RESUMEN

Proper MDT soon after detection of leprosy and anti-reaction measures with newer steroids, regular supervision and monitoring of those released from treatment (RFT) reduce the incidence of ocular leprosy to a remarkable extent. Today, most eye complications are because of normal ageing process or of other phenomena in normal healthy population. Cataract and lagophthalmos are the main causes of blindness. However, in India, though the rate of cataract surgical coverage is up to the mark, the same for lagophthalmos is lagging far behind. Integration of management of ocular leprosy into community eye health care service is the talk of the day along with other health care facilities delivered to people affected with leprosy (PAL). Routine eye examinations are necessary for all PB and MB patients, as well as for the RFT persons in order to detect and treat eyes that are at high risk. All eye surgeries can be performed when needed, irrespective of deformities and bacteriological status, by latest microsurgical techniques with good outcome, and better rehabilitation measures. Reorientation training in ocular leprosy is the immediate special need for ophthalmologists, paramedical ophthalmic assistants and eye health care managers working in general hospitals in those areas that were previously "leprosy endemic zones".

AIM: To describe the incidence and risk factors of vision reducing cataract in skin smear positive lepromatous patients. METHODS: Prospective longitudinal cohort study: 212 newly diagnosed lepromatous patients were followed during the two years of treatment with multidrug therapy and for a further five years, with biannual ocular examinations. Incidence of vision reducing (< or =6/18) cataract was calculated as the number of patients with cataract per person year of cataract-free follow up among those who did not have cataract at baseline. RESULTS: Cataract was present in 27 (11%) of lepromatous patients at diagnosis. Forty nine patients (2.87%/person year (95% confidence interval (CI), 2.17% to 3.80%)) developed cataract during a total follow up period of 1704 person years; 45 of these were > or =41 years old and were followed for a total of 638 person years with an incident rate of 0.070 (95% CI, 0.0523 to 0.094). Stepwise multiple regression confirmed the association of age (per decade) (hazard ratio (HR) = 2.50 (95% CI, 1.82 to 2.78), p<0.001), clofazimine crystals on the cornea (HR = 49.92 (5.48 to 454.82), p = 0.001), grade 2 deformity in all limbs (HR = 3.17 (1.12 to 8.97), p = 0.029), and uveal inflammation (HR = 3.52 (1.42 to 8.67), p = 0.006). No significant association was found with oral steroids. CONCLUSIONS: Cataract develops at the rate of 7%/person year in lepromatous patients over 40 years of age. It is associated with increasing age, subclinical intraocular inflammation, and grade 2 deformity.

RESUMEN

PURPOSE: To check the complications of cataract surgery with intraocular lens implantation in leprosy patients by the phacoemulsification technique, and to compare these results, with a control group. METHODS: The used method was a case control study, where 31 eyes from leprosy patients were submitted to cataract surgery, from June 1999 to January 2003, called Case Group. On the other hand, 31 eyes of healthy patients were selected for the Control Group. The comparison between the groups was performed using Fisher's exact test. The used program was EPI INFO 6.04. RESULTS: Sphincter tear was the only postoperative complication higher in the leprosy group (p=0.024). The other alterations did not show statistic differences (p>0.05). Visual acuity improvement was the same in both groups. CONCLUSIONS: This study showed that the visual acuity and complications after cataract surgery with intraocular lens are the same in leprosy patients and healthy ones.

OBJETIVO: Verificar as principais alterações e complicações relacionadas à cirurgia de catarata com o implante de lente intra-ocular em portadores de hanseníase, utilizando-se a técnica da facoemulsificação e, comparar estes resultados com um grupo de pacientes sem a doença. MÉTODOS: O método utilizado foi o estudo caso-controle, no qual foram incluídos 31 olhos de hansenianos operados no período de junho de 1999 a janeiro de 2003, denominados de Grupo Caso. Paralelamente foram selecionados outros 31 olhos de pacientes não hansenianos denominados de Grupo Controle. As comparações entre os Grupos foram realizadas pelo teste exato de Fisher. Foi utilizado programa de processamento estatístico EPI INFO versão 6.04. RESULTADOS: Na análise das complicações pós-operatórias, apenas a rotura de esfíncter evidenciou relação com a hanseníase, com um p=0,024. A distribuição da freqüência das demais alterações não mostrou significância estatística (p>0,05). A melhora na acuidade visual foi semelhante nos dois grupos. CONCLUSÃO: O estudo comprovou que, quanto à acuidade visual no pós-operatório e as complicações, os pacientes hansenianos assemelham-se à população em geral, quando utilizada a técnica da facoemulsificação com introdução de lente intra-ocular.

AIMS: To determine the coverage, outcome, and barriers to uptake of cataract surgery in leprosy villages of north eastern Nigeria. METHODS: People 30 years and above resident in eight leprosy villages were examined. Cataract blind people were questioned about the reasons they had not been treated. Subjects who had received an operation for cataract were examined to determine the outcome and, where applicable, the causes of poor outcome. RESULTS: 480 people were examined. Cataract was the commonest cause of blindness. The cataract surgical coverage (people) was 39.2% for orthodox surgery and 29.7% for couching. After surgery, visual acuity > or =3/60 had been restored to 82.1% of eyes that had had orthodox surgery, but only 58.6% of eyes that had been couched. Cost was the commonest reason given for not seeking treatment for cataract. CONCLUSIONS: Cataract is the major cause of blindness in this population but cataract surgical needs are currently not being met. There is a need for better collaboration between leprosy control and ophthalmic services, improved education of people affected by leprosy, a commitment to improving orthodox cataract surgery outcomes, and consideration of a possible role for traditional healers as sources of referral for orthodox surgical services.

