ABSTRACT

Introduction: Transplacental infections are frequent, especially in developing countries, where limited screening is performed to find infectious agents in the pregnant population. We aim to determine the clinical and epidemiological characteristics and seroinfection of antibodies against Toxoplasma, parvovirus B19, T. pallidum, and HIV in pregnant women who attended the Motupe Health Center in Lambayeque, Peru during July-August 2018. Methods: A descriptive cross-sectional study was conducted in 179 pregnant women interviewed with a standardized questionnaire. ELISA was used to determine antibodies to Toxoplasma and parvovirus B19. The detection of syphilis and HIV was conducted using immunochromatography, while the detection of hepatitis B was conducted using FTA-ABS and immunofluorescence, respectively. Results: Of 179 pregnant women, syphilis and HIV infections routinely included in the screening of pregnant women presented a seroinfection of 2.2 and 0.6%, respectively. Toxoplasmosis seroinfection was 25.1%, while IgM antiparvovirus B19 was 40.8%, revealing that pregnant women had an active infection at the time of study. Conclusion: The level of seroinfection of toxoplasmosis reveals the risk to which pregnant women who participated in the study are exposed. The high seroinfection of parvovirus B19 could explain the cases of spontaneous abortion and levels of anemia in newborn that have been reported in Motupe, Lambayeque, Peru. However, future causality studies are necessary to determine the significance of these findings.

ABSTRACT

The antithesis between childhood cancer survival rates in low- and middle-income countries (LMIC) and high-income countries (HIC) represents one of healthcare's most significant disparities. In HICs, the 5-year survival rate for children with cancer, including most brain tumors, exceeds 80%. Unfortunately, children in LMICs experience far worse outcomes with 5-year survival rates as low as 20%. To address inequities in the treatment of childhood cancer and disease burden globally, the World Health Organization (WHO) launched the Global Initiative for Childhood Cancer. Within this initiative, pediatric low-grade glioma (LGG) represents a unique opportunity for the neurosurgical community to directly contribute to a paradigm shift in the survival outcomes of children in LMICs, as many of these tumors can be managed with surgical resection alone. In this chapter, we discuss the burden of pediatric LGG and outline actions the neurosurgical community might consider to improve survival for children with LGG in LMICs.

ABSTRACT

OBJECTIVE: To report the registry of the HAE Peruvian patient's association. METHODS: We used the questionnaire of the Latin American HAE committee. Consent was requested from the patient's association to report the data. RESULTS: We report data of 63 patients, 51 Female, 12 Male, range age between 6 to 74 years. Nine under 18 years old, 5/9 between 6 to 13 years. Forty-five HAE C1-INH type I, 12 HAE-FXII, 5 HAE UNK, 1 AAE. Symptoms onset average age in 56/62 HAE patients was 16.8. In a group of 50/62 adult HAE patients, the average diagnostic delay approximately was 19.3 years. Laboratory tests: we can perform C4 complement C1-inhibitor antigenic and functional tests. Treatments: The patients have access to tranexamic acid (TA) and attenuated androgens. We do not have registered specific long-term prophylaxis treatments. We used moderate/high doses of TA, in most patients up to 6 gr i.v./in 24 hours, we start with the treatment immediately the HAE acute crisis is beginning, it helps to the HAE attacks are less symptomatic, resolves in a few days and decrease the frequency. CONCLUSIONS: We present 63 members of the Association of Patients with Hereditary Angioedema of Perú. We have improved blood tests for HAE diagnosis. Moderate and high doses of Tranexamic Acid are used for prophylaxis and acute crisis respectively, with acceptable response. No deaths have been reported due to HAE crisis in the patient's association.


OBJETIVO: Reportar el registro de pacientes de la Asociación de Pacientes con Angioedema Hereditario de Perú, AEH. MÉTODOS: Se utilizó el cuestionario del Comité de AEH, de la Sociedad Latinoamericana de Alergia, Asma e Inmunología (SLAAI). Se solicitó el consentimiento a la Asociación de Pacientes para reportar los datos. RESULTADOS: Se reportan datos de 63 pacientes, 51 mujeres y 12 hombres, en un rango de edad entre 6 y 74 años. Nueve menores de 18 años, 5/9, entre 6 y 13 años. 45 con AEH-C1-INH tipo I, 12 AEH-FXII, 5 AEH-D, 1 AEA. La edad promedio de inicio de síntomas en 56/62 pacientes fue de 16,8. En 50/62 pacientes adultos con AEH, el promedio de tiempo de espera en el diagnóstico fue de 19,3 años. Laboratorio: Se puede desarrollar C4 complemento, C1-Inhibidor antigénico y funcional. Tratamientos: Se cuenta con acceso al ácido tranexámico (AT) y andrógenos atenuados. No se cuenta con tratamientos específicos para profilaxis de largo plazo. Se utilizaron dosis moderadas/altas de (AT), hasta 6 g por I V/ en 24 horas, inmediatamente, al inicio de las crisis de AEH, ayuda a que los ataques no sean tan intensos y tengan menor duración y frecuencia. CONCLUSIONES: Se presentan 63 miembros de la Asociación de Pacientes con Angioedema Hereditario de Perú. Se han mejorado los exámenes sanguíneos para el diagnóstico del AEH. Se utilizaron dosis moderadas/altas de ácido tranexámico con aceptable respuesta en los pacientes. No se han presentado decesos por crisis de AEH en los miembros de la Asociación.

