摘要

The misuse of anabolic androgenic steroid associated or not with physical workouts disrupts gastrointestinal (GI) function homeostasis. Our goal was to investigate the effects of nandrolone decanoate (ND) and moderate swimming on the GI transit of solid meals, GI motor contractility, and intestinal histology in rats. Male Wistar rats were allocated to four groups that received intramuscular injections of ND (5.0 mg/kg) or vehicle (60.0 µL) and were submitted or not to swimming sessions (60 min, 5% body weight overload) for 4 weeks. Gastric emptying, intestinal transit, in vitro GI contractility, intestinal morphometry, and duodenal mucosal mast cells were evaluated in all experimental groups. ND treatment accelerated gastric emptying, slowed small intestine transit time, enhanced gastric carbachol-mediated reactivity, decreased crypt depth and villus height, reduced mucosal thickness, and increased the circular and longitudinal muscle layer thickness of the duodenum in sedentary rats. Moderate exercise accelerated intestinal transit time and reduced submucosa thickness. In vehicle-treated animals, a strong negative correlation was found between intestinal transit and mucosal mast cells, which was reversed by ND treatment. Combining ND treatment and swimming accelerated gastric emptying, increased duodenal cholinergic reactivity, inhibited the sodium nitroprusside relaxing response, increased the number of duodenal mast cells, decreased villus height, and increased the thickness of all muscle layers. ND changed the morphological and functional properties of the GI tract over time, with intense dysmotility, especially in sedentary animals, but moderate exercise seemed to have played a compensatory role in these harmful effects in the gut.

Presentación del caso. Se trata de una mujer de 26 años de edad, en seguimiento por la especialidad de reumatología desde los 17 años, cuando consultó con historia de un año de evolución de síndrome poliarticular de grandes y pequeñas articulaciones, aditivo, simétrico acompañado de fatiga, rigidez matutina mayor de una hora. Se reportó además factor reumatoide positivo. La radiografía de ambas manos presentó erosiones, que confirmó el diagnóstico de artritis reumatoide. Adicionalmente, la paciente tenía el antecedente de procesos sinobronquiales a repetición desde su infancia. En la evaluación médica se identificó dolor en los senos paranasales, dextrocardia y bronquiectasias, confirmados por los estudios de imágenes, que permitió concluir en el diagnóstico de síndrome de Kartagener. Intervención terapéutica. La paciente presentaba actividad clínica severa de la artritis reumatoide, se inició el tratamiento con metotrexato 10 mg vía oral un día a la semana, prednisona 5 mg al día y ácido fólico 5 mg a la semana y citas periódicas, controlando los datos de actividad y efectos adversos de los medicamentos, con pruebas hepáticas, hemograma y transaminasas. La especialidad de neumología recomendó la inclusión de la paciente en un programa de rehabilitación respiratoria, así como el uso de azitromicina 500 mg cada día por tres días en los períodos de agudización. Evolución clínica. El tratamiento logró mantener una actividad leve de la artritis reumatoide y sin exacerbación de los síntomas respiratorios


Case presentation. A 26-year-old woman, under follow-up by the rheumatology specialty since she was 17 years old, when she consulted with a history of one year of evolution of polyarticular disease of large and small joints, additive, symmetrical, accompanied by fatigue and morning stiffness for more than one hour. Positive rheumatoid factor was also reported. Additionally, the patient had a history of repeated sinobronchial processes since childhood. Medical examination revealed sinus pain in the paranasal sinuses, dextrocardia, and bronchiectasis, confirmed by imaging studies, which led to the diagnosis of Kartagener's syndrome. Treatment. The patient presented the severe clinical activity of rheumatoid arthritis. The treatment was started with methotrexate 10 mg orally one day a week, prednisone 5 mg a day, and folic acid 5 mg a week and periodic appointments, controlling the activity data and adverse effects of the drugs, with liver tests, hemogram, and transaminases. The pneumology department recommended the inclusion of the patient in a respiratory rehabilitation program as well as the use of azithromycin 500 mg every day for three days during periods of exacerbation. Outcome. The treatment was successful in maintaining a mild activity of the rheumatoid arthritis and without exacerbation of respiratory symptoms

摘要

ABSTRACT After bariatric surgery one of the most common complications is dysphagia. The etiology of this disease has not been fully elucidated but it is known that it may be due to structural changes due to surgery. This case describes a 65-year-old female with early and severe onset of dysphagia following laparoscopic sleeve gastrectomy. The patient's final diagnosis was postobesity surgery esophageal dysfunction and laparoscopic proximal gastrectomy with esophagojejunal Roux-en-Y anastomosis was performed. Physicians should be aware of this condition in order to offer early diagnosis and treatment.


