A pouca sedação em Unidades de Terapia Intensiva Pediátrica (UTIP) pode gerar estresse psicológico e físico desnecessário, além de extubação acidental. Em contrapartida, a super sedação pode provocar o uso da ventilação mecânica prolongada, levando ao maior tempo de internação dentro da UTIP, além de síndrome de abstinência, tolerância e delírio. Neste sentido, delimitou-se como objeto de estudo a identidade do cuidado de enfermagem frente às reações adversas do uso de analgésicos e sedativos nas crianças gravemente doentes. Os objetivos são: descrever as reações adversas do uso de sedativos e analgésicos identificadas pela equipe de enfermagem; analisar a prática do cuidado da equipe de enfermagem frente às reações adversas do uso de sedativos e analgésicos nas crianças; e discutir as implicações da prática do cuidado da enfermagem prestada à criança que apresenta reações adversas no uso de analgésicos e sedativos. Trata-se de um estudo de abordagem qualitativa. Foram entrevistados 15 membros da equipe de enfermagem que trabalhavam em uma UTIP de um hospital especializado em doenças crônicas e raras, utilizando-se a técnica de entrevista, o formulário de caracterização dos participantes, o diário de campo para observação não participante e a coleta de informações nos prontuários. Contou-se com o apoio do software Iramuteq para realizar a Análise Textual Discursiva. Obteve-se que as principais reações adversas identificadas pelos profissionais de enfermagem foram irritabilidade, choro e agitação. A prática do cuidado de enfermagem é desenvolvida de acordo com as reações adversas previamente identificadas e varia conforme profissional de enfermagem que atende a criança, não existindo um padrão institucional pré-estabelecido. Entre os cuidados descritos estão: banho momo, administração de doses fracionadas de sedação segundo a prescrição médica e acalento pelo familiar responsável ou pelo próprio profissional. No entanto, não foram observados registros no prontuário acerca dessas reações adversas, bem como desses cuidados relatados. Destaca-se que existe insegurança por parte da equipe de enfermagem no que tange os seus conhecimentos sobre as reações adversas relacionadas ao uso de sedativos e analgésicos e, como implicações para a prática, a falta de identidade no seu cuidado interferindo em sua autonomia e protagonismo profissional, sendo a figura do médico percebida como o mais atuante nessa temática. Conclui-se que a equipe de enfermagem reconhece alguns sinais e sintomas das reações adversas no uso de sedativos e analgésicos. Todavia, se faz necessário um aprimoramento e atualizações frente à utilização desses medicamentos, suas reações adversas e cuidados que devem ser prestados, de modo a reconhecer sua atuação e perceber sua posição dentro da equipe multiprofissional.
Inadequate sedation in Pediatric Intensive Care Units (PICU) can generate unnecessary psychological and physical stress, in addition to accidental extubating. In this sense, the identity of nursing care in the face of adverse reactions from the use of analgesics and sedatives in seriously ill children was defined as the object of study. The objectives are to describe the adverse reactions caused using sedatives and analgesics identified by the nursing team; analyze the care practice of the nursing team in the face of adverse reactions from the use of sedatives and analgesics in children; and discuss the implications of nursing care practice provided to children who present adverse reactions to the use of analgesics and sedatives. This is a qualitative study. Fifteen members of the nursing team who worked in a PICU of a hospital specializing in chronic and rare diseases were interviewed, using the interview technique, the participant characterization form, the field diary for non-participant observation and data collection. information in medical records. The Iramuteq software was supported, and discursive textual analysis was used. It was found that the main adverse reactions identified by nursing professionals were irritability, crying and agitation. The practice of nursing care is developed according to previously identified adverse reactions and varies depending on the nursing professional who cares for the child, and there is no pre-established institutional standard. Among the care described are warm bath, administration of fractional doses of sedation according to medical prescription and comfort by the responsible family member or by the professional himself, however, no records were observed in the medical records regarding these adverse reactions, as well as this reported care. It is noteworthy that there is insecurity on the part of the nursing team regarding their knowledge about adverse reactions related to the use of sedatives and analgesics and, as implications for practice, the lack of identity in their care, interfering with their autonomy and protagonism professional, with the doctor being perceived as the most active on this topic. It is concluded that the nursing team recognizes some signs and symptoms of adverse reactions in the use of sedatives and analgesics, however, improvement and updates are necessary regarding the use of these medications, their adverse reactions and the care that must be provided to recognize their performance and understand their position within the multidisciplinary team.
Una mala sedación en las Unidades de Cuidados Intensivos Pediátricos (UCIP) puede generar estrés psicológico y físico innecesario, además de extubaciones accidentales. Por otro lado, la sobresedación puede llevar al uso de ventilación mecánica prolongada, provocando una hospitalización más prolongada en la UCIP además de síndrome de abstinencia, tolerancia y dehrio. En este sentido, se definió como objeto de estudio la identidad del cuidado de enfermería ante las reacciones adversas por el uso de analgésicos y sedantes en niños gravemente enfermos. Los objetivos son: describir las reacciones adversas provocadas por el uso de sedantes y analgésicos identificadas por el equipo de enfermería; analizar la práctica de cuidados del equipo de enfermería ante las reacciones adversas por el uso de sedantes y analgésicos en niños; y discutir las implicaciones de la práctica de los cuidados de enfermería brindados a niños que presentan reacciones adversas al uso de analgésicos y sedantes. Este es un estudio cualitativo. Se entrevistó a quince miembros del equipo de enfermería que trabajaban en una UCIP de un hospital especializado en enfermedades crónicas y raras, utilizando la técnica de la entrevista, la ficha de caracterización participante, el diario de campo para la observación no parficipante y la recolección de información en historias clínicas. Se apoyó el software Iramuteq y se utiliz6 análisis textual discursivo. Se encontró que las principales reacciones adversas identificadas por los profesionales de enfermería fueron irritabilidad, llanto y agitaci6n. La práctica del cuidado de enfermería se desarrolla según reacciones adversas previamente identificadas y varía dependiendo del profesional de enfermería que atiende al niño, no existiendo un estándar institucional preestablecido. Entre los cuidados descritos se encuentran: baño tibio, administración de dosis fraccionadas de sedación según prescripción médica y confort por parte del familiar responsable o por el propio profesional, sin embargo, no se observaron registros en las historias clínicas respecto a estas reacciones adversas, así como estos ínformamn cuidados. Se destaca que existe inseguridad por parte del equipo de enfermería respecto de su conocimiento sobre las reacciones adversas relacionadas al uso de sedantes y analgésicos y, como implicaciones para la práctica, la falta de identidad en su cuidado, interfiriendo en su autonomía y protagonismo profesional, siendo el médico el más activo en este tema. Se concluye que el equipo de enfermería reconoce algunos signos y síntomas de reacciones adversas en el uso de sedantes y analgésicos, sin embargo, es necesario mejorar y actualizar respecto al uso de estos medicamentos, sus reacciones adversas y los cuidados que se deben brindar para reconocer su desempeño y comprender su posición dentro del equipo multidisciplinario.

