ABSTRACT

Tinosporae Radix, as a traditional Chinese medicinal herb, is the dried root tuber of Tinospora sagittata or T. capillipes. It was first recorded in the Compendium of Materia Medica Supplement in the Qing Dynasty and included in the previous edition of the Chinese Pharmacopoeia. Tinosporae Radix is excavated in autumn and winter and used after removing fibrous roots, washing, and drying. It is indicated for sore throat, carbuncle boils poison, waist and abdominal pain, and various heat syndromes and is commonly used to treat chronic inflammation. Its efficacy is significantly known as “broad-spectrum antibiotics in Zhuang medicine”. Tinosporae Radix is a traditional Chinese medicinal herb often taken by Zhuang and Yao nationalities in Guangxi province and has a wide range of application and development values and research significance. Modern studies have shown that Tinosporae Radix contains diterpenoids, alkaloids, sterols, anthraquinones, glycosides, fatty acids, volatile oils, and other compounds, which have many pharmacological activities such as anti-inflammatory and analgesic, antibacterial and antibacterial, antioxidant, anti-diabetic, and anti-tumor and anti-cancer effects, and it has achieved good efficacy in inhibiting inflammation and treating sore throat and other diseases. In recent years, there have been many research reports on the status, chemical constituents, pharmacological action, clinical application, and quality evaluation of Tinosporae Radix resources, but there is no systematic review and introduction at present. By consulting the literature and combining it with modern research, this paper systematically summarizes and collates Tinosporae Radix resources to provide guidance for the comprehensive development and utilization of Tinosporae Radix resources and subsequent in-depth study.

ABSTRACT

Pediatric cough is a heterogeneous condition in terms of symptoms and the underlying disease mechanisms. Symptom phenotypes hold complicated interactions between each other to form an intricate network structure. This study aims to investigate whether the network structure of pediatric cough symptoms is associated with the prognosis and outcome of patients. A total of 384 cases were derived from the electronic medical records of a highly experienced traditional Chinese medicine (TCM) physician. The data were divided into two groups according to the therapeutic effect, namely, an invalid group (group A with 40 cases of poor efficacy) and a valid group (group B with 344 cases of good efficacy). Several well-established analysis methods, namely, statistical test, correlation analysis, and complex network analysis, were used to analyze the data. This study reports that symptom networks of patients with pediatric cough are related to the effectiveness of treatment: a dense network of symptoms is associated with great difficulty in treatment. Interventions with the most different symptoms in the symptom network may have improved therapeutic effects.

ABSTRACT

Objective To evaluate the effect of traditional Chinese medicine nursing technology among patients with ulcerative colitis.Methods Totally 170 patients with ulcerative colitis were recruited.All patients received routine treatment and received transitional care in inpatient department including diet guidance,illness response and anal guidance on contraction exercise,etc.What's more,the observation group implemented traditional Chinese medicine nursing techniques,including auricular pressure paste,acupuncture point application,acupressure,moxibustion,etc.After 7 and 14 days,we compared the patients' disease activity,sleep quality by Grading scale for Traditional Chinese Medicine symptoms and signs of ulcerative colitis,improved Mayo question,Pittsburgh Quality Sleep Index(PQSI).Results On the 7th and 14th day after intervention,the scores of symptoms and signs in the observation group were (9.73 ± 3.49),(4.54 ± 2.75) points,lower than those in the control group (10.72 ± 4.00),(8.22 ±3.06) points,and the difference was statistically significant (t =1.973,7.878,P =0.042,0.000).On the 14th day after intervention,the Mayo score of observation group was (3.33 ± 1.82) points,lower than that of control group (4.56 ± 1.77) points.The difference was statistically significant (t =4.278,P =0.000).The PSQI score of observation group was (5.67 ± 2.87) points,lower than that of control group (8.53 ± 3.13)points.The difference was statistically significant (t =5.934,P =0.000).Conclusions Traditional Chinese medicine transitional care could effectively improve the symptom of disease and sleep quality among patients with ulcerative colitis.

