RESUMO
Context: Psoriasis is a chronic, proliferative, and inflammatory skin disease related to the systemic manifestation of numerous organs. The typical lesion comprises red, scaly, sharply demarcated, indurated plaques, especially over the extensor surfaces and scalp. The degree and severity vary after some time and between people. Morphological variations are normal; both hereditary and environmental factors play important roles in etiology and pathogenesis.Psoriasis is very difficult to treat due to its chronicity. As a medical system, Homoeopathy treats both patients' physical ailments and overall wellness. The use of highly potentized homoeopathic anti-miasmatic intercurrent medicines will be very beneficial in dealing with this. Fifty-millesimal (LM) potency was a great discovery by Dr. Samuel Hahnemann. While treating numerous chronic illnesses, he was able to quickly relieve symptoms without any aggravation by using fifty-millesimal (LM) potency. Case Report: The present case report is of 24-year-old male patient having psoriasis for one year. This case report describes the homoeopathic management of psoriasis using fifty-millesimal (LM) potency. On the basis of presenting sign and symptoms, Arsenic Album 30C was prescribed with frequent repetitions after repertorisation. During follow-ups, Arsenic Album 1LM was prescribed in to accelerate the cure. Psoriasis is a chronic condition that tends to recur. To prevent the recurrence, Psorinum 1000C was prescribed as an intercurrent anti-miasmatic medicine in a single dose during the course of treatment. Psorinum is a nosode that was chosen for therapy since it is recommended when symptoms reappear and the best selected homoeopathic medicine is not very effective. Psorinum was prescribed in this case to clear the miasmatic obstruction because the Arsenic Album was only temporarily easing the complaints. Result: The patient has exhibited improvement in their mental state and quick alleviation from physical issues after receiving multiple doses of Arsenic Album 1LM and a single dose of Psorinum 1000C, all without their symptoms getting worse. The patient's condition was followed up on at every appointment for a considerable amount of time, and no new concerns had surfaced as of yet. Conclusion: This case report shows favourable results of homoeopathic medicine in fifty-millesimal (LM) potency. Therefore, it can be concluded that using fifty-millesimal (LM) potency of accurately selected homoeopathic medicine will shorten the cure process without the aggravation of symptoms. Further research studies are required to explore the efficacy of fifty-millesimal (LM) potency of homoeopathic medicine in treatment of chronic illnesses.

Introduction: Psoriasis which affects the skin of palm and soles is called as Palmoplantar Psoriasis. The exact cause of it is unknown. It has predominantly hyperkeratotic plaques, thick scaly skin or an overlapping of both of them. Also called as chronic disease which produces functional disability or significant disfigurement and does substantial impairment of person's life. Topical therapy, light therapy, oral medications are part of conventional treatment which offers only a palliative effect. Besides conventional treatment patient choose Homoeopathy because it's a gentle and safe method of treatment that would appear to have positive effects in treatment of Palmoplantar Psoriasis. Summary: A 48 years old Male presented with dry, rough cracked skin with intense itching and burning on both palms and right sole. As he was tabla teacher his day to day activities were hampered. After proper case taking and Repertorization by Kentian approach, Sulphur 1M was prescribed after considering all general and particular characteristic symptoms. The repertory used was SYNTHESIS REPERTORY 8.1 from the Homoeopathic software RADAR. After 3 doses of Sulphur 1M at the gap of 2 months & at the end of 8 months, Re-epithelialisation was seen with an absence of itching and burning and no cracks in skin. This evidence based case report is an example of a successful Homoeopathic Management in cases of Palmoplantar Psoriasis with Kent's approach as Homoeopathy does not treat the disease but the diseased individual.