Introducción: El prolapso de órganos pélvicos (POP) o distopia genital, es el descenso o desplazamiento de los órganos del suelo pélvico a través del canal vaginal o fuera de este. Objetivo: Evaluar los resultados de la histeropexia vaginal en la corrección quirúrgica del prolapso genital apical grado III o IV, usando prótesis de polipropileno; además describir la tasa de éxito, recurrencias y complicaciones. Método: Estudio de cohorte, en 42 mujeres sometidas a histeropexia vaginal (histero-cistopexia ortotópica) mediante prótesis de polipropileno (Splentis®), entre 2016 y 2021. Se realizaron tres evaluaciones postoperatorias (tres, seis y 12 meses). Se hizo muestreo no probabilístico. Se utilizó estadística descriptiva. Resultados: La edad media fue de 56,19 ± 9,27 años. El tiempo quirúrgico de 58,95 ± 13,74 minutos, el sangrado quirúrgico de 119,85 ± 68,73 ml. La tasa de éxito a los 12 meses fue del 90,47%. La recurrencia del prolapso apical fue del 4,76% a los seis meses y del 9,52% a los 12 meses; el de compartimento anterior a los seis meses arrojó un 7,14%, frente al 11,9% a los 12 meses. El 14,28% de las pacientes presentaron complicaciones menores. La incidencia de incontinencia urinaria de esfuerzo a los 12 meses fue del 16,66%. Conclusiones: la histeropexia vaginal es un procedimiento efectivo y seguro, con bajas tasas de recurrencias o complicaciones. Es importante que se sigan haciendo estudios con mejores diseños estadísticos.
Introduction: Pelvic organ prolapse (POP), or genital dystopia, is the descent or displacement of pelvic floor organs through the vaginal canal or outside of it. Objective: To evaluate the results of vaginal hysteropexy in the surgical correction of grade III or IV apical genital prolapse, using polypropylene prosthesis; also describe the success rate, recurrences and complications. Method: Cohort study in 42 women undergoing vaginal hysteropexy (orthotopic hystero-cystopexy) using a polypropylene prosthesis (Splentis®); between 2016 and 2021. Three postoperative evaluations were carried out (three, six and twelve months). Non-probabilistic sampling was done. Descriptive statistics were used. Results: The mean age was 56.19 ± 9.27 years. Surgical time of 58.95 ± 13.74 minutes, surgical bleeding of 119.85 ± 68.73 ml. The success rate after twelve months was 90.47%. Apical prolapse recurrence was 4.76% at six months and 9.52% at twelve months; that of the previous compartment, after six months it showed 7.14%, compared to 11.9% after twelve months; 14.28% of the patients presented minor complications. The incidence of stress urinary incontinence, at twelve months, was 16.66%. Conclusions: Vaginal hysteropexy is an effective and safe procedure, with low rates of recurrence or complications. It is important that studies continue to be carried out with better statistical designs.

