摘要

Background: Cervical cancer is unique among human cancers which is mostly attributable to infection. Conventional PAP smear method is most effective for prevention and detection of cervical cancer but the accuracy of this method is low. This PAP smear now evolved to Liquid Based Cytology (LBC). Method: All the patients visiting Gynaecology OPD who fulfilled the inclusion Criteria in the duration starting from 1 January 2020 to 30 June 2021 in the Department of Pathology of Gajra raja Medical College, Gwalior (MP) were included in this study. 50 atypical smears by Conventional PAP test were then collected and these cases were subjected to Eziprep Liquid Based Cytology and Conventional PAP smear in private laboratory setup after taking history and clinical examination. The smears were studied by using 7 morphological parameters. Smears were analysed for adequate cellularity, clean background, uniformResult: distribution of cells, cellular overlapping, inflammation, distinct cell border, nuclear irregularity and then categorise by Bethesda reporting system. The results were significant only for clean background, uniform distribution of cells, cellular overlapping and inflammation. Conclusion: Results of cervical cytology smears by both methods showed that LBC provides more representative sample with reduced obscuring material, improved clarity allowing better morphological evaluation.

BACKGROUND: Cervical cytology for uterine cervical cancer screening has transitioned from conventional smear (CS) to liquid-based cytology (LBC), which has many advantages. The aim of this study was to compare the proportion of unsatisfactory specimens from CS versus LBC at multiple institutions including general hospitals and commercial laboratories. METHODS: Each participating institution provided a minimum of 500 Papanicolaou (Pap) test results for analysis. Pap tests were classified according to the participating institution (commercial laboratory or general hospital) and the processing method (CS, ThinPrep, SurePath, or CellPrep). The causes of unsatisfactory results were classified as technical problems, scant cellularity, or complete obscuring factors. RESULTS: A total of 38,956 Pap test results from eight general hospitals and three commercial laboratories were analyzed. The mean unsatisfactory rate of LBC was significantly lower than that of CS (1.26% and 3.31%, p = .018). In the LBC method, samples from general hospitals had lower unsatisfactory rates than those from commercial laboratories (0.65% vs 2.89%, p = .006). The reasons for unsatisfactory results were heterogeneous in CS. On the other hand, 66.2% of unsatisfactory results in LBC were due to the scant cellularity. CONCLUSIONS: Unsatisfactory rate of cervical cancer screening test results varies according to the institution and the processing method. LBC has a significantly lower unsatisfactory rate than CS.

摘要

Objective To explore the diagnostic value of thin prep cytology test(TCT) in central lung cancer. Methods All 127 patients suspected central lung cancer were detected by transbronchial needle aspiration(TBNA). The patients were randomly assigned into two groups, the group of conventional smear,CS (62 cases) and the group of CS+TCT(65 cases). The sensitivity and specificity of two methods were compared. Results The sensitivity and specificity of CS were 75.9%and 100.0% respectively. The sensitivity and specificity of CS+TCT were 96.2% and 100.0% respectively. The sensitivity of CS+TCT was significantly higher than sensitivity of CS in diagnosis of lung cancer (P=0.000046<0.01). Conclusion Combination of CS and TCT techniques can improve the TBNA diagnostic sensitivity of lung cancer. TBNA-TCT detection methods should be applied to clinical activites.

BACKGROUND: The aims of this study were to compare the cytomorphologic features diagnostic of atypical squamous cells (ASC) in liquid-based preparations (LBPs) and conventional Pap (CP) smears and to cytomorphologically assess the performance of the Cell Scan 1500(TM) in cervical cytology practice. METHODS: Cervicovaginal smears were obtained from 938 women. Two smears were obtained simultaneously from each individual, one for an LBP and the other for a CP smear; the smears were independently examined. ASC was diagnosed in 24 patients, and their samples were cytomorphologically and semiquantitatively analyzed. RESULTS: A total of 24 of the 938 women (2.6%) were diagnosed with ASC by one or both methods. Results from LBPs and CP smears were in agreement in 13 of 24 cases of ASC diagnosis (absolute direct agreement, 54.2%; k<0.20; p-value from chi-square test=0.085). Diagnostic features of ASC in the LBPs included squamous cell atypia and atypical squamous metaplasia. CONCLUSIONS: The cellular features diagnostic of ASC present in one preparation can manifest themselves differently in the other. Changes in individual cells, particularly nuclear changes, are the most reliable features for diagnosing ASC. The Cell Scan 1500(TM) processor is more effective at detecting ASC than are CP smears.

摘要

Objective To make a comparative analysis of conventional smear and liquid-based cytology method to diagnose lung cancer in sensitivity and false-negative,and evaluate the application value of liquid-based cytology in sputum samples.Methods We collected 287 sputum samples through clinical contrast conventional smear with liquid-based cytology method.The diagnosis was made by the same pathology panel in double blind way.We compared the two methods to diagnose lung cancer in sensitivity and false-negative.Results Liquid-based cytology method detected 62 cases of lung cancer,and conventional smear detected 49 cases of lung cancer,with the sensitivity rate being 21.6% and 17.1%,respectively.Combination of the two methods made the sensitivity rate 24.0%.The false-negative rate was 38.6% for liquid-based cytology method and 48.5% for conventional smear which did not differ significantly.By combining the two methods,the false-negative rate was 24.0%.There was no significant difference between liquid-based cytology method and conventional smear,and between liquid-based cytology method and the two-method combination in sensitivity and false-negative.But there was a significant difference between the two-method combination and conventional smear.Conclusion Through sputum sample detection of lung cancer cell,liquid-based cytology method is better than conventional smear,the former having obvious superiority;combining the two methods can detect more lung cancers.Therefore,it is a superior screening test in detecting lung cancer and worthy of clinical popularization.