ABSTRACT

Objective To investigate and evaluate the effectiveness and safety of continuous local incision infiltration anesthetic analgesia after total knee arthroplasty(TKA).Methods Forty-eight patients with initial unilateral TKA in the Affiliated Hospital of Traditional Chinese Medicine of Southwest Medical University from August 2015 to May 2016 were allocated to three groups according to systematic random sampling methods:16 cases in the control group received continuous local incision infiltration anesthesia and femoral nerve block analgesia,16 cases received continuous femoral nerve block analgesia after TKA(CFNB) and other 16 cases received local ropivacaine continuous local incision infiltration anesthesia analgesia(CWI).The preoperative general situation,visual analogue scale (VAS) score of operative site in movement and rest on postoperative 1-3 d,postoperative early function recovery and postoperative complication were compared among 3 groups.Results All cases were followed up.There was no statistically significant difference in the general situation postoperative complications among 3 groups(P>0.05).the VAS score under movement status on postoperative 1-3 d and rest status in the CWI group and CFNB group were similar,which were lower than those in the control group,the difference was statistically significant(P<0.05);the postoperative early function recovery in the CWI group was superior to that in the control group and CFNB group.Conclusion local incision infiltration anesthetic analgesia after TKA can relief postoperative pain,facilitate patient's early function exercise and rehabilitation,and improves patient's satisfaction.

ABSTRACT

<p><b>OBJECTIVE</b>To compare the efficacy difference between fire acupuncture with centro-square needles (FACSN) and filiform needling (FN) for knee osteoarthritis (KOA).</p><p><b>METHODS</b>Seventy-two patients were randomly assigned into an FACSN group and an FN group, 36 cases in each one.points, Xuehai (SP 10), Liangqiu (ST 34), Neixiyan (EX-LE 4), Dubi (ST 35), Zusanli (ST 36), Yanglingquan (GB 34) and Yinlingquan (SP 9) were selected in the two groups. The FACSN group was treated with FACSN, and three acupoints were selected for each treatment; the FN group was treated with FN, and all the acupoints were selected for each treatment. The cupping treatment was given after acupuncture in the two groups. The treatment was given once every other day, without treatment on Sundays. The treatment was given three times a week, 6 times as one course; totally 2 courses were provided. The visual analogue scale (VAS) and Western Ontario and McMaster Universities Arthritis Index (WOMAC) were observed in the two groups before treatment, two weeks, four weeks into treatment and at one-month follow-up visit. In addition, the comprehensive efficacy was compared between the two groups.</p><p><b>RESULTS</b>Compared before treatment, the score of VAS and the total score of WOMAC were improved in the two groups at each time point after treatment (all<0.01); the scores of VAS at each time point after treatment in FACSN group were lower than those in the FN group (all<0.05); four weeks into treatment and at one-month follow-up visit, the total score of WOMAC in the FACSN group was lower than that in the FN group (both<0.05). Two weeks into treatment, the total effective rate was 88.9% (32/36) in the FACSN group, which was higher than 61.1% (22/36) in the FN group (<0.01); four weeks into treatment and at one-month follow-up visit, the cured and remarkable effective rates were 66.7% (24/36) and 83.3% (30/36) in the FACSN group, which were higher than 41.7% (15/36) and 44.4% (16/36) in the FN group (<0.05,<0.01), respectively.</p><p><b>CONCLUSIONS</b>Fire acupuncture with centro-square needles has relatively high cured and remarkable effective rate for KOA, with rapid onset; as for pain relief, the efficacy is superior to filiform needling.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To observe the clinical effects of acupuncture combined with auricular point sticking based on the western medication for post stroke depression (PSD).</p><p><b>METHODS</b>Sixty patients with PSD were randomly assigned into an acupuncture plus auricular application group (a combination group) and a medication group, 30 cases in each one. 20 mg paroxetine hydrochloride was prescribed orally in the medication group, once a day for continuous 8 weeks. Based on the above treatment, 30-minute acupuncture was used in the combination group for 8 weeks at Baihui (GV 20), Sishencong (EX-HN 1), Shenting (GV 24), Yintang (GV 29), Shenmen (HT 7), Neiguan (PC 6), Taichong (LR 3), Hegu (LI 4), Zusanli (ST 36), Sanyinjiao (SP 6) and Fenglong (ST 40), once the other day and three times a week. Auricular point sticking therapy for 8 weeks was applied at shenmen (TF), pizhixia (AT), xin (CO), and gan (CO), with pressing 3 times a day and once 3-5 days. The total score and each factor scores of Hamilton's depression scale (HAMD) were observed in the two groups before and after treatment, and Asberg's antidepressant side-effect rating scale (SERS) and clinical effect were evaluated.</p><p><b>RESULTS</b>After treatment, the total HAMD scores of the two groups decreased compared with those before treatment (both<0.05), with better effect in the combination group (<0.05). The scores of the combination group after treatment were lower than those in the medication group, including the anxiety/somatization factor, sleep disturbance factor, hopelessness factor (all<0.05). The total effective rate of the combination group was 86.7% (26/30), which was better than 66.7% (20/30) of the medication group (<0.05). The SERS score of the combination group was lower than that of the medication group (<0.05).</p><p><b>CONCLUSIONS</b>Acupuncture combined with auricular point sticking can improve the clinical symptoms and are effective and safe for PSD.</p>