ABSTRACT

SARS-CoV-2 has spread widely throughout the world, and multiple mutations and variants of interest were detected in the late 2020 and continue to emerge. Although genetic diversity is common in RNA viruses, these variations at the genetic level have given it certain characteristics associated with transmission, resistance to neutralizing antibodies, and even suspected increased lethality. Better understanding of the genomic diversity in SARS-CoV-2 will help to take appropriate containment measures against the virus. It should be borne in mind that this diversity can originate anywhere in the world, especially in areas where there is a high number of infections. This highlights the need for continuous molecular surveillance to guide development, therapy, vaccine use and health policy.

ABSTRACT

PURPOSE: Efforts to address inequities in the treatment of pediatric CNS tumors and the burden of childhood cancer globally have prompted the designation of low-grade glioma as one of six index cancers for the World Health Organization Global Initiative for Childhood Cancer. Understanding the importance of neurosurgical interventions and evaluating pediatric neurosurgical capacity may identify critical interventions to improve outcomes for children with low-grade glioma and other CNS tumors. METHODS: An online, cross-sectional survey assessing pediatric neurosurgical practice and capacity was distributed to members of the International Society of Pediatric Neurosurgery. The survey included 36 items covering domains including patient volume, available infrastructure, scope of practice, case distribution, and multidisciplinary care. RESULTS: Responses from 196 individuals from 61 countries, spanning all WHO regions, were included. Ninety-six (49.0%) were from high-income countries, 57 (29.1%) were from upper-middle-income countries, 42 (21.4%) were from lower-middle-income countries (LMICs), and 1 was (0.5%) from a low-income country. Most respondents had a catchment population of ≥ 1 million and indicated the availability of basic neurosurgical resources such as a dedicated neurosurgical operating theater and surgical microscope. The presence of a neurosurgical intensive care unit, inpatient rehabilitation services, and infection monitoring showed similar availability across country groups. Quantitative scoring of 13 infrastructure and service items established that fewer resources were available in low-income countries/LMICs and upper-middle-income countries compared with high-income countries. The volume of pediatric CNS tumor cases and case distribution did not vary according to World Bank country groups. CONCLUSION: This study provides a comprehensive evaluation of pediatric neurosurgical capacity across the globe, establishing variability of resources on the basis of the country income level. Our findings suggest that pediatric neurosurgeons in LMICs may benefit from key neurosurgical instrumentation and increased support for multidisciplinary brain tumor programs and childhood cancer research efforts.

ABSTRACT

Introduction: There is substantial inequity in survival outcomes for pediatric brain tumor patients residing in high-income countries (HICs) compared to low- and middle-income countries (LMICs). To address disparities in pediatric cancer survival, the World Health Organization (WHO) established the Global Initiative for Childhood Cancer (GICC) to expand quality care for children with cancer. Research question: To provide an overview of pediatric neurosurgical capacity and detail the burden of neurosurgical diseases impacting children. Material and methods: A narrative review of the current context of global pediatric neurosurgical capacity as it relates to neurooncology and other diseases relevant to children. Results: In this article, we provide an overview of pediatric neurosurgical capacity and detail the burden of neurosurgical diseases impacting children. We highlight concerted advocacy and legislative efforts aimed at addressing unmet neurosurgical needs in children. Finally, we discuss the potential implications of advocacy efforts on treating pediatric CNS tumors and outline strategies to improve global outcomes for children with brain tumors worldwide in the context of the WHO GICC. Discussion and conclusion: With both global pediatric oncology and neurosurgical initiatives converging on the treatment of pediatric brain tumors, significant strides toward decreasing the burden of pediatric neurosurgical diseases will hopefully be made.

Introducción: Los murciélagos se destacan por ser los únicos mamíferos voladores, con alrededor de 1 400 especies que cumplen un rol fundamental como controladores de plagas y polinizadores de plantas nocturnas. Sin embargo, su influencia sobre la salud humana se ha evidenciado cada vez más, en particular después del surgimiento de brotes epidémicos de enfermedades virales asociadas a estos mamíferos. Objetivo: Analizar la influencia de los murciélagos en la salud humana, centrándose en su papel como portadores de enfermedades virales y su potencial como reservorios y vectores de enfermedades. Métodos: Se realizó una revisión bibliográfica de la literatura utilizando descriptores MeSH y términos como: Animals, Wild Chiroptera, Virus Diseases, Zoonoses, Disease Vectors, Disease Reservoirs, Public Health, bats, Communicable Disease Control, Disease Outbreaks, Prevention and Control. Se revisaron 1 442 artículos en bases de datos y documentos oficiales, se seleccionaron las fuentes relevantes con Mendeley Desktop 1.19.4. y se obtuvieron al final 47 artículos. Resultados: Existen varias especies de murciélagos que pueden afectar la salud del ser humano y que albergan en especial virus de las familias Filoviridae, Coronaviridae y Paramixoviridae. Los murciélagos se consideran incubadoras óptimas para la propagación de virus debido a su sistema inmune único que lo hace resistente a estos agentes infecciosos. Conclusiones: La vigilancia y monitoreo de los murciélagos, junto con acciones de educación pública y una gestión adecuada de sus hábitats, son fundamentales para la detección temprana y prevención de la transmisión de nuevos virus de estos mamíferos a los humanos.


