Introducción: La fractura de Maisonneuve es una lesión caracterizada por la fractura subcapital del peroné asociada a una lesión capsuloligamentaria de tobillo. Su tratamiento supone la restauración de la anatomía ósea y capsuloligamentaria normal para restablecer las fuerzas de contacto tibioastragalinas fisiológicas. Esta calidad de reducción puede ser difícil de alcanzar, sobre todo, con técnicas percutáneas. Objetivo: Evaluar mediante imágenes la calidad de la reducción de fracturas de Maisonneuve reducidas en forma cerrada (bajo visualización directa artroscópica) y fijadas por vía percutánea. Materiales y Métodos: Se analizaron radiografías e imágenes de tomografía computarizada preoperatorias y posoperatorias, comparativas de los tobillos operado y sano. Resultados: Se evaluaron 13 fracturas. Los parámetros radiográficos posoperatorios (espacio claro medial, solapamiento tibioperoneo distal, espacio claro tibioperoneo) no registraron diferencias, excepto por el intervalo tibioperoneo anterior que aumentó en un caso. La medición tomográfica posoperatoria del solapamiento tibioperoneo distal y el intervalo tibioperoneo anterior reveló que todas las reducciones eran satisfactorias. Dos pacientes tenían valores alterados en la sindesmosis tibioperonea distal con diferencia >2 mm respecto del tobillo sano. Conclusión: Recomendamos la asistencia artroscópica para el manejo de la fractura de Maisonneuve como herramienta de control intraoperatorio para una fijación percutánea más segura. Introduction: Maisonneuve fracture (MF) is an injury characterized by the subcapital fracture of the fibula associated with a capsuloligamentous injury of the ankle. Treatment involves the restoration of normal bone and capsuloligamentous anatomy in order to reestablish physiological tibiotalar contact forces. This quality of reduction can be difficult to achieve, especially with percutaneous techniques. Objective: To evaluate the quality of reduction in Maisonneuve fractures reduced in a closed manner (under direct arthroscopic visualization) and fixed percutaneously. Materials and Methods: We analyzed comparative preoperative and postoperative radiographs and CT scans of the operated and healthy ankles. Results: 13 fractures were evaluated. Radiographic parameters of postoperative procedures (medial clear space, distal tibiofibular overlap, tibiofibular clear space) did not register differences except for the anterior tibiofibular space, which had increased in 1 case. Postoperative tomographic measurements of tibiofibular clear space and anterior tibiofibular space showed 100% satisfactory reductions. Two patients presented altered distal tibiofibular overlap values with a difference greater than 2 mm compared to the healthy ankle. Conclusion: We recommend arthroscopy for the management of MF as an intraoperative control tool for safer percutaneous fixation. Level of Evidence: IV

The discoid meniscus is a common congenital meniscal malformation that is prevalent mainly in Asians and often occurs in the lateral discoid meniscus. Patients with asymptomatic discoid meniscus are usually treated by conservative methods such as observation and injury avoidance, while patients with symptoms and tears need to be treated surgically. Arthroscopic saucerization combined with partial meniscectomy and meniscus repair is the most common surgical approach., and early to mid-term reports are good. The prognostic factors are the patient's age at surgery、follow-up time and type of surgery. Some patients experience complications such as prolonged postoperative knee pain, early osteoarthritis, retears and Osteochondritis dissecans. The incidence of prolonged postoperative knee pain was higher and the incidence of Osteochondritis dissecans was the lowest. Retears of the lateral meniscus is the main reason for reoperation.

OBJECTIVE@#To investigate the efficacy and clinical results of total internal protection technique in anterior cruciate ligament reconstruction.@*METHODS@#A total of 56 patients undergoing anterior cruciate ligament reconstruction treated from January 2018 to December 2019 were selected. According to the different surgical methods, they were divided into total internal reconstruction group and standard bone tunnel group. There were 21 patients in the total internal reconstruction group, including 15 males and 6 females, aged from 20 to 48 with an average of (35.6±6.7) years old, and 35 patients in the standard tibial tunnel group, including 26 males and 9 females, aged 22 to 51 years old with an average of (33.7±9.6) years old. Preoperative examination of Lachman test was positive, magnetic resonance indicated anterior cruciate ligament rupture. There were no significant differences between the two groups in age, sex, body mass index, time from injury to ACL reconstruction, combined meniscus injury and operation method, operation time, ligament diameter, ligament length and other general information. Postoperative evaluation included operation duration, length and diameter of transplanted tendon after braid. International Knee Documentation Committee (IKDC) score, Lysholm score, Tegner score and perioperative complications 2 years after surgery.@*RESULTS@#Both groups were followed up, ranging from 24 to 30 months with an average of (26.9±3.4) months. Postoperative incision healing was good, and no failure or joint infection occurred at the last follow-up. There was no statistically significant difference between the two groups in IKDC score, Lysholm score and Tegner score before, 1 year and 2 years after surgery. However, IKDC score, Lysholm score and Tegner score at 1 year and 2 years after surgery.@*CONCLUSION@#The same postoperative function and stability of knee joint can be obtained by both the residual whole technique and the standardized reconstruction technique. In the residual whole group, only the semitendinosus muscle is taken, and the femoral thin muscle is retained, with greater tibial bone mass preserved, which is safe and effective in clinical practice.

