Se define «síndrome febril prolongado¼ a todo cuadro de hipertermia que persiste al menos 10 días sin ser diagnosticado. El síndrome de Good es una inmunodeficiencia primaria del adulto que se caracteriza por presentar hipogammaglobulinemia, disminución de linfocitos B y anormalidades en los linfocitos T. Entre el 15 y el 20% de los casos de «fiebre de origen desconocido¼ ocurren debido a neoplasias, y el cáncer de colon representa menos del 1% de todos estos casos. Se presenta una paciente de 49 años admitida en el hospital por presentar síndrome febril con un mes de evolución, con antecedentes de síndrome de Good. Se le diagnostica cáncer de colon. (AU)


A prolonged febrile syndrome (PFS) is defined as any episode of hyperthermia that persists for at least 10 days without being diagnosed. Good's syndrome is a primary immunodeficiency in adults characterized by hypogammaglobulinemia, decreased B lymphocytes, and abnormalities in T lymphocytes. Between 15 to 20% of fever of unknown origin (FOD) cases are due to neoplasms, and colon cancer represents less than 1% of all these cases. A 49-year-old patient with a history of Good's syndrome was admitted to the hospital due to a febrile syndrome lasting for a month. She was diagnosed with colon cancer. (AU)

Introducción: Las infecciones representan la etiología más frecuente del síndrome febril prolongado (SFP). Si bien las fiebres entéricas constituyen una causa posible, en Uruguay su prevalencia ha disminuido significativamente con la mejora de las condiciones socio sanitarias. Objetivo: Comunicar el caso de un adolescente con una etiología actualmente excepcional de SFP. Caso clínico 14 años, sano, zona suburbana. Comienza 2 semanas previo al ingreso con dolor en hemiabdomen superior. Agrega cefalea holocraneana leve y vómitos ocasionales. 5 días previos al ingreso fiebre 40°C axilar, un pico diario, sin otra sintomatología. Tránsito digestivo bajo y urinario normal. Examen físico: lúcido, buen aspecto general, abdomen doloroso a la palpación profunda en epigastrio. Sin irritación peritoneal. Resto normal. Analítica: Leucocitos 5200mm3, Proteína C reactiva 71.4mg/dL, hemocultivo sin desarrollo. Ecografía abdominal, radiografía de tórax y ecocardiograma normales. Serologías para Virus Epstein Barr, Citomegalovirus, y Bartonella henselae negativas. Orina normal, urocultivo sin desarrollo. Persiste con fiebre, agrega exantema macropapular evanescente en tronco, sin otros síntomas. Al 7° día de internación nuevo hemocultivo: Salmonella Typhi sensible a ampicilina que recibe por 14 días. Buena evolución. Discusión: La fiebre tifoidea es una enfermedad infectocontagiosa, aguda, potencialmente mortal. Las condiciones socioeconómicas son determinantes en su transmisión. La sensibilidad del hemocultivo es mayor durante la primera semana de enfermedad, por lo que en ocasiones es necesario reiterarlo. Sus manifestaciones clínicas inespecíficas y la baja incidencia hacen que esta etiología no sea habitualmente sospechada en nuestro medio. Por tanto, es importante aumentar el índice de sospecha y considerar entre los diagnósticos diferenciales de SFP esta etiología.


Introduction: Infections are the most frequent etiology of prolonged febrile illness (PFI). Although enteric fevers are a possible cause, their prevalence has significantly diminished in Uruguay, due to improved socio-sanitary conditions. Objective: To communicate the case of an adolescent with a currently exceptional etiology of PFI. Clinical case: 14 years old, healthy, suburban area. Two days prior to admission the patient has pain in upper hemi abdomen. Adds mild holocranial headache and occasional vomiting. 5 days prior to admission axilary temperature of 40°C, one daily peak, without other symptoms. Normal lower digestive and urinary transit. Physical examination: lucid, good general aspect, pain at deep palpation in epigastrium. No peritoneal irritation. Rest is normal. Laboratory: leukocytes 5200 mm3, C-reactive protein 71.4mg/dL, blood culture shows no growth. Abdominal sonogram, thoracic X-ray and echocardiogram are normal. Negative serology for Epstein Barr Virus, Cytomegalovirus and Bartonella henselae. Normal urine, urine culture with no growth. Fever persists, adds evanescent macropapular exanthema in on the trunk, without other symptoms. On the 7th day in hospital a new blood culture shows Salmonella Typhi sensitive to ampicillin, which he receives for 14 days. Good evolution. Discussion: Typhoid fever is an acute, life-threatening, infectious disease. Socioeconomic conditions are determinant in its transmission. Blood culture sensitivity is greater during the first week of the disease, that is why it must occasionally be repeated. Its unspecific clinical manifestations and low incidence make this etiology not be usually suspected in our surroundings. It is therefore important to increase our suspicion and to consider it amongst differential diagnosis in PFI.

