摘要

Objective:To establish a mouse model of breast cancer lung metastasis with miR-155 knockout(miR155-/-)mice,and to compare the difference of peripheral blood immune cell typing between miR155-/-mice and C57BL/6J wide-type(WT)mice.Methods:Bioinformatics analysis was used to explore the expression level of miR-155 in breast cancer tissues and peripheral serum,and its relationship with prognosis.Mouse model of lung metastasis of breast cancer was established by tail vein injection;peripheral blood was collected for flow cytometry,and the immune cell typing was analyzed;the lung tissues were collected for immunohisto-chemical detection to observe the tumor metastasis.Results:Percentage of T lymphocytes and monocytes in peripheral blood of miR155-/-mice was significantly decreased compared with WT mice(P<0.05),percentage of myeloid inhibitory cells(MDSCs)was increased significantly(P<0.05),in which the proportion of monocyte subsets(M-MDSC)was significantly decreased(P<0.05),while the proportion of granulocyte subsets(G-MDSC)was significantly increased(P<0.05).In lung metastasis model of breast can-cer,percentage of T lymphocytes in peripheral blood of miR155-/-mice was significantly higher compared with WT mice,while per-centage of NK cells was decreased significantly(P<0.05),percentage of neutrophil was significantly decreased(P<0.001),propor-tion of Th cells in T lymphocytes was significantly decreased(P<0.05),proportion of M-MDSCs was significantly decreased(P<0.01),while proportion of G-MDSCs was significantly increased(P<0.01).Conclusion:Deletion of miR-155 gene leads to significant differences in peripheral immune cell typing,making mice more susceptible to lung metastasis of breast cancer.

摘要

Objective:To investigate the correlation of atrophy and intestinal metaplasia (IM) stage with gastric cancer and to optimize biopsy strategy.Methods:Data of patients who underwent endoscopy and five-point biopsy at Shandong Provincial Hospital between November 2020 and October 2022 were collected. The baseline characteristics of gastric cancer and non-gastric cancer patients, as well as the occurrence and severity of atrophy and IM in different areas were compared. Logistic regression analysis was used to evaluate the correlation of operative link for gastritis assessment (OLGA) and operative link for gastric intestinal metaplasia assessment (OLGIM) staging with gastric cancer. The Kendall tau correlation coefficient was used to compare the consistency of different biopsy strategies (two-point, three-point, and four-point) with the standard five-point biopsy in OLGA and OLGIM staging. Receiver operating characteristic (ROC) curve analysis was further performed to compare the diagnostic performance of different biopsy strategies in identifying the OLGA and OLGIM Ⅲ-Ⅳ stage.Results:A total of 122 patients were included in the analysis, with age of 61.0±10.0 years. Multivariate analysis showed that OLGA staging was not associated with gastric cancer ( P=0.788), while OLGIM Ⅲ-Ⅳ staging was significantly correlated with gastric cancer ( P=0.006, OR=3.39, 95% CI: 1.41-8.17). The occurrence of atrophy and IM were higher in lesser curvature of the antrum [56.6% (69/122) and 66.4% (81/122)] and incisura angularis [57.4% (70/122) and 52.5% (64/122)], with higher severity, while lower in greater curvature of the corpus [2.5% (3/122) and 5.7% (7/122)], with lower severity. The consistency of four-point and three-point biopsies with standard five-point biopsy in OLGA and OLGIM staging was high. The consistency of three-point biopsy in incisura angularis, lesser curvature of the antrum and corpus was exceptionally high among them, with correlation coefficients of 0.969 and 0.987, respectively. Conclusion:OLGIM Ⅲ-Ⅳ stages increase the risk of gastric cancer. Three-point biopsy in incisura angularis, lesser curvature of the antrum and corpus are recommended for the screening and monitoring of atrophy or IM.