OBJECTIVE@#To investigate short-term effectiveness of arthroscopic repair via modified subacromial viewing portal (hereinafter referred to as modified viewing portal) in treatment of LafosseⅠsubscapularis tendon tears.@*METHODS@#A clinical data of 52 patients with LafosseⅠsubscapularis tendon tears, who underwent the arthroscopic repair via modified viewing portal between October 2020 and November 2022 and met the selective criteria, was retrospectively analyzed. There were 15 males and 37 females with an average age of 63.4 years (range, 41-76 years). Twelve patients had trauma history and the other 40 patients had no obvious inducement. The main clinical symptom was shoulder pain and the hug resistance tests were positive in all patients. The interval between symptom onset and admission ranged from 3 to 26 months (mean, 7.2 months). The shoulder pain and function were evaluated by visual analogue scale (VAS) score, American Shoulder and Elbow Surgeons (ASES) score, and University of California Los Angeles (UCLA) score before operation and at 12 months after operation. The shoulder range of motion (ROM) of forward flexion, abduction, and external rotation and the internal rotation strength were measured before operation and at 3 and 12 months after operation. MRI was performed at 3-6 months after operation to assess the tendon healing and the structural integrity and tension of reattached tendon. Patient's satisfactions were calculated at last follow-up.@*RESULTS@#All incisions healed by first intention, no complication such as incision infection or nerve injury occurred. All patients were followed up 12-37 months (mean, 18.5 months). The VAS, UCLA, and ASES scores at 12 months after operation significantly improved when compared with those before operation ( P<0.05). The ROMs of abduction and forward flexion and the internal rotation strength at 3 and 12 months significantly improved when compared with those before operation ( P<0.05); and the ROMs at 12 months significantly improved compared to that at 3 months ( P<0.05). However, there was no significant difference ( P>0.05) in the ROM of external rotation at 3 months compared to that before operation; but the ROM at 12 months significantly improved compared to that before operation and at 3 months after operation ( P<0.05). Thirty-one patients underwent MRI at 3-6 months, of which 28 patients possessed intact structural integrity, good tendon tension and tendon healing; 3 patients underwent tendon re-tear. At last follow-up, 41 patients (78.8%) were very satisfied with the effectiveness, 7 were satisfied (13.5%), and 4 were dissatisfied (7.7%).@*CONCLUSION@#Arthroscopic repair via modified viewing portal for Lafosse Ⅰsubscapularis tendon tears, which can achieve the satisfactory visualization and working space, can obtain good short-term effectiveness with low overall re-tear risk.

Abstract Objective To validate the low-cost model for arthroscopy training and analyze the acceptance and usefulness of the developed simulator in medical teaching and training. Method Ten medical students, ten third-year orthopedic residents, and ten shoulder surgeons performed predetermined tasks on a shoulder simulator twice. The parameters used were time to complete the tasks, number of looks at the hands, GOALS score (Global Operative Assessment of Laparoscopic Skills) and comparison between groups and within groups. An adapted Likert scale was applied addressing the individuals' impressions about the simulator and its applicability. Results In the intergroup comparison, the shoulder surgeons had better scores and times than the other groups. When the tasks were repeated, the group of surgeons had a 59% improvement in time (p < 0.05), as did the group of medical students. In the GOALS score, shoulder surgeons had consistently better scores than the other groups. And when we evaluated the evolution from the first to the second test, the group of surgeons and the group of academics had a statistically significant improvement (p < 0.05). In terms of lookdowns, there was a decrease in all groups. There was consensus that the simulator is useful in training. Conclusion The simulator developed allowed the differentiation between individuals with different levels of training in arthroscopic surgery. It was accepted by 100% of the participants as a useful tool in arthroscopic shoulder surgical training.


Resumo Objetivo Validar o modelo de baixo custo para treinamento em artroscopia e analisar a aceitação e utilidade do simulador desenvolvido no ensino e treinamento médico. Método Dez acadêmicos do curso de medicina, dez residentes do terceiro ano em ortopedia e dez cirurgiões de ombro realizaram tarefas pré determinadas em um simulador de ombro duas vezes. Os parâmetros utilizados foram o tempo para completar as tarefas, quantidade de olhares para as mãos, escore de GOALS (Global Operative Assessment of Laparoscopic Skills) e comparados entre os grupos e intragrupos. Uma escala de Likert adaptada foi aplicada abordando as impressões dos indivíduos acerca do simulador e de sua aplicabilidade. Resultados Na comparação intergrupos, os cirurgiões de ombro tiveram melhores escores e tempos que os demais grupos. Quando as tarefas foram repetidas, o grupo de cirurgiões, teve uma melhora de 59% no tempo (p < 0,05), assim como no grupo de acadêmicos. No escore de GOALS os cirurgiões de ombro apresentaram escores consistentemente melhores que os demais grupos. E quando avaliamos a evolução do primeiro para o segundo teste, o grupo de cirurgiões e o grupo de acadêmicos tiveram melhora estatisticamente significante (p < 0,05). No quesito de lookdowns houve diminuição em todos os grupos. Houve consenso em que o simulador é útil no treinamento. Conclusão O simulador desenvolvido permitiu a diferenciação entre indivíduos com diferentes níveis de treinamento em cirurgia artroscópica. Foi aceito por 100% dos participantes como uma ferramenta útil no treinamento cirúrgico artroscópico do ombro.

Objetivo: Apresentar as modalidades de tratamentos cirúrgicas mais usadas disponíveis no arsenal terapêutico das desordens temporomandibulares (DTMs). Revisão da literatura: As DTMs são muito frequentes e são responsáveis ​​por dor e desconforto em um número importante de pacientes. A avaliação e o diagnóstico são as chaves para determinar um plano de manejo adequado dessas doenças. Embora o tratamento conservador seja bem-sucedido na maioria dos pacientes, os tratamentos cirúrgicos podem ser a única opção para aqueles que não respondem ao tratamento conservador ou para casos com indicação cirúrgica inicial como, por exemplo, algumas neoplasias articulares. Dentre as alternativas cirúrgicas, podemos citar a artrocentese, artroscopia, reposicionamento do disco articular por cirurgia aberta, discectomia e tratamentos cirúrgicos para hipermobilidade e anquilose da articulação temporomandibular. Considerações finais: A seleção adequada dos casos é requisito obrigatório para uma intervenção cirúrgica bem-sucedida, a fim de alcançar o resultado desejado do tratamento, como alívio dos sintomas e melhora da função.


