ABSTRACT

Our study concerns 20 patients affected by "Sick Sinus Syndrome", these has been observed in our Coronary Care Unit during the period 1-7-1976 to 26-4-1979. All patients has been submitted to an electrophysiological study of the conduction system; of those only 16 has been treated with a permanent PMK, the other 4 with medical therapy. Our study's results underline the problematic nature of the "Sick Sinus Syndrome" and underline its etiopathogenetic, diagnostic and therapeutic aspects.

ABSTRACT

Admissions to our Coronary Care Unit between 1975 and 1978 were studied. During this period 234 patients with myocardial infarction were admitted and complete A/V block in 50 (21,37%) of them. From our study, 7 patients with transient first degree heart block were excluded. In 23 (53,49%) patients a complete heart block developed within 24 hours of admission. Of the 8 patients with infarction and block who died (16%) 5 patients had an anterior wall myocardial infarction, of which had a PMK treatment; the others 3 cases had an inferior or posterior location and did not need a PMK treatment. We Confirm that high mortality in patients with anterior all myocardial infarction complicated by block is essentially due to the haemodynamic complications and we discuss the indication and the utility of the precautionary stimulation.

ABSTRACT

Authors retain, on the basis of their experiences, that right chest leads proposed by Di Maria for the infer-posterior and postero-basic myocardium wall study can overall be useful in those cases which had posterior myocardial infarction and where standard leads did not show variations or showed untrustworthy variations. Therefore Authors also supported by vectorcardiographic findings, recommend the routine use of Di Maria leads.

ABSTRACT

After our study on 72 patients over 70 years, admitted to a coronary care unit since january 1975 to december 1977 and compared with a control group of 119 patients under 70 years, we found that the hospital mortality rate was of 32% for the geriatric subjects and of 9,3% for the control group. Women over 70 years had a higher mortality: 14 (over 72) to 3 (over 119). The AMI was the more diffused, with a death percent rate higher (40%) than infarctions elsewhere localized. Between death causes, cardiogenic shock often occurred in AMI (69,5%). From a follow-up carried out in january 1978 it resulted that 57% of the 49 geriatric dismissed patients 28 survived and 15 (30,5%) died.
