RESUMEN

Forest type and stand age are important biological factors affecting soil enzyme activities. However, the changes in soil enzyme activities across stand ages and underlying mechanisms under the two forest restoration strate-gies of plantations and natural secondary forests remain elusive. In this study, we investigated the variations of four soil enzyme activities including cello-biohydrolase (CBH), ß-1,4-glucosidase (ßG), acid phosphatase (AP) and ß-1,4-N-acetylglucosaminidase (NAG), which were closely associated with soil carbon, nitrogen, and phosphorus cycling, across Cunninghamia lanceolata plantations and natural secondary forests (5, 8, 21, 27 and 40 years old). The results showed that soil enzyme activities showed different patterns across different forest types. The acti-vities of AP, ßG and CBH in the C. lanceolata plantations were significantly higher than those in the natural secon-dary forests, and there was no significant difference in the NAG activity. In the plantations, AP activity showed a decreasing tendency with the increasing stand ages, with the AP activity in the 5-year-old plantations significantly higher than other stand ages by more than 62.3%. The activities of NAG and CBH decreased first and then increased, and ßG enzyme activity fluctuated with the increasing stand age. In the natural secondary forests, NAG enzyme activity fluctuated with the increasing stand age, with that in the 8-year-old and 27-year-old stand ages being significantly higher than the other stand ages by more than 14.9%. ßG and CBH enzyme activities increased first and then decreased, and no significant difference was observed in the AP activity. Results of the stepwise regression analyses showed that soil predictors explained more than 34% of the variation in the best-fitting models predicting soil enzyme activities in the C. lanceolata plantations and natural secondary forests. In conclusion, there would be a risk of soil fertility degradation C. lanceolata plantations with the increasing stand age, while natural secondary forests were more conducive to maintaining soil fertility.

RESUMEN

A 50-year-old man with a 15-year history of elevated blood glucose and an approximately 2-year history of diarrhea was admitted to the Peking Union Medical College Hospital. The initial diagnosis was type 2 diabetes. After repeated pancreatitis and pancreatoduodenectomy, severe pancreatic endocrine and exocrine dysfunction including alternating high and low blood glucose and fat diarrhea occurred. Tests for type 1 diabetes-related antibodies were all negative, C-peptide levels were substantially reduced, fat-soluble vitamin levels were reduced, and there was no obvious insulin resistance. Therefore, a diagnosis of pancreatic diabetes was clear. The patient was given small doses of insulin and supplementary pancreatin and micronutrients. Diarrhea was relieved and blood glucose was controlled. The purpose of this article is to raise clinicians' awareness of the possibility of pancreatic diabetes after pancreatitis or pancreatic surgery. Timely intervention and monitoring may reduce the occurrence of complications.

Drug instructions,the statutory and technical documents recording effectiveness and safety information,are an important basis for guiding doctors,pharmacists,and patients to use drugs rationally,and their scientificity,standardization,and accuracy directly affect the medication safety of the public. The sections of adverse drug events,contraindications,precautions,warnings,and application for specific populations in drug instructions directly express safety information and measures for rational use of drugs. In the drug life cycle,marketing authorization holders( MAHs) need to update safety information in the instructions promptly to ensure the safety and effectiveness of clinical drug medication. At present,revising instructions is an important measure to control drug risks. In the drug life cycle,in order to standardize the revision of safety information in the instructions by MAHs and eliminate inexact terms such as " unclear",the Technical Specifications for Revision of Safety Information in Marketed Chinese Patent Medicine Instructions,a series of group standards,have been established under the guidance of Standardization Department,China Association of Chinese Medicine. Therefore,on the basis of the existing rules and regulations,the standardized technical procedures for revising instructions came into being to help clinical safe and rational medication of drugs,and implement the strategy of " Healthy China".