Aim: To present the most commonly used surgical treatment modalities available in the therapeutic arsenal for temporomandibular disorders (TMD). Literature review: TMD is very common and is responsible for pain and dysfunction in a significant number of patients. Assessment and diagnosis are key to determining a management plan for these diseases. Although conservative treatment is successful in most patients, surgical treatments may be the only option for those who do not respond to conservative treatment or for some cases with an initial surgical indication, such as some joint neoplasms. Surgical alternatives include arthrocentesis, arthroscopy, repositioning of the articular disc by open surgery, discectomy and surgical treatments for temporomandibular joint hypermobility and ankylosis. Conclusions: Proper case selection is the mandatory requirement for successful surgical intervention in order to achieve the desired treatment outcome, such as symptom relief and improved function.

Disfunção Temporomandibular é um termo utilizado para indicar alterações e doenças das articulações temporomandibulares (ATM) e estruturas anexas responsáveis por sua função. Prevalente na população geral, seus sintomas causam desconforto e limitação da função mandibular, repercutindo diretamente na qualidade de vida de seus portadores. Dentre as alterações que compõem este grupo de doenças, destacam-se os desarranjos internos da ATM, para os quais o tratamento cirúrgico pode estar indicado. A Artroscopia da ATM, procedimento diagnóstico e terapêutico caracterizado pela introdução de óticas de pequeno calibre nos compartimentos articulares vem ganhando espaço e indicação como primeira opção terapêutica devido aos seus bons resultados e mínima invasividade. No entanto, sua dificuldade técnica, alto custo, necessidade de instrumentais específicos e sistema de videocirurgia ainda são fatores limitadores para a indicação desta pelos profissionais atuantes. Nosso objetivo foi validar, por meio de um estudo com simuladores realísticos validados e 10 examinadores qualificados de diferentes níveis de atuação, o uso de plataforma de adaptação de ótica para smartphone em artroscopia da ATM, afim de qualificar a resolução das imagens obtidas e grau de ergonomia durante seu uso por meio de classificação em três scores. Como resultados, obtivemos em geral, para qualidade da imagem 77,5% de nota máxima e 22,5% nota intermediaria e para ergonomia 62,5% de nota máxima e 37,5% de nota intermediaria. Concluímos que a plataforma é viável para a indicação de artroscopia de ATM em nível I e possível de ser utilizada em nível II.

Introducción: A pesar de su baja incidencia, las infecciones osteoarticulares pueden generar complicaciones y secuelas devastadoras para el niño en desarrollo con su esqueleto en crecimiento. El manejo general de la patología debe ser multidisciplinario, asociando la terapia antibiótica y un tratamiento quirúrgico adecuado si se requiere. El objetivo de nuestro trabajo es analizar los resultados sobre las diferentes opciones terapéuticas quirúrgicas (punción-aspiración, lavado artroscópico o lavado abierto) en el tratamiento de la artritis séptica en la edad, mediante la realización de revisión sistematizada de la bibliografía. Metodología: Realizamos una búsqueda bibliográfica online en los buscador PubMed y en la Biblioteca Virtual en Salud (BVS) utilizando los siguientes términos MESH: ("Arthritis, Infectious/surgery"(Mesh) OR "Arthritis, Infectious/therapy"(Mesh)), utilizando filtros de búsqueda y aplicando criterios de inclusión y exclusión según nuestro objetivo. De los artículos incluidos se obtuvieron entre otros los siguiente datos: país, año, nivel de evidencia, número de pacientes con artritis séptica, articulación afectada, edad media, seguimiento promedio, tipo de tratamiento quirúrgico realizado de inicio y sus resultados según cada autor : complicaciones (de la enfermedad y del procedimiento ) y/o secuelas (en base a evolución clínica y radiológica), necesidad de un tratamiento quirúrgico adicional ante la falla del método terapéutico inicial y además de conclusiones relevantes de cada autor. Resultados: Se incluyeron un total de 22 artículos: 8 para el lavado por artroscopia, 6 para el lavado por artrocentesis, 4 para el lavado por artrotomía, ademas de otros 4 artículos que realizaba un análisis comparativo: dos de ellos entre lavado artroscópico y lavado por artrotomía y los otros dos entre lavado por artrocentesis y lavado abierto. El lavado artroscópico se encuentra en lo más alto de nuestra revisión ya que presenta los mejores resultados con solo un 6% complicaciones y/o secuelas (en solo 4 de los 8 artículos incluidos), con un total de 13 pacientes (8,7% ) que requirieron un tratamiento adicional (5 artrotomías y 8 un nueva lavado artroscópico ) y se destacan ademas 3 artículos con un 100% de buenos resultados, en los que no se presentaron complicaciones y/o secuelas, ni se requirió un segundo lavado, mientras que solo en 3 trabajos, de los 8, se necesitó de la artrotomía adicional para la resolución final. Para el lavado por artrocentesis se obtuvo el menor porcentaje de complicaciones y/o secuelas con un 2%, (reportado en tres de los 6 trabajos), y solo el 9,7% necesitaron de un segundo método terapéutico diferente a la aspiración articular (23 artrotomías y 2 artroscopias), destacando ademas la repetición de la punción (hasta 4 veces) como dentro de la directiva terapéutica del autor en dos artículos, y que para éste método se encontró el mayor número de pacientes. El lavado por artrotomía como método inicial obtuvo los peores resultados, con un 12% de complicaciones y/o secuelas y un 22,6% de lavados adicionales. Conclusiones: A pesar de que el lavado abierto por artrotomía se toma como el ¨ gold-standard ¨, teniendo indicaciones absolutas en el tratamiento quirúrgico de la artritis séptica en niños (especialmente en cadera), de nuestro análisis se desprende que ésta opción arroja los peores porcentajes de resultados, duplicando el porcentaje de necesidad de lavado adicional y el de complicaciones, en comparación con el lavado artroscópico. Existen otros métodos terapéuticos que se presentan como procedimientos menos invasivos, seguros y eficaces, respaldados por buenos resultados en la biografiaría, como son: el lavado por artroscopia (que presentó el porcentaje más bajo de pacientes con necesidad de lavado adicional, con el mayor número de artículos con buenos resultados en general, con un porcentaje de complicaciones por debajo del lavado por artrotomía) y el lavado por artrocentesis (que presentó el porcentaje de compilaciones más bajo de las 3 opciones analizadas.


