ABSTRACT

INTRODUCCIÓN. La necrosis esofágica aguda es un síndrome raro que se caracteriza endoscópicamente por una apariencia negra circunferencial irregular o difusa de la mucosa esofágica intratorácica, la afectación es generalmente del esófago distal y la transición abrupta de mucosa normal en la unión gastroesofágica, con extensión proximal variable. CASOS. Se presentan dos casos con diferentes comorbiliades, presentación de signos y síntomas, antecedentes y tratamiento, teniendo en común el diagnóstico a través de endoscopía digestiva alta. RESULTADOS. Caso clínico 1: tratamiento clínico basado en hidratación, suspensión de vía oral, omeprazol intravenoso y sucralfato; mala evolución clínica caracterizada por: disfagia, intolerancia oral y recurrencia del sangrado digestivo alto, se realiza colocación de gastrostomía endoscópica. Caso clínico 2: esófago con mucosa con fibrina y parches de necrosis extensa, se realiza compensación tanto de foco infeccioso pulmonar como hidratación y nutrición, en estudios complementarios se observa masa colónica, con estudio histopatológico confirmatorio de adenocarcinoma de colon en estado avanzado. DISCUSIÓN. La esofagitis necrotizante aguda es una entidad inusual, de baja prevalencia e incidencia, asociada con estados de hipoperfusión sistémica y múltiples comorbilidades que favorezcan un sustrato isquémico. Al revisar los reportes de casos que hay en la literatura médica, los casos que reportamos se correlaciona con las características clínicas, epidemiológicas, endoscópicas y factores de riesgo causales de la enfermedad. La presentación clínica más frecuente es el sangrado digestivo alto, que se debe correlacionar con el hallazgo endoscópico clásico. Nuestro primer caso reportado termina con la colocación de una gastrostomía para poder alimentarse. CONCLUSIÓN. El pronóstico de la necrosis esofágica aguda es malo y se requiere un alto índice de sospecha clínica y conocimiento de esta infrecuente patología para un diagnóstico temprano y un manejo oportuno. Se requiere una evaluación por endoscopia digestiva alta. Es una causa de sangrado gastrointestinal que conlleva tasas altas de mortalidad, principalmente en adultos mayores frágiles. El reconocimiento temprano y la reanimación agresiva son los principios fundamentales para un mejor resultado de la enfermedad.


INTRODUCTION. Acute esophageal necrosis is a rare syndrome that is characterized endoscopically by an irregular or diffuse circumferential black appearance of the intrathoracic esophageal mucosa, the involvement is generally of the distal esophagus and the abrupt transition of normal mucosa at the gastroesophageal junction, with variable proximal extension. CASES. Two cases are presented with different comorbidities, presentation of signs and symptoms, history and treatment, having in common the diagnosis through upper gastrointestinal endoscopy. RESULTS. Clinical case 1: clinical treatment based on hydration, oral suspension, intravenous omeprazole and sucralfate; poor clinical evolution characterized by: dysphagia, oral intolerance and recurrence of upper digestive bleeding, endoscopic gastrostomy placement was performed. Clinical case 2: esophagus with mucosa with fibrin and patches of extensive necrosis, compensation of both the pulmonary infectious focus and hydration and nutrition is performed, in complementary studies a colonic mass is observed, with a confirmatory histopathological study of colon adenocarcinoma in an advanced state. DISCUSSION. Acute necrotizing esophagitis is an unusual entity, with low prevalence and incidence, associated with states of systemic hypoperfusion and multiple comorbidities that favor an ischemic substrate. When reviewing the case reports in the medical literature, the cases we report correlate with the clinical, epidemiological, endoscopic characteristics and causal risk factors of the disease. The most common clinical presentation is upper gastrointestinal bleeding, which must be correlated with the classic endoscopic finding. Our first reported case ends with the placement of a gastrostomy to be able to feed. CONCLUSION. The prognosis of acute esophageal necrosis is poor and a high index of clinical suspicion and knowledge of this rare pathology is required for early diagnosis and timely management. Evaluation by upper gastrointestinal endoscopy is required. It is a cause of gastrointestinal bleeding that carries high mortality rates, mainly in frail older adults. Early recognition and aggressive resuscitation are the fundamental principles for a better outcome of the disease.

ABSTRACT

O presente artigo é oriundo de pesquisa acadêmica que teve como objetivo estudar a clínica psicanalítica e seus efeitos por meio da apresentação de fragmentos de uma análise já encerrada. Para alcançar esse objetivo, utilizamos a ferramenta metodológica do fato clínico, articulando o material clínico com alguns pontos da teoria psicanalítica. Como resultado, apontamos que é possível fazer uma leitura teórico-clínica de uma experiência de análise sob a perspectiva dos movimentos discursivos, em especial do sujeito do inconsciente ($) e do objeto a, conforme apresentados na proposição dos Quatro Discursos de Lacan. Acreditamos que este estudo enfatiza a importância da especificidade da pesquisa em psicanálise em sua dimensão clínica e os possíveis efeitos produzidos por esse processo terapêutico. Dessa maneira, entendemos que a divulgação deste trabalho pode contribuir para a discussão da prática clínica entre colegas do campo psicanalítico e acadêmico, bem como para a difícil tarefa da apresentação do material clínico com a sustentação teórica necessária, a fim de fortalecermos a sempre fundamental transmissão da psicanálise.(AU)