摘要

Objective The combined detection of serum angiopoietin-like protein 8(ANGPTL8)and Vascular cell adhesion molecule-1(VCAM-1)levels was analyzed for the predictive value of cerebral vasospasm(CVS)after intracranial aneurysm embolization.Methods A total of 196 patients who underwent intracranial aneurysm embolization in our hospital from March 2019-March 2022 were selected as the study subjects,99 patients with CVS were in the CVS group,and 97 patients without CVS were in the non CVS group.Serum ANGPTL8 and VCAM-1 levels were detected by ELISA;the correlation between serum ANGPTL8 and VCAM-1 levels was analyzed by Pearson method,Logistic regression was used to analyze the influencing factors of CVS in patients undergoing intracranial aneurysm embolization;ROC curve was used to analyze the serum levels of ANGPTL8 and VCAM-1 to predict the cutoff value of CVS in patients undergoing intracranial aneurysm embolization;four grid table method was used to analyze the predictive value of ANGPTL8,VCAM-1 and their combination on the occurrence of CVS in patients undergoing intracranial aneurysm embolization.Results The differences between CVS and non-CVS groups were statistically significant in hypertension,Hunt-Hess grade,and Glasgow coma(GCS)scores(P<0.05).The serum ANGPTL8 and VCAM-1 levels in the CVS group were significantly higher than those in the non-CVS group(P<0.05).There was a positive correlation between serum ANGPTL8 and VCAM-1(r=0.468,P<0.05).Multivariate analysis showed that high level of ANGPTL8(OR=3.652,95%CI:1.434-9.302),high level of VCAM-1(OR=2.619,95%CI:1.212-5.658),Hunt Hess grade Ⅲ-Ⅳ(OR=1.927,95%CI:1.104-3.362),GCS score of 3-8(OR=2.813,95%CI:1.257-6.295)were independent risk factors for CVS in patients undergoing intracranial aneurysm embolization.The AUC of serum ANGPTL8 level in predicting CVS in patients undergoing intracranial aneurysm embolization was 0.844,and the cut-off value was 189.233 U/L;the AUC of serum VCAM-1 level in predicting CVS in patients undergoing intracranial aneurysm embolization was 0.795,and the cutoff value was 17.984 mg/L.The accuracy,sensitivity and specificity of the combined prediction for CVS were 89.81%,93.94%and 85.57%,respectively,which were obviously higher than those of the single prediction.Conclusion The serum levels of ANGPTL8 and VCAM-1 in CVS group are obviously higher than those in non CVS group.The combination of the two has a high predictive value for CVS after intracranial aneurysm embolization.

摘要

Objective@#To investigate the attitude and behaviors towards tobacco control among civil servants in smoke-free governments of Ningbo City, so as to provide insights into the construction of smoke-free governments.@*Methods@#Ten governments were randomly sampled from city-level and county/district-level smoke-free governments in Ningbo City using a stratified random sampling method, and 10 civil servants were sampled from each government using a convenient sampling method. Participants' demographics, attitude and behaviors towards tobacco control and second-hand smoke exposure were collected using a self-designed questionnaire and descriptively analyzed.@*Results@# A total of 1 100 civil servants were recruited, including 466 men (42.36%) and had a mean age of (39.02±9.38) years. There were 1 005 participants favoring “civil servants should refuse smoking firstly” (91.36%), 1 058 participants favoring “governments should build a smoke-free environment firstly” (96.18%), 1 075 participants favoring “smoking should be completely banned in any indoor public places” (97.73%), 913 participants thinking that smoking should be completely banned in any indoor public places (83.00%) and 813 participants thinking that tobacco control regulations are implemented satisfactorily in their workplaces (73.91%). The overall prevalence of current smoking was 11.91% among participants, and the prevalence of current smoking was 28.11% among male participants. Among all current smokers, 84 attempted to quit smoking (64.12%), and 89 had willingness or plans to quit smoking (67.94%). The proportion of second-hand smoke exposure in workplaces was 40.97% among non-smokers in the past one week.@*Conclusions@# The civil servants in Ningbo City has correct awareness of smoke-free governments, active attitudes towards tobacco control and low prevalence of smoking. However, smoke-free regulations remain to be improved and long-term tobacco control mechanisms remain to be created in governments in Ningbo City

