RESUMEN

Arm movements modulate leg activity and improve gait efficiency; however, current rehabilitation interventions focus on improving walking through gait-specific training and do not actively involve the arms. The goal of this project was to assess the effect of a rehabilitation strategy involving simultaneous arm and leg cycling on improving walking after incomplete spinal cord injury (iSCI). We investigated the effect of 1) non-gait-specific training and 2) active arm involvement during training on changes in over ground walking capacity. Participants with iSCI were assigned to simultaneous arm-leg cycling (A&L) or legs only cycling (Leg) training paradigms, and cycling movements were assisted with electrical stimulation. Overground walking speed significantly increased by 0.092 ± 0.022 m/s in the Leg group and 0.27 ± 0.072m/s in the A&L group after training. Whereas the increases in the Leg group were similar to those seen after current locomotor training strategies, increases in the A&L group were significantly larger than those in the Leg group. Walking distance also significantly increased by 32.12 ± 8.74 m in the Leg and 91.58 ± 36.24 m in the A&L group. Muscle strength, sensation, and balance improved in both groups; however, the A&L group had significant improvements in most gait measures and had more regulated joint kinematics and muscle activity after training compared with the Leg group. We conclude that electrical stimulation-assisted cycling training can produce significant improvements in walking after SCI. Furthermore, active arm involvement during training can produce greater improvements in walking performance. This strategy may also be effective in people with other neural disorders or diseases. NEW & NOTEWORTHY This work challenges concepts of task-specific training for the rehabilitation of walking and encourages coordinated training of the arms and legs after spinal cord injury. Cycling of the legs produced significant improvements in walking that were similar in magnitude to those reported with gait-specific training. Moreover, active engagement of the arms simultaneously with the legs generated nearly double the improvements obtained by leg training only. The cervico-lumbar networks are critical for the improvement of walking.

Introducción: esta propuesta se centra en el papel maternal, donde la madre constituye un proceso de interacción y desarrollo. Cuando la madre logra el apego de su hijo, adquiere experiencia en su cuidado y experimenta la alegría y la gratificación de su papel. Mercer abarca varios factores maternales: edad, estado de salud, relación padre-madre y características del lactante. Además define la forma en que la madre percibe los acontecimientos vividos que influirán al momento de llevar los cuidados a su hijo con características innatas de su personalidad. Materiales y Métodos: Reporte de caso de una Usuaria con 30 años de edad, asiste a la unidad Materno Infantil por control de Ginecobstetricia donde se ordena ecografía obstétrica la cual reporta oligoamnios; es trasladada a cirugía para extracción de producto de sexo masculino por cesárea. En el puerperio tardío presenta dehiscencia de la herida quirúrgica. Resultados: Son diversas las actividades que desarrolla la enfermera para lograr cada vez más una mejor situación de salud y por ende una mejor calidad de vida, con formación científica cumpliendo de ese modo con los criterios de la atención integral que implican la atención de lo físico, mental y espiritual. Discusión y Conclusiones: Es importante recalcar el cuidado materno perinatal de la profesión de enfermería que ejerce el rol desde diversos campos de acción y en las diferentes etapas de la procreación. La aplicación de esta teoría a la práctica asistencial sirvió como marco para la valoración, planificación, implantación y evaluación del cuidado del binomio madre – hijo, ayudando a simplificar un proceso muy complejo y facilitando su comprensión para la aplicación del mismo en la práctica.


Introduction: this proposal focuses on the maternal role, where the mother is a process of interaction and development. When a mother does her child's attachment, gain experience in their care and experience the joy and gratification of their role. Mercer covers several maternal factors: age, health status, father-mother and infant characteristics. It also defines how the mother perceives life events that influence the time to take care of your child with their innate characteris tics of her personality. Materials and Methods: Case report of a user with 30 years of age, attending the maternal and child unit by control of gynecology where obstetric ultrasound is ordered, which reports oligohydramnios, she is taken to surgery for male product extraction by cesarean. In the late puerperium presented dehiscence of the wound surgical. Results: A variety of activities carried out by the nurse to achieve increasingly better health status and thus a better quality of life, with scientific training thus meeting the criteria of comprehensive care involving the physical, mental and spiritual. Discussion and Conclusions: It is important to emphasize maternal and perinatal care in the nursing profession's role exerted from various fields of action and at different stages of procreation. The application of this theory to clinical practice served as a framework for assessment, planning, implementation and evaluation of care of the mother - child, helping to simplify a very complex process and thus facilitate understanding for its application in practice.

Lead intoxication risks were studied in a community of ceramic folk art workers in Oaxaca, a southern state of Mexico, where the manufacture of low-temperature lead ceramic ware is a family tradition and often the only source of income. Variables such as household characteristics, occupation, and lead exposure risk factors were explored. Study participants' mean blood lead concentration was 43.8 microg/dl (range=8.4-99.6 microg/dl), which is over the WHO guideline of 40 microg/dl for removing workers from exposure and is the concentration over which renal damage is accelerated. Best predictors for high blood lead concentrations by multivariable regression analysis were: occupation (P<0.0001), gender (P=0.0002), and the use of glazed stoneware (P<0.0001). This model explained 18% of blood lead variation among the study group. Exposure appears to be primarily associated with antiquated pottery manufacturing techniques and the high degree of contamination prevailing at the production sites, which in most cases are their living quarters. This consequently affects the lead levels of the entire community.