Introduction: Bats are the only flight mammals, with around 1,400 species playing critical roles as pest controllers and nocturnal plant pollinators. However, its impact on human health has become increasingly evident, especially after the appearance of epidemic outbreaks of viral diseases related to these mammals. Objetive: To analyze the influence of bats on human health, focusing on their role as carriers of viral diseases and their potential as reservoirs and vectors of diseases. Methods: A literature bibliographical review was conducted using MeSH descriptors and keywords such as: Animals, Wild Chiroptera, Virus Diseases, Zoonosis, Disease Vectors, Disease Reservoirs, Public Health, bats, Communicable Disease Control, Disease Outbreaks, Prevention and Control. 1442 articles in databases and official documents were reviewed, selecting the relevant sources with Mendeley Desktop 1.19.4., obtaining 47 articles at the end. Results: There are several species of bats that can affect human health and that mainly harbor viruses from the Filoviridae families, Coronaviridae and Paramyxoviridae. Bats are considered optimal incubators for the spread of the virus due to their unique immune system that makes them particularly resistant to these infectious agents. Conclusions: Surveillance and monitoring of bats, together with public education actions and proper management of their habitats, are essential for early detection and prevention of transmission of new viruses from these mammals to humans.

Introducción: Desde el inicio del brote de COVID-19 se han descrito diferencias entre las características clínicas, la evolución y el pronóstico de los pacientes de distintas localidades del Perú y del mundo. Objetivo: Determinar las características clínicas y epidemiológicas de la COVID-19 en pacientes de la comunidad y del Hospital Regional Lambayeque. Métodos: Se realizó un estudio de enfoque cuantitativo, nivel relacional y diseño observacional con datos secundarios. Se empleó la técnica de la documentación, se verificaron los datos recolectados en la ficha estandarizada de investigación clínica epidemiológica de COVID-19 del Ministerio de Salud del Perú. Resultados: De 4 463 pacientes analizados en el 36,4 % se detectó la presencia de SARS-CoV-2 y de éstos la mediana de edad fue de 40 años, siendo el sexo femenino el más frecuente (53,4 %). Del total de participantes, 23,7 % no registraron síntomas, sin embargo, los pacientes que declararon tos, dolor de garganta, malestar, fiebre/escalofrío, disnea, anosmia, ageusia y exudado faríngeo tenían más probabilidades de resultar infectados. De los positivos a la COVID-19, 40,2 % presentaban alguna comorbilidad. Las ocupaciones de policía/militar, ama de casa, estudiante y obrero de construcción civil tenían más probabilidades de dar positivo, mientras que, el personal sanitario tuvo 33 % menos probabilidad de infección. Conclusiones: Casi cuatro de cada 10 participantes tuvieron infección por SARS-CoV-2, existió predominio en los varones y personas en edad económicamente activa. La cuarta parte fueron asintomáticos. La tos, dolor de garganta, malestar y fiebre fueron los síntomas más frecuentes; menos frecuentes fueron la dificultad respiratoria, la anosmia, ageusia y exudado faríngeo, pero representaron mayor probabilidad de infección. Casi dos de cada 10 pacientes presentaron comorbilidad como problemas cardiovasculares, diabetes, asma y obesidad. En cuanto a las ocupaciones, el personal sanitario, policial y ama de casa fueron los grupos más afectados.


Introduction: Since the beginning of the outbreak, differences have been described between the clinical characteristics, evolution and prognosis of patients from different locations in Peru and the world. Objective: To determine the clinical and epidemiological characteristics of COVID-19 in patients from the community and the Lambayeque Regional Hospital. Methods: A study of quantitative approach, relational level and observational design with secondary data was carried out. The documentation technique was used, the data collected in the standardized COVID-19 clinical epidemiological investigation form of the Ministry of Health of Peru was verified. Results: Of 4,463 patients analyzed, 36.4 % had the presence of SARS-CoV-2, and of these the average age was 40 years, with the female sex being the most frequent (53.4 %). Of the total participants, 23.7 % had no symptoms, however, patients who reported cough, sore throat, malaise, fever/chills, dyspnea, anosmia, ageusia, and pharyngeal exudate were more likely to be infected with SARS- CoV-2. Of the positive patients for COVID-19, 40.2 % had some comorbidity. Likewise, the occupations of police/military, housewife, student and civil construction worker were more likely to test positive for COVID-19, while health personnel had a 33 % lower probability of SARS-CoV-2 infection. Conclusions: It is concluded that almost four out of 10 participants had SARS-CoV-2 infection, of which there was a predominance in men and people of economically active age. A quarter were asymptomatic. Cough, sore throat, malaise, and fever were the most frequent symptoms; likewise, respiratory distress, anosmia, ageusia, and pharyngeal exudate were less frequent, but represented a higher probability of infection. Almost two out of 10 patients had comorbidities such as cardiovascular problems, diabetes, asthma, and obesity. Regarding occupations, health personnel, police and housewives were the most affected groups.

