ABSTRACT

OBJECTIVE: To evaluate the functional efficacy of the superficial circumflex iliac artery perforator (SCIP) -lymphatic pedicled flap in the treatment of advanced male genital lymphedema. METHODS: From February 2018 to January 2022, we treated 26 male patients with scrotal and penoscrotal advanced lymphedema using reconstructive lymphatic surgery. Isolated scrotal involvement was seen in 15 patients, and 11 patients had penoscrotal involvement. Excision of genital lymphedematous fibrotic tissue was followed by reconstruction utilizing the SCIP-lymphatic flap. Patient characteristics, intraoperative data, and postoperative results were evaluated. RESULTS: The mean patient age was 39 ± 4.6, and the mean follow-up time was 44.9 months. the SCIP-lymphatic flap was used to reconstruct partial (n = 11) or total (n = 15) scrotum and used to reconstruct total penile skin in 9 cases and partial in 2 cases. The flap survival rate was 100%. Cellulitis rates were dramatically reduced after reconstruction (P value <.001). The mean genital lymphedema score (GLS) after surgery was 0.05, which was significantly lower than the preoperative 1, 6.2 (P < .001). The median Glasgow Benefit Inventory (GBI) total score was +41, all 26 patients (100%) showed a degree of quality of life improvement. CONCLUSION: The pedicled SCIP lymphatic transfer approach in advanced male genital lymphedema can provide a durable complete functional lymphatic system that improves the appearance and genital lymphatic drainage. This leads to an improvement in quality of life as well as in sexual functions.

ABSTRACT

BACKGROUND: Konjac glucomannan (KGM) showed a synergistic interaction with κ-carrageenan (CAR), which led to the formation of a promising compound hydrocolloid gel in the food field (such as jelly). Nevertheless, the mixed gels formed by adding KGM to CAR still displayed defects in gel strength and syneresis, and would hardly meet the quality requirements of some gel foods. However, deacetylated KGM and maltodextrin (MD) have always been used in gel foods and affect their viscosity and rheological properties. RESULTS: In our paper, different amounts of MD were first used to alter the textural properties, and the results showed that both tensile strength and elongation exhibited first an increasing and then a decreasing trend with the increasing MD proportion and achieved a maximum at a final maltodextrin proportion of 4 g kg-1 in the KGM/CAR/MD system. Based on the above results, we further explored the effects of deacetylation degree of KGM on the gel properties of mixed gel system. The results revealed that, compared to the native KGM, the partial deacetylated KGM was capable of significantly improving the tensile strength and elongation of KGM/CAR mixed gel. CONCLUSION: Our study found that the appropriate addition of MD (0.4%) and DKGM were able to alter the tensile properties of KGM/CAR mixed gel, with potential to meet the needs of consumers and further design innovative tensile gel products in the soft gel industry. © 2021 Society of Chemical Industry.

ABSTRACT

The chitosan chains were integrated with MCM-48 mesoporous silica in an eco-friendly composite (CH/MCM-48) of enhanced adsorption capacity. The prepared CH/MCM-48 composite was applied in systematic retention of U (VI) as well as Sr (II) ions from water as the commonly detected radioactive pollutants. It displayed promising retention capacities of 261.3 mg/g and 328.6 mg/g for U (VI) and Sr (II) considering the equilibrium time interval that was identified after 420 min. The composite showed the kinetic behavior of the Pseudo-First order model and the isotherm properties of the Langmuir assumption. The thermodynamic assessment of the reactions validated the retention of both U (VI) and Sr (II) ions by spontaneous, favorable, and exothermic reactions. Based on the theoretical values of entropy (-5.94 kJ mol-1 (U (VI)) and -2.93 kJ mol-1 (Sr (II))), Gibbs free energy (less than 20 kJ mol-1), and Gaussian energy (5.77 kJ mol-1 (U (VI)) and 4.56 kJ mol-1 (Sr (II))) the uptake processes are related to physical adsorption reactions. The CH/MCM-48 composite is of significant recyclability and showed considerable affinities for the studied radioactive ions even in the presence of other metal ions (Cd (II), Pb (II), Zn (II), and Co (II)).