RESUMEN Después de una cirugía bariátrica una de las complicaciones más comunes es la disfagia. La etiología de esta enfermedad no ha sido completamente dilucidada, pero se sabe que puede deberse a cambios estructurales debidos a la cirugía. En este reporte se describe el caso de una mujer de 65 años con disfagia severa de aparición temprana después de una en manga gástrica laparoscópica. El diagnóstico final del paciente fue de una disfunción esofágica posterior a una cirugía de obesidad y se planteó como manejo una gastrectomía proximal laparoscópica con anastomosis esofagoyeyunal en Y de Roux. Hay que tener en cuenta las complicaciones a corto y largo plazo que se pueden presentar luego de cirugías de obesidad para poder realizar un diagnóstico temprano y poder ofrecer un tratamiento adecuado.

摘要

Objective To clarify the effects of 5G mobile phone radiofrequency radiation exposure on male mouse fertility and to preliminarily explore the underlying mechanisms. Methods Healthy male C57BL/6 mice aged 7-8 weeks were randomly assigned to Sham group, 3.5 GHz radiofrequency radiation group, and 4.9 GHz radiofrequency radiation group, with 16 mice in each group. The mice were exposed to 3.5 GHz or 4.9 GHz mobile phone radiofrequency radiation for 42 consecutive days (1 h per day). The sperm quality was evaluated using sperm count, deformity rate, and motility. H&E staining was performed to assess testicular tissue structure by observing the morphology of spermatogenic cells at various development stages, the diameter of seminiferous tubules, and the thickness of seminiferous epithelium. The sperm mitochondrial function was assessed using sperm mitochondrial membrane potential and testicular ATP content. The fertility of mice was evaluated through fertility rate, litter size, and survival rate of offspring. The underlying mechanisms were explored by detecting the methylation of LRGUK gene and its mRNA and protein levels. Results Compared with the Sham group, there were no significant changes in sperm count in the 3.5 GHz and 4.9 GHz groups; however, the sperm abnormality rate significantly increased (P < 0.05) and sperm motility significantly decreased (P < 0.05). The structure of testicular tissue, the function of sperm mitochondria, and fertility of mice showed no significant changes as compared with the Sham group. The methylation level of LRGUK gene in the testes significantly increased, while the mRNA and protein expression levels significantly decreased. Conclusion Exposure to 3.5 GHz and 4.9 GHz mobile phone radiofrequency radiation for 42 consecutive days can lead to an increase in sperm deformity rate and a decrease in sperm motility in mice, but has no significant effect on fertility, which may be related to an increase in methylation level of the LRGUK gene in the testes.

摘要

The gastrointestinal motility disorder of the patients with Parkinson's disease(PD)occurs in the early stages of this disease,even before the onset of motor symptoms.The gastrointestinal motility disorder is one type of gastrointestinal dysfunction,not only affect the absorption of medication,exacerbating the progression of PD,but also severely impact the quality of life of the patients.Therefore,it is essential to find new therapeutic targets to alleviate PD-induced gastrointestinal dysmotility in order to improve the progression of the disease and the quality of life of the patients.The gastrointestinal motility function is highly dependent on the health of the gut and central nervous regulating the gastrointestinal movements.A healthy gut is closely related to the integrity of the intestinal barrier,gut microbiota,neuroinflammation,and the normal function of enteric neurons responsible for the contraction and relaxation of the gastrointestinal tract.The gut function of the PD patients is compromised to some extent.This review summarizes the effects of the enteric nervous system,central nervous system,and gut microbiota on the development of gastrointestinal motility disorder of the PD patients;it also outlines the current therapeutic methods available and their limitations,with the aim of providing the new insights into the treatment of gastrointestinal motility disorder of the PD patients.