El Síndrome de Guillain-Barré (SGB) es un raro trastorno del sistema nervioso, donde el sistema inmunológico del paciente ataca las neuronas periféricas en extremidades, causando debilidad muscular, pérdida de sensibilidad y a veces parálisis total. El origen de este trastorno, ha sido asociado a  respuestas inmunes desencadenadas luego de la  infección por Campylobacter spp.   Sin embargo, cuando no existe una causa evidente de la enfermedad, esta no suele ser investigada debido al mayor interés que requiere el tratamiento. Por ello, la mayoría de los casos  se notifican como origen idiopático.  Entre Enero y Marzo de 2016 a nivel mundial se registraron brotes del SGB en 8  países, vinculados con la emergencia del virus Zika. En Perú, desde finales de 2018 ha reportado con mayor frecuencia brotes de SGB y aunque no se ha confirmado asociación con Zika, el incremento de casos, la extensión geográfica donde se produjeron y las características clínicas de los pacientes afectados, tienen patrones comunes que llevan a sospechar un origen infeccioso principalmente de tipo viral . Por lo tanto, es importante conocer la evidencia científica actual acerca del rol que desempeñan algunos virus en este síndrome, permitiendo ampliar nuestro panorama epidemiológico con nuevas herramientas para hacer frente a esta enfermedad.


Guillain-Barré Syndrome (GBS) is a rare disorder of the nervous system, where the patient's immune system attacks the peripheral nerve cells in the arms and legs, causing muscle weakness, loss of sensation and sometimes total paralysis. The origin of this disorder has been associated with immune responses triggered by post-infection with Campylobacter spp. However, when there is no obvious cause of the disease, it is usually not investigated due to the greater interest in the treatment. Therefore, most cases are reported as idiopathic origin. Between January and March 2016 worldwide, GBS outbreaks were reported in 8 countries, linked to the emergence of the Zika virus. In Peru, since the end of 2018, outbreaks of GBS have been reported more frequently and although no association with Zika has been confirmed, the increase in cases, the geographical extension where they occurred and the clinical characteristics of affected patients, have common patterns that lead to suspect an infectious origin mainly of viral type. Therefore, it is important to know the current scientific evidence about the role that some viruses play in this syndrome, allowing us to expand our epidemiological picture with new tools to address this disease

Introducción: La viruela del simio es una enfermedad zoonótica viral, causada por el poxvirus del mismo nombre; endémica de África central y occidental. Sin embargo, su presencia se ha incrementado en otras regiones del mundo durante la última década en comparación con los 40 años anteriores. Objetivo. Describir y actualizar las características etiológicas, epidemiológicas y clínicas de la viruela del simio, así como discutir el potencial riesgo de diseminación internacional. Material y Métodos. Se realizó una revisión de literatura publicada en los últimos cinco años, utilizando los descriptores Medical Subject Headings. La búsqueda se hizo en las bases de datos MEDLINE / PubMed y LILACS, sin restricciones de idioma. Se excluyeron los estudios duplicados y aquellos que no se ajustaban al objetivo de la investigación, seleccionando 47 artículos. Resultados. El virus de la viruela del simio está emparentado con el de la viruela humana (erradicada en 1979). Se transmite desde un individuo enfermo mediante gotitas de fluido respiratorio, contacto con material de lesión cutánea y fluidos corporales; o mediante fómites contaminados. Después de 6 a 13 días de incubación aparecen los síntomas, semejantes al de la viruela humana: comenzando con un proceso febril agudo y seguido de erupción cutánea en rostro y cuerpo, que cura dejando cicatrices visibles. Conclusiones. Este virus puede ocupar el nicho ecológico dejado por el virus de la viruela humana. Actualmente se ha expandido a Europa y América, por lo que ya no debe considerarse una enfermedad limitada al África, sino una amenaza sanitaria para la salud pública global.


background:Monkeypox is a viral zoonotic disease caused by the poxvirus of the same name, endemic to central and western Africa. However, its presence has increased in other regions of the world during the last decade compared to the previous 40 years.To describe and update the etiologic, Objective.epidemiologic, and clinical features of simian pox, as well as to discuss the potential risk of international spread. . Areview of Material and Methods literature published in the last five years was performed using the descriptors Medical Subject Headings. The search was performed in the MEDLINE/PubMed and LILACS data bases, without language restrictions. Duplicate studies and those that did not fit the research objective were excluded, selecting 47 articles. Results.Simian smallpox virus is related to human smallpox virus (eradicated in 1979). It is transmitted from a sick individual by respiratory fluid droplets, contact with skin lesion material and body fluids; or by contaminated fomites. After 6 to 13 days of incubation, symptoms appear, similar to those of human smallpox: beginning with an acute febrile process and followed by a cutaneous eruption on the face and body, which heals leaving visible scars. This virus may occupy the Conclusions.ecological niche left by the smallpox virus. It has now spread to Europe and America, so it should no longer be considered a disease limited to Africa, but a health threat to global public health

