ABSTRACT

BACKGROUND: Benign Prostatic Hypertrophy (BPH) is common in older men. This study compared homeopathic treatment strategies using constitutional medicines (CM) or organopathic medicines (OM) alone or in combination (BCOM) in patients suffering from BPH. METHODS: 220 men aged 30-90 years were recruited in Odisha, India. Patients presenting symptoms of prostatism, with or without evidence of bladder outflow obstruction were included in the study. Patients with serum prostate specific antigen (PSA)> 4 nmol/mL, malignancy, complete urine retention, stone formation and gross bilateral hydronephrosis were excluded. Patients were sequentially allocated to OM, CM or BCOM. The main outcome measure was the International Prostate Symptom Score (IPSS). RESULTS: 73, 70 and 77 patients respectively were sequentially allocated to OM, CM or BCOM. 180 patients (60 per group) completed treatment and were included in the final analysis. Overall 85% of patients showed improvement of subjective symptoms such as frequency, urgency, hesitancy, intermittent flow, unsatisfactory urination, feeble stream, diminution of residual urine volume but there was no reduction in prostate size. Treatment response was highest with BCOM (38.24%) compared to OM (31.62%) and CM (30.15%). Effect sizes were highest for the decrease in IPSS, residual urine volume and urinary flow rate.

ABSTRACT

Dietary supplements are commonly used by patients as part of their medical care plan. Often clinicians may not be aware of their use, because patients do not always consider these to be medications. All clinicians need to continually ask patients about their use of dietary supplements when collecting a medication history. Dietary supplements and prescription medications often share similar enzymatic pathways for their metabolism. These interactions may lead to severe adverse reactions. This article reviews available evidence for a variety of dietary supplements in select disease categories.

ABSTRACT

La obstrucción del tracto de salida ha sido asociada con la hiperplasia prostática benigna (HPB), dado el crecimiento progresivo del adenoma, lo cual lleva a un incremento en la resistencia al flujo urinario, junto con un efecto deletéreo en la función renal. Se han descrito múltiples teorías asociadas con el desarrollo de la hiperplasia. Entre las cuales se encuentran: La edad, el incremento en la actividad simpática, alteraciones hormonales, el síndrome metabólico y la inflamación crónica. En los últimos años, el rol de la inflamación prostática local en el entendimiento de la patogénesis y progresión de la HPB ha cobrado relevancia. Esa hipótesis plantea que infiltrados inflamatorios conllevan a una lesión tisular, generando así un proceso crónico de cicatrización que condiciona el incremento en el tamaño prostático. El presente artículo se enfoca en describir los mecanismos inflamatorios involucrados en la fisiopatología de la hiperplasia prostática beniga


Urinary tract obstruction has been associated with benign prostatic hyperplasia (BPH), given the progressive adenoma growth. This process leads to an increase in urinary flow resistance, associated with a deleterious effect of renal function. Age, increased sympathetic activity, hormonal alterations, metabolic syndrome and chronic inflammation are among the multiple theories associated with the development of hyperplasia. In recent years, the role of local prostatic inflammation in understanding the pathogenesis and progression of BPH has become relevant. This hypothesis suggests that inflammatory infiltrates lead to a tissue injury, thus generating a chronic healing process that conditions the prostatic increase. The present article focuses on describing the inflammatory mechanisms involved on BPH pathophysiology.

ABSTRACT

OBJECTIVE: To investigate the effect of Qianlie Huichun capsule on the microstructure and ultranstructure of prostate glandular tissue in the model rat. METHOD: Hynertophy of prostate model rat was established by injecting testosterone to gelding male rats. After having been fed with Qianlie Huichun capsule for 30 days, the rats were killed and prostate tissues were resected for pathomorphological studies with microscope and electromicroscope, and the diameter of glandular lumer and the height of glandular epithelial cells were measured under the microspcope for different groups of rats. RESULT: In the model groups, the glandular epithelial cells mutiplycated notably, showing stratified and pseudostratified cells that made the glandular lumer cramped. Under the electromicroscope, the glandular epithelial cells became high columnor and the rough endoreticulum extremely expanded. But in treatment groups, the change of the diameter of the glandular lumer and the height of the glandular epithelial cells were less remarkable than those in model groups. So the differerence between the model group and the treatment groups was remarkable (P < 0.01). CONCLUSION: Qianlie Huichun capsule can depress the glandular epithelialceu multiplication of prostate gland in model rats.