摘要

Objective @#To use linear PCR fragment containing antibiotic resistance cassette to carry out homologous recombination and replacement of target gene fragment of Acinetobacter baumannii to achieve rapid gene knockout and functional verification.@*Methods@#Acinetobacter baumannii Ab4294 was used as the research object,and the upper (901 bp) and lower ( 1 028 bp) reaches of fim gene cluster (4 980 bp in length) were amplified by PCR , which was used as the recombinant homologous arm.Kanamycin antibiotic resistance cassette (KanR) was ampli- fied from pUC57 plasmid.The above three fragments were connected by overlapping extended PCR technique,and the connected fragments were transformed into wild Acinetobacter baumannii strains.The gene deletion mutant was screened,and the plasmid complement strain was constructed.The phenotype of the obtained strains was identi- fied,and the function of fim gene cluster was explored. @*Results @#A mutant strain of Acinetobacter baumannii Ab4294 with deletion of fim gene cluster was successfully constructed by homologous substitution of linear PCR frag- ment containing antibiotic resistance cassette.Compared with the wild strain,the growth curve of the deletion strain had no significant difference ,and the rubbing ability significantly decreased ,and the phenotype recovered after complementing the gene cluster.@*Conclusion @#The fim family genes of Acinetobacter baumannii Ab4294 is success- fully knocked out by homologous substitution of linear PCR fragment containing antibiotic resistance cassette,which encodes the product involved in the motile movement of Acinetobacter baumannii.

摘要

ObjectiveTo explore the possible mechanism of pediatric tuina therapy in treating anorexia nervosa. MethodsTotally 120 children with anorexia nervosa were randomly divided into a tuina group and a medication group, with 60 cases in each group. Sixty healthy children undergoing physical examinations were recruited as the healthy control group. Children in the tuina group received traditional pediatric tuina therapy, while those in the medication group received orally chewed Jianwei Xiaoshi tablets. Each treatment course lasted for 7 days, with a 1-day interval between courses, and a total of 4 courses were administered. Before and after treatment, seven indicators including gastric motility frequency, gastric area, gastric area 30 minutes after drinking, anterior-posterior diameter and area during gastric fundus dilation, anterior-posterior diameter and area during gastric fundus contraction were measured using a color Doppler ultrasound diagnostic instrument in children from the healthy control group, tuina group, and medication group. Additionally, gastric emptying rate at 30 minutes, changes in anterior-posterior diameter and area during gastric fundus contraction, and changes in gastric area were compared. ResultsThis study ultimately included 60 healthy children in the control group, 59 children in the tuina group, and 51 children in the medication group. Compared with the control group at baseline, the gastric area and the anterior-posterior diameter and area during gastric fundus contraction increased, while the gastric emptying rate, gastric motility frequency, and changes in anterior-posterior diameter during gastric fundus contraction decreased in both the tuina group and medication group, with only a decrease in gastric area during gastric fundus contraction observed in the tuina group (P<0.05). Compared with baseline, after treatment, the gastric emptying rate, gastric motility frequency, and changes in anterior-posterior diameter and area during gastric fundus contraction increased in the tuina group, while the gastric area and area during gastric fundus contraction decreased 30 minutes after treatment; in the medication group, gastric motility frequency and changes in anterior-posterior diameter during gastric fundus contraction increased, while the area during gastric fundus contraction decreased (P<0.05 or P<0.01). Compared with the medication group after treatment, the gastric area decreased 30 minutes after treatment, while the gastric emptying rate and gastric motility frequency increased (P<0.05). ConclusionThe possible mechanism of pediatric tuina therapy in treating anorexia nervosa is to promote gastric motility and gastric emptying, thereby improving gastrointestinal dysfunction in children.