This article comes from academic research that aimed to study the psychoanalytic clinic and its effects by presenting fragments of an analysis that has already ended. To achieve this goal, we used the methodological tool of the clinical fact, articulating the clinical material with some points of psychoanalytic theory. As a result, we point out that it is possible to make a theoretical-clinical reading of an analysis experience from the perspective of discursive movements, in particular, the subject of the unconscious ($) and of the object a, as presented in the proposition of Lacan's Four Discourses. We believe that this study emphasizes the importance of the specificity of research in psychoanalysis in its clinical dimension and the possible effects produced by this therapeutic process. In this way, we understand that the dissemination of this work can contribute to the discussion of clinical practice among colleagues in the psychoanalytic and academic fields, as well as to the difficult task of presenting clinical material with the necessary theoretical support, to strengthen the always fundamental transmission of psychoanalysis.(AU)


Este artículo surge de una investigación académica que pretendió estudiar la clínica psicoanalítica y sus efectos mediante la presentación de fragmentos de un análisis que ya terminó. Para lograr este objetivo, se utilizó la herramienta metodológica del hecho clínico articulando el material clínico con algunos puntos de la teoría psicoanalítica. El resultado permite señalar que es posible realizar una lectura teórico-clínica de una experiencia de análisis desde la perspectiva de los movimientos discursivos, en particular el sujeto del inconsciente ($) y el objeto a, como se presenta en la proposición de los cuatro discursos de Lacan. Este estudio enfatiza la importancia de la especificidad de la investigación en psicoanálisis en su dimensión clínica y los posibles efectos que produce este proceso terapéutico. De esta forma, la difusión de este trabajo puede contribuir a la discusión de la práctica clínica entre colegas del campo psicoanalítico y académico, así como a la difícil tarea de presentar material clínico con el soporte teórico necesario para fortalecer la siempre fundamental transmisión del psicoanálisis.(AU)

ABSTRACT

Este artigo tem como objetivo produzir uma análise histórica sobre as intersecções entre Psicologia e sexualidade desviantes da norma no Brasil, de fins do século XIX a meados da década de 1980. Esta temporalidade foi escolhida por abarcar o surgimento das pesquisas científicas sobre sexualidade e desvios sexuais, a consolidação dos estudos psicológicos sobre a temática e o processo mais recente de despatologização da homossexualidade. Em termos teóricos e metodológicos, foram adotados os pressupostos da História Social da Psicologia e da historiografia das homossexualidades no Brasil. Desse modo, buscou-se compreender como as ideias, concepções e práticas psicológicas foram mudando ao longo do tempo, em conexão com as transformações socioculturais e políticas que ocorreram durante o século XX. Para isto, foram utilizadas fontes primárias e secundárias de pesquisa com vistas à produção de interpretações sobre as conexões entre as ideias, os atores e os eventos narrados. Argumenta-se, ao longo do artigo, que as ideias e práticas psicológicas estão intrinsecamente conectadas aos contextos socioculturais e políticos de seu tempo, sendo os movimentos dinâmicos e os conflitos presentes nesses contextos fatores determinantes para a sua constituição.(AU)


This article aims to produce a historical analysis of the intersections between Psychology and sexualities that deviate from the norm in Brazil, from the late 19th century to the mid-1980s. This period was chosen because it encompasses the emergence of scientific research on sexuality and sexual deviations, the consolidation of psychological studies on the subject and the most recent process of de-pathologization of homosexuality. Theoretically and methodologically, the assumptions of the Social History of Psychology and the historiography of homosexualities in Brazil were adopted. Therefore, we sought to understand how psychological ideas, conceptions and practices have changed over time, in connection with the sociocultural and political transformations that occurred throughout the 20th century. For this, primary and secondary sources of research were used to produce interpretations about the connections between the ideas, the actors and the narrated events. It is argued, throughout the article, that the psychological ideas and practices are intrinsically connected to the sociocultural and political contexts of their time, being the dynamic movements and conflicts present in these contexts determining factors for their constitution.(AU)


Este artículo tiene como objetivo realizar un análisis histórico de las intersecciones entre la Psicología y las sexualidades desviadas de la norma en Brasil desde finales del siglo XIX hasta mediados de la década de 1980. Esta temporalidad fue elegida por abarcar el surgimiento de las investigaciones científicas sobre sexualidad y desvíos sexuales, la consolidación de los estudios psicológicos sobre el tema y el más reciente proceso de despatologización de la homosexualidad. En el marco teórico y metodológico, se adoptaron los presupuestos de la Historia Social de la Psicología y de la historiografía de las homosexualidades en Brasil. De esta manera, se pretende comprender cómo las ideas, concepciones y prácticas psicológicas han cambiado a lo largo del tiempo, en conexión con las transformaciones socioculturales y políticas ocurridas durante el siglo XX. Para ello, se utilizaron las fuentes de investigación primarias y secundarias con miras a generar interpretaciones sobre las conexiones entre las ideas, los actores y los eventos narrados. ABSTRACT

O objeto de estudo deste trabalho é a atuação de psicólogas(os) no campo da educação básica. Tivemos como objetivo investigar as práticas de atuação e os desafios enfrentados pelas(os) psicólogas(os) que trabalham na educação em Boa Vista (RR), com intuito de conhecer a inserção desses profissionais no sistema educacional. Trata-se de pesquisa qualitativa, orientada pelo referencial teórico-metodológico da Psicologia Escolar Crítica. Realizamos o processo de levantamento dos dados entre os meses de março e abril de 2018, por meio de entrevistas semiestruturadas, audiogravadas e transcritas. Encontramos 21 psicólogas e um psicólogo trabalhando em instituições educacionais e de ensino na cidade; a maioria atuava na educação básica; metade dos entrevistados ingressou por concurso público e a outra metade era contratada e comissionada; poucos(as) foram contratados(as) como psicólogos(as) escolares. Para a análise, selecionamos dez psicólogas(os) com mais tempo no cargo. Quanto às práticas de atuação, identificamos que 60% atuavam na modalidade clínica e 40% na modalidade clínica e institucional. Como desafios, encontramos melhoria das condições de trabalho; estabelecimento de relações hierárquicas e a dificuldade de fazer compreender as especificidades desse campo de trabalho; necessidade na melhoria das condições para formação continuada; atuação da(o) psicóloga(o) escolar enquanto ação institucional. Diante do exposto, compreendemos ser necessária uma mudança de paradigma na atuação das(os) psicólogas(os) que trabalham na educação na região, e a apropriação das discussões da área, principalmente, aquelas apresentadas pela Psicologia Escolar Crítica, vez que esta contribui para uma atuação que leve em conta os determinantes sociais, políticos, culturais e pedagógicos que constituem o processo de escolarização.(AU)