摘要

A simple, fast, and visual method for detecting antibodies against peste des petits ruminants virus (PPRV) using colloidal gold strips was developed. In this study, the pET-32a-N was transformed into Escherichia coli Rosetta (DE3) for expression. Hybridoma cell lines were generated by fusing SP2/0 myeloma cells with splenocytes from immunized mice with the expressed and purified N protein of PPRV. The PPRV N protein was labeled with colloidal gold particles as the gold-labeled antigen. The N protein served as the gold standard antigen and as the test (T) line-coated antigen, while the monoclonal antibody served as the quality control (C) line-coated antibody to assemble the colloidal gold immunochromatographic test strips for detecting antibodies against the N protein of PPRV. Hybridoma cell line designated as 1F1 was able to stably secrete the monoclonal antibody against the N protein of PPRV. The titer of 1F1 monoclonal antibody in ascites was 1:128 000 determined by indirect enzyme-linked immunosorbent assays (ELISA), and the immunoglobulin subtype of the monoclonal antibody was IgG1, with kappa chain. The obtained monoclonal antibody was able to specifically recognize the N protein of PPRV, as shown by Western blotting and indirect immunofluorescent assay (IFA). The developed colloidal gold test strip method was able to detect PPRV antibodies specifically, and there was no difference between different batches of the test strips. Testing of a total of 122 clinical sera showed that the compliance rate of the test strip with ELISA test was 97.6%.The test strip assay developed in this study has good specificity, reproducibility, and sensitivity, and it can be used for the rapid detection of PPRV antibodies.

摘要

Objective@#To analyze the prevalence of smoking among adults in Ningbo City from 2018 to 2021, so as to provide the scientific evidence for improving the tobacco control policy and evaluating the effectiveness of the tobacco control policy. @*Methods@#The permanent residents at ages of 15 to 69 years were recruited from 10 counties ( districts ) in Ningbo City using the stratified multistage random sampling method from 2018 to 2021, and subjects' demographic features, smoking and secondhand smoke exposure were collected using the National Questionnaire for Surveillance on Healthy Literacy and Tobacco Epidemic in Chinese Residents. All data were standardized to the Sixth National Population Census in 2010. The prevalence of smoking, smoking cessation and exposure to secondhand smoke and the trends for the prevalence were estimated among adults in Ningbo City from 2018 to 2021.@*Results@#A total of 6 581, 6 340, 6 380 and 6 400 valid questionnaires were recovered from 2018 to 2021, and the effective recovery rates were 99.20%, 97.46%, 97.76% and 98.02%, respectively. The respondents had male to female ratios of 1∶1.05, 1∶1.07, 1∶1.05 and 1∶1.14, and had urban-rural population ratios of 1∶0.75, 1∶0.62, 1∶0.60 and 1∶0.78 from 2018 to 2021, respectively. The prevalence of current smoking was 21.99%, 21.42%, 22.07% and 20.30%, and the standardized prevalence was 22.03%, 20.12%, 21.33% and 19.38% from 2018 to 2021, respectively, appearing no significant changing trend ( χ2trend=3.751, P=0.053 ). The prevalence of current smoking was significantly higher among rural residents than among urban residents, and higher in men than in women ( all P<0.05 ). The prevalence of smoking cessation was 24.44%, 27.11%, 26.97% and 29.59%, and the standardized prevalence was 21.96%, 26.90%, 24.92% and 28.38% from 2018 to 2021, appearing a tendency towards a rise ( χ2rend=11.193, P=0.001 ). The prevalence of exposure to secondhand smoke was 52.40%, 50.18%, 48.99% and 44.58%, and the standardized prevalence was 54.08%, 51.45%, 49.95% and 46.76% from 2018 to 2021, showing a tendency towards a decline ( χ2rend=62.094, P<0.001 ) .@*Conclusions@#The prevalence of current smoking was approximately 20% among adults in Ningbo City from 2018 to 2021, with a tendency towards a decline in the prevalence of exposure to secondhand smoke and a tendency towards a rise in the prevalence of smoking cessation. Rural male residents are key targets for tobacco control.