摘要

ObjectiveTo clarify the relationship between intestinal flora and intestinal motility in rats with slow transit constipation (STC) and qi stagnation syndrome by conducting a pseudo-sterile experiment and fecal microbiota transplantation (FMT) technology. MethodsTwenty-four Wistar rats were randomly divided into normal group (n=6), STC with qi stagnation pattern group (n=6) and pseudo-sterile group (n=12). In the STC group with qi stagnation pattern, 3 mg/kg of loperamide suspension by intragastric administration combined with tail clamping stimulation were performed to establish the rat model of STC with qi stagnation pattern. After successful modeling, fresh feces from the rats in the STC with qi stagnation pattern group and the normal group were collected to prepare 100 mg/ml of fecal bacterial suspension. In the pseudo-sterile group, the antibiotic cocktail method was used (a mixed antibiotic suspension containing bacitracin, streptomycin sulfate, and neomycin sulfate at 20 mg/ml each was administered intragastrically) to establish pseudo-sterile rats model. After successful modeling, the rats were randomly divided into normal fecal bacterial liquid group and STC with qi stagnation pattern fecal bacterial liquid group, with six rats in each group, and then were given 10 ml/kg of the prepared corresponding rat fecal bacterial suspension by gavage. Rats in STC with qi stagnation pattern group were given an equal volume of sterile water by gavage. All groups were administered once a day for 7 consecutive days. The small intestinal propulsion rate of the STC with qi stagnation pattern group, the normal fecal bacterial liquid group, and STC with qi stagnation pattern fecal bacterial liquid group were compared. ELISA method was used to detect serum 5-hydroxytryptamine (5-HT) levels. Immunohistochemistry was used to detect the positive expression levels of 5-hydroxytryptamine 3 receptor (5-HT3R) and 5-hydroxytryptamine 4 receptor (5-HT4R) in colon tissue. Western blot method was used to detect the protein expression levels of tryptophan hydroxylase 1 (TPH1), tryptophan hydroxylase 2 (TPH2), serotonin transporter (SERT), and monoamine oxidase A (MAO-A) in colon tissue. ResultsCompared to those in the normal fecal bacterial liquid group, the small intestinal propulsion rate, serum 5-HT level, positive expression of 5-HT3R and 5-HT4R in colon tissue, and protein expression of TPH1, TPH2, SERT and MAO-A significantly decreased in the STC with qi stagnation pattern group and STC with qi stagnation pattern fecal bacterial liquid group (P<0.05). There was no statistically significant difference in the indicators between the STC with qi stagnation pattern group and STC with qi stagnation pattern fecal bacterial liquid group (P>0.05). ConclusionThe intestinal flora in STC rats with qi stagnation pattern can lead to a slowdown in intestinal transmission function, whose mechanism may be related to intestinal motility disorders affected by the synthesis, transport, metabolism and other pathways of 5-HT.

摘要

Diabetic gastroenteropathy is a serious chronic complication that accompanies the progression of diabetes mellitus, severely impacting patients' quality of life and overall health. Nearly half of diabetic patients experience symptoms such as nausea, vomiting, early satiety, abdominal distension, and abdominal pain, which increases their anxiety and depression, prompting frequent medical visits and further burdening the healthcare system. In-depth research into the pathogenesis of diabetic gastroenteropathy has identified several core mechanisms, including hyperglycemia, autonomic and enteric nervous system dysfunction, abnormal secretion of gastrointestinal hormones, macrophage polarization, brain-gut axis dysregulation, microRNA deficiency, and oxidative stress-induced damage and apoptosis of interstitial cells of Cajal (ICC). Current clinical treatments mainly rely on prokinetic and antiemetic drugs. However, their notable adverse effects and diminishing efficacy with long-term use remain pressing issues. Traditional Chinese medicine (TCM), with its unique theoretical framework and extensive practical experience, potent in prescription formulation and acupoint selection guided by holistic concepts and syndrome differentiation, has gradually become an important option for treating diabetic gastroenteropathy. Numerous studies have confirmed that mechanisms include improving gastrointestinal hormone secretion, repairing ICC damage, regulating the nervous system, reducing oxidative stress, and modulating the brain-gut axis. These findings provide new insights into the treatment of diabetic gastroenteropathy. This article summarized the pathogenesis of diabetic gastroenteropathy and reviewed recent research on Chinese medicine and acupuncture-moxibustion therapy in improving gastrointestinal motility for diabetic gastroenteropathy treatment, aiming to offer clinical treatment insights and highlight the need for further research to explore comprehensive and individualized treatment approaches, providing better strategies for managing diabetic gastroenteropathy.