RESUMEN

Technical Specifications for Revision of Safety Information in Marketed Chinese Patent Medicine Instructions,a series of group standards,were proposed by Professor ZHANG Bing from Research Center for Pharmacovigilance and Rational Use of Traditional Chinese Medicine,and underwent centralized management by Chinese Association of Chinese Medicine. They were officially released on July 23 and implemented on July 31,2021. The series of group standards consist of six sections,including general principles,adverse drug events,contraindications,precautions,application for special populations,and warnings. The section of general principles is comprised of holistic and programmatic expressions,which explain the general technical requirements for revising the marketed Chinese patent medicine instructions. The other five sections focus on information collection,screening,transformation,and illustration of specific items,forming a standardized revision technical process. This series of standards is the result of multiple rounds of research and the suggestions of more than 200 experts in different professional fields of " medicine-pharmacy-management-law-enterprise" have been gathered therein to reach a consensus. With the purposes of establishing standardized technical specifications for the revision of safety information in the marketed Chinese patent medicine instructions,guiding marketing authorization holders in revising the instructions,filling the gaps in the research of Chinese patent medicine instructions,promoting the deve-lopment of pharmaceutical care and academic research,and encouraging the rational and safe medication of Chinese patent medicine,the series of group standards is of great significance.

RESUMEN

Qingjin Huatan Decoction is a classic prescription with the effects of clearing heat, moistening lung, resolving phlegm, and relieving cough. In order to explore the critical quality attributes of Qingjin Huatan Decoction, we identified the blood components of Qingjin Huatan Decoction by ultra-performance liquid chromatography quadrupole time of flight mass spectrometry(UPLC-Q-TOF-MS) under the following conditions, chromatographic column: Acquity UPLC BEH C_(18) column(2.1 mm×100 mm, 1.7 μm); mobile phase: 0.1% formic acid acetonitrile(A)-0.1% formic acid in water(B); gradient elution; flow rate: 0.2 mL·min~(-1); column temperature: 30 ℃; injection volume: 5 μL. The electrospray ionization(ESI) source was used to collect data in both positive and negative ion modes under the following conditions, capillary voltage: 3 kV for the positive ion mode and 2 kV for the negative ion mode; ion source temperature: 110 ℃; cone voltage: 30 V; cone gas flow rate: 50 L·h~(-1); nitrogen degassing temperature: 350 ℃; degassing volume flow rate: 800 L·h~(-1); scanning range: m/z 50-2 000. In this experiment, a total of 66 related components of Qingjin Huatan Decoction were identified, including 22 prototype components and 44 metabolites. The results of this study preliminarily revealed the pharmacodynamic material basis of Qingjin Huatan Decoction in vivo, which has provided an experimental basis for the determination of quality markers of Qingjin Huatan Decoction and the development of new drugs.

RESUMEN

A high-throughput screening machine learning model for mitochondrial function was constructed, and compounds of Aco-niti Lateralis Radix Praeparata were predicted. Deoxyaconitine with the highest score and benzoylmesaconine with the lowest score among the compounds screened by the model were selected for mitochondrial mechanism analysis. Mitochondrial function data were collected from PubChem and Tox21 databases. Random forest and gradient boosted decision tree algorithms were separately used for mo-deling, and ECFP4(extended connectivity fingerprint, up to four bonds) and Mordred descriptors were employed for training, respectively. Cross-validation test was carried out, and balanced accuracy(BA) and overall accuracy were determined to evaluate the performance of different combinations of models and obtain the optimal algorithm and hyperparameters for modeling. The data of Aconiti Lateralis Radix Praeparata compounds in TCMSP database were collected, and after prediction and screening by the constructed high-throughput screening machine learning model, deoxyaconitine and benzoylmesaconine were selected to measure mitochondrial membrane potential, reactive oxygen species(ROS) level and protein expression of B-cell lymphoma 2(Bcl-2), Bcl-2-associated X protein(Bax) and peroxisome proliferator-activated receptor-γ-coactivator 1α(PGC-1α). The results showed that the model constructed using gradient boosted decision tree+Mordred algorithm performed better, with a cross-validation BA of 0.825 and a test set accuracy of 0.811. Deoxyaconitine and benzoylmesaconine changed the ROS level(P<0.001), mitochondrial membrane potential(P<0.001), and protein expression of Bcl-2(P<0.001, P<0.01) and Bax(P<0.001), and deoxyaconitine increased the expression of PGC-1α protein(P<0.01). The high-throughput screening model for mitochondrial function constructed by gradient boosted decision tree+Mordred algorithm was more accurate than that by random forest+ECFP4 algorithm, which could be used to build an algorithm model for subsequent research. Deoxyaconitine and benzoylmesaconine affected mitochondrial function. However, deoxyaconitine with higher score also affected mitochondrial biosynthesis by regulating PGC-1α protein.

