Do Herbal Supplements and Probiotics Complement Antibiotics and Diet in the Management of SIBO? A Randomized Clinical Trial.

Small intestinal bacterial overgrowth (SIBO) arises from dysbiosis in the small intestine, manifesting with abdominal symptoms. This study aims to assess the efficacy of combined antibiotic therapy, herbal supplements, probiotics, and dietary modifications in SIBO management. A total of 179 SIBO-diagnosed patients underwent clinical evaluation and breath testing. Patients were categorized into hydrogen (H2-SIBO) and methane (CH4-SIBO) groups. The control group received standard antibiotic therapy and a low-FODMAP diet, while the intervention group received additional herbal antibiotics, probiotics, and prebiotics. After treatment, both groups exhibited reduced gas levels, particularly in CH4-SIBO. Clinical remission rates were higher in the intervention group, especially in CH4-SIBO cases. Logistic regression analysis showed gas concentrations at diagnosis as significant predictors of treatment success. In conclusion, adjunctive herbal supplements and probiotics did not significantly impact gas levels, but showed potential for clinical improvement, especially in CH4-SIBO.

An update on sparring taekwondo's internal logic / Uma atualização da lógica interna de competição do combate no taekwondo / Una actualización de la lógica interna de la competición del combate en el taekwondo

Abstract: This manuscript addresses the internal logic of sparring taekwondo, regulated by the World Taekwondo Federation, to explain motor actions used in competitions. A narrative review was conducted on Google Scholar, Scopus, Web of Science, Ebsco, PubMed, ProQuest, and Dialnet databases, guided by structural and functional taekwondo parameters with no limits as to year of publication or language used. The authors describe the various elements that make up the sport's internal logic such as regulation, space, time, motor communication, roles and sub-roles in addition to the techniques or execution models that explain their special characteristics. This could assist in further examining the characteristics of that sport, defining combat actions, and contextualizing the real competitive situation by adapting tactical taekwondo sequences. Being aware of this sport's unique characteristics will enable creating the necessary framework to continue research on Olympic-level taekwondo.

Resumo: O objetivo do manuscrito é abordar a lógica interna do taekwondo competição regulamentada pela Federação Mundial do Taekwondo para explicar a ação motora na competição. Uma revisão narrativa foi realizada com base nos parâmetros estruturais e funcionais do taekwondo nos bancos de dados do Google Scholar, Scopus, Web of Science, Ebsco, PubMed, ProQuest e Dialnet, sem limitar o ano de publicação nem o idioma utilizado. Os autores se referem aos diversos elementos que compõem a lógica interna mencionada, como regulação, espaço, tempo, comunicação motora, papéis e subfunções, além dos modelos de técnica ou execução, explicando suas particularidades. Isso poderia nos ajudar a aprofundar as características desse esporte, definir a ação de combate e contextualizar a situação real da competição, adaptando as sequências táticas do taekwondo. O conhecimento das particularidades desse esporte permite criar uma estrutura necessária para continuar o trabalho da pesquisa no taekwondo olímpico .

Resumen: El objetivo de este manuscrito es abordar la lógica interna del taekwondo de competición regulado por la Federación Mundial de Taekwondo para explicar la acción motriz en la competición. Se realizó una revisión narrativa basada en los parámetros estructurales y funcionales del taekwondo en las bases de datos del Google Scholar, Scopus, Web of Science, Ebsco, PubMed, ProQuest y Dialnet sin limitar el año de publicación ni el idioma. Los autores se refieren a los diversos elementos que componen la lógica interna mencionada, como reglamento, espacio, tiempo, comunicación motriz, roles y subfunciones, además de los modelos de técnica o ejecución, explicando sus particularidades. Esto podría ayudarnos a profundizar en las características del taekwondo, a definir la acción de combate y a contextualizar la situación real de la competición, adaptando las secuencias tácticas del taekwondo. El conocimiento de las particularidades de este deporte permite crear un marco necesario para continuar el trabajo de investigación en el taekwondo olímpico.

[Integration between cardiology and primary care: impact on clinical practice]. / Integración entre cardiología y atención primaria: impacto sobre la práctica clínica.

INTRODUCTION AND OBJECTIVES: To assess the impact of a program integrating cardiology and primary care in clinical practice, compared with usual care. The integrated care consists of a hospital cardiologist in each primary care clinic, shared clinical history, joint practice guidelines, consultation sessions, and other coordinating tools. METHODS: Observational, cross-sectional study of 2 series of chronic outpatients: conventional and integrated care. We analyzed patient distribution and the impact on good clinical practice indicators in patients with ischemic heart disease, heart failure and atrial fibrillation, along with primary care practitioner satisfaction and use of resources. RESULTS: We included 3194 patients (1572 usual care, 1622 integrated care). Integrated care changed the patient distribution, allowing the cardiologist to focus on serious pathologies while cardiovascular risk factors and stable patients were monitored in primary care. In ischemic heart disease, improvement was observed in cholesterol management and blood pressure control; optimal medical treatment was more frequently prescribed and ventricular function evaluated more often. In heart failure, ß-blockers treatment increased and functional class was assessed more often. In atrial fibrillation, an increase in anticoagulation prescription and echocardiography evaluation was observed. Satisfaction parameters improved with integrated care. The use of resources was not increased. CONCLUSIONS: Using our integration model, follow-up and chronic treatment of patients with ischemic heart disease, heart failure, and atrial fibrillation were improved. Monitoring of chronic patients was redistributed between primary care and cardiology, and family physicians' satisfaction levels improved. There was no increase in use of resources.