Introducción: Las Enterobacteriaceae productoras de betalactamasas de espectro extendido están presentes en las heces de los individuos de la comunidad. En Perú, la automedicación, el tipo de alimentación, condiciones sanitarias podrían asociarse a esta colonización. Objetivo: determinar la frecuencia de colonización rectal por EP-BLEE en pacientes de consulta externa del Hospital Regional Lambayeque, así como los factores asociados a la misma, durante los meses de julio 2018 a febrero 2019. Material y métodos: 331 pacientes participantes fueron entrevistados, de los cuales se obtuvieron tres muestras seriadas de heces recién emitidas. Las muestras fueron cultivadas en agar McConkey. Las EP-BLEE se confirmaron con la prueba de disco combinado (método americano). Resultados: el 85,8 % de los pacientes estuvieron colonizados por EP-BLEE, siendo Escherichia coli el aislamiento más frecuente (87,7 %). El análisis bivariado asoció el consumo de carne de cerdo (RP=1,15 IC 95%: 1,07 - 1,24), caprino (RP=1,18, IC 95%: 1,10 - 1,25) y el consumo de ensaladas frecuentemente (RP=1,15, IC 95 %: 1,05 - 1,28) con una mayor probabilidad de ser portador rectal de EP-BLEE. La automedicación presentó valores cercanos al límite de validez (p=0,051, RP 1,12, IC 95% 0,98 - 1,26). Conclusiones: Consumir carne de cerdo, caprino y ensaladas aumentan la probabilidad de ser portador de EP-BLEE, mientras que la automedicación podría estar asociada, por lo que es necesario seguir investigando, ya que se desconocen las razones de este hallazgo en pacientes de la comunidad.


Background:Extended-spectrumbeta-lactamase-producing Enterobacteriaceae(EP-BLEE) are present in the feces of individuals in the community. In Peru, self-medication, type of diet and sanitary conditions could be associated with this colonization. Objective:to determine the frequency of rectal colonization by EP-BLEE in outpatients of the "Hospital Regional Lambayeque", as well as the factors associated with it, during the months of July 2018 to February 2019. 331 participating patients Material and methods:were interviewed, and three serial samples of freshly emitted stool were obtained from them. The samples were cultured on McConkey agar. EP-BLEE were confirmed with the combined disc test (American method). 85.8% of patients were colonized by EP-BLEE, and Escherichia coliwas the most frequent isolate (87.7%). Bivariate Results:analysis associated the consumption of pork (RP=1.15, 95% CI: 1.07 - 1.24), goat (RP=1.18, 95% CI: 1.10 - 1.25) and frequent consumption of salads (RP=1.15, 95% CI: 1.05 - 1.28) with a higher probability of being a rectal carrier of EP-BLEE. Self-medication presented values close to the limit of validity (p=0.051, RP1.12, 95% CI 0.98 - 1.26). Consuming pork, goat meat and salads increase the probability Conclusions:of being a carrier of EP-BLEE, while self-medication could be associated, so further research is needed, since the reasons for this finding are unknown.

La actual pandemia producida por el coronavirus 2 del síndrome respiratorio agudo severo (SARS-CoV-2), ha impactado gravemente la economía y el sistema de salud en más de 190 países de todo el mundo en un evento sin precedentes, que desde su inicio ha dado lugar a innumerables informes de casos centrados en las manifestaciones sistémicas y respiratorias potencialmente mortales de la enfermedad. Sin embargo, aún no se conoce por completo el alcance de las posibles manifestaciones neurológicas causadas por este 2 nuevo virus. Comprender la interacción del SARS-CoV-2 con el sistema nervioso es esencial para evaluar las probables consecuencias patológicas a corto y largo plazo. Esta revisión busca reunir y discutir la evidencia sobre la ocurrencia de manifestaciones neurológicas y/o compromiso del sistema nervioso en pacientes infectados con SARS- CoV-2.


The current Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) pandemic has severely impacted the economy and health care system in more than 180 countries around the world in an unprecedented event, which since its inception has resulted in countless case reports focusing on the potentially fatal systemic and respiratory manifestations of the disease. However, the full extent of possible neurological manifestations caused by this new virus is not yet known. Understanding the interaction of SARS-CoV-2 with the nervous system is essential to assessing likely short- and long-term pathologic consequences. This review seeks to gather and discuss evidence on the occurrence of neurological manifestations and/or nervous system involvement in SARS- CoV-2 infected patients.

Introducción: La idea de exclusión competitiva es indiscutible cuando se trata de animales y bacterias que intentan ocupar el mismo nicho ecológico, pero su aplicación a la coinfección viral no es tan sencilla de interpretar. La interferencia viral es un fenómeno en el que un virus suprime competitivamente la replicación de otros virus coinfectantes y es el resultado más común de las coinfecciones virales. Objetivo: Comprender mejor el comportamiento de las infecciones respiratorias concomitantes en escenarios de brotes comunitarios y de forma individual en entornos hospitalarios e individuos con comorbilidades. Métodos: Se realizó una búsqueda de información en las bases de datos MEDLINE / PubMed, SciELO y LILACS. También se consideraron artículos publicados en el repositorio de preimpresión medRxiv y los informes de los Centros para el Control y Prevención de enfermedades de los Estados Unidos de América. Mediante el gestor de referencias Mendeley, se eliminaron los duplicados y aquellos que no se ajustaban al objetivo del estudio, seleccionando 48 artículos para la revisión. Análisis y síntesis de la in formación: En la literatura científica se encontró evidencia que sustenta la exclusión competitiva viral entre virus relacionados que comparten células susceptibles y permisivas. Conclusión: La exclusión competitiva impide que dos virus que comparten rutas de transmisión similares y el mismo órgano diana, infecten no sólo al mismo tiempo, sino que también se propaguen con éxito. Por lo tanto, la sindemia producida por virus que comparten estas características podría ser un evento improbable(AU)