This work has as object of study the role of psychologists in the field of Basic Education. We aimed to investigate practices and challenges faced by psychologists who work in education in Boa Vista/RR, to know the insertion of these professionals in the educational system. This is a qualitative research, guided by the theoretical-methodological framework of Critical School Psychology. The data collection process was carried out between March and April 2018, with semi-structured, audio-recorded, and transcribed interviews. We found 21 female psychologists and one male psychologist working in educational institutions in the municipality; most worked in Basic Education; half of the interviewees had applied to work as government employee and the other half were hired and commissioned; few were hired as school psychologists. To carry out the analysis, we selected ten psychologists with more time in the position. Regarding the practices, we have identified that 60% worked in the clinical modality and 40% in the clinical and institutional modality. As challenges, we find the improvement in working conditions; the establishment of hierarchical relationships and the difficulty of making the specificities of this field of work understood; the need to improve conditions for continuing education; the practice of the school psychologist as institutional action. In view of the above, we understand that a paradigm shift in the performance of psychologists working in education in that region is necessary, and the appropriation of discussions in the area, especially those presented by Critical School Psychology, contributes to an action that considers social, political, cultural, and pedagogical determinants that constitute the schooling process.(AU)


Este trabajo tiene como objeto de estudio la actuación profesional de las(os) psicólogas(os) en la educación básica. Su objetivo es investigar las prácticas y retos que enfrentan las(os) psicólogas(os) que trabajan en la educación en Boa Vista, en Roraima (Brasil), con la intención de conocer la inserción de estos profesionales en el sistema educativo. Esta es una investigación cualitativa que se guía por el marco teórico-metodológico de la Psicología Escolar Crítica. Se recopilaron los datos entre los meses de marzo y abril de 2018, mediante entrevistas semiestructuradas, grabadas en audio y después transcritas. Las 21 psicólogas y un psicólogo trabajan en instituciones educativas de la ciudad; la mayoría trabajaba en la educación básica; la mitad de los entrevistados ingresaron mediante concurso público y la otra mitad era contratada y de puesto comisionado; pocos fueron contratados como psicólogos escolares. Para el análisis, se seleccionaron diez psicólogas(os) con más tiempo en el cargo. Con respecto a las prácticas, el 60% trabajaba en la modalidad clínica; y el 40%, en la modalidad clínica e institucional. Los retos son mejora de las condiciones laborales; establecimiento de relaciones jerárquicas y dificultad para comprender las especificidades de este campo de trabajo; necesidad de mejorar las condiciones para la educación continua; y actuación dela (del) psicóloga(o) escolar como acción institucional. En vista de lo anterior, es necesario el cambio de paradigma en la práctica profesional de las(os) psicólogas(os) que trabajan en la educación de la región y la apropiación de las discusiones del área, especialmente las presentadas por la Psicología Escolar Crítica, que contribuye a una acción que tiene en cuenta los determinantes sociales, políticos, culturales y pedagógicos que constituyen el proceso de escolarización.(AU)

ABSTRACT

O consumo de medicamentos estimulantes cresceu nos últimos anos, no Brasil e no mundo. Pessoas de diferentes idades, especialmente crianças e adolescentes, passaram a consumir estimulantes como a principal terapêutica utilizada para tratar o transtorno do déficit de atenção e hiperatividade (TDAH). Nesse contexto, estimulantes como as anfetaminas e o metilfenidato, mais conhecidos pelos nomes comerciais de Adderall e Ritalina, ganharam visibilidade social em razão da associação desses psicofármacos ao aperfeiçoamento de funções psíquicas como a atenção e o aumento na qualidade e no tempo de rendimento dos sujeitos nas mais variadas atividades. Com isso, aumentou também a procura desses estimulantes por pessoas que não estão em tratamento médico, mas que buscam aprimorar seu desempenho nas atividades que realizam. Diante desse cenário, o objetivo deste artigo foi demonstrar como o crescimento no consumo de estimulantes, seja por sujeitos em tratamento médico ou não, está relacionado aos processos de socialização hegemônicos nas sociedades capitalistas atualmente. Articulando o contexto apresentado com os conceitos da psicanálise lacaniana, foi possível concluir que o consumo massivo de estimulantes está relacionado aos processos de patologização e medicalização da existência, colocados em movimento por uma articulação entre o discurso médico-científico e o discurso do capitalista na contemporaneidade.(AU)


The consumption of stimulating drugs has grown in recent years in Brazil and worldwide. People of all ages, especially children and adolescents, started to use stimulants as the main therapy used to treat attention deficit hyperactivity disorder (ADHD). In this context, stimulants such as amphetamines and methylphenidate, better known by the trade names Adderall and Ritalin, have gained social visibility due to the association of these psychoactive drugs with the improvement of psychic functions such as attention and the increase in quality and performance time of subjects in the most varied activities. As a result, the demand for these stimulants has also increased by people who are not undergoing medical treatment, but who seek to improve their performance in the activities they perform. Given this scenario, this article aimed to demonstrate how the growth in the consumption of stimulants, whether by subjects undergoing medical treatment or not, is related to the hegemonic socialization processes in capitalist societies today. Articulating the context presented with the concepts of Lacanian psychoanalysis, it was possible to conclude that the massive consumption of stimulants is related to the processes of pathologization and medicalization of existence, set in motion by an articulation between the medical-scientific discourse and the capitalist discourse in contemporary times.(AU)