Introduction: Psoriasis is a chronic inflammatory dermatological condition characterized by well-demarcated erythematous scaly plaques. It affects the immune system of the body causing inflammation and hyper-proliferation of the skin, where a key role is played by immune dysfunction and psychological stress. Psoriasis is a relapsing and remitting disease which shows its affections primarily for skin. This makes it more difficult to treat as there often is a mental maintaining cause associated in the background. Homoeopathy is an ideal system of medicine for the treatment of such diseases which show affections for both physical and mental sphere. Whilst treating such diseases, the homoeopathic physician often faces challenges when the most indicated remedy stops acting after an initial amelioration. This demands the usage of an intercurrent remedy which removes the obstacles in the path of cure and brings about recovery in the patient. Case Summary: A 28-year-old female presented with thick, coppery red eruptions on ear pinnae, scalp, extensor surface of left hand, back and ankles laterally. Based on the totality of symptoms, she was prescribed Staphysagria 1M which initially provided relief to the patient. The case was stood still for several months during which both placebo and Staphysagria 10M were prescribed. Still there was no progress whatsoever, the case was retaken, but the totality and the remedy were same. This was a clear call for prescribing an anti-miasmatic remedy to remove the miasmatic block. The patient was prescribed Psorinum 1M as an intercurrent anti-miasmatic remedy with remarkable physically and mental recovery. Staphysagria 10 M was repeated which later removed all the lesions and restored the patient mentally too.

Psoriasis is a chronic inflammatory disease which has multisystem involvement, predominantly skin and joints. It affects quality of life and leads to psychological distress in patients leading to poor self-esteem and depression, ultimately affecting interpersonal relationships. Homoeopathy believes in a holistic approach and, when prescribed on a constitutional basis, helps to cure the condition from its root cause. A diagnosed case of psoriasis in a 23-year-old male came into the Homoeopathic OPD with complaints of psoriatic patches on the face and scalp. Routine investigations were done to rule out other disorders. In this case, Sulphur was prescribed on a constitutional basis and proved to be beneficial. Considering the extent of psoriasis in this patient, the Homoeopathic approach relieved the case in a short period of time with prolonged treatment response, suggesting that this may represent the ideal cure in Homoeopathy.

BACKGROUND: Employing the secondary action or adaptative reaction of the organism as therapeutic response, homeopathy uses the treatment by similitude (similia similibus curentur) administering to sick individuals the medicines that caused similar symptoms in healthy individuals. Such homeostatic or paradoxical reaction of the organism is scientifically explained through the rebound effect of drugs, which cause worsening of symptoms after withdrawal of several palliative treatments. Despite promoting an improvement in psoriasis at the beginning of the treatment, modern biological therapies provoke worsening of the psoriasis (rebound psoriasis) after discontinuation of drugs. METHOD: Exploratory qualitative review of the literature on the occurrence of the rebound effect with the use of immunomodulatory drugs [T-cell modulating agents and tumor necrosis factor (TNF) inhibitors drugs] in the treatment of psoriasis. RESULTS: Several researches indicate the rebound effect as the mechanism of worsening of psoriasis with the use of efalizumab causing the suspension of its marketing authorization in 2009, in view of some severe cases. Other studies also have demonstrated the occurrence of rebound psoriasis with the use of alefacept, etanercept and infliximab. CONCLUSION: As well as studied in other classes of drugs, the rebound effect of biologic agents supports the principle of similitude (primary action of the drugs followed by secondary action and opposite of the organism).

INTRODUCTION: Psoriasis is a chronic inflammatory skin disorder, affecting the trunk and extensor surfaces of the limbs and scalp predominantly. Worldwide prevalence ranges between 0.1 and 11.4%, and in India between 0.4 and 2.8%; this creates a serious health burden. Psoriasis remains a frequently encountered condition in homeopathy practice, but there is a dearth of conclusive efficacy data supporting its use. METHODS: This 6-month, double-blind, randomized trial was conducted on 51 patients suffering from psoriasis at the National Institute of Homoeopathy, India. Patients were randomized to receive either individualized homeopathic medicines (IHMs; n = 25) in LM potencies or identical-looking placebos (n = 26). Psoriasis area and severity index (PASI; primary), psoriasis disability index (PDI), and dermatological life quality index (DLQI; secondary) were measured at baseline and every 2 months, up to 6 months. The intention-to-treat sample was analyzed using a two-way repeated measure analysis of variance. RESULTS: Although intragroup changes were significant in both groups in the outcome measures, improvements were significantly higher in the IHMs group than in placebos in PASI scores after 6 months of intervention (F1, 49 = 10.448, p = 0.002). DLQI daily activity subscale scores also yielded similar significant results favoring IHMs against placebos after 6 months (F1, 49 = 5.480, p = 0.023). Improvement in PDI total (F1, 49 = 0.063, p = 0.803), DLQI total (F1, 49 = 1.371, p = 0.247), and all remaining subscales were higher in the IHMs group than placebos after 6 months, but nonsignificant statistically. Calcarea carbonica, Mercurius solubilis, Arsenicum album, and Petroleum were the most frequently prescribed medicines. CONCLUSIONS: IHMs exhibited better results than placebos in the treatment of psoriasis. Further research is warranted.