The quality of Chinese materia medica is the premise to ensure its safety and effectiveness in clinical application, and the standardization of Chinese materia medica quality is the most important to realize the sustainable development of traditional Chinese medicine (TCM). At present, the quality control system of Chinese materia medica has been transformed from a single chemical evaluation to the overall quality control guided by clinical efficacy. However, some quality control items of decoction pieces are still lacking or imperfect in the drug standard of prescription, which makes it difficult to guarantee the effectiveness and safety of Chinese materia medica in clinical application. Based on this, the quality control models and innovative ideas of Chinese materia medica were analyzed and discussed from the perspectives of chemical analysis, biological evaluation and clinical application in this paper. Aiming at the existing problems and actual needs in the control system of Chinese materia medica, this paper proposed the improvement strategies in accordance with the characteristics of TCM, in order to provide theoretical basis for the related research on quality control of Chinese materia medica.

This study systematically combed and analyzed the use of such terms as "prohibition", "contraindication", and "use with caution" in ancient Chinese books on materia medica and prescriptions, Pharmacopoeia of the People's Republic of China(《中华人民共和国药典》), and teaching materials and monographs of Chinese materia medica by means of hermeneutics, commentariology, textology, and data mining, and explored the historical evolution and conceptual connotations of "prohibition", "contraindication", and "use with caution" in Chinese materia medica, so as to provide reference for standardizing their understandings and clinical reasonable medication. The "prohibition", "contraindication", and "use with caution" of Chinese materia medica were first proposed in the period of pre-Qin and Han dynasties. "Prohibition" and "contraindication" were separately developed in the Sui, Tang, and Five dynasties and Song, Jin, and Yuan dynasties and have been widely used since the Ming and Qing dynasties. The "use with caution" becomes popular rapidly in modern times and is often present in clinical medication together with "prohibition" and "contraindication". "Prohibition" basically means strictly prohibited and "contraindication" means to avoid as much as possible. The terms "prohibition", "contraindication", and "use with caution" have experienced evolution from the pre-Qin period to modern times, and they are used to describe the degree of restriction on drug use("prohibition" > " contraindication" > "use with caution").

Lonicerae Japonicae Flos, as common Chinese medicine, has been used for thousands of years in the treatment of inflammation and infectious diseases with definite efficacies. The complex composition of Lonicerae Japonicae Flos results in its extensive pharmacological effects, so the assessment of its quality by only a few index components is not comprehensive. Guided by the quality marker(Q-marker), the present study comprehensively analyzed and predicted the quality connotation of Lonicerae Japonicae Flos based on the chemical composition and component transfer, the phylogenetic relationship, chemical composition effectiveness, measurability, and specificity. Chlorogenic acid, isochlorogenic acids A, B, and C, luteoloside, rutin, sweroside, and secoxyloganin were predicted as candidate Q-markers of Lonicerae Japonicae Flos.