El consumo de drogas estimulantes ha crecido en los últimos años, en Brasil y en otros países. Diversas personas, especialmente niños y adolescentes, comenzaron a usar estimulantes como la terapia principal utilizada para tratar el trastorno por déficit de atención con hiperactividad (TDAH). En este contexto, los estimulantes como las anfetaminas y el metilfenidato, mejor conocidos por los nombres comerciales Adderall y Ritalina, han ganado visibilidad social debido a la asociación de estas drogas psicoactivas a la mejora de las funciones psíquicas, como la atención y el aumento de la calidad y el tiempo de rendimiento de los pacientes en diversas actividades. Como resultado, la demanda de estos estimulantes también ha aumentado por las personas que no reciben tratamiento médico, pero que buscan mejorar su desempeño en las actividades que realizan. Dado este escenario, el objetivo de este artículo era demostrar cómo el crecimiento en el consumo de estimulantes, ya sea por sujetos que reciben tratamiento médico o no, está relacionado con los procesos de socialización hegemónica en la sociedad capitalista actual. De la articulación del contexto presentado con los conceptos del psicoanálisis lacaniano se concluye que el consumo masivo de estimulantes está relacionado con los procesos de patologización y medicalización de la existencia, puestos en marcha por una articulación entre el discurso médico-científico y el discurso capitalista en los tiempos contemporáneos.(AU)

ABSTRACT

RESUMO. Trata-se de uma pesquisa qualitativa, des critiva e exploratória, realizada no Tratamento Fora de Domicílio, na cidade de Cuiabá-MT, Brasil, que objetivou levantar reflexões sobre os itinerários terapêuticos de pessoas trans, na busca pelo Processo Transexualizador. Participaram três homens trans, duas mulheres trans e uma mulher travesti, com faixa etária de 21 a 32 anos. Os da dos foram coletados por entrevistas semiestruturadas e analisados mediante análise de conteúdo. Os resultados mostram que essas pessoas trans seguem trajetórias diver sas, procurando serviços institucionalizados ou informais (redes de socialidade trans), para a afirmação de suas identidades de gênero. Destacam-se entraves atinentes à patologização, ao acolhimento, à continuidade do cuidado, à resolutividade e à referência na rede de atenção do processo transexualizador. Observaram-se importantes pontos críticos na assistência social, endocrinológica e para a psicologia, sendo a peregrinação pelos serviços de saúde demarcada por constantes discriminações institucionais, permitindo a compreensão de como o sistema de saúde se organiza em relação ao atendimento dessas pessoas, elencando questões para o trabalho da psicologia, nesse campo, a partir de uma perspectiva da experiência e materialidade do gênero.


RESUMEN. Esta es una investigación cualitativa, descriptiva y exploratoria realizada en el tratamiento fuera del domicilio en la ciudad de Cuiabá, Brasil, que tuvo como objetivo plantear reflexiones sobre los itinerarios terapéuticos de las personas trans en la búsqueda del proceso transexual. Participaron tres hombres trans, 2 mujeres trans y 1 mujer travesti de 21 a 32 años. Los datos fueron recogidos a través de entrevistas semiestructuradas y fueron analizados mediante el Análisis de Contenido. Los resultados muestran que estas personas trans siguen caminos divergentes en busca de servicios institucionalizados o informales (redes sociales trans) para afirmar sus identidades de género. Se destacan los obstáculos relacionados con la patologización, acogida, la continuidad de la atención, la resolución y la referencia en la red de atención del Proceso Transexualizador. Se observaron puntos críticos importantes en la asistencia social, la endocrinología y la psicología, em que la peregrinación por los servicios de salud es delimitada por la constante discriminación institucional que permite comprender cómo se organiza el sistema de salud en relación con la atención de estas personas que señalan los problemas para el trabajo de la Psicología en este campo desde una perspectiva de experiencia y materialidad de género


ABSTRACT. This qualitative, descriptive and exploratory research conducted in the Away from Home Treatment (Tratamento Fora de Domicílio [TFD]), in Cuiabá, Brazil, aimed to raise reflections on the therapeutic process itineraries of trans people in the search for the Transsexualizer Process (Processo Transexual [PT]). Three transgender men, two transgender women and one transvestite woman participated. They were aged between 21 and 32 years. Data were collected through semi-structured interviews and analyzed using Content Analysis. The results show that trans people follow different itineraries, looking for institutionalized or informal services (trans sociality networks) to affirm their gender identities. Obstacles related to pathologization, reception, continuity of care, resolution and reference in the care network of the Transsexualizer Process stand out. Important critical points were observed in social, endocrinological and psychological care. The pilgrimage by health services was marked by constant institutional discrimination, allowing for the understanding of how the health system is organized concerning the care of these people, listing issues for the work of Psychology, in this field, from a perspective of the experience and materiality of gender.

ABSTRACT

Hematopathologists are witnessing very exciting times, as a new era of unsurpassed technological advances is unfolding exponentially, enhancing our understanding of diseases at the genomic and molecular levels. In the evolving field of precision medicine, our contributions as hematopathologists to medical practice are of paramount importance. Social media platforms such as Twitter have helped facilitate and enrich our professional  interactions and collaborations with others in our field and in other medical disciplines leading to a more holistic approach to patient care. These platforms also have created a novel means for instantaneous dissemination of new findings and recent publications, and are proving to be increasingly useful tools that can be harnessed to expand our knowledge and amplify our presence in the medical community. In this Editorial, we share our experience as hematopathologists with Twitter, and how we leveraged this platform to boost scholarly activities within and beyond our subspecialty, and as a powerful medium for worldwide dissemination of educational material and to promote our remote teaching activities during the COVID-19 pandemic.

