Recognition of injuries on victims of vehicular fatalities should be performed with the help of means of precise instrumental diagnosis on the basis of information about the etiology of the injury which should be contained in the medical documents.

The cause of death of victims of traffic accidents is thought to be, as a rule, an associated injury of the skull and brain, locomotor system and inner organs followed by blood loss and shock and complicated by pneumonia and pyo-septic processes.

Clinical investigations performed in 735 patients have shown that in addition to the improvement of organisation of the medical aid and the universally recognized resuscitation measures for trauma shock, the prophylactics and treatment of intoxication, metabolic, immune and other disorders developing in the patients soon after trauma should be started as early as at the prehospital step.

A detailed discussion is provided of professional selection issues with regard to emergency care physicians and feldshers, their postgraduate training and certification. It is emphasized that advanced training and specialization of emergency care physicians and feldshers can be carried out not only at institutes and chairs of emergency care but also at multidisciplinary hospitals with compulsory involvement of emergency care specialists in teaching.

In Leningrad the reduction of a number of specialized teams of emergency medical aid has been achieved due to their replacement by intensive care teams (ICT). The technique for calculating economic efficiency of the above measure is described. The list of cycles and their sequence during training and continuous education of ICT physicians is given.