摘要

Objective:To study the changes and influencing factors of splanchnic regional saturation before and after feeding in preterm infants with feeding intolerance (FI).Methods:From December 2018 to August 2019, preterm infants with FI admitted to the neonatal intensive care unit of our hospital within 24 hours after birth were prospectively enrolled in this same-patient before-after study. Splanchnic regional saturation (rSsO 2) and cerebral regional oxygenation (rSc0 2) 5 minutes before feeding and 1 hour after feeding were monitored using near-infrared spectroscopy (NIRS). The average values of rScO 2, rSsO 2 and splanchnic-cerebral oxygenation ratio (SCOR) before and after feeding were calculated. The clinical data including postnatal age, corrected gestational age and feeding methods (breastfeeding or formula feeding) were collected. Single-factor correlation analysis and multiple linear regression were used to analyze the influencing factors of rSsO 2 before and after feeding. Results:A total of 41 preterm infants were included. No significant differences existed in rSsO 2, rScO 2 and SCOR before and after feeding ( P>0.05). The feeding methods showed relative prominent influences on the changes of rSsO 2 and SCOR before and after feeding. The breastfeeding infants had smaller changes of rSsO 2 and SCOR before and after feeding compared with formula feeding infants, the regression equations were Y=5.538-4.065X (model complex correlation coefficient was 0.414 determination coefficient R2=0.171, F=8.050, P<0.01) and Y=0.109-0.075X (model complex correlation coefficient was 0.405 determination coefficient R=0.1642, F=7.655, P<0.01). Conclusions:Proper feeding will not increase rSsO 2 in preterm infants with FI. Comparing with formula feeding infants, breastfeeding infants has more stable post-feeding rSsO 2.Breastfeeding should be the first choice for preterm infants with FI.

摘要

Downstaging treatment by local therapy combined with systemic therapy before liver transplantation for patients with recurrent hepatocellular carcinoma (HCC) can control tumor progression and reduce tumor burden, which resulting in reducing the push-out rate of patients during the waiting period for liver transplantation, providing an oncological observation window, enabling patients of beyond Milan criteria downstaged with better survival benefit. The authors introduce the clinical experience of a case with recurrent HCC of beyond Milan criteria who under-went liver transplantation after receiving atezolizumab plus bevacizumab combined with local therapy. Results show the patient achieving pathological complete remission without postoperative rejection and obtaining a good prognosis with life status improved.

Objective @#To explore the mechanism of nuclear factor E2-related factor 2 (Nrf2) in bleomycin (BLM) -induced DNA damage of lung cancer cells.@*Methods @#The fluorescence intensity of tissue reactive oxygen species (ROS) in human lung cancer cells A549 and H1299 was detected by a reactive oxygen species fluorescent probe ( DCFH-DA) .DNA damage of A549 and H1299 cells treated with 5 μg / ml BLM was detected by Western blot and micronucleus.Then,the Nrf2-low expression transient siRNA and the stably shRNA-transformed lung cancer cells were respectively constructed to detect the DNA damage after BLM.The interaction between Nrf2 and Rad51 in cells was detected by co-immunoprecipitation method. @*Results @#Under the action of BLM,ROS levels in A549 and H1299 cells increased significantly,and Nrf2 entered the cell nucleus for activation(P<0. 01) .Under the effect of BLM,resveratrol ( Res) stimulated A549 and H1299 cells to up-regulate the protein expression of Nrf2 ( P < 0. 01) ,while the expression of DNA damage marker protein γ-H2AX decreased(P<0. 01) .On the contrary,after interfering the protein expression of Nrf2 in A549 and H1299 cells with siRNA,the protein expression of γ-H2AX in A549 and H1299 cells was significantly up-regulated under the action of BLM(P<0. 01) .Western blot results showed that the expression of homologous recombination repair protein Rad51 and the expression of Nrf2 protein in A549 cells with low Nrf2 expression were significantly down-regulated (P <0. 01) .@*Conclusion @#The antioxidant transcription factor Nrf2 in tumor cells can interact with Rad51 to regulate the expression of Rad51 and reduce BLM-induced cellular DNA damage.