OBJECTIVE: To examine data from studies supporting the clinical efficacy of medical approaches from India traditional systems of medicines like Ayurveda, Unani, Siddha, and Homeopathy for psoriasis using outcome indicators employed in clinical practice and research. METHODS: Searches were conducted between December 2019 and September 2020 in databases PubMed, Scopus, Web of Science and Ovid Medline using search terms including traditional, complementary, psoriasis, Kushtha, Ayurveda, Siddha, Unani, Homeopathy and clinical. Controlled trials, case series and case reports published from India were included. RESULTS: Data of 17 selected studies were extracted. Treatment efficacy in terms of improvement in Psoriasis Area and Severity Index (PASI) score or/and percentage reduction in score (PASI 50, PASI 75 and PASI 90) or/and patient-reported outcomes using instruments like Dermatology Life Quality Index and Psoriasis Disability Index were noted. All studies reported good improvement as per the study specific outcome. However, study characteristics, including study design, sample size, follow-up period, inclusion and exclusion criteria were heterogeneous, and the choice of outcome measures was not adequate to conclude the effectiveness of intervention. The use of some herbs as common ingredients in several formulations across different systems of medicines were noted in analyzing individual formulation. CONCLUSIONS: Future studies must incorporate a comprehensive study design with specific outcome measures like PASI, PASI 75, PASI 90, quality of life parameters, compliance to medications, adverse reactions, remission period, relapse rate and cost-effectiveness with long term follow-up. The currently available evidence on the roles of these herbs at molecular level in psoriasis is preliminary.

Psoriasis is usually treated with local and systemic medications that have varying degrees of efficacy and safety profiles. We investigated the efficacy and safety of an alternative treatment from natural sources, Mahonia aquifolium, for the management of mild to moderate psoriasis. Two hundred subjects participated in a randomized, double-blind, placebo-controlled study using either the topical cream Reliéva (a homeopathic product containing a proprietary M. aquifolium extract) or control (placebo) twice a day for 12 weeks. Efficacy and safety were assessed using the Psoriasis Area Severity Index (PASI) and the Quality of Life Index (QLI) questionnaires at different times throughout the 12-week study. The PASI was evaluated by the physician at the beginning (week 0) and end (week 12) of the study. The QLI was assessed by patients at weeks 0, 4, 8, and 12. The results indicate statistically significant (P < 0.05) improvements in PASI and QLI in the Mahonia-treated group, compared with the control group. The side effects reported were infrequent, < 1% and minor; the most frequent side effects were rash, a burning sensation when applying the cream, and clothing stain. These data indicate that Reliéva, a proprietary form of M. aquifolium, is effective and well tolerated in patients with mild to moderate psoriasis.

A patient suffering from long-standing pustular psoriasis of the palms was treated for 3 weeks with a mercury-containing drug. Exacerbation into generalized pustular psoriasis developed. Mercury levels in blood and urine were increased. After withdrawal of the mercury preparation, therapy with DMPA (2,3-Dimercapto-1-propane-sulfonic acid), a mercury antidote, was initiated, together with short-term treatment with aromatic retinoids and PUVA. Within a few days mercury levels decreased significantly and the skin lesions practically disappeared.
