Introdução: A associação entre perda auditiva e Diabetes Mellitus tipo 1 (DM1) é ainda pouco estudada. A perda auditiva é uma das complicações crônicas relacionadas ao grau de controle glicêmico, que os pacientes podem apresentar com a progressão da doença. Objetivo: Investigar o comprometimento auditivo por meio das emissões otoacústicas transitórias (EOAT) por banda de frequência em adolescentes com DM1 e relação com o controle glicêmico. Métodos: Foram incluídos 80 adolescentes, 50% do gênero masculino, entre 10 e 19 anos de idade: 40 com DM1 e 40 controles saudáveis, pareados por gênero e idade. Os dados clínicos e laboratoriais foram pesquisados nos prontuários médicos. O controle glicêmico foi avaliado por meio dos exames de hemoglobina glicada e os pacientes com DM1 analisados de acordo com o controle glicêmico. A avaliação auditiva foi realizada por meio da imitanciometria, audiometria, e posteriormente EOAT, em sala tratada acusticamente, pelo protocolo "TE Test" de clique não-linear (1 KHz a 4 kHz) a 80 dB NPS de intensidade (AuDX - Biologic). Resultados: As respostas às EOAT foram ausentes em 5,12% em pacientes com DM1, com diferença significativa em relação aos controles (p=0,04). A análise das EOAT por bandas de frequência mostrou maior proporção de alteração nos adolescentes com DM1 mal controlados quando comparados aos bem controlados, nas frequências de 1000Hz, 2000Hz e 3000Hz (p<0,05). Conclusão: As EOAT por bandas de frequência permitiram a identificação precoce de comprometimento auditivo em adolescentes com DM1 e mostraram associação entre DM1 mal controlado e perda auditiva. Introduction: The association between hearing loss and type 1 diabetes mellitus (DM1) is still poorly studied. Hearing loss is one of the chronic complications related to the degree of glycemic control that patients may present with the progression of the disease. Objective: To investigate auditory impairment through transient otoacoustic emissions (TEOAE) by frequency band in adolescents with DM1 and in relation to glycemic control. Methods: Were included 80 adolescents, 50% males, between 10 and 19 years of age: 40 with DM1 and 40 healthy controls, matched by gender and age. Clinical and laboratory data were taken from the medical records. Glycemic control was evalueted by glycated hemoglobin and the patients with DM1 were analyzed according to glycemic control. To the auditory evaluation were used the immittance and audiometry, and the TEOAE. The test was performed in the acoustically treated room, the non-linear TE test protocol (1 KHz to 4 kHz) at 80 dB SPL (AuDX - Biologic ). Results: TEOAE responses were absent in 5.12% of patients with DM1, with a significant difference in relation to controls (p = 0.04). The analysis of TEOAE by frequency bands showed a higher proportion of alteration in adolescents with DM1 poorly controlled when compared to well controlled ones, in the frequencies of 1000Hz, 2000Hz and 3000Hz (p <0.05). Conclusion: TEOAE by frequency bands allowed the early identification of auditory impairment in adolescents with DM1 and showed an association between poorly controlled DM1 and hearing loss. (AU)

PURPOSE: To analyze the correlation between the satisfaction of professionals from the Hearing Health Care network in two micro-regions of Minas Gerais state and the sociodemographic profile, work process, and work performance in the health service. METHODS: This is a cross-sectional, observational, analytic study with a non-probabilistic sample including 34 professionals from the Hearing Health Care services. Data collection occurred through individual interviews in the municipality of professional practice. Associations between the Professional Satisfaction variable and the explanatory variables Sociodemographic Data, Work Routine, and Developed Actions were conducted. RESULTS: Professionals with graduate studies were more satisfied with the human resources policy and the activities developed, whereas health civil servants showed more satisfaction with the wage policy and the work schedule. The correlation analysis between work process and satisfaction revealed a moderate positive correlation between items such as Health Promotion Actions, Satisfaction with Diagnostic Equipment, and Satisfaction with Maintenance Equipment. CONCLUSION: The present study revealed a higher level of satisfaction among professionals with graduate studies (human resources policy and activities developed) and civil servants (wage policy and work schedule). The relevance of this study lies on the important role that health professionals play on the Health Care Network. Additionally, the study of satisfaction level can provide a search for improvements, considering that satisfied professionals not only improve service quality, but also show greater creativity, commitment, and performance.

RESUMO Objetivo Analisar a correlação entre satisfação de profissionais da Rede de Atenção à Saúde Auditiva de duas microrregiões de Minas Gerais com perfil sociodemográfico, processo de trabalho e atuação no serviço de saúde. Método Estudo observacional analítico do tipo transversal, com amostra não probabilística composta por 34 profissionais vinculados aos Serviços de Atenção à Saúde Auditiva de duas microrregiões de Minas Gerais. A coleta de dados ocorreu por meio de entrevistas individuais no município de atuação do profissional. Foram realizadas associações entre a variável resposta Satisfação do Profissional com as variáveis explicativas Dados sociodemográficos, Rotina de trabalho e Ações desenvolvidas. Resultados Profissionais com pós-graduação estavam mais satisfeitos com política de Recursos Humanos e atividades desenvolvidas e os concursados com política salarial e agenda. Já a análise de correlação entre processo de trabalho e satisfação revelou correlação positiva de magnitude moderada entre os itens Ações de promoção de saúde e Satisfação com equipamentos de diagnóstico e Satisfação com manutenção de equipamentos. Conclusão O presente estudo evidenciou maior nível de satisfação entre os profissionais com pós-graduação (política de recursos humanos e atividades desenvolvidas) e concursados (política salarial e agenda). Destaca-se a importância do estudo, devido ao papel fundamental que o profissional de saúde desempenha dentro da Rede. Além disso, o estudo do grau de satisfação pode proporcionar busca por melhorias, uma vez que o profissional satisfeito não só melhora a qualidade do serviço, como também apresenta maior criatividade, comprometimento e rendimento.