摘要

Chaihu Shugansan composed of Bupleuri Radix, Paeoniae Radix Alba, Chuanxiong Rhizoma, Aurantii Fructus, Citri Reticulatae Pericarpium, Cyperi Rhizoma, and Glycyrrhizae Radix et Rhizoma has the effects of soothing liver, relieving depression, regulating Qi movement, and relieving pain. It is a classic formula for treating gastric distension recommended by doctors of later ages. This article systematically reviews the clinical application and basic experimental progress of Chaihu Shugansan in the treatment of functional dyspepsia. In modern clinical practice, Chaihu Shugansan and its modified formulas are used to treat functional dyspepsia, and they can be applied in combination with other formulas (Si Junzitang, Jinlingzisan, Zhizhuwan, etc.), western medicine (domperidone tablets, deanxit, Saccharomyces boulardii, etc.), traditional Chinese medicine (TCM) acupuncture and other therapies. The results of clinical studies have shown that Chaihu Shugansan and its modified formulas can significantly reduce the Hamilton depression scale (HAMD) score, Hamilton anxiety scale (HAMA) score, and TCM syndrome score, ameliorate the symptoms, improve the quality of life, and decrease the recurrence rate. The experimental pharmacological studies have demonstrated that Chaihu Shugansan can inhibit the autophagy of Cajal interstitial cells, regulate the endoplasmic reticulum stress signaling pathway, and modulate the brain-gut peptide level to improve the gastrointestinal motility. Chaihu Shugansan can inhibit the expression of 5-hydroxytryptamine in the colon tissue and reduce the abdominal withdrawal reflex (AWR) score to improve visceral hypersensitivity. Furthermore, Chaihu Shugansan can lower the levels of pro-inflammatory cytokines such as interleukin-6 and tumor necrosis factor-α to repair duodenal mucosal inflammation. In addition, it can regulate intestinal flora to maintain intestinal flora balance. The main active ingredients such as saikosaponin, paeoniflorin, hesperidin, and naringin in Chaihu Shugansan can exert anti-inflammatory, antioxidant, and antimicrobial effects.

摘要

Objective:To investigate the significance of small intestinal manometry in clinical diagnosis and treatment of patients with gastrointestinal motility disorders.Methods:The clinical data of 20 suspected patients with gastrointestinal motility disorders from February 2019 to January 2020 in Shanghai Jiaotong University Affiliated Ninth People′s Hospital were retrospectively analyzed. All patients were performed small intestinal manometry, and the influence on diagnosis and treatment as well as the clinical value of this technique were analyzed.Results:Among the 20 patients, 18 patients successfully accepted small intestinal manometry. Among the 18 patients, 14 patients showed significant motility abnormalities, among which 9 patients showed significant nutritional improvement after treatment and 5 patients were treated with maintaining nutritional support. In 10 patients, small intestinal manometry served as a deciding diagnostic tool, and in 12 patients, major modifications of treatment strategy were made according to results of small intestinal manometry.Conclusions:The small intestinal manometry significantly improves diagnosis and treatment decision in the patients with gastrointestinal motility disorders and shows great value on the clinical practice on this group of patients.

摘要

ObjectiveTo explore the effect of behavioral vision training on post-stroke ocular motility disorders (PSOMD). MethodsFrom August to Octobor, 2023, a total of 21 PSOMD patients in Beijing Bo'ai Hospital were selected. They received behavioral vision training, including brain-based visual training and visual fusion training, for four weeks. Visual function was assessed with Snellen eye chart, strabismus prism and Titmus near stereopia. ResultsThe vision of both right and left eyes improved after training (Z right eye = -3.601, Z left eye = 3.012, P < 0.01), while the strabismus prism reduced significantly (t = 8.930, P < 0.001). But Titmus near stereopia showed no difference (P > 0.05). ConclusionBehavioral vision training could improve vision and strabismus after stroke.

摘要

OBJECTIVE To investigate the effects of esketamine for multimodal analgesia on opioid consumption and gastric motility in mechanically ventilated non-surgical intensive care unit (ICU) patients. METHODS Forty cases of mechanically ventilated non-surgical patients in the ICU of our hospital from February 1st, 2023 to July 31st, 2023 were selected and randomly divided into control group and esketamine (S-K) group using grouping method with opaque envelopes, with 20 cases in each group. Control group was given sufentanil, and S-K group was infused with Esketamine hydrochloride injection at a constant rate of 0.2 mg/(kg·h)+ sufentanil. The treatment period length, analgesic compliance rate, sedation level, analgesic and sedative consumption, and gastric motility indexes were compared between the two groups. RESULTS There was no statistically significant difference in the treatment period length, analgesic compliance rate, sedation level, or the consumption of propofol and midazolam between the two groups (P>0.05). The consumption of sufentanil in the S-K group was significantly less than control group (P< 0.05). Compared with 1 h after randomization, the antral contraction frequency, antral contraction amplitude and antral motility index of patients in the S-K group were significantly higher at 72 h after randomization and were significantly higher than control group (P<0.05). CONCLUSIONS Esketamine may reduce opioid consumption and improve gastric motility in mechanically ventilated non-surgical ICU patients while ensuring a level of analgesic sedation.