ABSTRACT

ObjectiveTo evaluate the therapeutic effect ofZishen decoction combined with standard treatment of western medicine for for stage IV diabetic nephropathy with Qi and Yin deficiency.MethodsA total of 112 patients with stage IV diabetic nephropathy and Qi and Yin deficiency were randomized to thestandard treatment and the combined treatment groups, 56 in each. The standard treatment group received conventional treatment, including blood glucose controlling, antihypertensive, blood lipid regulating and diet controlling. The combined treatment group receivedZishen decoction on the basis of conventional treatment. All the patients were treated for 3 months. The blood urea nitrogen (BUN), serum creatinine (SCr), total cholesterol (TG), triacylglycerol (TC), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C) and glycated hemoglobin (HbA1c) were measured by an automatic chemistry analyzer. The urinary albumin excretion rate (UAER) was measured by the enzyme-linked immunosorbent assay.ResultsCompared with the standard treatment group, the SCr (53.51 ± 18.12μmol/Lvs. 62.66 ± 21.14μmol/L;t=2.459,P<0.05), UAER(100.73±84.24μg/minvs. 156.24 ± 96.38μg/min;t=3.245,P<0.05), TG(1.73±0.22 mmol/Lvs. 2.06 ± 0.21 mmol/L;t=8.112,P<0.01), TC(4.56 ± 0.62 mmol/Lvs. 5.10 ±0.31 mmol/L;t=5.830, P<0.01), LDL-C (2.42 ± 1.05 mmol/Lvs. 3.31 ± 0.81 mmol/L;t=5.022,P<0.01) in the combined treatment group decreased significantly, and the HDL-C (1.67 ± 0.33 mmol/Lvs. 1.36 ± 0.41 mmol/L;t=4.460,P<0.01) increased significantly. ConclusionZishen decoction on the basis of conventional treatment can improve the SCr and UAER, and regulate the blood lipid in the patients with stage IV diabetic nephropathy and Qi-Yin deficiency.