Introduction: The idea of competitive exclusion is undisputed when it comes to animals and bacteria trying to occupy the same ecological niche, but its application to viral coinfection is not so simple to interpret. Viral interference is a phenomenon in which one virus competitively suppresses the replication of other co-infecting viruses and is the most common outcome of viral co-infections. Objective: To better understand the behavior of concomitant respiratory infections in community outbreak settings and individually in hospital settings and individuals with comorbidities. Methods: A search for information was performed in the MEDLINE / PubMed, SciELO and LILACS databases. Articles published in the preprint repository medRxiv and reports from the US Centers for Disease Control and Prevention were also considered. Using the Mendeley reference manager, duplicates and those that did not fit the study objective were eliminated, selecting 48 articles for the review. Analysis and synthesis of information: Evidence supporting viral competitive exclusion between related viruses sharing susceptible and permissive cells was found in the scientific literature. Conclusion: Competitive exclusion prevents two viruses that share similar transmission routes and the same target organ from infecting not only at the same time, but also from spreading successfully. Therefore, syndemia produced by viruses sharing these characteristics could be an unlikely event(AU)

RESUMEN Fundamento: la COVID-19 se ha propagado con rapidez por todo el mundo y los estudios sobre el desarrollo de nuevos medicamentos y vacunas continúan en proceso. El uso de equipos de protección personal, el lavado de manos y el distanciamiento social, a la fecha son las principales medidas de defensa contra la infección. Objetivo: describir la evidencia actual sobre la efectividad del uso de mascarillas de tela frente a las mascarillas quirúrgicas y respiradores, así como la eficacia del uso comunitario de mascarillas para controlar la propagación del SARS-CoV-2. Métodos: se realizó una revisión de la literatura y análisis de la información utilizando los descriptores: COVID-19, SARS-CoV-2, personal protective equipment, masks, efficacy, efficiency, population. A partir de la información obtenida, se realizó una revisión bibliográfica de un total de 1 788 artículos publicados en las bases de datos MEDLINE/PubMed, SciELO y LILACS, sin restricciones de idioma se eliminaron los duplicados y aquellos que no se ajustaban al objetivo del estudio, seleccionando para la revisión 52 artículos. Resultados: existe mucha evidencia sobre el beneficio del uso continuo y masivo de mascarillas como herramienta para mitigar la transmisión de COVID-19 y otras enfermedades respiratorias en la comunidad. Las mascarillas de tela, de seguir las recomendaciones para su adecuada fabricación, ofrecen buena protección y su rendimiento es comparable al de las mascarillas quirúrgicas en la comunidad. Los respiradores deben ser reservados para el personal sanitario en el ámbito nosocomial, ya que ofrecen una apropiada protección en ambientes cerrados y de alta carga viral. Conclusiones: el uso masivo de mascarillas puede complementar las estrategias de contención y prevención frente a enfermedades infecciosas de origen respiratorio incluida la COVID-19. Sin embargo, es necesario realizar más estudios específicos sobre el tema que ayuden a comprender mejor su rendimiento en campo.


ABSTRACT Background: COVID-19 has spread rapidly around the world and studies on the development of new drugs and vaccines are still in progress. Therefore, the use of personal protective equipment, hand washing and social distancing, to date are the main defense measures against infection. Objective: to describe the current evidence on the effectiveness of the use of cloth masks compared to surgical masks and respirators, as well as the efficacy of community use of masks to control the spread of SARS-CoV-2. Methods: a literature review and analysis of the information was carried out using the Medical Subject Headings (MeSH) descriptors linked to free terms: COVID-19, SARS-CoV-2, Personal Protective Equipment, Masks, Efficacy, Efficiency, Population. Based on the information obtained, a bibliographic review of a total of 1,788 articles published in the MEDLINE / PubMed, SciELO and LILACS databases was carried out. Without language restrictions, duplicates and those that did not meet the objective of the study were eliminated from the study, selecting 52 articles for the present review. Results: there is much evidence about the benefit of the continuous and massive use of masks as a tool to mitigate the transmission of COVID-19 and other respiratory diseases in the community. Cloth masks, if they follow the recommendations for their proper manufacture, offer good protection and their performance is comparable to that of surgical masks in the community. Respirators should be reserved for healthcare personnel in the nosocomial setting as they offer appropriate protection in closed environments with a high viral load. Conclusions: the massive use of masks can complement the containment and prevention strategies against infectious diseases of respiratory origin including COVID-19. However, more specific studies are needed on the subject to help better understand its performance in the field.