Gastrointestinal motility disorder is an important cause of digestive system diseases. Patients often suffer from nausea, vomiting, gastric retention, gastroparesis, constipation, and many other symptoms, and their quality of life is seriously reduced. Prokinetic agents are routinely used in clinical practice, but their long-term use is prone to problems such as reduced efficacy and increased adverse reactions. Since the incidence of gastrointestinal diseases has continued to rise globally in recent years, there is an urgent need for clinical development of safe and effective treatment strategies. Aurantii Fructus, a traditional Chinese medicine, has the effect of smoothing Qi and eliminating distention, and it has been used to treat gastrointestinal diseases for thousands of years. In modern clinical practice, it is mainly used for the treatment and auxiliary treatment of various gastrointestinal diseases such as functional dyspepsia, functional constipation, and irritable bowel syndrome. The efficacy is remarkable, and no adverse reactions have been reported at conventional doses. Therefore, it can greatly improve the symptoms of patients with gastrointestinal diseases and improve their quality of life. Modern research has revealed that there are many active components in Aurantii Fructus, among which flavonoids have the highest content and the most types. Flavonoids are the main active components in Aurantii Fructus to regulate gastrointestinal motility. Aurantii Fructus and its active components can affect gastrointestinal hormones, neural pathways, Cajal mesenchymal cells, and other multiple mechanisms. They can adjust gastrointestinal motility and correct gastrointestinal motility disorders, showing potential application value in the treatment of gastrointestinal motility disorders. However, a comprehensive analysis of Aurantii Fructus in this aspect is still lacking. This study summarized the pharmacological activities of active components of Aurantii Fructus extract and its flavonoids, volatile oils, alkaloids, and coumarin on the regulation of gastrointestinal motility and explored the latest research progress on its mechanism. Finally, the adverse reactions of Aurantii Fructus were summarized. It aims to provide a scientific basis for the research and clinical application of Aurantii Fructus and its active components in the regulation of gastrointestinal motility.

ABSTRACT Background: The current definition for hypercontractile esophagus was arbitrarily set at the uppermost range in volunteers for a specific equipment. Objective: This study aims to critically analyze the concept of hypercontractile waves to redefine hypercontractile esophagus parameters. Methods: We reviewed 500 unselected and consecutive HRM tests (5000 waves) performed in a water -perfused system. Results: Mean distal contractility integral (DCI) was 825±1492 (0-42775) mmHg.cm.s, two standard deviations above average = 3810; 95th percentile = 2798 mmHg.cm.s. Conclusion: In healthy volunteers, two standard deviations above average is 4000 mmHg.cm.s, we thus suggest this value to define hypercontractile waves and define hypercontractile esophagus in a water-perfused HRM system.

A Retículo Peritonite Traumática está diretamente ligada à ingestão de objetos metálicos perfuro cortantes, como pregos e pedaços de arame, que podem penetrar ou perfurar a parede do retículo. Embora a ocorrência de corpos estranhos de alumínio seja rara, outros elementos, como fios de pneus usados para fixar lonas em silagens e até cerdas de arame de escovas utilizadas em pequenos aeroportos, têm sido registrados como causas dessa afecção. A ingestão desses objetos pode resultar em lesões no retículo, desencadeando episódios de retículo peritonite traumática e suas sequelas, como pericardite traumática, inflamação do fígado, abscessos no baço e muito mais. Os sinais clínicos podem variar, mas incluem comportamento anormal, motilidade reduzida do rúmen, fezes mal digeridas, febre e sinais de dor. O diagnóstico baseia-se na avaliação dos sinais clínicos do animal, juntamente com exames complementares, como hemograma, bioquímico, ultrassom e radiografia. É importante considerar as alterações hematológicas, como leucocitose e hiperfibrinogenemia, como indicadores-chave desta afecção. O diagnóstico também envolve testes de dor ao corpo estranho. O tratamento pode ser conservativo ou cirúrgico. A abordagem conservativa envolve a administração de antibióticos, anti-inflamatórios e soluções intravenosas, além do tratamento de deficiências nutricionais subjacentes. A opção cirúrgica exige a remoção do corpo estranho do retículo, seguida de cuidados pós-operatórios.