ABSTRACT

Objective To evaluate the effect of the self-made Zishen decoction combined with conventional western medicine therapy and health education on the patients with early diabetic nephropathy. Methods The 112 patients with DN were randomly assigned to 2 groups (each group 56 patients) at a ratio of 1:1. The control group was treated with control of glucose, blood pressure, lipid, and diet therapy, and the treatment group was with self-made prescription of invigorating the kidney and health education based on the control group treatment. All patients were treated for 6 months. The SF-36 scale was used to assess the quality of life, and the clinical effect was determined based on the blood biochemical indexes. Results Total clinical effect of the treatment group was 85.7%(48/56), and the control group was 67.9%(38/56) (χ2=4.057, P=0.044). Compared with the control group after treatment for 6 months, the physical condition (72.17 ± 13.41 vs. 64.59 ± 11.83, t=3.172), social function (64.58 ± 14.54 vs. 58.94 ± 14.62, t=2.047), physical role function (55.82 ± 10.11 vs. 47.46 ± 10.18, t=4.360), emotional role function (60.43 ± 10.20 vs. 56.04 ± 11.44, t=2.143), energy (69.86 ± 11.43 vs. 62.47 ± 11.12, t=3.468), general health status (68.57 ± 11.25 vs. 62.45 ± 11.78, t=2.812) of the treatment group were significantly improved (P<0.01 or P<0.05). Conclusion The self-made Zishen decoction and health education combined with conventional western medicine can improve the clinical effect and quality of life of patients with DN.

ABSTRACT

The development and compilation of Evidence-based Guidelines of Clinical Practice with Acupuncture and Moxibustion: Adult Bronchial Asthma are introduced from three aspects, named the guideline methodology, the guideline structure and the guideline content. Based on the acupuncture-moxibustion practice and clinical research, the evidence-based medicine method is adopted. During the development and compilation of the guideline, the characteristics and advantages of acupuncture and moxibustion are specially considered in the treatment of this disease; the latest optimum evidences at home and abroad, experts' experience and patients' value are closely integrated with each other. Additionally, the worldwide accepted assessments of evidence quality and the recommendation (GRADE system) are combined with the clinical evidences of the ancient and modern famous acupuncture-moxibustion experts, and the clinical research evidences are with the experts' consensus to the large extent. The purpose of the guideline is to provide the maximal guidance to the clinical physicians.

ABSTRACT

<p><b>OBJECTIVE</b>By using questionnaire method to understand the evaluation regarding cost-effectiveness of acupuncture for migraine in both doctors and patients.</p><p><b>METHODS</b>By using questionnaire method, questionnaires for doctors and patients were made respectively; by using network platform, two sets of questionnaires were publicly released, which were filled in online. The results were real-time background collected and then analyzed.</p><p><b>RESULTS</b>Totally 696 effective questionnaires for patients and 114 questionnaires for doctors were collected. The results indicated that (1)the direct cost (including treatment fee and material fee) for preventive treatment of migraine was 109 yuan per time in regular acupuncture, 152 yuan per time in regular acupuncture + electroacupuncture (EA) and 238 yuan per time in acupoint catgut embedding. The travelling expense was 42 yuan per time within the city and 193 yuan per time between cities. The indirect cost included cost for loss of working time (208 yuan per time), cost for treatment time (acupoint catgut embedding: 10 to 30 min per time, once one to three weeks; the remaining two treatments: 20 to 50 min per time, once to 5 times every week), time cost for arriving and departing hospital (1 to 3 hours per time within the city and 4 hours per time between cities). From the angle of treatment course, acupoint catgut embedding reduced the treatment frequency, leading to a lower total cost. (2)A mong the reasons to select different acupuncture methods to treat migraine, the results of patient questionnaire indicated that the focus in the patients who selected acupoint catgut embedding was different from that in the patients who selected two other treatments. The main advantage of acupoint catgut embedding was superior and lasting efficacy with low cost. The results of doctor questionnaire indicated the main reason to select acupoint catgut embedding was "lasting efficacy after single treatment" (87. 5%) and "lower frequency of treatment" (75. 0%). (3) The main reason to obstruct the popularization of acupoint catgut embedding was this method was not widely known.</p><p><b>CONCLUSION</b>The acupoint catgut embedding is one ideal preventive treatment for migraine, which has higher cost-effectiveness, however, currently it is not widely applied. In future advertisement and training program should be strengthened to perform targeted popularization of acupoint catgut embedding for migraine.</p>

