ABSTRACT

Objective:To investigate the effect of target-controlled infusion propofol combined with inhalation sevoflurane to maintain anesthesia on the postoperative changes of serum β-amyloid protein (β-AP), neuron specific enolase (NSE) and cognitive function in elderly patients with non-small cell lung cancer (NSCLC).Methods:The clinical data of 78 elderly patients with NSCLC who underwent thoracoscopic surgery from December 2017 to December 2019 in Jinhua Hospital of Traditional Chinese Medicine of Zhejiang Province were retrospectively analyzed. Among them, target-controlled infusion propofol to maintain anesthesia was in 39 cases (control group), and target-controlled infusion propofol combined with inhalation sevoflurane to maintain anesthesia was in 39 cases (study group). The serum levels β-AP, NSE and cognitive function (assessed by mini mental state examination, MMSE) before and after operation, the postoperative recovery (eye opening time, response time and extubation time) and incidence of adverse reactions were compared between 2 groups.Results:There were no statistical differences in β-AP and NSE before operation between 2 groups ( P>0.05); the β-AP and NSE immediately and 6 h after operation in study group were significantly lower than those in control group, β-AP: (416.13 ± 22.81) μg/L vs. (510.73 ± 24.27) μg/L and (373.53 ± 21.72) μg/L vs. (430.68 ± 22.15) μg/L, NSE: (8.35 ± 0.66) μg/L vs. (11.13 ± 0.73) μg/L and (7.81 ± 0.61) μg/L vs. (9.12 ± 0.68) μg/L, and there were statistical differences ( P<0.01); there were no statistical differences in β-AP and NSE 24 h after operation between 2 groups ( P>0.05). There was no statistical difference in MMSE score before operation between 2 groups ( P>0.05); the MMSE score 6, 24 and 72 h after operation in study group was significantly higher than that in control group: (22.32 ± 2.05) scores vs. (20.54 ± 2.31) scores, (25.19 ± 1.33) scores vs. (23.61 ± 1.08) scores and (26.84 ± 0.97) scores vs. (25.01 ± 1.15) scores, and there was statistical difference ( P<0.01); there was no statistical difference in MMSE score 7 d after operation between 2 groups ( P>0.05). The eye opening time, response time and extubation time in study group were significantly shorter than those in control group: (14.15 ± 3.20) min vs. (19.32 ± 3.14) min, (18.08 ± 3.52) min vs. (24.63 ± 4.10) min and (16.21 ± 4.40) min vs. (22.31 ± 4.71) min, and there were statistical differences ( P<0.01). There was no statistical difference in the incidence of adverse reactions between 2 groups ( P>0.05). Conclusions:Target-controlled infusion propofol combined with inhalation sevoflurane to maintain anesthesia in elderly patients with NSCLC can reduce the increase of serum levels of β-AP and NSE, reduce the damage to cognitive function, make patients recover quickly after operation, and will not increase the incidence of adverse reactions. Its security is higher.

ABSTRACT

Objective To observe the effects of enhanced exercise and combined vitamin D and calcium supplementation on muscular strength and fracture occurrence in postmenopausal women with a high risk of osteoporosis.Methods Totally 614 postmenopausal women at high risk factors of osteoporosis were enrolled in Dongcheng district of Beijing and randomized into four groups:group A(control group,n=173),group B(regular Tai Chi exercise,n=171),group C(calcium 600 mg/d+VitD800 U/d,n=139),and group D[calcium 600 mg/d+25 hydroxyl vitamin D(25OHD) 0.25 μg/d,n=131].Muscular strength was measured at baseline and one and two years after intervention.Bone turnover markers were measured at baseline and during the two-year follow-up.Falls and fractures were recorded.Results The incidence of 25OHD<50 nmol/L was approximately 92.6%.During the follow-up,the left grip strength decreased significantly two years after intervention(t=-3.252,P=0.001)in group A.Right grip strength decreased significantly in group B(t=2.460,P=0.015)while left grip strength improved significantly in group C(t=-2.051,P=0.043)one year after intervention.In group D,muscular strength in both 12-month and 24-month did not change compared with baseline(both P>0.05).Furthermore,serum procollagen type I N-terminal propeptide elevated significantly in group A(t=-2.962,P=0.004),group B(t=-2.888,P=0.005),and group C(t=-2.441,P=0.016),whereas β-C-terminal telopeptide of type I collagen decreased significantly in group B(t=2.285,P=0.024)and group D(t=2.596,P=0.011)two years after intervention.Conclusion Enhanced exercise and combined calcium vitamin D supplementation may help sustain muscle strength in postmenopausal women,while calcium and vitamin D supplementation may improve muscular strength within a short period of time.

