PURPOSE: To investigate the presence of auditory and nonauditory symptoms in professionals working in ambulances. METHODS: This was a descriptive cross-sectional study with a convenience sample. Thirty-six professionals working in mobile support units, including drivers and nursing technicians from two private urgency and emergency services in the city of Belo Horizonte, Minas Gerais, Brazil, participated in the study. A questionnaire containing 17 multiple-choice questions was applied to the participants with questions regarding life history and occupation, family history of hearing loss, use of medications, and presence of auditory and nonauditory symptoms, among others. The professionals answered the questionnaire individually, in their workplaces, and received help from the researcher to understand the content of questions, if needed. Data were statistically analyzed using the Statistical Package for the Social Sciences (SPSS) software, version 16.0. RESULTS: The most reported auditory symptoms were tinnitus, intolerance to intense sounds, and ear plenitude. The most reported nonauditory symptoms were irritability, headache, talking difficulties in noisy environments, and sleep alterations. A difference (p≤0.05) was observed when the relation between self-perception of drivers and nursing technicians on hearing acuity, presence of tinnitus, irritability, and communication difficulty was analyzed. CONCLUSION: Auditory and nonauditory symptoms are frequent in workers from mobile support units. An association between the worker's symptoms and the performed function was also observed. The results indicate a need of developing preventive actions regarding general health, which are aimed at the preservation of hearing health and quality of life of these professionals. .
OBJETIVO: Investigar a presença de sintomas auditivos e não auditivos em profissionais que atuam em ambulâncias. MÉTODOS: Estudo transversal descritivo com amostra de conveniência. Participaram 36 profissionais que atuam em unidades de suporte móveis, incluindo motoristas e técnicos de enfermagem de dois serviços particulares de urgência e emergência na cidade de Belo Horizonte, Minas Gerais. Foi aplicado um questionário contendo 17 questões de múltipla escolha sobre a história de vida e ocupação, antecedente familiar de perda auditiva, uso de medicamentos, presença de sintomas auditivos e não auditivos, entre outras investigações. Os profissionais responderam o questionário individualmente, no seu ambiente de trabalho, e, na existência de dificuldade de compreensão das questões, a pesquisadora explicou o conteúdo da questão. Os dados foram analisados estatisticamente utilizando-se o software Statistical Package for the Social Sciences (SPSS) 16.0. RESULTADOS: Os sintomas auditivos mais relatados pelos profissionais foram zumbido, intolerância a sons intensos e plenitude auricular. Os sintomas não auditivos mais frequentes foram irritabilidade, dor de cabeça, dificuldade de conversar em ambiente ruidoso e alteração do sono. Foi observada diferença (p≤0,05) ao se analisar a relação entre autopercepção de motoristas e técnicos de enfermagem sobre acuidade auditiva, presença de zumbido, irritabilidade e dificuldade de comunicação. CONCLUSÃO: Sintomas auditivos e não auditivos são frequentes em trabalhadores de unidades de suporte móveis. Pôde-se observar associação entre sintomas e função desempenhada pelo trabalhador. Dessa forma, verifica-se a necessidade do desenvolvimento de ações preventivas voltadas à saúde geral, visando à preservação da saúde auditiva e qualidade de vida desses profissionais. .