Embora as febres entéricas sejam uma causa possível, no Uruguai sua prevalência diminuiu significativamente com a melhoria das condições sociossanitárias. Objetivo: Relatar o caso de um adolescente com etiologia atualmente excepcional de SFP. Caso clínico 14 anos, saudável, zona suburbana. Começa 2 semanas antes da admissão com dor no abdome superior. Adiciona dor de cabeça holocraniana leve e vômitos ocasionais. 5 dias antes da admissão febre 40°C axilar, pico diário, sem outros sintomas. Trânsito digestivo inferior e trânsito urinário normais. Exame físico: lúcido, bom aspecto geral, abdome doloroso à palpação profunda no epigástrio. Sem irritação peritoneal. Resto normal. Análise: Leucócitos 5200mm3, proteína C reativa 71,4mg/dL, hemocultura sem desenvolvimento. Ultrassonografia abdominal, radiografia de tórax e ecocardiograma foram normais. As sorologias para vírus Epstein Barr, Citomegalovírus e Bartonella henselae foram negativas. Urina normal, urocultura sem desenvolvimento. Persiste com febre, acrescenta erupção macropapular evanescente no tronco, sem outros sintomas. No 7º dia de internação, nova hemocultura: Salmonella Typhi sensível à ampicilina, que recebeu por 14 dias. Boa evolução. Discussão: A febre tifóide é uma doença infecciosa aguda, potencialmente fatal. As condições socioeconômicas são decisivas na sua transmissão. A sensibilidade da hemocultura é maior durante a primeira semana da doença, por isso às vezes é necessário repeti-la. Suas manifestações clínicas inespecíficas e baixa incidência fazem com que essa etiologia não seja usualmente suspeitada em nosso meio. Portanto, é importante aumentar o índice de suspeição e considerar essa etiologia entre os diagnósticos diferenciais da SFP.

Introducción: la fiebre es un motivo muy frecuente de consulta y hasta en un 20% de los pacientes no se encuentra la causa. En el ámbito de la emergentología pediátrica clásicamente ha existido interés en homogeneizar la forma de evaluar los lactantes febriles menores de tres meses. Contar con un protocolo que permita detectar precozmente el niño que cursa una infección bacteriana invasiva (IBI) sin realizar conductas desproporcionadas es todo un desafío. Objetivo: evaluar y comparar la capacidad para identificar IBI en la pauta actual de fiebre sin foco (FSF) como en la estrategia step by step, en lactantes con FSF valorados en el DEP-CHPR. Material y métodos: estudio observacional, descriptivo, retrospectivo y de pruebas diagnósticas. Criterios de inclusión: lactantes menores de 90 días de vida que consultaron en 2017 y 2018 en DEP-CHPR con diagnóstico de FSF. Resultados: se incluyeron 261 lactantes evaluados con la pauta de FSF actual, en ellos se aplicó la estrategia step by step. El rango de edad fue de 84 días (4-88 días) con una media de 41 días. Sexo masculino 148 niños (56,7%). Se registraron 37 infecciones bacterianas (14,2%) de las cuales 3 fueron IBI (1,1%) y 34 fueron no-IBI (13,1%). La sensibilidad para step by step fue de 0,94% y de 0,89 para la pauta actual, con un VPN de 0,98 para ambas estrategias. Discusión: los lactantes menores de 3 meses son más susceptibles por características fisiológicas a infecciones bacterianas invasivas y cuanto más pequeño aumenta aún más la frecuencia. El step by step discrimina a menores de 1 mes en menores de 21 días y otro grupo de más de 21 días. Nuestra pauta no hace esta discriminación y realiza por igual laboratorio en sangre, orina y líquido cefalorraquídeo; realizando en ocasiones estudios cruentos no necesarios. Conclusiones: ambas estrategias aplicadas en esta población resultaron altamente sensibles para identificar infección bacteriana con un VPN elevado. La aplicación de step by step presenta como beneficio adicional evitar con seguridad la punción lumbar en recién nacidos entre los 21 y 28 días.