ABSTRACT

OBJECTIVE: To investigate the effect of Qianlie Huichun capsular on Fas expression and cell apoptosis of prostate gland tissue in the model rat. METHOD: Hypertrophy of prostate model rat was established by injecting testosterone to gelding male rats. After being treated with Qianlie Huichun ig 30 days, the rats were killed and prostate glands were resected for examination. The Fas expression was examined by immunobistochemical SABC. The cell apoptosis and the peak of cell apoptosis in the prostate gland of the rats were examined by flow cytometry. Compared with model group, the weight of prostage gland tissue in groups treated with Qianlie Huichun capsular was light (P < 0.01). RESULT: Compared with model group, the Fas expression in all treatment groups increase(P < 0.05, P < 0.01). Compared with normal and model groups, cell apoptosis in all treatment groups increase at different level(P < 0.01). CONCLUSION: The Qianlie Huchun capsular increases the Fas expression and cell apoptosis of model rats, and shows a definite treatment effect on the hypertrophy of prostate by promoting the apoptosis of prostate cell.

A study of urological patients confirmed the therapeutic effect of homeopathic agents in high dilutions. According to authors' concept the acting basis of this treatment is the information-force field appearing in the diluent and increasing in the course of subsequent dilutions of the agent. The application point are the higher regulatory centers of the body.

The results of treatment with homeopathic remedies in dilutions 10(-400), 10(-2000), 10(-20000) are represented. The informative energetic hologramic conception of homeopathy is proposed. The homeopathic drugs are considered as holograms and the human organic as a biohologram. The resonance is a mode of their interaction with a wide therapeutic effect as a result.

BACKGROUND AND OBJECTIVE: At hospital admission drugs prescribed by the general practitioner (GP) are often changed. This may have a negative impact on the relationship between family and hospital physicians as well as on the family doctor-patient-relationship. The study set out to examine the attitudes of hospital physicians towards GPs' prior ambulatory medication, especially in the case of drugs of unproven efficacy (e.g. certain drug combinations and homeopathic or herbal drugs). PARTICIPANTS AND METHODS: A total of 129 doctors of the surgical and medical wards of the Göttingen University Hospital received a standardized questionnaire focusing on drugs prescribed by referring GPs (response rate: 65.9%). Three case vignettes were presented describing "popular" GP prescriptions. Doctors of surgical and medical departments were asked whether or not they would follow these prescriptions. Differences in the answers between the groups of doctors were tested by Fisher's exact test. RESULTS: More doctors on the surgical than on the medical wards would usually follow GPs' medication (82 vs 25%; P < 0.001). According to these attitudes, more doctors of medical departments would stop the prescription of drug combination (82 vs 41%; P < 0.001); both groups would be hesitant to accept homeopathic drugs (89 vs 59%; P < 0.01) or herbal drugs (89 vs 55%; P < 0.01) as prescribed by the GP. The critical attitude especially of doctors of the medical departments towards drug combinations and herbal drugs was in line with their decision in the case vignettes (e.g. Capozide, Tebonin forte). CONCLUSION: Especially doctors working in medical departments keep in line with conventional clinical pharmacological criteria. If they have to decide whether or not to follow patients' ambulatory medication, they may overlook GPs' decision-making process underlying their prescribing.

37 patients with prostatic adenomas were treated with homoeopathic for 6-9 months in an open study. Most showed subjective improvement in urinary and sexual function and improved objective measures of urinary function. Testosterone levels rose, but there was no reduction in size of the prostate on ultrasound. Possible modes of action are discussed

Paciente con diagnostico de adenoma de prostata de 18 meses de evolucion y de litiasis renal de anos de evolucion. Se receto como medicamento constitucional Nux Vomica 10M y 50M, y Sulphur 50M. En la pagina 425 del Tratado de Materia Medica Homeopatica tomo III el Dr. Vijnovsky dice: "En Sulphur, las caracteristicas generales son mas abundantes e importantes que las mentales. Ademas de los casos en que la similitud de sintomas y caracteristicas con las del paciente lo aconsejan, esta indicado en los siquientes casos: a) Cuando el remedio bien elegido no actua (como se utilizan casi siempre los nosodes), b) Comp accion intercurrente, que permite eliminar parcial o totalmente el obstaculo psorico que se opone a la accion del 'similimun', especialmente en enfermedades agudas, pero no unicamente. Es el principal antipsorico. c) Cuando la convalescencia no llega o es muy prolongada." Si bien es cierto que Nux Vomica habia actuado en el caso, aun se notaba un enlentecimiento en el proceso de curacion. Se aplicaron en la practica las ensenanza del Dr. Vijnovsky del parrafo anterior, con el resultado exitoso del caso; y se receto Sulphur 50M que complemento la accion de Nux Vomica