Background: Exposure to Biological Agents during work is an emergent type of occupational risk. Aim: To characterize occupational biological risk exposure among Chilean workers which have been registered by the Toxicology Information Center, between January 2006 and December 2009. Material and Methods: All incoming calls reporting exposure to biological agents during the studied period were analyzed. The information obtained from the caller was registered using the Communication Record Instrument of the WHO International Programme on Chemical Safety (IPCS INTOX). Results: In the studied period, 77 calls were received. The mean age of exposed patients was 35 ± 15 years and 57% of them were females. The most common involved agents were vaccines for veterinary use (42%) followed by Loxosceles laeta bites in 16%. The main routes of exposure were injections, cuts and needle stick injuries in 39% and stings and bites in 38%. The highest exposure rates were observed in Southern Chile due to self-inoculation of veterinary vaccines used in the salmon industry (22.7/100.000 actual workers). Fifty-eight percent of calls were from health care workers, and 51% of them were from health care facilities. Sixty percent of exposures occurred during summer and spring. There was a fourfold higher risk of calls involving women exposed to bites or stings (odds ratio (OR) 4.5 (CI95 1.5-13.9, p < 0.01). Men had a fourfold higher risk of being exposed to vaccines or medications for veterinary use (OR 4.2, CI95 1.4-12.6 p < 0.01). Conclusions: Most calls involving an exposure to a biological agent were caused by self-inoculation of veterinary medications.

A controlled cross-sectional study of family growers of fruit and vegetables was conducted between October 2009 and October 2010 to characterize the use of pesticides, establish the socio-demographic profile, and analyze cholinesterase activity in small-scale agricultural workers in Southern Brazil. Data was collected for 173 workers and 179 controls. A structured questionnaire was applied collecting socio-demographic information and determining knowledge and work practices in relation to pesticide use. The benchmarks for total cholinesterase (ChEs) and butyrylcholinesterase (BuChE) were obtained from the average enzymatic activity of the occupationally unexposed group (control). The mean age of the exposed population was 40.0 ± 11.2 years. The symptoms differed significantly (p<0.05) between the exposed and unexposed populations. Forty (23.1%) workers showed symptoms associated with exposure to pesticides. The average value of enzymatic activity in the occupationally unexposed group for ChEs was 6.3 μmol/mL/min among males and 5.6 μmol/mL/min among females while for BuChE was 2.4 μmol/mL/min among males and 2.0 μmol/mL/min among females. Eight (4.6%) workers had high inhibition (>30%) of ChEs activity, whereas no workers showed high inhibition (>50%) of BuChE. Potential factors involved include gender, education, pesticide orientation, exposure, and hygiene measures.
Realizou-se um estudo transversal controlado, com hortifruticultores, durante o período de outubro de 2009 a outubro de 2010, a fim de caracterizar o uso de praguicidas, estabelecer o perfil sócio demográfico e analisar a atividade das colinesterases em trabalhadores rurais do sul do Brasil. Os dados foram obtidos de 173 trabalhadores e 179 controles. Um formulário estruturado foi aplicado obtendo informações sócio demográficas, conhecimento e práticas de trabalho relacionadas com o uso de praguicidas. Os valores de referência para colinesterases totais (ChEs) e butirilcolinesterase (BuChE) foram obtidos a partir da média da atividade enzimática do grupo controle. A idade média da população exposta foi de 40,0±11,2 anos. Os sinais/sintomas foram significativamente diferentes (p<0,05) entre a população exposta e não exposta. Quarenta trabalhadores (23,1%) apresentaram sinais/sintomas relacionados com a exposição aos praguicidas. O valor médio do grupo controle foi de 6,3 μmol/mL/min para o gênero masculino e 5,6 μmol/mL/min para o gênero feminino na ChEs; 2,4 μmol/mL/min para o gênero masculino e 2,0 μmol/mL/min para o gênero feminino na BuChE. Do total de trabalhadores (n=173), 08 (4,6%) apresentaram inibição elevada (>30%) das atividades das ChEs, e para BuChE nenhum trabalhador apresentou alta inibição (>50%). Dentre os fatores que poderiam estar envolvidos destacam-se o gênero, escolaridade, orientação para trabalhar com praguicidas, exposição e medidas de higiene.