INTRODUCTION@#The use of periarticular (PA) tranexamic acid (TXA) and its efficacy in comparison with intra-articular (IA) TXA have not been well explored in the literature. This retrospective cohort study aimed to compare the effects of IA and PA TXA with analgesic components in reducing blood loss and improving immediate postoperative pain relief and functional outcomes in patients after unilateral primary total knee arthroplasty (TKA).@*METHODS@#A total of 63 patients underwent TKA, and they were divided into the IA TXA delivery group ( n = 42) and PA TXA delivery group ( n = 21). All patients were administered 1 g of TXA. They also received pericapsular infiltration consisting of 0.5 mL of adrenaline, 0.4 mL of morphine, 1 g of vancomycin, 1 mL of ketorolac and 15 mL of ropivacaine. Outcomes for blood loss and surrogate markers for immediate functional recovery were measured.@*RESULTS@#Of the 63 patients, 54% were female and 46% male. The mean drop in postoperative haemoglobin levels in the PA and IA groups was 2.0 g/dL and 1.6 g/dL, respectively, and this was not statistically significant ( P = 0.10). The mean haematocrit drop in the PA and IA groups was 6.1% and 5.3%, respectively, and this was also not statistically significant ( P = 0.58). The postoperative day (POD) 1 and discharge day flexion angles, POD 1 and POD 2 visual analogue scale (VAS) scores, gait distance on discharge and length of hospitalisation stay were largely similar in the two groups.@*CONCLUSION@#Our study showed that both IA and PA TXA with analgesic components were equally efficient in reducing blood loss and improving immediate postoperative pain relief and functional outcomes.

OBJECTIVE@#To compare the accuracy and effectiveness of orthopaedic robot-assisted minimally invasive surgery versus open surgery for limb osteoid osteoma.@*METHODS@#A clinical data of 36 patients with limb osteoid osteomas admitted between June 2016 and June 2023 was retrospectively analyzed. Among them, 16 patients underwent orthopaedic robot-assisted minimally invasive surgery (robot-assisted surgery group), and 20 patients underwent tumor resection after lotcated by C-arm X-ray fluoroscopy (open surgery group). There was no significant difference between the two groups in the gender, age, lesion site, tumor nidus diameter, and preoperative pain visual analogue scale (VAS) scores ( P>0.05). The operation time, lesion resection time, intraoperative blood loss, intraoperative fluoroscopy frequency, lesion resection accuracy, and postoperative analgesic use frequency were recorded and compared between the two groups. The VAS scores for pain severity were compared preoperatively and at 3 days and 3 months postoperatively.@*RESULTS@#Compared with the open surgery group, the robot-assisted surgery group had a longer operation time, less intraoperative blood loss, less fluoroscopy frequency, less postoperative analgesic use frequency, and higher lesion resection accuracy ( P<0.05). There was no significant difference in lesion resection time ( P>0.05). All patients were followed up after surgery, with a follow-up period of 3-24 months (median, 12 months) in the two groups. No postoperative complication such as wound infection or fracture occurred in either group during follow-up. No tumor recurrence was observed during follow-up. The VAS scores significantly improved in both groups at 3 days and 3 months after surgery when compared with preoperative value ( P<0.05). The VAS score at 3 days after surgery was significantly lower in robot-assisted surgery group than that in open surgery group ( P<0.05). However, there was no significant difference in VAS scores at 3 months between the two groups ( P>0.05).@*CONCLUSION@#Compared with open surgery, robot-assisted resection of limb osteoid osteomas has longer operation time, but the accuracy of lesion resection improve, intraoperative blood loss reduce, and early postoperative pain is lighter. It has the advantages of precision and minimally invasive surgery.