Introduction: Despite its low incidence, osteoarticular infections can generate devastating complications and sequelae for the developing child with his growing skeleton. The general management of the pathology must be multidisciplinary, associating antibiotic therapy and adequate surgical treatment if required. The objective of our work is to analyze the results on the different surgical therapeutic options (puncture-aspiration, arthroscopic lavage or open lavage) in the treatment of septic arthritis in the pediatric age, by carrying out a systematic review of the bibliography. Methodology: We conducted an online bibliographic search in the PubMed search engine and in the Virtual Health Library (VHL) using the following MESH terms: ("Arthritis, Infectious/surgery"(Mesh) OR "Arthritis, Infectious/therapy"(Mesh)), using search filters and applying inclusion and exclusion criteria according to our objective. The following data were obtained from the included articles, among others: country, year, level of evidence, number of patients with septic arthritis, affected joint, mean age, average follow-up, type of initial surgical treatment and its results according to each author: complications (of the disease and of the procedure) and/or sequelae (based on clinical and radiological evolution), the need for additional surgical treatment in the event of failure of the initial therapeutic method, and in addition to relevant conclusions of each author. Results: A total of 22 articles were included: 8 for arthroscopic lavage, 6 for arthrocentesis lavage, 4 for arthrotomy lavage, in addition to another 4 articles that performed a comparative analysis: two of them between arthroscopic lavage and lavage. by arthrotomy and the other two between lavage by arthrocentesis and open lavage. Arthroscopic lavage is at the top of our review as it presents the best results with only 6% complications and/or sequelae (in only 4 of the 8 articles included), with a total of 13 patients (8.7% ) that required additional treatment (5 arthrotomies and 8 a new arthroscopic lavage) and also highlight 3 articles with 100% good results, in which there were no complications and/or sequelae, nor did a second lavage be required, while that only 3 of the 8 studies required an additional arthrotomy for the final resolution. For lavage by arthrocentesis, the lowest percentage of complications and/or sequelae was obtained with 2% (reported in three of the 6 works), and only 9.7% required a second therapeutic method other than joint aspiration (23 arthrotomies and 2 arthroscopies), also highlighting the repetition of the puncture (up to 4 times) as within the author's therapeutic directive in two articles, and that the largest number of patients was found for this method. Arthrotomy lavage as the initial method obtained the worst results, with 12% complications and/or sequelae and 22.6% additional lavages. Conclusions: Even though open lavage by arthrotomy is taken as the ¨ gold-standard ¨, having absolute indications in the surgical treatment of septic arthritis in children (especially in the hip), from our analysis it can be deduced that this option yields the worse percentages of results, doubling the percentage of need for additional lavage and the percentage of complications, compared to arthroscopic lavage. There are other therapeutic methods that are presented as less invasive, safe and effective procedures, supported by good results in the biography, such as: arthroscopic lavage (which presented the lowest percentage of patients requiring additional lavage, with the highest number of articles with good results in general, with a percentage of complications below lavage by arthrotomy) and lavage by arthrocentesis (which presented the lowest percentage of compilations of the 3 options analyzed).