Debido a su fácil accesibilidad, recolección menos invasiva, menos restricciones éticas asociadas con la diferenciación en líneas múltiples más grandes y más rápidas que las de los dientes permanentes, las células madre han ganado mayor notoriedad e inversión en investigación. Actualmente, se ha demostrado que estas células son potenciales en el tratamiento regenerativo de tejidos de numerosas patologías. Caso clínico: Presentar un caso clínico de extracción de dientes con retención prolongada de un niño varón de 7 años como una posible forma de obtener células madre pulpares. Después del diagnóstico y la radiografía, en la que tenía 1/3 raíces, la cirugía fue la opción de tratamiento. Inmediatamente después de la extracción, los dientes se colocaron en un tubo de ensayo con medio de transporte y se enviaron a un laboratorio de cultivo celular. La pulpa dental se retiró con la ayuda de archivo endodóntica y se colocó para cultivo celular. El comienzo de la proliferación comenzó después de 4 días de cultivo. Conclusión: se ha demostrado que las células madre de dientes deciduos son una alternativa terapéutica prometedora, pero comprender la biología de estas células requiere más pruebas in vivo. En este trabajo concluimos que las células de la pulpa dental en dientes con retención prolongada pueden ser una alternativa para obtener células madre.


As células tronco provenientes da polpa de dentes decíduos, em virtude à sua fácil acessibilidade, coleta menos invasiva, poucas restrições éticas, associados com a diferenciação em multilinhagem maior e mais rápida que as dos dentes permanentes, vêm ganhando mais notoriedade e investimentos em pesquisas. Atualmente, essas células tem se mostrado potenciais no tratamento regenerativo tecidual de inúmeras patologias. Caso clinico: apresentar um relato de caso da exodontia de dentes com retenção prolongada, de uma criança do sexo masculino de 7 anos de idade como possível forma de obtenção de células tronco pulpares. Após o diagnóstico clínico e radiográfico, no qual apresentavam 1/3 de raízes, a cirurgia foi opção de tratamento. Imediatamente após a remoção os dentes foram colocados em tubo de ensaio como meio de transporte e encaminhados para laboratório de cultivo celular. A polpa dentária foi retirada com ajuda de lima endodôntica e colocada para cultivo celular. O inicio da proliferação começou após 4 dias de cultivo. Conclusão: As células tronco de dentes decíduos tem se mostrado uma alternativa terapêutica promissora, porém a compreensão da biologia dessas células necessita de mais testes in vivo. Nesse trabalho concluímos que as células da polpa dentária em dentes com retenção prolongada podem ser uma alternativa para obtenção de células tronco.


Due to their easy accessibility, less invasive collection, few ethical restric-tions associated with differentiation into larger and faster multilining than those of permanent teeth, stem cells have gained greater notoriety and investment in research. Currently, these cells have been shown great potential in tissue regenerative treatment of numerous pathologies. Case report: We present a case report of prolonged teeth retention extraction from a 7-year-old male child as a possible way to obtain pulp stem cells. After clinical and radiographic diagnosis, in which it had 1/3 roots, surgery was the treatment option. Immediately after removal, the teeth were placed in a testing tube as a mean of transportation and sent to a cell culture laboratory. The dental pulp was removed with the help of an endodontic file and placed for cell culture. The beginning of proliferation began after 4 days of cultivation. Conclusion: Deciduous teeth stem cells have been shown to be a promising therapeutic alternative, but understanding the biology of these cells requires further testing in vivo. In this paper we concluded that dental pulp cells in teeth in prolonged retention could be an alternative to obtaining stem cells.

ABSTRACT

A busca pela saúde e longevidade atrai cada vez mais olhares da comunidade científica. Há diversas formas de se conseguir alterações fisiológicas que previnam doenças ou colaborem para modificar estados patológicos já existentes no organismo humano. Atividades diferenciadas, como é o caso da dança, têm sido um recurso utilizado na busca da prevenção de doenças e de agravos, devido à proporção terapêutica e à ação sistêmica que aquela exerce, aliada ao seu baixo custo, em com paração com outros recursos de tratamento multidisciplinar. Este artigo trata-se de uma Pesquisa Bibliográfica na modalidade revisão integrativa da literatura, tendo como objetivo, conhecer e demonstrar o que vem sendo publicado na comunidade científica sobre o impacto do exercício da dança na fisiologia humana. Como método, utilizou-se as bases de dados: PUBMED, SCIELO, LILACS, com os seguintes descritores associados: "dança, lterações fisiológicas e terapia através da dança", "dança e terapia através da dança", "dança e alterações fisiológicas", "alterações fisiológicas e terapia através da dança", tendo como critérios de inclusão do material utilizado: artigos originais, disponíveis na íntegra, publicados nas línguas portuguesa, inglesa e espanhola, entre os anos de 2013 a 2018, e que estejam relacionados com as modificações fisiológicas através da dança. Como resultados da busca englobando estas combinações, foram encontrados 2.643 artigos, e destes, foram selecionados para estudo 47 artigos. Conclui-se pelos estudos revisados e organizados no quadro sinóptico, que na relação entre a fisiologia e a dança, há diversas alterações positivas no organismo humano obtidas através da dança, seja como coadjuvante em tratamento, ou seja como fator de prevenção...(AU)