Tumors are new organisms formed by uncontrollable cell proliferation of local tissues driven by various oncogenic factors. The cause of tumors is unknown with life-threating outcome. Tumors can be classified into benign tumors, borderline tumors, and malignant tumors according to their pathological properties. Among them, malignant tumor is commonly known as cancer, with no specific medicines or reliable cure means, so this is a hot spot and difficult point in current medical research. In ancient literatures, there are many records about the efficacy of Chinese herbal medicine in treating tumor, and modern pharmacological researches have shown that more and more active ingredients of traditional Chinese medicine(TCM) have gradually highlighted their inhibitory effect on various types of tumor. Caulis sinomenii has been used for treatment of rheumatic diseases in TCM for a long history. Sinomenine is a major bioactive alkaloid presented in C. sinomenii, which has demonstrated a wide range of pharmacological activities such as anti-inflammation, immunosuppression, analgesia and sedation, and due to its slightly soluble in water, it is commonly used in clinic in the form of hydrochloride, with its commercial name of Zhengqing Fengtongning. Recent studies show that sinomenine alone or combined with chemoradiotherapy can inhibit growth of several tumors significantly or in a synergistic way, so it is termed as an inhibitor of tumors. Anti-tumor effect of sinomenine involve inhibition of tumor cell proliferation, induction of tumor cell apoptosis, blockade of tumor cell cycle, suppression of tumor invasion and metastasis, induction of autophagy of tumor cells, and reversal of multidrug resistance of tumor cells. Upon combination with nanomaterials, it can enhance efficiency and reduce toxicity. Here we summarized and reviewed recent advances on basic anti-tumor research of sinomenine, and then made a classification and description according to its in vivo and in vitro pharmacological action and mechanism of action, so as to elucidate the great potential of sinomenine as a promising anti-tumor drug, and provide reference for further research on its anti-tumor mechanism.

In view of the current controversy in the clinical evaluation of the terms of 'prohibition' 'contraindication', and 'use with caution', the present study summarized their evaluation elements to provide references for the classification of 'prohibition' 'contraindication', and 'use with caution' and clinical rational medication of drugs. Based on the ancient and modern representative traditional Chinese medicine (TCM) literature, such as the records on herbal medicines and prescriptions, medical classics, pharmacopoeia, clinical monographs, and papers, this study proposed the evaluation elements and the underlying ideas of 'prohibition' 'contraindication', and 'use with caution' around the risks and benefits of medication. The results indicate that the evaluation elements of 'prohibition' 'contraindication', and 'use with caution' include TCM property,syndrome,symptom, TCM compatibility,dosage,and treatment course. When evaluating 'prohibition' 'contraindication', and 'use with caution' of TCM under specific conditions of medication,we can determine the properties of prohibited or contraindicated drugs prior to figuring out the differences in 'prohibition' 'contraindication', and 'use with caution'. It is feasible to evaluate the clinical 'prohibition' 'contraindication', and 'use with caution' in TCM from Chinese medicine, body, and the clinical medication, which are correlated with each other in the practice implementation.

The small size, moderate drug loading, and targeting properties of nano-preparations make them can be excellent delivery tools for drugs, genes or proteins crossing the cell or blood-brain barrier (BBB). Currently, facilitating drug crossing BBB with innovative nano-drug delivery systems is considered as a strategic approach for the prevention, diagnosis and treatment of central nervous system (CNS) diseases. However, with the deepening of the research, the adverse reactions and toxicity of nanocarriers have gradually attracted the attention of researchers. Based on this, this paper summarized the situation of BBB-penetrating targeted nano-preparations at home and abroad in recent years from the perspective of classification of types and properties of nanocarriers, and analyzed the advantages and disadvantages of each carrier. The results showed that nano-preparations with active ingredients of traditional Chinese medicine (TCM) as carriers have become a promising way of cancer treatment, but the complexity and diversity of TCM components limited its application to a certain extent. Further studies should be strengthened to lay a foundation for the application and development of TCM nano-preparations in the field of CNS diseases.

Nardostachyos Radix et Rhizoma is the dry root and rhizome of Nardostachys jatamansi (Valerianaceae) with a long medical history and a broad range of application, which is effective in regulating Qi, relieving pain, resolving depression, and enlivening spleen, as well as dispelling dampness and relieving swelling by external application. It can be used for the treatment of abdominal distension, loss of appetite, and vomiting. Besides, it can also relieve toothache and treat dermatophytosis and pyogenic infection by external use. Moreover, it serves as a common medicinal material in ancient Ayurveda and Unani medical systems in India and also as an ingredient in spices, foods, and cosmetics. Modern pharmacological studies have shown that Nardostachyos Radix et Rhizoma possesses multiple pharmacological activities, such as sedation, anti-epilepsy, anti-convulsion, anti-depression, anti-arrhythmia, anti-malaria, anti-inflammation, anti-bacteria, anti-oxidation, and blood sugar metabolism improvement due to its multiple compounds contained, including terpenes, flavonoids, coumarins, and lignans. The main active components are sesquiterpenoids represented by nardosinone. The pharmacological activities, chemical compositions, and clinical applications of Nardostachyos Radix et Rhizoma have been investigated, but the research on resources, distribution, quality control, cultivation status, and applications are rarely reported. As an important genuine medicinal material from the Qinghai-Tibet plateau, Nardostachyos Radix et Rhizoma is obtained mainly from wild N. jatamansi. Accordingly, the conservation and sustainable utilization of N. jatamansi have attracted much attention all around the world. Based on the resource survey, cultivation research, and relevant literature available, the present study reviewed resources, geographical distribution, chemical compositions, pharmacological activities, quality control, cultivation, and applications of N. jatamansi, aiming to provide references for the conservation and development of N. jatamansi.

