ABSTRACT

OBJECTIVE: To explore the effect and mechanism of surround needling combined with acupoint injection on acute herpetic neuralgia (AHN). METHODS: Ninety-nine patients with T6-T10 segment AHN were randomly divided into 3 groups, 33 cases in each group, including 2 cases dropped off in the surround needling group, 4 cases dropped off in the acupoint injection group, and 3 cases dropped off in the combined group. Oral valacyclovir was given in each group, 0.3 g each time, 2 times a day for 10 days. Oblique insertion of needle used at ashi points around the herpes in the surround needling group, and continuous wave was stimulated to tolerance for 20 min; the same acupoints were selected as the surround needling group, stimulated with the mixture injection of mecobalamin and lidocaine in the acupoint injection group; After the surround needling, acupoint injection was performed in the combined group. The treatment was given once a day, 14 times for a course, and one course was needed in all groups. The skin healing conditions (blistering, crusting, and dislocation time) of each group were compared after treatment. The pain scores, pain area and quality of life scores in each group were observed before and after treatment. The levels of neuron specific enolase (NSE), substance P (SP) and calcitonin gene-related peptide (CGRP) in the local blister fluid were measured before and after treatment in all groups. RESULTS: The blistering, crusting and dislocation time in the combined group were earlier than the other two groups (all P<0.05). The pain score and pain area in the each group were significantly lower than those before treatment, and the quality of life score was significantly higher than that before treatment (all P<0.05). The improvements of pain score and quality of life score in the combined group were more obvious than the other two groups (all P<0.05). After treatment, the levels of NSE, SP and CGRP in the local blister fluid in each groups were significantly lower than those before treatment (all P<0.05). The indexes in the combined group were significantly lower than those in the other two groups (all P<0.05). CONCLUSION: Both surround needling and acupoint injection have an adjuvant effect on AHN. The combination of the two is better, the skin is healed quickly, the analgesia is significant, and the contents of local NSE, SP and CGRP are significantly decreased. The mechanism of action is to exert neuroprotective effects.

ABSTRACT

OBJECTIVE: In the initial assessment of thyroid nodules, thyrotropin (TSH) has very low sensitivity for assessing functional thyroid nodules (FTNs). The false negativity in FTNs and the false positivity in non-FTNs misinterpreted by TSH will raise unnecessary assessment costs. Therefore, the aim of this study is to explore the values of the TSH and color flow Doppler sonography (CFDS) combined strategies in reducing the unnecessary assessment costs. METHODS: 2383 patients with thyroid nodules were retrospectively analyzed, including 107 FTNs and 2276 non-FTNs. Four strategies including TSH, CFDS, Combination 1 (TSH+/CFDS+, TSH+/CFDS-, and TSH-/CFDS+ defined as positive; TSH-/CFDS- defined as negative) and Combination 2 (TSH+/CFDS+ defined as positive; TSH+/CFDS-, TSH-/CFDS+, and TSH-/CFDS- defined as negative) were separately used for initial assessment. The four strategies were compared using the testing cost ratio of fine-needle aspiration (FNA) to thyroid scintigraphy (TS) (marked as CFNA/TS) as main outcome measure. RESULTS: Compared with TSH, Combination 1 prevented 15.89 % of FTNs from unnecessary FNA, but increased the number of non-FTNs subjected to unnecessary 99mTc-TS by 9.31 %. Combination 2 prevented 5.32 % of non-FTNs from unnecessary TS, but increased the number of FTNs subjected to unnecessary FNA by 18.69 %. When CFNA/TS was <6.05, the lowest total cost was found in Combination 2. The TSH and Combination 1 were optimal at 6.05 ≤ CFNA/TS ≤ 12.47 and CFNA/TS > 12.47, respectively. CONCLUSIONS: The combined strategies can be used to supplement TSH in the initial assessment of thyroid nodules in iodine-adequate areas, depending on the testing costs of FNA and TS.

ABSTRACT

Although Berberine (BER) is popular in treating gastrointestinal (GI) disorders, its mechanisms are not clear yet. In order to investigate the effects and possible mechanism of BER on GI motility in rodents, we first explored GI motility by recording the myoelectrical activity of jejunum and colon in rats, and upper GI transit with a charcoal marker in mice. Then, the plasma levels of gastrin, motilin, somatostatin and glucagon-like-peptide-1 (Glp-1) were measured by ELISA or radioimmunoassay (RIA). Furthermore, endogenous opioid-peptides (ß-endorphin, dynorphin-A, met-enkephalin) were detected by RIA after treatment with BER. Our results showed that BER concentration-dependently inhibited myoelectrical activity and GI transit, which can be antagonized by opioid-receptor antagonists to different extents. The elevated somatostatin and Glp-1, and decreased gastrin and motilin in plasma, which were caused by BER application, also could be antagonized by the opioid-receptor antagonists. Additionally, plasma level of ß-endorphin, but not dynorphin-A and met-enkephalin, was increased by applying BER. Taken together, these studies show that BER plays inhibiting roles on GI motility and up-regulating roles on somatostatin, Glp-1 and down-regulating roles on gastrin, motilin. The pharmacological mechanisms of BER on GI motility and plasma levels of GI hormones were discovered to be closely related to endogenous opioid system.
