ABSTRACT

Resumen: La historia o ficha clínica ha servido de instrumento de registro de las actividades sanitarias desde el inicio de las profesiones de salud, otorgándosele diversas utilidades y valoraciones, según su objetivo, un valor docente, en investigación, judicial, entre otros. Ante las diferentes interpretaciones de las normas vigentes en Chile se ha debido legislar en su uso, pertenencia, contenido y otros aspectos, aclarando algunas situaciones pero restringiendo su acceso en otros aspectos. Dado las distintas leyes y normativas a las que se asocia su uso, se hizo necesario elaborar un documento que incluya sus aspectos más importantes. Aún quedan elementos asociados a las costumbres, creencias, especialidades en salud, entre otras, que no han sido abordados por las leyes, además de la labor docente de este instrumento.


Abstract: The history or clinical record has served as an element of registration of health activities since the beginning of the health professions, granting him other utilities and valuations according to their objective, as a teaching value, in research, judicial, among others. Given the different interpretations of the norms in force in Chile, it has had to legislate in its use, membership, content and other aspects of the clinical file, clarifying some situations, but restricting their access in other aspects. Given the different laws and regulations associated with its use, it became necessary to produce a document that collects its most important aspects. There are still aspects associated with customs, beliefs, specialties in health, among others, that have not been addressed by the Laws, in addition to the teaching work of this instrument.


Resumo: A história ou ficha clínica tem servido como instrumento de registro das atividades sanitárias desde o início das profissões de saúde, concedendo-se a elas vários utilitários e valorações de acordo com sua finalidade: ensino, pesquisa, valor jurídico, entre outros. Para as diferentes interpretações das normas vigentes no Chile havia de legislar em seu uso, composição, conteúdo e outros aspectos da ficha clínica, esclarecendo algumas situações, mas restringindo seu acesso em outros aspectos. Tendo em conta as diferentes leis e regulamentos aos quais o seu uso está associado, tornou-se necessário apresentar um documento que reúne os aspectos mais importantes. Existem ainda os aspectos associados a costumes, crenças, especialidades em saúde, entre outros, que não foram abordadas pelas leis, além do ensino deste instrumento.

ABSTRACT

The maldistribution of biomedical services creates a dilemma for Indian patients. They encounter a bewildering array of medical services, ranging from qualified traditional medical practitioners to untrained, self-taught purveyors of medicines and cures. Research on Indian healthcare has decried the inefficient distribution of services in rural and urban areas. The studies discussed here reveal the ground reality of the consequences of limited choices for patients, characterised as "forced pluralism," with no state regulation of type of care, quality of care, or credentials of practitioners.

ABSTRACT

This article presents partial findings from a study on trends towards greater or lesser proximity between homeopathic and allopathic physicians, from the perspective of the latter. Forty-eight health professionals were interviewed (faculty, managers, and physicians working in the public health system). This specific article focused only on the interviews with health system managers. The following concepts were used as references: social and scientific field (Bourdieu); medical rationalities (Madel Luz); technological arrangements in health work (Mendes-Gonçalves); and physician's professional identity (Donnangelo & Schraiber). According to the findings, support by managers for the presence of Homeopathy in the Unified National Health System is related to their perception of social demand, defense of patients' right to choose, and the observation that it is a medical practice that reclaims the humanist dimension of medicine, thus contributing to user satisfaction. The difficulties and resistances identified by managers highlight that the lack of information on homeopathic procedures limits the possibilities for use of Homeopathy because it leads to insecurity towards this area of medicine.

ABSTRACT

Este artigo apresenta parte dos resultados de pesquisa que investigou características do movimento de aproximação e afastamento entre homeopatas e médicos da Biomedicina, segundo o ponto de vista dos profissionais não homeopatas. Foram entrevistados 48 profissionais de saúde (docentes, gestores e médicos que trabalham na rede publica). Toma-se para análise apenas os resultados das entrevistas com gestores. Foram usadas como referências as concepções de: campo social e científico de Bourdieu; racionalidades médicas de Madel Luz; arranjos tecnológicos do trabalho em saúde de Mendes-Gonçalves e de identidade profissional de médico de Donnangelo e de Schraiber. Os resultados indicam que o apoio de gestores à presença da Homeopatia no SUS relaciona-se à percepção da demanda social, à defesa do direito de escolha dos usuários e à constatação de tratar-se de uma prática médica que resgata a dimensão humanista da medicina, contribuindo assim para a satisfação do usuário. As dificuldades e resistências apontadas pelos gestores ressaltam que a falta de informações sobre os procedimentos homeopáticos limita as possibilidades de utilização da Homeopatia porque gera insegurança sobre esta medicina.

ABSTRACT

It is not clear if medical records belong to the physician, the institution or the patient. There is a bill being discussed in the Chilean Congress that establishes that "the patient, personally or through a representative, has the right to access and review his medical record. In case of death, this right may be exerted by his inheritors". In this article we postulate that this bill infringes a number of legal norms in force and universally accepted ethical principles. We distinguish between patient's advocacy to be informed and their free access to medical records. The main ethical principles violated are those of beneficence and non maleficence