Zexietang is derived from Jingui Yaolue (《金匮要略》), which is composed of Alismatis Rhizoma and Atractylodis Macrocephalae Rhizoma, and has the effect of inducing diuresis and invigorating the spleen to produce water. Compared with western medicine in the treatment of related diseases, Zexietang can not only improve the curative effect, but also reduce the occurrence of adverse reactions, so as to achieve long-term stable administration. The authors sorted out and analyzed the chemical composition, pharmacological effect and clinical application of Zexietang in recent years. It was found that the main active components of Zexietang were alismol A and B, 23-acetyl-alismol B and C, atractylenolides (atractylenolide Ⅰ, Ⅱ, Ⅲ) and polysaccharides. Pharmacological experiments showed that they had diuretic, hypolipidemic, anti-inflammatory and others. And it can be used in the treatment of hypertension, hyperlipidemia, vertigo, cerebral vascular insufficiency and other diseases combined with other Chinese materia medica, and the curative effect is obvious. By summarizing the research status of Zexietang in recent years, its active components and pharmacological mechanism can be further clarified, which provides the basis for the clinical application of Zexietang and guides the direction of its further research.

In order to solve the problems of confusion in clinical medication and imperfect instructions in Chinese patent medicines(CPMs), the Standardization Department of the China Association of Chinese Medicine and Center for Pharmacovigilance and Rational use of Chinese Medicine in Beijing University of Chinese Medicine jointly compiled the Instructions for Clinical Application of Chinese Patent Medicines(CPMs). As the interpretation and supplement of drug instruction information, it aims to guide clinical safety and rational use of CPMs. In addition, the technical specification for clinical application description of CPMs has been formulated, which covers the seven processes of "carding instructions, clinical investigation, data retrieval, data screening, evidence classification, path transformation and writing format". It will enable readers of Instructions for Clinical Application of Chinese Patent Medicines to understand the work behind the compilation.

We investigated soil C:N:P stoichiometry and nutrient dynamics of Cunninghamia lanceolata plantations at different stand ages (5, 8, 21, 27 and 40 years old) in Fujian Baisha Fores-try Farm. We measured the concentrations of soil total carbon (TC), total nitrogen (TN), total phosphorus (TP), total potassium (TK), total calcium (Ca), total magnesium (Mg), and soil C:N:P stoichiometry at 0-10, 10-20, and 20-40 cm soil layers during different growth stages. The results showed that soil TC and TN concentrations and C:N remained unchanged during stand development. Soil TP content showed an increase-decrease-increase trend with increasing stand ages. Soil TP content was lowest, whereas C:P and N:P were highest at the mature stage of C. lanceolate plantation in the 0-10 and 10-20 cm soil layers. However, soil TP content showed no significant differences in all stand ages at the 20-40 cm soil layer. The contents of Ca and Mg were lowest at the mature stage of C. lanceolata stand. The TC was positively correlated with soil C:N, C:P and N:P. The TP was significantly and negatively correlated with soil C:P and N:P. Soil TP was a key factor regulating soil C:P and N:P stoichiometry. The development of mature plantation was mainly limited by soil P availability. To sustain the development of C. lanceolata plantations and improve nutrient cycling, phosphorus fertilizer could be applied during the rapid growth period of C. lanceolata. In addition, an appropriate extension of the rotation period of C. lanceolata plantation could facilitate soil nutrient restoration.