RESUMEN

A world-wide study on the ocular complications of leprosy has been carried out over the past ten years. The data from 4772 patients, designed to give baseline information for a five-year incidence study, have been analysed. Blindness due to leprosy was seen in 3.2% of the sample and 7.1% had Grade 2 visual disability. The causes of visual impairment in the disease are discussed and it is emphasized that a high proportion of these are preventable, particularly through the early use of multidrug therapy. The active participation of ophthalmologists in the management of the disease is still required since many of the blinding complications respond well to surgery.

In Belo Horizonte, Brazil, 70 eyes of 53 leprosy patients had extracapsular cataract extraction and intraocular lens implantation done during a period of four years. The authors analyzed the outcome regarding restoration of vision and complications after this procedure. The visual acuity improved in 92.9% of the eyes and in 65.7% the acuity had improved by four lines or more on the Snellen chart. The post-operative complications could not be associated only to leprosy infiltration; in any case, they were not too serious and could be controlled.

Cataract is a blinding disease occurring all over the world. One of the causes of cataract is leprosy. Sixty leprosy and 100 nonleprosy patients were assessed and underwent intracapsular cataract extraction. Leprosy patients with cataract were much younger than nonleprosy patients. The leprosy group had a significantly higher rate of complications and this was seen more in paucibacillary cases. There was a higher rate of visual disability in the leprosy group than in the nonleprosy group. Cataract was seen in younger patients in the leprosy group. This raised the possibility of leprosy being the cause of the cataract. The leprosy group consisted mostly of multibacillary cases, however unlike in other studies the rate of complications tended to be higher in the paucibacillary group. There were no preoperative findings that correlated with a low postoperative intraocular pressure.

In Liberia, 43 eyes of 30 patients with ocular leprosy underwent cataract extraction; 33 eyes had extracapsular cataract extraction (ECCE) and 10 eyes had intracapsular cataract extraction (ICCE). ICCE was performed in eyes with poor visualization of the anterior chamber. In 95% of the eyes, the postoperative vision improved by 2 Snellen lines or more, but functional visual acuity (better than 20/200) was achieved in only 65% (82% post-ECCE and 10% post-ICCE). Fewer postoperative complications were observed after ECCE. These findings may have been related to less ocular involvement by leprosy preoperatively. ECCE should be attempted when the visualization of the anterior chamber is fair.

RESUMEN

Although leprosy is considered rare in Australia outside the Northern Aboriginal population, its presence within Indian and Southeast Asian migrant groups must be considered. A case of ocular leprosy is presented in which the definitive diagnosis was delayed because lid changes had been diagnosed as senile ectropion. The pathology of ocular leprosy is discussed, together with relevant therapeutic considerations.

Of 744 leprous patients, 61 (8.2%) had cataract-induced blindness; 46 patients (6.2%) were unilaterally blind and 15 (2.0%) were bilaterally blind. The mean age of patients with cataracts was 63.4 years in the tuberculoid-type leprosy and 56.4 years in the lepromatous type. The appearance of cataracts in lepromatous patients at an earlier age than in tuberculoid patients is significant (p less than 0.005). Of 41 cataractous eyes with posterior iris synechiae, 42.1% were observed in patients with tuberculoid-type leprosy and 59.1% in patients with the lepromatous type. Histopathological examination of 24 lenses showed that irregularities of the lens epithelium were present in 79.2%. Posterior iris synechiae were seen in 66.7% and fibrous pseudometaplasia in 41.7%. Posterior migration of the lens epithelium in the posterior subcapsular area was observed in 54.2%. In view of these clinical and histopathological findings, we discuss the probability that most cataracts in leprous patients are complicated in nature.

Clinical and pathological investigations in a group of 113 patients with leprosy of long duration demonstrated the importance of iris changes as a cause of blindness. In lepromatous leprosy the so called "chronic iritis" produces iris atrophy with small nonreacting pupils which exaggerate the visual impairment created by developing lens changes and corneal opacities. The cause of this "chronic iritis" is believed to be neuroparalytic from early involvement of the small nerves of the iris, particularly the autonomic supply. Clinical and pharmacological evidence for this theory is supported by the histological changes observed in 18 specimens of iris removed during the course of cataract surgery, with progressive atrophy of the iris preferentially affecting the dilator muscle and leading to a nonreacting miosed pupil. Further pharmacological and histological studies are to be undertaken on lepromatous patients with iris involvement with a view to establishing possible methods of prevention.

Clinical examination of 113 patients in South Korea with lepromatous leprosy and severe visual impairment showed that the main cause of visual loss was the combined effect of corneal and lens opacities associated with small nonreacting pupils and iris atrophy. Cataract surgery with broad iridectomy and inferior sphincterotomy offers these patients with chronic lepromatous complications the best chance of preserving vision. Eighty-one cataract operations were performed under local anaesthesia, and in 90% vision improved; in 60% this improvement was 2 Snellen's lines or more.