INTRODUCTION: In order to meet the demands of the patient population with hearing impairment, the Hearing Health Care Network was created, consisting of primary care actions of medium and high complexity. Spatial analysis through geoprocessing is a way to understand the organization of such services. OBJECTIVE: To analyze the organization of the Hearing Health Care Network of the State of Minas Gerais. METHODS: Cross-sectional analytical study using geoprocessing techniques. The absolute frequency and the frequency per 1000 inhabitants of the following variables were analyzed: assessment and diagnosis, selection and adaptation of hearing aids, follow-up, and speech therapy. The spatial analysis unit was the health micro-region. RESULTS: The assessment and diagnosis, selection, and adaptation of hearing aids and follow-up had a higher absolute number in the micro-regions with hearing health services. The follow-up procedure showed the lowest occurrence. Speech therapy showed higher occurrence in the state, both in absolute numbers, as well as per population. CONCLUSION: The use of geoprocessing techniques allowed the identification of the care flow as a function of the procedure performance frequency, population concentration, and territory distribution. All procedures offered by the Hearing Health Care Network are performed for users of all micro-regions of the state. .


INTRODUÇÃO: Para atender às demandas da população com deficiência auditiva, o SUS criou as Redes Estaduais de Atenção à Saúde Auditiva, compostas por ações na atenção básica, média e alta complexidade. Uma forma de se compreender a organização desses serviços nos territórios de saúde é utilizando-se análise espacial, realizada por meio do geoprocessamento. OBJETIVO: Analisar a organização da Rede Estadual de Atenção à Saúde Auditiva em Minas Gerais. MÉTODO: Estudo analítico transversal, utilizando-se técnicas de geoprocessamento. Foram analisadas a frequência absoluta e frequência por mil habitantes das variáveis: avaliação e diagnóstico, seleção e adaptação de AASI, acompanhamento e terapia fonoaudiológica. A unidade de análise espacial foi a microrregião de saúde. RESULTADOS: Os procedimentos avaliação e diagnóstico, seleção e adaptação de AASI e acompanhamento tiveram maior número absoluto nas microrregiões onde há Serviços de Saúde Auditiva credenciados. O procedimento de acompanhamento apresentou a menor ocorrência no Estado. O procedimento terapia fonoaudiológica apresentou maior ocorrência, tanto em valor absoluto quanto por população. CONCLUSÃO: A utilização do geoprocessamento permitiu identificar o fluxo do atendimento em função da frequência de realização dos procedimentos, concentração populacional e distribuição territorial. Todos os procedimentos analisados são realizados em usuários em todas as microrregiões do Estado. .

BACKGROUND: The use of Auditory Steady-State Responses (ASSRs) for auditory screening in school-aged children, particularly in children who are difficult to test and children with disabilities, has not been explored yet. This pilot study investigated the use of ASSR for auditory screening in school-aged children. MATERIALS AND METHODS: A cross-sectional pilot study of 23 children aged 9 to 11 with normal-hearing thresholds and seven age-matched children with permanent moderate-to-profound bilateral hearing loss were examined. The tested carrier frequencies were 500, 1,000, 2,000, and 4,000 Hz, and the stimulus was modulated between 77 and 107 Hz. The ASSRs decreased according to the tested intensity levels of 50, 40, and 30 dB sound pressure level (SPL). Sensitivity and specificity were estimated from the responses of the children with normal hearing and those with hearing loss. RESULTS: For the children with normal hearing, the 2,000-Hz frequency was detected more often in both ears and at all intensity levels compared to the other frequencies. The 500- and 2,000-Hz frequencies resulted in different response patterns in both ears. The time until response detection increased in parallel with amplitude reduction, as expected. The overall time required for the test was 15 minutes, including the time spent in volunteer preparation. The sensitivity was 97% for the three intensities, and the best specificity value was 100%, which was observed at 50 dB. DISCUSSION: The response analysis indicated that a screening protocol for school-aged children could include 1,000, 2,000, and 4,000 Hz and that the recording of ASSRs was highly sensitive to internal and external factors. Fifty dB SPL should be considered a cut-off criterion for screening purposes because this was the intensity level with a sensitivity of 97% and a specificity of 100%. CONCLUSION: The use of ASSRs might be particularly useful in school-aged children who have difficulty performing subjective hearing tests. The sensitivity and specificity data suggested that the use of ASSRs was feasible as an auditory screening tool. In order to determine a protocol for screening, future studies should include a larger sample and children with mild hearing loss.