Descorna cirúrgica em bovinos é uma prática bastante realizada na medicina veterinária, com indicação principalmente para evitar acidentes com fraturas, trabalhadores e outros animais. O objetivo do presente relato seria ressaltar o uso de analgésicos e sedativos em bovinos submetidos a este procedimento cirúrgico, compartilhar técnica cirúrgica utilizada, informações de monitoramento de sinais vitais no trans cirúrgico e recomendações pré, trans e pós-cirúrgicas, assim como possíveis complicações. Foi atendido um bovino da raça Jersey, fêmea, 1 ano de idade, pesando cerca de 222kg, submetido a descorna bilateral devido ao comportamento agressivo. Contido em tronco de contenção e tranquilizado com acepromazina, recebeu meloxicam e associação de sulfa com trimetropim. Após tricotomia e antissepsia foi realizado bloqueio perineural do ramo cornual do nervo zigomático temporal e bloqueio infiltrativo ao contorno de cada corno com lidocaína sem vasoconstritor. Com os cornos insensíveis, se deu início ao procedimento de descorna cirúrgica pela associação das técnicas de serra de gigli com o alicate do tipo Dick Kaber. Concluímos ser de extrema importância a execução da técnica cirúrgica por médico veterinário apto, realizando o procedimento de forma asséptica e com analgesia visando o bem estar do animal. No caso em questão, a associação das técnicas de serra de gigli com o alicate do tipo Dick Kaber foi parcialmente satisfatória, o animal apresentou boa margem de fechamento da sutura e sem deiscência dos pontos, porém houve dificuldades de retirada de margens ósseas pontiagudas após uso do alicate. A tranquilização associada ao bloqueio local foi efetiva para tal procedimento.(AU)
Surgical dehorning in cattle is widely performed practice in veterinary medicine, primarily indicated to prevent accidents involving workers, fights between animals, and skull fractures. This procedure involves the removal of the horns of the horns of cattle and should be performed using ethical methods by a qualified veterinarian. The objective of this case report is to highlight the use of analgesics and sedatives in cattle undergoing this surgical procedure, share the surgical technique employed, information on vital sign monitoring during the procedure, and pre-, intra-, and post-surgical recommendations, as well as potential complications. A Jersey cattle, female, 1 year old, weighing approximately 222kg, underwent bilateral dehorning due to aggressive behavior. Restrained in a cattle chute and tranquilized with acepromazine, she received meloxicam and a combination of sulfadiazine with trimethoprim. After trichotomy and antiseptic preparation, perineural blockade of cornual branch of the zygomatic temporal nerve was performed, follows by infiltrative blockade around each horn with lidocaine without vasoconstrictor. With the horns desensitized, the surgical dehorning procedure began using a combination of Gigli saw and Dick Kaber-type wire snare. In conclusion, it is of utmost importance for the surgical technique to be performed by a qualified veterinarian, ensuring aseptic procedure and correct analgesia for the well-being of the cattle. In this case, the combination of Gigli saw and Dick Kaber-type wire snare was partially satisfactory; the animal had a good suture closure margin and did not present suture dehiscence, but there were difficulties in we moving pointed bony margins after using the wire snare. Tranquilization combined with local blockade was effective for this procedure.(AU)
El descornado quirúrgico del ganado bovino es una práctica muy utilizada en medicina veterinaria, indicada principalmente para prevenir accidentes que involucren fracturas, trabajadores y otros animales. El objetivo de este informe sería resaltar el uso de analgésicos y sedantes en bovinos sometidos a este procedimiento quirúrgico, compartir la técnica quirúrgica utilizada, información sobre el monitoreo de signos vitales durante la cirugía y recomendaciones pre, trans y posquirúrgicas, así como posibles complicaciones. Se trata de una hembra bovina Jersey, de 1 año de edad, con un peso aproximado de 222 kg, siendo sometida a descornado bilateral por comportamiento agresivo. Se recibieron contenidos en un baúl de contención y tranquilizados con acepromacina, meloxicam y una combinación de sulfas y trimetopim. Mediante tricotomía y antisepsia se realizó bloqueo perineural de la rama cornual del nervio temporal cigomático y bloqueo infiltrativo en todo el contorno de cada miembro con lidocaína sin vasoconstrictor. Con cuerpos insensibles se inició el procedimiento de descornado quirúrgico, combinando técnicas de aserrado con alicates Dick Kaber. Concluimos que es de suma importancia realizar la técnica quirúrgica por un veterinario calificado, realizando el procedimiento de manera aséptica y con analgesia administrada para el bienestar del animal. En el caso que nos ocupa, la asociación de técnicas de la sierra con la pinza tipo Dick Kaber fue parcialmente satisfactoria, el animal presentó un buen margen de cierre de sutura y ninguna dehiscencia de los puntos, sin embargo, hubo dificultades para eliminar los márgenes óseos afilados después de usar alicates La tranquilidad asociada al bloqueo local fue eficaz para este procedimiento.(AU)

Abstract Introduction: Bone cancer metastasis may produce severe and refractory pain. It is often difficult to manage with systemic analgesics. Chemical neurolysis may be an effective alternative in terminally ill patients. Case report: Female terminally ill patient with hip metastasis of gastric cancer in severe pain. Neurolytic ultrasound-guided blocks of the pericapsular nerve group and obturator nerve were performed with 5% phenol. This led to satisfactory pain relief for 10 days, until the patient's death. Discussion: This approach may be effective and safe as an analgesic option for refractory hip pain due to metastasis or pathologic fracture in terminally ill patients.