Introduction: fever is a very frequent reason for consultation and in up to 20% of patients the cause has not been found. In the field of pediatric emergentology, there has been a traditional interest in homogenizing the way of assessing febrile infants under three months of age. Having a protocol that enables early detection of children with IBIs without engaging in disproportionate procedures is a challenge. Objective: to evaluate and compare the ability to identify IBIs in the present FSF regimen as in the Step-by-Step strategy, in infants with FSF assessed at the Pereira Rossell Pediatric Hospital Center. Material and methods: observational, descriptive, retrospective study and diagnostic tests. Inclusion criteria: Infants under 90 days of age who consulted in 2017 and 2018 at the DEP-CHPR with a diagnosis of FSF. Results: 261 infants diagnosed with FSF regimen were included and they all received a Step-by-Step approach. The age range was 84 days (4 - 88) days with a mean of 41 days. Males 148 children (56.7%). There were 37 bacterial infections (14.2%), of which 3 were IBI (1.1%) and 34 were Non-IBI (13.1%). The sensitivity for the Step-by-Step approach was 0.94% and 0.89 for the current regimen, with a NPV of 0.98 for both strategies. Discussion: infants younger than 3 months-old are more susceptible due to physiological characteristics to invasive bacterial infections, and the younger they are, the higher the frequency. The Step-by-Step Approach splits children of under 1 month of age into those under or over 21 days of age. Our guideline does not make this discrimination and performs the same blood, urine and cerebrospinal fluid laboratory tests sometimes carrying out blood tests is not necessary. Conclusions: both approaches used in this population were highly sensitive to the identification of bacterial infections with a high NPV. The application of the "Step-by-Step" approach has the additional benefit of avoiding lumbar puncture to newborns of between 21 and 28 days of age.


Introdução: a febre é um motivo muito comum de consulta e em até 20% dos pacientes a causa não é encontrada. No campo da emergência pediátrica, tradicionalmente tem havido interesse em homogeneizar a forma de avaliação de lactentes febris menores de três meses de idade. Ter um protocolo que permita a detecção precoce de uma criança com IBI sem realizar procedimentos desproporcionais é um desafio. Objetivo: avaliar e comparar a capacidade de identificação de IBI na atual Diretriz da FSF e na estratégia Passo a Passo, em lactentes com FSF avaliados no DEP-CHPR. Material e métodos: estudo observacional, descritivo, retrospectivo e de testes diagnósticos. Critérios de inclusão: Lactentes com menos de 90 dias de idade que consultaram em 2017 e 2018 no Hospital Pediátrico Pereira Rossell do Uruguai com diagnóstico de FSF. Resultados: Foram incluídos 261 lactentes avaliados com a atual diretriz da FSF, nos quais foi aplicada a estratégia Passo a Passo. A faixa etária foi de 84 dias (4 - 88) dias com média de 41 dias. Sexo masculino 148 crianças (56,7%). Foram registradas 37 infecções bacterianas (14,2%), sendo 3 IBI (1,1%) e 34 Não IBI (13,1%). A sensibilidade para Passo a Passo foi de 0,94% e 0,89 para o esquema atual, com VPN de 0,98 para ambas estratégias. Discussão: crianças menores de 3 meses de idade são mais suscetíveis a infecções bacterianas invasivas devido às características fisiológicas e quanto menores, mais frequentes. O Passo a Passo separa crianças menores de 1 mês em dois grupos: menores de 21 dias e acima de 21 dias. Nossa diretriz não faz essa discriminação e realiza exames laboratoriais de sangue, urina e líquido cefalorraquidiano da mesma forma; às vezes realizando estudos de sangue que não são necessários. Conclusões: ambas as estratégias aplicadas nesta população foram altamente sensíveis para identificar infecção bacteriana com alto VPN. A aplicação do "Passo a Passo" apresenta como benefício adicional evitar a punção lombar em recém-nascidos entre 21 e 28 dias.

El carcinoma de cérvix constituye una causa rara de fiebre de origen desconocido (FOD). Se presenta el caso de una paciente femenina de 42 años de edad con fiebre de 80 días de evolución. Había estado ingresada en dos hospitales sin que se llegase a diagnosticar su causa. Se le habían administrado múltiples regímenes antibióticos sin lograrse remisión de la hipertermia. Fue remitida al hospital Hermanos Ameijeiras para continuar estudios en el protocolo de atención a pacientes con ese tipo de afección. Tenía anemia ferropénica y refería hipermenorrea. Al examen físico se encontró engrosamiento del cuello uterino al tacto vaginal y se visualizó una tumoración del mismo al examen con espéculo. La biopsia mostró un carcinoma y recibió tratamiento con radioterapia y quimioterapia con lo cual desapareció la fiebre. Se concluye que esta neoplasia puede presentarse a forma de fiebre prolongada producida directamente por el tumor primario, por lo que debe mantenerse un índice de sospecha en pacientes con FOD. Resulta importante tener en cuenta la asociación de fiebre y sangramiento vaginal como posible forma de presentación. Puede constituir una pista diagnóstica relevante para no perder tiempo. Este caso resalta la importancia de realizar un examen físico completo y exhaustivo a todos los pacientes(AU)