The search for health and longevity attracts more and more views from the scientific community. There are several ways to achieve physiological changes that prevent diseases or collabo rate to modify pathologicalstates already existing in the human body. Differentiated activities as in the case of dance have been a resource used in the search for prevention oficial deseases and aggravation, due to t he therapeutic proportion and systemic action, as well as the low cost compaired to other multidiscip linary treatment resources. This article is about a bibliographical research in the integrative literature review modality, with the aiming to know and demonstrate what has been published in the scientific comm unity about the impact of dance exercise on human physiology. As a method, the following databases were used: PUBMED, SCIELO, LILACS, with the following associated descriptors: "dance, physiological chan ges and therapy through dance", "dance and therapy through dance" and "dance and physiological ch anges" , "Physiological changes and therapy through dance", with the following inclusion criteria: original articles, available in full mode, published in Portuguese, English and Spanish languages, between the years 2013 to 2018, and that have been related to the physiological changes through the dance. As search results encompassing these combinations were found 2.643 articles, and seleções for study 4 7 articles. It was conclude from the reviewed studies that were organized in the synoptic table, that the relationship between physiology and dance, there are several positive changes in the human organism obtained through dance, either as an adjunct in treatment, or as a prevention factor...(AU)

Based on the three books of - (1931), - (1940) and - (1955) written by Mr. -, the classification of facial diseases as well as the records and evolution process of Chinese and western disease names are summarized and analyzed to discuss Mr. -'s understanding of facial diseases in different periods. Through the systematic analysis and comparison in the trilogy of acupuncture and moxibustion, the characteristics of syndrome differentiation and diagnosis-treatment of acupuncture-moxibustion treatment for facial diseases by Mr. - are summarized, including clinical syndrome differentiation and treatment, which is adjusted with syndrome changes; simplified selection of acupoints, with attention on empirical acupoints; the strength of acupuncture is based on efficacy; acupuncture and moxibustion has specific indication; combination of acupuncture and medication could bring out the best in each other.

ABSTRACT

Charcot-Marie-Tooth disease (CMT) is the commonest form of inherited neuropathy and has an incidence of 1/2500. CMT1A is the commonest subtype of CMT, which is caused by duplication of peripheral myelin protein 22 (PMP22) gene and accounts for approximately 50% of CMT diagnosed by genetic testing. Duplication of PMP22 may influence the production of PMP22 mRNA and protein, and interfere with the proliferation, differentiation and apoptosis of Schwann cells. In addition, deregulation of NRG1/ErbB pathway and lipid metabolism can also lead to dysfunction of Schwann cells. Such factors may disturb the myelination process, leading to axon degeneration, muscle weakness, and atrophy subsequently. Accordingly, drug therapies for CMT1A are developed by targeting such factors. PXT3003, antisense oligonucleotides (ASOs) and small interfering RNA (siRNA) are supposed to down-regulate the level of PMP22 mRNA, while recombinant human NRG-1 (rhNRG1) and neurotrophin-3 (NT-3) may enhance Schwann cells survival and differentiation. In addition, lipid-supplemented diet may remedy the defect of lipid metabolism and maintain the proper structure of myelin. Other targeting drugs include ascorbic acid, progesterone antagonists, IFB-088, ADX71441, and ACE-083. This review is to sum up the pathogenesis of CMT1A and promising targeting drug therapies for further research.

ABSTRACT

In routine regulatory toxicology studies, the anatomic pathology endpoints are frequently the most significant element of the study data. They may profoundly influence subsequent clinical development and use of a test article, with implications for both human safety and for the fate of key commercial assets. Unfortunately (in common with other perceptual medical specialties), anatomic pathology data are also among the most subjective endpoints in regulatory toxicology studies - a challenge magnified by the fact that not only the diagnostic data but the anatomic pathologist's interpretation of it in their narrative report represent raw data within a regulated study (United States Federal Register, 1987). A strategy for minimizing and managing the risk of misdiagnosis/misinterpretation of pathology data is critical for any preclinical toxicology development program and is a collaborative approach between study directors, study monitors and toxicologists and toxicologic pathologists. The article provides a basic understanding of the sources of error and limitations of anatomic pathology evaluation, a starting point for troubleshooting and a basis for a sound management strategy. It describes common reasons for unexpected or inconsistent pathology findings and sets out to provide a framework for toxicologists to approach commissioning and critically evaluating their pathology data, and for identifying situations where additional third-party advice and review may be justified.

ABSTRACT

La terapia de oxigenación hiperbárica (TOHB) es usada en el tratamiento de diferentes patologías y es prioritario conocer su injerencia en la bioquímica del paciente y establecer el perfil de seguridad. Con el objetivo de establecer la seguridad y conocer la bioquímica de seguimiento de pacientes que realizan el tratamiento, se realizó un estudio retrospectivo de individuos sanos evaluando el efecto de la TOHB (100% O2 a 1,4 atm) sobre algunos parámetros bioquímicos. Se estudió el estado basal, y a las 10 y 20 sesiones, de los valores de los indicadores de coagulación, hematológicos, función hepática y reactantes de fase aguda. En los 20 individuos sanos no hubo alteraciones en el examen clínico general debido a la exposición a la TOHB. La media (con DE) de los valores bioquímicos a las sesiones 0, 10 y 20 estuvieron siempre dentro del rango de valores de referencia. Estos resultados sugieren que el tratamiento en estas condiciones no produce cambios estadísticamente significativos en los parámetros estudiados.


Hyperbaric oxygenation therapy (HBOT) is used in the treatment of different pathologies and it is a priority to establish its safety profile. A retrospective study in healthy individuals was performed, evaluating the effect of HBOT at 1.4 ATA on biochemical indicators. The general and basal status and after 10 and 20 sessions of biochemical parameters in blood sample, on the biochemical markers of coagulation, hematologic values, hepatic function status, and acute reaction were analysed as well. For the 20 healthy subjects, there were no alterations in the general clinical examination due to exposure to HBOT. The mean values (with SD) of the biochemical parameters at 0, 10 and 20 sessions were mostly within reference values range. The results suggest that the treatment does not produce statistically significant changes in the biochemical parameters studied in these conditions.