ABSTRACT

Objective To evaluate the effect for stroke(cerebral infarction)at acute stage intervened by traditional Chinese medicine(TCM) clinical pathway. Methods A retrospective analysis was carried out in the patients before and after intervention by TCM clinical pathway during the year of 2013-2016. The outcomes included the entrance rate,completion percentage,aberration rate,severity,hospitalization time,and hospitalization fee as well as functional prognosis 30 d after discharge. Results A total of 792 cases was included,241 cases admitted in the year of 2013, 191 in the year of 2014, 135 in the year of 2015, and 225 in the year of 2016. In the year of 2014-2016,the entrance rate,completion percentage,hospitalization time,hospitalization fee,critical case percentage,and gender constituent ratio were steadily(P > 0.05). Modified Rankin Scale(mRS)for short-term functional prognosis mRS 0 ~ 2 percentage and aberration rate in the year of 2016 were superior to those in the year of 2014 and 2015(P < 0.05). Functional improvement on discharge day 30 in the year of 2014-2016 was superior to that in the year of 2013(P < 0.05). The hospitalization time and hospitalization fee in the year of 2014-2016 were a liffle higher than those in the year of 2013(P < 0.05), showing no obvious increase. Conclusion The application of TCM clinical pathway can make the diagnosis and treatment procedure of stroke at acute stage standardized,enhance the therapeutic effect,achieve or even exceed the goal of manage scheme.

ABSTRACT

To establish quality standards of Euonymus fortunei, and supply scientific evidence for the quality control of Euonymus fortunei. Empirical and microscopic identification methods were adopted to observe morphological and histological characters. The contents of water, total ash, acid-insoluble ash and alcohol-soluble extractive were analysed according to the methods of Chinese Pharmaco- poeia (2010). Dulcitol and reference herbs were used to identify materia medica of Euonymus fortunei by TLC method. The total flavonol glycosides contents were analysed by HPLC method, using quercetin and kaempferol as reference substances. Quercetin and kaempferol were separated on a C18 column (4.6 mm x 250 mm, 5 μm) with methanol-0.1% formic acid(51:49) as the mobile phase and detected at 366 nm. The flavonoid aglycones content was then multiplied by a conversion coefficient, and the result was the total flavonol glycosides content. The macroscopical identification, microscopic features and TLC methods were proper. The average contents of water, total ash, acid-insoluble ash, alcohol-soluble extractive and total flavonol glycosides were 8.76%, 6.48%, 0.31%, 17.48% and 0.211% , respectively. The quality standards established on the basis of the research results were suitable for the quality evaluation of Euonymus fortunei.

ABSTRACT

<p><b>OBJECTIVE</b>To analyze the relationship between the features of preoperative contrast-enhanced ultrasound and the differentiation of symptoms and signs for syndrome classification by traditional Chinese medicine (TCM).</p><p><b>METHODS</b>Eighty-two patients with malignant breast tumor were examined with contrast-enhanced ultrasound before the operation. The tumor diameter was greater than 2.0 cm in 48 cases and below 2.0 cm in 34 cases. According to the curative effect standard of TCM disorder, differentiation of symptoms and signs for classification of syndrome were performed, and the ultrasonic manifestations of different syndrome types were compared.</p><p><b>RESULTS</b>In patients with maximum tumor diameter < or = 2 cm, Chong and Ren disorder type and liver Qi stagnation type were prevalent. In those with tumor diameter of <2 cm, Zeyi liver Qi stagnation type and virtual drug junction were more common. The mass peak intensity, slope of increase, enhancement intensity index and time-intensity curve of ultrasound differed significant between different syndrome types (P<0.05).</p><p><b>CONCLUSION</b>The preoperative ultrasound features of breast cancer are associated with the syndrome types in TCM.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To observe the incidence of elimination delay in high dose methotrexate (HDMTX) therapy, its side effects and influence to next course of chemotherapy and analyze the relationship between the dosage, the duration of MTX infusion and the morbidity of the elimination delay.</p><p><b>METHODS</b>A total of 121 childhood acute lymphoblastic leukemia (ALL) (497 infusions of HDMTX) were analysed in this study. The elimination delay rate and the adverse effects in different dose groups (3 g/m2 vs 5 g/m2) and different infusion duration groups (7 h vs 24 h) were compared. The adverse effect evaluation was based on the World Health Organization (WHO) Toxicity Grading Criteria. The rescue dosages of calcium folinate (CF) among these groups were compared through CF/MTX index.</p><p><b>RESULTS</b>The overall morbidity of elimination delay was 12.1% with a relative risk of 30.6% for the first time. The relative risk for the second time of occurrence was increased to 45.9% (P < 0.01) and it was not significantly increased for the third time (35.3%). Children with elimination delay had lower platelet count (P < 0.01) and higher CF rescue dosage (P < 0.01), while the damage of oral mucous membrane was more severe (P < 0.05) and the next course of chemotherapy would be postponed for a median of 4 days in 3 g group. There was no significant difference in elimination delay rates between 3 g and 5 g groups (12.1% vs 12.0%, P > 0.05), and between 7 h and 24 h MTX infusion groups (13.6% vs 11.9%, P > 0.05). The only side effect occurred in 5 g group was gastrointestinal morbidity. The CF/MTX index of 5 g group without elimination delay was less than that of 3 g group (P < 0.01).</p><p><b>CONCLUSION</b>Elimination delay in HDMTX therapy accompanies the suppression of bone marrow and damage of oral mucous membrane, which need more CF rescues and will postpone the following course of chemotherapy. Elimination delay is not associated with the duration of the infusion and the dosage of MTX within the range of 3 approximately 5 g/m2 but there are individual differences.</p>