El dolor lumbar crónico es un problema frecuente que afecta la calidad de vida de muchos pacientes y representa un desafío para los sistemas de salud. Ante la búsqueda de alternativas para aliviar el dolor, el yoga ha surgido como una opción prometedora. A partir de un caso clínico real, el autor de este artículo realiza una búsqueda bibliográfica y sintetiza la mejor evidencia disponible sobre la efectividad del yoga para reducir el dolor en pacientes con lumbalgia crónica.Finalmente, concluye que, si bien podría ser beneficioso, aún persiste incertidumbre y se necesitan más estudios para determinar si este es superior a otros tipos de ejercicio físico centrados en el espalda. Chronic low back pain is a common problem that affects the quality of life of many patients and represents a challenge for health systems. In the quest for alternatives to relieve pain, yoga has emerged as a promising option. Based on a real clinical scenario, the author of this article conducts a literature search and synthesizes the best available evidence on the effectiveness of yoga for reducing pain in patients with chronic low back pain. Finally, he concludes that, although it maybe beneficial, uncertainty still persists and more studies are needed to determine whether it is superior to other physical exercises focused on the back. Abstract Background and aims: Dexamethasone as adjunct to local anesthetic solution improves the quality of brachial plexus block (BPB). However, evidence for its efficacy at low doses (< 4 mg) is lacking. This study was designed to evaluate the duration of analgesia attained with low dose dexamethasone as adjuvant to local anesthetic for creation of arteriovenous fistula (AVF) under BPB. Methods: Sixty-six patients scheduled for AVF creation were randomly allocated to receive either saline (control) or 2 mg dexamethasone, together with 0.5% ropivacaine and 0.2% lignocaine. The primary outcome was duration of analgesia, defined as time from performing the block to the first analgesic request. The secondary outcomes were time from injection to complete sensory block, time from injection to complete motor block, duration of motor block, postoperative analgesic consumption, and fistula patency at three months. Results: All the blocks were effective. In the group that received dexamethasone, the time to first analgesic request was significantly delayed (432 ± 43.8 minutes vs. 386.4 ± 40.2 minutes; p < 0.01). The onset of sensory and motor blockade occurred faster in dexamethasone group and overall analgesic consumption was also reduced. However, dexamethasone addition did not prolong the duration of motor block. There was no statistically significant difference in the patency of fistulas between the two groups at three months. (p = 0.34). Conclusion: Addition of low-dose perineural dexamethasone to local anesthetic solution significantly prolonged the duration of analgesia. Further trials are warranted to compare the adverse effects between dexamethasone doses of 4 mg and lower.

Abstract Background Opioids are the cornerstone in managing postoperative pain; however, they have many side effects. Ketamine and Magnesium (Mg) are NMDA receptor antagonists used as adjuvant analgesics to decrease postoperative opioid consumption. Objective We assumed that adding Mg to ketamine infusion can improve the intraoperative and postoperative analgesic efficacy of ketamine infusion alone in cancer breast surgeries. Methods Ninety patients aged between 18 and 65 years and undergoing elective cancer breast surgery were included in this prospective randomized, double-blind study. Group K received ketamine 0.5 mg.kg-1 bolus then 0.12 mg.kg-1.h-1 infusion for the first 24 hours postoperatively. Group KM: received ketamine 0.5 mg.kg-1 and Mg sulfate 50 mg.kg-1, then ketamine 0.12 mg.kg-1.h-1 and Mg sulfate 8 mg.kg-1.h-1 infusions for the first 24 hours postoperative. The primary outcome was the morphine consumption in the first 24 hours postoperative, while the secondary outcomes were: intraoperative fentanyl consumption, NRS, side effects, and chronic postoperative pain. Results Group KM had less postoperative opioid consumption (14.12 ± 5.11 mg) than Group K (19.43 ± 6.8 mg). Also, Group KM had less intraoperative fentanyl consumption. Both groups were similar in postoperative NRS scores, the incidence of side effects related to opioids, and chronic neuropathic pain. Conclusion Adding Mg to ketamine infusion can safely improve intraoperative and postoperative analgesia with opioid-sparing effect in cancer breast surgery.

Kelussia odoratissima fruits are utilized in Persian traditional medicine as a painkiller and to prevent inflammation-based disorders. Considering the higher content of essential oil in the fruits, the oil's anti-inflammatory and analgesic activities were investigated via the paw edema triggered in mice and the writhing test and hot plate technique, respectively. It was observed that the 100, and 200 mg/Kg doses of the oil revealed an analgesic impact (p<0.001) considering the increment in the reaction time needed for the hot plate approach. Furthermore, 100 and 200 mg/Kg doses of the oil caused a reduction in the frequency of writhes in the mice (p<0.01 and p<0.001, respectively). Using all examined doses of theoil (25, 50, and 100 mg/Kg) caused inflammatory reduction (p<0.001). The findings indicated that the oil possess significant activities against acute inflammation. It had both peripheral and central pain-killing impacts. The main components 3-n-butylphthalide (28.3%) and germacrene D (17.3%) can be considered as the responsible compounds to manage the inflammation and pain.
Las frutas de Kelussia odoratissima se utilizan en la medicina tradicional persa como analgésico y para prevenir los trastornos basados en la inflamación. Teniendo en cuenta el mayor contenido de aceite esencial en las frutas, se investigaron las actividades antiinflamatorias y analgésicas del aceite a través del edema de la pata desencadenado en ratones y la prueba de contorsiones y la técnica del plato caliente, respectivamente. Se observó que las dosis de 100 y 200 mg / kg del aceite revelaron un impacto analgésico (p<0,001) considerando el incremento en el tiempo de reacción necesario para el enfoque de placa caliente. Además, dosis de 100 y 200 mg / kg del aceite provocaron una reducción en la frecuencia de retorcimientos en los ratones (p<0,01 y p<0,001, respectivamente). El uso de todas las dosis examinadas del aceite (25, 50 y 100 mg/kg) provocó una reducción inflamatoria (p<0,001). Los hallazgos indicaron que el aceite posee actividades significativas contra la inflamación aguda. Tiene impactos analgésicos tanto periféricos como centrales. Los principales componentes 3-n-butilftalida (28,3%) y germacreno D (17,3%) pueden considerarse como los compuestos responsables del manejo de la inflamación y el dolor.