摘要

Objective:To study the application of systemic meta-competence and its difficulties in the clinical practice of initial family therapy by qualitative research, with the aim of providing an empirical basis for developing local family therapists' clinical practice competence in systemic thinking.Methods:A purposive sample of 4 senior and 4 novice family therapists were gained, and a total of 12 hours of initial interviews were recorded.The materials were coded by thematic analysis with reference to the theoretical models of National Institute for Health and Care Excellence(NICE) and the American Association for Marriage and Family Therapy (AAMFT), and one-on-one interviews (totally 4 hours)were conducted with the therapists based on the coding results.Results:The application of systemic meta-competence in the initial family therapy was mainly manifested in four superordinate themes: adapting a non-pathological perspective, maintaining a relational orientation, focusing on interpersonal interactions and applying systemic intervention strategies, under which a total of 16 subordinate themes were included.The application difficulties of therapists with different qualification had both commonalities and differences.Conclusion:The systemic meta-competency is a key competence in the practice of family therapy.The application of systemic meta-competence in initial interview will benefit the therapeutic relationship, family information collections and formulation of therapy goals, and the application difficulties may reflect the culturally practical needs of professional trainings and supervisions.

摘要

Objective:To explore the effect of exercise intervention on regulation of Toll-like receptor 4 (TLR4) signaling pathway in overweight and obese pregnant women.Methods:The cohort was based on a randomized controlled trial (RCT) carried out by the same research group in Peking University First Hospital from December 2014 to July 2016. Overweight and obese patients who delivered by elective cesarean section without pregnancy complications were recruited, among which 12 cases in the exercise group and 11 cases in the control group were selected. Real-time polymerase chain reaction, Western Blot, and Luminex experiments were used to compare the expression of TLR4-myeloid differentiation factor 8(MyD88)-nuclear factor-κB(NF-κB) pathway in peripheral blood mononuclear cell (PBMC), rectus abdominis muscle, omental adipose, and subcutaneous adipose, as well as the levels of inflammatory factors (TNF-α, IL-1β, IL-10) in plasma between the two groups. Two independent samples t-test, generalized estimating equation, Chi-square test, and Pearson correlation analysis were adopted for statistical analysis. Results:(1) The expression of inflammatory factors TNF-α and IL-1β in the exercise group showed a downward trend compared with the control in the second and third trimester, but none of the differences were statistically significant (all P>0.05). (2) The mRNA expression of TLR4, MyD88, and NF-κB and the protein expression of TLR4 and NF-κB in PBMC of the exercise group were significantly lower than those in the control group during pregnancy (TLR4 mRNA: 0.06±0.03 vs 0.10±0.04 in the second trimester, 0.05±0.02 vs 0.11±0.05 in the third trimester, χ2=8.07; MyD88 mRNA: 0.09±0.03 vs 0.11±0.03 in the second trimester, 0.10±0.04 vs 0.17±0.06 in the third trimester, χ2=5.81; NF-κB mRNA: 0.10±0.03 vs 0.17±0.08 in the second trimester, 0.08±0.03 vs 0.20±0.08 in the third trimester, χ2=14.71; TLR4 protein: 1.7±0.5 vs 1.9±0.8 in the second trimester, 1.7±0.4 vs 2.3±0.8 in the third trimester, χ2=5.83; NF-κB protein: 1.0±0.4 vs 1.5±0.4 in the second trimester, 1.2±0.3 vs 1.5±0.5 in the third trimester, χ2=4.73; all P<0.05). Moreover, the differences in the mRNA expression of TLR4, MyD88, and NF-κB and TLR4 protein expression in PBMC between the two groups gradually increased. (3) NF-κB in rectus abdominis and omental adipose tissue (0.04±0.02 vs 0.08±0.04, t=-3.72; 0.25±0.05 vs 0.63±0.21, t=-5.41; both P<0.05) and TLR4 and MyD88 in subcutaneous adipose tissue (0.12±0.03 vs 0.30±0.10, t=-5.30; 0.24±0.09 vs 0.44±0.08, t=-5.38; both P<0.05) were observed a decreased mRNA level in the exercise group compared with the control group. The protein level of MyD88 and NF-κB in omental adipose tissue and NF-κB in subcutaneous adipose tissue in the exercise group were significantly lower than those in the control group (1.1±0.5 vs 2.0±0.8, t=-3.15; 1.3±0.5 vs 2.0±0.9, t=-2.23; 1.2±0.5 vs 1.9±0.8, t=-2.80, all P<0.05). (4) The expressions of TLR4 and NF-κB mRNA ( r=0.453 and 0.485) in rectus abdominis muscle, NF-κB mRNA, TLR4 and MyD88 protein ( r=0.539, 0.437 and 0.527) in omental adipose in the two groups were positively correlated with the level of fasting blood glucose ( P<0.05). Conclusions:Regular exercise during pregnancy can down-regulate the expression and activation of the TLR4-MyD88-NFκB pathway in overweight and obese pregnant women. The expression of related factors along this pathway has a certain correlation with fasting blood glucose.

