Implications of ICD-10 for Sarcopenia Clinical Practice and Clinical Trials: Report by the International Conference on Frailty and Sarcopenia Research Task Force.

Establishment of an ICD-10-CM code for sarcopenia in 2016 was an important step towards reaching international consensus on the need for a nosological framework of age-related skeletal muscle decline. The International Conference on Frailty and Sarcopenia Research Task Force met in April 2017 to discuss the meaning, significance, and barriers to the implementation of the new code as well as strategies to accelerate development of new therapies. Analyses by the Sarcopenia Definitions and Outcomes Consortium are underway to develop quantitative definitions of sarcopenia. A consensus conference is planned to evaluate this analysis. The Task Force also discussed lessons learned from sarcopenia trials that could be applied to future trials, as well as lessons from the osteoporosis field, a clinical condition with many constructs similar to sarcopenia and for which ad hoc treatments have been developed and approved by regulatory agencies.

Characterization of microbial cellulose from a high-producing mutagenized Acetobacter pasteurianus strain.

A wild-type Acetobacter pasteurianus was subjected to chemical mutagenesis for the induction and isolation of a cellulose overproducing strain. A mutagenized strain capable of synthesizing double amounts of cellulose compared to the wild type was obtained. Cellulose, both from the wild-type and the mutagenized strain, was extracted and purified for chemical characterization and investigation of its physico-chemical properties. The comparison of the two microbial polysaccharides shows that the putative mutation of A. pasteurianus strain had no effect on some cellulose features such as chemical structure, polymorphic form, crystallinity.

From adaptive licensing to adaptive pathways: delivering a flexible life-span approach to bring new drugs to patients.

The concept of adaptive licensing (AL) has met with considerable interest. Yet some remain skeptical about its feasibility. Others argue that the focus and name of AL should be broadened. Against this background of ongoing debate, we examine the environmental changes that will likely make adaptive pathways the preferred approach in the future. The key drivers include: growing patient demand for timely access to promising therapies, emerging science leading to fragmentation of treatment populations, rising payer influence on product accessibility, and pressure on pharma/investors to ensure sustainability of drug development. We also discuss a number of environmental changes that will enable an adaptive paradigm. A life-span approach to bringing innovation to patients is expected to help address the perceived access vs. evidence trade-off, help de-risk drug development, and lead to better outcomes for patients.

Early development of patella infera after knee fractures.

The condition referred to as patella infera is characterized by a permanent shortening of the patellar ligament and is associated with a severely limited range of motion of the knee joint. Patella infera is a common complication of injury or surgery to the knee joint, and it is generally considered a condition that presents after a variable, albeit considerable time after injury. A review of the knee roentgenograms of patients with arthrofibrosis and patella infera showed this condition to be an immediate complication of knee fractures. Therefore, in order to better define the development of patella infera, the patellar height was studied during treatment of 146 knee joint fractures (46 supracondylar fractures, 50 patellar fractures and 50 tibial plateau fractures). Using the Caton-Deschamps method patella infera was detected in eight cases (17.39%) immediately after supracondylar fractures, in six cases (12%) after patellar fractures, and only in two cases (4%) after tibial plateau fractures. The patellar ratio remained unchanged or worsened further after treatment, and its incidence was unaffected by the type of treatment. These observations rule out etiologies such as inflammatory or algodystrophic phenomena and quadriceps inhibition, because the reduction of patellar height was evident immediately and persisted at follow-up examinations.
