Objective To explore the diagnostic value of Likert score and EPE grade score based on multiparameter magnetic resonance imaging(mpMRI)for extracapsular extension in prostate cancer(PCa).Methods The MR imaging and histopathology data from 272 PCa patients were analyzed retrospectively.All patients underwent mpMRI examination within 2 months before radical prostatectomy.Two radiologists with over 10 years of experience assessed the mpMRI images according to the Likert score and EPE grade score,respectively,and compared with pathological findings.The consistency between the two radiologists was evaluated by weighted Kappa test.The statistical analysis was performed using MedCalc 20.0 software.The sensitivity,specificity and other indicators were calculated to analyze the optimal cut-off value of Likert score and EPE grade score for diagnosing extracapsular extension in PCa.The area under the curve(AUC)was used to compare the diagnostic performance of the two scoring systems for extracapsular extension in PCa.Results Among 272 PCa patients,there were 45 cases with extracapsular extension and 227 cases without extracapsular extension.The weighted Kappa coefficients were 0.730 and 0.820 for Likert score and EPE grade score,respectively,indicating good consistency.The optimal cut-off values for diagnosing extracapsular extension in PCa were Likert score 3 and EPE grade score 2.The sensitivity and specificity were 68.8%and 77.5%for Likert score 3,and 64.4%and 84.5%for EPE grade score 2,respectively.Both Likert score(AUC=0.780)and EPE grade score(AUC=0.797)had high accuracy in predicting extracapsular extension in PCa,with no significant difference(P>0.05).Conclusion Both Likert score and EPE grade score have good diagnostic performance in detecting extracapsular extension in PCa,which provides important diagnostic basis for clinical staging of PCa.

Objective To improve the survival rate and life quality of peritoneal dialysis(PD)patients,we es-tablished a retraining model based on ADDIE model,including optimizing the content,form and frequency.Methods From January 1,2022 to May 3,2023,based on the 5 stages of ADDIE model,we investigated the needs of pa-tients,invited 55 experts in the peritoneal dialysis field to design the final draft of the retraining model through 2 rounds of Delphi expert consultations,and 23 peritoneal dialysis patients were preexperimented to evaluate and re-vise the retraining model.Results The questionnaire recovery rates of the 2 rounds of expert consultation were 100％and 96.36％,respectively.The coordination coefficients of the first-level catalog were 0.379 and 0.384,and the coordination coefficients of the second-level catalog were 0.446 and 0.427,respectively.The Chi-square test showed that P＜0.05,indicating statistical significance.The content of the retraining model included 4 sections,33 subdirectories and 9 training forms,which were combined online and offline.The training frequency was different due to the different contents,and the single content of a single training form was mainly 15 min.Conclusion The PD patient retraining model constructed in this study is scientific,reliable and innovative.Its content is easy to un-derstand and diverse in forms.The training duration and frequency are in line with the memory rule,and the eval-uation takes into account both process and result.

Objective:To evaluate the clinical value of abdominal color Doppler ultrasonography in the differential diagnosis of colonic polyps in children.Methods:From January 2017 to January 2020, 300 children with suspected colonic polyps were studied.All patients were examined by colonoscopy and abdominal color Doppler ultrasound respectively.The results of colonoscopy were taken as the gold standard to evaluate the diagnostic efficacy of abdominal color Doppler ultrasound in the differential diagnosis of colonic polyps in children.The diagnostic accuracy of abdominal color Doppler ultrasound in diagnosis of colonic polyps in children with different location, type and focus size was analyzed.Results:The diagnostic sensitivity and specificity of abdominal color Doppler ultrasound for colonic polyps in children were 87.60%(113/129) and 97.08%(166/171), respectively.The overall diagnostic accuracy was 93.00% (279/300), the positive predictive value was 95.76%(113/118), and the negative predictive value was 91.21% (166/182). The diagnostic accuracy of abdominal color Doppler ultrasound were 85.71%(12/14), 83.33%(10/11), 87.50%(28/32), 86.79%(46/53) and 89.47%(17/19) for children's polyps in ascending colon, transverse colon, descending colon, sigmoid colon and rectum, and were 86.17%(81/94), 92.31%(24/26) and 88.89%(8/9) for juvenile polyps, black spot polyp syndrome and adenomatous polyps, respectively.There were no statistically significant differences between the two groups (χ 2=0.250, 0.721; P=0.993, 0.697). The diagnostic accuracy of abdominal color Doppler ultrasound in children with diameter ≤1 cm (38.09%) was significantly lower than that in children with 1 cm<diameter<3 cm and diameter ≥ 3 cm (95.38% and 100.00%) (χ 2=34.400, 33.404; P=0.000, 0.000). Conclusion:Abdominal color Doppler ultrasound can accurately diagnose children's colonic polyps, which is suitable for screening children's colonic polyps, and has high clinical application value.However, it has certain limitations for the detection of lesions with diameter ≤ 1 cm.

Objective To explore the clinical effect of Jiangru Formula(Herbal formula for soothing the liver,clearing heat,activating blood and removing stasis)combined with TCM surgical treatment for plasmocyte mastitis.Methods Fifty five patients with plasmocyte mastitis were treated with Jiangru Formula orally and TCM surgical treatment to see the effect of this method.Results The total effective rate was 100%.The average treatment period was(28?13.61)days.The recurrence rate was 1.82%.Conclusion The combination of Jiangru Formula with TCM surgical treatment for plasmocyte mastitis is effective in shortening the treatment period,reducing the recurrence rate and keeping the shape of the breast.