摘要

Objective:Build a research index performance model with discipline growth evaluation, to evaluate the scientific research performance level of various disciplines in hospital more objectively, fairly and dynamically.Methods:Take the research projects, papers, patents and achievements as the main evaluation indicators and establish the hierarchical and classified scoring standards, to form the weight index evaluation model for the total score, per capita score and growth of the discipline research.Results:Using the growth research index to evaluate the scientific research performance of departments and individual researcher between January 2018 and December 2020. Excellent departments were selected for the top 10% of the scientific research index, those whose scientific research scores increased by more than 20% compared with the previous year were selected as the progressive departments, and the top 1% of individual scientific research scores were selected as the advanced researchers, which were commended and encouraged. For the departments ranked in the bottom 5% or whose scientific research index significantly declined compared with previous year, early warning, guidance and supervision were implemented. Since the implementation of the evaluation system, the research performance of disciplines has been significantly improved, and many achievements were made.Conclusions:This evaluation mode can stimulate the enthusiasm of the disciplines and scientific researchers for entrepreneurship and innovation to set up the standards and promote the continuous improvement of the research capacity of the whole hospital.

摘要

Objective:To investigate the expression of placental glucose transport protein (GLUT)1 and 4 in women with hyperglycemia in pregnancy.Methods:A total of 48 full-term singleton pregnant women who underwent elective cesarean section at Peking University First Hospital from January 2017 to December 2019 were included retrospectively, and were divided into pregestational diabetes mellitus (PGDM) group, gestational diabetes mellitus (GDM) group, and normal glucose tolerance (NGT) group (16 women in each). According to the blood glucose level after control during pregnancy, the PGDM and GDM groups were further divided into PGDM-, GDM-well controlled subgroups (both n=10), and PGDM-, GDM-uncontrolled subgroups (both n=6). The expression of GLUT1 and GLUT4 proteins, detected by Western blot, between different groups were compared and the correlation between GLUT1 and GLUT4 protein expression and the pre-pregnancy body mass index (BMI), pre-delivery weight,gestational weight gain (GWG), and neonatal birth weight were analyzed using two independent samples t-test, one-way analysis of variance, Kruskal-Wallis H-test, Chi-square test, and Pearson correlation analysis. Results:(1) The pre-pregnanct BMI and insulin treatment ratio in PGDM group were higher than those in GDM group and NGT group [27.0 kg/m 2 (22.1-29.9 kg/m 2) vs 22.9 kg/m 2 (20.5-25.7 kg/m 2) and 21.7 kg/m 2 (20.5-24.3 kg/m 2); 13/16 vs 1/16 and 0/16; all P<0.05]. In the PGDM group, fasting blood glucose at the second trimester and mean glycosylated hemoglobin during the second and the third trimester were higher than those in the GDM group [(6.1±1.2) vs (5.0±0.4) mmol/L; (5.9±0.5)% vs (5.2±0.3)%; both P<0.05]. (2) GLUT1 protein level was the highest in the PGDM group, followed by the GDM and NGT group (1.251±0.354 vs 1.004±0.368 vs 0.733±0.216, both P<0.05). And all the subgroups had higher protein level than NGT group [PGDM-good (1.270±0.417); PGDM-uncontrolled (1.218±0.245), GDM-well controlled (0.900±0.424); GDM-uncontrolled (1.177±0.158); LSD test,all P<0.05]. The variation of GLUT4 protein level was consistent with GLUT1. There was no significant difference in GLUT1 and GLUT4 expression between PGDM-uncontrolled and PGDM-well controlled subgroups, neither between GDM-uncontrolled and GDM-well controlled subgroups (all P>0.05). (3) GLUT1 and GLUT4 in the PGDM-well controlled subgroup, as well as GLUT1 in the GDM-uncontrolled subgroup, had a positive correlation with GWG ( r=0.635, 0.810, and 0.833, all P<0.05). Conclusions:The increased expression of placental GLUT1 and GLUT4 were associated with different types of gestational hyperglycemia and GWG. However, glucose control has little effect on placental GLUT expression.