RESUMEN Fundamento: el virus SARS-CoV-2 es responsable de la segunda pandemia del siglo XXI. Desde su aparición en China a finales de 2019, se asocia a neumonía y considera como un virus respiratorio más. Sin embargo, durante su diseminación global demuestra su capacidad para producir daño a otros órganos con manifestaciones clínicas nunca antes descritas para otros virus respiratorios. Objetivo: describir la evidencia científica que respalde el daño extrapulmonar directo producido por el virus SARS-CoV-2 en etapas tardías de la infección, que apoyan su naturaleza bifásica y distinta frente a otros virus respiratorios. Métodos: se realizó una búsqueda de artículos referente al tema en las bases de datos MEDLINE accedido desde PubMed, SciELO y LILACS. También se tuvieron en cuenta artículos publicados en los repositorios de preimpresión como medRxiv, BioRxiv. Mediante el gestor de búsqueda y administrador de referencias Mendeley, se eliminaron los duplicados y aquellos que no se ajustaban al objetivo del estudio, se seleccionaron 63 artículos para la revisión. Resultados: la evidencia sugiere que el SARS-CoV-2 tiene tropismo no solo limitado a las vías respiratorias. La progresión clínica de la COVID-19 presenta un curso bifásico, con manifestaciones de tipo gripal en la primera fase y episodios postagudos y persistentes en la fase tardía, ocasionados por el daño directo al sistema nervioso central, cardiovascular, endocrino y renal. Conclusiones: la infección por SARS-CoV-2 no debe considerarse solo como una infección aguda y circunscrita a las vías respiratorias.


ABSTRACT Background: the SARS-CoV-2 virus is responsible for the second pandemic of the 21st century. Since its appearance in China at the end of 2019, it has been associated with pneumonia and considered to be just another respiratory virus. However, during its global spread, it shows its ability to damage other organs with clinical manifestations never before described for other respiratory viruses. Objective: to describe the scientific evidence that supports the direct extra-pulmonary damage produced by the SARS-CoV-2 virus in late stages of infection, which supports its biphasic nature and different from other respiratory viruses. Methods: a search of the articles was carried out in the MEDLINE databases accessed from PubMed, SciELO and LILACS. Articles published in prepress repositories such as medRxiv, BioRxiv were also taken into account. Using the Mendeley reference manager and search manager, duplicates and those that did not meet the objective of the study were eliminated, selecting 63 articles for the present review. Results: the evidence suggests that SARS-CoV-2 has a tropism not only limited to the respiratory tract. The clinical progression of COVID-19 presents a biphasic course, with flu-like manifestations in the first phase and post-acute and persistent episodes in the late phase, caused by direct damage to the central nervous, cardiovascular, endocrine and renal systems. Conclusions: SARS-CoV-2 infection should not be considered only as an acute infection limited to the respiratory tract.

RESUMEN La capacidad de propagación y letalidad del SARS-CoV-2 en todo el mundo motiva la urgente necesidad de desarrollar una estrategia terapéutica apropiada para controlar los casos de COVID-19. El desarrollo de nuevos fármacos frente a este nuevo virus es apremiante debido a su rápida diseminación. Se han propuesto alternativas paralelas empleando fármacos ya disponibles para fines similares. Esta revisión describe el potencial antiviral de la ivermectina, así como sus mecanismos de acción frente a algunos virus, y discute su probable aplicación contra el SARS-CoV-2.


ABSTRACT The global spread and lethality of SARS-CoV-2 prompt the urgent need to develop an appropriate therapeutic strategy to control COVID-19 cases. The development of new drugs to fight this novel virus is urgent due to its rapid spread. Parallel alternatives have been proposed by using drugs already available for similar purposes. This review article describes the antiviral potential of ivermectin as well as its mechanisms of action against some viruses, and discusses its probable use to fight SARS-CoV-2.

Introducción: La variabilidad genética del SARS-CoV-2 ha incrementado notablemente desde que se declaró la pandemia, lo que le ha permitido representar un continuo desafió para las políticas de salud destinadas a su control. Objetivo. Describir la nomenclatura genómica utilizada para la comunicación general y científica sobre el SARS-CoV-2, así como describir las mutaciones, evolución, origen y variantes del virus. Material y Metodos. Se realizó una revisión narrativa de literatura, para el cual se realizó una búsqueda y análisis de la información hasta el 15 de diciembre de 2021. Se revisaron 74 fuentes seleccionados desde las bases de datos MEDLINE/ PubMed, SciELO, LILACS y páginas web oficiales; sin restricciones de idioma. Resultados. Las mutaciones son cambios en la secuencia de nucleótidos del genoma viral, que, al producir afectación de la dinámica epidemiológica en una población, dan lugar a variantes, y estos a su vez a clados diferenciados. Entre las variantes de interés destacan Lambda y Mu, identificadas por primera vez en Perú y Colombia, respectivamente. Mientras que, las variantes de preocupación, en orden cronológico, son la Alfa (británica), Beta (sudafricana), Gamma (brasilera), Delta (india) y recientemente Ómicron. Conclusiones. Se concluye que la diversidad genómica del SARS-CoV-2 se debe a su elevada tasa de mutaciones que pueden constituirse en variantes y clados. La mejor comprensión de esta diversidad permite tomar medidas de control más eficaces, guiando el desarrollo y uso de vacunas, terapias, diagnóstico y políticas en salud.