ABSTRACT

By combing differe nt modelin g factors and evaluation indicators of experimental animals of polycystic o-vary syndrome (PCOS), application scope, advantages and disadvantages were analyzed among various kinds of animal models, in order to screen models of etiology and pathogenesis in traditional Chinese medicine (TCM). Through the collection of 77 Chinese literatures published since 1979, the database on PCOS of modeling factors (i.e., animal species, age, modeling reagent and dose, etc.) and evaluation indicators (i.e., ovarian morphology and hormone levels) were established, analyzed and summarized. The results showed that the experimental PCOS models established by researchers both at home and abroad included androgen method, combination method of androgen with HCG, combi-nation method of progestrone with HCG, estrogen method, aromatic cyclase inhibitor method, combination method of HCG with insulin, combination method of androgen with insulin, insulin method, and etc. The SD and Wistar rats at the age of 3-50 days were commonly used. Chemicals, such as dehydroepiandrosterone (DHEA), testosterone propi-onate, sodium prasterone sulfate, dihydrotestosterone (DHT), levonorgestrel, estradiol valerate, letrozole, insulin and HCG were chosen to build the animal models. The evaluation indicators included ovarian morphology, the total num-ber of follicles at different levels, sex hormones (E2, P, T, FSH, LH) and insulin resistance (IR) index (FINS, FPG, HOMA). It was concluded that each modeling method described above had its own advantages and disadvantages, which cannot completely simulate clinical onset process of PCOS patients. Researchers should choose appropriate reagent and evaluation index according to different purpose. DHEA, DHT slow-release, progesterone combined with HCG modeling methods are more suitable for the study of the role of IR in the pathogenesis of PCOS. Kidney-defi-ciency and dampness-phlegm is fundamental etiology and pathogenesis of obese type of PCOS. DHEA method, com-bination method of levonorgestrel with HCG, aromatic cyclase inhibitor method conformed obese type of PCOS. The evaluation indicators included body weight, sex hormones, IR index and ovarian morphology. The method of kidney-tonifying and phlegm-reducing was often used in the treatment of obese type of PCOS in the clinical practice.

ABSTRACT

<p><b>OBJECTIVE</b>To study the technique of the callus induction from anther and plant regeneration of medicinal plants Liriope spicata var. prolifera.</p><p><b>METHOD</b>Callus was induced from anther of L. spicata var. prolifera on a MS medium supplemented with different hormones. The squash methods combined with a microscope were used to analyze chromosomes of regenerated plantlets.</p><p><b>RESULT</b>MS +2,4-D 1.0 mg x L(-1) + KT 2.0 mg x L(-1) gave the highest induction ratio which was 41.07%. MS +6-BA 1.5-2.0 mg x L(-1) + NAA 0.1-0.3 mg x L(-1) was suitable for the induction and proliferation of indefinite buds. The buds were transferred to 1/2 MS medium supplemented with NAA 0.1-0.3 mg x L(-1) for rooting. The shoots produced roots of culture and formed complete plantlets. The regenerated plantlets originated from somatic cells. At the same time, the effects of pretreatment of low temperature at 4 degrees C on the callus induction were studied and discussed.</p><p><b>CONCLUSION</b>This paper sets up the method of tissue culture of anther somatic-cells and intermediate propagation of L. spicata var. prolifera.</p>