摘要

Objective:To study the intraoperative surgical factors which influenced non-immediate postoperative tracheal extubation (IPTE) after liver transplantation.Methods:The clinical data of all liver transplant recipients operated at the Qilu Hospital of Shandong University from January 2011 to July 2019 were reviewed. Patients who returned to a surgical ward with a tracheal cannula or who underwent re-intubation within 48 hours after IPTE because of hypoxemia were assigned to the cannula-preserving group (non-IPTE). The remaining liver recipients were assigned to the control group (IPTE). Univariate and multivariate logistic regression analysis were used to analyze the risk factors influencing IPTE.Results:Of 70 patients enrolled into this study, there were 30 patients in the cannula-preserving group (with 25 males, 5 females, and age of 51.8±7.3). And 40 patients in the control group (with 35 males, 5 females, and age of 48.4±9.6). Univariate logistic regression analysis showed anhepatic phase >45 min, blood loss >800 ml and intraoperative hypothermia were related with non-IPTE after liver transplantation(all P<0.05). Multivariate logistic regression analysis revealed anhepatic phase >45 min ( OR=3.972, 95% CI: 1.193-13.220, P=0.025) and intraoperative hypothermia ( OR=23.682, 95% CI: 2.434-230.438, P=0.006) increased the risk of unsuccessful IPTE. Conclusion:A long anhepatic phase and intraoperative hypothermia were surgical risk factors affecting non-IPTE after liver transplantation. Surgeons should avoid patients having hypothermia and a prolong anhepatic phase during liver transplantation.

摘要

Pregnant women with preexisting diabetes have increased adverse maternal and neonatal outcomes.Preconception planning is necessary for avoiding unintended pregnancies and mitigating risk of congenital defects.The recommended glycosylated hemoglobin goals are <6.5% before conception and <6.0% during pregnancy.Screening and management for diabetic complications are critical,strict blood pressure control goal need to be achieved,especially for those complicated by nephropathy.Continuous glucose monitoring during pregnancy might help improve blood glucose control for women with type 1 diabetes.Insulin is still the first-line therapy for pregnant women with preexisting diabetes.Optimization of glycemic control,appropriate medication regimens and close attention to comorbidities can help minimize the matemal and neonatal adverse outcomes and ensure the quality of clinical management for women with preexisting diabetes before,during,and after pregnancy.

However, although porcine reports are rare they are quite essential, as the pig is an important animal model for the in vitro generation of human organs. In this study, we reprogramed porcine embryonic fibroblasts into porcine iPSCs (piPSCs), and differentiated them into cluster of differentiation 31 (CD31)-positive endothelial cells (ECs) (piPSC-derived ECs, piPS-ECs) using an optimized single-layer culture method. During differentiation, we observed that a combination of GSK3β inhibitor (CHIR99021) and bone morphogenetic protein 4 (BMP4) promoted mesodermal differentiation, resulting in higher proportions of CD31-positive cells than those from separate CHIR99021 or BMP4 treatment. Importantly, the piPS-ECs showed comparable morphological and functional properties to immortalized porcine aortic ECs, which are capable of taking up low-density lipoprotein and forming network structures on Matrigel. Our study, which is the first trial on a species other than human and mouse, has provided an optimized single-layer culture method for obtaining ECs from porcine PSCs. Our approach can be beneficial when evaluating autologous EC transplantation in pig models.

