RESUMO

Drugs used to treat pain are associated with adverse effects, increasing the search for new drugs as an alternative treatment for pain. Therefore, we evaluated the antinociceptive behavior and possible neuromodulation mechanisms of triterpene 3ß, 6ß, 16ß-trihydroxylup-20(29)-ene (CLF-1) isolated from Combretum leprosum leaves in zebrafish. Zebrafish (n = 6/group) were pretreated with CLF-1 (0.1 or 0.3 or 1.0 mg/mL; i.p.) and underwent nociception behavior tests. The antinociceptive effect of CFL-1 was tested for modulation by opioid (naloxone), nitrergic (L-NAME), nitric oxide and guanylate cyclase synthesis inhibitor (methylene blue), NMDA (Ketamine), TRPV1 (ruthenium red), TRPA1 (camphor), or ASIC (amiloride) antagonists. The corneal antinociceptive effect of CFL-1 was tested for modulation by TRPV1 (capsazepine). The effect of CFL-1 on zebrafish locomotor behavior was evaluated with the open field test. The acute toxicity study was conducted. CLF-1 reduced nociceptive behavior and corneal in zebrafish without mortalities and without altering the animals' locomotion. Thus, CFL-1 presenting pharmacological potential for the treatment of acute pain and corneal pain, and this effect is modulated by the opioids, nitrergic system, NMDA receptors and TRP and ASIC channels.

We investigated the use of one- and two-mediator systems in amperometric BOD biosensors (BOD, biochemical oxygen demand) based on the yeast Debaryomyces hansenii. Screening of nine mediators potentially capable of electron transfer - ferrocene, 1,1'-dimethylferrocene, ferrocenecarboxaldehyde, ferroceneacetonitrile, neutral red, 2,6-dichlorophenolindophenol, thionine, methylene blue and potassium ferricyanide - showed only ferrocene and neutral red to be efficient electron carriers for the eukaryotes studied. Two-mediator systems based on combinations of the investigated compounds were used to increase the efficiency of electron transfer. The developed two-mediator biosensors exceeded their one-mediator analogs by their characteristics. The most preferable two-mediator system for developing a BOD biosensor was a ferrocene-methylene blue combination that ensured a satisfactory long-time stability (43 days), selectivity, sensitivity (the lower limit of the determined BOD5 concentrations, 2.5mg О2/dm3) and speed (assay time for one sample, not greater than 10min) of BOD determination. Analysis of water samples showed that the use of a ferrocene-methylene blue two-mediator system and the yeast D. hansenii enabled registration of data that highly correlated with the results of the standard method (R=0.9913).

BACKGROUND: Granulomatous dermatitis is a distinctive histopathologic cutaneous reaction pattern against various infectious and noninfectious agents. Cytologically, granulomatous dermatitis shows granulomas and multinucleated giant cells. Various etiologic agents of granulomatous diseases can also be identified. OBJECTIVE: We aimed to investigate Tzanck smear findings in granulomatous skin diseases. METHODS: Patients who had granulomas and/or multinucleated giant cells of Langhans, foreign body- and/or Touton type in Tzanck smear tests were included in the study. In these patients, Tzanck preparations were then further evaluated for additional cytologic findings. Samples stained with May-Grünwald-Giemsa stain were evaluated by the same dermatologist throughout the study. In some patients, methylene blue, Gram and/or Erlich-Ziehl-Nielsen stains were also performed. In all of the study cases, the final diagnosis was established after the evaluation of clinical and laboratory findings (including, when appropriate, potassium hydroxide examination; bacterial, leishmanial, and fungal cultures; histopathology; tuberculosis and leishmania polymerase chain reaction). We also calculated the sensitivity and specificity of the Leishman-Donovan body for cutaneous leishmaniasis. RESULTS: Over a 2-year period, 94 of 950 patients (9.9%) in whom Tzanck smear tests were performed had cytologic findings consistent with a granulomatous reaction. In 74 (78.7%) and 20 (21.3%) patients, the granulomatous reaction was due to infectious and noninfectious causes, respectively. Infectious causes included cutaneous leishmaniasis in 65 patients (87.8%), candidal granuloma in two patients, botyromycosis in two patients, and aspergillosis, blastomycosis, mucormycosis, leprosy, and cutaneous tuberculosis in one patient each. In 58 of 74 patients (78.4%) with infectious granulomatous dermatitis, the causes of the granulomas were identified. Noninfectious granulomatous reactions were due to granuloma annulare in 7 patients, sarcoidosis in 5 patients, a foreign body in 4 patients, necrobiosis lipoidica in 2 patients, and juvenile xanthogranuloma in 2 patients. In 17 of 20 patients (85%) with noninfectious granulomatous reactions, the cytologic findings were characteristic of the final diagnoses. The sensitivity and specificity of Leishman-Donovan bodies for cutaneous leishmaniasis were 76.9% and 100%, respectively. LIMITATIONS: All of the samples were evaluated by the same dermatologist throughout the study; therefore no comment could be made regarding the reliability of the Tzanck smear test. In addition, the sensitivity and specificity of Tzanck smear test findings for diseases other than cutaneous leishmaniasis could not be calculated because of an insufficient number of patients. CONCLUSION: The Tzanck smear test may be a useful diagnostic tool for certain granulomatous skin diseases.

