RESUMO

Mice injected interperitoneally with 1.5 X 10(8) Mycobacterium lepraemurium develop progressive visceral alterations that are reflected in the lactic dehydrogenase (LDH), glutamate-pyruvate transaminase (GPT), and glutamate-oxalacetate transaminase (GOT) levels. The rise in GPT and GOT levels starts earlier (about 30 days post-infection) than the rise in LDH activity (about 70 days), but the latter shows the most impressive increases. Differences between infected and control groups, however, reach statistical significance only at 75 days (LDH), 90 days (GOT), and 150 days (GPT) post-inoculation, still well before the appearance of obvious external signs of infection (about 240 days in our model). It is suggested that the ratio of enzyme levels in infected to enzyme levels in uninfected animals could be taken as a reliable index to follow the progress of the infection with M. lepraemurium.

Armadillos (Dasypus novemcinctus) were inoculated with Mycobacterium leprae isolated from lepromas taken from untreated lepromatous patients or from the spleen of an armadillo previously infected with human M. leprae. The effect of the infection on the serum levels of lactic dehydrogenase (LDH), alkaline phosphatase (AlkP), glutamate-oxalacetate (GOT) and glutamate-pyruvate (GPT) transaminases was investigated. In general, there was a good correlation between positive evidences of infection and alterations in the levels of LDH, GOT, and GPT. Although elevations in LDH levels were more striking, elevations in GOT and GPT levels were more consistent with the disease. When an absolute increase in the total LDH activity was not observed in a M. leprae-infected animal, an increase in the level of LDH isozyme V was still clearly evident. Serum levels of alkaline phosphatase were not affected by the disease. The ratio GOT/GPT (greater than 1.0) in the infected animals reflected and supported the chronic nature of the disease and the liver involvement. The enzymatic alterations are not, however, specific for leprosy.

Twenty patients with suspected DDS resistance and repeated attacks of lepra reactions were selected for the study. Clofazimine was administered in different doses over a period of 12 months. Elevated levels of transaminases and Alkaline phosphatase prior therapy attained values to near normalcy. Progressive fall in serum Bilirubin and Proteins with normal A/G ratio at the end of therapy was also observed. Clofazimine by its anti-inflammatory and antibacterial action could inhibit the process of liver damage and happened to have minimal deleterious effect on liver by studying the liver function tests.

Serum enzyme profile was studied in 74 patients with Tuberculoid leprosy and compared with that of 100 healthy controls. The enzymes studied were Aspartate and Alanine Transaminase, Isocitric dehydrogenase, Alkaline phosphatase Choline esterase and Creatine kinase. Both Transaminase and Alkaline phosphatase were in general within normal limits in Tuberculoid leprosy. Creatine kinase levels were found to be significantly elevated. In some of these cases raised levels of Serum Aspartate Transaminase was also noticed--evidence for skeletal muscle destruction. Mild increases in Isocitric dehydrogenase and a concomitant increase in Alanine Transaminase in some of the patients suggest a sub-clinical hepatic damage. A statistically significant decrease in Serum Choline esterase level was noticed in this study on Tuberculoid leprosy patients.

RESUMO

Ninety seven male patients attending the Leprosy clinic of the Dermatology Out-patients' Department were studied for the detection of Australia antigen. There was a 20% antigen positivity in patients suffering from Lepromatous Leprosy and 5.7% in the cases with Tuberculoid Leprosy. SGPT level was found to be significant in patients with Lepromatous Leprosy having Australia antigen as compared to other groups.

The liver function was studied in 100 hanseniasis patients. They were classified in the following 10 groups: quiescent LL (20 patients), quiescent BL (13 patients), quiescent BB (7 patients), quiescent BT (9 patients), quiescent TT (12 patients), reactional LL (12 patients), reactional BL (10 patients), reactional BB (7 patients), reactional BT (5 patients), reactional TT (5 patients); a comparison was made with a control group (10 healthy individuals). It was found a hyperproteinemia with high levels of globulin and normal levels of albumin in the serum of all clinical forms, and a typical pattern of "light damage hepatic cells" with raised in the enzymatic activity of GOT, GPT and alkaline phosphatase with no changes in turbidity tests and bilirrubinemia, in all reactional patients. The physiopathology of this problem and the types III and IV hypersensitivity phenomena are discussed.

A double blind trial involving intermittent administration of Rifampicin in addition to daily DDS has been undertaken in order to evaluate the efficacy as also the potential dangers of such a regimen. Twenty untreated LL cases who were otherwise healthy were included in the study. Ten cases received weekly 900 mg Rifampicin for 6 weeks in addition to 100 mg daily DDS, while the rest were treated likewise but were given similar looking placebo capsules instead of RFP. A nine month follow-up, as also mouse foot pad results indicate that the efficacy of this regimen was found to be better than that with DDS alone and this compares favourably with trials involving 600 mg Rifampician administration daily. No major untoward side effects were encountered in the trial group thought the incidence of ENL was slightly higher in the trial group.

In this series, the commonest aetiology was tuberculosis (30 cases, 28%), followed by sarcoidosis (18 cases, 17,7%), mediterranean fever (Olmer's disease) (13 cases, 12,1%), brucellosis (8 cases, 7,4%), typhoid fever (7 cases, 6,6%) and idiopathic forms (8 cases, 7,4%). These were followed by Hodgkin's disease, toxoplasmosis, adenosarcoma, and leprosy. Finally, there were single cases due to infectious mononucleosis, B.C.G. reaction, hypogammaglobulinaemia, coeliac disease and temporal arteritis. Half of the patients had hepatomegaly and an increase, in general moderate, in hepatic enzymes (transaminases, alkaline phosphatase). The highest enzyme levels were seen in cases of brucellosis, hepatic enzymes being normal in patients with sarcoidosis.

Thirty eight patients of leprosy were classified on the basis of clinical and biopsy findings into 10 lepromatous (all reactional), 20 tuberculoid (7 in reaction) and 8 dimorphic (5 in reaction). The liver function tests--serum proteins, albumin/globulin ratio, fractions in paper electrophoresis, serum glutamic pyruvic transaminase (SGPT) and thymol turbidity tests--and the serum cholesterol was estimated in each patient. The abnormalities were a reversal of the albumin/globulin ratio in all the three groups and an increase in beta/globulin fraction in the lepromatous group, but total proteins were within normal limits. The thymol turbidity test showed moderately elevated levels in all three groups. SGPT was slightly raised in a few cases. Only a single patient with tuberculoid leprosy showed serum cholesterol to be over 250 mg per 100 ml.

RESUMO

Urinary 17-ketosteroids were estimated in 29 lepromatous leprosy cases. Correlation between 17-ketosteroid values, histopathologic findings, and serum G.G.P.T. values is discussed. Low values of 17-ketosteroids were associated with definite leproma to liver indicating the value of liver damage to 17-ketosteroids. This was more marked in males than in females.