Carcinoma of the cervix is a rare cause of fever of unknown origin. We present the case of a 42-year-old female patient with fever of 80 days of evolution. She had been admitted to two hospitals without a diagnosis of the cause. She had been administered multiple antibiotic regimens without achieving remission of hyperthermia. She was referred to the "Hermanos Ameijeirasˮ Hospital to continue studies under the protocol for the care of patients with this type of condition. She had iron deficiency anemia and reported hypermenorrhea. Physical examination showed thickening of the cervix on vaginal examination and a tumor was visualized on speculum examination. The biopsy showed a carcinoma and she was treated with radiotherapy and chemotherapy, with which the fever disappeared. It is concluded that this neoplasm can present as a prolonged fever directly produced by the primary tumor, so an index of suspicion should be maintained in patients with fever of unknown origin. It is important to take into account the association of fever and vaginal bleeding as a possible form of presentation. It may constitute a relevant diagnostic clue to avoid wasting time. This case highlights the importance of performing a complete and thorough physical examination in all patients(AU)

INTRODUCTION Actually, there are more than 200 different causes of unknown fever, it is necessary for the doctor to identify the most prevalent causes of unknown fever in our Clinical boar. PRESENTATION A 52-year-old male patient with no history of chronic diseases, which was received in the Emergency Service of the Hospital San Pablo, Coquimbo, on 02/03/20 began a clinical picture characterized by fever up to 39 ºC associated with diaphoresis, evaluated several times in primary health care without response to symptomatic treatment. He denies dyspnea, cough, sputum production, headache, muscle weakness, myalgia, fainting, decreased visual acuity, nausea, vomiting, chest pain, abdominal pain, diarrhea, constipation, altered bowel movements, weight loss, dysuria, bladder tenesmus, pain lumbar, skin lesions. As relevant information, the patient reports having taken amoxicillin/clavulanate for 2 days on his own account. DISCUSSION Physicians should be aware of the rare extent of an unusual presentation of knowledge origin fever, probably associated with Still disease, as well as medical options for treatment. The literature does not conclude on a gold standard for the method of approach. CONCLUSION In our case, the etiological agent that could cause Fever of unknown origin (FUO) was Sars cov-2 given the presence of elevated inflammatory factors and acute phase proteins and the presence of neutrophilic infiltration

Objective: To explore the etiologies and clinical characteristics of fever of unknown origin (FUO) and to provide clues for early diagnosis of FUO. Methods: The data about etiology, age, sex, clinical course, length of hospital stays and the expression levels of inflammatory factors in fever phase of 357 pediatric inpatients who were diagnosed with FUO in Children's Hospital of Fudan University from 1 January 2016 to 31 December 2020 were collected and retrospectively analyzed. Participants were grouped into infectious disease, inflammatory disease, malignancy and others and according to the classification of diseases and also grouped into those aged<1 year, 1-<3 years,3-<6 years, 6-<12 years and 12-<18 years. Comparisons between groups were performed using the Mann-Whitney U test, Kruskal-Wallis H test and χ² test. Results: Among the 357 patients (217 males and 140 females). The age of onset was 3.9 (1.3, 9.2) years and visiting age was 5.1 (2.0, 9.3) years. The time-consuming of diagnosis was 94 (66, 213) days. The hospital stay was 8 (6, 14) days. The most frequently identified cause of FUO was infectious diseases (163 cases, 45.7%), followed by non-infectious inflammatory diseases (133 cases, 37.2%), malignancy (21 cases, 5.9%) and others (40 cases, 11.2%). The patients at younger age were more likely to be attacked by malignancy, oncologic diagnoses, and others, nevertheless patients at older age were more likely to be attacked by non-infectious inflammatory diseases oppositely (9.8 (3.6, 11.5) vs. 3.0 (1.2, 7.0), 2.3 (1.0, 5.2), 0.9 (0.5, 1.8) years, U=41.30, 15.94, 37.08, all P<0.01);106 (65%) patients were male, and 57 (35%) patients were female. This result indicated that boys were more susceptible to infectious diseases (χ²=14.73, P<0.01). Analysis of inflammatory factors in serum among 103 patients, interleukin (IL)-6 level in 40 infectious diseases patients (9 (2, 38) ng/L) was significantly lower than those of 6 tumor patients (89 (64, 599) ng/L) and 57 non-infectious inflammatory diseases patients (25 (8, 78) ng/L, U=51.05, 15.70, both P<0.05), no significant difference was observed in IL-2, IL-4, IL-10, tumor necrosis factor α and interferon among the groups (all P>0.05). The patients grouped into those aged 1-<3 years and 3-<6 years were more likely to be attacked by infectious diseases (51.3% (59/115) and 57.1% (40/70)), while patients grouped into those aged 6-<12 years and 12-<18 years were more likely to be attacked by non-infectious inflammatory diseases (55.6% (65/117) and 72.4% (21/29)). Conclusions: Infectious disease is still the main cause of FUO in children and the boys are more susceptible to infectious diseases. However, the morbidity of non-infectious inflammatory diseases increases to number 1 in FUO of children over 6 years of age.