Traumatic Reticulum Peritonitis this directly linked to the ingestion of sharp metallic objects, such as nails and pieces of wire, which can penetrate or pierce the wall of the reticulum. Although the occurrence of aluminum foreign bodies is rare, other elements, such as tire wires used to attach tarpaulins to silage and even wire bristles from brushes used in small airports, have been recorded as causes of this condition. Ingestion of these objects can result in damage to the reticulum, triggering episodes of traumatic reticulum peritonitis and its sequelae, such as traumatic pericarditis, liver inflammation, spleen abscesses and more. Clinical signs may vary but include abnormal behavior, reduced rumen motility, poorly digested feces, fever and signs of pain. The diagnosis is based on the evaluation of the animal's clinical signs, together with complementary tests, such as blood count, biochemistry, ultrasound and radiography. It is important to consider hematological changes, such as leukocytosis and hyperfibrinogenemia, as key indicators of this condition. Diagnosis also involves foreign body pain testing. Treatment can be conservative or surgical. The conservative approach involves administering antibiotics, anti-inflammatories and intravenous solutions, in addition to treating underlying nutritional deficiencies. The surgical option requires removal of the foreign body from the reticulum, followed by post-operative care.


La peritonitis traumática del retículo esto directamente ligada a la ingestión de objetos metálicos punzantes, como clavos y trozos de alambre, que pueden penetrar o perforar la pared del retículo. Aunque la aparición de cuerpos extraños de aluminio es rara, se han registrado como causas de esta afección otros elementos, como los alambres de neumáticos utilizados para fijar las lonas al ensilaje e incluso las cerdas de alambre de los cepillos utilizados en los aeropuertos pequeños. La ingestión de estos objetos puede provocar daños en el retículo, desencadenando episodios de peritonitis traumática del retículo y sus secuelas, como pericarditis traumática, inflamación del hígado, abscesos del bazo y más. Los signos clínicos pueden variar, pero incluyen comportamiento anormal, motilidad ruminal reducida, heces mal digeridas, fiebre y signos de dolor. Obesity is a very common pathology worldwide. Among the management alternatives are glucagon-like peptide-1 (GLP-1) analogues, a hormone secreted mainly by the intestine. Apart from its effects as an incretin, effects on gastrointestinal motility have been described, which seem to be fundamental for its effect on obesity, but also the cause of its most frequent potential adverse effects. There is discussion regarding the large number of case reports in relation to the retention of gastric contents at the time of endoscopy. There is currently insufficient evidence to state categorically that they produce a significant change in gastric emptying. Nevertheless, it is recommended to inquire about the use of these drugs before endoscopic procedures that require sedation and, in the presence of symptoms, to suggest changes in the preparation


La obesidad es una patología muy frecuente a nivel global. Dentro de las alternativas del manejo están los análogos del péptido 1 similar al glucagón (GLP-1), hormona secretada principalmente por el intestino. Aparte de sus efec- tos como incretina, se han descrito efectos sobre la motilidad gastrointestinal, los que parecen ser fundamentales para su efecto sobre la obesidad, pero también los causales de sus potenciales efectos adversos más frecuentes. Existe discusión en relación con la gran cantidad de reportes de casos en relación con la retención de contenido gástrico al momento de una endoscopia. Actualmente no existe evidencia suficiente para afirmar categóricamente que producen un cambio significativo en el vaciamiento gástrico. No obstante, se recomienda indagar sobre el uso de estos fármacos antes de procedimientos endoscópicos que requieran sedación y, ante la presencia de síntomas, sugerir cambios en la preparación.