摘要

Objective To investigate the long-term effects of regular exercise during pregnancy on overweight and obese gravidas. Methods This study was based on a previous randomized controlled trial (RCT) regarding Effect of Regular Exercise Commenced in Early Pregnancy on the Incidence of Gestational Diabetes Mellitus in Overweight and Obese Pregnant Women conducted at Peking University First Hospital from December 2014 to July 2016. Totally 265 women whose prepregnant body mass index (BMI) ≥24.0 kg/m2 were enrolled in the RCT and were randomly assigned to exercise group (n=132) and control group (n=133). This study was a follow-up of all the participants at one year after delivery from March 2016 to August 2017. Body weight, body fat, fasting and 2 h glucose level in 75 g oral glucose tolerance test (OGTT), insulin resistance index and lipid profiles were compared. We also evaluated their physical activities with International Physical Activity Questionnaire (IPAQ), their dietary habits with modified Adult Dietary Behavior Assessment Scale and depression condition with Postpartum Depression Screen Scale (PDSS). Two independent samples t-test, Mann-Whitney U test and Chi-square test were used for statistical analysis. ResuLts The overall follow-up rate was 51.7% (137/265), while 64.4% (85/132) in the exercise group and 39.1% (52/133) in the control group. No significant difference in body weight loss [2.3 ( - 6.0 to 5.0) vs 1.0 ( - 1.3 to 4.7) kg], body fat percentage [(38.04±4.54)% vs (37.51±4.90) %], 2 h glucose level in 75 g OGTT [(7.30±2.80) vs (7.32±1.61) mmol/L], abnormal glucose tolerance ratio [30.0% (9/30) vs 28.8% (17/59)] or insulin resistance index (2.92±1.77 vs 2.86±1.92) was found between the control and exercise group (t or Z= - 0.940, 0.312, 3.415, 0.005, 1.743, all P>0.05). However, the fasting blood glucose in 75 g OGTT was higher in the control group [(5.53±0.92) vs (5.28±0.43) mmol/L, t=9.268, P=0.003]. The two groups showed no significant difference (exercise group vs control group) in physical activity level [219 (99-504) vs 195 (99-351)], total score of dietary habits [79.00 (72.50-87.50) vs 76.00 (70.00-82.00)] or PDSS score of depression [55.00 (41.00-77.00) vs 70.00 (46.25-84.75)] at follow-up (Z=-0.808, -1.822, -1.620, all P>0.05). Moreover, there was no significant difference in the level of serum triglyceride [(1.25±1.04) vs (1.42±0.85) mmol/L], total cholesterol [(4.56±0.71) vs (4.40±0.67) mmol/L], high density lipoprotein-cholesterol [(1.32±0.29) vs (1.22±0.21) mmol/L] or low density lipoprotein-cholesterol [(2.67±0.56) vs (2.55±0.52) mmol/L] between the exercise and control group (t=0.001, 0.020, 3.255, 0.303, all P>0.05). ConcLusions Regular exercise during pregnancy has no long-term effect on maternal health in the absence of continuing lifestyle intervention after delivery. Therefore, postpartum follow-up and continued education on healthy lifestyle should be emphasized.

Objective@#To understand the health literacy and healthy city satisfaction of residents in Ningbo,and to provide a basis for further development of healthy city .@*Methods@#The permanent residents aged 15 years old and above in six urban districts of Ningbo were selected by stratified multi-stage sampling and probability-proportionate-to-size sampling method. The health literacy and healthy city satisfaction of residents were surveyed by a self-designed questionnaire. Multivariate linear regression model was used to analyze the correlation between health literacy and healthy city satisfaction .@*Results@#A total of 3 300 people were investigated and 3 035 valid questionnaires were returned,with an effective rate of 91.97%. The scores of health literacy were 22.67±4.83 in males and 23.04±3.13 in females;the scores of healthy city satisfaction were 3.30±0.32 in males and 3.31±0.33 in females;there were no statistically significant differences in the scores above between genders(P>0.05). The scores of health literacy were 25.65±4.14 in the residents aged 35-44 years and 20.34±3.54 in the residents aged 15-34 years;the scores of healthy city satisfaction were 3.16±0.32 in the former and 3.45±0.31 in the latter;the former scored higher in health literacy but lower in healthy city satisfaction than the latter (both P<0.05). The scores of health literacy were 23.80±3.90 in the married residents and 18.94±3.22 in the single,divorced or widowed ones;the scores of healthy city satisfaction were 3.35±0.33 in the former and 3.22±0.32 in the latter;the former scored higher both in health literacy and healthy city satisfaction than the latter (both P<0.05). The results of multivariate linear regression analysis showed that the higher the scores of health literacy,the higher the scores of healthy city satisfaction (β'=0.028,P<0.05) .@*Conclusion@#Improving health literacy of residents could improve their satisfaction with healthy city construction.
