摘要

Purpose To investigate the diagnostic value of 18F-FDOPA PET/CT imaging and semi-quantitative analysis platform for the diagnosis of Parkinson's disease(PD).Materials and Methods There were 27 healthy controls and 56 clinically diagnosed PD patients,including 33 early PD(Hoehn-Yahr class Ⅰ-Ⅱ)and 23 advanced PD(Hoehn-Yahr class Ⅲ-Ⅳ),underwent 18F-FDOPA PET imaging in Nanjing First Hospital,Nanjing Medical University were consecutively enrolled from January 2018 to December 2019.The striatal to occipital ratio(SORs)in radioactivity was calculated by HERMES BRASS platform,thereby completing the semi-quantitative analysis of the brain based on regions of interest and observing the asymmetry of the striatal subregions in early-stage PD and late-stage PD patients.Using artificial intelligence techniques to perform principal component analysis on the SORs of the striatal subregions in PD group and healthy control group,the degree of data aggregation and the distinguishability between groups were observed.Results The SORs was significantly reduced in the whole caudate,anterior,posterior putamen and striatum of advanced PD patients(t=9.02-11.72,P<0.000 1).The area under the curve was 0.952,0.973,0.995 and 0.982,respectively.Compared with the healthy control group,the loss of striatal asymmetry index(mean)in each subregion of the striatum in early PD group was caudate(7.61±5.50)%,anterior putamen(11.43±8.97)%,posterior putamen(17.17±11.63)%,and whole striatum(10.65±7.46)%,respectively.The uptake of 18F-FDOPA in the striatum of PD patients was significantly reduced,and the most obvious loss of early PD patients was contralateral posterior putamen,with a decrease of 34%.Conclusion The platform semi-quantitative analysis of 18F-FDOPA PET/CT images provides objective semi-quantitative values for early diagnosis and differential diagnosis of PD.Asymmetry in the striatum,especially in the putamen,may be an important parameter for early diagnosis of PD..

Objective:To study the feasibility of magnetic resonance imaging(MRI)technique with amide proton transfer(APT)in predicting the prognosis of cerebral stroke.Methods:A total of 71 patients with acute cerebral stroke who admitted to the Nanjing First Hospital,Nanjing Medical University from September 2022 to May 2023 were selected.All of them underwent the test of National Institute of Health Stroke Scale(NIHSS),and received the MRI examination with chemical exchange saturation transfer(CEST).According to the modified Rankin scale(mRS)values of 1-month follow-up,they were divided into favorable recovery group(mRS<2,44 cases)and poor group(mRS≥2,27 cases).The asymmetric magnetization transfer ratio(MTRasym)image(APT)was obtained by analyzing data with special software.And then,the difference(△APTw)of APT values between ischemic zone and contralateral normal tissue was further calculated.The △APTw values of two groups were compared and analyzed,and the Pearson correlation analysis was adopted to analyze the correlation among △APTw,NIHSS and mRS.The receiver operating characteristics(ROC)curve was drawn,and the area under curve(AUC)of ROC curve was calculated.Results:There were significant positive correlations among △APTw,NIHSS and mRS scores(R2=0.659,0.522,P<0.001),and the differences of △APTW,NIHSS and mRS scores between the favorable recovery group and poor group were significant(t=5.73,6.36,13.92,P<0.05),respectively.The AUC value was 0.886,and the sensitivity and specificity of prediction were respectively 77.8%and 95.5%.The positive and negative predictive values were respectively 91.3%and 87.5%.Conclusion:APT imaging technique has feasibility in predicting the prognosis of acute cerebral ischemic stroke.

摘要

Objective:To investigate the effect of hyperbaric oxygen preconditioning on attention network functioning after chemotherapy.Methods:A total of 68 patients scheduled to receive chemotherapy were randomly divided into a hyperbaric oxygen preconditioning group (HBOPC, n=22), a hyperbaric oxygen sensitizing chemotherapy group (HBOSC, n=26), and a control group ( n=20). After 6 cycles of chemotherapy, the attention network test was administered to the patients in all three groups. Results:After the treatment, the average alerting efficiencies of the HBOPC and HBOSC groups were significantly higher than that of the control group. Their executive control was also significantly better. There were also significant differences in executive control between that the HBOPC and HBOSC groups, with the HBOPC group performed better. The post-treatment total reaction times of both the HBOPC and HBOSC groups were significantly shorter than that of the control group. That of the HBOPC group was significantly shorter than the HBOSC group′s total.Conclusions:Hyperbaric oxygen therapy can effectively reduce network function damage in terms of alerting and executive control. HBOPC protects executive control better than HBOSC.

摘要

Objective To investigate the regulative effect of exogenous Angiotcnsin-(1-7) [Ang(1-7)] on nuclear factor-κB (NF-κB) and its downstream proinflammatory cytokines in rats after cerebral ischemic-reperfusion injuries.Methods Forty-two male SD rats were randomly divided into sham-operated group,control group and Ang-(1-7) treatment group (n=14); cerebral ischemia in control group and Ang-(1-7) treatment group was induced by transient middle cerebral artery occlusion (MCAO).The rats in the sham-operated group and control group were infused with artificial cerebrospinal fluid (aCSF,0.5 μL/h) while rats in the Ang-(1-7) treatment group were with Ang-(1-7)(100 pmol,0.5 μL/h)into the lateral ventricle by implanted osmotic minipumps following reperfusion.At 24 h after reperfusion,all rats were sacrificed to detect the expression of NF-κB p65 subunits in cell nuclei of the ischemic cortex by Western blotting.The spatial distribution of NF-κB p65 subunits in ischemic cerebral tissues was detected by immuno-histochemical assay.The concentrations of tumor necrosis factor-α (TNF-α) and interleukin-1β (IL-1β) in serum were detected by ELISA.Results A significant reduction of NF-κB p65 expression in cell nuclei by 46% in the Ang-(1-7) treatment group was noted as compared with that in control group (P<0.05).The nuclear translocation of NF-κB p65 in the ischemic cortical cells in the Ang-(1-7) treatment group was significantly reduced,and the rate of NF-κB p65 positive cells was decreased by 29% as compared with that in control group (P<0.05).Besides,the serum levels of TNF-α (71.603±18.539 pg/mL) and IL-1 β (44.648±10.387 pg/mL) in the Ang-(1-7) treatment group were obviously decreased as compared with those in the control group (104.763±24.412 pg/mL,64.787±14.441 pg/mL,P<0.05).Conclusion Exogenous administration of Ang-(1-7) could attenuate inflammatory reaction following cerebral ischemia,perhaps by interacting with Mas receptor or through the antagonism against the pro-inflammatory effect of Ang Ⅱ.