ABSTRACT

AIM: To examine the effect of traditional chinese medicine recipe, Taoren Honghua(semen persicae-flos carthami) decoction, on hyperlipidemia without symptom. METHODS: The plasma TC, TG, LDL, HDL, apolipoprotein(Apo) A, Apo B of the patients with hyperlipidemia without symptom were measured using automatic analyzer (shimadiu CL-7200), the production of nitric oxide(NO) was detected by Greiss reaction, and SOD activity and MDA formation were examined using o-trihydroxy benzene and barbituric methotheds, respectively. RESULTS: After oral administration of Taoren Honghua decoction, the plasma levels of TC, TG, LDL, and MDA of the patients were markedly decreased, however, the plasma levels of ApoA, HDL, SOD and NO were significantly increased and almost no change was detected in the plasma level of Apo B. AIM: To investigate the effect of treatment based on syndrome differentiation only and combined with Shexiang Baoxin Pill for acute ischemic stroke(AIS). METHODS: Fifty-eight cases of AIS were randomly (allocated) to two groups.Thirty cases of group A were treated with treatment based on syndrome differentiation combined with Shexiang Baoxin Pill and twenty-eight cases of group B with the treatment based on syndrone differentiation. ABSTRACT

Objective: To evaluate effect of refining of the aqneous extract solution of compound Chinese traditional medicine by microfiltration technique of inorganic ceramic membrane. Methods: The aqneous extract solution of Ephedra, Apricot kernel, Gypsum, and Licorice Decoction and Redujing Granules were processed by microfiltration technique of inorganic ceramic membrane. The changes in the characters, whole solids and effective componds before and after process were studied. Results: The aqueous extract solution of compound Chinese traditional medicine all were turbid before microfiltering and became clear after microfiltrating. The whole solid of Ephedra, Apricot kernel, Gypsum, and Licorice Decoction were decreased by 16.12%, and the lost rates of ephedrine and amygdalin were 20.11% and 18.06% respectively. The whole solid of Redujing Granules were decreased by 27.58%, and the lost rates of chlorogenic acid and rhein were 18.28% and 22.86% respectively. Conclusion: The microfiltration technology of inorganic ceramic membrane has the better effects of clarification and removing impurity on the aqueous extract solution of compound Chinese traditional medicine.

ABSTRACT

Objective: To evaluate effect of refining of decoctions of medicinal materials of radix and rhizome by microfiltration of inorganic ceramic membrane. Methods: The decoctions of 7 medicinal materials of radix and rhizome were processed by inorganic ceramic membrane, and the whole solids and effective ingredients were determined. Results: The decoctions of Chinese traditional medicine became clear after microfiltration. The whole solids were decreased by 15~38%, and the lost rate of effective ingredients was lower than that of whole solids. Conclusion: The microfiltrating technology of inorganic ceramic membrane can make decoctions of Chinese Traditional Medicine clear.