<p><b>OBJECTIVE</b>To research on the variation regularity of the five main flavonoids contents in Epimedium and processed Epimedium.</p><p><b>METHOD</b>The contents of Epimedin A, Epimedin B, Epimedin C, Icariin and Baohuoside I in Epimedium and processed Epimedium were determined, respectively.</p><p><b>RESULT</b>There were varieties in the contents of Epimedin A, Epimedin B, Epimedin C, Icariin and Baohuoside in Epimedium and processed Epimedium. There were different variety tendencies in different species and different batches.</p><p><b>CONCLUSION</b>The variety tendencies of the contents of five main flavonoids depend on the processing technology and the flavonoids proportion in Epimedium and processed Epimedium.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To reveal the mechanism of sterility of Liriope spicata var. prolifera.</p><p><b>METHOD</b>Meiotic behavior and pollen development of L. spicata var. prolifera were described in detail. Its proto-variety L. spicata was also investigated for comparison.</p><p><b>RESULT</b>During the meiosis of microspore mother cells (MMC), most of cells displayed normally in L. spicata, but abnormally in L. spicata var. prolifera. The abnormity was showed that: some chromosomes or their fragments moved out of the spindle of the cell at metaphase; some chromosome bridges, fragments and lagging ones were formed at anaphase; and many microspores displayed the micronucleus at the telophase. The pollen development was abnormal in L. spicata var. prolifera and normal in L. spicata, with the aberration rate of pollen was 95.81% and 3.44% separately.</p><p><b>CONCLUSION</b>These results indicate that some abnormalities of meiotic behavior and pollen development are main reasons for inducing microspore abortion during its development.</p>

Objective To discuss the laws of treatment of chronic fatigue syndrome(CFS) by acupuncture and moxibustion.Methods The literature between 1998-2008 were searched by means of electronic retrieval.The database was established by Epidata 3.1.Applying Excel 2000 to analyze the literatures on the following aspects:the diagnosis of CFS in TCM,the therapeutic methods and manipulations,acupoint selection,course of treatment and therapeutic effects of CFS treated by acupuncture and moxibustion.Results Of the 78 literatures enrolled,50 diagnozed CFS in TCM as vacuity taxation or vacuity detriment,22 as depression pattern.53 identified disease and administered treatment with settled prescription of acupoints,21 identified patterns and combined points.84 body points and 18 auricular points were founded in the prescriptions involving the points of 14 meridians and extra points.28 were treated with one kind of acupuncture and moxibustion methods,50 with more than one kind of acupuncture and moxibustion methods.48 were acupunctured by means of filiform needle and 8 cooperated with electro-acupuncture,18 by means of moxibustion,18 cupping,9 pricking blood,6 auricular therapy,4 dermal needle,3 point-injection,2 embedding therapy and 2 acupoint application with herbal medicine.44 were treated once a day,21 once every other day.The means of total treatment times were 22?11.52 had the total efficiency above 90%,and 17 between 80%~90%.Conclustion Among all kinds of acupuncture and moxibustion methods in treating CFS,filiform needle,electro-acupuncture,moixbustion and cupping were mostly used,and generally more than one kinds of methods were cooperated.The points selected mainly in UB meridian of foot Taiyang,Ren meridian and Du meridian.The points used in high frequency are as follows:ST36,SP6,UB23,DU20,UB15,UB18,UB20,REN4,REN6 and LIV3,generally it needed comparatively long time of treatment to acquire better therapeutic effects.

ABSTRACT

Objective To assess the quality of literature of clinical studies on acupuncture in treatment of chronic fatigue syndrome.Methods The literatures between 1998-2008 were searched by means of electronic retrieval.Type and methodology,general condition,criteria for diagnosis and assessment of therapeutic effects,following-up and side effects etc.in clinical studies were evaluated according to principles and methods of clincial epidemiology and evidence-based medicine.Results Of the 78 literatures enrolled,76 were studied in China and published in Chinese journal of China,only 2 were English literature and published in English journal of China.Only 7 were authentic randomized controlled trials(RCTs),21 quasi-randomized controlled trials and RCTs can't contact with authors,3 non-randomized concurrent controlled trials and 47 narrative studies.50 had clear international recognized diagnostic ctriteria.22 mentioned enrolled or excluded criteria.26 adopted TCM criteria for therapeutic effects,27 evaluated therapeutic effects with grading.4 the sample content above 100,33 between 60 and 99,4 mentioned compliance evaluation,3 described retreat cases,2 reported follow-up.11 reported adverse reaction.Conclustion Clinical studies on acupuncture in treatment of chronic fatigue syndrome were mainly carried through in China,and the quality of study and literature is low comparatively.Therefore,high quality's RCTs with strict design are needed urgently in this area and the researchers should contribute to journals overseas.