摘要

Objectif : déterminer les étiologies des fièvres prolongées inexpliquées au service de médecine interne du CHU de Bouaké. Matériel et méthodes : Il s'agissait d'une étude rétrospective à visée descriptive mené de janvier 2019 à décembre 2020. Nous avons inclus les patients hospitalisés pour fièvre prolongée inexpliquée ou ayant présentés une fièvre prolongée durant l'hospitalisation. Les données anamnestiques, cliniques et paracliniques ont été analysées. Résultats : Nous avons retenus 204 patients sur 1650 patients hospitalisés (12,6%). L'âge moyen des patients était de 32±16,1 ans avec des extrêmes de 16 et 87 ans. Le sexe ratio était de 1,1. La fièvre (32,4%) et AEG (22%) constituaient les motifs d'hospitalisation les plus fréquents. La fièvre était le plus fréquent des symptômes physiques (94,1%) suivi de l'amaigrissement (50%), de la déshydratation (18,1%), de l'hépatomégalie (12,7%) et de l'obnubilation (12%). Les principales étiologies étaient les infections (82,3%), les tumeurs (10,3%), et les maladies inflammatoires (1,5%). Dans 5,9% des cas l'étiologie n'avait pas été retrouvée. Les principales infections étaient le VIH et ses complications (38,1%), la tuberculose (21,4%), et les infections urogénitales (19,9%). Les tumeurs étaient dominées par le cancer primitif du foie (33,4%) et le cancer de la prostate (19,4%). La polyarthrite rhumatoïde, le lupus érythémateux disséminé et la sclérodermie étaient maladies inflammatoires retrouvées. La létalité était de 36%. Conclusion : Les étiologies des fièvres prolongées inexpliquées sont variées. Elles sont dominées dans notre contexte par la tuberculose, l'infection à VIH et ses complications infectieuses.

摘要

La fiebre de origen desconocido (FOD) constituye una forma de presentación atípica o infrecuente de una patología común, que se manifiesta como una alteración rara. No se ha consensuado el diagnóstico de FOD de manera sistemática, sino que se establece a medida que evoluciona el cuadro sin un hallazgo etiológico cierto. El tratamiento es muy discutido y no existe consenso al respecto. Caso clínico: se presenta el caso de un paciente de sexo masculino de 45 años, que consulta a la guardia médica del Hospital Naval Puerto Belgrano (HNPB) por fiebre persistente con 6 días de evolución, acompañada de cefalea frontal y dolor abdominal generalizado. Refiere haber tomado levofloxacina (500mg/día). Al exámen físico presenta lucidez, signos vitales conservados, hemodinámicamente estable sin particularidades en la semiología cardiovascular y respiratoria. Estudios de laboratorio, ecográficos y colangio pancreatografía por resonancia magnética descartaron un proceso obstructivo y/o infeccioso (bacteriológico/viral) y tumoral. Por tomografía axial computada de cerebro, tórax, abdomen y pelvis se informó una lesión parietal en ángulo esplénico del colon y se indicó una colonoscopía sin hallazgo de patología neoplásica. El cuadro febril persistente se trató con antibióticos de forma empírica y con corticoides (meprednisona). Conclusiones: La evolución del paciente fue favorable. Fue definido como enfermedad de Still por exclusión, considerando los resultados negativos de los diferentes estudios impartidos. Se otorgó alta hospitalaria sin secuelas. (AU)


Fever of unknown origin (FUO) is an atypical or infrequent presentation of a common pathology, which manifests itself as a rare disorder. Currently, it has not been possible to formulate the diagnosis of FUO in a systematic way, with the difficulties that it entails in order to address a behavior in this regard. The diagnosis is made as the picture evolves without a true etiological finding. Treatment is much discussed with no consensus about it. Clinical Case: the case of a 45-year-old male patient is presented, who consults the doctor on-call at the Puerto Belgrano Naval Hospital for persistent fever for 6 days, accompanied by frontal headache and generalized abdominal pain. He refers having taken levofloxacin (500mg/ day). At the physical examination he shows alertness, preserved vital signs, hemodynamic stability without particularities in cardiovascular and respiratory semiology. Laboratory, ultrasound and magnetic resonance cholangio-pancreatography studies ruled out an obstructive and / or infectious (bacteriological/viral) disease and tumor process. Computed axial tomography of the brain, thorax, abdomen and pelvis reported a parietal lesion in the splenic angle of the colon, and a colonoscopy was reported without findings of neoplastic pathology. The persistent febrile condition was treated with antibiotics empirically and with corticosteroids (meprednisone). Conclusions: The evolution of the patient was favorable. A diagnosis of Still's disease was reached by exclusion, considering the negative results of the different studies carried out. Hospital discharge was granted without sequelae. (AU)

