Resumo Este trabalho apresenta a produção de um Protocolo de Enfermagem para o Manejo Clínico do Aleitamento Materno na Atenção Primária à Saúde (APS) para embasar ações e cuidados de enfermagem. O objetivo da pesquisa foi desenvolver um protocolo de enfermagem para o manejo clínico do aleitamento materno na APS. Trata-se de uma pesquisa metodológica que contemplou as fases de elaboração e validação contidas nas Diretrizes para a Elaboração de Protocolos de Enfermagem na APS pelos Conselhos Regionais. A fase de elaboração perpassou por três etapas na primeira, foi realizada uma revisão integrativa da literatura a respeito das fragilidades e potencialidades do cuidado de enfermagem no apoio ao aleitamento materno na APS; na segunda, um levantamento de evidências cientificas por meio da busca por políticas, estratégias, recomendações, guias, manuais, notas, cartilhas, ferramentas, instrumentos, fluxogramas, escalas, protocolos clínicos da Academy of Breastfeeding Medicine, protocolos da atenção básica e demais protocolos, legislações, publicações científicas, publicações de órgãos nacionais, internacionais e regulamentadores, bem como livros clássicos relacionados ao tema; e na terceira, a fim de identificar ações e cuidados de enfermagem a serem contemplados no protocolo, foram desenvolvidas oficinas com profissionais de enfermagem da APS de um município da região metropolitana de Curitiba, o qual tem implantada uma Rede Municipal de Apoio ao Aleitamento Materno. Como resultado, produziu-se um protocolo que conta com 13 vídeos e 9 ilustrações inéditas, fundamentados nos dados obtidos pela revisão integrativa da literatura, levantamento das evidências científicas e oficinas realizadas com a equipe de enfermagem da APS. O protótipo do protocolo foi submetido ao consenso da equipe de enfermagem durante as oficinas. Posteriormente, tanto o protocolo, quanto os vídeos e ilustrações, foram validados por enfermeiros juízes. Concluiu-se que o protocolo contempla o embasamento teórico/prático e meios para conduzir profissionais de enfermagem a superarem parte dos desafios impostos ao estabelecimento e manutenção do aleitamento materno, além de contribuir para a educação em saúde com a divulgação de informações e orientações à população com o uso dos vídeos e ilustrações, bem como incentivar e alicerçar práticas de educação permanente em saúde, possibilitando a sua replicação para outros temas e níveis de atenção. A tecnologia desenvolvida, por meio de pesquisa, é capaz de cumprir o propósito de minimizar parcialmente as fragilidades identificadas pela equipe de enfermagem frente ao atendimento voltado ao aleitamento materno na APS. Esta conjuntura, associada a aspectos institucionais e gerenciais, possibilita que auxiliares, técnicos de enfermagem e enfermeiros se vinculem e aproximem dos usuários, famílias e comunidade, desenvolvam ações e cuidados em prol da promoção, proteção e apoio ao aleitamento materno, contribuindo para o aumento dos seus índices e para a redução das perdas econômicas globais derivadas do "não amamentar".
ABSTRACT
Abstract This paper presents the production of a Nursing Protocol for the Clinical Management of Breastfeeding in Primary Health Care (PHC) to support nursing actions and care. The objective of the research was to develop a nursing protocol for the clinical management of breastfeeding in PHC. This is a methodological research that included the stages of elaboration and validation contained in the Guidelines for the Elaboration of Nursing Protocols in PHC by the Regional Councils. The elaboration phase involved three stages in the first, an integrative review of the literature was carried out regarding to the weaknesses and potential of nursing care in supporting breastfeeding in PHC; in the second, a survey of scientific evidence through the search for policies, strategies, recommendations, guides, manuals, notes, booklets, tools, instruments, flowcharts, scales, clinical protocols of the Academy of Breastfeeding Medicine, primary care protocols and other protocols, legislation, scientific publications, publications of national, international and regulatory bodies, as well as classic books related to the topic; and in the third, in order to identify nursing actions and care to be covered in the protocol, workshops were developed with PHC nursing professionals from a town in the metropolitan region of Curitiba, which has implemented a Municipal Network of Support for Breastfeeding. As a result, a protocol was produced that has 13 videos and 9 unpublished illustrations, based on data obtained from the integrative literature review, survey of scientific evidence and workshops held with the PHC nursing team. The protocol prototype was submitted to the consensus of the nursing team during the workshops. Subsequently, both the protocol and the videos and illustrations were validated by nurse judges. It was concluded that the protocol includes the theoretical/practical basis and means to lead nursing professionals to overcome part of the challenges imposed on the establishment and maintenance of breastfeeding, in addition to contributing to health education with the dissemination of information and guidelines to the population with the use of videos and illustrations, as well as encouraging and supporting practices of continuing education in health, enabling its replication to other themes and levels of care. The technology developed, through research, is able to fulfill the purpose of partially minimizing the weaknesses identified by the nursing team in the face of breastfeeding-oriented care in PHC. This conjuncture, associated to institutional and managerial aspects, allows assistants, nursing technicians and nurses to bond and approach users, families and the community, develop actions and care for the promotion, protection and support of breastfeeding, contributing to the increase of its indices and for the reduction of global economic losses derived from "not breastfeeding".