ABSTRACT

BACKGROUND AND OBJECTIVE: The use of aromatic and herbal plants as a feed additive became a new trend in feeding dairy cows to enhance animal productivity. This study was conducted to evaluate the effect of adding some herbal and aromatic plants (Melissa, Fennel and Caraway) at 0.7% of total DM of rations. MATERIALS AND METHODS: Three herbal plants were evaluated in this study (melissa, fennel and caraway) by using thirteen Holstein Friesian cows. Cows were in 3rd and 5th lactation seasons were used as one experimental group by swing over design in lactation experiment (control then T1 then T2 then T3 then control). Nutrients digestibility, milk production, milk composition and blood constituents were determined. RESULTS: Data indicated that nutrients digestibility and nutritive value were improved with herbal plants addition, especially with caraway treatment which recorded significantly (p>0.05) the highest values. In the same trend, Actual milk yield, 4% FCM and fat content were significantly (p>0.05) increased when cows fed rations supplemented with fennel and caraway. The addition of melissa decreased the concentration of Total Saturated Fatty Acids (TSFA) in the milk. Normal ranges for blood parameters with no negative impact on animal health were observed with experimental rations. CONCLUSION: It could be concluded that caraway addition at 0.7% of the total dry matter of dairy rations, has a great potential to improve animal productivity and enhance the quality of milk fatty acids profile.

BACKGROUND: The optimum target in surgery for Parkinson's disease (PD) is still controversial, especially in patients with tremor-dominant PD. We aim to compare results in tremor-dominant patients undergoing pallidotomy vs. those undergoing simultaneous posteroventral pallidotomy (PVP) and ventral intermediate nucleus (VIM) thalamotomy. METHODS: Twenty-four patients with tremor-dominant PD were included in this study. Twelve patients received unilateral PVP contralateral to the most affected side. The other 12 patients received simultaneous unilateral PVP and VIM thalamotomy contralateral to the most affected side. Assessment of results in both groups was achieved using both UPDRS "off" motor scores and UPDRS rest tremor subscores. RESULTS: The mean UPDRS off motor score improved in the pallidotomy group from 61.3 preoperatively to 36.8 at 12 months. In the combined group, it improved from 59.6 to 35.2 at 12 months, with no statistically significant difference between both groups. On the other hand, while the mean tremor subscore in the pallidotomy group improved from a mean of 2.3-0.8, the tremors were abolished in all of the patients in the combined group except for 1 patient who showed slight infrequent tremors at 12 months. CONCLUSION: Patients with tremor-dominant PD achieve more improvement in tremor control after combined PVP and VIM thalamotomy.

ABSTRACT

OBJECTIVE: To investigate the effects of electromyographic (EMG) biofeedback training on pain, quadriceps strength, and functional ability in juvenile rheumatoid arthritis (JRA). DESIGN: This is a randomized controlled study; 36 children (11 boys and 25 girls) with polyarticular JRA, with ages ranging from 8 to 13 years, were selected and assigned randomly, using computer-generated random numbers, into 2 groups. The control group (n = 18) received the conventional physical therapy program, whereas the study group (n = 18) received the same program as the control group in addition to EMG biofeedback-guided isometric exercises for 3 days a week for 12 weeks. Pain, peak torque of quadriceps strength, and functional ability were evaluated before, after 6 weeks, and at the end of 12 weeks of the treatment program. RESULTS: By 6 weeks, significant differences were observed in the study group (P < 0.05) in all measured variables except pain levels, whereas nonsignificant differences were observed in all measured variables in the control group. By 12 weeks, each group demonstrated significant improvements in pain, quadriceps strength, and functional ability (P < 0.05), with significantly greater improvements seen in the study group compared to the control group (P < 0.05). Both groups showed significant improvement at 12 weeks compared to that at 6 weeks. CONCLUSIONS: Electromyographic biofeedback may be a useful intervention modality to reduce pain, improve quadriceps strength, and functional performance in JRA.

ABSTRACT

The authors investigated the effect of curcumin on markers of oxidative stress and liver damage in rats that chronically ingested alcohol and heated oil. Nine groups of ten Wistar male rats received combinations of curcumin 100 mg/kg body weight daily, ethanol 5 mg/kg, 15% dietary sunflower oil and 15% heated sunflower oil for 12 weeks. Serum and liver tissue were collected. Groups 4-6, which had received compounds causing oxidative stress, showed increased serum aspartate aminotransferase, alanine aminotransferase, alkaline phosphatase, total bilirubin, cholesterol, triglycerides, low density lipoprotein, very low density lipoprotein and reduced high density lipoprotein, protein and albumin, compared with the controls. Reductions were observed in glutathione peroxidase and reductase gene expression, superoxide dismutase activity, glutathione peroxidase activity, glutathione reductase activity, reduced glutathione concentration and catalase enzyme activity. Groups 7, 8 and 9 which received curcumin with heated oil, ethanol or both, showed lower elevations in serum and oxidative damage markers compared with the corresponding non-curcumin treated groups. It can be concluded that curcumin reduces markers of liver damage in rats treated with heated sunflower oil or ethanol.