Resumen: Introducción: Un 20% de los niños con síndrome febril se presenta como síndrome febril sin foco (SFSF). Las es trategias de manejo en este grupo presentan alta sensibilidad, pero baja especificidad. Objetivos: Ca racterizar las infecciones bacterianas serias (IBS) en menores de 3 meses hospitalizados por SFSF, y evaluar utilidad de parámetros clínicos y de laboratorio en la identificación de pacientes con alto riesgo de IBS. Pacientes y Método: Estudio prospectivo en pacientes < 3 meses hospitalizados entre enero 2014 y noviembre 2015 por SFSF en dos hospitales pediátricos de la Región Metropolitana. Criterios de inclusión: edad 4 días - 3 meses, fiebre > 38°C de < 72 h de evolución sin causa demostra ble. Criterios de exclusión: uso de antimicrobianos hasta 7 días previo a su ingreso, prematuros < 34 semanas, peso de nacimiento < 2 kg e inmunocomprometidos. Se registraron datos demográficos, clínicos, y exámenes de laboratorio, hemograma y PCR, diagnóstico de egreso, IBS descartada, IBS probable o confirmada. Resultados: 32% de los pacientes egresó con diagnóstico de IBS, 28% con diagnóstico de infección viral o probablemente viral, 34% con diagnóstico de SFSF no especificado y 6% SFSF por otras causas. No se encontraron diferencias significativas en PCR, leucocitosis, aspecto tóxico ni horas de fiebre al ingreso al comparar los grupos con y sin IBS (p > 0,05). La combinación de parámetros clínicos y de laboratorio mostro sensibilidad de 27%, especificidad de 90%, VPP 60% y VPN 71%. Conclusión: No fue posible establecer que parámetros clínicos y de laboratorio permitan identificar menores de 3 meses con alto riesgo de IBS, manteniendo su utilidad como indicadores de bajo riesgo. Es necesario contar con otros elementos clínicos y de laboratorio que permitan discrimi nar IBS de infecciones virales.


Abstract: Introduction: In 20% of children with febrile syndrome, it appears as fever of unknown origin (FUO) syndrome. Management strategies in this group have high sensitivity but low specificity. Objectives: To cha racterize serious bacterial infections (SBI) in children younger than three months old hospitalized because of FUO syndrome and to evaluate the utility of clinical and laboratory parameters in the identification of patients that are at high risk of SBI. Patients and Method: Prospective study in patients aged < 3 months hospitalized due to FUO syndrome between January 2014 and November 2015 in two pediatric hospitals in the Metropolitan Region. Inclusion criteria: age 4 days - 3 months, fever > 38°C longer than 72 hours after onset without demonstrable cause. Exclusion criteria: anti microbial use up to 7 days before admission, preterm infants < 34 weeks, birth weight < 2 kg, and im munocompromised. Demographic, clinical, and laboratory tests data were recorded as well as blood count and CRP, discharge diagnosis, and ruled out, probable or confirmed SBI. Results: 32% of the patients were discharged with diagnosis of SBI, 28% with diagnosis of viral or probably viral infec tion, 34% with diagnosis of not specified FUO syndrome, and 6% due to other causes. There were no significant differences in the CRP value, altered WBCs count, toxic aspect, or hours of fever at the admission when comparing groups with and without SBI (p < 0.05). The combination of clinical and laboratory parameters showed 27% of sensitivity, 90% of specificity, 60% of PPV, and 71% of NPV. Conclusion: It was not possible to establish clinical and laboratory parameters that allow the identifi cation of children younger than 3 months old at high risk of SBI, however, they maintain their value as low risk indicators. It is necessary further investigation of other clinical and laboratory elements that allow discriminating SBI from viral infections.

RESUMEN Introducción: Los progresos en el concepto de la Salud Pública han introducido nuevos enfoques como el concepto de vigilancia sindrómica, dentro de las que se encuentra el síndrome febril inespecífico. Objetivo: Caracterizar los pacientes con diagnóstico de dengue en pacientes con síndrome febril inespecífico. Método: Se realizó un estudio observacional descriptivo, de corte transversal, en el área de salud del Policlínico Docente "Ángel Machaco Ameijeiras" de Guanabacoa, La Habana, entre enero y diciembre de 2019; la población de estudio fue de 580 pacientes. Se empleó un modelo de recolección de datos y las variables a medir fueron: edad, sexo, resultado de inmunoglobulina M, mes, zona de procedencia de la paciente urbana o semiurbana y síntomas como cefalea, artralgia, decaimiento, mareos, náuseas, rash, fiebre, dolor retro ocular, mialgias y dolor abdominal. Resultados: Predominó el grupo de edad 1-19 años con 281 pacientes para un 48,44 %, el aumento de los casos con inicio en julio y culminó en diciembre, predominio de los casos negativos sobre los positivos, representado el sexo femenino, las zonas urbanas presentó la mayor cantidad de casos con un 72,58 % y valor p=0,00. Predominó la fiebre como síntoma fundamental, presente en el 87 % de los pacientes. Conclusiones: El grupo de edad más representativo fue de 1-19 años, el aumento de los casos inició en el mes de julio y 229 presentaron dengue, predominando la zona urbana, con la fiebre como síntoma cardinal.