Abstract Backgraund: This prospective observational cohort study aimed to investigate the relationship between preoperative anxiety levels and postoperative pain and analgesic requirement in patients undergoing laparoscopic sleeve gastrectomy. Methods: Forty two female patients with body mass index ≥ 35, who underwent laparoscopic sleeve gastrectomy for treatment of obesity were included in the study. Spielberger's state and trait anxiety scales were used in this study. Demographic data of the patients, anesthetic and analgesic drugs during the surgery, pain levels measured with verbal analog scale at the postoperative 1st, 4th, 12th, and 24th hour, sedation levels measured with the Ramsay sedation scale, and the amount of analgesic consumed were recorded. Anesthesiologist, surgeon, and patient were not informed of the anxiety level results. The relationship between preoperative anxiety and postoperative pain and analgesic consumption was evaluated by Spearman's correlation analysis. Stepwise multiple linear regression analysis was applied. Normal Distribution control was performed by applying the Shapiro-Wilk test to residual values obtained from the final model. Results: There was no relationship between trait anxiety level and postoperative pain and analgesic consumption. A correlation was found between state anxiety level and pain level up to 24 hours and analgesic consumption (p < 0.05). According to the obtained model it had been observed that the university graduates consumed more analgesic compared to other education level groups. Conclusion: In this study, a relationship was found between preoperative state anxiety level and 24-hour pain scores and analgesic consumption in patients who underwent laparoscopic sleeve gastrectomy under general anesthesia.

Abstract Background There has been a growing interest in the use of ketamine following orthopedic surgeries. We hypothesized that low dose intravenous ketamine during surgery would help in mobilization following total knee replacement (TKR) in oncology patients as assessed by the timed to up and go (TUG) test at 72 hours post-surgery. Our secondary objectives were to compare the opioid requirement at the end of 72 hours, pain scores, satisfaction with pain management, adverse effects, range of joint movement achieved in the post-operative period and the functional recovery at the end of 1 month. Methods After the ethics commitee approval, registration of the trial with the Clinical Trial Registry - India (CTRI), and informed consent, this double-blinded trial was conducted. Using computer generated randomization chart, an independent team randomized the patients into ketamine group which received at induction, a ketamine bolus dose of 0.5 mg.kg-1 before the incision followed by 10 µg.kg-1min-1 infusion which was maintained intraoperatively till skin closure and the saline group received an equivalent volume of saline. Postoperatively, patient controlled morphine pumps were attached and the pain score with morphine usage were recorded for 72 hours. The TUG tests and range of motion were assessed by the physiotherapists until 72 hours. Results Fifty-two patients were enrolled in the trial. Demographics were comparable. No significant intraoperative hemodynamic changes and post-operative adverse events were noted between the groups. A decrease in the TUG test, along with decreased opioid usage with a better range of movements was noted in the ketamine group, but this was not statistically significant. Day of discharge, patient satisfaction score, and functional recovery assessed by Oxford Knee Score (OKS) were comparable between the groups. Conclusion In conclusion, low dose intraoperative ketamine infusion does not provide clinical benefit in perioperative pain management and postoperative rehabilitation following total knee endoprosthetic replacement in oncology.