Introdução: Apesar de sua baixa incidência, as infecções osteoarticulares podem gerar complicações e sequelas devastadoras para a criança em desenvolvimento com seu esqueleto em crescimento. O manejo geral da patologia deve ser multidisciplinar, associando antibioticoterapia e tratamento cirúrgico adequado, se necessário. O objetivo do nosso trabalho é analisar os resultados das diferentes opções terapêuticas cirúrgicas (punção-aspiração, lavagem artroscópica ou lavagem aberta) no tratamento da artrite séptica em idade pediátrica, através da realização de uma revisão sistemática da bibliografia. Metodologia: Foi realizada uma busca bibliográfica online no buscador PubMed e na Biblioteca Virtual em Saúde (BVS) utilizando os seguintes termos MESH: ( "Arthritis, Infectious/surgery"(Mesh) OR "Arthritis, Infectious/therapy"(Mesh) ), utilizando filtros de busca e aplicando critérios de inclusão e exclusão de acordo com nosso objetivo. Os seguintes dados foram obtidos dos artigos incluídos, entre outros: país, ano, nível de evidência, número de pacientes com artrite séptica, articulação afetada, idade média, seguimento médio, tipo de tratamento cirúrgico inicial e seus resultados de acordo com cada autor: complicações (da doença e do procedimento) e/ou sequelas (com base na evolução clínica e radiológica), necessidade de tratamento cirúrgico adicional em caso de falha do método terapêutico inicial e, além das conclusões relevantes de cada autor. Resultados: Foram incluídos 22 artigos: 8 para lavagem artroscópica, 6 para lavagem artrocentese, 4 para lavagem artrotômica, além de outros 4 artigos que realizaram uma análise comparativa: dois deles entre lavagem artroscópica e lavagem por artrotomia e a outros dois entre lavagem por artrocentese e lavagem aberta. A lavagem artroscópica está no topo da nossa revisão por apresentar os melhores resultados com apenas 6% de complicações e/ou sequelas (em apenas 4 dos 8 artigos incluídos), com um total de 13 doentes (8,7%) que necessitaram de tratamento adicional (5 artrotomias e 8 uma nova lavagem artroscópica) e também destacam 3 artigos com 100% de bons resultados, nos quais não houve complicações e/ou sequelas, nem foi necessária uma segunda lavagem, enquanto que apenas 3 dos 8 estudos necessitaram de uma artrotomia para a resolução final. Para a lavagem por artrocentese, o menor percentual de complicações e/ou sequelas foi obtido com 2% (relatado em três dos 6 trabalhos), e apenas 9,7% necessitaram de um segundo método terapêutico diferente da aspiração articular (23 artrotomias e 2 artroscopias), destacando também a repetição da punção (até 4 vezes) conforme diretriz terapêutica do autor em dois artigos, e que o maior número de pacientes foi encontrado para este método. A lavagem por artrotomia como método inicial obteve os piores resultados, com 12% de complicações e/ou sequelas e 22,6% de lavagens adicionais. Conclusões: Apesar de a lavagem aberta por artrotomia ser tida como ¨padrão-ouro¨, tendo indicações absolutas no tratamento cirúrgico da artrite séptica em crianças (especialmente no quadril), da nossa análise pode-se deduzir que esta opção rende os piores percentuais de resultados, dobrando o percentual de necessidade de lavagem adicional e o percentual de complicações, em comparação com a lavagem artroscópica. Existem outros métodos terapêuticos que se apresentam como procedimentos menos invasivos, seguros e eficazes, corroborados por bons resultados na biografia, tais como: a lavagem artroscópica (que apresentou a menor percentagem de doentes que necessitaram de lavagem adicional, com o maior número de artigos com boa resultados em geral, com percentual de complicações abaixo da lavagem por artrotomia) e lavagem por artrocentese (que apresentou o menor percentual de compilações das 3 opções analisadas).

Introducción: El tratamiento artroscópico del síndrome de fricción femoroacetabular (SFFA) en displasia de cadera es controversial. Inicialmente, algunos estudios demostraron una tasa elevada de fallas, mientras que otros más recientes describieron una mejoría clínica comparable con pacientes sin falta de cobertura acetabular. El propósito de este estudio fue comparar los resultados clínicos y funcionales de la artroscopía de cadera en dos cohortes: pacientes con displasia borderline y pacientes con ángulo centro-borde normal. Materiales y métodos: evaluamos los resultados clínicos y funcionales utilizando el Harris Hip Score (HHS), Hip Outcome Score (HOS) y l Escala Visual Análoga del Dolor (EVA) del tratamiento artroscópico del SFFA para dos grupos de pacientes: el grupo 1 conformado por aquellos que presentaban displasia borderline (DB) y el grupo 2, compuesto por pacientes con un valor del ángulo centro-borde normal (ACBN). Resultados: los valores postoperatorios de las escalas de HHS y EVA no mostraron diferencias estadísticamente significativas entre los grupos (87.0 ± 5.3 versus 85.8 ± 3.6; p = 0.200 y 1.5 ± 0.6 versus 1.3 ± 0.5; p = 0.07, respectivamente).No se observaron diferencias significativas con respecto a las actividades de la vida diaria del score de HOS (91.8 ± 6.6 versus 93.2 ± 5.9; p = 0.28), ni de deportes, (85.1 ± 7.7 ± 8.3 versus 88.3 ± 11.9; p = 0.19). Conclusión: los pacientes sometidos a una artroscopía de cadera con displasia borderline, alcanzan resultados clínicos y funcionales similares que aquellos con ACBN con una media de seguimiento de tres años. Introduction: Arthroscopic treatment of femoral-acetabular impingement syndrome in patients with hip dysplasia is controversial. There are some reports that observed an increased failure rate in this type of patients. More recent studies described good patients clinical and functional outcomes, comparable with patients with a normal acetabular coverage. The purpose of this study was to assess functional and clinical outcomes of arthroscopic treatment of FAI in two cohorts: patients with Borderline Dysplasia and patients with a normal lateral center-edge angle. Materials and methods: we assessed patients reported outcomes of two groups of patients: group 1 that consisted in patients with Borderline Dysplasia and group 2, with patients with a normal lateral center-edge angle. The minimum follow-up required was three years. Results: there were no statistically significant differences regarding Harris Hip Score and Visual Analogue Scale of Pain respectively (87.0 ± 5.3 versus 85.8 ± 3.6; p = 0.200 y 1.5 ± 0.6 versus 1.3 ± 0.5; p = 0.07) after surgery between both groups. La condromatosis sinovial es una rara enfermedad benigna del tejido sinovial que puede causar daño articular si no se trata adecuadamente. Mientras que la lesión tipo cam provoca una fricción entre el borde acetabular y la unión cuello-cabeza femoral que puede generar un daño condrolabral y, en su evolución natural, llegar a la artrosis. El tratamiento de la condromatosis sinovial de cadera es controvertido, entre la cirugía abierta o artroscópica, pero debe incluir la extracción completa de los cuerpos libres y la sinovectomía para evitar recurrencias. Por el contrario, la lesión tipo cam puede manejarse con artroscopia en la mayoría de los casos. Presentamos un caso clínico en el que se asocian ambas patologías y que fue tratado mediante luxación controlada de cadera. La elección de la luxación controlada permite una visión completa de la articulación y se ha utilizado con éxito en casos de condromatosis sinovial asociada a la lesión tipo cam. Synovial chondromatosis is a rare benign disease of the synovial tissue that can cause joint damage if not properly treated. On the other hand, cam deformity causes friction between the acetabular rim and the femoral neck-head junction, which can lead to chondrolabral damage and, in its natural progression, result in osteoarthritis. The treatment of synovial chondromatosis of the hip is controversial, involving open surgery or arthroscopy, but it should include complete removal of loose bodies and synovectomy to prevent recurrences. In contrast, a cam lesion can often be managed with arthroscopy. We present a clinical case where both conditions were associated and treated with controlled hip dislocation. The choice of controlled dislocation allows for a comprehensive view of the joint and has been successfully used in cases of synovial chondromatosis associated with cam lesions. El tratamiento de las fracturas de clavícula distal se puede lograr a través de varias opciones. Las placas de bloqueo asociadas con los botones coracoclaviculares son una alternativa para estabilizar las lesiones de ligamentos asociadas con las fracturas de tipo IIB de Craig. La artroscopía puede ayudar a colocar el botón subcoracoideo. Presentamos el caso de una mujer de treinta y cinco años quien, al caer de una bicicleta, resultó con traumatismo directo en el hombro izquierdo. Las radiografías mostraron fractura conminuta de la clavícula distal asociada con lesiones de ligamentos (tipo IIB de Craig). Se optó por tratamiento quirúrgico con placa bloqueada con un botón pegado asociado a otro botón subcoracoideo asistido por artroscopía. A través de una vía de acceso directo sobre la clavícula mediolateral, se redujeron anatómicamente los fragmentos de fractura y se fijaron con una placa bloqueada para la clavícula distal y siete tornillos. Se realizó portal artroscópico posterior para visualizar la articulación. Mediante portal anterior se expusieron el espacio rotador y la región subcoracoidea. Con la ayuda de guías especiales se instalaron dos botones (uno subcoracoideo y otro pegado a la placa) conectados por cables especiales. Tres meses después de la cirugía, la paciente estaba sin dolor y volvió a practicar deportes al nivel previo a la lesión. Se logró un ROM normal y radiografías con la fractura anatómicamente consolidada y distancia coracoclavicular normal. Nivel de Evidencia: IV