To analyze the study advance of Cichorium intybus, a kind of medicinal food, this study systematically retrieved Chinese and English literatures of C.intybus-related studies based on CNKI and the Web of Science database platforms by literature metrological research method and CiteSpace 5.5.R2 software, and visualized the literatures. A total of 1 247 Chinese literatures and 1 488 English literatures were included. Author analysis showed that HAN Yongfen in Guizhou Grass Industry Research Institute and W VAN DEN ENDE in Belgium Catholic University of Leuven had the most publications in Chinese and English C.intybus literatures respectively. ZHANG Bing's team was the core study team of C. intybus in China. Beijing University of Chinese Medicine and Belgium Catholic University of Leuven had the largest number of Chinese and English Cichorium literatures. There was a few cooperation between Chinese study institutions and close cooperation between foreign research institutions. Keywords analysis showed that C.intybus studies mainly focused on breeding and growth performance, composition analysis and content determination, nutritional value and medicinal value, in which C.intybus composition and its role in reducing uric acid were the hotspots of current studies. This study found that C.intybus has a high economic value and medicinal value, and its food nutrition and medicinal activity will be the main directions of future studies.

"Preventive treatment of disease" is one of the characteristics and advantages of traditional Chinese medicine. It is the most influential theory of traditional Chinese medicine. The core of the theory is "pre-prevention before disease", "prevention of disease from progress" and "prevention of recovery after disease", and the prevention concepts are "prevention of disease from deve-lopment" and "blockage and reversal". Based on the analysis of large numbers of ancient books, we put forward the overall pharmacovigilance theoretical frame that is composed of toxicity cognition, application, prevention and detoxification. With rich prospective prevention, the pharmacovigilance theory of traditional Chinese medicine was the essence of drug use experience in China for thousands of years. It coincides with the idea of preventing disease with precautions and preventing the occurrence of drug-induced diseases. This article intends to consider the theory and practice of pharmacovigilance of Chinese medicine from the "preventive treatment of disease", in order to provide reference for clinical safety medication. It has puts forward the practical measures of pharmacovigilance of traditional Chinese medicine, such as giving medical advice, paying attention to monitoring, preventing biological change, planning for the long term, preventing disease from changing, delaying the disease progress, protecting vital Qi, controlling internal disorder and preventing recurrence. It also indicated that pharmacovigilance of traditional Chinese medicine is an important part of the "preventive treatment of disease", and an important guarantee for the safe and rational use of medicines in clinical practice, which is conducive to protecting and promoting people's health and improving people's quality of life.

Chronic long-term glucocorticoids (GC) use is associated with glucocorticoid-induced osteoporosis (GIOP) by inhibiting the survival and impairing the functions of osteoblasts. Autophagy and mitophagy play key roles in osteoblast differentiation, mineralization and survival, and mounting evidence have implicated osteoblast autophagy and mitophagy as a novel mechanism in the pathogenesis of GIOP. Vitamin K2 (VK2) is an essential nutrient supplement that have been shown to exert protective effects against osteoporotic bone loss including GIOP. In this study, we showed that the glucocorticoid dexamethasone (Dex) deregulated osteoblast autophagy and mitophagy by downregulating the expression of autophagic and mitophagic markers LC3-II, PINK1, Parkin. This consequently led to inhibition of osteoblast differentiation and mineralization function in vitro. Interestingly, co-treatment with VK2 significantly attenuated the Dex-induced downregulation of LC3-II, PINK1, Parkin, thereby restoring autophagic and mitophagic processes and normal osteoblastic activity. In addition, using an established rat model of GIOP, we showed that VK2 administration can protect rats against the deleterious effects of Dex on bone by reinstating autophagic and mitophagic activities in bone tissues. Collectively, our results provide new insights into the role of osteoblast autophagy and mitophagy in GIOP. Additionally, the use of VK2 supplementation to augment osteoblast autophagy/mitophagy may significantly improve clinical outcomes of GIOP patients.