La dapsona es una sulfona. Esta ha sido de gran ayuda en el tratamiento de distintos tipos de lepra. Es también efectiva en el tratamiento de la dermatitis herpetiforme, pénfigos y en el tratamiento y prevención de la neumonía por Pneumocystis carinii y de la toxoplasmosis en pacientes con SIDA. Los efectos adversos más comunes son la metahemoglobinemia y la anemia hemolítica; esta última ha sido descripta en pacientes con déficit de glucosa 6-fosfato dehydrogenasa. Estas reacciones están frecuentemente relacionadas con las dosis. Presentamos dos pacientes que tomaron intencionalmente una sobredosis de 3 y 30 (100 mg) tabletas respectivamente. Los signos de intoxicación fueron cianosis, náuseas, vómitos, dolor abdominal y cefalea. Las concentraciones de metahemoglobina variaron de 29,3% a 50%. El tratamiento incluyó medidas de sostén y azul de metileno endovenoso. El Joven de 14 años desarrolló una elevación de enzimas hepáticas. La metahemoglobinemia se normalizó en 36 horas en el primer paciente y en una semana en el segundo

We report the case of a 14-year-old girl who deliberately ingested 8-9 g of dapsone and presented with severe methaemoglobinaemia and altered mental status. Prompt treatment with repeated doses of methylene blue and organ support brought about control of the methaemoglobinaemia and averted organ failure.

We report clinical findings and pharmacokinetic data regarding a combined dapsone and clofazimine intoxication in a man, who ingested 50 tablets of dapsone (100 mg) 20 capsules of clofazimine (100 mg) and two tablets of rifampicin (600 mg). Oral administration of activated charcoal (50 grams) and sodium sulphate (20 grams) after gastric lavage resulted in an elimination half-life in plasma of 11.1 and 10.8 h for dapsone and its main metabolite, monoacetyldapsone, respectively. A rapid initial decrease of the plasma concentration of clofazimine was observed after gastric lavage and administration of activated charcoal and sodium sulphate. 15 h after this treatment, clofazimine plasma levels remained relatively constant. Dapsone-induced methaemoglobinaemia (48% at admission) was treated successfully with methylene blue.

Acute poisoning from excessive dapsone (4,4-diaminodiphenylsulfone) intake in uncommon in the United States; no cases were reported during 1992. However, with the increasing use of dapsone for diseases other than leprosy and dermatitis herpetiformis, such as acne vulgaris, psoriasis, and Pneumocystis carinii pneumonia infection in acquired immunodeficiency syndrome, clinicians should be aware of potential toxicities. This report describes a case of accidental dapsone intoxication resulting in severe cyanosis with a modest elevation in methemoglobin concentration.

The measurement of glycosaminoglycans (GAG) in urine is very important because GAG is an essential constituent of urinary diseases. Since previously reported methods of determination of GAG are quite complicated, dangerous or not stable, we developed a very simple and rapid method to estimate the content of GAG in urine using a microplate and microplate reader. An aliquot of diluted urine is mixed with a 1,9, dimethylmethylene blue (DMB) dye. The assay is based on the metachromatic shift in absorption maximum which occurs when sulfated glycosaminoglycans are added to the dye. The recovery of GAG is good. Other urinary compounds such as sugar, protein and blood did not influence the measurement of GAG. A good correlation was obtained between the modified DMB method and Blumenkrantz and Asboe-Hansen's method (r = 0.737, p < 0.001). The accuracy of this method makes it useful for screening urinary diseases.