The treatment of distal clavicle fractures can be accomplished through several options. Locking plates associated with coracoclavicular buttons are an alternative to stabilize ligament injuries associated with Craig's type IIB fractures. Arthroscopy can assist in positioning the subcoracoid dog button.We present the case of a thirty-five-year-old female fell from a bicycle with direct trauma to her left shoulder. Radiographs showed comminuted fracture of the distal clavicle associated with ligament injuries (Craig's type IIB). We opted for surgical treatment with a locked plate with a coupled button associated with a subcoracoid dog bone, assisted by arthroscopy. Through a direct region route over the mediolateral clavicle, the fracture fragments were anatomically reduced and fixed with a locked plate for the distal clavicle and seven screws. Posterior arthroscopic portal was performed to visualize the joint. Anterior portal was performed and the rotator space and subcoracoid region was exposed. With the aid of special guides, two buttons were installed (one sub-coracoid button and other attached to the plate) connected by special wires ("tapes"). Three months after the surgery, the patient was pain-free and returned to sports at the level prior to the injury. Normal ROM was achieved and radiographs with the anatomically consolidated fracture and normal coracoclavicular distance. Level of Evidence: IV

El labrum acetabular es una estructura fibrocartilaginosa análoga a los meniscos, labrum glenoideo o fibrocartílago triangular. Cumple diferentes funciones biomecánicas como sellado articular, estabilidad articular, resistencia a la traslación, distribución de presiones, etc. En 2003 se describe que el pinzamiento femoroacetabular y la lesión labral son una de las causas de osteoartritis de la cadera. Existen múltiples clasificaciones para lesiones labrales, sin embargo, la de MAHORN incluye en su tipo II al labrum hipoplásico. La literatura define como labrum acetabular hipoplásico cuando el ancho es ≤5 mm, y puede ser de tipo primario (variante anatómica) o secundario (iatrogénica por desbridamiento previo). Existe un amplio espectro en el tratamiento de las lesiones labrales, que dependerá principalmente del tamaño del labrum, el patrón de la lesión, las características del tejido y la zona afectada; desde desbridamiento hasta reconstrucción labral con el fin de preservar la función biomecánica normal de la cadera y evitar el desarrollo de osteoartritis a largo plazo.A continuación, se describe el caso de un paciente de sexo masculino, de veintiocho años, con antecedente de pinzamiento femoroacetabular mixto bilateral y labrum acetabular hipoplásico bilateral, sometido a reconstrucción primaria de labrum derecho con aloinjerto cadavérico de peroneus longus e izquierdo con aloinjerto cadavérico de tendo Achillis, para el que se obtuvo un resultado clínico y funcional favorable. The acetabular labrum is a fibrocartilaginous structure analogous to the menisci, glenoid labrum, or triangular fibrocartilage. It fulfills different biomechanical functions, such as: joint sealing, joint stability, resistance to translation, pressure distribution, etc. In 2003 it was described that femoroacetabular impingement and labral injury is one of the causes of hip osteoarthritis. There are multiple classifications for labral lesions, however the MAHORN classification includes the hypoplastic labrum in its type II. The literature defines a hypoplastic acetabular labrum when its width is ≤5 mm, and it can be primary (anatomical variant) or secondary (iatrogenic due to previous debridement).There is a wide spectrum in the treatment of labral lesions, which mainly depends on the size of the labrum, the pattern of the lesion, the characteristics of the tissue and the affected area; from debridement to labral reconstruction in order to preserve the normal biomechanical function of the hip and avoid the development of osteoarthritis in the long term.The case of a 28-year-old male patient is described below, with a history of bilateral mixed-type femoroacetabular impingement and bilateral hypoplastic acetabular labrum, who underwent primary reconstruction of the right labrum with peroneus longus cadaveric allograft and left labrum with Achilles tendon cadaveric allograft, obtaining a favorable clinical and functional outcome. Se presenta el caso de un paciente de veinte años a quien le queda un fragmento de la ceja acetabular posterior interpuesto dentro de la articulación tras la reducción de una luxación traumática de cadera. Se planteó un tratamiento por vía miniinvasiva anterior, con artroscopía seca con el fin de retirar el fragmento interpuesto, y la limpieza de la avulsión del ligamento redondo. En mesa de tracción se pudo retirar el fragmento interpuesto y evaluar el estado articular. Se muestran excelentes resultados clínicos e imagenológicos que desafían el concepto del abordaje posterior para lesiones posteriores. Nivel de Evidencia: III