ABSTRACT Introduction: Progress in public health concept has introduced new approaches such as the concept of syndromic surveillance, which includes non-specific febrile syndrome. Objective: To describe patients diagnosed with dengue virus in patients with non-specific febrile síndrome. Method: A cross-sectional, descriptive, observational study was conducted in the health area of the "Angel Machaco Ameijeiras" Teaching Polyclinic in Guanabacoa, Havana, from January to December 2019; the population studied was 580 patients. A data collection model was used and the variables to be measured were: age, sex, immunoglobulin M result, month, urban or semi-urban patient's area of origin and symptoms such as headache, arthralgia, malaise, dizziness, nausea, rash, fever, retro ocular pain, myalgias and abdominal pain. Results: The age group between 1 and 19 years predominated, 281 patients for a 48.44 %, the increase of cases starting in July and ending in December, negative cases rather than positive one are predominant, representing the female sex, urban areas presented the highest number of cases with 72.58 % and p value=0.00. Fever was the predominant symptom, present in 87% of the patients. Conclusions: The most representative age group with fever was between 1 and 19 years, the increase in cases began in July and 229 patients presented dengue virus, predominantly in urban areas, with fever as a cardinal symptom.

Introducción: La fiebre de origen desconocido es un síndrome que genera un gran desafío para el pensamiento clínico; el patrón recurrente es el subtipo más retador y de difícil diagnóstico etiológico. Objetivo: Caracterizar los pacientes con fiebre de origen desconocido con patrón recurrente. Método: Se realizó un estudio descriptivo de corte transversal en el periodo de enero de 2008 a diciembre de 2015. La muestra quedó constituida por 48 pacientes, los cuales fueron hospitalizados y se les realizó una evaluación clínica en el Hospital Clínico Quirúrgico Hermanos Amejeiras. Para el análisis de los datos se utilizaron medidas de resumen para las variables cualitativas y para las cuantitativas, media y desviación estándar. Resultados: La edad media fue 41,3 años, predominaron los hombres (62,5 por ciento), la duración media de la fiebre fue de 497 días y las pistas clínicas más importantes fueron los hallazgos de adenopatías (43,8 por ciento) y hepatomegalia (22,9 por ciento). Los casos sin diagnóstico fueron 45,8 por ciento. Conclusiones: La fiebre de origen desconocido con patrón recurrente es de difícil diagnóstico. A pesar de una evaluación clínica minuciosa, la mayoría de los pacientes quedan sin diagnóstico(AU)


Introduction: Fever of unknown origin is a syndrome that generates great challenge for clinical thought; the recurrent pattern is the most challenging subtype and with difficult etiological diagnosis. Objective: To describe patients with fever of unknown origin with recurring pattern. Method: A descriptive cross-sectional study was carried out from January 2008 to December 2015. Forty-eight patients formed the sample. They were hospitalized and underwent clinical evaluation at Hermanos Amejeiras Surgical Clinical Hospital. For data analysis, summary measures were used for qualitative variables and mean and standard deviation variables were used for quantitative. Results: The mean age was 41.3 years, men predominated (62.5 percent), the mean duration of fever was 497 days, and the most important clinical signs were the findings of lymphadenopathy (43.8 percent) and hepatomegaly. (22.9 percent). Undiagnosed cases were 45.8 percent. Conclusions: Fever of unknown origin with recurring pattern is difficult to diagnose. Despite careful clinical evaluation, most patients remain undiagnosed(AU)

It is defined as temperature ≥ 37.8 ° C on several occasions, lasting ≥ three weeks, in the absence of diagnosis after three days of hospital investigation or 3 outpatient visits. The main etiologies are infectious, neoplastic, and non-infectious inflammatory diseases. The diagnosis is based on the detailed clinical history and physical examination of these patients, in order to direct the specific complementary tests to be performed in each case. The initial diagnostic approach of the FUO patient should include non-specific complementary exams. Empirical therapy is not recommended (with few exceptions) in patients with prolonged fever, as it may disguise and delay the diagnosis and conduct to treat the specific etiology. The prognosis encompasses mortality of 12-35%, varying according to the baseline etiology.

We report the first case demonstrating an association between Kawasaki disease (KD) and erythema nodosum (EN). A 3-year-old girl presented with EN as an initial manifestation of KD. At the initial visit, she showed high fever of 40℃, injection of the oropharynx, cervical lymphadenopathy, and red-purple cutaneous nodules, particularly on the lower limbs. She complained of severe pain in the neck and cutaneous lesions. Initially, the development of EN was attributed to Salmonella spp infection, which was detected in stool culture. However, the patient did not respond to high-dose ampicillin/sulbactam to which the Salmonella spp is sensitive. Echocardiography performed as screening for fever of unknown origin revealed medium-sized aneurysms of the left anterior descending artery. EN masked the diagnosis of KD, and the patient developed a coronary artery lesion. KD should be considered in the differential diagnosis of refractory EN in pediatric patients.