O câncer de pulmão é multicausal. Fatores hereditários, genéticos e ambientais interagem na sua gênese. O principal fator de risco é o tabagismo. Entretanto, o ambiente de trabalho é um local de possível exposição a agentes cancerígenos. Atualmente, a International Agency for Research on Cancer lista 19 substâncias/situações de trabalho/ocupações comprovadamente associadas ao câncer de pulmão (grupo 1). A abordagem da ocupação em pacientes portadores de câncer de pulmão é fraca, impactando negativamente na busca da causalidade e, consequentemente, no desvelamento de casos de câncer ocupacional. Os objetivos desta revisão foram elencar os agentes reconhecidamente indutores de câncer de pulmão, discutir a contribuição da ocupação no desenvolvimento da doença, citar as publicações nacionais sobre o tema e sugerir uma lista de procedimentos que são essenciais para uma adequada investigação da relação de causalidade entre câncer de pulmão e ocupação.
Lung cancer is a multifactorial disease. Hereditary, genetic, and environmental factors interact in its genesis. The principal risk factor for lung cancer is smoking. However, the workplace provides an environment in which there is a risk of exposure to carcinogens. The International Agency for Research on Cancer currently lists 19 substances/work situations/occupations that have been proven to be associated with lung cancer (group 1). Thorough occupational history taking is not widely practiced in patients with lung cancer, which has a negative impact on the investigation of causality and, consequently, on the identification of cases of occupational cancer. The objectives of this review were to list the agents that are recognized as causes of lung cancer, to discuss the contribution of occupation to the development of the disease, to cite national studies on the subject, and to propose a list of procedures that are essential to the appropriate investigation of causality between lung cancer and occupation.

Los pesticidas utilizados en agricultura pueden representar un riesgo potencial para la salud de los agricultores expuestos y para el medio ambiente. El objetivo de este trabajo fue evaluar trabajadores frutihortícolas expuestos a plaguicidas, categorizados por: exposición directa (n = 45), exposición indirecta (n = 50) y controles (n = 50) mediante biomarcadores de exposición y efecto: colinesterasa (ChE), acetilcolinesterasa (AChE), catalasa (CAT), peroxidación de lípidos (TBARS), Indice de Daño Ensayo Cometa (IDEC) e Indice de Daño Ensayo Reparación (IDER). Los resultados indican: a) inhibición significativa de AChE (p < 0.001) en expuestos directos e indirectos; b) aumento en los niveles de TBARS (p < 0.001) en los directos; c) reducción de CAT significativa (p < 0.01) y d) aumento de IDEC e IDER (p < 0.001) en ambos grupos. Los resultados obtenidos reflejan modificaciones en el balance oxidativo junto con daño al ADN en los trabajadores estudiados. Estos hallazgos representan una contribución en la evaluación subclínica de exposición a agroquímicos en nuestro país.
Pesticides are used in agriculture to protect crops but may represent a potential risk to farmers and the environment. The aim of this work was to evaluate horticultural workers exposed to pesticide, categorized by: direct exposure (n = 45), indirect exposure (n = 50) and controls (n = 50) using exposure and effect biomarkers: cholinesterase (ChE), acetylcholinesterase (AChE), catalase (CAT), lipid peroxidation (TBARS), Damage Index Comet Assay (IDEC) and Damage Index Repair Assay (IDER). Our results show: a) an AChE inhibition in directly and indirectly exposed population (p < 0.001), b) significant increase in the levels of TBARS in direct exposure (p < 0.001), c) the CAT reduction was significant (p < 0.01), d) a significant increase in IDEC and IDER in both exposed groups (p < 0.001). Our results evidence variations in oxidative balance and DNA damage in exposed workers. These findings represent a contribution to the sub-clinical evaluation of subjects exposed to agrochemicals in our country.

