RESUMO

We have developed a BASIC program for the Apple II microcomputer which can simulate the effect (degree of paralysis) time curve obtained following bolus intravenous administrations of pancuronium. The program is based on a combined pharmacokinetic/pharmacodynamic model and has practical application to the anaesthetist under operating room conditions. Knowing the disease state of the patient and the doses and times of administration of pancuronium the microcomputer can predict the degree of paralysis which exists at any time and so assists in the timing of the next dose of relaxant and in deciding when to effect reversal.

A computer-aided forced oscillation technique was used to examine the effects of halothane on bronchial calibre in three adult cows after anaesthesia had been induced with xylazine and thiopentone. The administration of halothane failed to produce bronchodilatation, possibly owing to low resting bronchomotor tone in the animals. However, an increase in expiratory reserve volume, associated with a small fall in airway resistance, was observed, suggesting that changes in elastic recoil may make a significant contribution to changes in airway resistance during anaesthesia. The results also emphasise the importance of relating airway resistance to lung volume.

Pharmacodynamics of vecuronium have been studied in children 1 to 10 years old. Doses from 0.015 to 0.1 mg/kg were given and the effects measured mechanically and by electromyography. For 0.1 mg/kg the pharmacokinetics were established. A smaller distribution volume and higher plasma clearance in children as compared to adults have been found, while the dose-effect ratio is the same. The action of vecuronium is shorter in children than in adults.

RESUMO

A simple microcomputer program written in Microsoft Basic estimates pharmacokinetic parameters using the coordinate search technique to minimize the sum of squared errors. The program developed for portable computers combines a plot of data and curve fitting so as to find rapidly the initial parameters with the subsequent optimization of the parameter estimate.

In order to assess the course of methadone (Heptadone) substitution therapy, 29 inpatients at the Vienna Psychiatric University Clinic (21 males, mean age = 27 years, SD 4 years; 8 females, mean age 29.75 years, SD 5.28 years) who were addicted to opium tea or to a mixture of opium and heroin were investigated by means of computer-assisted "static"- and "light-evoked dynamic" pupillometry. Pupillary measurements were carried out before the start of withdrawal, on the 2nd day 48 h after the administration of 10 mg methadone, and again after the maximum and half of the maximum dose of methadone had been administered. The constricted pupils (the effect of opiate) showed dilatation after the withdrawal syndrome appeared, but immediately after the start of the detoxification treatment, as well as 1 day after administration of the maximum methadone dose a decrease of pupillary diameter was observed. The narrowing of the pupil was followed by an increase in pupillary diameter, which peaked 48 h after the last minimal dose of methadone and nearly reached the normal level. The widening of the pupil reflects an increase of noradrenergic activity under conditions of opiate withdrawal. An increase of spontaneous fluctuations was observed during withdrawal and was only inhibited by the maximum dose of methadone. Finally, pupillary dynamics (shortening of latency time and increase of relative changes) improved during therapy. The pupillary measurement corresponded with clinical observations as well as with self-evaluation during treatment. Thus pupillometry seems to be a useful instrument for assessment of treatment of opiate-addicted patients.