BACKGROUND@#Treatment duration of wrist-ankle acupuncture (WAA) is uncertain for post-thyroidectomy pain relief.@*OBJECTIVE@#This study evaluated the effect of different WAA treatment duration on post-operative pain relief and other discomforts associated with thyroidectomy.@*DESIGN, SETTING, PARTICIPANTS AND INTERVENTION@#This randomized controlled trial was conducted at a single research site in Guangzhou, China. A total of 132 patients receiving thyroidectomy were randomly divided into the control group (sham WAA, 30 min) and three intervention groups (group 1: WAA, 30 min; group 2: WAA, 45 min; group 3: WAA, 60 min), with group allocation ratio of 1:1:1:1. Acupuncture was administered within 1 hour of leaving the operating room.@*OUTCOMES AND MEASURES@#Primary outcome was patients' pain at the surgical site assessed by visual analogue scale (VAS) at the moment after acupuncture treatment (post-intervention). Secondary outcomes included the patients' pain VAS scores at 6, 12, 24, 48 and 72 h after the thyroidectomy, the 40-item Quality of Recovery (QoR-40) score, the grade of post-operative nausea and vomiting (PONV), and the use of additional analgesic therapy.@*RESULTS@#The adjusted mean difference (AMD) in VAS scores from baseline to post-intervention in group 1 was -0.89 (95% confidence interval [CI], -1.02 to -0.76). The decrease in VAS score at post-intervention was statistically significant in group 1 compared to the control group (AMD, -0.43; 95% CI, -0.58 to -0.28; P < 0.001), and in groups 2 and 3 compared to group 1 (group 2 vs group 1: AMD, -0.65; 95% CI, -0.81 to -0.48; P < 0.001; group 3 vs group 1: AMD, -0.66; 95% CI, -0.86 to -0.47; P < 0.001). The VAS scores in the four groups converged beyond 24 h after the operation. Fewer patients in group 2 and group 3 experienced PONV in the first 24 h after operation. No statistical differences were measured in QoR-40 score and the number of patients with additional analgesic therapy.@*CONCLUSION@#Compared with the 30 min intervention, WAA treatment with longer needle retention time (45 or 60 min) had an advantage in pain relief within 6 h after surgery. We wished to establish an expert consensus on late stage of critical care (CC) management. The panel comprised 13 experts in CC medicine. Each statement was assessed based on the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) principle. Then, the Delphi method was adopted by 17 experts to reassess the following 28 statements. (1) ESCAPE has evolved from a strategy of delirium management to a strategy of late stage of CC management. (2) The new version of ESCAPE is a strategy for optimizing treatment and comprehensive care of critically ill patients (CIPs) after the rescue period, including early mobilization, early rehabilitation, nutritional support, sleep management, mental assessment, cognitive-function training, emotional support, and optimizing sedation and analgesia. (3) Disease assessment to determine the starting point of early mobilization, early rehabilitation, and early enteral nutrition. (4) Early mobilization has synergistic effects upon the recovery of organ function. (5) Early functional exercise and rehabilitation are important means to promote CIP recovery, and gives them a sense of future prospects. (6) Timely start of enteral nutrition is conducive to early mobilization and early rehabilitation. (7) The spontaneous breathing test should be started as soon as possible, and a weaning plan should be selected step-by-step. (8) The waking process of CIPs should be realized in a planned and purposeful way. (9) Establishment of a sleep-wake rhythm is the key to sleep management in post-CC management. (10) The spontaneous awakening trial, spontaneous breathing trial, and sleep management should be carried out together. (11) The depth of sedation should be adjusted dynamically in the late stage of CC period. (12) Standardized sedation assessment is the premise of rational sedation. (13) Appropriate sedative drugs should be selected according to the objectives of sedation and drug characteristics. (14) A goal-directed minimization strategy for sedation should be implemented. (15) The principle of analgesia must be mastered first. (16) Subjective assessment is preferred for analgesia assessment. (17) Opioid-based analgesic strategies should be selected step-by-step according to the characteristics of different drugs. (18) There must be rational use of non-opioid analgesics and non-drug-based analgesic measures. (19) Pay attention to evaluation of the psychological status of CIPs. (20) Cognitive function in CIPs cannot be ignored. (21) Delirium management should be based on non-drug-based measures and rational use of drugs. (22) Reset treatment can be considered for severe delirium. (23) Psychological assessment should be conducted as early as possible to screen-out high-risk groups with post-traumatic stress disorder. (24) Emotional support, flexible visiting, and environment management are important components of humanistic management in the intensive care unit (ICU). (25) Emotional support from medical teams and families should be promoted through"ICU diaries"and other forms. (26) Environmental management should be carried out by enriching environmental content, limiting environmental interference, and optimizing the environmental atmosphere. (27) Reasonable promotion of flexible visitation should be done on the basis of prevention of nosocomial infection. (28) ESCAPE is an excellent project for late stage of CC management.