The case of a twenty-year-old patient who has a fragment of the posterior acetabular rim interposed within the joint after reduction of a traumatic hip dislocation is presented. Anterior minimally invasive treatment was proposed, with dry arthroscopy to remove the interposed fragment and clean the round ligament avulsion. On the traction table, the interposed fragment could be removed, and the joint status evaluated. Excellent clinical and imaging results are shown what challenges the concept of a posterior approach for posterior lesions. Level of Evidence: III

A pesar de ello, se presentan molestias y dolor en la región supraclavicular, los que son motivo de consulta postquirúrgica y necesidad de revisión para retiro.Presentamos el caso de un paciente de sexo masculino, de treinta y nueve años, quien luego de una caída desde su bicicleta sufre una luxación acromioclavicular tipo V, la que fue tratada mediante sistema de tenosuspensión coracoclavicular y cerclaje acromioclavicular asistida por artroscopía, con buenos resultados. Treatment of acromioclavicular dislocations seeks to reduce and control vertical, horizontal, and rotational instability. The use of tenosuspension systems with buttons offers good results. Despite this, there is discomfort and pain in the supraclavicular region, which are the reason for post-surgical consultation and the need for revision for removal.We present a case of a thirty-nine-year-old male patient who suffered a type V acromioclavicular dislocation after falling from a bicycle, which was treated with a coracoclavicular tenosuspension system and arthroscopically assisted acromioclavicular cerclage with good results. Introducción: El objetivo de este estudio fue comparar la eficacia de los bloqueos interescalénico y supraescapular, solos y combinados, como analgesia posoperatoria en las primeras 3 horas tras la reparación artroscópica del manguito rotador. Materiales y métodos: Estudio de cohorte comparativo retrospectivo, realizado entre 2019 y 2021. El criterio de valoración principal fue el puntaje del dolor de hombro en la sala de recuperación evaluado con una escala analógica visual por el paciente. Los criterios de valoración secundarios fueron el consumo de opioides en la sala de recuperación y las complicaciones de la anestesia locorregional. Resultados: Se incluyó a 175 pacientes, 13 en el grupo de bloqueo interescalénico, 61 en el grupo de bloqueos interescalénico más supraescapular y 101 en el grupo de bloqueo supraescapular. Los grupos de bloqueo interescalénico y de bloqueo interescalénico más supraescapular tuvieron significativamente menos dolor en la sala de recuperación y una tasa total menor de opioides consumidos en miligramos equivalentes de morfina que el grupo de bloqueo supraescapular (p = 0,001 y p <0,01, respectivamente). No hubo diferencias significativas en el dolor ni el consumo de opioides entre el bloqueo interescalénico solo o combinado con bloqueo supraescapular. Conclusiones: El bloqueo interescalénico fue más eficaz que el bloqueo supraescapular para aliviar el dolor y disminuir el consumo de opioides en la sala de recuperación tras la reparación artroscópica del manguito rotador. La combinación de bloqueo interescalénico más bloqueo supraescapular no resultó en un incremento de la eficacia, y se sugiere no combinar estas dos técnicas. Nivel de evidencia: III


Introduction: This study aimed to compare the efficacy of interscalene block (ISB) and suprascapular nerve block (SSNB), individually and in combination (ISB+SSNB), used as postoperative analgesia within the first 3 hours after arthroscopic rotator cuff repair. Materials and methods: Retrospective comparative cohort study, conducted between 2019 and 2021. The primary endpoint was shoulder pain score in the immediate postoperative period as reported on a visual analog scale (VAS) by the patient. Secondary endpoints were opioid use in the recovery room (first 3 hours) and locoregional anesthesia complications. Results:175 patients were included; 13 in the ISB group, 61 in the ISB+SSNB group, and 101 in the SSNB group. The ISB group and the ISB+SSNB group had significantly less pain in the recovery room than the SSNB group (p = 0.001 and p < 0.001, respectively). The percentage of patients who required at least one dose of opioid and the total number of opioids consumed in milligrams of morphine equivalent were significantly lower for the ISB and ISB+SSNB groups than for the SSNB group (p < 0.001). There were no significant differences in pain or opioid use between ISB alone or combined with SSNB (ISB+SSNB). Conclusions: In this retrospective comparative study, ISB was more effective in relieving pain and reducing opioid use in the recovery room after ar-throscopic rotator cuff repair than SSNB. The ISB+SSNB combination did not increase effectiveness, and therefore it is suggested not to combine these two techniques. .Level of Evidence: III

La neuropatía del nervio supraescapular es una causa poco común de dolor y debilidad en el hombro y, por lo tanto, a menudo, es mal diagnosticada. Como consecuencia, el diagnóstico erróneo puede llevar a indicar un tratamiento conservador inapropiado o un procedimiento quirúrgico fallido. Se presenta el caso de un hombre de 55 años con síndrome de atrapamiento del nervio supraescapular. El paciente fue sometido a una artroscopia de hombro y se le diagnosticó una compresión del nervio supraescapular por el ligamento transverso escapular superior. La liberación artroscópica del nervio supraescapular alivió el dolor, la debilidad y la atrofia de los músculos supraespinoso e infraespinoso. Nivel de Evidencia: IV