A terapia de oxigenação hiperbárica (TOHB) é utilizada no tratamento de diferentes patologias e é prioritário conhecer sua ingerência na bioquímica do paciente e estabelecer o perfil de segurança. A fim de estabelecer segurança e conhecer a bioquímica de monitoramento de pacientes que realizam o tratamento, foi realizado um estudo retrospectivo de indivíduos saudáveis avaliando o efeito da OHB (100% O2 a 1,4 atm) em alguns parâmetros bioquímicos. O estado basal foi estudado e em 10 e 20 sessões os valores de coagulação, hematológicos, função hepática e reagentes de fase aguda foram estudados. Nos 20 indivíduos saudáveis, não houve alterações no exame clínico geral devido à exposição à TOHB. A média (com o DS) dos valores bioquímicos nas sessões 0, 10 e 20 estavam sempre dentro da faixa de valores de referência. Estes resultados sugerem que o tratamento sob estas condições não produz mudanças estatisticamente significativas ao nível dos parâmetros estudados.

ABSTRACT

GlaxoSmithKline has recently made significant organizational changes to its nonclinical safety, drug metabolism and pharmacokinetic, and laboratory animal science/veterinary functions, with the goal to increase our focus on scientific partnership with the discovery part of the organization. One specific change was bringing together pathologists and comparative medicine veterinarians and scientists into a single functional unit. We describe our early activities (assessing our capabilities and gaps, external benchmarking, listening to our discovery partners, redesigning some of our working practices) aimed at implementing these changes. In addition, early on we held a Discovery Engagement Workshop attended by all pathologists and comparative medicine veterinarians and scientists, as well as selected discovery scientists. The purpose of this workshop was to share learnings from the above activities and devise plans aimed at achieving our overall goal of functional integration: driving pathobiology expertise into drug discovery and increasing the human (translational) relevance of experimental data. This review describes the new organizational structure, the workshop activities, and implementation plans; updates our progress; and considers the opportunity for a pan-industry network of discovery-focused pathologists and comparative medicine veterinarians and scientists.

Avaliar perfil, estado nutricional, índices antropométricos e variação de peso de pacientes idosos atendidos em um ambulatório de nutrição do Sul do Brasil. Material e métodos: Estudo descritivo, realizado de 2014 a 2017. A avaliação foi feita através da análise de dados secundários de prontuários dos pacientes. Foram avaliados na primeira consulta: idade, sexo, patologias, história familiar, altura, hábitos de vida, funcionamento intestinal e estilo de vida. Na primeira e na última consulta: data dos atendimentos, prática e tempo de exercício físico, peso, circunferência da cintura e do pescoço, sendo calculados: número de consultas no período, tempo de intervenção, IMC, índice de conicidade, porcentagem de variação de peso e de perda de excesso de peso entre a primeira e a última consulta. As análises estatísticas foram realizadas no Stata® 11.1 com nível de significância (p<0,05). Resultados: A amostra foi constituída por 68 idosos, essencialmente composta por mulheres (63,24%), com ensino fundamental completo (32,35%) ou incompleto (32,35%), casada (57,35%) e de cor branca (79,41%). A maioria apresentava hipertensão (70,59%), sedentarismo (66,18%), estava acima do peso ideal (82,35%) e com valores acima do recomendado de circunferência da cintura (94,11%), pescoço (89,70%) e do índice de conicidade (97,05%). 82,98% apresentou perda de peso, sendo que 11,77% das mulheres apresentaram perda de mais de 5% do peso entre a primeira e a última consulta. Conclusão: O atendimento ambulatorial nutricional mostrou-se efetivo para promoção da redução das medidas antropométricas, comprovando a necessidade e eficácia da presença do profissional de nutrição neste modelo de atendimento (AU)


To evaluate the profile, nutritional status, anthropometric indices and weight variation of elderly patients attended at a nutrition clinic in the South of Brazil. Material and methods: Descriptive study, carried out from 2014 to 2017. The evaluation was made through the analysis of secondary data of patients' medical records. They were evaluated at the first consultation: age, sex, pathologies, family history height, life habits, intestinal functioning and lifestyle. In the first and last visit: date of the visits, practice and time of physical exercise, weight, waist circumference and neck, being calculated: number of visits in the period, intervention time, BMI, conicity index, percentage of variation weight and loss of excess weight between the first and last consultation. Statistical analyzes were performed on Stata® 11.1 with significance level (p <0.05). Results: The sample consisted of 68 elderly individuals, consisting essentially of women (63.24%), with complete primary education (32.35%) or incomplete (32.35%), married (57.35%) and color (79.41%). The majority had hypertension (70.59%), sedentary lifestyle (66.18%), was above ideal weight (82.35%) and had values above the recommended waist circumference (94.11%), neck (89, 70%) and the conicity index (97.05%). 82.98% presented weight loss, and 11.77% of the women presented loss of more than 5% of the weight between the first and the last consultation. Conclusion: Nutritional outpatient care was effective to promote the reduction of anthropometric measures, proving the need and effectiveness of the presence of the nutrition professional in this model of care

ABSTRACT

BACKGROUND: Pain, limitations in opening, asymmetrical jaw movements, and temporomandibular joint (TMJ) sounds are the most common findings in temporomandibular joint disorders (TMDs), which causes excruciating pain, inflammation of the surrounding muscles, posterior fibers, and synovial fluid. This study aimed to evaluate and compare the effects of ultrasound heat therapy and low-level laser therapy (LLLT) in reducing TMD-related pain. METHODS: This prospective study included 42 patients (age range, 25–45 years), who were divided into two groups of 21 patients each. All patients were prescribed a non-steroidal anti-inflammatory drug (NSAID) twice a day for 5 days for temporary relief of pain prior to the commencement of treatment. Patients were kept on a soft diet and asked to restrict mouth opening during the same period. Fifteen sessions of LLLT (Group A) or ultrasound therapy (Group B) were administered to the affected side. RESULTS: Post-therapy, the mean visual analog scale score for group A and group B was 4.81 (2.01) and 6.19 (1.20), respectively; the difference was statistically significant and favoring the LLLT group. Similarly, the mean mouth opening for group A and group B was 3.99 (0.40) and 3.65 (0.41), respectively; the difference was statistically significant and favoring the LLLT group. CONCLUSION: Our study recommends LLLT for treating TMD-related pain with no underlying bony pathology.