PURPOSE: To apprehend the central and periphery elements on the occupational risk and the level of satisfaction with the work, emitted by workers of an academic public hospital. METHODS: The sample was constituted by 84 different graduate health professionals who work for the hospital. The data collection was based on the following instruments: the free evocation from the [quot ]occupational[quot ] risk, an interview semi-structured aiming to capture socio-demographic data and the level of satisfaction of the work developed within the hospital environment, as well as the participant observation. For the analysis of the socio-demographic, it was used the descriptive statistical treatment based on the software Evoc 2000, as well as the Analysis of Content, according to Bardin (1977). These analysis aimed to capture the registrations obtained from the interviews as well as daily notes to verify the possible contradictions and consensus on the professional postures during their work shift. In addition, the analysis were also based on the Theory of the Social Representations and the Theory of the Central Nucleus. RESULTS: The results have demonstrated that the average age of the group is around 38,4 years old; (72.6%) of the work force with a graduate degree is female; 51% of the professionals interviewed work in other places besides the hospital; 83% are considered to be satisfied and only 17% are considered to be unsatisfied with the work accomplished in the hospital. Regarding to the occupational risk, some specific aspects were mentioned as the following categorization: risks of physical nature, chemistry, organic, mechanic, physiologic and psychic. Regarding to the content analysis, the following categories emerged: love to the profession; convenience and professional growth; reflection and critic. Based on the direct observation it was verified that it is common the noncompliance of the relative universal norms regarding to the use of protection equipments during the work. CONCLUSION: The organic occupational risks constituted the central nucleus of the representation of the workers in the hospital. It was observed that physiologic, mechanics and chemical risks are considered as periphery elements of that representation. In that perspective, it was suggested that some subjective aspects related to the organizational culture, in terms of control and elimination of the environmental risks, administration, process, organization and difficulties of the.

Foi elaborado um Programa de Monitoração Ocupacional em radiologia médica para o Hospital Universitário Clementino Fraga Filho da Universidade Federal do Rio de Janeiro, seguindo as normas e legislação nacional. Para a construção do Programa, realizou-se um levantamento de todos os Serviços e funcionários diretamente expostos às radiações ionizantes. Constatou-se que a maioria desses funcionários não era monitorada, que apenas três Serviços mantinham um controle de dose de trabalhadores com contratos de prestação de serviço independentes e que não havia controle para os casos de doses elevadas. Com a implantação do Programa, os passos seguintes foram a contratação da uma nova empresa que prestasse o serviço de monitoração pessoal, reduzindo significativamente o custo anual de operação com o contrato único para o hospital; a inclusão de mais sete Serviços que trabalham com radiações ionizantes, aumentando em mais de 60 por cento o número de trabalhadores monitorados; o controle de doses elevadas, principalmente no setor de Hemodinâmica, que apresentou a maior média de dose (0,32 mSv/mês); e a monitoração de área, que foi realizada em períodos intercalados nos pontos considerados de maior risco para a população e os trabalhadores. Paralelamente, o Programa de Monitoração Ocupacional foi informatizado, sendo construído um banco de dados com o propósito de controlar e armazenar as doses funcionais. Com a implantação do Programa, começa-se a compreender os riscos de se trabalhar com radiações ionizantes, a partir dos conceitos de proteção radiológica, e conscientiza-se quanto ao uso correto dos monitores de dose
An occupational monitoring program in diagnostic radiology was implemented at the "Hospital Universitário Clementino Fraga Filho UFRJ", Rio de Janeiro, Brazil, in accordance with the Brazilian legislation. Previously, a survey of all personnel involved with ionizing radiation was performed. Many problems were observed: the great majority of the workers were not properly monitored; only three departments of the hospital kept an independent survey of the occupational doses; there was not a follow-up control of the high doses. With the implementation of the program, a new laboratory was chosen to read the dosemeters and this initiative resulted in reduction of the hospital costs. The inclusion of seven more departments in the program represented an increase of 60% in the number of monitored workers. The program also provided a system to control the high doses, especially in the Hemodynamics department, which presented the highest mean dose value (0.32 mSv/month). An area survey program was performed during different periods in places considered of high risk for the workers and for the public as well. At the same time, a software was used to build a database with the aim of controlling all personnel data. The implementation of the program provided all personnel involved a better knowledge of the risks associated with ionizing radiation and of radioprotection, and also awareness of the need of correct use of the personal dose monitors.