Suprascapular neuropathy is a rare cause of shoulder pain and weakness and is therefore often misdiagnosed. As a consequence, misdiagnosis may lead to inappropriate conservative treatment or a failed surgical procedure. A case of a 55-year-old man suffering from suprascapular nerve entrapment syndrome is presented. The patient underwent shoulder arthroscopy, where the compression of the suprascapular nerve by the superior transverse scapular ligament was diagnosed. Arthroscopic release of the suprascapular nerve relieved pain, weakness, and atrophy of the supraspinatus and infraspinatus muscles. Level of Evidence: IV

Se han descrito múltiples técnicas para la fijación del hueso, pero no se han estandarizado el orden y los pasos para llevar a cabo esta reparación, lo que generó inconsistencias y heterogeneidad en los resultados de la reparación. En este artículo, se propone una nueva clasificación de las lesiones del manguito rotador que les permitirá a los cirujanos tomar decisiones durante la cirugía de reparación artroscópica del manguito rotador. OBJECTIVE@#To compare the mid-term clinical effect of arthroscopic surgery versus conservative treatment on the middle aged early knee osteoarthritis (EKOA) patients, with the hope to provide clinical evidence for their individual therapy.@*METHODS@#A total of 145 middle aged EKOA patients(182 knees) who received arthroscopic surgery or conservative treatment from January 2015 to December 2016 were retrospectively enrolled, including 35 males and 110 females, aged from 47 to 79 years old with an average of (57.6±6.9) years old, and the duration of disease ranged from 6 to 48 months with an average of(14.6±8.9) months. According to treatment method, patients were divided into arthroscopic surgery group (47 patients, 58 knees) and conservative treatment group(98 patients, 124 knees). Before treatment, patients presented with symptoms of knee joint, such as pain, swelling, locking, limited flexion and extension, and weakness, as well as abnormal findings in knee X-ray (without or suspicious joint space narrow, and a few of osteophyte formation) or in knee MRI (injury or degeneration of articular cartilage or meniscus, loose body in the joint cavity and synovial hyperemia edema, etc). Related data were collected, including duration of knee symptoms, presence of meniscus injury, loose body in the joint cavity or mechanical symptoms such as locking, and visual analogue scale (VAS) and Lysholm knee function score before treatment and at the latest follow-up. Statistical analysis was performed to compare the differences in VAS or Lyshilm score before or after treatment between the low groups and within each group.@*RESULTS@#Patients in the two groups were followed up from 60 to 76 months. In the arthroscopic surgery group, the incision healing was good and no surgical complications occurred. There were no significant differences in age, gender, BMI and follow-up time between the two groups(P>0.05). Before treatment, compared with conservative group, duration of symptoms in the arthroscopic group was longer (P<0.001), comorbidity rates of meniscus injury (P<0.001), free body (P=0.001) and mechanical symptoms (P<0.001) were higher, VAS (P<0.001) and Lysholm score (P<0.001) were worse. At the final follow-up, VAS and Lysholm score in either the conservative group or the arthroscopic group were significantly better than before treatment (P<0.05), while no significant differences between the two groups were found. The VAS was (1.5±1.2) scores in the arthroscopic group and (1.6±1.0)scores in the conservative group(P=0.549), and the Lysholm score was (84.9±12.5) scores in the arthroscopic group and (84.2±9.9) scores in the conservative group (P=0.676).@*CONCLUSION@#Both arthroscopic surgery and conservative treatment have satisfactory intermediate clinical effect middle- aged patients with EKOA, without statistically differences. However, most of the patients before surgery in the arthroscopic treatment group had mechanical locking symptoms caused by meniscus injury or loose body. Therefore, for the middle-aged EKOA patients with mechanical locking symptoms or without obtaining satisfactory outcome after conservative treatment, arthroscopic surgery may be considered.

OBJECTIVE@#To explore risk factors for infections after arthroscopic rotator cuff repair, and improve the under standing for reducing infection.@*METHODS@#Clinical data of 2 591 patients who underwent arthroscopic rotator cuff repair from January 2019 to January 2022 were retrospectively analyzed, including 1 265 males and 1 326 females, aged from 25 to 82 years old with an average age of (51.5±15.6) years old. They were divided into infection group(n=18) and uninfected group(n=2 573) according to whether or not patients had postoperative infection. Gender, age, smoking, diabetes, body mass index, local closure within 1 month before operation, operation time, preventive use of antibiotics, and internal fixation implantation between two groups were recorded. Univariate Logistic regression analysis screened factors associated with infections after arthroscopic rotator cuff repair. Theresultswere entered into the multivariate logistic regression analysis, screening the high risk factors for infections after arthroscopic rotator cuff repair.@*RESULTS@#In 2 591 patients, 18 patients were infected after operation, infection rate was 0.69%. Univariate Logistic regression analysis showed that gender, age, operation time, antibiotic prophylaxis, internal fixation implantation were risk factors for infections after arthroscopic rotator cuff repair. Multivariate Logistic regression analysis showed male(OR=14.227), age≥65 years(OR=34.313), operation time≥2 h (OR=15.616), without antibiotic prophylaxis(OR=4.891), and internal fixation implantation(OR=5.103) were major risk factors for infection after arthroscopic rotator cuff repair(P<0.05).@*CONCLUSION@#Male, age≥65 years, operation time≥2 h, without antibiotic prophylaxis and internal fixation implantation were independent risk factors for infection after arthroscopic rotator cuff repair. Early diagnosis and timely treatment should be carried out to reduce the incidence of infection.