ABSTRACT

We investigated the anti-inflammatory effect of Pyunkang-tang extract (PGT), a complex herbal extract based on traditional Chinese medicine that is used in Korea for controlling diverse pulmonary diseases, on cigarette smoke-induced pulmonary pathology in a rat model of chronic obstructive pulmonary disease (COPD). The constituents of PGT were Lonicerae japonica, Liriope platyphylla, Adenophora triphilla, Xantium strumarinum, Selaginella tamariscina and Rehmannia glutinosa. Rats were exposed by inhalation to a mixture of cigarette smoke extract (CSE) and sulfur dioxide for three weeks to induce COPD-like pulmonary inflammation. PGT was administered orally to rats and pathological changes to the pulmonary system were examined in each group of animals through measurement of tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) levels in bronchoalveolar lavage fluid (BALF) at 21 days post-CSE treatment. The effect of PGT on the hypersecretion of pulmonary mucin in rats was assessed by quantification of the amount of mucus secreted and by examining histopathologic changes in tracheal epithelium. Confluent NCI-H292 cells were pretreated with PGT for 30 min and then stimulated with CSE plus PMA (phorbol 12-myristate 13-acetate), for 24 h. The MUC5AC mucin gene expression was measured by RT-PCR. Production of MUC5AC mucin protein was measured by ELISA. The results were as follows: (1) PGT inhibited CSE-induced pulmonary inflammation as shown by decreased TNF-α and IL-6 levels in BALF; (2) PGT inhibited the hypersecretion of pulmonary mucin and normalized the increased amount of mucosubstances in goblet cells of the CSE-induced COPD rat model; (3) PGT inhibited CSE-induced MUC5AC mucin production and gene expression in vitro in NCI-H292 cells, a human airway epithelial cell line. These results suggest that PGT might regulate the inflammatory aspects of COPD in a rat model.

ABSTRACT

An observational study was conducted at the Section of Clinical Chemistry, Department of Pathology and Laboratory Medicine, to assess the iodine status using the World Health Organization (WHO), United Nations International Children's Emergency Fund (UNICEF), and the International Council for Control of Iodine Deficiency Disorders (ICCIDD) consensus criteria, which state that >3% prevalence of serum thyroid stimulating hormone (TSH) ≥10 mIU/L in the population is an indicator of iodine deficiency. Serum neonatal TSH was analyzed from January to December 2013. In a period of one year, a total of 11,597 neonates with the mean (25 percentile, 75 percentile value) age of 2.0 days (0.5–3.5) were tested for serum TSH. The overall mean TSH level was 3.38 mIU/L (5.63–1.96), with optimal levels (1–39 mIU/L) in 93%, <1 mIU/L in 6.3%, and ≥40 mIU/L in 0.3% neonates. Of all the neonates, 7.9% (N=916) showed TSH ≥10 mIU/L which is higher than the recommended WHO/UNICEF/ICCIDD criteria for mild endemicity for iodine deficiency in the population. These results suggest that iodine deficiency is still prevalent in our population, indicating a need for effective intervention programs and increasing awareness regarding the use of iodized salt and supplementation in all reproductive-aged women to prevent iodine deficiency in neonates.

ABSTRACT

OBJECTIVE@#To study the preventive and therapeutic effects of safflower water extract on systemic scleroderma (SSc) in mice and its mechanism.@*METHODS@#Sixty BALB/C mice were randomly divided into the control group, model group, prednisone group and safflower low, middle, high dose groups, 10 mice in each group.The control group was injected with normal saline, and the other five groups were subcutaneously injected with bleomycin hydrochloride with 100 μl at the concentration of 200 μg /ml on the back, once a day for 28 days to establish the SSc models.At the same time, the control group and model group were treated with normal saline (10 ml/kg), the prednisone group was treated with prednisone 4.5 mg/kg (10 ml/kg), and the low, middle, and high dose safflower groups were treated with safflower at the doses of 1.5, 3, 6 g/kg (10 ml/kg), and all groups were treated for 28 days.After 28 days, all mice were decapitated. The blood samples and back skin of the BLM injection part were collected.After that, all the tissue slices were taken to measure the dermal thickness, and the content of hydroxyproline (HYP) in the skin tissues was detected by hydrolysis method.The contents of tissue growth factor (CTGF) and transforming growth factor-β (TGF-β ) in the skin tissues and the levels of interleukin-6 (IL-6) and interleukin-17 (IL-17) in serum were determined by ELISA.@*RESULTS@#Compared with the control group, the dermal thickness of the model group was increased(P<0.05), the contents of CTGF, TGF-β and HYP in the skin tissues and the levels of IL-6 and IL-17 in the serum of the model group were increased(P<0.05); compared with the model group, the dermal thickness in the prednisone group and safflower groups was decreased (P<0.05), the levels of CTGF, TGF-β and HYP in the skin tissues and the serum levels of IL-6 and IL-17 in the prednisone group and safflower groups were decreased (P<0.05).@*CONCLUSION@#Safflower water extract can improve skin condition (or dermal thickness) in SSc mice, and its mechanism may be related to reducing immune inflammatory response.
