic beverages and their consumption pattern were high in the studied population, consistent with other student populations already investigated. The low prevalence of smoking is consistent with the literature that admits the success of public policies aimed at controlling this consumption in Brazil.