OBJECTIVE@#To observe the clinical efficacy of Miao medicinal crossbow acupuncture therapy as adjuvant treatment for lung cancer pain based on oxycodone hydrochloride extended-release tablet.@*METHODS@#A total of 60 patients with lung cancer pain were randomized into an observation group (30 cases, 1 case dropped off) and a control group (30 cases). In the control group, oxycodone hydrochloride extended-release tablet was given orally, 10 mg a time, once every 12 hours. On the basis of the treatment in the control group, Miao medicinal crossbow acupuncture therapy was applied once every other day in the observation group. The treatment of 14 days was required in the two groups. Before and after treatment, the numerical rating scale (NRS) score, number of break-out pain and Karnofsky performance status (KPS) score were observed in the two groups. The equivalent oxycodone consumption and rate of adverse reactions were recorded, the analgesic effect was evaluated in the two groups.@*RESULTS@#Compared before treatment, the NRS scores and number of break-out pain were decreased while the KPS scores were increased after treatment in the two groups (P<0.01). After treatment, the NRS score and number of break-out pain in the observation group were lower than the control group (P<0.01), the KPS score in the observation group was higher than the control group (P<0.05). The equivalent oxycodone consumption of whole course and the rate of adverse reactions i.e. constipation, drowsiness, nausea and vomiting in the observation group were lower than the control group (P<0.05). The analgesic effect rate was 93.1% (27/29) in the observation group, which was superior to 63.3% (19/30) in the control group (P<0.05).@*CONCLUSION@#On the basis of oxycodone hydrochloride extended-release tablet, Miao medicinal crossbow acupuncture therapy as adjuvant treatment can effectively relieve the pain degree, reduce the number of break-out pain and improve the health status and quality of life in patients with lung cancer pain, enhance the efficacy of medication and reduce its adverse reactions.

To further standardize the sedation and analgesia treatment for neurocritical care patients, the National Center for Healthcare Quality Management in Neurological Diseases and Chinese Society of Critical Care Medicine organized national experts in this fields to form Working group of the Expert consensus on sedation and analgesia for neurocritical care patients in order to update the Expert consensus on sedation and analgesia for patients with severe brain injury (2013) based on evidence-based medicine. This update aims to provide scientific guidance for the clinical diagnosis and treatment of neurocritical care patients. The working group followed the definition of clinical practice guidelines by the Institution of Medicine (IOM) and the World Health Organization guidelines development handbook and Guidelines for the formulation/revision of clinical guidelines in China (2022) to register and draft the Expert consensus on sedation and analgesia for neurocritical care patients. The working group will strictly adhere to the consensus development process to formulate and publish the Expert consensus on sedation and analgesia for neurocritical care patients (2023). This protocol primarily introduces the development methodology and process of the Expert consensus on sedation and analgesia for neurocritical care patients (2023), including the purpose of the update, the target population, the composition of the consensus development working group, the presentation and collection of clinical questions, evidence evaluation and summarization, and the generation of recommended opinions. This will make the consensus development process more standardized and transparent.

The rostral agranular insular cortex (RAIC) has been associated with pain modulation. Although the endogenous cannabinoid system (eCB) has been shown to regulate chronic pain, the roles of eCBs in the RAIC remain elusive under the neuropathic pain state. Neuropathic pain was induced in C57BL/6 mice by common peroneal nerve (CPN) ligation. The roles of the eCB were tested in the RAIC of ligated CPN C57BL/6J mice, glutamatergic, or GABAergic neuron cannabinoid receptor 1 (CB1R) knockdown mice with the whole-cell patch-clamp and pain behavioral methods. The E/I ratio (amplitude ratio between mEPSCs and mIPSCs) was significantly increased in layer V pyramidal neurons of the RAIC in CPN-ligated mice. Depolarization-induced suppression of inhibition but not depolarization-induced suppression of excitation in RAIC layer V pyramidal neurons were significantly increased in CPN-ligated mice. The analgesic effect of ACEA (a CB1R agonist) was alleviated along with bilateral dorsolateral funiculus lesions, with the administration of AM251 (a CB1R antagonist), and in CB1R knockdown mice in GABAergic neurons, but not glutamatergic neurons of the RAIC. Our results suggest that CB1R activation reinforces the function of the descending pain inhibitory pathway via reducing the inhibition of glutamatergic layer V neurons by GABAergic neurons in the RAIC to induce an analgesic effect in neuropathic pain.

Although sympathetic blockade is clinically used to treat pain, the underlying mechanisms remain unclear. We developed a localized microsympathectomy (mSYMPX), by cutting the grey rami entering the spinal nerves near the rodent lumbar dorsal root ganglia (DRG). In a chemotherapy-induced peripheral neuropathy model, mSYMPX attenuated pain behaviors via DRG macrophages and the anti-inflammatory actions of transforming growth factor-β (TGF-β) and its receptor TGF-βR1. Here, we examined the role of TGF-β in sympathetic-mediated radiculopathy produced by local inflammation of the DRG (LID). Mice showed mechanical hypersensitivity and transcriptional and protein upregulation of TGF-β1 and TGF-βR1 three days after LID. Microsympathectomy prevented mechanical hypersensitivity and further upregulated Tgfb1 and Tgfbr1. Intrathecal delivery of TGF-β1 rapidly relieved the LID-induced mechanical hypersensitivity, and TGF-βR1 antagonists rapidly unmasked the mechanical hypersensitivity after LID+mSYMPX. In situ hybridization showed that Tgfb1 was largely expressed in DRG macrophages, and Tgfbr1 in neurons. We suggest that TGF-β signaling is a general underlying mechanism of local sympathetic blockade.