In Spurr-embedded biopsies for ultrastructural examinations, Mycobacterium leprae hardly differed from the surrounding tissues using the staining technique of Richardson. Also, the usual histological staining methods of Ziehl-Neelsen and Fite did not achieve positive results for the determination of M. leprae. Therefore, we applied the methylene blue-borax and basic fuchsin technique for the demonstration of the bacilli in plastic-embedded tissue of 11 patients suffering from Hansen's disease. In every patient the diagnosis was confirmed by histological examination of skin biopsies. Portions of the biopsies of nine patients were then fixed in glutaraldehyde and osmium tetroxide and embedded according to Spurr. In the other two cases, the material was first fixed in 10% Formalin and embedded in paraffin. After cutting 3-micron sections for routine histological examination, the remaining material was prepared adequately for ultrastructural examination. Using the methylene blue-borax and basic fuchsin technique, the semithin sections of plastic-embedded material presented a considerably more differentiated picture than other comparable methods. M. leprae located in foamy cells or in the tissue stained violet. These findings were corroborated in subsequent electron-microscopic examinations. The semithin sections thus prepared allow, through the clear demonstration of the microorganisms, a precise demarcation of the ultrathin area.

Authors analyse the counterstain with methylene blue solution in the usual Ziehl-Neelsen method. Considerations are made on the characteristics of the dye substance. A technique alteration is proposed: a "concomitant alkalization" of the classic aqueous methylene blue solution, by adding some drops of sodium hydroxide solution, 1:500, on the slide at the moment of the staining. By this technique it was observed: a) a larger validity period of the solution; b) an absence of precipitate in the slides; c) an easier visualization of the substrate; d) a larger contrast among the substrate and the bacilli.

A identificaçäo do M.leprae, em material obtido de lesöes cutâneas de pacientes suspeitos ou doentes de hanseníase, é fundamental nas atividades de controle da endemia. É feita com coloraçäo da linfa proveniente das lesöes, espalhada em lâmina, pelo método de Ziehl-Neelsen. Dada a escassez de muco nas preparaçöes rotineiras a técnica teve de ser modificada com alcalinizaçäo prévia do azul de metileno. Tal modificaçäo, embora melhorando as possibilidades de uso do Ziehl-Neelsen, apresenta inconvenientes de ordem prática, como os decorrentes da metacromasia e precipitaçäo do corante. A coloraçäo de fundo se apresenta, entäo, arroxeada, dificultando a visualizaçäo do bacilo corado em vermelho.A precipitaçäo reduz o tempo de uso do corante. Os autores, após demonstrar tais inconvenientes, propöem alteraçäo técnica denominada de "alcalinizaçäo concomitante" que, basicamente, consiste em adicionar poucas gotas de soluçäo de hidróxido de sódio, a l:500, sobre a coloraçäo clássica, no momento de sua execuçäo. Obtém-se assim: a) maior tempo de uso do corante; b)ausência de precipitado em lâmina; c)evidenciaçäo mais fácil do substrato; d) contraste maior entre o substrato e a coloraçäo do bacilo. Recomendam o uso da variante técnica proposta na rotina, principalmente nas preparaçöes com material escasso ou de controle terapêutico

RESUMO

A technique is described using soluble blue instead of the conventional methylene blue as a counterstain for the Ziehl-Neelsen procedure. This increases the intensity of blue staining of non-bacterial contaminants and is useful in monitoring the progress of purification of M. leprae from host tissue.

Using M. lepraemurium suspensions obtained from leproma of rats inoculated by subcutaneous route or from nodules of the peritoneal cavity, the AA. have tested the capacity of these bacilli in reduce the methylene blue and formation of formazan from the tetrazolium. The time or intensity of reduction of methylene blue or formation of formazan are proportional to the concentration of bacilli in the suspensions and is increased with the storage in the refrigerator at 1øC. In comparison with tests perfomed upon other acid fast bacilli the ones with M. lepraemurium are developed very slowly.

The AA. have made bactericidal studies of drug solution (promin, sulphetrone and sulfanilamide) at 40 miligrams per 100 ml. upon M. lepraemurium suspensions and using the method described in previous report, they tested the capacity of these bacillary suspensions in reduce the methylene blue. By these method they failed to show any bactericidal activity of these drugs on the M. lepraemurium.
