ABSTRACT

The outcomes of traditional epineural, microsurgical epineural and interfascicular sutures of the median and ulnar nerves in the lower third of the forearm were compared in children. Evidence is provided for the differential approach to applying an interfascicular and microsurgical epineural suture, as well as for limitations in the use of an interfascicular suture. The authors show it necessary to make investigations to clarify whether the fascicles on the ends of a diseased nerve may be more validly identified and whether the studies into connective and nerve tissue relationships in the suture line.

ABSTRACT

The grounds for expediency of pancreatic resection in various periods of the necrotic pancreatitis clinical course are presented together with the description of a simplier technique of a left-side resection of the gland combined with spleenectomy. The left-side resection of the pancreas was carried out in 74 cases, pancreatoduodenal resection--in 1 patient. 4 patients died.

ABSTRACT

In acute and chronic experiments on 52 animals (rabbits and dogs), carried out by means of neurophysiologic and neurohistologic methods, the authors have studied functional and histologic changes in the nerves following a one-step forced traction. Traction injuries have been found to cause early trophic disorders, persistent and long-lasting alterations of the nerve conductability and excitability. Some considerable retrograde changes in the nerves and related segments of the spinal cord have been noted together with the delay of the resorption of the decay products of the axons and the myelin sheath in the peripheral nerve fragment. The severity degree of the alterations depends on the degree of the traction.

ABSTRACT

Results of the investigation of immunological status in 62 patients with destructive pancreatitis complicated by hepatic insufficiency are presented. An extremely severe degree of immunodepression in the given contingent of patients was shown. It was established that the degree of inhibition of the cellular and humoral immunity in patients with destructive pancreatitis had direct relation to the degree of the functional insufficiency of the liver. A conclusion was made about the necessity to include immunocorrecting measures in the complex therapy.

ABSTRACT

The characteristics of specific features of acute and chronic processes in the caudal pancreatis and surrounding retroperitoneal fat is given on the basis of an analysis of 273 cases of pancreatitis with the predominant lesion of this portion. It was established that acute caudal pancreonecrosis at an early period had atypical symptoms and more favourable course than that of the proximal parts of the pancreas. Later periods of the development of this variant of the disease are characterized by great frequency and spread of pyo-necrotic lesion of the retroperitoneal fat. Specific features of the clinical course, diagnosis and treatment were formulated and their being taken into account allowed to reduce lethality of patients with caudal pancreatitis from 39.0% to 25.0%.