Constipation is a disorder of the gastrointestinal (GI) and some of the main etiological mechanisms are directly related to changes in GI physiology. The capacity to carry out paired assessments and measure GI parameters under the influence of constipation is a relevant point in selecting a suitable methodology. We aimed to perform a non-invasive investigation of gastrointestinal motility in constipated rats using the alternating current biosusceptometry system (ACB). The animals were split into two groups: the pre-induction stage (CONTROL) and post-induction loperamide stage (LOP). We assessed GI motility parameters using the ACB system. Colon morphometric and immunohistochemical analyses were performed for biomarkers (C-kit) for interstitial cells of Cajal (ICC). Our results showed a significant increase in gastrointestinal transit in the LOP group in addition to a reduction in the dominant frequency of gastric contraction and an arrhythmic profile. A change in colonic contractility profiles was observed, indicating colonic dysmotility in the LOP group. We found a reduction in the number of biomarkers for intestinal cells of Cajal (ICC) in the LOP group. The ACB system can evaluate transit irregularities and their degrees of severity, while also supporting research into novel, safer, and more efficient treatments for constipation.

Capsulorhexis is an integral step of cataract surgery, and continuous curvilinear capsulorhexis is crucial during phacoemulsification to prevent intraoperative complications. However, sometimes during phacoemulsification in complicated and hard cataract cases, rhexis extension may occur, resulting in posterior capsular rent, nucleus drop, cortex drop, and aphakia. It may not always be possible to continue with phacoemulsification in all cases. In this perspective, the authors describe a novel flap motility sign (FMS) to predict the extent of anterior capsular tear during phacoemulsification. A total of 21,678 patients underwent phacoemulsification for three years, from July 2016 to June 2019. One hundred and twenty-one patients had an anterior capsular tear. There were 102 cases (84.3%) with pre-equatorial tears and 19 cases (15.70%) with postequatorial tears. All pre-equatorial flaps were everted and fluttering, and all postequatorial flaps were inverted and nonfluttering. Posterior capsule rupture (PCR) was observed in all 19 cases of postequatorial flaps (100%). No PCR was observed in patients with fluttering and everted flaps (0%). In-the-bag and scleral-fixated intraocular lens implantations succeeded in pre-equatorial and postequatorial tears, respectively. There was no case of a nucleus drop. This study validates FMS as a predictor for identifying the extent of anterior capsular tears, thereby determining the endpoint of safe phacoemulsification and the site for intraocular lens implantation. Pre-equatorial tears allow for the continuation of safe phacoemulsification and in-the-bag intraocular implantation. Postequatorial tears necessitate timely conversion to small-incision cataract surgery or extracapsular cataract extraction.

SUMMARY: This study aimed at clarifying the impact of long-term prenatal and postnatal exposure to exogenous progesterone on sperm production and function, relative sex organs weights, and the levels of the relevant hormones in rats. Sixty male Wistar rats were included and classified into three groups (n=20 in each). A test I group had mature rats born to dams treated with progesterone prenatally. A test II group included rats exposed to progesterone during prenatal as well as postnatal periods, and a control group had rats treated with a placebo (olive oil). The test groups revealed a significant reduction in sperm count, motility, and viability with higher abnormal forms than the control group (P< 0.05). Similarly, the test groups revealed significantly lower serum testosterone and higher FSH and LH levels (P< 0.001). Interestingly, the test II group showed pronounced sperm abnormalities, an alarming decrease in sperm viability and motility, and a significant accretion in the relative testicular weight compared to the test I group (p <0.001). Long-term (prenatal and early postnatal) treatment with synthetic progesterone hurts sperm quantity and quality, adversely affecting future male fertility.


Este estudio tuvo como objetivo aclarar el impacto de la exposición prenatal y posnatal a largo plazo a la progesterona exógena en la producción y función de los espermatozoides, el peso relativo de los órganos sexuales y los niveles de las hormonas relevantes en ratas. Sesenta ratas macho Wistar fueron incluidas y clasificadas en tres grupos (n=20 en cada uno). Un grupo de prueba I tenía ratas maduras nacidas de madres tratadas con progesterona prenatalmente. Un grupo de prueba II incluyó ratas expuestas a progesterona durante los períodos prenatal y posnatal, y un grupo de control tenía ratas tratadas con un placebo (aceite de oliva). Los grupos de prueba revelaron una reducción significativa en el recuento, la motilidad y la viabilidad de los espermatozoides con formas anormales más altas que el grupo de control (P < 0,05). De manera similar, los grupos de prueba revelaron niveles significativamente más bajos de testosterona sérica y niveles más altos de FSH y LH (P < 0.001).