OBJECTIVE@#To investigate the clinical significance of bone marrow unclassifiable cells in diagnosis of fever of unknown origin(FUO).@*METHODS@#The clinical data of 60 patients with FUO admitted in the first affiliated hospital of Xi'an Jiaotong university from June 2014 to May 2016 were collected, and 60 patients with FUO were divided into 2 group: group A(30 cases) in which the unclassifiable cells in bone marrow were observed by bone marrow examination, and group B(30 cases) in which the unclassifiable cells in bone marrow not were found by bone marrow examination. The clinical characteristics, bone marrow features, immunophenotypes of bone marrow cells and prognosis of patients in 2 groups were analyzed retrospectively.@*RESULTS@#Out of 30 patients in group A, 18 were diagnosed as malignant tumors including 12 cases of lymphoma, while out of 30 patients in group B, 5 cases were diagnosed as malignant tumor, including 3 cases of lymphoma. For the patients with non-tumor diseases, the bone marrow unclassifiable cells disappeared after the patients condition was improved.@*CONCLUSION@#The bone marrow examination including the smear and biopsy shonld be performed routinely for the patients with FUO. If the unclassifiable cells are observed morphologically in bone marrow of patients with FUO, the disease of patients should be considered as malignant tumor, especially, lymphoma.

Resumen La babesiosis o "Fiebre de garrapatas" es una enfermedad febril y anemizante, producida en animales domésticos y salvajes y ocasionalmente en humanos por especies del genero Babesia, las cuales son protozoarios intraeritrocíticos. Se denomina ehrlichiosis y anaplasmosis a un grupo de infecciones bacterianas transmitidas por garrapatas duras (Ixodidae), que afectan al ser humano y a los animales. Son de distribución universal, y están relacionadas con varias especies de los géneros Anaplasma y Ehrlichia (familia Anaplasmataceae). La fiebre de origen desconocido en pacientes con historia de viajes a zonas endémicas de Ixodes resulta con alto índice de sospecha para la búsqueda de infecciones por Babesia, Borrelia y Ehrliquia, ya que pueden ocurrir simultáneamente, particularmente con estos dos últimos géneros. Se reporta un caso de paciente femenina de 49 años de edad, procedente de Tumeremo estado Bolívar, ocupación minera, quien consultó por fiebre con escalofríos, cefalea y mialgias predominantes en miembros inferiores. Ingresó a Terapia intensiva por cuadro de distres respiratorio y shock séptico. Se descartó malaria por gota gruesa seriada negativas, el hemocultivo y urocultivo reportaron ausencia de crecimiento bacteriano. Se realizó frotis de capa blanca siendo positiva para Ehrliquia monocítica y Babesia bigemina. La paciente evolucionó satisfactoriamente luego del tratamiento con Doxiciclina, clindamicina más meropenem. El fin de este reporte es concientizar a la comunidad médica de la existencia de la ehrlichiosis como entidad clínica emergente en nuestro país y la posibilidad de coexistir con otros microorganismos que comparten el mismo vector, con el fin de considerar tratamiento empírico oportuno en pacientes con factores de riesgo en las zonas rurales.


Abstract Babesiosis or "Tick fever" is a feverish and anemic disease, produced in domestic and wild animals and occasionally in humans by species of the genus Babesia, which are intra-erythrocytic protozoans. Ehrlichiosis and anaplasmosis are called a group of bacterial infections transmitted by hard ticks (Ixodidae), which affect humans and animals. They are of universal distribution, and are related to several species of the genera Anaplasma and Ehrlichia (family Anaplasmataceae). Fever of unknown origin in patients with a history of travel to endemic areas of Ixodes results in a high index of suspicion for the search for Babesia, Borrelia and Ehrliquia infections, since they can occur simultaneously, particularly with these last two genera. We report a case of female patient, 49 years old, from Tumeremo Bolívar state, mining occupation, who consulted for fever with chills, headache and myalgias predominant in lower limbs. He entered intensive therapy for respiratory distress and septic shock. Malaria was ruled out by gross negative strains, blood culture and urine culture showed no bacterial growth. White-coat smears were positive for monocytic Ehrliquia and Babesia bigemina. The patient progressed satisfactorily after treatment with Doxycycline, clindamycin plus meropenem. The purpose of this report is to make the medical community aware of the existence of ehrlichiosis as an emerging clinical entity in our country and the possibility of coexisting with other microorganisms that share the same vector in order to consider timely empirical treatment in patients with factors risk in rural areas.