Introduction: The genetic variability of SARS-CoV-2 has increased markedly since the pandemic was declared, allowing it to pose a continuing challenge to health policies aimed at its control. Objective. To describe the genomic nomenclature used for general and scientific communication about SARS-CoV-2, as well as to describe the mutations, evolution, origin and variants of the virus. Material and Methods. A narrative literature review was conducted, for which a search and analysis of the information was performed until December 15, 2021. Seventy-four selected sources were reviewed from MEDLINE/PubMed, SciELO, LILACS databases and official web pages; without language restrictions. Results. Mutations are changes in the nucleotide sequence of the viral genome, which, by affecting the epidemiological dynamics in a population, give rise to variants, and these in turn to differentiated clades. Among the variants of interest are Lambda and Mu, identified for the first time in Peru and Colombia, respectively. Meanwhile, the variants of concern, in chronological order, are Alpha (British), Beta (South African), Gamma (Brazilian), Delta (Indian) and recently Omicron. Conclusions. It is concluded that the genomic diversity of SARS-CoV-2 is due to its high rate of mutations that can constitute variants and clades. A better understanding of this diversity allows more effective control measures to be taken, guiding the development and use of vaccines, therapies, diagnostics and health policies.

Tras su erradicación en la región de las Américas en el 2016, ha reemergido y el número de casos va en progresivo aumento. Objetivo: Profundizar y actualizar los aspectos más importantes de la epidermiología molecular del virus sarampión en las Américas. Métodos: La búsqueda y análisis de la información se realizó en un periodo de cinco meses (primero de noviembre de 2019 al 31 de marzo de 2020) para lo cual se emplearon las siguientes palabras: measles, epidemiology molecular, América,outbreak, genotype, epidemic, en las bases de datos PubMed, Hinari, SciELO y Medline. Así mismo, se tomaron en cuenta los informes epidemiológicos de la Organización Panamericana De La Salud (OPS)y entidades gubernamentales de distintos países de América. Resultados: Dos linajes del genotipo D8están diseminándose ampliamente en la región de las Américas. Y aunque aún no podemos conocer el impacto de la actual pandemia producida por el SARS-CoV-2, la baja tasa de inmunización, los elevados movimientos migratorios antes del 2020, factores socioculturales y religiosos sumados a la crisis socia y política que afectan a algunos países de la región, están contribuyendo a que este problema sea creciente. Conclusión: La revisión brinda el conocimiento de la epidemiología molecular del virus. Su empleo y correcta interpretación permitirá establecer un adecuado manejo y medidas de contención con el fin de recuperar la condición de enfermedad erradicada en las Américas.


Introduction: Measles is one of the most contagious diseases that affect humans. After its eradicationin the Americas region in 2016, it has reemerged and the number of cases is progressively increasing..Objective: To deepen and update the most important aspects of the measles virus molecularepidermiology in the Americas. Methods: The search and analysis of the information was carriedout over a period of five months (November 1, 2019 to March 31, 2020) for which the following wordswere used: measles, molecular epidemiology, America, outbreak, genotype, epidemic, in the PubMed,Hinari, SciELO and Medline databases Likewise, the epidemiological reports of the Pan American HealthOrganization (PAHO) and government entities from different countries of America were taken intoaccount. Results: Two lineages of the D8 genotype are spreading widely in the Americas region. Andalthough we still cannot know the impact of the current pandemic produced by the SARS-CoV-2, thelow immunization rate, the high migratory movements before 2020, socio-cultural and religious factorsadded to the social and political crisis that affect to some countries in the region, they are helping toincrease this problem. Conclusion: The review provides knowledge of the molecular epidemiologyof the virus. Its use and correct interpretation will allow establishing adequate management andcontainment measures in order to recover the eradicated disease condition in the Americas.

RESUMEN Los persistentes bacterianos son variantes transitorias de una población genéticamente homogénea, generada por exposición al estrés, como el que ocurre durante el tratamiento antibiótico. Es un fenómeno epigenético o un fenotipo no heredado, que puede ser llamado primera línea de defensa antes de que se adquiera la resistencia antimicrobiana. A pesar de su descubrimiento hace más de 70 años, su definición, mecanismos de formación, clasificación y morfologías adoptadas de implicancia clínica son temas de investigación actual. En el presente estudio se describe la relación de persistentes con infecciones crónicas y formación de biopelículas como factores importantes en la recaída, recidivas y mayor virulencia en las infecciones. Así mismo, se hace una revisión breve de los diversos mecanismos implicados en la persistencia bacteriana y su eliminación ineficaz por tolerancia antibiótica para terminar con la presentación de posibles estrategias de tratamiento. En conjunto, se cree que la persistencia impone una carga significativa de atención en salud pública, que se estima, provocará hasta 10 millones de víctimas al año para el 2050. Una mejor comprensión de este fenotipo es fundamental en la lucha contra las bacterias patógenas con la finalidad de obtener una mejor perspectiva en las terapias futuras.


ABSTRACT Persistent bacteria are the transient variants of a genetically homogeneous population generated by exposure to stress as in antibiotic treatment. They are an epigenetic phenomenon or a non-inherited phenotype, which may be considered as the first line of defense before developing antimicrobial resistance. Despite their discovery more than 70 years ago, their definition, mechanisms of formation, classification and morphologies of clinical implication are still current research topics. In the present research, we describe the relationship between chronic persistent infections and the formation of biofilms as important factors in recurrences, relapses and greater virulence in infections. Likewise, a brief review of the various mechanisms involved in bacterial persistence, their ineffective elimination due to antibiotic tolerance and possible treatment strategies is provided. Overall, it is believed that persistence poses a significant burden of public health care. It is estimated that up to 10 million people will be yearly affected by 2050. Thus, a better comprehension of this bacterial phenotype is essential to fight against pathogenic bacteria and improve therapeutic results in the future.
