ABSTRACT

Quality control of medical care and safety of medical practice in the military medical organizations of the Ministry of Defence of Russia is one of the priorities. Guidelines set by The plan for improvement of effectiveness of health protection in servicemen and certain categories of citizens, which is approved by the relevant order of the Minister of Defence of the Russian Federation, can be regarded as a criteria for quality assessment of medical care. The internal quality control and safety of medical practice is a multilevel system of making and implementing. management decisions aimed at ensuring the rights of patients to receive quality medical care, as.well as provides a continuous internal audit of medical diagnostic work-military medical organization. The overriding objective is the use of technical means during all forms of internal control. . ..

ABSTRACT

Description of clinical observation and literature review. Myocardial infarction in patients younger than 45 years is rare, but it is an important clinical, organizational and psychological problem. A case of myocardial infarction in 19-years old patient, who suffered since 6 years from kidney disease, is described. Transmural left-ventricular myocardial infarction has developed on the background of chronic glomerulonephritis, excessive exercise, and traditional risk factors for cardiovascular disease. Coronary venous bypass with the benefit-pleasing outcome is performed. When analysing the literature, the authors emphasize that in comparison with elderly patients, young people have different profiles of risk factors, clinical manfestations and prognosis of myocardial infarction. It is emphasized that kidney chronic disease, regardless the stage, worsen short-term and long-term outcomes of cardiovascular disease. Early stabilization is possible under the condition of risk stratification and-early revascularization, which leads to better clinical outcomes. Particular attention should be given to a comprehensive assessment, it prognostic criteria, risk factor modification, secondary prevention of major and associated diseases, clinical- and -dynamic observation, including patients with asymptomatic course of the disease.

ABSTRACT

The article covers organizational aspects of development of innovative technologies in the field of regenerative medicine. It is shown that for the effective design and implementation into medical practice of regenerative medicine requires a united complex of military health care, military medical research and education. The main goal is to formate a biological insurance of personnel to treat different consequences of radiological incidents, burn disease, identification of the remains of the victims; the maximum returning to action after disturbed as a result of health services. Proposes the creation of "Interdepartmental Clinical Research and Education Center for Regenerative Medicine", combining research, clinical, industrial and educational potential of the leading institutions of various departments that will enhance the national security of the Russian Federation.

ABSTRACT

Authors consider the problem of delivery cardiac surgical care to contingent of the Defence Ministry. Perspective directions of development of cardiac surgery in the Armed Forces of the Russian Federation are the development of minimally invasive cardio surgery, endovascular development of modern methods of diagnosis and treatment, further development of electrophysiological methods for diagnosis and treatment of disorders of rhythm and conduction, the introduction of various kinds of auxiliary mechanical circulatory support systems in acute and chronic heart failure, development of transplantation in cardiac surgery, improvement of algorithm selection and referral of patients requiring cardiac care by providing primary health care to troop central military medical institutions, creating a single register of cardiac patients as part of the Armed Forces in order to determine the order and place of treatment, etc.

ABSTRACT

Chronic kidney disease (CKD) is independent factor of progression of ischemic heart disease (IHD). In that case, it is important to know the peculiarities of diagnosis and treatment of IHD in patients with CKD. The importance of this problem is increasing due to increase of patients with CKD. Cardio-vascular morbidity in patients with CKD is higher than in patients with other diseases. Myocardial revasculization may lead to significant improvement of survivability in patients with CKD. Among the prevention and treatment facilities the Vishnevsky Central Military Clinical Hospital N 3 is a leader in preforming the myocardial revasculization. The authors gave an example of successful coronary artery bypass surgery in patient with IHD and end-stage of CKD. Development of modem medical technologies, systematization of experience and knowledge allow in XXI century to consider CKD as non-absolute contradiction to elective surgery in case of different forms of IHD, including the end-stage of CKD (dialysis treatment). It is advisable to perform the myocardial revasculization in patients with the end-stage of CKD in specialized multi-field hospital, where can provided not only cardiosurgical and cardiological treatment but also specialized treatment for neurological patients, including haemodialysis and hemofiltration.

ABSTRACT

The researches of biological age and rates of aging of liquidators of the sequels to radiation accidents were carried out. The biological age (BA) index can serve as a characteristic of social-hygienic factors influence on state of health of the officers and veterans of special risk subdivisions, liquidators of the sequels to radiation accidents. Chronic diseases exert influence on the biological age and proper biological age (PBA) index, increase mean value of BA-PBA index, bring contribution into acceleration of human organism aging. A direct connection between increase of functional class of the BA and duration of service in the Navy and ships was revealed. It was ascertained that radiation influence was a factor that increased the BA and rates of aging of liquidators of the sequels to radiation accidents.
